Objective This retrospective study evaluates the occurrence and frequency of different fracture patterns in a series of computed tomography (CT) scans in terms of the AOCMF Trauma Classification (TC) orbit module and correlates the assigned defects with measurements of the fracture area in order to get an approximate guideline for fracture size predictions on the basis of the classification. Material and methods CT scans of patients with orbital floor fractures were evaluated using the AOCMFTC to determine the topographical subregions. The coding consisted of: W = orbital wall, 1 = anterior orbit, 2 = midorbit, i = inferior, m = medial. The 3-dimensional surface area size of the fractures was quantified by the “defect body” method (Brainlab, Munich, Germany). The fracture area size and its confidence and prediction interval within each topographical subregion was estimated by regression analysis. Results A total of 137 CT scans exhibited 145 orbital floor fractures, which were combined with 34 medial orbital wall fractures in 31 patients. The floor fractures – W1(i)2(i) (n = 86) and W1(i) (n = 19) were the most frequent patterns. Combined floor and medial wall fractures most frequently corresponded to the pattern W1 (im)2 (im) (n = 15) ahead of W1 (im) 2(i) (n = 10). The surface area size ranged from 0.11 cm2 to 6.09 cm2 for orbital floor and from 0.29 cm2 to 5.43 cm2 for medial wall fractures. The prediction values of the mean fracture area size within the subregions were computed as follows: W1(i) = 2.25 cm2, W2(i) = 1.64 cm2, W1(i)2(i) = 3.10 cm2, W1(m) = 1.36 cm2, W2(m) = 1.65 cm2, W1(m)2(m) = 2.98 cm2, W1 (im) = 3.35 cm2, W1 (im) 2(i) = 4.63 cm2, W1 (im)2(m) = 4.06 cm2 and W1 (im)2 (im) = 7.16 cm2. Conclusion The AOCMFTC orbital module offers a suitable framework for topographical allocation of fracture patterns inside the infero-medial orbital cavity. The involvement of the subregions is of predictive value providing estimations of the mean 3-D fracture area size.

Purpose We aimed to compare complication rates and functional outcomes in patients with bilateral mandibular fractures treated with different degrees of internal fixation rigidity. Patients and Methods This international, multicenter randomized controlled trial included adults with bilateral mandibular fractures located at either the angle and body, angle and symphysis, or body and symphysis. Patients were treated with either a combination of rigid fixation for the anterior fracture and nonrigid fixation for the posterior fracture (mixed fixation) or nonrigid fixation for both fractures. The primary outcome was complications within 6 weeks after surgery. Secondary outcomes were complications within 3 months, Helkimo dysfunction index, and mandibular mobility at 6 weeks and 3 months after surgery. Results Of the 315 patients enrolled, 158 were randomized to the mixed fixation group and 157 to the nonrigid fixation group. The overall complication rate at 6 weeks in the intention-to-treat population was 9.6% (95% confidence interval [CI], 5.3% to 15.6%) in the mixed fixation group and 7.8% (95% CI, 4.0% to 13.5%) in the nonrigid fixation group. With an unadjusted odds ratio of 1.25 (95% CI, 0.51 to 3.17), there were no statistically significant differences in complication rates between the 2 groups (P = .591). A multivariable model for complication risk at 6 weeks found no significant differences between treatment groups, but patients with moderate or severe displacement had a higher complication rate than those with no or minimal displacement (adjusted odds ratio, 4.58; 95% CI, 1.16 to 18.06; P = .030). There were no significant between-group differences in complication rates at 3 months. Moreover, no significant differences in Helkimo dysfunction index and mandibular mobility index at 6 weeks and 3 months were found between groups according to treatment allocated and treatment received. Conclusions A combination of rigid and nonrigid fixation in patients with bilateral mandibular fracture has similar complication rates and functional outcomes to nonrigid fixation for both fractures.

Purpose The aim of this study was to evaluate the clinical, radiologic, and functional outcomes in a retrospective cohort study of patients with condylar base and neck fractures treated with open reduction and internal fixation via lambda plates to define selection criteria for their application. Patients and Methods Eleven patients underwent open reduction and internal fixation using lambda plates for osteosynthesis of condylar base and neck fractures. The mean follow-up period was 8 months. All kinds of complications were recorded. Postoperative maximum jaw opening and occlusion were determined. Pain on palpation of the joint area, pain on movement, and muscle pain were examined. The criteria for the intraoperative selection of a lambda plate were recorded. Results Positioning and fixation of lambda plates were feasible in all cases. A condylar neck fracture with limited bone surface on the proximal fragment for internal fixation prevailed as the primary indication for the selection of a lambda plate. Avoiding extensive soft tissue stripping at the proximal fragment when using a transoral endoscopically assisted approach in cases of condylar base fractures also was a selection criterion. The overall complication rate was low. Conclusions The indications for lambda plates for internal fixation of condylar fractures are neck fractures with limited bone surface along the proximal fragment for internal fixation. Application of the plate through a transoral endoscopic approach to avoid extensive soft tissue stripping at the proximal fragment is favorable in high condylar base fractures.

Purpose: The known preformed osteosynthesis plates for the midface are helpful tools for a precise and fast fixation of repositioned fractures. The purpose of the current study is to analyze the precision of newly developed prototypes of preformed osteosynthesis plates for the mandible. Methods: Four newly designed preformed osteosynthesis plates, generated by a statistical shape model based on 115 CT scans, were virtually analyzed. The used plates were designed for symphyseal, parasymphyseal, angle, and condyle fractures. Each type of plate has three different sizes. For analysis, the shortest distance between the plate and the bone surface was measured, and the sum of the plate-to-bone distances over the whole surface was calculated. Results: A distance between plate and bone of less than 1.5 mm was defined as sufficient fitting. The plate for symphyseal fractures showed good fitting in 90% of the cases for size M, and in 84% for size L. For parasymphyseal fractures, size S fits in 80%, size M in 68%, and size L in 65% of the cases. Angle fractures with their specific plate show good fitting for size S in 53%, size M in 60%, and size L in 47%. The preformed plate for the condyle part fits for size S in 75%, for size M in 85%, and for size L in 74% of the cases. Conclusion: The newly developed mandible plates show sufficient clinical fitting to ensure adequate fracture reduction and fixation.

