This study examined HRQoL changes after biologic treatment initiation among a large, real-world cohort of patients with SA.Patients and methods: CHRONICLE is an ongoing observational study of subspecialist-treated adults with SA who receive biologics or maintenance systemic corticosteroids or are uncontrolled on high-dosage inhaled corticosteroids with additional controllers. Patients enrolled February 2018–February 2023 were asked to complete the St. George’s Respiratory Questionnaire (SGRQ) every 6 months (total score range of 0– 100 [0=best possible health], meaningful change threshold is a 4-unit reduction in the total score). Changes in SGRQ responses from 6 months before initiation to 12 to 18 months after initiation were summarized.Results: A total of 76 patients completed the SGRQ 0 to 6 months before and 12 to 18 months after biologic initiation. The mean (SD) SGRQ total score decreased from 52.2 (20.6) to 41.9 (23.8), with improvement across the symptoms (− 14.5), activity (− 11.0), and impacts (− 8.3) components. For specific impairments reported by ≥ 50% of patients before biologic initiation, fewer reported each impairment after biologic initiation; the largest reductions were for “Questions about what activities usually make you feel short of breath these days [Walking outside on level ground]” (67% to 43%), “Questions about other effects that your respiratory problems may have on you these days [I feel that I am not in control of my respiratory problems]” (55% to 34%), and “Questions about your cough and shortness of breath these days [My coughing or breathing disturbs my sleep]” (63% to 45%).Conclusion: In this real-world cohort of adults with SA, biologic initiation was associated with meaningful improvements in asthma-related HRQoL. An easy-to-implement measure of asthma control that assesses both symptom impairment and exacerbation risk and reflects the impact of asthma on patients’ lives is lacking. Hence, the objective of this study was to assess the Asthma Impairment and Risk Questionnaire (AIRQ®) construct validity relative to patient self-perception of asthma status and validated disease-specific patient-reported outcome (PRO) measures.Patients and methods: Baseline data were analyzed from patients (aged ≥ 12 years) with asthma participating in a 12-month observational study assessing the ability of AIRQ to predict exacerbations. At entry, patients completed a sociodemographic questionnaire, AIRQ, 3 questions addressing self-perceived asthma status, Saint George’s Respiratory Questionnaire (SGRQ), mini-Asthma Quality of Life Questionnaire (AQLQ), and Adult Asthma Adherence Questionnaire (AAAQ). Descriptive statistics were calculated for demographic and clinical characteristics. AIRQ construct validity was evaluated by assessing correlations between total AIRQ score and patient self-assessments, SGRQ, mini-AQLQ, and AAAQ scores. Comparisons of SGRQ, mini-AQLQ, and AAAQ total and component/domain scores by AIRQ control category were performed using general linear models and Scheffe’s post hoc adjustments for pairwise comparisons.Results: A total of 1112 patients were enrolled: 70% female, 78% White, mean (standard deviation) age 43.9 (19.5) years. There were highly significant correlations between AIRQ score and patient self-perception of overall control (r = 0.69; p < 0.001), total SGRQ (r = 0.74, p < 0.001), and mini-AQLQ (r = − 0.78, p < 0.001) scores. As AIRQ control category worsened, so did total and domain SGRQ, mini-AQLQ, and AAAQ impediment-to-inhaled-corticosteroid-adherence scores (all pairwise comparisons p < 0.001).Conclusion: Findings demonstrate the construct validity of AIRQ relative to patient self-perception of asthma status, disease-specific PRO measures, and treatment adherence barriers. The objective of this study was to describe the prevalence of potential adverse effects of long-term mSCS therapy in adults with specialist-confirmed SA in the United States (US). CHRONICLE is an ongoing, noninterventional, observational study of US adults with SA treated by allergists/immunologists and pulmonologists. Once enrolled, patients’ duration of mSCS therapy was reported by sites based on medical record review. For patients enrolled between February 2018 and February 2021, the prevalence of SCS-associated conditions was evaluated among those with no reported history of mSCS use, or mSCS use with < 2 years or ≥ 2 years cumulative duration. Prevalence and incidence estimates were adjusted for age and smoking history. Of 2793 patients enrolled, 311 and 231 had mSCS use for < 2 and ≥ 2 years, respectively. In adjusted analyses, adrenal insufficiency, pneumonia, type 2 diabetes, osteoporosis/osteopenia, congestive heart failure, coronary artery disease, hypertension, anxiety, and depression were statistically significantly associated with any mSCS use. By duration, mSCS use ≥ 2 years was associated with osteopenia/osteoporosis, coronary artery disease, adrenal insufficiency, and diabetes-related neuropathy; mSCS use < 2 years was associated with depression and osteopenia/osteoporosis, and diagnoses of depression, anxiety, and hypertension during the 12 months prior to enrollment. Overall, among patients with specialist-confirmed SA, mSCS use was associated with a high prevalence of multiple adverse conditions. Healthcare professionals should employ mSCS-sparing treatment strategies to avoid these negative consequences.Keywords: duration, steroids, diabetes, pneumonia, osteoporosis

