There is no established acquisition protocol based on scientific evidence for the acquisition of cone-beam computed tomography (CBCT) exams to evaluate the temporomandibular joint (TMJ).To evaluate the influence of acquisition protocols and jaw positioning on the diagnostic accuracy of TMJ condylar morphological alterations (CMA), dimension, position and excursion.Thirty-six TMJs on 18 dry skulls were imaged using a CBCT unit (OP300 Maxio, Instrumentarium, Tuusula, Finland) at two exposure settings (4.5 and 6.3 mA), three voxel resolutions (0.085, 0.125 and 0.280 mm), three jaw positions (concentric, anteriorised and posteriorised) and three jaw excursions (normoexcursion, hyperexcursion and hypoexcursion). The macroscopic anatomy examination and high-resolution CBCT images were used as ground truth for CMA. Twenty-five TMJs had at least one CMA with 11 healthy TMJs serving as controls. Three experienced oral and maxillofacial radiologists evaluated the parasagittal images for the presence of CMA, position and excursion and measured dimensions. The area under the ROC curve, sensitivity and specificity were calculated. Weighted Kappa (α = 0.05) was used to determine intra- and interexaminer reliability and comparisons between dependent variables analysed by Analysis of Variance at an a prior level of significance of 0.05.The agreement of the evaluation of the position and excursion with the reference standard was high, independent of the protocol (range, 0.75-0.91). Various combinations of acquisition protocols and jaw position did not influence the CMA evaluation. Erosion was overdiagnosed in protocols with larger voxel sizes and the detection of osteophytes greater in images with smaller voxel sizes. The anteroposterior dimension was greater in the open jaw position (p .05).CBCT protocols using reduced radiation exposure from the CBCT machine evaluated in this study can be used to assess condylar morphology, dimension, position and excursion, without compromising diagnostic performances for these parameters.

OBJECTIVE: To evaluate the influence of metal artefact reduction (MAR) in the diagnosis of dental implant contact with the mandibular canal (MC) using cone beam computed tomography (CBCT).METHODS: Dental implants were installed with surgical guides in the posterior hemiarches of 10 dry human mandibles: 0.5 mm above to the MC cortex (G1/n = 8) and 0.5 mm inside the MC (G2/n = 10). The experimental set-up was scanned with two CBCT equipment using 85 kV and 90 kV, MAR ON or OFF, and different tube currents (4 mA, 8 mA and 10 mA). Two dentomaxillofacial radiologists (DMFRs) and two dentists (DDS) scored the relation between the dental implant and MC. Descriptive statistics were used to observe the absolute frequency of scores. Sensitivity, specificity and accuracy were calculated considering the known relation between the dental implant and the MC interior. McNemar's test (α = .05) was applied to compare the diagnostic efficacy of MAR ON versus MAR OFF.RESULTS: Overall specificity was higher than sensitivity for both DDS and DMFR (97% vs. 50% and 92.0% vs. 78.0% respectively). There was a significant effect of MAR (p = .031) for DMFR in the case of contact between the dental implant with the MC interior, in which sensitivity decreased with MAR activation from 90% to 40%. DMFR observers showed a better diagnostic performance compared with the DDS observers (accuracy of 84.0% and 71.0%, respectively).CONCLUSIONS: Due to the limited efficacy of MAR, it should not be used when conducting CBCT scans for the evaluation of contact between the implant and the mandibular canal.

The aim of this study was to observed the anesthetic efficacy of the alveolar nerve block on nine patients that CBCT diagnosed unilateral retromolar canal on a double-blind, split-mouth approach. The assessments of patient response to thermal (pulp vitality test) and pressure (compression of soft tissue) stimuli were carried out before and 5 minutes after the inferior alveolar nerve block procedure, using both visual analog scale (VAS) and Mc Gill pain questionnaires (McG). The mean percentage of patient response decreased after alveolar nerve block, according to both VAS and McG, and was statistically similar among hemi mandibles with and without retromolar canal (Wilcoxon>0.05); however, those without retromolar canal presented greater reduction in patient response in 6 out of 9 cases. Therefore, the retromolar canal is not a determinant factor of inferior alveolar nerve block failure. Resumen El objetivo de este estudio fue observar la eficacia anestésica del bloqueo del nervio alveolar en nueve pacientes que CBCT diagnosticó canal retromolar unilateral en un abordaje de boca dividida doble ciego. Las evaluaciones de la respuesta del paciente a los estímulos térmicos (prueba de vitalidad pulpar) y de presión (compresión de los tejidos blandos) se realizaron antes y 5 minutos después del procedimiento de bloqueo del nervio alveolar inferior, utilizando tanto la escala analógica visual (VAS) como los cuestionarios de dolor de Mc Gill ( McG). El porcentaje medio de respuesta de los pacientes disminuyó tras el bloqueo del nervio alveolar, según EVA y McG, y fue estadísticamente similar entre hemimandíbulas con y sin canal retromolar (Wilcoxon>0,05); sin embargo, aquellos sin canal retromolar presentaron mayor reducción en la respuesta del paciente en 6 de 9 casos. Por lo tanto, el canal retromolar no es un factor determinante del fracaso del bloqueo del nervio alveolar inferior.

To evaluate whether the automatic exposure compensation in the presence of high-density materials can affect the measurement of alveolar bone level.Thirty regions of seven dry skulls and six mandibles were radiographed with and without a high-density material, using two digital radiographic technologies: photostimulable phosphor plate (PSP, Digora Optime) and sensor (CMOS, Digora Toto), totaling 120 images. The distances from the cement-enamel junction to the alveolar bone crest were measured using cone-beam computed tomography (CBCT) images to represent the reference standard. The same measurements of alveolar bone level and the average of the pixel values of the image were evaluated on the radiographs. Paired t test compared the average pixel values and alveolar bone-level measurements between images with and without high-density material. One-way analysis of variance compared the difference between radiographic and CBCT measurements (α = 0.05).The high-density material reduced the pixel values in PSP (p = 0.002) and CMOS (p 0.001) technologies, demonstrating the AEC functioning in both technologies. There was no difference in bone-level measurements between the images without and with the high-density material for both technologies (p ≥ 0.091), or between the tomographic and radiographic measurements (p ≥ 0.319).In the presence of high-density material, the automatic exposure compensation reduces the average pixel values of the images (i.e., images get darker), but does not influence the radiographic measurements of alveolar bone level.