Since MRI using dedicated imaging sequences has recently shown promising results in direct visualization of the inferior alveolar nerve (IAN) and the lingual nerve (LN) with high spatial resolution, the aim of this study was to generate suitable standard specifications to reliably depict the IAN and LN in MRI and to delineate the anatomy and its variants of these nerves in healthy subjects. Thirty healthy volunteers were examined on a 3-T scanner (Elition, Philips Healthcare, Best, the Netherlands). The sequence protocol consisted of 3D STIR, 3D DESS, and 3D T1 FFE “black bone” sequences. The study reconfirmed a good feasibility of direct visualization of proximal and peripheral portions of the IAN and of the proximal course of the LN. The STIR sequence showed the highest apparent signal to noise ratio (aSNR) and best apparent nerve-muscle contrast to noise ratio (aNMCNR) for IAN and for the LN. The applied MRI sequences allowed to differentiate the tissue composition of the neurovascular bundle inside the mandibular canal. Dedicated MRI sequence protocols proved effectively to detect the IAN and LN and their course in healthy volunteers. The tissue composition of the mandibular neurovascular bundle was conclusively distinguishable as was the varying topography inside multiple bony channels. The presented data on the precise and valid visualization of the IAN and LN have clinical implications in respect to local anesthesia prior to dental treatments in the mandible but also regarding surgical procedures and implant insertion in the molar region.

The purpose of the study was to evaluate the different fracture pattern of patients being treated with trapezoidal plate fixation of condylar base and neck fractures using the AOCMF Classification System to determine specific fracture pattern that are associated with failure of trapezoidal plate fixation. Ten patients with mandibular condylar base and neck fractures underwent internal fixation using trapezoidal plates. Fractures of the condylar process were classified according to the AOCMF Classification System using computed tomography data. The fracture pattern was compared to complications according to hardware failure such as plate or screw loosening and plate fracture. Plate and screw loosening of trapezoidal plates for fixation of condylar base and neck fractures was associated with fracture location in the neck region and major fragment displacement with contact loss between the proximal and distal fragment of the fracture. Further investigations such as finite-element analysis of different plating systems for fixation of condylar neck fractures are necessary to establish the optimal fixation technique for condylar neck fractures.

Ausgedehnte kombinierte Defekte der Unterlippe, der Kinnregion, des Unterkiefers, des Mundbodens und der vorderen Mundhohle entstehen hauptsachlich als Folge tumorablativer Eingriffe. Aber auch Schussverletzungen, hohergradige Stadien der infizierten Osteoradionekrose und Infektionen konnen ursachlich sein. Eine grose rekonstruktive Herausforderung stellen die funktionellen Einbusen durch die fehlende Fixierung der Zungen- und suprahyoidalen Muskulatur, die Instabilitat der Kiefersegmente, fehlende Zahne sowie den Verlust der oralen Sphinkterfunktion und der motorischen und sensiblen Nervenversorgung dar. Neben den funktionellen liegen haufig massive asthetische Beeintrachtigungen im augenfalligen, markanten Bereich der unteren Gesichtspartie durch den Verlust der Kinnprominenz und ggf. des Lippenrots vor, die zur sogenannten Andy-Gump-Deformitat fuhren. In der onkologischen Behandlung wird das rekonstruktive Konzept zusatzlich durch die Notwendigkeit adjuvanter Therapien, v. a. Strahlenbehandlung, beeinflusst. Abhangig vom Defektvolumen kommen am haufigsten mikrochirurgische Knochen-Weichgewebe-Transplantate ggf. in Kombination mit lokalen Lippenersatzplastiken zum Einsatz. Verschiedene Vorschlage fur Behandlungsalgorithmen wurden publiziert. Besondere Aufmerksamkeit richtet sich dabei auf die orale Sphinkterfunktion. Aufhangungs- und Zugelungsplastiken, aber auch knocherne Abstutzungen werden zur Wiederherstellung der Lippenkompetenz eingesetzt. Gemessen an der erheblichen Problematik gibt es vergleichsweise wenige Publikationen mit nur geringen Fallzahlen. Im vorliegenden Beitrag werden eigene Erfahrungen mit 27 Patientenverlaufen im Kontext der Literatur diskutiert. Fur eine befriedigende asthetische und funktionelle Wiederherstellung gibt es bis heute keine umfassend optimale Losung.

Purpose Reconstruction of orbital wall fractures is demanding and has improved dramatically with the implementation of new technologies. True-to-original accuracy of reconstruction has been deemed essential for good clinical outcome, and reasons for unfavorable clinical outcome have been researched extensively. However, no detailed analysis on the influence of plate position and surface contour on clinical outcome has yet been published. Materials and methods Data from a previous study were used for an ad-hoc analysis to identify predictors for unfavorable outcome, defined as diplopia or differences in globe height and/or globe projection of >2 mm. Presumed predictors were implant surface contour, aberrant implant dimension or position, accuracy of reconstructed orbital volume, and anatomical fracture topography according to the current AO classification. Results Neither in univariable nor in multivariable regression models were unfavorable clinical outcomes associated with any of the presumed radiological predictors, and no association of the type of implant, i.e., standard preformed, CAD-based individualized and non−CAD-based individualized with its surface contour could be shown. Conclusion These data suggest that the influence of accurate mechanical reconstruction on clinical outcomes may be less predictable than previously believed, while the role of soft-tissue−related factors may have been underestimated.

Introduction Bone grafts from the iliac crest are most commonly used for osteoplasties of the cleft alveolus. To preclude undue donoresite morbidity custom-milled allogeneic bone grafts might be an appropriate choice. Material and methods This technical note showcases the repair of an alveolar cleft using an individualized allogeneic bone graft in a 36-year old female patient. She was asking for an alternative to the iliac crest bone grafting. Her alveolus was successfully build up by a custom-milled cancellous bone block allograft (maxgraft ® 80 bonebuilder). Results Custom-milled cancellous bone block allografts can greatly facilitate alveolar cleft repair and may present an effective treatment option under the premise that resorption resistance corresponds to autografts. Conclusion Further clinical studies are needed to explore the potential of bone block allografts for alveolar cleft osteoplasty.

ZusammenfassungCMF-Traumen im Alter 65+ sind durch besondere Merkmale gekennzeichnet: Lokalisation und Frakturmuster, Stand des Involutionsprozesses mit Abnahme von Knochenquantität und -qualität, Begleitverletzungen, altersassoziierte physische und psychische Funktionseinbußen und präexistente Komorbiditäten. Der Stand des Wissens und aktuelle bevölkerungsbezogene Daten dieser Variablen werden zu Anfang dieser Übersicht dargestellt. Vor diesem Hintergrund wird dann zu den verschiedenen Frakturen und Frakturtypen in den einzelnen Regionen des Gesichtsschädelskeletts Stellung genommen und die Indikation zu einem konservativen oder operativen Prozedere in den Alterspopulationen diskutiert. Spezielle Aufmerksamkeit kommt dabei Fraktursituationen bei Zahnlosigkeit und Atrophie im Ober- und/oder Unterkiefer zu. Innerhalb des weitgesteckten Gesamtspektrums der Frakturszenarien haben schonende, situationsadaptierte, ggf. auch unkonventinelle Lösungen Priorität vor Standardtherapieverfahren. Neue Techniken wie computerassistierte Chirurgie und patientenspezifische Implantate spielen eine immer größere Rolle und werden an Einzelbeispielen demonstriert. Betont werden muss, dass die physischen Reserven und nötigenfalls die geriatrietypische Multimorbidität jedes Patienten individuell einzuschätzen ist und vorrangige Bedeutung vor dem kalendarischen Lebensalter hat.

