To investigate the efficacy and survival benefit of neoadjuvant chemotherapy (NACT) followed by type C radical surgery in patients with locally advanced cervical cancer (LACC) and moderate risk factors for recurrence. The clinical and pathological data of 338 cases with stage IB3 and IIA2 cervical carcinoma who received NACT followed by radical surgery from January 2008 to December 2018 were collected. In line with the clinical response, the patients were divided into a chemotherapy-effective group (n = 309) and a chemotherapy-ineffective group (n = 29), and the clinical and pathological characteristics of the two groups were compared. Survival was analyzed using the Kaplan-eier method. After NACT and surgical resection, 309 (91.4%, 309/338) patients experienced a clinical response. Fewer patients had a maximum tumor diameter ≥5 cm in the chemotherapy-effective group compared with the chemotherapy-ineffective (p = 0.021). The 5-year progression-free survival (PFS) and overall survival (OS) rates were higher in the chemotherapy-effective group (86.29% and 85.75%, respectively) than in the chemotherapy-ineffective group (77.64% and 75.56%, respectively; p < 0.001). Among patients with high-risk and moderate-risk factors, the 5-year PFS and OS rates were higher in the patients who received chemotherapy (95.90% and 94.81%) than in those who received CCRT (78.06% and 77.25%, respectively; p < 0.001). NACT followed by surgical resection may be an appropriate treatment strategy for young patients with LACC, allowing them to avoid radiochemotherapy and the resultant damage to ovarian and vaginal function. Background: To estimate the short term (5 years) and long term (30 years) economic burden of stroke among younger adults (18-64 years), and to calculate the loss of health-related quality of life in these individuals, in Australia.Methods: A Markov microsimulation model was built to simulate incidence of stroke among younger adults in Australia. Younger adults with stroke commenced in the model via health states defined by the modified Rankin Scale at 12 months from the AVERT study (A Very Early Rehabilitation Trial), and transitioned through these health states. Costs in Australian dollars (AUD) were measured from a societal perspective for a 2018 reference year and categorised into medical, non-medical and indirect costs. Probabilistic sensitivity analyses were performed to test the robustness around the cost of illness estimates. The loss of health-related quality of life due to stroke among younger adults was calculated by determining the difference in estimated quality-adjusted life years (QALYs) between the stroke population and the general population. This was determined by multiplying the predicted remaining life years for the modelled stroke cohort and the age-matched general population, by their corresponding age-dependent utilities.Results: The economic burden of stroke among younger adults was estimated to be AUD2.0 billion over 5 years, corresponding to a mean of $149,180 per stroke patient. Over 30 years, the economic impact was AUD3.4 billion, equating to a mean of $249,780 per case. Probabilistic sensitivity analyses revealed a mean cost per patient of $153,410 in the short term, and a mean cost per patient of $273,496 in the long term. Compared to the age-matched general population, younger adults with stroke experienced a loss of 4.58 life years and 9.21 QALYs.Conclusions: The results of our study suggests high economic and health burden of stroke among younger adults and highlights the need for preventive interventions targeting this age group.Trial Registration: ACTRN12606000185561 , retrospectively registered. Women faced with a diagnosis of breast cancer as young adults commonly experience negative effects of cancer and cancer treatment on their reproductive and sexual health (RSH) that are inadequately addressed by their healthcare providers (HCPs). The objectives of this study were to 1) identify approaches to improving RSH communication from YA breast cancer survivors' perspectives, 2) identify facilitators and barriers to the approaches identified, and 3) identify specific strategies to improve patient-centered RSH communication. We conducted individual telephone interviews with 29 women who were diagnosed with breast cancer under age 40 years. We used a grounded theory approach to identify themes, and explored how the themes related to the PCC framework to elucidate specific strategies for improving communication. Three main themes emerged: 1) Normalizing and integrating assessment of RSH concerns; 2) HCP conveying genuine caring and investment; and 3) Improving accessibility of comprehensive RSH services after cancer. Results revealed concrete strategies for improving patient-centered RSH communication at the patient-provider and health system levels. These included reminding patients that RSH concerns are common, routinely asking about RSH, using active listening, and connecting patients to HCP who can address their RSH concerns. Background: In Mozambique, HIV infection disproportionately affects young adults, particularly women. Despite awareness and knowledge of HIV transmission, many university students have not received HIV testing and continue to engage in high-risk sexual behaviors, including inconsistent condom use. Further understanding of patterns of engagement with HIV prevention and testing is key to reducing HIV transmission in this at-risk population.Methods: This study used a sequential mixed methods approach to examine patterns of engagement and perceptions of HIV prevention and testing services among higher education students in Mozambique. Survey data were collected from a representative sample of 501 students from Universidade Eduardo Mondlane (UEM) in Maputo, Mozambique to assess the primary outcomes of 1) HIV testing within the last 12 months; and 2) condom use during last sexual encounter. We employed univariate and multivariate regression models. The survey was followed by qualitative interviews with 70 survey participants which were analyzed using an inductive, content-focused analysis to further explain and contextualize survey findings.Results: Over 85% of students reported to be sexually active, among these 74% reported condom use during their last sexual encounter, and 64.2% reported an HIV test within the past 12 months. Females were more likely to have had HIV testing in the past 12 months in comparison to their male peers (aOR 1.82, 95% CI 1.11, 2.99), but were half as likely to have used a condom with their last sexual encounter (aOR 0.52, 95% CI 0.33, 0.83), when controlling for other factors. Qualitative data suggests that these discrepancies may be explained by differential perceptions in risk and trust/mistrust, with women being more concerned about infidelity by their male partner(s) and assuming more responsibility for knowing their own serostatus. Women were also subject to negative stereotypes for possessing condoms in comparison to men, which could explain lower propensity for use.Conclusion: Given gendered differences in uptake of condom use and HIV testing, and perceived HIV risk, interventions tailored specifically to male and female students may impact engagement with HIV prevention and testing and empower informed choices about sexual behaviors. Background: Smoking impairs lung immune function and damages upper airways, increasing risks of contracting and severity of infectious diseases. This paper quantifies the association between smoking and COVID-19 disease progression.Methods: We searched PubMed and Embase for studies published from January 1-May 25, 2020. We included studies reporting smoking behavior of COVID-19 patients and progression of disease, including death. We used random effects meta-analysis, meta-regression and locally weighted regression and smoothing to examine relationships in the data.Results: We identified 46 peer-reviewed papers with a total of 22,939 COVID-19 patients, 5421 (23.6%) experienced disease progression and 2914 (12.7%) with a history of smoking (current and former smokers). Among those with a history of smoking, 33.5% experienced disease progression, compared with 21.9% of non-smokers. The meta-analysis confirmed an association between ever smoking and COVID-19 progression (OR 1.59, 95% CI 1.33-1.89, p = 0.001). Ever smoking was associated with increased risk of death from COVID-19 (OR 1.19, 95% CI 1.02-1.39, p = 0.003). We found no significant difference (p = 0.864) between the effects of ever smoking on COVID-19 disease progression between adjusted and unadjusted analyses, suggesting that smoking is an independent risk factor for COVID-19 disease progression. We also found the risk of having COVID-19 