At enrollment, patients completed the St. George’s Respiratory Questionnaire (SGRQ) and Work Productivity and Activity Impairment (WPAI) questionnaire. Results were analyzed for those enrolled between February 2018 and February 2020.Results: Among patients who completed enrollment questionnaires (n = 1109), mean age was 54 years and most were women (70%). Among SGRQ respondents (n = 960), mean (SD) total score was 43 (23); 51% reported good/very good health. Among WPAI respondents (n = 1057; 566 employed), mean (SD) overall work impairment was 21% (25). Patients receiving biologics (vs mSCS, HD ICS+ only) had better SGRQ total scores (38 vs 59, 48) and lower work impairment (17% vs 34%, 27%). Patients with better SGRQ activity scores relative to symptom scores had better SGRQ impacts scores, total scores, and reported better overall health.Conclusion: SA significantly affects HRQoL, work productivity, and activity. The SGRQ is a valuable research instrument for evaluating HRQoL in SA. 6. Rigor and Relevance in Research Methods: Reflections from a Professor and Alumni of an Online EdD Program

Abstract

EdD students represent diverse individuals with established professional identities who enroll in doctoral programs seeking relevant, useable content. Instructors and program directors must find ways to incorporate rigor and relevance into the readings, assessments, and training for EdD students. This essay explores the evolution of research methods courses in an online education doctoral program. Hochbein and Perry (2013) noted that "research training need not be diluted, but rather tailored to the specific needs of scholarly practitioners" (p.182). Our narrative integrates the unique perspectives of a former EdD research methods faculty member and two recent alumni to describe one program's efforts to maintain the rigor of doctoral research methods courses and better align student experiences to their needs and professional context. Relying on research literature and experiential evidence, we offer a rich recounting of a rationale for change and how these adjustments contributed to scholarly practitioner training and research journeys. The essay identifies requisite knowledge of scholarly practitioners and describes the integration of learning opportunities across the courses.

Abstract

Absolute cross sections measured using electromagnetic devices to separate and detect heavy recoiling ions need to be corrected for charge state fractions. Accurate prediction of charge state distributions using theoretical models is not always a possibility, especially in energy and mass regions where data is sparse. As such, it is often necessary to measure charge state fractions directly. In this paper we present a novel method of using a scintillation screen along with a CMOS camera to image the charge dispersed beam after a set of magnetic dipoles. A measurement of the charge state distribution for 88Sr passing through a natural carbon foil is performed. Using a Bayesian model to extract statistically meaningful uncertainties from these images, we find agreement between the new method and a more traditional method using Faraday cups. Future work is need to better understand systematic uncertainties. Our technique offers a viable method to measure charge state distributions.