MatrixMANDIBLE Preformed Reconstruction Plates (MMPRPs) were developed to overcome laborious bending procedures of conventional reconstruction plates. The design comprises three sizes with a nonbendable centerpiece and two bendable sections (proximal and distal). According to the surgical protocol unnecessary parts are trimmed after the last used screw hole. In the present retrospective study postoperative radiographs from 130 patients (average age 63 years) that received treatment with MMPRPs were assessed. There was no statistical correlation between plate-size, location (left/right) or age. 82.98% of the small and 91.80% of the medium MMPRPs were trimmed by at least the terminal screw hole of the ramus part. In all patients receiving a large MMPRP, the terminal screw hole of the ramus was unused accordingly all inserted large MMPRPs were trimmed by at least the terminal screw hole. The majority of the bridged defects were located within the area of the body indicating a feasible plate design. With the emergence of solid free form fabrication of Ti-alloys and economic need to reduce the waste of resources this study may help to further improve the MMPRP design and prevent the loss of medical-grade titanium.

Der Midface Level 2 der AO-CMF-Trauma-Klassifikation fur Erwachsene gibt die Lokalisation von Frakturen innerhalb definierter Subregionen des zentralen und lateralen Mittelgesichts wieder. Frakturen des Orbitarahmens und der Orbitawande bzw. des Apex orbitae werden dabei separat erfasst. Die hier veranschaulichte Level-3-Klassifikation der Orbitafrakturen ermoglicht eine weitere Prazisierung des Frakturmusters in nochmals untergliederten topographischen Abschnitten der Orbita: Orbitarahmen, anteriore Orbitawande, mittlere Orbitawande und Apex. Zudem kann die Frakturdokumentation auf wichtige Strukturen innerhalb der Orbita, die eine besondere klinische Relevanz haben, ausgeweitet werden. Dementsprechend lassen sich Frakturen im Bereich der Fissura orbitalis inferior und superior, des grosen Keilbeinflugels, des medialen inneren orbitalen Pfeilers sowie des Canalis opticus und des Tranenbeins im Einzelnen erfassen. In diesem Beitrag werden die Regeln und Vereinbarungen zur Bestimmung der Lokalisation und Morphologie der Orbitafrakturen sowie deren Codierung erlautert und an computertomographischen Fallbeispielen demonstriert.

Es bestand die Notwendigkeit einer systematischen Klassifikation von Frakturszenarien im Unterkiefer nach einheitlichen Kriterien. Entwickelt werden sollten Normen und eine festgeschriebene Matrix zur Dokumentation von Unterkieferfrakturen. Die Methodik umfasste die Entwicklung und Validierung von Klassifikationsvorschlagen in Form einer Ikonographie bis zu einem aktuell gultigen Modell. Zur Frakturlokalisation wird der Unterkiefer in 9 topographische Subregionen unterteilt. Der Zuordnung von Frakturverlaufen im Grenzbereich der Subregionen dienen 4 Transitionszonen. In einer weitergehenden Prazisierungsstufe lassen sich individuelle Patientenmerkmale (z. B. pratraumatischer Zahnstatus, Alveolarfortsatzatrophie bei Zahnlosigkeit) erfassen. Des Weiteren ist es moglich, Zahnhartsubstanz- und parodontale Verletzungen sowie Alveolarfortsatzfrakturen zu dokumentieren. Morphologieeigenschaften, wie Fragmentation, konnen als weitere Charakteristika zur Vervollstandigung der Klassifikation hinzugefugt werden. Auf der Basis dieses Klassifikationsvorschlags entstehen klare Konventionen mit dem Zweck, Frakturmuster im Unterkiefer in ihrem ganzen Variantenreichtum und bis in feine Details vergleichbar aufzuzeichnen.

Die Diagnose und Klassifikation von Frakturen der Schadelkalotte, der Schadelbasis, des Mittelgesichts und der Mandibula konnen nur auf Basis einer genauen klinischen und radiologischen Analyse korrekt erfolgen. Mithilfe der Bildgebung sind Aussagen uber den Schweregrad der Verletzung und die Indikationen fur das weitere diagnostische und therapeutische Vorgehen moglich. Eine der Fragestellung angepasste Aufnahmetechnik sowie eine korrekte und prazise Interpretation basierend auf genauen anatomischen Kenntnissen sind Voraussetzung fur die korrekte Anwendung der neuen AO-CMF-Trauma-Klassifikation. Diese stutzt sich auf die Bildgebung zur genauen Dokumentation und Beschreibung der Frakturen. Mit der Computertomographie konnen die unterschiedlichen Frakturmuster des Schadelskeletts und die Lokalisation von Fragmenten genau dargestellt werden, ebenso ist eine Beurteilung der Weichgewebe moglich. Die durch die Klassifikation definierten 3 Prazisionslevel lassen sich mithilfe der Bildgebung korrekt und genau dokumentieren. In diesem Beitrag werden die notwendigen Voraussetzungen der Bildgebung sowie die kritischen Strukturen bzw. Fragestellungen bezogen auf den Prazisionslevel und die Frakturregion des AO-CMF-Klassifikationssystem dargestellt.

Die AO-CMF-Trauma-Klassifikation fur Erwachsene beruht auf einem hierarchischen Algorithmus, der auf 3 Ebenen (Prazisionslevel) mit zunehmender Komplexitat und immer detaillierterer Frakturbeschreibung aufbaut. Mittelgesichtsfrakturen einschlieslich der Orbita werden in Level 1 zusammenfassend mit dem Code 92 bezeichnet. Level 2 gibt die Lokalisation der Frakturen innerhalb definierter Subregionen des zentralen („upper“, „intermediate“, „lower“) und lateralen (Zygoma, Processus pterygoideus) Mittelgesichts wieder, wobei Frakturen des Orbitarahmens und der Orbitawande bzw. des Apex orbitae separat erfasst werden. Gangige Frakturmuster wie die En-bloc-Frakturen des Jochbeins und der nasoorbitoethmoidalen Region werden ebenso berucksichtigt wie Frakturen der LeFort-Ebenen. Level 3 erlaubt aufgrund einer detaillierteren anatomischen Topographie eine hohere Prazisierung der Frakturmuster als Level 2. Als wichtiges Merkmal ermoglicht Level 3 die Dokumentation des Fragmentierungsgrads, der Dislokation von Fragmenten sowie eines eventuellen Knochenverlusts. Zudem konnen Zahnstatus und Atrophiegrad des Alveolarfortsatzes beschrieben und traumatisch bedingte Zahnschaden sowie eine Beteiligung des Alveolarfortsatzes erfasst werden.