progression higher among younger adults (p = 0.001), with the effect most pronounced among younger adults under about 45 years old.Conclusions: Smoking is an independent risk for having progression of COVID-19, including mortality. The effects seem to be higher among young people. Smoking prevention and cessation should remain a priority for the public, physicians, and public health professionals during the COVID-19 pandemic. Background: Youth are globally recognized as being vulnerable to HIV. Younger age has been correlated with worse health outcomes. Mobile health (mHealth) interventions have the potential to interact with youth where they are, using a device they already access. Objective: Using predefined benchmarks, we sought to evaluate the feasibility and acceptability of WYZ, an mHealth app, for improved engagement in care and antiretroviral therapy (ART) adherence among youth and young adults living with HIV. WYZ was designed and developed with input from youth and young adults living with HIV using a human-centered design approach and was based on the information, motivation, and behavioral skills framework to address common barriers to care and ART adherence among youth and young adults living with HIV. Methods: We recruited youth and young adults living with HIV (18-29 years old) from the San Francisco Bay Area to take part in a 6-month pilot trial. Their participation included completing baseline and exit surveys, and participating in seven phone check-ins about their use of WYZ. Results: Youth and young adults living with HIV (N=79) reported high levels of feasibility and acceptability with WYZ use. We met predefined benchmarks for recruitment (79/84, 94%), mean logins per week (5.3), tracking ART adherence (5442/9393, 57.9%), posting chat topics per week (4.8), and app crashes reported per week (0.24). The ease of app download, install, and setup, and comfort with security, privacy, and anonymity were highly rated (all over 91%). Additionally, participants reported high satisfaction for a research project that was remotely conducted. Participants used the app for shorter timeframes compared to the predefined benchmark. Conclusions: We noted high feasibility and acceptability with WYZ. Further research to examine the efficacy of WYZ will enable youth and young adults living with HIV and their providers to make informed decisions when using, recommending, and prescribing the app for improved engagement in HIV care and ART adherence. Objective: The COVID‐19 pandemic has had considerable economic repercussions for young workers. The current study was undertaken to examine the impact of the pandemic on the employment of young adults with rheumatic disease and on perceptions of work and health. Methods: Surveys were administered to young adults with rheumatic disease prior to and following the onset of the COVID‐19 pandemic. Surveys asked about employment status and collected information on sociodemographic, disease/health, and work‐context factors. Items also asked about the perceived impact of the COVID‐19 pandemic on work and health. A generalized estimating equation model was fitted to examine the effect of the pandemic on employment. Results: In total, 133 young adults completed the pre–COVID‐19 pandemic survey (mean age 28.9 years, 82% women). When compared to the pre–COVID‐19 pandemic period, employment decreased from 86% to 71% following the pandemic, but no other changes were identified in sociodemographic, disease/health, or work‐context factors. The time period following the COVID‐19 pandemic was associated with a 72% lower odds of employment compared to the pre‐pandemic period (odds ratio 0.28 [95% confidence interval 0.11–0.71]). Those with a postsecondary education or who reported more mental job demands were more likely to be employed following the onset of the pandemic. Also, a majority of participants reported that the pandemic affected health care (83%), treatment access (54%), working conditions (92%), and occupational health and safety (74%). Conclusion: The onset of the COVID‐19 pandemic had socioeconomic implications for young people with rheumatic disease. To support economic recovery for individuals with rheumatic disease, strategies to promote employment should be designed that account for the young adult life phase and occupational characteristics. Purpose: To characterise demographic and clinical parameters among individuals with dental anxiety and exaggerated gag reflex compared to a control group and to analyse the associations between the various parameters. Materials and Methods: Sixty-eight patients with dental anxiety and 54 patients with exaggerated gag reflex were compared to a control group of 200 individuals undergoing dental treatment. The collected data included demographic parameters, health status, smoking habits, Decayed, Missing and Filled Teeth (DMFT) and plaque index (PI) scores. Results: PI was significantly higher among patients with exaggerated gag reflex (1.91 ± 0.95) and dental anxiety patients (1.82 ± 0.89) compared to the control group (1.27 ± 0.74; p < 0.001). DMFT was significantly higher among patients with dental anxiety (13.64 ± 7.57) compared to patients with exaggerated gag reflex (10.52 ± 5.42; p = 0.033), and between both groups compared to the control group (4.09 ± 4.034; p < 0.001). Multivariate logistic regression analysis revealed that PI, DMFT and age were positively associated with exaggerated gag reflex and that DMFT and educational level were positively associated with dental anxiety compared to the control group. DMFT was the only significant parameter positively associated with dental anxiety compared to exaggerated gag reflex. Conclusion: DMFT and PI scores were higher among patients with dental anxiety and exaggerated gag reflex. Clinicians should consider additional oral hygiene measures and education, maintenance meetings and recall visits in those patients, as well as using supplementary aids, such as fluoride mouthwash and fluoride varnish applications, to maintain oral hygiene without triggering the exaggerated gag reflex. Background: Newly reported hepatitis C virus (HCV) infections in California increased 50% among people 15-29 years of age between 2014 and 2016. National estimates suggest this increase was due to the opioid epidemic and associated increases in injection drug use. However, most of California's 61 local health jurisdictions (LHJs) do not routinely investigate newly reported HCV infections, so these individuals' risk factors for infection are not well understood. We sought to describe the demographics, risk behaviors, and utilization of harm reduction services in California's fastest-rising age group of people with newly reported hepatitis C infections to support targeted HCV prevention and treatment strategies.Methods: California Department of Public Health invited LHJs to participate in enhanced surveillance if they met criteria indicating heightened population risk for HCV infection among people ages 15-29. From June-December 2018, eight LHJs contacted newly reported HCV cases by phone using a structured questionnaire.Results: Among 472 total HCV cases who met the inclusion criteria, 114 (24%) completed an interview. Twenty-seven percent of respondents (n = 31) had ever been incarcerated, of whom 29% received a tattoo/piercing and 39% injected drugs while incarcerated. Among people who injected drugs (PWID)-36% (n = 41) of all respondents-68% shared injection equipment and many lacked access to harm reduction services: 37% knew of or ever used a needle exchange and 44% ever needed naloxone during an overdose but did not have it. Heroin was the most frequently reported injected drug (n = 30), followed by methamphetamine (n = 18). Pre-diagnosis HCV risk perception varied significantly by PWID status and race/ethnicity: 76% of PWID vs. 8% of non-PWID (p < 0.001), and 44% of non-Hispanic White respondents vs. 22% of people of color (POC) respondents (p = 0.011), reported thinking they were at risk for HCV before diagnosis. Eighty-nine percent of all respondents reported having health insurance, although only two had taken HCV antiviral medications.Conclusions: Among young people with HCV, we found limited pre-diagnosis HCV risk perception and access to harm reduction services, with racial/ethnic disparities. Young adults with attention-deficit/hyperactivity disorder (ADHD) who are prescribed stimulant medications may face a higher risk of developing cardiomyopathy, according to new research. The risk of cardiomyopathy increases with the duration of treatment. Previous research has shown a link between amphetamines used to treat ADHD and elevated blood pressure and increased resting heart rate, as well