Sandwich panels are commonly used as structure, based on fiber reinforced composites, with the goal of high flexural stiffness and low mass. It is most common to separate two high performance composite facesheets with a low-density core, generally in the form of a foam or honeycomb. A recent concept has been to replace these traditional core materials with fiber reinforced truss-like structures, with the goal of further reducing mass. A system is described that can radically reduce the amount of tooling required for truss core sandwich panel manufacture. This system, which is a digital manufacturing platform for the extrusion of continuous fiber reinforced commingled glass fiber/PET tow, was developed to demonstrate the rigidization of composites both on, and off, a tool surface. Navtruss core panels were successfully manufactured using this digital manufacturing platform, without conventional tooling, and the resulting through thickness compression moduli and panel shear moduli were within 14.6% and 23% of the values baseline compression molded specimens. Thus, the results suggest that, with further development, complex truss core structures with performance approaching that of compression molded panels can be manufactured with radically reduced tooling requirements from high volume fraction, continuous fiber reinforced thermoplastic matrix composites.

The paper introduces the main concepts and criteria presented in the Guidelines on the Analysis, Conservation and Structural Restoration elaborated by the ICOMOS/ISCARSAH committee. The guidelines have been developed with the aim to provide guidance to experts and practitioners working in the study and conservation of structures or the architectural heritage. The guidelines are based on the ICOMOS Charter on the Principles for he Analysis, Conservation and Structural Restoration of Architectural Heritage elaborated by ISCARSAH and adopted by ICOMOS in 2003. The guidelines are intended to assist the multidisciplinary teams involved in the management of historical structures across all the stages of the study and operation, including the investigation and documentation works, the structural verification and the selection and design of appropriate minimum interventions.

During the development of digitally manufactured, commingled tow continuous fiber reinforced composites, consolidation force was controlled using a controlled spring force that yielded a repeatable tow width. However, the use of the extruder face to consolidate the material requires that the extruder remain perpendicular to the placement surface throughout the process. When considering more complex tool contours including sloped surfaces, more than three axes of motion are necessary to maintain the perpendicularity of the extruder tip to the surface. In this effort, a five-axis system is developed and used to demonstrate the ability to consolidate over complex contours. In addition, the nozzle face temperatures required for good consolidation and wetout result in poor tow path fidelity when complex paths are introduced. The implementation of an automated, computer-controlled localized cooling system enables both good wetout and consolidation while also enabling more accurate changes in tow path due to improvements in local tow tack. With the development of the five-axis system it is also shown that the tow width can be adjusted by rotating the existing placement nozzle to angles not equal to 90°. Thus, through a combination of controlled localized cooling and real-time control of the nozzle angle, a possible approach to control of tow width, independent of the tow placement angle and radius of curvature during tow steering, is described.

Background: Unicameral bone cyst (UBC) is a common lesion in skeletally immature patients. Multiple treatments are proposed as curettage and autologous bone graft, percutaneous local corticoids injections, decompression with internal fixation, and injection of bioresorbable cement. Decompression, curettage, and percutaneous bioresorbable cement injection showed interesting results, but until now, no long-term follow-up was reported in pediatric patients with UBC. Methods: We retrospectively evaluated 13 pediatric patients with UBC treated with curettage, decompression, and injection of a calcium phosphate composite (CPC) at a single institution with an average F-U of 5.46 years (range 5–7 years). Functional outcomes were evaluated according to the Musculoskeletal Tumour Society (MSTS) Score. Radiographic healing was assessed with the modified Neer Outcome Rating System. Complications were recorded. Results: The mean MSTS score was 29.61 (range 28–30). No joint limitation or any pain was recorded. All patients returned to their previous level of activity. Complete healed cysts were observed in 76.9% of patients (10 of 13) and partially healed in 23.1% (3 of 13). Three fractures of the humerus occurred without any further consequence. In two cases, CPC overflow in the surrounding soft tissues was reported on post-operative X-rays, but at last follow-up, no CPC remained in the soft tissues. Conclusion: Our observations suggest that the use of curettage, decompression, and injection of CPC results in a high rate of good clinical outcomes with low recurrence rates and complications in the pediatric population with UBC at a long-term follow-up