Im Prazisionslevel 3 der AO-CMF-Trauma-Klassifikation werden Kiefergelenkfortsatzfrakturen in Frakturen des Gelenkkopfs (Walze, Capitulum), des Gelenkhalses und der Gelenkfortsatzbasis unterteilt. Kopffrakturen weisen 2 typische Bruchverlaufe auf. Bei einer Fraktur im medialen Walzenbereich (Frakturtyp m) bleibt die Relation lateraler Kondylus zu Fossa und Diskus i.d.R. erhalten. Eine Fraktur im Bereich des lateralen Walzenpols (Frakturtyp p) ist ublicherweise mit einem Hohenverlust assoziiert; der Diskus mit dem kleinen (proximalen) Fragment wird nach anteromedial disloziert, was mit oder ohne Verletzung des lateralen Kapselansatzes einhergehen kann. Die Frakturmorphologie wird neben dem Hohenverlust durch die Attribute Fragmentation („minor“, „major“), vertikale Fragmentapposition sowie Dislokation, Luxation und Distorsion des proximalen Fragments in Bezug auf die Fossa naher definiert. Gelenkfortsatzbasis- und Gelenkhalsfrakturen werden gemas einer neuen Ein-Drittel- zu Zwei-Drittel-Regel fur den Frakturverlauf ober- bzw. unterhalb der sog. Loukota-Linie („sigmoid notch line“) differenziert. Attribute zur Definition der Morphologie sind auch hier das Vorliegen einer Luxation oder Dislokation und deren Richtung, die Angulation des gelenktragenden Fragments und Parameter wie Fragmentation, Hohenverlust und Dislokationen in der Kondylus-Fossa-Relation.

Eine umfassende Trauma-/Frakturklassifikation fur samtliche Untereinheiten des Schadelskeletts (CMF) von Erwachsenen soll auf den Weg zur allgemeinen Anwendung und Uberprufung der Praxistauglichkeit gebracht werden. Der methodologische Hintergrund zur Validierung und die Grundkonzeption des Klassifikationssystems werden dargestellt, um damit erste Uberzeugungsarbeit fur die Erprobung in der Routine zu leisten. Das aktuell gultige Klassifikationsmodell ist das Produkt eines iterativen Entwicklungsprozesses auf der Grundlage einer Serie von Pilotstudien. Es zeichnet sich durch eine Einteilung des Schadelskeletts in immer kleinere anatomische Untereinheiten aus, in denen Frakturen in 3 Prazisionsleveln mit zunehmender Detailgenauigkeit erfasst werden konnen. Ein spezielles Softwareprogramm, die CMF-Applikation des Comprehensive Injury Automatic Classifier der Arbeitsgemeinschaft fur Osteosynthesefragen (AOCOIAC) mit einer klar strukturierten Ikonographie der anatomischen Regionen und Subregionen macht die systematische Aufzeichnung der Frakturmuster und -morphologie sowie die Umsetzung in Kodierungsformeln zur Abspeicherung in Datenbanken auf einfache Weise moglich. Der jetzige Entwicklungsstand der neuen AO-CMF-Trauma-Klassifikation hat ein Niveau erreicht, um beim Einsatz im klinischen Alltagtest bestatigt und optimiert zu werden.

Das Atemwegsmanagement bei mund-, kiefer- und gesichtschirurgischen Eingriffen in Allgemeinanasthesie stellt eine besondere Herausforderung dar. Die anatomischen Atemwegsstrukturen konnen sowohl durch angeborene als auch durch erworbene Pathologien verandert sein, sodass nach sorgfaltiger Evaluation durch einen Anasthesisten die sicherste Strategie zur Atemwegssicherung gefunden werden muss. Dies gelingt nur in eingespielter Zusammenarbeit der verschiedenen Disziplinen und unter differenzierter Verwendung unterschiedlicher Techniken. Als Standardverfahren zur Intubation des schwierigen Atemwegs gilt die fiberoptische Wachintubation des spontan atmenden Patienten. Im Falle des unerwarteten schwierigen Atemwegs kann ein erfahrener Anasthesist mit modernen Geraten, wie z. B. Videolaryngoskopen, die meisten problematischen Situationen bewaltigen.

Purpose Lingual positioning of mandible reconstruction plates can prevent typical complications such as plate exposure and orocutaneous fistulas. The aim of this study was to evaluate the feasibility and clinical outcomes of lingually applied patient-specific reconstruction plates. Materials and Methods A retrospective cohort study of patients managed with reconstruction plates positioned at the lingual side of the mandible was performed. The following outcome variables were analyzed: feasibility and handling of the lingual application, fitting accuracy, and postoperative complications. Results The study sample included 8 patients (4 women, 4 men) with a mean age of 68.8 years (range, 43 to 79 yr). The indications for plate use were defect-like zones owing to multiple fractures in the atrophic mandible (n = 2) and segmental defects attributed to squamous cell carcinoma, osteomyelitis, and bisphosphonate-related osteonecrosis (n = 6). Off-the-shelf reconstruction plates, which were intraoperatively bent (n = 2) or pre-bent to a stereolithographic model (n = 1), or fully computer-assisted designed and computer-assisted manufactured (CAD/CAM) patient-specific reconstruction plates (n = 5) were used. Lingual application of reconstruction plates was feasible in all cases and considerably facilitated by the CAD/CAM plates. The fitting accuracy was high. Within an average follow-up period of 15.3 months, no loosening of plates or screws, plate fracture, or orocutaneous fistula occurred and no dysphagia was observed with the exception of 1 case, in which plate exposure was due to extreme soft tissue thinning and tumor recurrence. Conclusions Lingual application of CAD/CAM patient-specific reconstruction plates is technically feasible with high accuracy. It has great potential for low complication rates, especially in unfavorable soft tissue conditions.