To evaluate the influence of sagittal and vertical skeletal patterns (skeletal classes and facial profiles), and sex on the distance between upper central incisors roots and incisive canal.Cone-beam computed tomography exams of 172 patients were classified into skeletal classes I, II, and III and according to the facial profile into mesofacial, brachyfacial, and dolichofacial. In sagittal reconstructions, linear measurements were done between central incisors roots and incisive canal at three levels: incisive foramen opening, intermediate region, and the region close to the apex of upper central incisors. In axial reconstructions, measurements of incisive canal width and distance between the roots were obtained. Analysis of Variance (three-way ANOVA) was used to evaluate measurement differences in relation to skeletal classes, facial profiles, and sex.The distance between upper central incisors roots and incisive canal anterior region was little influenced by sagittal and vertical skeletal patterns and sex. Men had significantly greater incisive canal (mean=2.85mm; SD=1.18) width compared to women (mean=2.40mm; SD=1.04), regardless of skeletal class and facial profile (P0.05). In general, inter-root distance between medial points of central incisors roots and incisive canal opening was greater in the region close to upper central incisors apex (mean=3.84mm; SD=1.44) than in incisive foramen opening (2.44mm; SD=1.04), (P0.05).The distance between upper central incisors roots and incisive canal is little influenced by sagittal and vertical skeletal patterns and sex.

This study aimed to evaluate the influence of the number of basis images and the metal artifact reduction (MAR) tool on the production of artifacts near and far from a zirconium implant in cone-beam computed tomography (CBCT). CBCT scans of a mandible were acquired before and after insertion of an implant, using 450 and 720 basis images, with and without MAR activation. The mean and standard deviation (SD) of the gray values of the regions of interest (ROIs) located on the cortices adjacent to the implant and at different distances from it (in the soft tissue) were calculated. The mean of the gray values was used to calculate the absolute contrast difference (ACD) between the control and implant scans. In general, the number of basis images did not affect the SD and the ACD values of the buccal and lingual ROIs (p > 0.05). The implant increased the SD in the lingual cortical plate (p < 0.05). In this case, MAR activation decreased SD (p < 0.05). All ROIs located at different distances from the implant showed higher SD on scans acquired with 450 basis images (p < 0.05), regardless of MAR condition. A higher number of basis images reduces the magnitude of artifacts but does not influence the image quality in bone cortical plates. MAR improves the image in the areas most affected by artifacts. The number of basis images is known as a factor capable of influencing the image quality and radiation dose for the patient. Therefore, it is important to investigate its effect on the expression of artifacts in the CBCT images.

Objective The aim of this study was to evaluate the influence of image enhancement filters on detection of fractured endodontic instruments and on image noise and pixel intensity. Study Design In total, 26 mandibular molars (68 root canals), divided into 4 groups (presence/absence of root canal filling, and presence/absence of a fractured endodontic instrument) were radiographed by using a complementary metal oxide semiconductor (CMOS) and a photostimulable phosphor (PSP) system. Images with 4 enhancement filters (Sharpen UM, Edges All, BC Low, and BC High) were compared with nonfiltered images. The area under the receiver operating characteristic curve (Az), sensitivity, and specificity in detection of fractured instruments were compared for each imaging condition. Means and standard deviations (SD) to measure noise and changes in pixel intensity were calculated. Analysis of variance was used to analyze the data. Results There were no significant differences in Az, sensitivity, and specificity values between nonfiltered and filtered images in detection of instruments in root canals without and with root canal filling (P ≥ .360). SD was reduced by filters in CMOS but not PSP radiographs. Sharpen UM caused more changes in the mean values in both systems. Conclusions Filters did not improve diagnostic performance in detecting instruments. Noise and pixel intensity were affected differently.

The present study correlated the mineralization of third molars to chronological age using a modified classification based on Demirjian’s stages in a Brazilian subpopulation and compared with the original classification. A total of 1082 patients with age ranging from 6 to 26 years were included in the sample, with at least one third molar on panoramic radiographs. The third molars were classified according to the original Demirjian classification (8 stages) and a new model based on the Demirjian method, where the original stages were grouped into four stages: AB—enamel mineralization; CD—crown dentin mineralization; EFG—root formation; and H—complete development. Statistical analyses were performed by Kruskal-Wallis/Dunn tests (α = 0.05) and the multinomial logistic regression model. Data were analyzed according to percentiles for the probability of an individual being over 18 years old. The main aim is to identify, by means of different imaging modalities, the early bone changes in patients “at risk” and in stage 0 MRONJ. A search of the literature was performed on PubMed, Embase, Web of Science, and Cochrane Library databases, until June 9, 2020. No language or year restrictions were applied. Screening of the articles, data collection, and qualitative analysis was done. The Newcastle-Ottawa Scale (NOS) was used for observational studies, and the Systematic Review Centre for Laboratory Animal Experimentation’s (SYRCLE) risk of bias tool for the animal studies. A total of 1188 articles were found, from which 47 were considered eligible, whereas 42 were suitable for the qualitative analysis. They correspond to 39 human studies and 8 animal studies. Radiographic findings such as bone sclerosis, osteolytic areas, thickening of lamina dura, persisting alveolar socket, periapical radiolucency, thicker mandibular cortex, widening of the periodontal ligament space, periodontal bone loss, and enhancement of the mandibular canal were identified as early bone changes due to antiresorptive therapy. All those findings were also reported later in Stage 0 patients. The main limitations of these results are the lack of prospective data and comparisons groups; therefore, careful interpretation should be made. It is a fact that radiographic findings are present in antiresorptive-treated patients, but the precise timepoint of occurrence, their relation to the posology, and potential risk to develop MRONJ are not clear. The importance of a baseline radiographic diagnosis for antiresorptive-treated patients.

To assess the influence of sagittal and vertical skeletal patterns, and sex in the angulation and relationship between the maxillary central incisors and the nasopalatine canal.Cone-beam computed tomography (CBCT) exams of 172 patients were classified into sagittal (class I, II, and III) and vertical skeletal patterns (mesofacial, brachyfacial and dolichofacial). Hard palate-nasopalatine canal and hard palate-long axis of maxillary central incisors angles were measured in sagittal reconstructions. The obtained angles were used to calculate the angulation between maxillary central incisors and nasopalatine canal by triangle geometric relationship. Analysis of variance (multi-way ANOVA) and Tukey post-hoc test were used for data analysis (α=5%).Sagittal and vertical skeletal patterns, and sex presented a low influence on the angulation of maxillary central incisors and nasopalatine canal. Dolichofacial class II women showed the greatest hard palate-nasopalatine canal angulations (P0.05) and maxillary central incisors and nasopalatine canal angulations (P0.05). Mesofacial class II and III men presented the lowest maxillary central incisors and nasopalatine canal angulations (P0.05).The sagittal and vertical skeletal patterns, and sex have a low influence on the angulation of the maxillary central incisors and the nasopalatine canal.

