Fire can be catastrophic in the confined space of a hyperbaric chamber. From 1923 to 1996, 77 human fatalities occurred in 35 hyperbaric chamber fires, three human fatalities in a pressurized Apollo Command Module, and two human fatalities in three hypobaric chamber fires reported in Asia, Europe, and North America. Two fires occurred in diving bells, eight occurred in recompression (or decompression) chambers, and 25 occurred in clinical hyperbaric chambers. No fire fatalities were reported in the clinical hyperbaric chambers of North America. Chamber fires before 1980 were principally caused by electrical ignition. Since 1980, chamber fires have been primarily caused by prohibited sources of ignition that an occupant carried inside the chamber. Each fatal chamber fire has occurred in an enriched oxygen atmosphere (> 28% oxygen) and in the presence of abundant burnable material. Chambers pressurized with air (< 23.5% oxygen) had the only survivors. Information in this report was obtained from the literature and from the Undersea and Hyperbaric Medical Society's Chamber Experience and Mishap Database. This epidemiologic review focuses on information learned from critical analyses of chamber fires and how it can be applied to safe operation of hypobaric and hyperbaric chambers.

A simple manifold is described that enables accurate measurement of tidal volume and exhaled minute volume during high-frequency jet ventilation. The system was tested at zero end-expiratory pressure and during continuous positive airway pressure of up to 15 cm H2O, at jet drive pressures of up to 40 psig.

The flow-resistive characteristics of a variety of commercially available expiratory positive-pressure valve systems used to provide continuous positive airway pressure (CPAP) and positive end-expiratory pressure were evaluated. One flow-resistor and seven threshold-resistor expiratory pressure valve systems were set at 5, 10, 15, 20, and 25 cm H2O of expiratory pressure, and sinusoidal exhaled flows peaking at 50,100, and 200 L/min were directed through each valve at each level of expiratory pressure. The Siemens flow-resistor valve demonstrated the greatest deviation in pressure above set CPAP levels at peak flow rates of 100 and 200 L/min, which suggests high resistance to exhaled flow. The Vital Signs threshold-resistor valve demonstrated the least deviation in pressure from set CPAP levels at all rates of exhaled flow, which suggests low flow resistance. The Emerson and IMV Bird threshold-resistor systems resisted flow less than the BEAR-2 and the Puritan-Bennett MA-2 and 7200 inflatable-balloon threshold-resistor-like valve systems. These data suggest that threshold resistors may be classified as low-resistance or high-resistance types. Using only low-resistance threshold resistors for CPAP may minimize the incidence of barotrauma and other deleterious effects related to airway pressure.
