Abstract

We construct systematically experimental steady-state bifurcation diagrams for entorhinal cortex neurons. A slowly ramped voltage-clamp electrophysiology protocol serves as closed-loop feedback controlled experiment for the subsequent current-clamp open-loop protocol on the same cell. In this way, the voltage-clamped experiment determines dynamically stable and unstable (hidden) steady states of the current-clamp experiment. The transitions between observable steady states and observable spiking states in the current-clamp experiment reveal stability and bifurcations of the steady states, completing the steady-state bifurcation diagram.

Abstract

Synchronization is a widespread phenomenon in the brain. Despite numerous studies, the specific parameter configurations of the synaptic network structure and learning rules needed to achieve robust and enduring synchronization in neurons driven by spike-timing-dependent plasticity (STDP) and temporal networks subject to homeostatic structural plasticity (HSP) rules remain unclear. Here, we bridge this gap by determining the configurations required to achieve high and stable degrees of complete synchronization (CS) and phase synchronization (PS) in time-varying small-world and random neural networks driven by STDP and HSP. In particular, we found that decreasing $P$ (which enhances the strengthening effect of STDP on the average synaptic weight) and increasing $F$ (which speeds up the swapping rate of synapses between neurons) always lead to higher and more stable degrees of CS and PS in small-world and random networks, provided that the network parameters such as the synaptic time delay $\tau_c$, the average degree $\langle k \rangle$, and the rewiring probability $\beta$ have some appropriate values. When $\tau_c$, $\langle k \rangle$, and $\beta$ are not fixed at these appropriate values, the degree and stability of CS and PS may increase or decrease when $F$ increases, depending on the network topology. It is also found that the time delay $\tau_c$ can induce intermittent CS and PS whose occurrence is independent $F$. We increase the complexity of the underlying slow-fast system by considering a degree-four polynomial as parametrization of the critical manifold. Abstract

Abstract Introduction There is a sizable niche for a minimally invasive analgesic technique that could facilitate ambulatory video-assisted thoracoscopic surgery (VATS). Our study aimed to determine the analgesic potential of a single-shot erector spinae plane (ESP) block for VATS. The primary objective was the total hydromorphone consumption with patient-controlled analgesia (PCA) 24 h after surgery. Methods We conducted a randomized, controlled, double-blind study with patients scheduled for VATS in two major university-affiliated hospital centres. We randomized 52 patients into two groups: a single-shot ESP block using bupivacaine or an ESP block with normal saline (control). We administered a preoperative and postoperative (24 h) quality of recovery (QoR-15) questionnaire and assessed postoperative pain using a verbal numerical rating scale (VNRS) score. We evaluated the total standardized intraoperative fentanyl administration, total postoperative hydromorphone consumption (PCA; primary endpoint), and the incidence of adverse effects. Results There was no difference in the primary objective, hydromorphone consumption at 24 h (7.6 (4.4) mg for the Bupivacaine group versus 8.1 (4.2) mg for the Control group). Secondary objectives and incidence of adverse events were not different between the two groups at any time during the first 24 h following surgery. Conclusion Our multi-centre randomized, controlled, double-blinded study found no advantage of an ESP block over placebo for VATS for opioid consumption, pain, or QoR-15 scores. Further studies are ongoing to establish the benefits of using a denser block (single-shot paravertebral with a continuous ESP block), which may provide a better quality of analgesia.