to evaluate trueness and precision of digital casts from intraoral scanning (IOS) and cone beam computed tomography (CBCT); trueness and precision of 3D-printed casts using digital light processing (DLP) and fused deposition modeling (FDM); the influence of digitizing method in the 3D-printed casts and, to compare STL data after DICOM segmentation and conversion.a reference cast was digitized with IOS and CBCT, and 3D-printed using FDM and DLP. Linear measurements of occlusocervical (OC), interarch (IEA), and mesiodistal (MD) dimensions were taken on reference, digital and 3D-printed casts. Trueness was observed as the distortion, and precision was observed as the variation of measurements. One and Two-way ANOVA, Student t-test, and Chi-Square were applied to analyze data.distortion varied between digital casts for all dimensions; at OC, both showed expanded dimensions with IOS being significantly greater; in turn, CBCT digital casts showed higher distortion at IEA and MD. Dimensions of 3D-printed casts showed a predominance of shrinkage, DLP presented higher distortion compared to FDM for both digitizing methods. Digitizing methods influenced the 3D-printing of casts, especially for DLP. Regarding precision, no statistical difference was found. STL converted from DICOM showed statistical difference in IEA (p 0.001).digital casts showed distortion depending on the digitizing method. IOS was better in IEA and MD, and CBCT in OC dimensions. Overall, DLP casts presented higher distortion compared to FDM. The digitizing method influences trueness on 3D-printed casts. File conversion from DICOM to STL per se could change the dimension.This investigation showed that digital casts from IOS and CBCT as well 3D-printed casts from FDM and DLP can show different trueness. It is clinically relevant as clinicians have various workflows available in Digital Dentistry which involve these digitizing and manufacturing methods.

This study evaluated influence of addition of lead foil coupled to different types of digital receptors on image noise and on diagnosis of fractured instruments in teeth without and with root filling. Twenty-six molars had their root canals (n=67) cleaned and shaped and were divided into groups with and without root filling and with and without fractured instruments. Images were acquired in PSP and CMOS sensor, with and without addition of a lead foil coupled to the digital receptors. Diagnostic values were obtained by the area under the ROC curve, and one-way ANOVA compared the groups. Noise in images was measured by the standard deviation of mean gray values in images with and without the lead foil, and compared by two-way ANOVA (a=0.05). Addition of lead foil did not interfere in the diagnostic accuracy for both digital systems (p>0.05) and did not decrease noise in the images (p>0.05). However, independently of the addition of the lead foil, the phosphor-storage plate presented higher noise compared to the sensor (p ?0.05). Addition of lead foil to the digital receptor did not interfere in image noise and with the diagnostic accuracy for detecting fracured endodontic instrument in ex-vivo conditions.

Objectives: To evaluate the effect of cone-beam computed tomography (CBCT) tube current (mA) on the magnitude of artefacts at different distances from titanium or zirconia implants, with and without activation of a proprietary metal artefact reduction (MAR). Material and methods: Human mandibles were scanned on an OP300 Maxio CBCT unit (Instrumentarium, Tuusula, Finland) before and after the installation of dental implants, with four different tube currents (4 mA, 6.3 mA, 8 mA and 10 mA), with and without activation of proprietary MAR. The effect of mA on the standard deviation (SD) of gray values and contrast to noise ratio (CNR) were assessed in regions of interest located 1.5 cm, 2.5 cm, and 3.5 cm from implants. Results: In the presence of titanium implants, a significant decrease in SD was found by increasing tube current from 4 mA to 6.3 mA or 8 mA. For zirconia implants, 8 mA yielded better results for all distances. MAR improved CNR in the presence of zirconia implants at all distances, whereas no differences were observed with the use of MAR for titanium implants. Conclusion: Increased tube current can improve overall image quality in the presence of implants, at all the distances tested. When a zirconia implant is present, such increase in mA should be higher in comparison to that for examinations with titanium implants. Activation of OP300 Maxio proprietary MAR improved image quality only among examinations with zirconia implants. Clinical relevance: Artefact-generating implants are common in the field of view of CBCT examinations. Optimal exposure parameters, such as tube current, ensure high image quality with lowest possible radiation exposure.

To compare radiographic predictors of medication-related osteonecrosis of the jaw in dental extraction sites.Forty-one oncological patients undergoing intravenous or subcutaneous antiresorptive treatment, with a history of dental extraction visualised by panoramic imaging, were included in this retrospective study. Age-, sex- and extracted tooth-matched healthy patients who had previously undergone panoramic imaging were selected as controls (n = 57). A total of 288 extraction sites were independently evaluated by two oral and maxillofacial radiologists, who assessed eight distinct radiographic features. The radiographic features of extraction sites were noted to allow comparison between and within subjects regarding healing and osteonecrosis development. The association between radiographic findings, underlying dental disease and medication-related osteonecrosis of the jaw was also tested. The level of significance was set at 5%.Patients under antiresorptive treatment presented with widening of the periodontal ligament space, thickening of the lamina dura, sclerotic bone pattern, horizontal bone loss and periapical radiolucency with bone reaction (P ≤ 0.05). Development of medication-related osteonecrosis of the jaw was associated with altered bone pattern, angular bone loss, furcation involvement and unsatisfactory endodontic treatment (P ≤ 0.05). An association between medication-related osteonecrosis of the jaw and previous dental disease was also found, particularly for periapical lesions and endodontic-periodontal disease (P ≤ 0.05).Radiographic predictors of further development of medication-related osteonecrosis of the jaw in extraction sites include heterogeneous bone pattern, angular bone loss and furcation involvement. Extraction sites with underlying bony changes related to endodontic and endodontic-periodontal disease are more prone to development of medication-related osteonecrosis of the jaw.

Objective: To create and validate two new Image Receptor-Holding devices (IRHDs) to reduce proximal surfaces overlapping in bitewing radiography in comparison to a regular well-established and commercially available IRHD. Methods: Two IRHDs for bitewing radiographs with a wedge on the biting surface were designed and 3D-printed. These IRHDs, with a large wedge (Pr-Lw) and small wedge (Pr-Sw), were compared with a regular commercially available IRHD (Rinn XCP®) during image acquisition of bitewing radiographs of four posterior regions (one upper region and three lower regions) in two dry skulls and mandibles. A total of 156 interproximal regions on bitewing radiographs were radiographed by 13 oral radiology graduate students and independently assessed by two oral radiologists (10 years of experience). IRHDs were compared by Cochran’s Q test regarding the number of overlapped proximal surfaces in the acquired radiographs with a significance level of 5%. Results: The observers were in perfect agreement (κ = 1.0) to classify the proximal surfaces overlapping. The Pr-Lw IRHD presented the lowest number of surfaces overlapping (19.2%) followed by the Pr-Sw IRHD (48.1%) and the XCP®IRHD with the highest surfaces overlapping (71.2%). The Pr-Lw IRHD surfaces overlapping was statistically different from the XCP IRHD (p < 0.001), and the Pr-Sw IRHD (p = 0.014). Conclusions: The Pr-Lw IRHD demonstrated the most efficient performance in overlapping surfaces reduction, compared with the Pr-Sw IRHD and the XCP® IRHD in adult dry skulls and mandibles.