Purpose Mirroring and manual adaptation as the main virtual reconstruction method of midfacial defects is time demanding and ignores asymmetrical skull shapes. By using a statistical shape model (SSM), the reconstruction can be automatized and specified. The current study aims to show the ability of the SSM in the virtual reconstruction of artificial bilateral defects. Methods Based on 131 pathologically unaffected CT scans of the adult midface region, an SSM was created. DICOM data were generated, segmented and registered on one mesh, which serves as template for the SSM. The SSM consists of the registered surface meshes and includes the shape variability of the cranial vault. Fractured or missing parts were calculated by the known shape variability of healthy midface data. Using 25 CT scans not included in the SSM, the precision of the reconstruction of virtually placed bilateral defects of the orbital floor (Group 1) and bilateral naso-orbital-ethmoid (NOE) fractures (Group 2). Distances to the corresponding parts of the intact skull were calculated to show the accuracy of the virtual reconstruction method. Results All defects could be reconstructed by using the SSM and GM technique. The analysis shows a high accuracy of the SSM-driven reconstruction, with a mean error of 0.75 ± 0.18 mm in group 1 and with a mean error of 0.81 ± 0.23 mm in group 2. Conclusion The precision of the SSM-driven reconstruction is high and its application is easy for the clinician because of the automatization of the virtual reconstruction process in the field of computer-assisted surgery (CAS). Respecting of the natural asymmetry of the skull and the methods of GM are reasons for the high precision and the automatization of the new shown reconstruction workflow.

Introduction There has been a shift toward surgical treatment of ZMC (zygomaticomaxillary complex) fractures with open reduction and subsequent fixation in the past decades. Anatomically preformed osteosynthesis plates, already used in the field of craniomaxillofacial surgery for the treatment of fractures of the mandible and the orbit, might be a suitable option for ZMC fractures as well. Material and methods A statistical shape model was created from 179 cranial CT scans. Based on this surface model, an anatomically preformed plate for the reduction and fixation of ZMC fractures was developed in 3 sizes (S, M, L). Virtual analysis of the accuracy of the plate was performed on a dataset consisting of 120 CT scans. Results Within a determined tolerance range of 0–1.5 mm, analysis revealed a high accuracy of the plate in 70–87 % of the CT scans. The S-sized plate has the highest overall accuracy, whereas the L-sized plate has highest accuracy at the “base” region which is essential for the placement of the plate. Discussion The newly developed plate can be placed via an intraoral approach and analysis of the plate has confirmed its accuracy to be sufficient to ensure an adequate fracture reduction and fixation. It thus might allow for a less extensive approach and less approaches/incisions necessary overall to reduce and fixate ZMC fractures.

The AO CMF has recently launched the first comprehensive classification system for craniomaxillofacial (CMF) fractures. The AO CMF classification system uses a hierarchical framework with three levels of growing complexity (levels 1, 2, and 3). Level 1 of the system identifies the presence of fractures in four anatomic areas (mandible, midface, skull base, and cranial vault). Level 2 variables describe the location of the fractures within those defined areas. Level 3 variables describe details of fracture morphology such as fragmentation, displacement, and dislocation. This multiplanar radiographic image-based AO CMF trauma classification system is constantly evolving and beginning to enter worldwide application. A validation of the system is mandatory prior to a reliable communication and data processing in clinical and research environments. This interobserver reliability and accuracy study is aiming to validate the three current modules of the AO CMF classification system for mandible trauma in adults. To assess the performance of the system at the different precision levels, it focuses on the fracture location within the mandibular regions and condylar process subregions as core components giving only secondary attention to morphologic variables. A total of 15 subjects individually assigned the location and features of mandibular fractures in 200 CT scans using the AO CMF classification system. The results of these ratings were then statistically evaluated for interobserver reliability by Fleiss’ kappa and accuracy by percentage agreement with an experienced reference assessor. The scores were used to determine if the variables of levels 2 and 3 were appropriate tools for valid classification. Interobserver reliability and accuracy were compared by hierarchy of variables (level 2 vs. level 3), by anatomical region and subregion, and by assessor experience level using Kruskal-Wallis and Wilcoxon's rank-sum tests. The AO CMF classification system was determined to be reliable and accurate for classifying mandibular fractures for most levels 2 and 3 variables. Level 2 variables had significantly higher interobserver reliability than level 3 variables (median kappa: 0.69 vs. 0.59, p < 0.001) as well as higher accuracy (median agreement: 94 vs. 91%, p < 0.001). Accuracy was adequate for most variables, but lower reliability was observed for condylar head fractures, fragmentation of condylar neck fractures, displacement types and direction of the condylar process overall, as well as the condylar neck and base fractures. Assessors with more clinical experience demonstrated higher reliability (median kappa high experience 0.66 vs. medium 0.59 vs. low 0.48, p < 0.001). Assessors with experience using the classification software also had higher reliability than their less experienced counterparts (median kappa: 0.76 vs. 0.57, p < 0.001). At present, the AO CMF classification system for mandibular fractures is suited for both clinical and research settings for level 2 variables. Accuracy and reliability decrease for level 3 variables specifically concerning fractures and displacement of condylar process fractures. This will require further investigation into why these fractures were characterized unreliably, which would guide modifications of the system and future instructions for its usage.

Purpose The aim of the study was to evaluate the resorption of the mandibular condylar head after open reduction and internal fixation of fractures with small-fragment screws and to evaluate the functional outcome. Patients and method Forty-eight patients (55 condylar head fractures) underwent surgical treatment. All kinds of complications were recorded. In a subgroup of 20 patients, the average loss of bone height of the condylar head joint surface was gauged by measuring the vertical distances from 3 selected reference points of the fixation hardware to the vertex of the condyle in views of multiplanar radiographs after 6 months. Functional outcome was analysed by Helkimo index score. Results Complication rates were low. Osteosynthesis loosening occurred in 4 cases. The mean bone resorption along the condylar head joint surface in the subgroup of 20 patients (with 23 joints) was 0.7 mm. Functional outcome was rated as follows: 8 patients - good, 11 patients – slightly impaired, 1 patient - moderately impaired. Conclusion Open reduction and internal fixation of condylar head fractures with small-fragment screws leads to reliable results in terms of joint surfaces relatively resistant to resorption and function. Low bone resorption on the joint surface of the condylar head can be expected.

A case is presented of a 16-year-old girl with an aneurysmal bone cyst (ABC) of the mandibular condyle of the temporomandibular joint (TMJ). This lesion rarely involves the mandibular condyle, and involvement of condylar head is even rarer. To our knowledge, only 12 cases have been reported in the literature so far. This is the first case of ABC of the mandibular condyle with 3D planning of costochondral graft reconstruction described in the literature.