Stormwater runoffs are loaded with metals, PAHs, pesticides and pathogenic bacteria, conveyed for many in particulate form. One of the techniques used to manage this contaminated stormwater is to use retention basins in order to allow settling of the suspended solids. The deposits thus set up represent zones of contamination and a particular waste. The deposits thus constituted represent waste that must be managed. The objectives of this paper are: i) to characterize the sediments of a stormwater retention basin, ii) to identify the variability and heterogeneity of its microbiological, physical, chemical and ecotoxicological characteristics and to highlight the key factors that could explain this variability, iii) establish correlations between characteristics and identify trends in the change of sediment quality. The deriving results show that the median diameter of the sediments is between 50 and 150 μm. They are mostly contaminated with PAH, metals and 4-n-nonylphenol. Concerning the microbiological characterization, the bacteria “cores” observed in the sediments are composed of the bacterial genus which may contain pathogenic forms: Mycobacteria, Pseudomonas, Aeromonas, Nocardia. Nocardia counts are higher in sediments composed of fine and light particles (grain size between 1.1 and 3.3 μm). The ecotoxicity analysis of these sediments using the “ostracodes” test revealed that “fresh” sediments collected during a storm event were more ecotoxic than old ones. Sediment characteristics trended to become homogenous after 5 years of monitoring. Regarding investigations on the identification of the sources of microbiological contaminants observed in the retention basin, socio-urban observations revealed a direct relationship between the microbiological contamination of runoff and the presence of items and marks leave by socio-economic activities undertaken on the watershed. These results are useful for the managers in order to guide the choice or to stimulate the development of sectors or strategies of treatment and valorization of these sediments.

Resume La neurofibromatose de type 1 (NF1) est une maladie genetique predisposant au developpement de tumeurs malignes et benignes. La mutation du gene NF1 entraine une deregulation de la voie de signalisation RAS-MAP-kinase aboutissant a un dysfonctionnement du controle de la proliferation cellulaire et a la proliferation tumorale. L’epidemiologie des cancers chez les enfants atteints de neurofibromatose est tres differente de celle de la population pediatrique generale, imposant une surveillance specifique et rapprochee. Le neurofibrome est la tumeur benigne la plus frequente. Elle peut etre tres invalidante selon sa localisation et sa taille. Il n’existe actuellement pas de traitement specifique pour ces tumeurs. Les tumeurs malignes les plus frequentes sont les leucemies, les rhabdomyosarcomes, les gliomes et les tumeurs malignes des nerfs peripheriques. La prise en charge therapeutique doit prendre en compte le risque de developpement de tumeurs secondaires aux traitements. Nous rapportons le cas d’un enfant ayant developpe avant l’âge de 5 ans deux tumeurs.

El dolor en los pies no es un fenomeno normal en los ninos en crecimiento. Es un sintoma al que el medico debe prestar atencion y que, por lo tanto, no debe pasar por alto. Esto constituye un motivo suficiente que justifica un razonamiento diagnostico ante el pie doloroso. A traves de este razonamiento, es preciso distinguir entre el dolor cronico y el dolor agudo. El dolor agudo sugiere un traumatismo o una infeccion, sobre todo si existe fiebre. Aunque se trate de una enfermedad poco frecuente, el dolor cronico debe hacer pensar, en primera instancia, en un tumor maligno. A continuacion, la exploracion ha de distinguir entre pie cavo y pie plano. El pie cavo tiene una causa neurologica mientras no se demuestre lo contrario. El pie plano contracturado orienta hacia una sinostosis. Las causas mas frecuentes son las osteocondrosis; a la cabeza de estas se encuentra la enfermedad de Sever. Sin embargo, en la practica, el razonamiento debe llevar a pensar, en primer lugar, en los diagnosticos graves y, luego, en los demas diagnosticos por eliminacion.