Objectives:To evaluate the influence of artefacts in cone beam CT (CBCT) images of filled root canals in isthmus-containing molars.Methods:10 teeth presenting canals with an isthmus were instrumented and filled with a thermoplasticised obturation technique. The teeth were scanned using a micro-CT device and two CBCT devices: 3D Accuitomo 170 (ACC) and NewTom VGi evo (NT), with different acquisition protocols: larger and smaller voxel size. Three examiners assessed the CBCT images for: (1) detection of filling voids; (2) assessment of under- or overestimation of the filling material and (3) resemblance of CBCT images to the reference standard. Analyses of Task 1 yielded accuracy, sensitivity and specificity for detection of filling voids. For tasks 2 and 3, statistical analysis was performed using Wilcoxon test. The level of significance was set at p < .05.Results:For Task 1, ACC showed higher sensitivity, whereas NT presented higher specificity. No significant difference was found between the protocols in ACC, however, for NT, differences between protocols were significant for all diagnostic values. In Task 2, visualisation of the filling was overestimated for NT, while for ACC, underestimation was observed. For Task 3, images with smaller voxel size were more similar to the reference image, for both CBCT devices.Conclusions:Different artefacts compromise the detection of filling voids on CBCT images of canals in mandibular molars with isthmus. ACC and NT present rather similar diagnostic accuracy, even though artefact expression remains device-specific.

Introduction This study aimed to evaluate the association between the root canal configuration (RCC), endodontic technical errors, and periapical hypodensities in molars using cone-beam computed tomographic (CBCT) images. Methods Two hundred twenty-one roots were assessed from 79 patients referred for CBCT examination because of symptomatology in endodontically treated molars. Two oral and maxillofacial radiologists classified the RCC of each dental root according to Vertucci's classification. Root canals were assessed for the presence of technical errors from endodontic treatment. The presence of periapical hypodensity for each dental root and the coronal sealing condition of each tooth were also recorded. Results For RCC type I, the most prevalent technical error was underfilling (17.4%). For types II and VIII, nonfilled canals were found in 54% and 100% of the cases, respectively. For type III, underfilling, nonhomogeneous filling, and nonfilled canals were equally frequent (33.3%), whereas for type IV underfilling and nonfilled canals had a prevalence of 42%. In general, there was a higher prevalence of apical hypodensities among roots with technical errors. It was also observed for type I, with an increase from 41.2%–62.9% when a technical error was present. Conclusions The prevalence of different endodontic technical errors varies depending on the RCC for molars. A greater complexity of RCC is related to a higher occurrence of errors and a higher prevalence of periapical hypodensity.

The purpose of this preliminary study was to evaluate the diagnostic accuracy of the combined use of 2 cone-beam computed tomographic (CBCT) volumes obtained with the tooth of interest positioned at different orientations in the detection of vertical root fracture (VRF).Thirty single-rooted teeth were divided into 2 main groups (n = 15): control and with VRF. The teeth were individually placed in a phantom composed of a human skull and mandible, which was CBCT scanned in 2 spatial orientations: conventional (with the Frankfurt plane parallel to the floor) and angled acquisition (tilted 90° backward). Also, each tooth was scanned with gutta-percha, a metal post, and without any intracanal material. Three oral radiologists individually evaluated the images resulting from conventional acquisition and verified the presence or absence of VRF (conventional CBCT assessment), setting a score on a 5-point scale. Subsequently, the observers evaluated both images resulting from the conventional and angled acquisitions (combined CBCT assessment). The diagnostic values of the conventional and combined assessments were compared using 2-way analysis of variance with the post hoc Tukey test. The significance level was set at 5% (α = 0.05).The combined CBCT assessment showed higher accuracy and sensitivity in the VRF diagnosis of teeth filled with gutta-percha (P.05). In teeth with a metal post, all diagnostic values were higher in the combined CBCT assessment (P.05).This preliminary study suggests that the CBCT-based diagnosis of VRF in teeth with intracanal material was improved when the assessment combines images obtained at 2 orientations.

All ROIs located at different distances from the implant showed higher SD on scans acquired with 450 basis images (p0.05), regardless of MAR condition.A higher number of basis images reduces the magnitude of artifacts but does not influence the image quality in bone cortical plates. MAR improves the image in the areas most affected by artifacts.The number of basis images is known as a factor capable of influencing the image quality and radiation dose for the patient. Therefore, it is important to investigate its effect on the expression of artifacts in the CBCT images.

Aim To assess the influence of artefacts generated by metal posts on the detection of simulated internal root resorption (IRR) in adjacent teeth using cone-beam computed tomography (CBCT) and to verify the impact of metal artefact reduction (MAR) on these cases. Methodology CBCT images of 14 premolar teeth were acquired before and after IRR simulation using chemical and mechanical procedures, in an OP300 Maxio unit, with and without MAR. Each tooth was placed in the socket of a human mandible and scanned under three different conditions: (i) without adjacent teeth - control group; (ii) distal adjacent tooth restored with metal post; and (iii) with both adjacent teeth restored with metal post. Five oral radiologists scored the IRR detection using a 5-point scale. Diagnostic values were obtained for the tested groups and compared using two-way analysis of variance (α = 0.05). Results The presence of a single adjacent tooth restored with metal post did not significantly influence the diagnostic values for IRR detection (P > 0.05). The presence of both adjacent teeth with metal posts, without MAR application, was associated with a significantly lower area under the ROC curve (Az) compared to the control (P = 0.0182). In this case, the application of MAR increased Az, leading to nonsignificant differences from the control group and the group with one adjacent restored tooth (P > 0.05). Sensitivity decreased significantly when two adjacent restored teeth were present, regardless of MAR application (P = 0.0379). Specificity was not affected by the conditions tested (P > 0.05). Conclusion CBCT detection of IRR was impaired by artefacts only when both adjacent teeth restored with metal posts were present. In such cases, activation of MAR improved the performance on this diagnostic task.

To evaluate the performance of periapical radiography assessed under different radiographic brightness and contrast variations in the detection of simulated internal (IRR) and external (ERR) root resorption lesions. Additionally, observers’ preferences related to image quality for these diagnostic tasks were evaluated. Thirty single-root teeth were divided into two groups (n = 15): IRR, in which lesions were simulated using mechanical and biochemical processes; and ERR, in which cavities standardized with drills of different sizes were performed on the root surfaces. Digital radiographs were obtained and subsequently adjusted in 4 additional combinations, resulting in 5 brightness/contrast variations (V1–V5). Five radiologists evaluated the radiographs. The observers’ preference on the image quality was also recorded. Objective The aim of this study was to calculate the sensitivity, specificity, and positive and negative predictive values (PPV and NPV) for the detection of the presence and continuity of peripheral cortication (PC) around benign intraosseous lesions on panoramic radiography (PR), with cone beam computed tomography (CBCT) as the reference standard. Study Design The sample consisted of the mesial, distal, superior, and inferior borders of 40 bony lesions (n = 160) depicted on both PR and CBCT. Ten observers assessed the PRs. CBCT images were assessed by 2 radiologists. For each lesion, observers classified the 4 borders with regard to the presence of PC, and if present, its continuity. Sensitivity, specificity, PPV, and NPV were calculated for the presence and continuity of PC. Intra- and interobserver agreements were calculated. Results PR had sensitivity, specificity, PPV, and NPV for PC detection of 0.844, 0.435, 0.435, and 0.844, respectively. With regard to PC continuity, those values were 0.750, 0.810, 0.577, and 0.904. Observer agreement was slight to fair. Conclusions The high sensitivity and low specificity for the presence of PC indicate a large false-positive rate. The high NPVs suggest greater validity for negative decisions. The findings corroborate the influence of the eggshell effect. Observer agreement rates indicate that interpretation of PC is substantially subjective.