10. Application of Machine Learning in Seismic Fragility Assessment of Bridges with SMA-Restrained Rocking Columns

Abstract

This paper evaluates the seismic fragility of a two-span reinforced concrete (RC) bridge with shape memory alloy (SMA)-restrained rocking (SRR) columns through machine learning (ML) techniques. SRR columns incorporate a combination of replaceable superelastic NiTi (SMA) links and mild steel energy-dissipating links to achieve self-centering and energy dissipation, respectively, while their rocking joints are protected against compressive concrete damage through steel jacketing. To produce seismic fragility functions, initially, multi-parameter probabilistic seismic demand models (PSDMs) are generated for various engineering demand parameters through five different ML techniques (including neural network) and considering various sources of uncertainty, and the most accurate PSDMs are selected. The selected PSDMs are then interpreted using four different methods to investigate the effects of two key SRR column design parameters (self-centering coefficient and SMA link initial strain) and ambient temperature on the seismic performance of SRR columns. Subsequently, using neural networks, the PSDMs developed earlier, and appropriate capacity models, multi-parameter fragility functions are developed for various bridge damage states. After examining the effects of the two SRR column design parameters on the seismic fragility of the bridge, its seismic fragility is compared with those of the same bridge with monolithic RC and posttensioned (PT) rocking columns. It is shown that, in general, increasing the initial strain of the SMA links and decreasing the self-centering coefficient as possible (i.e., without compromising the self-centering) reduce the overall bridge damage. In addition, even considering the ambient temperature's uncertainty, SRR columns are proven, at least, as effective as PT columns in mitigating the seismic damage of the bridges of monolithic RC columns.

Summary: Heat shock (HS) coincides with the assembly of translationally arrested heat shock messenger ribonucleoprotein particles (HS-mRNPs) and condensates. Here, we present a protocol to reconstitute HS-mRNPs and HS condensates with eIF4G, eIF4E, Pab1p, and mRNA in vitro. In addition, we describe the necessary steps to measure the effect of HS-mRNPs and HS condensates on translation in yeast extracts. Abstract

Silicones such Sylgard 184 are widely employed in biological applications due to their versatile properties. However, their inherently adhesive surfaces can restrict their application, especially in direct contact with damaged biological tissues, potentially compromising patient comfort. To enhance the surface properties of Sylgard 184 while maintaining its transparency in the visible spectrum, a novel low-temperature method (70 °C) has been developed. This method involves immersing PDMS in a solution of titanium (IV) ethoxide in THF, thus inducing swelling of the silicone's polymer network, followed by the diffusion and condensation of titanium (IV) ethoxide within the polymer matrix. The resulting hybrid material, incorporating amorphous titanium oxide within the silicone network, exhibits significantly increased surface hardness compared to unmodified Sylgard 184, while retaining transparency and improving biological behaviour. The elaborated method holds promising potential for enhancing the performance of silicone-based materials in diverse biomedical applications.

13. Promoting Resilience in Direct Support Professionals of Adults with Intellectual and Developmental Disabilities: A Qualitative Descriptive Study

Abstract

The direct support professional (DSP) workforce shortage crisis has reached untenable levels during the COVID-19 pandemic as DSPs rapidly exit the workforce. To gain a better understanding of factors that contribute to DSP resilience during stressful and challenging times, we interviewed 10 DSPs identified by colleagues as resilient to elicit strategies to promote DSP resilience. Our content analysis revealed nine distinct strategies: (1) communication; (2) self-worth and recognition; (3) authentic, equitable relationships; (4) embracing change and learning; (5) establishing and maintaining boundaries; (6) cultivating an intentional mindset; (7) self-care; (8) spirituality/"the bigger picture"; and (9) a daily practice of humor and fun.

17. Large-scale phosphoproteomics reveals activation of the MAPK/GADD45β/P38 axis and cell cycle inhibition in response to BMP9 and BMP10 stimulation in endothelial cells

Abstract

Abstract Background BMP9 and BMP10 are two major regulators of vascular homeostasis. These two ligands bind with high affinity to the endothelial type I kinase receptor ALK1, together with a type II receptor, leading to the direct phosphorylation of the SMAD transcription factors. Apart from this canonical pathway, little is known. Interestingly, mutations in this signaling pathway have been identified in two rare cardiovascular diseases, hereditary hemorrhagic telangiectasia and pulmonary arterial hypertension. Methods To get an overview of the signaling pathways modulated by BMP9 and BMP10 stimulation in endothelial cells, we employed an unbiased phosphoproteomic-based strategy. Identified phosphosites were validated by western blot analysis and regulated targets by RT-qPCR. Cell cycle analysis was analyzed by flow cytometry. Results Large-scale phosphoproteomics revealed that BMP9 and BMP10 treatment induced a very similar phosphoproteomic profile. These BMPs activated a non-canonical transcriptional SMAD-dependent MAPK pathway (MEKK4/P38). We were able to validate this signaling pathway and demonstrated that this activation required the expression of the protein GADD45β. In turn, activated P38 phosphorylated the heat shock protein HSP27 and the endocytosis protein Eps15 (EGF receptor pathway substrate), and regulated the expression of specific genes (E-selectin, hyaluronan synthase 2 and cyclooxygenase 2). This study also highlighted the modulation in phosphorylation of proteins involved in transcriptional regulation (phosphorylation of the endothelial transcription factor ERG) and cell cycle inhibition (CDK4/6 pathway). Accordingly, we found that BMP10 induced a G1 cell cycle arrest and inhibited the mRNA expression of E2F2, cyclinD1 and cyclinA1. Conclusions Overall, our phosphoproteomic screen identified numerous proteins whose phosphorylation state is impacted by BMP9 and BMP10 treatment, paving the way for a better understanding of the molecular mechanisms regulated by BMP signaling in vascular diseases.