In contrast to odontogenic cysts, keratocystic odontogenic tumours often recur and require more aggressive surgical treatment, so we tried to find features that distinguished between them on magnetic resonance imaging (MRI). Without knowing the diagnosis, two radiologists reviewed intensity (low, intermediate, or high) and homogeneity (homogeneous or heterogeneous) of signals in short-tau-inversion-recovery (STIR), T1- and T2-weighted, and fat-suppressed, contrast-enhanced MRI in 20 consecutive patients with oval, radiolucent lesions of the mandible on panoramic radiography, and who were subsequently confirmed histopathologically to have either an odontogenic cyst or a keratocystic odontogenic tumour (n=10 in each group). Purpose To evaluate clinical, radiologic, and functional outcomes of patients with condylar neck or base fractures treated with open reduction and internal fixation using strut plates. Patients and Methods Thirty patients with 34 condylar neck and base fractures were included in the study. Transoral endoscopically assisted, submandibular, or transparotid approaches were performed. Clinical and radiologic outcomes were evaluated. Functional outcomes were assessed with the Helkimo index in a subgroup of 15 patients. Results Strut plates were successfully placed in all surgical approaches used in this study. Fracture healing occurred without complications in 33 condylar processes. One plate fracture occurred. In the subgroup, 7 patients had a good functional outcome, 7 had a slightly impaired functional outcome, and 1 patient had a moderately impaired functional outcome. Conclusion Strut plates are appropriate for retromandibular transparotid, endoscopically assisted transoral, or submandibular approaches in the management of condylar neck or base fractures. Intraoperative handling and fitting accuracy of the strut plates are good. Good to slightly impaired functional outcome was observed in a subgroup of patients.

This tutorial outlines the details of the AOCMF image-based classification system for fractures of the mandibular arch (i.e. the non-condylar mandible) at the precision level 3. It is the logical expansion of the fracture allocation to topographic mandibular sites outlined in level 2, and is based on three-dimensional (3D) imaging techniques/computed tomography (CT)/cone beam CT). Level 3 allows an anatomical description of the individual conditions of the mandibular arch such as the preinjury dental state and the degree of alveolar atrophy. Trauma sequelae are then addressed: (1) tooth injuries and periodontal trauma, (2) fracture involvement of the alveolar process, (3) the degree of fracture fragmentation in three categories (none, minor, and major), and (4) the presence of bone loss. The grading of fragmentation needs a 3D evaluation of the fracture area, allowing visualization of the outer and inner mandibular cortices. To document these fracture features beyond topography the alphanumeric codes are supplied with distinctive appendices. This level 3 tutorial is accompanied by a brief survey of the peculiarities of the edentulous atrophic mandible. Illustrations and a few case examples serve as instruction and reference to improve the understanding and application of the presented features.

This tutorial outlines the details of the AOCMF image-based classification system for fractures of the midface at the precision level 3. The topography of the different midface regions (central midface—upper central midface, intermediate central midface, lower central midface—incorporating the naso-orbito-ethmoid region; lateral midface—zygoma and zygomatic arch, palate) is subdivided in much greater detail than in level 2 going beyond the Le Fort fracture types and its analogs. The level 3 midface classification system is presented along with guidelines to precisely delineate the fracture patterns in these specific subregions. It is easy to plot common fracture entities, such as nasal and naso-orbito-ethmoid, and their variants due to the refined structural layout of the subregions. As a key attribute, this focused approach permits to document the occurrence of fragmentation (i.e., single vs. multiple fracture lines), displacement, and bone loss. Moreover, the preinjury dental state and the degree of alveolar atrophy in edentulous maxillary regions can be recorded. On the basis of these individual features, tooth injuries, periodontal trauma, and fracture involvement of the alveolar process can be assessed. Coding rules are given to set up a distinctive formula for typical midface fractures and their combinations. The instructions and illustrations are elucidated by a series of radiographic imaging examples. A critical appraisal of the design of this level 3 midface classification is made.

Die Frakturbehandlung im zahnlosen atrophen Unterkiefer ist problematisch. Dies hat seine Grunde in lokalen Faktoren, wie der reduzierten ossaren Quantitat und Qualitat sowie in altersassoziierten Komorbiditaten. Die Therapiemoglichkeiten reichen von minimalistisch konservativen Ansatzen uber die Applikation von Miniplatten unter Verwendung transoraler chirurgischer Zugange bis hin zur aggressiven transfazial-submandibularen Dissektion mit Reposition und Montage von Makro- bzw. Rekonstruktionsplatten. Wenn Rekonstruktionsplattensysteme mit einem hochatrophen „Pencilbone“-Unterkiefer zusammentreffen, wird ein „Overengineering“ deutlich, da die Massivitat des Osteosynthesematerials und der residuale Knochen in krassem Missverhaltnis zueinander stehen. Wegen der hohen Stabilitat der mit Rekonstruktionsplatten uberbruckten Frakturen gelten diese jedoch nach wie vor als Referenz fur Vergleiche mit anderen internen Fixationsverfahren bzw. Plattenvarianten und Konstrukten.

Neue Bildgebungstechnologien haben Einzug in klinische Routineanwendungen gehalten und zu innovativen Behandlungskonzepten gefuhrt, was v. a. bei Kiefergelenkfortsatzfrakturen und Verletzungen des Orbitarahmens sowie der internen Orbita (Orbitawande) zum Ausdruck kommt. Das neue AO-CMF-Trauma-Klassifikationssystem fur Erwachsene (3. Generation) beabsichtigt, den Fortschritten in der Frakturdarstellung auf CT-Basis Rechnung zu tragen. Drei Prazisionslevel bieten die Moglichkeit, Frakturlinienverlaufe und -muster nach ihrer Lokalisation und hinsichtlich morphologischer Eigenschaften abgestuft bis zu einer bisher nicht dagewesenen Genauigkeit zu dokumentieren. Zur Dokumentation wird die CMF-Komponente des AOCOIAC-(AO COmprehensive Injury Automatic Classifer-)Software-Programms verwendet. AOCOIAC CMF ist benutzerfreundlich strukturiert mit Visualisierung der Regionen und Subregionen innerhalb der anatomischen Module. Durch Anklicken wird die Topografie und Ausdehnung einer Fraktur in den Illustrationen markiert. Uber Pull-down-Menus sind die Eigenschaften der Frakturen ebenso anwahlbar wie uber Datentabellen mit ankreuzbaren Kastchen. In Level 2 wird der Unterkiefer in 9 Subregionen unterteilt: Symphysis (S) + Bilateral: Body (B), Angle/Ascending Ramus (A), Coronoid Process (C), Condylar Process (P). Midface Level 2 differenziert laterale (= Zygoma/Zygomatic Arch Ensemble) und zentrale Kompartimente, wobei das zentrale Mittelgesicht 3 Etagen umfasst: Upper Central Midface (UCM), Intermediate Central Midface (ICM) und Lower Central Midface (LCM). In Level 3 werden die Subregionen im Processus condylaris und im Mittelgesicht in noch feinere Einheiten aufgegliedert. Zudem sind individuelle Patientenmerkmale, namlich der pratraumatische Zahnstatus und die Alveolarfortsatzatrophie bei Zahnlosigkeit registrierbar. Auf dieser Basis wird es dann einfach, typische Frakturentitaten, d. h. Unterkiefer-Gelenkfortsatzfrakturen, Nasenskelett und Naso-Orbit-Ethmoidalfrakturen, Jochbein/Jochbogenfrakturen und Frakturen der Orbitawande in ihrem ganzen Variantenreichtum und bis ins Detail aufzuzeichnen, sowie Zahnhartsubstanz-, Zahnhalteapparatverletzungen und Alveolarfortsatzfrakturen zu dokumentieren. Die Morphologieeigenschaften der Frakturen (Fragmentation, Displacement/Dislokation, Knochendefekte) lassen sich als Attribute hinzufugen, um die Klassifikation auf dieser Stufe zu vervollstandigen. In der Hoffnung der Autoren wird die vorliegende AO-CMF-Traumaklassifikation ein Nukleus zu einer verbesserten Standardisierung in der Befunderhebung und Diagnose von Traumen des gesamten Schadelskeletts, damit in Zukunft gleiche Worte auch das Gleiche meinen.