as a connection between dilated cardiomyopathy and methamphetamine use. This study highlights the need for a better understanding of the cardiovascular risks associated with ADHD stimulant medications and the importance of considering alternative treatments. Objective: The pathogenesis of sudden cardiac death may differ between younger and older adults in schizophrenia, but evidence remains scant. This study investigated the age effect on the incidence and risk of the physical and psychiatric comorbidity for sudden cardiac death.Methods: Using 2000 to 2016 data from the Taiwan National Health Insurance Research Database and Department of Health Death Certification System, we identified a national cohort of 170,322 patients with schizophrenia, 1,836 of whom had a sudden cardiac death. Standardized mortality ratios (SMRs) were estimated. Hazard ratios and population attributable fractions of distinctive comorbidities for sudden cardiac death were assessed.Results: The SMRs of sudden cardiac death were all >1.00 across each age group for both sexes, with the highest SMR in male patients aged <35 years (30.88, 95% CI: 26.18-36.18). The fractions of sudden cardiac death attributable to hypertension and congestive heart failure noticeably increased with age. By contrast, the fraction attributable to drug-induced mental disorder decreased with age. Additionally, chronic hepatic disease and sleep disorder increased the risk of sudden cardiac death in patients aged <35 years. Dementia and organic mental disorder elevated the risk in patients aged between 35-54 years. Ischemic heart disease raised the risk in patients aged ≥55 years.Conclusions: The risk is increased across the lifespan in schizophrenia, particularly for younger male patients. Furthermore, physical and psychiatric comorbidities have age-dependent risks. INTRODUCTION: Acute ischemic stroke (AIS) in the young age (≤50 years) is a major cause of disability. The underlying mechanism of AIS in this age group is usually different from elderly. Transthoracic echocardiography (TTE) is used to detect the potential cardiac sources of embolism in AIS patients. Transthoracic echocardiogram (TEE) is superior to detect specific underlying cardio-aortic source of embolism when compared to TTE. We aim to evaluate the diagnostic yield and therapeutic impact of TEE in AIS of young adults. METHODS: We retrospectively reviewed the consecutive patients with AIS in our comprehensive center in a 5-year period from our prospectively collected registry. We selected patients with age ≤50 years who had acute infarcts on brain magnetic resonance imaging or head computed tomography and underwent TEE as part of their diagnostic workup. Demographic details including, age, gender, body mass index, cardiovascular risk factors profile, and TEE findings were collected. RESULTS: Among a total 7,930 patients, 876 (11.04%) were found to be ≤50 years old. Among those, TEE was done in 113 patients (12.8%) in addition to TTE. Those who underwent TEE had a mean age of 40.4 ± 7.9 years, 60 were male (53%), 7 (6.2%) had a history of coronary artery disease, 38 (33%) had a history of diabetes, and 45 (40%) had a history of smoking. TEE showed new abnormal findings in a total of 15 patients (13.2%) that were not reported in their TTEs. Out of these, left atrial appendage thrombus was found in 5, infective endocarditis in 4, atrial septal aneurysms associated with patent foramen ovale (PFO) in 3, and spontaneous mobile echo density in three patients. Overall, new findings from TEE resulted in change in the secondary stroke prevention strategy in 14 patients of those who underwent TEE (12.3%). TEE also confirmed the presence of PFO, which was present on TTE with bubble study in 20 (17.6%) patients. CONCLUSION: TEE may provide additional information in the evaluation of the AIS in young adults, which could lead to change of the secondary stroke prevention strategy. [ABSTRACT FROM AUTHOR]

INTRODUCTION: Young adults with alcohol-associated liver disease (ALD) are the fastest increasing demographic contributing to liver-related deaths; their outcomes after liver transplantation (LT) are understudied. METHODS: Using the United Network for Organ Sharing registry, we performed sex-specific analyses because of a significant interaction between sex and the explanatory variable, age. Cox regression was used with overall post-LT death as the primary outcome, adjusted for survival characteristics and center clustering. We calculated the absolute difference in adjusted 5-year post-LT survival between patient groups. Causes of death were supplemented by manual review of free-text entries. RESULTS: Among 42,014 LT recipients, 16,190 women (2,782 with ALD and 13,408 without ALD) and 25,824 men (9,502 with ALD and 16,322 without ALD), age of 40-50 years had the lowest risk of death. Women with ALD younger than 40 years had incrementally lower adjusted 5-year survival (95% confidence interval): 74% (63%-88%) for those aged 18-29 years, 82% (78%-87%) for those aged 30-39 years, and90% (88%-92%) for those aged 40-49 years. Among women without ALD, men with ALD, and men without ALD, adjusted 5-year survival for ages 18-29, 30-39, and 40-49 years was similar. Among women, not men, there were significant interactions between younger age and ALD. Adjusted hazard for mortality for women with ALD vs without ALD was greater for those who aged 18-29 years (2.82 vs 1.09, P = 0.002) and 30-39 years (1.83 vs 1.09, P = 0.007 [reference age 40-49 years]). Amongwomen with ALD, those aged 18-29 and 30-39 years had an absolute 17.7% and 9.5% excess in adjusted 5-year mortality vs similarly aged women without ALD. DISCUSSION: Young women (age < 40) with ALD have excess mortality beyond one-year post-LT. Background Management of cardiovascular sequelae to Kawasaki disease (KD) is challenging to adult cardiologists. Vasculitis of medium-sized arteries especially coronary arteries often leads to focal intimal thickening and aneurysmal dilatation of one or more coronary arteries. It needs special attention to recognize coronary artery involvement because of potential long-term morbidity and mortality. We present a case of diagnostic dilemma in young adult Chinese male with KD. Case summary This asymptomatic patient was found to have deep Q waves in anterior leads on screening electrocardiography and was thought to have myocarditis after depiction of wall motion abnormality on echocardiography, later to be confirmed to have left anterior descending artery (LAD) territory infarct on cardiac magnetic resonance imaging. Coronary computed tomography angiogram depicted proximal LAD aneurysm with calcified plaque/thrombus. Additionally, there was an 18 mm giant right coronary artery (RCA) aneurysm with braid-like appearance and soft plaque (mural thrombus). His previous medical history included fever and cervical lymphadenopathy. Because of the high risk he was commenced on long-term low-dose aspirin and β-adrenergic-blocking agent to reduce myocardial oxygen consumption; however, 3 years later, he presented to the emergency department with acute inferior myocardial infarction. He was noted to have total occlusion of the proximal RCA and was treated aggressively with thrombectomy and percutaneous balloon angioplasty followed by medical management with β-blockers, sacubitril/valsartan, clopidogrel, and rivaroxaban without subsequent adverse cardiovascular events. This study investigated the influence of illness on sexual risk behavior in adolescence and the transition to adulthood, both directly and through moderation of the impact of social disadvantage. We hypothesized positive effects for social disadvantages and illness on sexual risk behavior, consistent with the development of faster life history strategies among young people facing greater life adversity. Using the first two waves of the National Longitudinal Study of Adolescent to Adult Health, we developed a mixed-effects multinomial logistic regression model predicting sexual risk behavior in three comparisons: risky nonmonogamous sex versus safer nonmonogamous sex, versus monogamous sex, and versus being sexually inactive, by social characteristics, illness, interactions thereof, and control covariates. Multiple imputation was used to address a modest amount of missing data. Subjects reporting higher levels of illness had lower odds of having safer nonmonogamous sex (OR = 0.84, p <.001), monogamous sex (OR = 0.82, p <.001), and being sexually inactive (OR = 0.74, p <.001) versus risky nonmonogamous sex, relative to subjects in better health. Illness significantly moderated the sex (OR = 0.88, p <.01), race/ethnicity (e.g., OR = 1.21, p <.001), and childhood SES (OR = 0.94; p <.01) effects for the sexually inactive versus risky nonmonogamous sex comparison. Substantive findings were generally robust across waves and in sensitivity analyses. These findings offer general support for the predictions of life history theory. Illness and various social disadvantages are associated with increased sexual risk behavior in adolescence and the transition to adulthood. Background: Hyperlipidemia is associated with arterial stiffness. Herein, We examined the effect of prolonged exposure to hyperlipidemia on the risk of arterial stiffness in young adults.Methods: A study cohort (35-55 years old) that received health check-ups in the Kailuan study (2014-2016) were assessed. Hyperlipidemia was defined as a low-density lipoprotein cholesterol ≥160 mg/dL according to the Chinese Guidelines for the Management of Dyslipidemia in Adults. Subjects were divided into three groups based on the number of years with hyperlipidemia: normal (0 years), low exposure (1-5 years), and high exposure (5-10 years) groups. Arterial stiffness was defined as brachial-ankle pulse wave velocity > 1400 cm/s. For all subjects and subjects that did not meet statin treatment criteria under guidelines, logistics regression was used to analyze the effect of prolonged hyperlipidemia exposure on arterial stiffness in different age groups.Results: Among 12,431 subjects, the mean age was 46.42 ± 5.34 years with 9000 men (72.4%). Brachial-ankle pulse wave velocity gradually increased with increased exposure duration. Logistic regression analysis showed that hyperlipidemia exposure was a risk factor for arterial stiffness in the low (1.22 times) and high (1.49 times) exposure groups compared with the normal group. In the different age groups, the risk of arterial stiffness increased with the duration of hyperlipidemia exposure, apart for the 35-40-year-old population. The effect of hyperlipidemia exposure duration on arterial stiffness in young adults that did not meet statin treatment criteria under guidelines was similar to the general population.Conclusions: Prolonged exposure to hyperlipidemia in young adults increases the risk of arterial stiffness. The aim of this study was to extensively describe the epidemiological, clinical and therapeutic outcomes of adolescents and young adults (AYA) population with classical Hodgkin Lymphoma (cHL). Then, a comparison between AYAs and adults and between the subgroups of AYAs treated with the same adult protocol was accomplished to further inform on optimal therapy approach of choice for adolescent patients. In this mono-centric, retrospective study, we reviewed the medical records. We analyzed 112 consecutive North Tunisian patients, including 66 AYAs (15 to 39 years) and 46 adults (≥ 40 years) affected by cHL treated from 2000 to 2015 at Salah Azaiez Institute. Then, we performed a comparative analysis between AYA and 46 adult patients and a subgroup analysis between adolescents and young adults. All patients were treated according to the national protocol for HL, edited by the Tunisian Society of Hematology. The treatment included chemotherapy and involved-field radiotherapy (RT) at a dose of 20 or 30 Grays (Gy) for responders and 36 Gy for non-responders. AYA patients presented with adverse features with nodular sclerosis subtype (p = 3.88 × 10−02) and mediastinal mass involvement (p = 9.40 × 10−04). At a median follow-up of 51 and 32 months for AYAs and adults, respectively, no statistical difference in terms of 3 and 5-years overall survival (OS) and event-free survival (EFS) was shown. Using the Kaplan-Meier method, in AYAs, the ABVD regimen has an impact on 3-years EFS (p = 4.63 × 10−02). The 36 Gy RT was associated with the best 3-years EFS (p = 9.24 × 10−03). Besides, AYA patients with advanced-stage had the worst 3-years OS (76%) (p = 2.41 × 10−02). Although the adolescents and young adults shared similar clinical presentation, we noted that the adolescent group had the worst 3-years EFS (48%), but the best 3-years OS (91%). We identified 15% of primary refractory patients and a rate of toxicity of 5.3% in AYA. The treatment approach used is well tolerated by adult patients. However, the AYA patients and particularly adolescent subgroup had more advanced disease at diagnosis and should be treated more intensively in dedicated units. This study investigated the prevalence, awareness, treatment, and control of hypertension and associated factors among urban adults in southwestern China. The study was conducted from 2013–2014 and used a multistage cluster sampling method to select a representative sample of 11,517 people in southwestern China, aged 35–79 years. Hypertension was defined as either systolic blood pressure of 140 mmHg or greater, diastolic blood pressure of 90 mm Hg or greater, or self-reported current treatment for hypertension with antihypertensive medications. In the study population, hypertension prevalence was found to be 38.4%, with rates of 40.0% and 37.5% for men and women, respectively (p = 0.03). Hypertension prevalence increased with age in both men and women (trend p both <0.01). Among hypertensive patients, 47.9% were aware of their hypertension, 40.1% were undergoing antihypertensive treatment, and 10.3% achieved BP control. A multiple-factor analysis revealed that age, male gender, low educational achievement, family history of hypertension, overweight or obesity, abdominal obesity, and hypertriglyceridemia were positively related to hypertension, while physical exercise was negatively related to hypertension. The prevalence of hypertension among urban adults aged 35 to 79 years in southwestern China was high, while levels of awareness, treatment, and control of hypertension were low. Sexually transmitted infections (STIs) are among the most common infectious diseases in the United States with nearly 20 million new cases diagnosed each year in young adults (age 15–24). Communicating with romantic partners about STIs can positively influence health outcomes, as treatment can then be sought, yet the discussions themselves can pose several face risks to both the person requesting STI information from a partner and the receiver of that request. This study examines how young adults enact and manage facework strategies in STI-related conversations with romantic partners, using Politeness Theory and Communication Privacy Management Theory as theoretical frameworks. Specifically, this study seeks to understand how self-efficacy in conversations about STIs, intentions to talk about STIs and willingness to communicate about health in general predicts four face response strategies (avoid, communicate with partner/talk, defend self, consider other), and to investigate how boundary permeability moderates each of these relationships. We found that boundary permeability affected the relationships between intentions regarding STIs and the communicate with partner and consider other face management responses in conversations about STIs. Purpose: To evaluate the effect of tobacco smoking and salivary calcium on the periodontal status of young adults. Materials and Methods: Plaque index, gingival bleeding on probing, supragingival calculus, DMFT index, salivary low, pH and salivary calcium in unstimulated salivary samples were recorded in smokers and nonsmokers. Results: There were no significant differences between smokers and nonsmokers with respect to salivary low (P = 0.08) and calcium level (P = 0.09). Significant correlations (P < 0.05) were found between a higher calcium level and higher probing depth (r = 0.60), higher number of teeth with probing depth of more than 4 mm (r = 0.70), greater clinical attachment level (r = 0.49) and lower number of teeth bleeding on probing (r = -0.50). Conclusion: Moderate smoking of tobacco cigarettes in young healthy subjects may not have a significant impact on salivary low or calcium concentrations in unstimulated saliva, but an association exists between an increased level of salivary calcium and development of periodontal disease. Aim: The aim of the present explorative cross-sectional study was to evaluate the potential role of some periodontal variables in predicting sub-clinical atherosclerosis and systemic inflammation in young adults (=40 years) with severe periodontitis compared with healthy individuals without periodontitis. Materials and methods: A total of 90 systemically healthy subjects, 45 (mean age 36.35±3.65 years) affected by severe periodontitis (test group) and 45 individuals (mean age 33.78±3.28 years) without a history of periodontal disease (control group) were enrolled in the present study. Test and control groups were paired for age, gender, body mass index (BMI) and smoking habits. Carotid intima-media