Abstract

The Nordic countries are, together with the United States, forerunners in online record access (ORA), which has now become widespread. The importance of accessible and structured health data has also been highlighted by policy makers internationally. To ensure the full realization of ORA’s potential in the short and long term, there is a pressing need to study ORA from a cross-disciplinary, clinical, humanistic, and social sciences perspective that looks beyond strictly technical aspects. In this viewpoint paper, we explore the policy changes in the European Health Data Space (EHDS) proposal to advance ORA across the European Union, informed by our research in a Nordic-led project that carries out the first of its kind, large-scale international investigation of patients’ ORA—NORDeHEALTH (Nordic eHealth for Patients: Benchmarking and Developing for the Future). We argue that the EHDS proposal will pave the way for patients to access and control third-party access to their electronic health records. In our analysis of the proposal, we have identified five key principles for ORA: (1) the right to access, (2) proxy access, (3) patient input of their own data, (4) error and omission rectification, and (5) access control. ORA implementation today is fragmented throughout Europe, and the EHDS proposal aims to ensure all European citizens have equal online access to their health data. However, we argue that in order to implement the EHDS, we need more research evidence on the key ORA principles we have identified in our analysis. Results from the NORDeHEALTH project provide some of that evidence, but we have also identified important knowledge gaps that still need further exploration.

Abstract

The US health care delivery system does not systematically engage or support family or friend care partners. Meanwhile, the uptake and familiarity of portals to personal health information are increasing among patients. Technology innovations, such as shared access to the portal, use separate identity credentials to differentiate between patients and care partners. Although not well-known, or commonly used, shared access allows patients to identify who they do and do not want to be involved in their care. However, the processes for patients to grant shared access to portals are often limited or so onerous that interested patients and care partners often circumvent the process entirely. As a result, the vast majority of care partners resort to accessing portals using a patient’s identity credentials—a “do-it-yourself” solution in conflict with a health systems’ legal responsibility to protect patient privacy and autonomy. The personal narratives in this viewpoint (shared by permission) elaborate on quantitative studies and provide first-person snapshots of challenges faced by patients and families as they attempt to gain or grant shared access during crucial moments in their lives. As digital modalities increase patient roles in health care interactions, so does the importance of making shared access work for all stakeholders involved—patients, clinicians, and care partners. Electronic health record vendors must recognize that both patients and care partners are important users of their products, and health care organizations must acknowledge and support the critical contributions of care partners as distinct from patients.

Abstract

Abstract Introduction Consumer‐oriented health information technologies (CHIT) such as the patient portal have a growing role in care delivery redesign initiatives such as the Learning Health System. Care partners commonly navigate CHIT demands alongside persons with complex health and social needs, but their role is not well specified. Methods We assemble evidence and concepts from the literature describing interpersonal communication, relational coordination theory, and systems‐thinking to develop an integrative framework describing the care partner's role in applied CHIT innovations. Our framework describes pathways through which systematic engagement of the care partner affects longitudinal work processes and multi‐level outcomes relevant to Learning Health Systems. Results Our framework is grounded in relational coordination, an emerging theory for understanding the dynamics of coordinating work that emphasizes role‐based relationships and communication, and the Systems Engineering Initiative for Patient Safety (SEIPS) model. Cross‐cutting work systems geared toward explicit and purposeful support of the care partner role through CHIT may advance work processes by promoting frequent, timely, accurate, problem‐solving communication, reinforced by shared goals, shared knowledge, and mutual respect between patients, care partners, and care team. We further contend that systematic engagement of the care partner in longitudinal work processes exerts beneficial effects on care delivery experiences and efficiencies at both individual and organizational levels. We discuss the utility of our framework through the lens of an illustrative case study involving patient portal‐mediated pre‐visit agenda setting. Conclusions Our framework can be used to guide applied embedded CHIT interventions that support the care partner role and bring value to Learning Health Systems through advancing digital health equity, improving user experiences, and driving efficiencies through improved coordination within complex work systems.