Objectives The aim of this study was to assess the influence of different viewing conditions (combinations of computer display, horizontal viewing angle, and ambient lighting) on the detection of approximal caries lesions on digital radiographic images. Study Design Forty posterior teeth were mounted in a plaster block and radiographed by using a CMOS sensor-based system. Three oral radiologists assessed the radiographic images under different conditions of computer display (iMac 5K 27”, Barco MDRC-2124, and Dell P2314H), horizontal viewing angles (90 degrees, 67.5 degrees, and 45 degrees), and ambient lighting (low, medium, and high) and were asked to detect the presence of approximal caries lesions, which were confirmed by means of micro–computed tomography (micro-CT), using a 5-point scale. The area under the receiver operating characteristic curve, sensitivity, and specificity were calculated and compared by analysis of variance testing (α = 0.05). Intra- and interobserver agreements were assessed with the Kappa test. Results Computer display, horizontal viewing angle, and ambient lighting did not significantly influence the diagnosis of approximal caries lesions. In general, moderate values of accuracy, low sensitivity, and high specificity were found for all viewing conditions. Conclusions The viewing conditions assessed in this study did not influence the detection of approximal caries lesions on digital radiographic images. Dental practitioners are encouraged to find their most comfortable viewing condition for this diagnostic task.

Introduction The aim of the present study was to evaluate the detection of vertical root fracture (VRF) in the presence of adjacent teeth restored with a metal post and the influence of acquisition parameters (tube current and metal artifact reduction [MAR] algorithm) on this diagnostic task. Methods Cone-beam computed tomographic images of 10 single-rooted teeth were acquired before and after the simulation of VRF. The acquisitions were set up to simulate different conditions regarding the presence of adjacent teeth restored with a metal post (control, 1 adjacent tooth, and both adjacent teeth), different tube currents (4, 8, and 10 mA), and the use of MAR (without MAR and with MAR). Images were assessed by 5 oral and maxillofacial radiologists using a 5-point scale for the presence of VRF. Diagnostic values were calculated and compared by 2-way analysis of variance (significance level of 5%). Results The area under the receiver operating characteristic curve (Az) values for VRF detection were affected by the presence of adjacent teeth and the variation of milliamperes. For 4 mA, when both restored teeth were present, Az values were significantly lower than the control group (P ≤ .05). In the presence of both restored teeth, 8 mA presented significantly higher Az values compared with 4 mA (P ≤ .05). Conclusions The presence of both adjacent teeth restored with a metal post impairs VRF detection; however, an increase in tube current up to 8 mA may aid in this diagnostic task. Moreover, the MAR tool does not seem to be efficient in those cases.

Purpose This study was performed to evaluate the magnitude of artifacts produced by gutta-percha and metal posts on cone-beam computed tomography (CBCT) scans obtained with different tube currents and with or without metal artifact reduction (MAR). Materials and Methods A tooth was inserted in a dry human mandible socket, and CBCT scans were acquired after root canal instrumentation, root canal filling, and metal post placement with various tube currents with and without MAR activation. The artifact magnitude was assessed by the standard deviation (SD) of gray values and the contrast-to-noise ratio (CNR) at the various distances from the tooth. Data were compared using multi-way analysis of variance. Results At all distances, a current of 4 mA was associated with a higher SD and a lower CNR than 8 mA or 10 mA (P0.05). Conclusion Increasing the tube current from 4 mA to 8 mA may reduce the magnitude of artifacts from metal posts. The magnitude of artifacts arising from metal posts was significantly higher at distances of 1.5 cm or less than at greater distances. MAR usage improved image quality near the metal post, but had no significant influence farther than 1.5 cm from the tooth.

To evaluate the effect of enhancement tools of intraoral digital radiographs on the assessment of vertical root fracture (VRF) and to quantify the resultant image noise.Thirty single-rooted human teeth (15 control and 15 fractured) were each radiographed in four intracanal conditions: no filling, gutta-percha, metal post, and fiberglass post, totaling 120 original images. Two filters were applied to the original images-Sharpen filter (SF) and Edge Enhancement filter (EE), and brightness and contrast were adjusted in four combinations (BC1 to 4), resulting in 840 images. Five oral radiologists analyzed the images for VRF detection. Pixel intensity was obtained in two regions from the radiographs. Diagnostic values were calculated and compared by two-way ANOVA, and the SD values of pixel intensity values were compared by one-way ANOVA (α = 0.05).There were no significant differences in accuracy for VRF detection between the experimental groups (p 0.05). Teeth with metal post presented the lowest sensitivity (p 0.05) for all experimental conditions, except for SF and EE (p 0.05). BC2, BC3, and BC4 had higher specificity than SF (p ≤ 0.05) for all intracanal conditions. Analysis of pixel intensity showed that all enhanced images presented statistically significant higher noise compared to those of the original images (p ≤ 0.05).Digital enhancement tools in digital radiography increase image noise; however, they can be used without compromising VRF detection.The use of digital enhancement does not impair the detection of VRF and, therefore, can be applied for this purpose according to the observer preference.

Age estimation methods take into account the organism variation during its development or involution. Dental methods can often be considered, because the dental development presents a series of recognizable events with small variation. Each method presents a different methodological proposal, so it is important to consider the type of dentition, the presence or absence of teeth, available information and the method’s validation, as dental development may differ among different populations. The present study aimed to assess the agreement between the ages estimated by the methods of Demirjian and Willems and the real age of Brazilian individual. To compose the sample and apply the methods, 220 panoramic radiographs of individuals from 6 to 16 years were selected, being 110 females and 110 males. After applying the methods by three examiners, it was possible to observe that both methods showed high correlation values ​​among males. However, the results for female brought lower values. In addition, the Willems’ method was more suitable to the population considered when compared to the Demirjian’s method, despite the overestimation of real ages by both methods.