El diagnostico de dolor en el pie del nino pequeno forma parte de un enfoque global, basado inicialmente en una exploracion fisica local y general cuidadosa. La manera como el recien nacido y el nino expresan el dolor, a traves de su comportamiento, puede resultar enganosa. La sistematica diagnostica debe basarse en la anamnesis del entorno y, a continuacion, en pruebas complementarias sencillas y accesibles en urgencias: busqueda de un posible sindrome inflamatorio biologico, pruebas de imagen como radiografias o ecografias. Aparte de un contexto traumatico sugestivo, el enfoque debe centrarse en descartar, en primer lugar, una etiologia infecciosa, que constituye una urgencia diagnostica y terapeutica. La radiografia es la exploracion de primera linea, independientemente de la edad. La ecografia, aunque reservada para equipos adiestrados, desempena un papel fundamental para localizar la lesion. La resonancia magnetica permite el diagnostico precoz de la osteoartritis, gracias a su capacidad para detectar precozmente el edema oseo. No debe, sin embargo, retrasar el tratamiento y la instauracion de una antibioticoterapia intravenosa.

OBJECTIVES The transient breath holding sign (TBHS) is a clinical sign often associated with magnetic resonance imaging (MRI) spine traumatic lesions. The aims of this study were to prospectively evaluate the TBHS in the detection of thoracolumbar lesions in a large cohort of children and to establish a comprehensive strategy on the use of MRI in spine traumas in children. STUDY DESIGN All conscious 5- to 16-year-old patients admitted for a spine trauma in our institution were prospectively included in the study. All patients were asked for the TBHS and underwent a full spine MRI. Sensitivity and specificity of the TBHS were derived from the confusion matrix. All MRI lesions were analyzed and classified. RESULTS One hundred ninety-eight patients were included. The sensitivity of the TBHS was 92%, the specificity was 83%, the positive predictive value was 83%, and the negative predictive value was 91%. The x-rays missed 67% of the vertebrae injured in the MRI. The MRI lesions consisted in an upper end plate injury, in the sagittal plane only, in 90% of the cases. The vertebral canal and the spinal cord were never injured. CONCLUSIONS This study confirms that the TBHS is a relevant clinical tool that should be added in the routine questionnaire after any trauma at admission. Magnetic resonance imaging should be restricted to patients with a TBHS positive at admission. A single T2 Short T1 Inversion Recovery (STIR) sagittal sequence seems sufficient to make the diagnosis and could replace the use of standard x-rays in pediatric spine traumas.

During industrialization, humans have been exposed to increasing numbers of foreign chemicals. Failure of the immune system to tolerate drugs, cosmetics, and other skin products causes allergic contact dermatitis, a T cell-mediated disease with rising prevalence. Models of αβ T cell response emphasize T cell receptor (TCR) contact with peptide-MHC complexes, but this model cannot readily explain activation by most contact dermatitis allergens, which are nonpeptidic molecules. We tested whether CD1a, an abundant MHC I-like protein in human skin, mediates contact allergen recognition. Using CD1a-autoreactive human αβ T cell clones to screen clinically important allergens present in skin patch testing kits, we identified responses to balsam of Peru, a tree oil widely used in cosmetics and toothpaste. Additional purification identified benzyl benzoate and benzyl cinnamate as antigenic compounds within balsam of Peru. Screening of structurally related compounds revealed additional stimulants of CD1a-restricted T cells, including farnesol and coenzyme Q2. Certain general chemical features controlled response: small size, extreme hydrophobicity, and chemical constraint from rings and unsaturations. Unlike lipid antigens that protrude to form epitopes and contact TCRs, the small size of farnesol allows sequestration deeply within CD1a, where it displaces self-lipids and unmasks the CD1a surface. These studies identify molecular connections between CD1a and hypersensitivity to consumer products, defining a mechanism that could plausibly explain the many known T cell responses to oily substances.