Abstract

The rate of the final step in the astrophysical $\alpha$p-process, the $^{34}$Ar($\alpha$,\textit{p})$^{37}$K reaction, suffers from large uncertainties due to lack of experimental data, despite having a considerable impact on the observable light curves of x-ray bursts and the composition of the ashes of hydrogen and helium burning on accreting neutron stars. We present the first direct measurement constraining the $^{34}$Ar($\alpha$,p)$^{37}$K reaction cross section, using the Jet Experiments in Nuclear Structure and Astrophysics (JENSA) gas jet target. The combined cross section for the $^{34}$Ar,Cl($\alpha$,p)$^{37}$K,Ar reaction is found to agree well with Hauser-Feshbach predictions. The $^{34}$Ar($\alpha$,2p)$^{36}$Ar cross section, which can be exclusively attributed to the $^{34}$Ar beam component, also agrees to within the typical uncertainties quoted for statistical models. This indicates the applicability of the statistical model for predicting astrophysical ($\alpha$,p) reaction rates in this part of the $\alpha$p process, in contrast to earlier findings from indirect reaction studies indicating orders-of-magnitude discrepancies. Abstract

Ice deposits on Earth provide an extended record of volcanism, planetary climate, and life. On Mars, such a record may extend as far back as tens to hundreds of millions of years (My), compared to only a few My on Earth. Here, we propose and demonstrate a compact instrument, the Melter-Sublimator for Ice Science (MSIS), and describe its potential use cases. Similar to current use in the analysis of ice cores, linking MSIS to downstream elemental, chemical, and biological analyses could address whether Mars is, or was in the recent past, volcanically active, enable the creation of a detailed climate history of the late Amazonian, and seek evidence of subsurface life preserved in ice sheets. The sublimation feature can not only serve as a preconcentrator for in-situ analyses, but also enable the collection of rare material such as cosmogenic nuclides, which could be returned to Earth and used to confirm and expand the record of nearby supernovas and long-term trends in space weather. Missions to Ocean Worlds such as Europa or Enceladus will involve ice processing, and there MSIS would deliver liquid samples for downstream wet chemistry analyses. Our combined melter-sublimator system can thus help to address diverse questions in heliophysics, habitability, and astrobiology., Comment: 10 pages, 5 figures

Abstract

Absolute cross sections for the addition of $s$- and $d$-wave neutrons to $^{14}$C and $^{14}$N have been determined simultaneously via the ($d$,$p$) reaction at 10 MeV/u. The difference between the neutron and proton separation energies, $\Delta S$, is around $-20$ MeV for the $^{14}$C$+$$n$ system and $+8$ MeV for $^{14}$N$+$$n$. The population of the $1s_{1/2}$ and $0d_{5/2}$ orbitals for both systems is reduced by a factor of approximately 0.5 compared to the independent single-particle model, or about 0.6 when compared to the shell model. This finding strongly contrasts with results deduced from intermediate-energy knockout reactions between similar nuclei on targets of $^{9}$Be and $^{12}$C. Abstract

Nuclear Astrophysics is a field at the intersection of nuclear physics and astrophysics, which seeks to understand the nuclear engines of astronomical objects and the origin of the chemical elements. This white paper summarizes progress and status of the field, the new open questions that have emerged, and the tremendous scientific opportunities that have opened up with major advances in capabilities across an ever growing number of disciplines and subfields that need to be integrated. We take a holistic view of the field discussing the unique challenges and opportunities in nuclear astrophysics in regards to science, diversity, education, and the interdisciplinarity and breadth of the field. Abstract

The $^{30}$P$(p,\gamma)^{31}$S reaction plays an important role in understanding nucleosynthesis of $A\geq 30$ nuclides in oxygen-neon novae. The Gaseous Detector with Germanium Tagging was used to measure $^{31}$Cl $\beta$-delayed proton decay through the key $J^{\pi}=3/2^{+}$, 260-keV resonance. The intensity $I^{260}_{\beta p} = 8.3^{+1.2}_{-0.9} \times 10^{-6}$ represents the weakest $\beta$-delayed, charged-particle emission ever measured below 400 keV, resulting in a proton branching ratio of $\Gamma_p / \Gamma = 2.5^{+0.4}_{-0.3} \times 10^{-4}$. By combining this measurement with shell-model calculations for $\Gamma_{\gamma}$ and past work on other resonances, the total $^{30}$P$(p,\gamma)^{31}$S rate has been determined with reduced uncertainty. The new rate has been used in hydrodynamic simulations to model the composition of nova ejecta, leading to a concrete prediction of $^{30}$Si/$^{28}$Si excesses in presolar nova grains and the calibration of nuclear thermometers., Comment: 7 pages, 2 figures, accepted to Physical Review Letters on April 4, 2022