Objectives: This study observed whether changes in diagnosis caused by analysis of three-dimensional images can lead to alterations in the treatment plans of impacted lower third molars (ILTMs). Methods: Sets of panoramic (PAN) – cone beam computed tomography (CBCT) of 218 patients were assessed for ILTM classification, contact with mandibular canal, contact and resorption of the lower second molar (LSM), intraoperative planning and post-operative expectations. Results: Percentage agreement and McNemar test compared PAN vs CBCT assessments. Logistic regression analyzed the dependency of change in surgical planning considering the changes in diagnostic features; descriptive statistics was used to observe the expectation of post-operative complications and paresthesia. Differences were found between PAN vs CBCT for classification of impaction and positioning, LSM relationship, choice for crown and root sectioning and expectation of post-operative complications (all with p < 0.001). Logistic regression indicated that the change in diagnosis caused by CBCT examination did not change the clinical decision to extract ILTM but altered the planning of intraoperative steps such as osteotomy, crown sectioning and relaxing incision. The expectation of post-operative complications decreased when professionals planned the ILTM removal using tri-dimensional images. Conclusions: We concluded that changes in the diagnosis after CBCT examination can lead to alterations in the treatment plan of impacted lower third molar.

Objectives: The objective of the study is to incorporate the nanostructured silver vanadate (AgVO3) decorated with silver nanoparticles at endodontic sealers AH Plus, Sealapex, Sealer 26, and Endofill, at concentrations of 2.5%, 5%, and 10%, and to evaluate physical-chemical properties. Materials and Methods: The study was to evaluate the radiopacity using digital radiography (n = 5), the tooth color change in 48 incisors (n = 3) for 7, 30, 90, and 180 days in a spectrophotometer, and the topographic distribution in a confocal laser (n = 5). The radiopacity was analyzed by Kruskal–Wallis test and the permutation (α = 0.05) and the color and topographical distribution by descriptive statistical analysis. Results: The AgVO3 had no effect on the radiopacity of Endofill and Sealapex (P > 0.05) and at 2.5% concentrations increased the radiopacity of Sealer 26 and AH Plus (P < 0.05). The Endofill 10% showed less color change, and the major changes were the modified groups in 180 days. The AgVO3 showed a circular topographic distribution in areas of the sealers. Conclusion: It was found that the addition of AgVO3 did not affect the radiopacity of Endofill and Sealapex however, increased the radipacity of Sealer 26 and AH Plus. For modified groups, the greatest color change was promoted after 180 days, except for Endofill with 10%. Topographic distribution of nanomaterial affected the color change of theevaluated sealers.

Objective The aim of this study was to assess the influence of reconstruction protocols of micro-computed tomography (micro-CT) images on analyses of bone microarchitecture. Study Design Micro-CT images of the maxillae of 5 Wistar rats were reconstructed with different protocols by varying the levels of the following tools: smoothing filter (SF; 2–6); ring artifact correction (RAC; 5–15); and beam hardening correction (BHC; 15%–60%). A control protocol (P0; without any correction tool) and a standard protocol (SP; according to the manufacturer's recommendation: SF = 2; RAC = 5; BHC = 45%) were also obtained. For each protocol, 8 bone microarchitecture parameters were calculated (bone volume/total volume [BV/TV], bone surface/total volume [BS/TV], trabecular number [Tb.N], trabecular thickness [Tb.Th], trabecular separation [Tb.Sp], degree of anisotropy [DA], connectivity density [Conn.D], and total porosity [Po-tot]) by using CTAn software. Test protocols were compared with the SP by using 1-way analysis of variance and the Dunnet post hoc test (α = 0.05). Results An inverse relationship was observed between BHC tool levels and all microarchitecture parameters except BV/TV and Tb.Th. The combination of BHC and SF significantly influenced all microarchitecture parameters except for DA, for which all protocols were similar to the SP (P > .05). Conclusions Calculation of bone microarchitecture parameters is influenced by the applied levels of artifact correction tools, mainly BHC and SF. It is necessary to standardize such tool levels for correct data interpretation.

Purpose Preoperative recognition of the relative position of the mandibular third molars may contribute to more reliable treatment planning, avoiding injuries and decreasing surgical and recovery times. The research questions for this study were as follows: 1) Is panoramic radiography (PR) equivalent to cone-beam computed tomography (CBCT) in determining the degree of mandibular third molar impaction? 2) For PR, is the external oblique ridge a more reliable reference to determine the degree of mandibular third molar impaction? Materials and Methods This retrospective case-series study assessed whether the imaging modalities (primary predictor variable) are equivalent in determining the degree of impaction of third molars (primary outcome variable) according to the parameters of the Pell and Gregory classification. Two oral and maxillofacial radiologists evaluated PR and CBCT images presenting at least 1 mandibular third molar. The degrees of impaction related to the ascending ramus and related to the external oblique ridge (secondary outcome variables) also were assessed. The agreement rates between PR and CBCT were presented, and the imaging modalities were compared by the McNemar-Bowker test (α = .05). Results A total of 173 patients were included (66 male and 107 female patients). Among these patients, 313 mandibular third molars were assessed. In relation to the occlusal plane, the agreement rate between PR and CBCT was 82.1% for Pell and Gregory class A, 90.5% for class B, and 65.6% for class C (P = .116). The overall agreement rate between the classifications in relation to the mandibular ramus on PR and CBCT ranged from 66.8% (considering the ascending ramus, P Conclusions PR performs similarly to CBCT in the classification of impaction in relation to the occlusal plane. However, PR shows a tendency to underestimate the space for accommodation of the third molar compared with CBCT. Furthermore, the external oblique ridge is not a reliable alternative landmark on PR.

Objectives: To evaluate the influence of the level of three micro-CT reconstruction tools: beam-hardening correction (BHC), smoothing filter (SF), and ring artefact correction (RAC) on the fractal dimension (FD) analysis of trabecular bone. Methods: Five Wistar rats’ maxillae were individually scanned in a SkyScan 1174 micro-CT device, under the following settings: 50 kV, 800 µA, 10.2 µm voxel size, 0.5 mm Al filter, rotation step 0.5°, two frames average, 180° rotation and scan time of 35 min. The raw images were reconstructed under the standard protocol (SP) recommended by the manufacturer, a protocol without any artefact correction tools (P0) and 35 additional protocols with different combinations of SF, RAC and BHC levels. The same volume of interest was established in all reconstructions for each maxilla and the FD was calculated using the Kolmogorov (box counting) method. One-way ANOVA with Dunnet’s post-hoc test was used to compare the FD of each reconstruction protocol (P0–P35) with the SP (α = 5%). Multiple linear regression verified the dependency of reconstruction tools in FD. Results: Overall, FD values are not dependent on RAC (p = 0.965), but increased significantly when the level of BHC and SF increased (p < 0.001). FD values from protocols with BHC at 45% combined with SF of 2, and BHC at 30% combined with SF of 4 or 6 had no statistical difference compared to SP. Conclusions: BHC and SF tools affect the FD values of micro-CT images of the trabecular bone. Therefore, these reconstruction parameters should be standardized when the FD is analyzed.

