Aims To estimate whether the use of wagering inducements has a significant impact on the gambling behaviors of on-line gamblers and describe this temporal relation under naturalistic conditions. Design This longitudinal observational study is part of the second stage of the Screening for Excessive Gambling Behaviors on the Internet (EDEIN) research program. Setting Gambling tracking data from the French national on-line gambling authority (poker, horse race betting and sports betting) and from the French national lottery operator (lotteries and scratch games). Participants A total of 9306 gamblers who played poker, horse race or sports betting and 5682 gamblers who played lotteries and scratch games completed an on-line survey. The gender ratio was largely male (between 87.1% and 92.9% for poker, horse race betting and sports betting, and equal to 65.1% for lotteries). Median age ranged from 35 (sports betting) to 53 (horse race betting and lotteries). Measurements The survey used the Problem Gambling Severity Index (PGSI) to determine the status of the gamblers (at-risk or not). Gambling tracking data included weekly gambling intensity (wagers, deposits), gambling frequency (number of gambling days), proxies of at-risk gambling behaviors (chasing and breadth of involvement) and use of wagering inducements. Findings The use of wagering inducements was associated with an increase of gambling intensity [β between -0.06 (-0.08; -0.05) and 0.57 (0.54; 0.60)], gambling frequency [β between 0.12 (0.10; 0.18) and 0.29 (0.28; 0.31)] and at-risk gambling behaviors [odds ratio between 1.32 (1.16; 1.50) and 4.82 (4.61; 5.05)] at the same week of their use. This effect was stronger for at-risk gambling behaviors and at-risk gamblers. Conclusions Wagering inducements may represent a risk factor for developing or exacerbating gambling problems.

International audience; Background and aims: Since June 2018, gaming disorder (GD) has been recognized as a disease. It is frequently associated with attention deficit hyperactivity disorder (ADHD), as there are common vulnerability factors and bidirectional interactions between the two disorders. This study aims to evaluate the presence of ADHD symptoms and predictive factors of ADHD among patients with GD. Methods: Ninety-seven patients ≥16 years old referred to the University Hospital of Nantes between 2012 and 2020 for GD were included. The diagnosis of GD was given a posteriori in accordance with the new ICD-11 GD definition. ADHD was screened using the Adult-ADHD Self-Report Scale and the Wender-Utah Rating Scale. A multivariate logistic regression model was used to identify explanatory factors for ADHD-GD comorbidity. Results: The rate of GD patients who screened positive for ADHD was 39%. Predictive factors of ADHD-GD comorbidity were impulsivity (higher score on the negative urgency dimension) and low self-esteem. Discussion: The rate of ADHD found among patients with GD is consistent with that from the literature on internet GD but higher than that found for other behavioural addictions. The identification of a higher negative urgency score and low self-esteem as predictive factors of AHDH-GD comorbidity indicates that gaming could be considered a dysfunctional way to cope with emotional dysregulation in ADHD or to virtually escape. Conclusions: Comorbid ADHD must be taken into consideration to minimize its functional impact on GD patients and gamingrelated damage. In contrast, the evaluation of gaming habits in patients with ADHD could be useful for both prevention and care.

Ma thématique de recherche s’articule autour des différents niveaux de vulnérabilité aux addictions comportementales, qu’ils tiennent à l’environnement, à l’individu ou à son fonctionnement cognitif, avec pour objectif de mieux les comprendre pour mieux les prévenir et les soigner. Mon travail s’inscrit dans une dynamique de recherche pluridisciplinaire, s’appuyant à la fois sur l’expertise clinique des praticiens du service d’Addictologie du CHU de Nantes, sur l’activité de recherche clinique de l’Unité d’Investigation Clinique « Psychiatrie et Santé Mentale » que je coordonne, et sur les recherches en méthodologie de la recherche en santé effectuées au sein de l’UMR INSERM 1246 SPHERE « methodS in Patient-centered outcomes and HEalth ResEarch » dont je suis membre permanent depuis 2011 (membre de l’EA4275 de 2011 à 2016, jusqu’à sa transformation en UMR INSERM 1246 en 2017).