Abstract Context Engaging with peers is gaining increasing interest from healthcare systems in numerous countries. Peers are people who offer support by drawing on lived experiences of significant challenges or ‘insider’ knowledge of communities. Growing evidence suggests that peers can serve as a bridge between underserved communities and care providers across sectors, through their ability to build trust and relationships. Peer support is thus seen as an innovative way to address core issues of formal healthcare, particularly fragmentation of care and health inequalities. The wide body of approaches, goals and models of peer support speaks volumes of such interest. Navigating the various labels used to name peers, however, can be daunting. Similar terms often hide critical differences. Objectives/Background This article seeks to disentangle the conceptual multiplicity of peer support, presenting a conceptual map based on a 3‐year knowledge synthesis project involving peers and programme stakeholders in Canada, and international scientific and grey literature. Synthesis/Main Results The map introduces six key questions to navigate and situate peer support approaches according to peers' roles, pathways and settings of practice, regardless of the terms used to label them. As a tool, it offers a broad overview of the different ways peers contribute to integrating health and community care. Discussion We conclude by discussing the map's potential and limitations to establish a common language and bridge models, in support of knowledge exchange among practitioners, policymakers and researchers. Patient or Public Contribution Our team includes one experienced peer support worker. She contributed to the design of the conceptual map and the production of the manuscript. More than 10 peers working across Canada were also involved during research meetings to validate and refine the conceptual map.

22. Cross-scale excitability in networks of synaptically-coupled quadratic integrate-and-fire neurons

Abstract

From the action potentials of neurons and cardiac cells to the amplification of calcium signals in oocytes, excitability is a hallmark of many biological signalling processes. In recent years, excitability in single cells has been related to multiple-timescale dynamics through canards, special solutions which determine the effective thresholds of the all-or-none responses. However, the emergence of excitability in large populations remains an open problem. Here, we show that the mechanism of excitability in large networks and mean-field descriptions of coupled quadratic integrate-and-fire (QIF) cells mirrors that of the individual components. We initially exploit the Ott-Antonsen ansatz to derive low-dimensional dynamics for the coupled network and use it to describe the structure of canards via slow periodic forcing. We demonstrate that the thresholds for onset and offset of population firing can be found in the same way as those of the single cell. We combine theoretical analysis and numerical computations to develop a novel and comprehensive framework for excitability in large populations, applicable not only to models amenable to Ott-Antonsen reduction, but also to networks without a closed-form mean-field limit, in particular sparse networks.

27. Slow passage through a Hopf-like bifurcation in piecewise linear systems: application to elliptic bursting

Abstract

The phenomenon of slow passage through a Hopf bifurcation is ubiquitous in multiple-timescale dynamical systems, where a slowly-varying quantity replacing a static parameter induces the solutions of the resulting slow-fast system to feel the effect of a Hopf bifurcation with a delay. This phenomenon is well understood in the context of smooth slow-fast dynamical systems. In the present work, we study for the first time this phenomenon in piecewise linear (PWL) slow-fast systems. This special class of systems is indeed known to reproduce all features of their smooth counterpart while being more amenable to quantitative analysis and offering some level of simplification, in particular through the existence of canonical (linear) slow manifolds. We provide conditions for a PWL slow-fast system to exhibit a slow passage through a Hopf-like bifurcation, in link with the number of linearity zones considered in the system and possible connections between canonical attracting and repelling slow manifolds. In doing so, we fully describe the so-called way-in/way-out function. Finally, we investigate this slow passage effect in the Doi-Kumagai model, a neuronal PWL model exhibiting elliptic bursting oscillations.