OBJECTIVE The use of electronic cigarettes (ECIGs) has recently increased as an alternative to conventional smoking tobacco products. The literature is scarce on the effects of ECIGs on dental and oral structures. Therefore, the objective of this study was to observe whether ECIG aerosol could alter the color of dental enamel. MATERIALS AND METHODS Sixty-three bovine enamel specimens were randomly separated into groups (n = 7) and treated with aerosols with different e-liquid flavors (neutral, menthol, and tobacco) and nicotine content (0, 12, and 18 mg). The initial color assessment was performed using a spectrophotometer (Easy Shade-Vita). Dental enamel was exposed to 20 cycles of ECIG aerosol in a smoking machine, and the final color was measured. The color change was evaluated using ΔEab, ΔE00, and Whiteness Index (WID ) formulae. Differences in L* a* b* coordinates were calculated, and data were analyzed (two-way ANOVA, uncorrected Fisher's LSD post hoc test, α = 0.05). Objective: This study aimed to assess if additional vertical bitewing (VBW) and/or occlusal (OC) radiographs may change initial judgment based only on periapical radiograph (PAR) about the final position of orthodontic mini-implants (OMI). Methods: Subjective and objective analyses were performed. Radiographic images of 26 OMI were divided into four groups: PAR, PAR+VBW, PAR+OC and ALL (PAR+VBW+OC). For subjective analysis, five observers were asked to assess if the position of OMI was favorable to its success, using questionnaires with a four-point scale for responses: 1= definitely not favorable, 2= probably not favorable, 3= probably favorable, or 4= definitely favorable. Each group containing sets of images was presented to them in four different viewing sessions. Objective evaluation compared horizontal distances between OMI tip and the root nearest to the device in PAR and VBW. Results: Most of observers (3 out of 5) changed their initial judgment based on PAR about OMI position when additional radiographs were analyzed. Differences between groups (i.e. PAR vs. PAR+VBW; PAR vs. PAR+OC; and, PARvs.ALL) were statistically significant for these observers. For those that changed their judgment about OMI position, confidence level could significantly increase, decrease or even be maintained, not indicating a pattern. There was no agreement for distances between OMI tip and the root nearest to the device in PAR and VBW. Conclusion: Considering the limitations of the study, it is concluded that additional radiographic images may change the judgement about OMI final position without necessarily increasing the degree of certainty of such judgment. RESUMO Objetivo: avaliar se a adição de radiografias interproximais verticais (IV) e/ou oclusais (OC) pode alterar o julgamento inicial sobre a posição final dos mini-implantes ortodônticos (MI) baseado somente na radiografia periapical (PA). Métodos: foram realizadas análises subjetivas e objetivas. Imagens radiográficas de 26 regiões contendo MI foram divididas em quatro grupos: PA, PA+IV, PA+OC e TODAS (PA+IV+OC). Na análise subjetiva, cinco observadores foram convidados a avaliar se a posição do MI era favorável para o seu sucesso, utilizando questionários com uma escala de quatro pontos para respostas: 1 = definitivamente não favorável; 2 = provavelmente não favorável; 3 = provavelmente favorável; ou 4 = definitivamente favorável. Cada grupo contendo conjuntos de imagens foi apresentado aos observadores em quatro sessões diferentes. Adicionalmente, uma avaliação objetiva comparou as distâncias horizontais entre a ponta do MI e a raiz dentária mais próxima ao dispositivo na PA e IV. Resultados: a maioria dos observadores (3 de 5) mudou seu julgamento inicial sobre a posição do MI baseado na PA quando radiografias adicionais foram analisadas. Diferenças entre os grupos (ou seja, PA vs PA+IV; PA vs PA+OC; e PAvsTODAS) foram estatisticamente significativas para esses observadores. Para aqueles que mudaram seu julgamento sobre a posição do MI, o nível de confiança das respostas aumentou, diminuiu ou foi mantido, não indicando um padrão. Houve diferença estatisticamente significante entre as distâncias da ponta do MI para a raiz mais próxima ao dispositivo na PA e IV. Conclusão: considerando-se as limitações desse estudo, concluiu-se que imagens radiográficas adicionais podem alterar o julgamento sobre a posição final de MI sem, necessariamente, aumentar o grau de certeza de tal julgamento.

The knowledge of the internal anatomy of three-rooted mandibular molars may help clinicians to diagnose and plan the root canal treatment in order to provide adequate therapy when this variation is present. Objectives: To determine the prevalence of three-rooted mandibular molars in a Brazilian population using cone beam computed tomography (CBCT) and to analyze the anatomy of mandibular first molars with three roots through micro-CT. Material and Methods: CBCT images of 116 patients were reviewed to determine the prevalence of three-rooted first mandibular molars in a Brazilian subpopulation. Furthermore, with the use of micro-CT, 55 extracted three-rooted mandibular first molars were scanned and reconstructed to assess root length, distance between canal orifices, apical diameter, Vertucci's classification, presence of apical delta, number of foramina and furcations, lateral and accessory canals. The distance between the orifice on the pulp chamber floor and the beginning of the curvature and the angle of canal curvature were analyzed in the distolingual root. Data were compared using the Kruskal-Wallis test (α=0.05). Results: The prevalence of three-rooted mandibular first molars was of 2.58%. Mesial roots showed complex distribution of the root canal system in comparison to the distal roots. The median of major diameters of mesiobuccal, mesiolingual and single mesial canals were: 0.34, 0.41 and 0.60 mm, respectively. The higher values of major diameters were found in the distobuccal canals (0.56 mm) and the lower diameters in the distolingual canals (0.29 mm). The lowest orifice distance was found between the mesial canals (MB-ML) and the highest distance between the distal root canals (DB-DL). Almost all distal roots had one root canal and one apical foramen with few accessory canals. Conclusions: Distolingual root generally has short length, severe curvature and a single root canal with low apical diameter.