thickness (carotid IMT) was bilaterally assessed by ultrasonography at the level of the common carotid artery to evaluate sub-clinical atherosclerosis. Blood samples to assess inflammatory markers (leucocytes, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, total serum cholesterol, high sensitivity C-reactive protein, glucose and haemoglobin A1c% were taken. Results: In periodontal patients, mean probing depth (PD) was a predictor of mean carotid IMT (P=0.0005), BMI (P=0.0002), systolic blood pressure (P=0.0300) and diastolic blood pressure (P= 0.0199). Full-mouth bleeding score (FMBS) was a predictor of C-reactive protein levels (P=0.0218). In controls, full-mouth plaque score was a predictor of glucose levels (P=0.0074) and higher FMBS of lower triglycerides levels (P=0.0312). Objective. To study the peculiarities of the realization of psychosomatic disorders in patients with gastroesophageal reflux disease with concomitant diabetes mellitus type 2 and to develop methods for their correction. Materials and methods. In order to obtain the results we conducted a study of two groups of patients suffering from GeRD with concomitant mild to moderate diabetes mellitus type 2 and isolated GeRD. The survey program included: determining the degree of carbohydrate metabolism compensation; state of aggression factor. For the evaluation of gastric secretion we used the daily pH-metry. also, all patients underwent an ultrasound investigation with water load. Indices of oxidative stress were assessed by the diameter of the celiac trunk and blood flow velocity in it. psychosomatic disorders and quality of life of the patients were estimated by conducting a survey using unified questionnaires. Results. We established that in patients with GeRD with concomitant diabetes mellitus there was a clear tendency towards stabilization of the glycemic profile. In patients of both groups, episodes of heartburn, frequency and intensity of belching, and dyspeptic phenomena decreased. We found that, after a 4-week course of therapy, oxidative stress levels in patients of both treatment groups decreased. The findings suggest that psychosomatic disorders are factors that not only make the course of GeRD and DM type 2 more difficult, but also take a direct part in the formation of GeRD itself in DM type 2. The results of our studies also clearly indicate that the additional assigning of actovegin in the general scheme of treatment for patients with DM type 2 with concomitant GeRD has an evident positive effect on the reduction of the onset of clinical remission of both DM type 2 and concomitant GeRD. The inclusion of actovegin in the general treatment scheme of patients with GeRD and DM contributes to a significant reduction in the recrudescence period of both DM and concomitant GeRD. The psychosocial consequences of living with a depressed parent have been well characterized. Less well known, however, is how this exposure is predictive of later physical health problems. The present study evaluated how parental depression across youths' adolescence (ages 11-18) was associated with youth metabolic syndrome at age 25 (n = 391). Youth self-regulation and health behaviors were considered as possible moderators of the link between parental depression and youth metabolic syndrome. Analyses revealed that parental depression in adolescence was associated with a composite score reflecting metabolic syndrome components in early adulthood. Furthermore, self-regulation and health behaviors moderated this link, such that links between parental depression and the metabolic syndrome existed only for youth with low self-regulation or unhealthy behaviors. Adolescents and young adults are often in a particularly vulnerable position following acquired brain injury (ABI). In addition to neurological and cognitive impairment, they are faced with issues concerning education, job, family, and social life. Moreover, they may be limited in meeting peers and may be left alone with psychosocial issues. This paper investigates how this patient group may benefit from meeting like-minded peers. From information gathered through a questionnaire and interviews with participants in a peer support group, the study aimed to investigate the social and psychological advances such a group can offer, and how this may contribute to psychosocial recovery following ABI. Ewing's sarcoma (ES) is a small round cell tumor of adolescents or young adults that usually arises in the deep soft tissues of the extremities. The tumor cells have uniform round nuclei, fine powdery chromatin and indistinct nucleoli. CD99 (O13) is a product of the MIC 2 gene that is highly sensitive to ES but not specific. A panel of markers should be used for the differential diagnosis of small round cell tumors because nearly all others, on occasion, show membranous staining for CD99. One of the defining feature of ES is the presence of 22q12 gene rearrangement. The presented case is a 6 year-old boy complaining of swelling on his right leg. The biopsy was compatible with classic ES in terms of histopathological, immunohistochemical and cytogenetic criteria. Wide surgical resection was performed after chemotherapy. The posttreatment specimen was composed of uniformly small round cells mixed with areas of ganglion cells embedded in neurophil-like fibrillary background. Immunohistochemically, neoplastic cells revealed strong CD99 (O13) and NSE staining and the tumor had EWSR1 gene rearrangement. Morphologic alterations due to treatment are commonly seen in pediatric tumors. Single case reports have defined neural differentiation in ES but to the best of our knowledge this is the first report of ES in the literature with all histopathological, immunohistochemical, and cytogenetic criteria evaluated in both pretreatment and posttreatment specimens. Background & aims: The offspring of patients with premature coronary artery disease (P-CAD) are at higher risk for cardiovascular disease, compared with subjects without a family history (FH) of P-CAD. The increased risk for cardiovascular disease in subjects with FH of early-onset CAD results from unfavorable genetic variants as well as social, behavioral and environmental factors, which are more prevalent in this group. Previous studies have shown that specific sex hormone levels may be associated with the risk of cardiovascular disease. The aim of this study was to compare wide range of biochemical marker levels including i.e. the levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), progesterone, estradiol, testosterone and sex-hormone binding globulin (SHBG) between young healthy male adults with and without FH of P-CAD. Methods: The study group consisted of young healthy Polish male adults enrolled in a MAGNETIC case-control study, who were recruited between July 2015 and October 2017. The inclusion criteria were as follows: male sex, age ≥18 and ≤35 years old, FH of P-CAD (cases) or no P-CAD in first-degree relatives (controls). The comparison of continuous and categorical variables was performed using the Student’s t-test or the U-Mann–Whitney test, and Fisher’s exact test, respectively. The correlations between FSH, LH, testosterone, progesterone, SHBG and other laboratory parameters were assessed using the Spearman rank correlation test. Both univariable and multivariable logistic regression analyses were performed to assess the association between analyzed variables and FH of P-CAD. Results: A total of 411 subjects (223 cases and 188 controls) were included in the study. There was a higher prevalence of major cardiovascular risk factors in subjects with FH of P-CAD (smoking, higher total and LDL cholesterol levels, higher body mass index and lower HDL cholesterol level). Moreover, the offspring of patients with P-CAD had lower SHBG level, and higher LH and progesterone levels in the crude comparison, compared with individuals without FH of P-CAD. After adjustment for confounding variables, progesterone and LH were determined to be independently associated with FH of P-CAD. Introduction: Periodontitis is ideally considered as one of the most ubiquitous diseases and is identified by bone and connective tissue destruction as a consequence of periodontal pathogens affecting the host response, leading to an inflammatory reaction. The main aim of this study was to determine the pediatric dentist's familiarity and effective treatments of periodontal disease in children. Materials and Methods: A closed-ended questionnaire was given to 100 pediatric dentists working in private clinics and in an institution. The questionnaire was handed out in person to each of the practicing pedodontist and their responses were analyzed and tabulated accordingly. Results: From the data obtained, it was noted that majority of the pediatric dentists were not only acquainted with the various gingival and periodontal diseases occurring in children but also familiar with the various treatment measures available to control the severity of such diseases in younger adults. Conclusion: There are hardly any studies regarding the knowledge and awareness of pediatric dentists toward the several gingival and periodontal diseases affecting the children. Inflammatory bowel disease is a chronic inflammatory disease that consist of Crohn's disease and ulcerative colitis. A dramatic increase in the prevalence if inflammatory bowel disease (IBD) within developing countries including in Asia makes IBD a substantial global medical burden and modern refractory disease, and it is imperative that Asian healthcare systems especially physician are aware of this and familiar with the clinical manifestation and management. The aim of this case report is to present a case of IBD in young adult in Indonesia. Patient came with complaint of chronic diarrhea and had seen multiple doctor without improvement. Inflammatory bowel disease should always be considered in patient with chronic diarrhea with or without extraintestinal manifestation especially after poor response of usual antidiarrheal medication. Diagnosing ofIBD is based on clinical manifestation, physical examination, laboratory result and endoscopy. Until now, ileocolonoscopy and biopsy is the most accurate diagnostic modality for IBD and can be used to differentiate between ulcerative colitis and Crohn's disease. Aim: The aim of the study was to define the patterns of attachment in adults with foetal alcohol syndrome from foster, adoptive and biological families. It was assumed that adults with foetal alcohol syndrome develop insecure attachment styles to a greater extent compared to the general population. Material and methods: Two groups of 30 people participated in this two-stage research. In the first stage, foetal alcohol syndrome was diagnosed in children using the University of Washington 4-Digit Diagnostic Code for foetal alcohol spectrum disorders. During the second stage, attachment style of young adults was assessed with the Polish version of the Experience in Close Relationships-Revised questionnaire. Furthermore, in-depth interviews were conducted with the parents or caregivers of individuals with foetal alcohol syndrome at both stages. In order to compare research results, a control group of 30 young adults with excluded foetal alcohol syndrome was formed. Results: Adults diagnosed with foetal alcohol syndrome achieved high scores in insecure attachment where a significant role is played by avoidant attachment and the fear of building close relationships. The degree of secure attachment in persons with foetal alcohol syndrome is markedly higher compared to the general population. Findings showed that in terms of attachment prototypes, the preoccupied type was prevalent (40%) and was followed by secure type (27%), and dismissing-avoidant type (23%) in the foetal alcohol syndrome group, whereas secure style dominated (61%) in the control group, and it was followed by the preoccupied (21%) and avoidant (13%) styles. Conclusions: The research showed that insecure attachment styles based on avoidance and anxiety dominate in adults diagnosed with foetal alcohol syndrome, which can cause disorders in creating bonds. International Journal of Exercise Science 12(3): 898-903, 2019. Hypertension is highly prevalent and associated with non-communicable diseases and increased premature mortality risk. However, the impact of the new hypertension diagnostic criteria on the prevalence of hypertension diagnoses has yet to be examined among college students. The purpose of this study was to compare the prevalence of hypertension between the JNC7 and the ACC/AHA hypertension diagnostic criteria among college students. The blood pressure of 5,945 college students was assessed, and chi-square tests for independence examined differences between JNC7 and ACC/AHA criteria. The mean age of participants was 21.30 ± 1.05 years, and the majority identified as men (60.5%). Men were found to have significantly higher systolic (p < .001, n² = .10) and diastolic (p < .001, n² = .04) blood pressure, so all analyses were separated by sex. Hypertension guideline changes resulted in significant changes in hypertension categorization of both men, X² = 7,178, p < .001, Фc = .816 and women, X² = 4,670, p < .001, Фc = .816. Under the JNC7 guidelines, 292 (8.2%) men and 67 (2.8%) women were hypertensive. Using the ACC/AHA guidelines, 1455 (40.5%) men and 521 (22.3%) women were hypertensive. Hypertension guideline changes resulted in a significant increase in the prevalence of hypertension among college students, highlighting the potential demand for targeted prevention programs focused on fostering healthy lifestyle behaviors, i.e. physical activity and healthy eating, among students. Migraine is a disease that peaks in late adolescence and early adulthood. The aim of this study was to evaluate age-related brain changes in resting state functional connectivity (rs-FC) in migraineurs vs. age-sex matched healthy controls at two developmental stages: adolescence vs. young adulthood. The effect of the disease was assessed within each developmental group and age- and sex-matched healthy controls and between developmental groups (migraine-related age effects). Globally the within group comparisons indicated more widespread abnormal rs-FC in the adolescents than in the young adults and more abnormal rs-FC associated with sensory networks in the young adults. Direct comparison of the two groups showed a number of significant changes: (1) more connectivity changes in the default mode network in the adolescents than in the young adults; (2) stronger rs-FC in the cerebellum network in the adolescents in comparison to young adults; and (3) stronger rs-FC in the executive and sensorimotor network in the young adults. The duration and frequency of the disease were differently associated with baseline intrinsic connectivity in the two groups. fMRI resting state networks demonstrate significant changes in brain function at critical time point of brain development and that potentially different treatment responsivity for the disease may result. Purpose: The goal of the fixation of painful osteochondritis dissecans of the femoral condyles in adults is to integrate the osteochondral fragment and thus achieve a normal hyaline cartilaginous coverage. The addition of a biological process to primary fixation may result in improved fragment integration (hybrid fixation). Osteochondral plugs may fulfil this role. The aim of this study was to evaluate long-term clinical and radiological results after hybrid fixation of unstable osteochondritis dissecans. The hypothesis was that the rate of secondary osteoarthritis would be low.Methods: Nine patients treated by hybrid fixation were retrospectively reviewed at a median follow-up of 10.1 years (range 7-14). The median age at surgery was 21 (range 17-28). Six of them were evaluated as ICRS grade II and three, as ICRS grade III. The mean surface of the lesion was 4.5 cm2. All patients were followed up clinically (IKDC, KOOS, Lysholm) and radiologically [Kellgren-Lawrence score (KL)].Results: During arthroscopic assessment at the time of screw removal (3 months after surgery), the fragments were stable, and autograft plugs were all well integrated. At the most recent follow-up visit, the median IKDC score was 85.8 (range 51.72-100), the KOOS score was 87.7 (52.4-100), and the Lysholm scale score was 89.8 (77-100). In 7 out of 9 patients, radiographs showed a joint space KL grade of 0 or 1.Conclusion: Hybrid fixation for treating osteochondritis dissecans lesions of the femoral condyles using mechanical and biological fixation provides healing of the osteochondral fragments with good long-term outcomes. Aim: Major depressive disorder (MDD) onset generally occurs in adolescence/early adulthood. However, pharmacotherapy use in younger populations is restricted due to black box warnings. Aerobic exercise may be a viable treatment option for mild‐to‐moderate MDD, but little is known about its acceptability/effectiveness in young adults. Methods: Unmedicated and relatively inactive 18‐to‐24 olds with MDD completed fitness/clinical assessments at baseline and after 12 weeks of supervised aerobic exercise (3×/wk; 30‐minute sessions in target heart rate [HR] zone), with the aim of increasing cardiovascular fitness (VO2max‐indexed). Results: Post‐intervention, predicted VO2max increased, whereas depression scores decreased. A correlation existed between