Arthrogryposis multiplex congenita (AMC) consists of congenital joint contractures that affect at least two joints. There are two types: in the first, arthrogryposis is an additional sign in the context of various pathologies (neuromuscular diseases); in the second, it is the main and constant symptom. In the first type, the progression of the causal underlying disease must be considered. In the second type, there are two specific forms: Amyoplasia corresponds to a significant congenital absence of muscles (epigenetic disease or vascular origin) while distal arthrogryposis has a genetic component and is transmissible. The orthopedic surgeon's purpose, which is usually to enhance movement, is not appropriate for an arthrogryposis patient. One must keep in mind that without muscle, movement is impossible. The goal differs between the upper and lower limbs: for the upper limb, it is to allow grasping, and, if possible, to bring the hand to the mouth; for the lower limb, it is to ensure ambulation with plantigrade support, and the knees extended, which is the only stable position possible with little to no muscles. The rehabilitation, orthoses and/or surgical techniques are chosen to achieve this singular aim. While it may appear modest, it is crucial for patients. The goal is to achieve useful mobility, not maximum mobility. This multidisciplinary treatment, which evolves over time, must be explained to the family to get its adherence.

International audience; Elbow injuries are frequent but occult fractures are difficult to diagnose on x-rays. However, any delay in the diagnosis may severely impair the prognosis of some fractures. Simple tips may help the clinician read x-rays properly and avoid the classical pitfalls of elbow injuries in children. The chronology of appearance of ossification nuclei around the elbow is important to distinguish normal features from abnormality. Drawing simple geometric constructions on the x-rays may clarify most occult elbow fractures in children.

Background: Patients with decompensated cirrhosis (DC) and/or hepatocellular carcinoma (HCC) have a high symptom burden and mortality and may benefit from palliative care (PC) and hospice interventions. Objective: Our aim was to search published literature to determine the impact of PC and hospice interventions for patients with DC/HCC. Methods: We searched electronic databases for adults with DC/HCC who received PC, using a rapid review methodology. Data were extracted for study design, participant and intervention characteristics, and three main groups of outcomes: healthcare resource utilization (HRU), end-of-life care (EOLC), and patient-reported outcomes. Results: Of 2466 results, eight were included in final results. There were six retrospective cohort studies, one prospective cohort, and one quality improvement study. Five of eight studies had a high risk of bias and seven studied patients with HCC. A majority found a reduction in HRU (total cost of hospitalization, number of emergency department visits, hospital, and critical care admissions). Some studies found an impact on EOLC, including location of death (less likely to die in the hospital) and resuscitation (less likely to have resuscitation). One study evaluated survival and found hospice had no impact and another showed improvement of symptom burden. Conclusion: Studies included suggest that PC and hospice interventions in patients with DC/HCC reduce HRU, impact EOLC, and improve symptoms. Given the few number of studies, heterogeneity of interventions and outcomes, and high risk of bias, further high-quality research is needed on PC and hospice interventions with a greater focus on DC.

Giant cell tumors (GCT) are benign primary bone tumors, locally aggressive, affecting in long bones in young adults during the third decade. It is rare to experience this lesion in skeletally immature patients. GCT are related to a risk of local recurrence and malignant transformation. We report a rare case of a giant cell tumor of the thoracic spine in a skeletally immature girl presenting with a painful right scoliosis. MRI, CT scan, and bone scintigraphy were discordant and the percutaneous biopsy non-contributive. A marginal “en bloc” resection was performed and revealed the GCT. Based on a literature review, the diagnosis and the surgical management of this case are discussed.