Abstract

The SEparator for CApture Reactions (SECAR) is a next-generation recoil separator system at the Facility for Rare Isotope Beams (FRIB) designed for the direct measurement of capture reactions on unstable nuclei in inverse kinematics. To maximize the performance of this system, stringent requirements on the beam alignment to the central beam axis and on the ion-optical settings need to be achieved. These can be difficult to attain through manual tuning by human operators without potentially leaving the system in a sub-optimal and irreproducible state. In this work, we present the first development of online Bayesian optimization with a Gaussian process model to tune an ion beam through a nuclear astrophysics recoil separator. We show that this method achieves small incoming angular deviations (\textless 1 mrad) in an efficient and reproducible manner that is at least three times faster than standard hand-tuning. Additionally, we present a Bayesian method for experimental optimization of the ion optics, and show that it validates the nominal theoretical ion-optical settings of the device, and improves the mass separation by 32\% for some beams.

Abstract

An in-flight beam of $^{16}$N was produced via the single-neutron adding ($d$,$p$) reaction in inverse kinematics at the recently upgraded Argonne Tandem Linear Accelerator System (ATLAS) in-flight system. The amount of the $^{16}$N beam which resided in its excited 0.120-MeV $J^{\pi}=0^-$ isomeric state (T$_{1/2}\approx5$ $\mu$s) was determined to be 40(5)% at a reaction energy of 7.9(3) MeV/$u$, and 24(2)% at a reaction energy of 13.2(2) MeV/$u$. The isomer measurements took place at an experimental station $\approx30$ m downstream of the production target and utilized an Al beam-stopping foil and a HPGe Clover detector. Composite $^{16}$N beam rate determinations were made at the experimental station and the focal plane of the Argonne in-flight radioactive ion-beam separator (RAISOR) with Si $\Delta$E-E telescopes. A Distorted Wave Born Approximation (DWBA) approach was coupled with the known spectroscopic information on $^{16}$N in order to estimate the relative $^{16}$N isomer yields and composite $^{16}$N beam rates. In addition to the observed reaction-energy dependence of the isomer fraction, a large sensitivity to angular acceptance of the recoils was also observed., Comment: 21 pages, 7 figures

Abstract

The isomeric state of $^{16}$N was studied using the $^{16}$N$^{g,m}$($d$,$^3$He)~proton-removal reactions at \mbox{11.8~MeV/$u$} in inverse kinematics. The $^{16}$N beam, of which 24% was in the isomeric state, was produced using the ATLAS in-fight facility and delivered to the HELIOS spectrometer, which was used to analyze the $^{3}$He ions from the ($d$,$^{3}$He) reactions. The simultaneous measurement of reactions on both the ground and isomeric states, reduced the systematic uncertainties from the experiment and in the analysis. A direct and reliable comparison of the relative spectroscopic factors was made based on a Distorted-Wave Born Approximation approach. The experimental results suggest that the isomeric state of $^{16}$N is an excited neutron-halo state. Abstract

Due to its importance as an astronomical observable in core-collapse supernovae (CCSNe), the reactions producing and destroying $^{44}$Ti must be well constrained. Generally, statistical model calculations such as Hauser-Feshbach are employed when experimental cross sections are not available, but the variation in such adopted rates can be large. Here, data from the literature is compared with statistical model calculations of the $^{44}$Ti($\alpha$,p)$^{47}$V reaction cross section and used to constrain the possible reaction rate variation over the temperatures relevant to CCSNe.