Frakturen des Jochbeinkomplexes und der Orbita sind haufig. Stumpfe Traumata (Schlag, Sturz) fuhren zu einer Impression der Jochbeinprominenz mit Frakturen an anatomisch vorgegebenen Schwachstellen. Eine Beteiligung des Orbitabodens ist obligat. Ein relevanter Defekt des Orbitabodens kann u. U. erst nach der Reposition des Jochbeinkomplexes sichtbar werden, weshalb eine Exploration des Orbitabodens nach Reposition und Osteosynthese erfolgen sollte, sofern keine intraoperative Bildgebung zur Kontrolle zur Verfugung steht. Hochgeschwindigkeitstraumata fuhren zu komplexen Mittelgesichtsverletzungen und irregularen Frakturmustern. Die CT-Diagnostik des Mittelgesichts und der Orbitae hat die konventionelle Bildgebung verdrangt. Knocherne Verletzungen der Orbitae konnen initial ohne wesentliche klinische Symptomatik einhergehen, weshalb erst in der Feinschichtcomputertomografie eine Fraktur der Orbita erkannt werden kann. Die Indikation zur operativen Versorgung einer Fraktur wird nach Berucksichtigung von klinischen und radiologischen Befunden gestellt. Doppelbildsehen ist ein wichtiges klinisches Kriterium fur die Operation, kann aber nur von temporarer Dauer sein. Neben der Ausdehnung der Defektfraktur im CT (≥ 3 cm2) ist ein Enophthalmus (≥ 2 mm) der wichtigste klinische Parameter fur die Entscheidung zur operativen Revision. Fur die Rekonstruktion der Orbitawande werden heute mehrheitlich Titangitter mit spezifischem Design eingesetzt. In bestimmten klinischen Situationen (Rekonstruktion nach Tumorresektion) sind autologe Transplantate (tabula externa) immer noch zu favorisieren. Bezuglich Prazision und postoperativer Komplikationsrate bestehen keine Unterschiede zwischen autologen Transplantaten und Titanimplantaten. Die 3-dimensionale Planung und intraoperative Navigation hat sich v. a. bei den sekundaren Korrekturen von in Fehlstellung verheilten Mittelgesichtsverletzungen bewahrt. Die funktionellen Resultate nach sekundarer Korrektur eines Enophthalmus sind nicht ermutigend. Eine besondere Gewichtung erhalt deshalb die prazise klinische und radiologische Diagnostik, die korrekte Indikationsstellung und die primare sowie zeitnahe Versorgung von knochernen Verletzungen der Orbita.

Heutzutage gilt der totale alloplastische Kiefergelenkersatz bei Pathologien mit irreversiblen Form- und Dimensionsanderungen des Processus condylaris mandibulae als verlassliche Behandlungsoption. Bei Defekten, die uber den Gelenkfortsatz und die Gelenkgrube hinaus weitere Unterkiefereinheiten und/oder die Jochbogen/Jochbeinregion involvieren, lasst sich das Design individuell gefertigter Endoprothesen so gestalten, dass benachbarte Knochendefizite ausgeglichen werden konnen. Alloplastische Rekonstruktionen dieser Art kommen infrage, wenn autologe Rekonstruktionsverfahren mit erheblichem Zusatzaufwand verbunden oder nicht durchfuhrbar sind. Wahrend die anatomische Form und Konturgebung in asthetisch und funktionell befriedigender Weise wiederhergestellt werden konnen, darf von einem alloplastischen Gelenkersatz mit Reduktion auf eine einfache Biomechanik von vornherein nicht erwartet werden, die Bewegungsablaufe und Interaktionen innerhalb des komplexen stomatognathen Systems vollkommen zu replizieren.

Die Verwendung des Fixateur externe stellt grundsatzlich eine temporare Alternative zur internen lasttragenden Plattenosteosynthese bei Defektzonen im Unterkiefer dar. Er setzt sich aus 3 Hauptkomponenten zusammen, die allesamt aus Titan bestehen: 1) Pins oder Schanz-Schrauben, 2) vorgeformten Verbindungsstaben, 3) verstellbaren, kombinierten Klemmbacken mit Schnappmechanismus. Das Gesamtkonstrukt kann sowohl in „Visor-style“-Technik als auch in Modulartechnik zusammengesetzt werden. Der Gebrauch ist speziellen Indikationen vorbehalten: a) pathologische Frakturen, meist im Rahmen von infizierten Osteoradionekrosen, b) weitspannige Defekte nach ablativer Tumorchirurgie, c) Notfallmanagement beispielsweise bei Polytraumapatienten mit Unterkiefertrummerfrakturen. Unserer Erfahrung nach ist die Technik komplikationsarm und verlasslich anwendbar. Sie genugt den Anforderungen einer lasttragenden Osteosynthese. Der Mandible External Fixator ist nicht zuletzt aufgrund der geringen Weichgewebstraumatisierung eine interessante Option bei multifragmentaren Frakturen oder Defektzonen, die mit einer reduzierten Knochenqualitat und/oder Weichgewebsdefiziten einhergehen.