Adolescent idiopathic scoliosis is a 3D spine deformity that worsens during the whole growth. New methods for spinal growth modulation with flexible spinal implants have been described to avoid progression of the deformity during growth spurt. The main limitations are that no specific ancillaries and devices are available, which makes the surgery technically demanding.We have developed a new method of spinal growth tethering using minimal invasive videothoracoscopic approach. Fixation is performed with staples and synthetic ligament on the lateral aspect of vertebral bodies on main curvature convexity. Patients with progressive thoracic idiopathic scoliosis despite the brace treatment were included. The clinical and radiological examinations were compared before and 2 years after surgery.Six patients with flexible thoracic curves with a mean age 11.2 ± 1.2 years and a mean Cobb angle 45° ± 10° (35-60) were operated. All were skeletally immature. At last follow-up, the Cobb angle was stable. None of the patient underwent fusion.The procedure allowed a stabilization of the deformity during growth spurt. Validated devices and further studies with longer term follow-up are needed to confirm the efficiency of this technique. This small cohort of patients is a source of reflection for further medical devices developments.Level 4 case series comparing to not randomized studies.

Atopic dermatitis is a common pruritic skin disease in which barrier dysfunction and cutaneous inflammation play a role in pathogenesis. Mechanisms underlying the associated inflammation are not fully understood, and while CD1a-expressing Langerhans cells are known to be enriched within lesions, their role in clinical disease pathogenesis has not been studied. Here we observed that house dust mite (HDM) generates neolipid antigens for presentation by CD1a to T cells in the blood and skin lesions of affected individuals. HDM-responsive CD1a-reactive T cells increased in frequency after birth and showed rapid effector function, consistent with antigen-driven maturation. To define the underlying mechanisms, we analyzed HDM-challenged human skin and observed allergen-derived phospholipase (PLA2) activity in vivo. CD1a-reactive T cell activation was dependent on HDM-derived PLA2 and such cells infiltrated the skin after allergen challenge. Filaggrin insufficiency is associated with atopic dermatitis, and we observed that filaggrin inhibits PLA2 activity and inhibits CD1a-reactive PLA2-generated neolipid-specific T cell activity from skin and blood. The most widely used classification schemes of hypersensitivity, such as Gell and Coombs are predicated on the idea that non-peptide stimulants of T cells act as haptens that modify peptides or proteins. However our results point to a broader model that does not posit haptenation, but instead shows that HDM proteins generate neolipid antigens which directly activate T cells. Specifically, the data identify a pathway of atopic skin inflammation, in which house dust mite-derived phospholipase A2 generates antigenic neolipids for presentation to CD1a-reactive T cells, and define PLA2 inhibition as a function of filaggrin, supporting PLA2 inhibition as a therapeutic approach.

In children, sevoflurane is the most commonly used anaesthetic. Its excellent haemodynamic tolerance gives it a wide therapeutic index. This halogenated agent can abolish movement [minimal alveolar concentration (MAC)] or haemodynamic responses (MACBAR) to noxious stimulus in children as in adults. Reflex pupillary dilatation (RPD) has been demonstrated as a very sensitive measure of noxious stimulation. In order to investigate the effect of sevoflurane on the RPD, a subcortical reflex, we determined the MAC of sevoflurane inhibiting the RPD in 50% of the subjects in response to skin incision (MACpup) in pre- and post-pubertal subjects.We included 30 pre-pubertal children and 19 post-pubertal subjects. Patients received sevoflurane at preselected concentrations according to an 'up and down' design, and after a steady-state period, skin incision was performed. The RPD was considered as significant when the pupillary diameter increased by more than 100%. Heart rate (HR) and bispectral index (BIS) changes were analysed according to the pupillary response.The MACpup of sevoflurane was 4.8% (95% confidence interval, 4.6-5.1%) in pre-pubertal children vs 3.4% (3.5-3.3%) in post-pubertal subjects (P0.001). Inhibition of RPD was always associated with lack of significant HR response. In pre-pubertal children receiving high concentrations of sevoflurane, RPD in response to noxious stimulation was frequently associated with lack of HR response and low BIS values.MACpup was higher than MAC and close to the MACBAR. Inhibition of RPD in pre-pubertal children required higher sevoflurane concentrations compared with post-pubertal subjects, suggesting that the relationship between the brain structure sensitivities may differ with brain maturation.