Svrha ovog istraživanja bila je procijeniti dimenzije maksilarnih sinusa odraslih ljudi na ortopantomogramu i mogućnost primjene tog pokazatelja u određivanju spola u forenzične svrhe. Materijal i postupci: Uzorak su činila 64 ortopantomograma pojedinaca starijih od 20 godina (32 muškarca i 32 žene) iz baze podataka s kompletnom trajnom denticijom (ili bez umnjaka). Jedan ispitivač mjerio je širinu i visinu desnih i lijevih gornjih sinusa softverom Image J 1.47v (National Institutes of Health, Bethesda, SAD). Mjerenja su ponovljena kako bi se izračunala ponovljivost ispitivača. U analizi rezultata korišteni su hi-kvadrat test, kappa, ANOVA i Studentov t-test, uz razinu značajnosti postavljenu na p ≤ 0,05. Vrijednost kappa kretala se u rasponu od 0,38 do 0,96. Rezultati: Ženama je prosječna visina i širina lijevoga maksilarnog sinusa iznosila 28,7856 mm, odnosno 44,6178 mm. Za desni maksilarni sinus visina je bila 27,7163 milimetra, a širina 45,1850 milimetara. Muškarcima je bila izmjerena prosječna visina i širina lijevoga maksilarnog sinusa od 30,9981 milimetar, odnosno 48,7753 milimetra. Za desni maksilarni sinus visina je bila 30,7403 milimetra, a širina 48,5753 milimetra. Utvrđena je statistički značajna razlika u visini i širini maksilarnih sinusa između muškaraca i žena. Zaključak: Može se zaključiti da visina i širina maksilarnih sinusa na ortopantomogramu može poslužiti u određivanju spola odraslih osoba., The purpose of this study was to evaluate dimensions of adult human maxillary sinuses on panoramic radiographs and their possible application on the sex determination for forensic purposes. The sample comprised 64 database panoramic radiographs from individuals aged 20 years or older (32 male and 32 female subjects), with complete permanent dentition (or absence of third molars). One examiner measured the width and height of the right and left maxillary sinuses using the software Image J 1.47v (National Institutes of Health, Bethesda, USA). Measurements were repeated to calculate intra-observer agreement. Chi-Square test, Kappa, ANOVA and T-Student were used for results analysis for p≤ 0.05. Intra-observer agreement with correlation Kappa ranged between 0.38 and 0.96. For female subjects, the mean height and width of the left maxillary sinus were 28.7856mm and 44.6178mm, respectively. And right maxillary sinus was 27.7163mm for height and 45.1850mm for width. Male subjects were found to have the mean height and width of the left maxillary sinus 30.9981mm and 48.7753mm, respectively. And right maxillary sinus was 30.7403mm for height and 48.5753mm for width. There was a statistically significant difference in the height and width of maxillary sinuses between males and females. It can be concluded that maxillary sinuses height and width on panoramic radiographs can be used to determine the gender of adult human subjects.

OBJECTIVES: To assess the influence of brightness and contrast adjustments of digital radiographs on the diagnosis of proximal caries lesions, and to compare with observers' preferences for subjective image quality. METHODS: 80 proximal surfaces of posterior teeth were radiographed using an intraoral digital system (Digora Toto, Soredex, Finland). Initial images and four different combinations of brightness and contrast for each radiography were analysed. Five observers scored the images for the presence and extension of caries lesions. Micro–CT images were used as gold standard. In a second stage, the observers were asked which of the radiographs they preferred for the assessment of caries lesions. RESULTS: No differences were found between the original and adjusted radiographic images regarding the area under the receiver operating characteristic curve, sensitivity, and specificity (p > 0.05). There was a significant difference between the micro-CT and the intraoral radiographs (p < 0.0001). Images with high brightness and low contrast presented higher number of true negative cases, but also a decrease in caries detection. On the other hand, there were more cases of overestimation of the presence and extension of caries lesions in images with low brightness and high contrast. The subjective evaluation of image quality showed that radiographs with lower brightness and higher contrast tended to be preferred by observers. CONCLUSIONS: Brightness and contrast adjustments in digital intraoral radiographs within the range tested in this study do not significantly influence the diagnosis of proximal caries lesions, although observers tend to prefer lower brightness and higher contrast images.

This study investigated whether professional experience and type of image examination (panoramic radiography (PAN) or cone beam computed tomography (CBCT)) could influence the pre-surgical planning for dental implant treatment. Six dentists, from two different levels of experience (senior (Sr) and junior (Jr)) performed simulated pre-surgical planning by using PANs and after 1 month, by using CBCTs, considering implant length (IL), implant width (IW), need for bone graft (BG), and other surgical procedures (OP). Bland-Altman test and Kappa coefficient were used to identify agreements. Bland-Altman test showed good agreement in the plans for inter-professional (i.e., Sr vs Jr) comparisons. Bland-Altman plots displayed intra-observer agreement (i.e., differences between PAN and CBCT), showing discrepancy between imaging modalities for IL and a tendency towards selecting larger dental implant options when using PAN and smaller options with CBCT. Kappa showed almost perfect (0.81–1.0) agreement between Jr and Sr for OP (PAN and CBCT). For BG, agreement was substantial (0.61–0.80) when planning was done with PAN and CBCT. Descriptive statistics showed evidence that when Jrs used CBCT, they no longer indicated the BG they had planned when they used PAN. There were differences in the pre-surgical planning for treatment with dental implants depending on the professional experience and the type of imaging examination used. Variation in dental implant planning can affect treatment time, cost, and morbidity in patients.

Purpose This study was conducted evaluate the influence of reconstruction parameters of micro-computed tomography (micro-CT) images on bone mineral density (BMD) analyses. Materials and Methods The sample consisted of micro-CT images of the maxillae of 5 Wistar rats, acquired using a SkyScan 1174 unit (Bruker, Kontich, Belgium). Each acquisition was reconstructed following the manufacturer's recommendations (standard protocol; SP) for the application of artifact correction tools (beam hardening correction [BHC], 45%; smoothing filter, degree 2; and ring artifact correction [RAC], level 5). Additionally, images were reconstructed with 36 protocols combining different settings of artifact correction tools (P0 to P35). BMD analysis was performed for each reconstructed image. The BMD values obtained for each protocol were compared to those obtained using the SP through repeated-measures analysis of variance with the Dunnett post hoc test (α=0.05). Results The BMD values obtained from all protocols that used a BHC of 45% did not significantly differ from those obtained using the SP (P>0.05). Introduction There is no consensus about the accuracy of cone-beam computed tomography (CBCT) for detecting vertical root fractures (VRFs), nor is there certainty about the isolated effect of different tube current parameters on the diagnosis of VRF through CBCT scans. This study aimed to evaluate how tube current affects the detection of VRF on CBCT examinations in the absence of intracanal materials and in the presence of gutta-percha (GP) and metal (MP) or fiberglass (FP) intracanal posts. Methods The sample consisted of 320 CBCT scans of tooth roots with and without VRF divided into 8 groups: no fracture/no intracanal material; no fracture + GP; no fracture + MP; no fracture + FP; fracture/no intracanal material; fracture + GP; fracture + MP; fracture + FP. The scans were acquired with an OP300 unit using 4 different milliamperes (4 mA, 8 mA, 10 mA, 13 mA). Five oral radiologists analyzed the images. The area under the receiver operating characteristic curve (Az), sensitivity, specificity, positive and negative predictive values, and interobserver agreement were calculated. Results Diagnostic performance for the different milliamperes tested was similar for teeth without root filling materials or with FP. Teeth with GP and MP showed the highest Az values for 8 mA and 10 mA, respectively. For teeth with MP, specificity was significantly higher when 10 mA was used. Conclusions For teeth without root filling materials or with FP, the use of a reduced milliampere does not seem to influence the detection of VRF in a significant manner. For teeth with GP and MP, an increased milliampere may lead to increased diagnostic performance.