International audience; Les troubles addictifs et de l'humeur sont fréquemment cooccurrents. Le trouble lié au jeu d'argent est ainsi souvent accompagné de troubles de l'humeur, notamment d'épisode dépressif majeur, de trouble bipolaire ou de comportement suicidaire. De nombreuses études ont observé leur association, évalué leur prévalence et leur chronologie d'apparition, mais peu ont exploré l'impact d'un trouble de l'humeur, ou thymique, sur l'évolution du trouble lié au jeu d'argent. L'objectif de cette étude était d'évaluer le devenir à 1 an (rémission précoce) d'une cohorte de joueurs pathologiques traités, en fonction de la présence ou non d'antécédents de trouble de l'humeur à l'inclusion. Dans cette étude longitudinale monocentrique, le trouble lié au jeu d'argent et les troubles thymiques ont été évalués à l'inclusion et 12 mois après, au cours d'un entretien clinique standardisé. Près de 100 patients (N = 94, 45 sans trouble de l'humeur et 49 avec trouble de l'humeur) se sont présentés aux 2 visites. À 1 an, une rémission précoce du trouble lié au jeu d'argent a été observée chez 42 patients, avec un taux de rémission de 39 % en cas de présence et de 51 % en cas d'absence d'antécédent de trouble de l'humeur. Nous n'avons pas démontré qu'un antécédent de trouble de l'humeur à l'inclusion était un facteur prédictif de la rémission précoce du trouble lié au jeu d'argent. Le potentiel manque de puissance de notre étude peut expliquer ce résultat. Cependant, les prédicteurs de rémission précoce identifiés dans cette étude étaient la vie en couple, l'absence de trouble anxieux et l'absence de trouble lié à la consommation d'alcool.

International audience; BACKGROUND: Internet gamblers may be at risk for excessive gambling, but internet gambling also provides a unique opportunity to monitor gambling behavior in real environments and intervene early with those who encounter difficulties. OBJECTIVE: The objective of the present study was to model the early gambling trajectories of online lottery gamblers during the first months after the creation of their accounts. METHODS: Anonymized gambling-related records from 1,152 clients of the French national lottery operator who created their Internet gambling accounts between September 2015 and February 2016 were analyzed using a two-step approach combining growth mixture modeling and latent class analysis. The analysis was based on behavioral markers of the raw gambling activity (money wagered, number of gambling days) and the potential for gambling problems (breadth involvement and chasing). The obtained profiles were described based on the probabilities of following the trajectories identified for the four indicators, and on various covariates (age, sex, deposits, type of lotteries, etc.). More specifically, net losses, voluntary self-exclusion and Playscan status (a Responsible Gambling tool that provides each gambler with a risk assessment: green for low risk, orange for medium risk and red for high risk) were used as external verification of problem gambling. RESULTS: We identified 5 distinct profiles of online lottery gamblers. Classes 1 (57%), 2 (15%) and 3 (14%) were characterized by low to medium gambling activity and low levels of problem gambling markers. They displayed low levels of net losses, no recourse to the voluntary self-exclusion measure and predominantly green tags (90%-98%) within the Playscan classification. Class 4 (10%) was characterized by medium to high gambling activity, a diversification of games played (1 to 6) and zero to few chasing episodes. They displayed a high level of net losses, but were predominantly tagged as green (66%) or orange (25%) in the Playscan classification and did not use the voluntary self-exclusion measure. Finally, class 5 (5%) was characterized by a medium to very high gambling activity, a higher diversification of games played (up to 17) and a high number of chasing episodes (0-5). They experienced the highest net losses, far beyond the losses of the other classes, the highest proportion of orange (32%) and red (39%) tags within the Playscan classification, and represented the only class in which voluntary self-exclusion was present. CONCLUSIONS: The first three classes may be considered recreational gamblers. The fourth class had a higher gambling activity and breadth involvement and may represent gamblers at risk for future gambling problems. The last class stood out in terms of a much higher gambling activity and breadth involvement and the presence of chasing behavior. The last two classes may benefit from early preventive measures.
