Abstract

Immunotherapy combined with chemotherapy regimen has been shown to be effective in recurrent or metastatic (R/M) head and neck squamous cell carcinoma (HNSCC). However, due to the small number of patients, its efficacy remains controversial in Asian populations, particularly in mainland China. Here a randomized, double-blind phase 3 trial evaluated the efficacy and safety of finotonlimab (SCT-I10A), a programmed cell death 1 (PD-1) monoclonal antibody, combined with cisplatin plus 5-fluorouracil (C5F) for the first-line treatment of R/M HNSCC. Eligible patients (n= 370) were randomly 2:1 assigned to receive finotonlimab plus C5F (n= 247) or placebo plus C5F (n= 123). The primary endpoint was overall survival (OS). In the finotonlimab plus C5F group, OS was 14.1 months (95% confidence interval (CI) 11.1–16.4), compared with 10.5 months (95% CI 8.1–11.8) in the placebo plus C5F group. The hazard ratio was 0.73 (95% CI 0.57–0.95, P= 0.0165), meeting the predefined superiority criteria for the primary endpoint. Finotonlimab plus C5F showed significant OS superiority compared with C5F alone and acceptable safety profile with R/M HNSCC, supporting its use as a first-line treatment option for R/M HNSCC. These results validate the efficacy and safety of the combination of finotonlimab and C5F in Asian patients with R/M HNSCC. ClinicalTrials.gov identifier: NCT04146402.

Abstract

Aims: Embedded system plays a vital role in today’s life. Hence, our interest is in areadelay- energy efficient embedded system design in post-CMOS technology, i.e., QCA. Objectives: The research is focused on efficient area-delay-energy Configurable Logic Block (CLB) design for Field-Programmable Gate Array architecture (FPGA) with successful simulation-based on next-generation technology, Quantum-dot cellular automata. Methods: Each proposed circuit is designed on post CMOS 4 dot 2 electron technology, i.e. QCA (Quantum dot Cellular Automata), is adopted in circuit implementation due to low power dissipation, high clock frequency and high package density. QCADesigner is used to verify the functionality of every circuit. QCAPro tool is used for determining power dissipation. Results: In contrast, a new approach of using de-multiplexer replacing the decoder has been introduced that results in the reduction of the average energy dissipation by almost 57%. A NOR based D flip-flop memory architecture and multiplexer are also used in the lookup table for the configurable logic block. The proposed architecture thus reduces the overall latency. The proposed CLB consists of 6356 QCA cells covering 7.44 um2 area. Write and read latency of proposed CLB are 12 and 7.25 QCA clock, respectively. Conclusion: The present paper concludes that read and write latency reduction occurs; average energy dissipation, leakage, and switching energy dissipation are reduced in a large amount resulting in an advantage of the overall minimization of the latency for the proposed CLB in the process.

Abstract

Lymphatic vascular invasion (LVI) is a poor prognostic factor for hypopharyngeal squamous cell carcinoma (HPSCC), but the risk factors of LVI and its relationship with clinicopathological of HPSCC remain unclear. This study aims to explore these issues. We retrospectively analyzed the clinicopathological data of 170 patients with HPSCC from January 2011 to December 2015. The relationship between LVI and clinicopathologic was analyzed by Chi-square test or Fisher’s exact test. The risk factors of LVI were examined using a logistic regression model, while risk factors of survival rate were carried out using the Cox regression model. LVI occurred in 59 cases (34.7%). In multivariate analysis, T3–4 stage (HR = 2.877; 95% CI: 1.379–6.004; p = 0.005), N2–3 stage (HR = 2.325; 95% CI: 1.120–4.824; p = 0.024), and poor differentiation (HR = 2.983; 95% CI: 1.229–7.242; p = 0.016) were independent risk factors for LVI; positive LVI was an independent risk factor for local recurrence (HR = 2.488; 95% CI: 1.150–5.383; p = 0.021), poor 5-year OS (HR = 0.375; 95% CI: 0.232–0.606; p

Abstract

On May 5, 2014, an earthquake with a magnitude of Mw 6.1 (the largest earthquake in Thailand so far) occurred in Chiang Rai of the Golden Triangle area in northern Thailand. We had an opportunity to conduct field survey immediately after the earthquake. Serious damage to buildings and casualties of lives were observed, and the estimated Maximum Mercalli Intensity (MMI) of the earthquake is VIII (evaluated according to the MMI scale of the Chinese Standard). No long continuous ground ruptures were produced during the earthquake, but in the epicenter (commonly within MMI VIII extent), massive small linear ruptures (usually several tens of meters long) developed and displayed intriguing structural features, offsetting many roads several centimeters left laterally on NE trending cracks or offsetting right laterally on NW trending ones. The focal mechanism solution of earthquake shows that this is a pure strike‐slip event, and two nodal planes in NW and NE directions had the same motion senses respectively as those of breakage associated with the earthquake. The long axis of the isoseismals and aftershock distributions are in NE direction, which is consistent with the strike of Luang Namtha fault. The 230‐km‐long Luang Namtha fault which starts from the border of China and Laos, runs through northern Laos, and terminates at Chiang Rai of Thailand is predominated by left‐lateral strike‐slip and active in late Quaternary, and two earthquakes over Ms 6.0 occurred along the fault in 1925 and 2007 respectively. This Mw 6.1 earthquake occurred at the southwestern end of the fault. All related features such as evident structural rupturing, elongated orientation of MMI and aftershock distribution, as well as the location of the epicenter, suggest that the Luang Namtha fault may be responsible for the 2014 Northern Thailand earthquake.