time spent in target HR zone and anxiety symptom decreases. Exercise adherence and satisfaction were high, and drop‐out was minimal. Conclusions: This pilot is among the first to assess the feasibility of aerobic exercise as an antidepressant treatment strategy in young adults, a group for which options have limited acceptability. Aim: To determine the long-term course of difficulty in participation of individuals with cerebral palsy (CP) without intellectual disability between 16 years and 34 years of age.Method: One hundred and fifty-one individuals with CP aged 16 to 20 years were included (63% male, 37% female; Gross Motor Function Classification System [GMFCS] levels I-IV; without intellectual disability). The Assessment of Life Habits questionnaire 3.0 general short form was used up to three times biennially and at 13-year follow-up (13-year follow-up: n=98). Scores (range 0-10) reflect difficulty and assistance in participation in housing, education and employment, interpersonal relationships, recreation, community life, and responsibilities. Multilevel models were used to determine the course of difficulty in participation by GMFCS level.Results: Despite high average participation levels, 41% to 95% of adolescents and young adults with CP experienced difficulty. Difficulty in participation in housing and interpersonal relationships increased from age 16 years onwards and in most other life areas in the mid- and late 20s. In adolescents in GMFCS levels III and IV, participation in recreation and community life improved up to age 23 years.Interpretation: Individuals with CP experience increasing difficulties in participation in their mid- and late 20s. Summary: Hodgkin lymphoma (HL) commonly occurs in adolescents and young adults (AYA), defined by the National Cancer Institute as people diagnosed with cancer between the ages of 15 and 39 years. Despite therapeutic advances, the AYA population has derived less incremental benefit compared to both paediatric and adult counterparts. Although the exact aetiology is unclear, contributing factors probably include differences in disease biology, delayed diagnosis, decreased participation in clinical trials and treatment adherence secondary to complex social factors. As such, while HL remains highly curable, there is not a clear consensus regarding the management of patients within this age range, specifically whether paediatric or adult regimens are preferred or how best to incorporate emerging therapeutic advancements. Ongoing clinical trials, as well as continued collaborative efforts are required to address the needs of this population, investigate the potential for unique biological factors and allow for optimization of treatment. Here we review current prognostic and treatment strategies for paediatric and adult patients with HL and highlight complexities around the management of this patient population. The timing and location of the first cases of the 1918 influenza pandemic are still controversial, a century after the pandemic became widely recognized. Here, we critically review competing hypotheses on the timing and geographical origin of this important outbreak and provide new historical insights into debates within military circles as to the nature of putative pre-1918 influenza activity. We also synthesize current knowledge about why the 1918 pandemic was so intense in young adults. The aim of this study was to investigate the relationship between serum ferritin level and antioxidative status and metabolic dysregulation in young adult obese population. This cross-sectional study included 300 subjects of either sex, grouped as obese and non-obese subjects. The body mass index, total iron binding capacity, fasting blood glucose, superoxide dismutase activity, and levels of serum ferritin, iron, total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, triglyceride, glutathione, and vitamin C were estimated. Analysis showed a significant alteration in all the parameters in obese adults. The correlation of ferritin level and body mass index showed a positive correlation (r = −0.81, p < 0.001, respectively) with levels of fasting blood glucose, superoxide dismutase, total cholesterol, low-density lipoprotein cholesterol, and triglyceride in obese individuals, whereas an insignificant correlation with vitamin C and glutathione level was observed in obese individuals. The significant positive correlation of ferritin level with the metabolic parameters and some antioxidative parameters in obese individuals signifies the development of metabolic disorders. Summary: The incidence of acquired aplastic anaemia (AA) peaks in adolescents and young adults (AYA). Although age has been associated with response after immunosuppressive therapy (IST), few data exist about the specific outcome of AYA. We retrospectively compared the outcome of 29 children (aged <15 years), 32 AYA (15–25 years) and 23 adults (>25 years) with AA treated front‐line with IST in Saint‐Louis Hospital. The cumulative incidence of response was lower in adults compared with AYA (subdistribution hazard ratio [SHR] = 0·38, 95% confidence interval [CI] [0·96–1·00], P = 0·008), but no difference was observed between children and AYA (SHR = 0·84, 95% CI [0·96–1·00], P = 0·56), with a 6 months cumulative incidence of partial response of 44·8% in children, 62·5% in AYA and 21·7% in adults. The 5‐year failure‐free survival was 48·4%, without impact of age, with a 5‐year relapse rate of 20·7%. With a median follow‐up of 5·4 years, the 5‐year overall survival was 86·5%, without significant difference between children and AYA overall survival (hazard ratio [HR] 1·51, 95% CI [0·25–9·02], P = 0·66), while adults displayed poorer survival than AYA (HR 4·98, 95% CI [1·00–24·73], P = 0·049). This study confirms that age is a prognostic factor in AA patients treated with IST. However, AYA patients have a similar outcome to children in terms of response rate and survival. Abstract Heart failure is an important cause of late morbidity and mortality in children, adolescents, and young adults with congenital heart disease. Heart failure is the first cause of mortality in adult congenital heart disease population accounting for 30% to 50% of death while the incidence related to ventricular arrhythmias accounts for another 20%. Symptomatic heart failure requiring hospitalization is associated with 3% to 5% mortality during hospital stay and a 25% incidence of death within the first year after admission. Patients at highest risk for death as a result of heart failure are those with single-ventricle physiology or systemic right ventricular anatomy; however, patients with left-sided obstruction or chronic volume overloaded right ventricle are also at a higher risk than the general population. Medical therapy is scarcely effective in most cases of congenital heart disease related heart failure in children, adolescents and young adults. Lack of donors limits access to heart transplantation and results in adult congenital heart disease population are worse than those in other cardiac populations with heart failure. This study uses narrative interviews to describe how 25 young adult (YA) close supporters (i.e., romantic partners, siblings) experience an often invisible and uniquely burdensome biographical disruption following a young adult cancer survivor's (YACS; i.e., 18-39) diagnosis. YA supporters reported varying levels of biographical interdependence with the YACS and family system uncertainty with other important supporters. These factors led YA romantic partners and siblings to engage in different types of communication work to negotiate and maintain primary and secondary support roles with parents and in-laws, to ensure that the YACS received helpful social support by gatekeeping (i.e., correcting, blocking) others' support attempts, and to accommodate both their own and the YACS's disrupted biographies. Objective. We investigated the preliminary efficacy of cathodal transcranial direct current stimulation (tDCS) combined with bimanual training in children and young adults with unilateral cerebral palsy based on the principle of exaggerated interhemispheric inhibition (IHI). Methods. Eight participants with corticospinal tract (CST) connectivity from the lesioned hemisphere participated in an open-label study of 10 sessions of cathodal tDCS to the nonlesioned hemisphere (20 minutes) concurrently with bimanual, goal-directed training (120 minutes). We measured the frequency of adverse events and intervention efficacy with performance (bimanual—Assisting Hand Assessment (AHA)—and unimanual—Box and Blocks), self-report (Canadian Occupational Performance Measure (COPM), ABILHAND), and neurophysiologic (motor-evoked