Wahrend bei Erwachsenen weitgehend klare Indikationen fur die operative Versorgung von Kiefergelenkfortsatzfrakturen bestehen und diese Frakturen mehrheitlich operativ versorgt werden, wird bei Kindern und Jugendlichen immer noch ein nicht operatives (konservatives) Behandlungskonzept bevorzugt. Argument dafur ist das vermeintlich hohe spontane Regenerationspotenzial bis ins fruhe Teenageralter. Stark dislozierte (> 45° Achsabweichung) und luxierte Kiefergelenkfortsatzbasis- und Kiefergelenkhalsfrakturen, die nicht operativ behandelt wurden, fuhren bei Kindern und Jugendlichen nicht selten zu Wachstumsstorungen mit Gesichtsasymmetrien, Bissstorungen und Dysplasien im Kiefergelenkbereich. Aktuelle Therapiekonzepte tendieren deshalb auch bei Kindern und Jugendlichen mit Luxationen oder erheblichen Dislokationen zu einer operativen Versorgung mit anatomisch korrekter Reposition und rigider Fixierung. Dieser Paradigmenwechsel von geschlossener Behandlung zur Osteosynthese uber einen transfazialen oder insbesondere den hier dargestellten transoralen Zugangsweg hat sich erst auf der Grundlage von Fortschritten in der Behandlung von Kiefergelenkfortsatzfrakturen bei Erwachsenen entwickelt. Im Folgenden werden Uberlegungen zu einem Klassifikationsvorschlag fur padiatrische Kiefergelenkbasis- und Kiefergelenkhalsfrakturen, die typische klinische Symptomatik einschlieslich der bildgebenden Diagnostik und das derzeitige Behandlungskonzept mit dem operativ-technischen Vorgehen dargestellt.

Lasionen des N. lingualis oder N. alveolaris inferior, die keine Aussicht auf eine Spontanregeneration und Wiederherstellung der sensiblen Restitution besitzen, kommen im Rahmen von Tumorresektionen, Traumen und akzidentell bei dentoalveolaren oder orthognath-chirurgischen Eingriffen vor. Bei geschlossenen Nervenverletzungen ist die klinische Differenzierung einer irreversiblen Lasion und somit die Indikationsstellung zur operativen Reparatur oder Rekonstruktion nur durch Verlaufsbeobachtung moglich. Ein fruhzeitiges Timing bis zur Wiederherstellungsoperation gilt als kritisch fur die funktionelle neurosensorische Restitution. Der Frage, welche Grosenordnung dem optimalen Zeitfenster zugemessen werden sollte, wird in einer Literaturubersicht des letzten Jahrzehnts nachgegangen. Nach Darstellung der heute verfugbaren mikroneuralen Operationstechniken werden unter Berucksichtigung der derzeitigen Datenlage Uberlegungen und Wege zur Entscheidungsfindung sowie zur Operationsindikationsstellung in verschiedenen Verletzungsszenarien vorgestellt.

Purpose To evaluate the possible benefits of open surgery, endoscopically assisted reduction and fixation using a transoral route was used in a selected series of pediatric patients with displaced condylar base and neck fractures. Patients and Methods A cohort of 6 patients (1 male and 5 female; age range, 7 to 15 yr; mean, 13.4 yr) with displaced condylar base and neck fractures (n = 9) were included. Inclusion criteria were age younger than 16 years, fracture of the condylar base or neck, and displacement of the fracture by at least 45°. Fractures were classified using conventional radiography, cone-beam computed tomography, or computed tomography. Patients underwent transoral endoscopically assisted open reduction and fixation using miniplate osteosynthesis. Postoperatively, patients were followed clinically and radiographically for 18 months. Results Complete follow-up varied from 18 to 35 months (median, 24.5 months). All patients showed normal occlusion and pain-free unrestricted function of the temporomandibular joint at 3, 6, 12, and 18 months postoperatively. There were no signs of incomplete remodeling or deformation of the condyles. Conclusion Transoral endoscopically assisted surgical treatment of severely displaced condylar base and neck fractures in children and young teenagers offers a reliable solution to preclude the sequelae of closed treatment, such as altered morphology and functional disturbances, eliminates visible scars, and lowers the risk of facial nerve damage compared with open reduction using an extraoral approach.

Purpose Magnetic resonance imaging (MRI) is an excellent imaging modality for displaying peripheral nerves. Since the knowledge about MRI of the inferior alveolar nerve (IAN) is limited, this pilot study aims to identify the prospects and limitations of MRI of the IAN, with special consideration of metal artifacts. Materials and methods Initially, in vitro MRI of a dental implant was performed to establish an optimized protocol for metal artifact reduction using WARP sequences (a software package provided by Siemens Healthcare, Erlangen, Germany) including view angle tilting (VAT) and slice-encoding metal artifact correction (SEMAC) techniques. MRI with this optimized protocol was performed in three volunteers and four patients presenting with postoperative IAN impairment. Measuring the evaluable area and the artifact size was performed to assess the benefit of the specific artifact reduction sequences. Results In vitro imaging of a dental implant demonstrated that WARP sequences with VAT and SEMAC techniques led to a volume reduction of the artifact of up to 69.1%. Observations in both volunteers and patients with neurosensory IAN impairment showed a distinct artifact reduction with the MRI protocol adapted to metallic materials. Additionally VAT and SEMAC techniques improved the imaging due to further artifact reduction. As a main drawback of the VAT technique, the image quality was compromised by a blurring effect. Still, on 3-T MRI the resolution was high enough to reveal even fine structures. Imaging of the IAN was successful in all cases despite metallic material in the region of interest, and structural IAN changes could be detected in correlation with clinical symptoms. Conclusion In contrast to conventional radiography and computed tomography, MRI can directly depict the IAN and provide reliable information on its position and exact course within the mandible. MRI offers an objective assessment of IAN injuries, supporting the decision-making process regarding surgical exploration and microneural repair. With the advent of specialized MRI techniques such as VAT and SEMAC, reduction of metal artifacts is considerably improved.

Sie eignen sich sowohl fur eine Insertion uber den klassischen extraoralen Zugang als auch fur die transorale Applikation. Erste klinische Erfahrungen deuten darauf hin, dass das Risiko von Ermudungsbruchen und die Operationszeit aufgrund der Praformierung der Platten sinken. Insgesamt sind MMPRP als ein geeignetes Standardsystem fur die winkelstabile und lasttragende Osteosynthese im Bereich des Unterkiefers zu bewerten. Dies gilt fur eine Vielzahl von Indikationen aus der Tumorchirurgie, Traumatologie sowie beim Management von Osteonekrosen.