28. Bursting in a next generation neural mass model with synaptic dynamics: a slow-fast approach

Abstract

We report a detailed analysis on the emergence of bursting in a recently developed neural mass model that takes short-term synaptic plasticity into account. The one being used here is particularly important, as it represents an exact meanfield limit of synaptically coupled quadratic integrate & fire neurons, a canonical model for type I excitability. In absence of synaptic dynamics, a periodic external current with a slow frequency {\epsilon} can lead to burst-like dynamics. The firing patterns can be understood using techniques of singular perturbation theory, specifically slow-fast dissection. In the model with synaptic dynamics the separation of timescales leads to a variety of slow-fast phenomena and their role for bursting is rendered inordinately more intricate. Canards are one of the main slow-fast elements on the route to bursting. They describe trajectories evolving nearby otherwise repelling locally invariant sets of the system and are found in the transition region from subthreshold dynamics to bursting. For values of the timescale separation nearby the singular limit {\epsilon} = 0, we report peculiar jump-on canards, which block a continuous transition to bursting. In the biologically more plausible regime of {\epsilon} this transition becomes continuous and bursts emerge via consecutive spike-adding transitions. The onset of bursting is of complex nature and involves mixed-type like torus canards, which form the very first spikes of the burst and revolve nearby fast-subsystem repelling limit cycles. We provide numerical evidence for the same mechanisms to be responsible for the emergence of bursting in the quadratic integrate & fire network with plastic synapses. The main conclusions apply for the network, owing to the exactness of the meanfield limit.

Abstract

BackgroundTherapeutic education and patient self-management are crucial in diabetes prevention and treatment. Improving diabetes self-management requires multidisciplinary team intervention, nutrition education that facilitates self-management, informed decision-making, and the organization and delivery of appropriate health care services. The emergence of telehealth services has provided the public with various tools for educating themselves and for evaluating, monitoring, and improving their health and nutrition-related behaviors. Combining health technologies with clinical expertise, social support, and health professional involvement could help persons living with diabetes improve their disease self-management skills and prevent its long-term consequences. ObjectiveThis scoping review’s primary objective was to identify the key digital tool features of complex telehealth interventions used for type 2 diabetes or prediabetes self-management and monitoring with health professional involvement that help improve health outcomes. A secondary objective was to identify how these key features are developed and combined. MethodsA 5-step scoping review methodology was used to map relevant literature published between January 1, 2010 and March 31, 2022. Electronic searches were performed in the MEDLINE, CINAHL, and Embase databases. The searches were limited to scientific publications in English and French that either described the conceptual development of a complex telehealth intervention that combined self-management and monitoring with health professional involvement or evaluated its effects on the therapeutic management of patients with type 2 diabetes or prediabetes. Three reviewers independently identified the articles and extracted the data. ResultsThe results of 42 studies on complex telehealth interventions combining diabetes self-management and monitoring with the involvement of at least 1 health professional were synthesized. The health professionals participating in these studies were physicians, dietitians, nurses, and psychologists. The digital tools involved were smartphone apps or web-based interfaces that could be used with medical devices. We classified the features of these technologies into eight categories, depending on the intervention objective: (1) monitoring of glycemia levels, (2) physical activity monitoring, (3) medication monitoring, (4) diet monitoring, (5) therapeutic education, (6) health professional support, (7) other health data monitoring, and (8) health care management. The patient-logged data revealed behavior patterns that should be modified to improve health outcomes. These technologies, used with health professional involvement, patient self-management, and therapeutic education, translate into better control of glycemia levels and the adoption of healthier lifestyles. Likewise, they seem to improve monitoring by health professionals and foster multidisciplinary collaboration through data sharing and the development of more concise automatically generated reports. ConclusionsThis scoping review synthesizes multiple studies that describe the development and evaluation of complex telehealth interventions used in combination with health professional support. It suggests that combining different digital tools that incorporate diabetes self-management and monitoring features with a health professional’s advice and interaction results in more effective interventions and outcomes.

Ao modo de uma reconstrução, examinaremos algumas figuras que caracterizam de modo exemplar esta abordagem. A guisa de introdução do assunto, buscaremos primeiro, do lado dos mestres do Ricoeur, Gabriel Marcel e Karl Jaspers, as fontes possíveis de uma via longa na sua respectiva filosofia. Examinaremos em seguida quatro figuras que constituem meditações hermenêuticas: o desvio da consciência pelos saberes históricos, o padrão hermenêutico, a inserção do problema hermenêutico na fenomenologia e a dialética entre explicar e compreender. No fim do percurso, notaremos que a via longa da compreensão recobre diversas acepções, bem como se estende a todo projeto filosófico de Ricoeur.