potential amplitude, cortical silent period (CSP) duration, and motor mapping) assessments. Results. All participants completed the study with no serious adverse events. Three of 8 participants showed gains on the AHA, and 4 of 8 participants showed gains in Box and Blocks (more affected hand). Nonlesioned CSP duration decreased in 6 of 6 participants with analyzable data. Cortical representation of the first dorsal interosseous expanded in the nonlesioned hemisphere in 4 of 6 participants and decreased in the lesioned hemisphere in 3 of 4 participants with analyzable data. Conclusions. While goal achievement was observed, objective measures of hand function showed inconsistent gains. Neurophysiologic data suggests nonlinear responses to cathodal stimulation of the nonlesioned hemisphere. Background & Aims: Insulin resistance is a risk marker for non‐alcoholic fatty liver disease, and a risk factor for liver disease progression. We assessed temporal trajectories of insulin resistance and β‐cell response to serum glucose concentration throughout adulthood and their association with diabetes risk in non‐alcoholic fatty liver disease. Methods: Three thousand and sixty participants from Coronary Artery Risk Development in Young Adults, a prospective bi‐racial cohort of adults age 18‐30 years at baseline (1985‐1986; Y0) who completed up to 5 exams over 25 years and had fasting insulin and glucose measurement were included. At Y25 (2010‐2011), non‐alcoholic fatty liver disease was assessed by noncontrast computed tomography after exclusion of other liver fat causes. Latent mixture modelling identified 25‐year trajectories in homeostatic model assessment insulin resistance and β‐cell response homeostatic model assessment‐β. Results: Three distinct trajectories were identified, separately, for homeostatic model assessment insulin resistance (low‐stable [47%]; moderate‐increasing [42%]; and high‐increasing [12%]) and homeostatic model assessment‐β (low‐decreasing [16%]; moderate‐decreasing [63%]; and high‐decreasing [21%]). Y25 non‐alcoholic fatty liver disease prevalence was 24.5%. Among non‐alcoholic fatty liver disease, high‐increasing homeostatic model assessment insulin resistance (referent: low‐stable) was associated with greater prevalent (OR 95% CI = 8.0, 2.0‐31.9) and incident (OR = 10.5, 2.6‐32.8) diabetes after multivariable adjustment including Y0 or Y25 homeostatic model assessment insulin resistance. In contrast, non‐alcoholic fatty liver disease participants with low‐decreasing homeostatic model assessment‐β (referent: high‐decreasing) had the highest odds of prevalent (OR = 14.1, 3.9‐50.9) and incident (OR = 10.3, 2.7‐39.3) diabetes. Conclusion: Trajectories of insulin resistance and β‐cell response during young and middle adulthood are robustly associated with diabetes risk in non‐alcoholic fatty liver disease. Thus, how persons with non‐alcoholic fatty liver disease develop resistance to insulin provides important information about risk of diabetes in midlife above and beyond degree of insulin resistance at the time of non‐alcoholic fatty liver disease assessment. Purpose: For many cancers, adolescents and young adults (AYA) have worse outcomes than for children and adults. Many factors may contribute to the AYA survival gap, including differences in biology, therapeutic intent, and adherence to therapy. It has been observed that male AYAs have poorer outcomes than females. The purpose of this work was to test the proposition that gender-related pharmacologic factors may account for a component of the AYA survival gap.Patients and Methods: A prospective, multi-institutional pharmacologic study of 79 patients in total with chemosensitive cancers (Ewing sarcoma, osteosarcoma and Hodgkin lymphoma) was conducted, with conventional doxorubicin treatment. Pharmacokinetic data of 13 children, 40 AYAs and 13 adults were valid for analysis. Population pharmacokinetics models were developed for doxorubicin and its metabolite doxorubicinol based on the data created in this study. Consequently, model-based analysis was conducted to investigate the relevant topics.Results: The clearance of doxorubicinol (normalized to body surface area), the main active metabolite of doxorubicin, appears faster in male AYAs than female (p = 0.04, 95% CI 0.1-3.9 L/h). The exposure of doxorubicinol (normalized to dose) is lower in male AYA than female (p = 0.03, 95% CI - 0.005 to - 0.0002 h/L). These might be correlated to the observed difference on nadir neutrophil count between male AYA and female (p = 0.027, 95% CI 0.09-1.4).Conclusion: Gender-related differences in doxorubicin pharmacology may account for worse outcomes for male AYAs with chemosensitive cancers compared to females. Abstract Objective To identify factors impacting overall survival (OS) in children, adolescents, and young adults with high-risk renal tumors (HRRTs). Methods The National Cancer Database was queried for patients ≤30 years old diagnosed with anaplastic Wilms tumors (AWT), clear cell sarcoma of the kidney (CCSK), or rhabdoid tumor of the kidney (RTK) between 2004 and 2013. Demographic, clinical, and OS data were abstracted. OS between groups was compared with a Kaplan-Meier curve. Univariate and multivariate survival analyses were performed. Results A total of 349 patients were identified meeting criteria; 133 (38.1%) AWT, 120 (34.4%) CCSK, and 96 (27.5%) RTK. Patients with RTK were less likely to undergo surgery than those with AWT or CCSK (77.1% vs 94% vs 99%, P <.001) and less likely to receive chemotherapy (84.4% vs 96.2% vs 95%, P =.013) or radiation (52.1% vs 81.2% vs 86.7%, P <.001). Estimated 5-year OS was 76.1% (95% confidence interval [CI] 67.9-84.4) for AWT, 92.7% (95% CI 87.4-97.9) for CCSK, and 33.5% (95% CI 23.1-43.9) for RTK (P <.001). On multivariate analysis, AWT (HR 3.372, P =.032) and RTK histology (HR 12.595, P <.001) were significantly associated with worse OS, while receiving radiation (HR 0.43, P =.006) was associated with improved OS. LN positivity, margin status, and undergoing surgery were not. Analyzing the HRRTs individually, for AWT, undergoing surgery was associated with OS (HR 0.308, P =.031). For RTK, factors associated with OS included undergoing surgery (HR 0.209, P =.007) and radiation (HR 0.411, P =.008). Conclusion Within the HRRTs, RTK is associated with worse outcomes than either AWT or CCSK. Overweight and obesity are major causes of worldwide morbidity and mortality. A two-armed randomized controlled trial (n = 104) examined the effectiveness of Hit n Run, a video game based on the principles of Go/No-Go inhibition training, in young adults who reported disinhibited eating. Adults (aged 18 to 30) were randomly assigned to play Hit n Run or received an informative brochure (Healthy Eating Step by Step; HESbS). Prior to and directly following the intervention week general and food-specific inhibitory control, caloric intake, and perceived attractiveness of food pictures were assessed. Results revealed no improvements in food-specific inhibitory control or caloric intake in either intervention group. Similar improvements for general inhibitory control and similar decreases in perceived attractiveness of food-related stimuli were observed for both Hit n Run and HESbS. Future research should aim to clarify how video game design can implement working mechanisms of cognitive training tasks to facilitate the development of effective game-based interventions. Objective The aim of this study was to verify the serum GDNF levels in individuals with anxiety disorder (AD) in a population-based study. Methods This was a cross-sectional study population-based, with people aged 18 to 35. AD's assessment was performed using the Mini International Neuropsychiatric Interview (M.I.N.I). Serum GDNF was measured by ELISA using a commercial kit. Results The prevalence was 3.3% for post-traumatic stress disorder, 6.7% for panic disorders, 17% generalized anxiety disorder, 5.1% for obsessive- compulsive disorder and 7.5% for social phobia. Serum GDNF levels was higher in individuals with panic disorders (p = 0.013), generalized anxiety (p = 0.035), obsessive- compulsive disorder (p = 0.005) and social phobia (p = 0.004), when compared to individuals without ADs. Only post traumatic stress disorder is not associated with serum GDNF levels (p = 0.119). Conclusion In this paper, we observed increased serum levels of GDNF in individuals with anxiety disorders, suggesting that this biomarker can be used as a putative marker for AD's. The knowledge of the physiological changes related to anxiety disorders can provide a better understanding of AD's pathogenesis, as well as, mechanisms involved in the progression of this condition.