37. Experiences and opinions of general practitioners with patient online record access: an online survey in England

Objective To describe the experiences and opinions of general practitioners (GPs) in England regarding patients having access to their full online GP health records.Design Convenience sample, online survey.Participants 400 registered GPs in England.Main outcome measures Investigators measured GPs’ experiences and opinions about online record access (ORA), including patient care and their practice.Results A total of 400 GPs from all regions of England responded. A minority (130, 33%) believed ORA was a good idea. Most GPs believed a majority of patients would worry more (364, 91%) or find their GP records more confusing than helpful (338, 85%). Most GPs believed a majority of patients would find significant errors in their records (240, 60%), would better remember their care plan (280, 70%) and feel more in control of their care (243, 60%). The majority believed they will/already spend more time addressing patients’ questions outside of consultations (357, 89%), that consultations will/already take significantly longer (322, 81%) and that they will be/already are less candid in their documentation (289, 72%) after ORA. Nearly two-thirds of GPs believed ORA would increase their litigation (246, 62%).Conclusions Similar to clinicians in other countries, GPs in our sample were sceptical of ORA, believing patients would worry more and find their records more confusing than helpful. Most GPs also believed the practice would exacerbate work burdens. However, the majority of GPs in this survey also agreed there were multiple benefits to patients having online access to their primary care health records. The findings of this survey also contribute to a growing body of contrastive research from countries where ORA is advanced, demonstrating clinicians are sceptical while studies indicate patients appear to derive multiple benefits.

BackgroundHealthy eating during pregnancy has favorable effects on glycemic control and is associated with a lower risk of gestational diabetes mellitus (GDM). According to Diabetes Canada, there is a need for an effective and acceptable intervention that could improve glucose homeostasis and support pregnant individuals at risk for GDM.AimsThis unicentric randomized controlled trial (RCT) aims to evaluate the effects of a nutritional intervention initiated early in pregnancy, on glucose homeostasis in 150 pregnant individuals at risk for GDM, compared to usual care.MethodsPopulation: 150 pregnant individuals ≥18 years old, at ≤14 weeks of pregnancy, and presenting ≥1 risk factor for GDM according to Diabetes Canada guidelines. Intervention: The nutritional intervention initiated in the first trimester is based on the health behavior change theory during pregnancy and on Canada’s Food Guide recommendations. It includes (1) four individual counseling sessions with a registered dietitian using motivational interviewing (12, 18, 24, and 30 weeks), with post-interview phone call follow-ups, aiming to develop and achieve S.M.A.R.T. nutritional objectives (specific, measurable, attainable, relevant, and time-bound); (2) 10 informative video clips on healthy eating during pregnancy developed by our team and based on national guidelines, and (3) a virtual support community via a Facebook group. Control: Usual prenatal care. Protocol: This RCT includes three on-site visits (10–14, 24–26, and 34–36 weeks) during which a 2-h oral glucose tolerance test is done and blood samples are taken. At each trimester and 3 months postpartum, participants complete web-based questionnaires, including three validated 24-h dietary recalls to assess their diet quality using the Healthy Eating Food Index 2019. Primary outcome: Difference in the change in fasting blood glucose (from the first to the third trimester) between groups. Abstract

The actin cytoskeleton plays a pivotal role in a broad range of physiological processes including directing cell shape and subcellular organization, determining cell mechanical properties, and sensing and transducing mechanical forces. The versatility of the actin cytoskeleton arises from the ability of actin filaments to assemble into higher order structures through their interaction with a vast set of regulatory proteins. Actin filaments assemble into bundles, meshes, and networks, where different combinations of these structures fulfill specific functional roles. Analyzing the organization and abundance of different actin structures from optical microscopy data provides a valuable metric for assessing cell physiological function and changes associated with disease. However, quantitative measurements of the size, abundance, orientation, and distribution of different types of actin structure remains challenging both from an experimental and image analysis perspective. In this review, we summarize image analysis methods for extracting quantitative values that can be used for characterizing the organization of actin structures and provide selected examples. We summarize the potential sample types and metric reported with different approaches as a guide for selecting an image analysis strategy.

Abstract

Ladder operators for the hyperbolic Rosen-Morse (RMII) potential are realized using the shape invariance property appearing, in particular, using supersymmetric quantum mechanics. The extension of the ladder operators to a specific class of rational extensions of the RMII potential is presented and discussed. Coherent states are then constructed as almost eigenstates of the lowering operators. Some properties are analyzed and compared. The ladder operators and coherent states constructions presented are extended to the case of the trigonometric Rosen-Morse (RMI) potential using a point canonical transformation.

Abstract

Cancers of the head and neck and their treatment can cause disfigurement and loss of functioning, with a profound negative impact on the person’s self-image and psychosocial wellbeing. This can lead to experiences of shame and stigma, which are important targets for psychosocial interventions. Accurate measurement and identification of these problems enables clinicians to offer appropriate interventions and monitor patients’ progress. This study aimed to validate the Canadian version of the Shame and Stigma Scale (SSS) among French- and English-speaking head and neck cancer patients. Data from 254 patients from two major Canadian hospitals were analysed. The existing four-factor structure of the SSS was supported, with the following subscales: Shame with Appearance, Sense of Stigma, Regret, and Social/Speech Concerns. The Canadian SSS showed adequate convergent and divergent validity and test–retest reliability. Rasch analysis suggested scale improvement by removing two misfitting items and two items with differential functioning between French- and English-speaking patients. The final 16-item scale version was an adequate fit with the Rasch model. The SSS provides more accurate measures for people with high levels of shame and stigma, and thus has utility in identifying patients with more severe symptoms who may be in need of psychosocial interventions.

Abstract

We study a class of planar integrate and fire (IF) models called adaptive integrate and fire (AIF) models, which possesses an adaptation variable on top of membrane potential, and whose subthreshold dynamics is piecewise linear (PWL). These AIF models therefore have two reset conditions, which enable bursting dynamics to emerge for suitable parameter values. Such models can be thought of as hybrid dynamical systems. We consider a particular slow dynamics within AIF models and prove the existence of bursting cycles with $N$ resets, for any integer $N$. Furthermore, we study the transition between $N$- and $(N+1)$-reset cycles upon vanishingly small parameter variations and prove (for $N=2$) that such transitions are organised by canard cycles. Finally, using numerical continuation we compute branches of bursting cycles, including canard-explosive branches, in these AIF models, by suitably recasting the periodic problem as a two-point boundary-value problem.

47. Characteristics and Energy Use of Volume Servers in the United States

Abstract

Servers’ field energy use remains poorly understood, given heterogeneous computing loads, configurable hardware and software, and operation over a wide range of management practices. This paper explores various characteristics of 1- and 2-socket volume servers that affect energy consumption, and quantifies the difference in power demand between higher-performing SPEC and ENERGY STAR servers and our best understanding of a typical server operating today. We first establish general characteristics of the U.S. installed base of volume servers from existing IDC data and the literature, before presenting information on server hardware configurations from data collection events at a major online retail website. We then compare cumulative distribution functions of server idle power across three separate datasets and explain the differences between them via examination of the hardware characteristics to which power draw is mostsensitive. We find that idle server power demand is significantly higher than ENERGY STAR benchmarks and the industry-released energy use documented in SPEC, and that SPEC server configurations—and likely theassociated power-scaling trends—are atypical of volume servers. Next, we examine recent trends in server power draw among high-performing servers across their full load range to consider how representative these trends are of all volume servers before inputting weighted average idle power load values into a recently published model of national server energy use. Finally, we present results from two surveys of IT managers (n=216) and IT vendors (n=178) that illustrate the prevalence of more-efficient equipment and operational practices in server rooms and closets; these findings highlight opportunities to improve the energy efficiency of the U.S. server stock.

48. Canonical models for torus canards in elliptic bursters

Abstract

We revisit elliptic bursting dynamics from the viewpoint of torus canard solutions. We show that at the transition to and from elliptic burstings, classical or mixed-type torus canards can appear, the difference between the two being the fast subsystem bifurcation that they approach, saddle-node of cycles for the former and subcritical Hopf for the latter. We first showcase such dynamics in a Wilson-Cowan type elliptic bursting model, then we consider minimal models for elliptic bursters in view of finding transitions to and from bursting solutions via both kinds of torus canards. We first consider the canonical model proposed by Izhikevich (ref. [22] in the manuscript) and adapted to elliptic bursting by Ju, Neiman, Shilnikov (ref. [24] in the manuscript), and we show that it does not produce mixed-type torus canards due to a nongeneric transition at one end of the bursting regime. We therefore introduce a perturbative term in the slow equation, which extends this canonical form to a new one that we call Leidenator and which supports the right transitions to and from elliptic bursting via classical and mixed-type torus canards, respectively. Throughout the study, we use singular flows ($\varepsilon=0$) to predict the full system's dynamics ($\varepsilon>0$ small enough). We consider three singular flows: slow, fast and average slow, so as to appropriately construct singular orbits corresponding to all relevant dynamics pertaining to elliptic bursting and torus canards.

Abstract

BACKGROUND: Sharing outpatient notes with patients may bring clinically important benefits, but notes may sometimes cause patients to feel judged or offended, and thereby reduce trust. OBJECTIVE: As part of a larger survey examining the effects of open notes, we sought to understand how many patients feel judged or offended due to something they read in outpatient notes, and why. DESIGN: We analyzed responses from a large Internet survey of adult patients who used secure patient portals and had at least 1 visit note available in a 12-month period at 2 large academic medical systems in Boston and Seattle, and in a rural integrated health system in Pennsylvania. PARTICIPANTS: Adult ambulatory patients with portal accounts in health systems that offered open notes for up to 7 years. APPROACH: (1) Quantitative analysis of 2 dichotomous questions, and (2) qualitative thematic analysis of free-text responses on what patients found judgmental or offensive. KEY RESULTS: Among 22,959 patient respondents who had read at least one note and answered the 2 questions, 2,411 (10.5%) reported feeling judged and/or offended by something they read in their note(s). Patients who reported poor health, unemployment, or inability to work were more likely to feel judged or offended. Among the 2,411 patients who felt judged and/or offended, 2,137 (84.5%) wrote about what prompted their feelings. Three thematic domains emerged: (1) errors and surprises, (2) labeling, and (3) disrespect. CONCLUSIONS: One in 10 respondents reported feeling judged/offended by something they read in an outpatient note due to the perception that it contained errors, surprises, labeling, or evidence of disrespect. The content and tone may be particularly important to patients in poor health. Enhanced clinician awareness of the patient perspective may promote an improved medical lexicon, reduce the transmission of bias to other clinicians, and reinforce healing relationships.

Abstract

Abstract Background Behavioural sciences have been shown to support the development of more effective interventions aimed at promoting healthy lifestyles. However, the operationalization of this knowledge seems to be sub-optimal in public health. Effective knowledge transfer strategies are thus needed to optimize the use of knowledge from behavioural sciences in this field. To this end, the present study examined public health practitioners’ perceptions and use of theories and frameworks from behavioural sciences to design health promotion interventions. Methods This study adopted an exploratory qualitative design. Semi-structured interviews were conducted among 27 public health practitioners from across Canada to explore current intervention development processes, the extent to which they integrate theory and framework from behavioural sciences, and their perceptions regarding the use of this knowledge to inform intervention design. Practitioners from the public sector or non-profit/private organizations who were involved in the development of interventions aimed at promoting physical activity, healthy eating, or other healthy lifestyle habits (e.g., not smoking) were eligible to participate. Results Public health practitioners generally agreed that behaviour change is an important goal of public health interventions. On the other hand, behavioural science theories and frameworks did not appear to be fully integrated in the design of public health interventions. The main reasons were (1) a perceived lack of fit with current professional roles and tasks; (2) a greater reliance on experiential-produced knowledge rather than academic knowledge (mainly for tailoring interventions to local setting characteristics); (3) the presence of a fragmented knowledge base; (4) the belief that theories and frameworks require too much time and resources to be operationalized; and 4) the belief that using behavioural sciences might undermine partnership building. Conclusions This study provided valuable insights that may inform knowledge transfer strategies that could be optimally designed to support the integration of behavioural sciences theories and frameworks into public health practices.