Le LCPC a organise et participe a un concours de prevision du comportement d'un ouvrage en Terre Armee dans le cadre des Journees Nationales de Geotechnique et de Geologie de l'Ingenieur en 2008. On presente deux modelisations numeriques par elements finis effectuees par le LCPC pour repondre a ce concours : la premiere a ete realisee a partir de motifs predefinis et la deuxieme utilise un modele homogeneise. Dans un premier temps, on compare les resultats des calculs effectues avant les mesures, entre eux et avec les donnees experimentales. On a ensuite cherche, pour les deux modelisations, a estimer l'influence des caracteristiques mecaniques du sol et de l'interface sur le comportement de l'ouvrage. On a egalement essaye de recaler a posteriori differents parametres sur la base des mesures, notamment sur la base d'essais d'extraction d'armatures dans l'ouvrage experimental. Une fois parvenu a un calage juge satisfaisant, on a utilise les modeles pour simuler la reponse du mur a un nouveau chargement statique vertical plus eleve que celui applique pour le concours. (A). Athletes at Olympic Developmental Program (ODP) camps experience unusually high levels of expectations and inherent mental and physical challenges within such a short span of time. With the increasing emphasis on talent development, there has been consensus by the ODP staff to more clearly define present levels of coping skills, in order to enhance athletic prediction, maximize training efforts, identify the predisposition to injury, and focus on areas pertinent to successful performance. This study examined athletic and pain coping skills of U. S. ODP soccer athletes not previously investigated. Following written informed consent, 70 males completed the Athletic Coping Skills Inventory and the Sports Inventory for Pain. Data were analyzed by competitive level (U-14, U-15), and skill position (goalkeeper/defense, midfield/foward). MANOVA indicated a significant main effect across competitive level (Wilks' Lambda F(12,57) = 2.27; p = 0.02; n-beta = 0.915) but no significant effect by skill position (Wilks' Lambda F(12,57) = 0.931; p = 0.523; n-beta = 0.457). Post hoc analyses indicated that U-15 athletes scored significantly higher in concentration (p = 0.01) and body awareness (p = 0.03), but lower in avoidance (p = 0.01) than U-14 competitors. In conclusion, older, more experienced athletes revealed more positive athletic and pain coping skills than younger, less experienced athletes, although athletes in skill positions requiring spontaneous decision-making skills and split-second adjustment in a constantly changing sport environment (forwards, midfielders) did not exhibit more positive athletic and pain coping skills than those positions requiring reaction and protection (defenders, goalkeepers).

In 2002 and 2003, the Bishops Falls hydroelectric generating facility on the Exploits River, insular Newfoundland, Canada, underwent extensive refurbishing including replacement of turbines and installation of a ‘retrofitted’ bypass and fish handling system. The effectiveness of this new bypass system has been assessed during the annual downstream run of wild Atlantic salmon smolt and kelt in 2003 and 2004. In 2003, 195 smolt were radio tagged and released between June 9 and July 2, in the forebay of the hydro plant (19 releases) and one upstream (in-river) release. Fish guidance efficiency (FGE) of the system overall was 63% (123 of 195 fish) with 36 fish passing through the turbines, and six known mortalities. In 2004, between June 9 and July 2, a total of 358 smolt and 103 kelt were released in the forebay in 45 and 13 releases (n = 8 per release), respectively. The FGE of the system for smolt was 71.7% (257 of 358 fish) and for kelt was 92.3% (95 of 103 fish). In 2004, 96 tagged smolt passed through the turbines and 43 (44.8%) were detected at a downstream station confirming they had survived turbine passage, suggesting an overall survival of smolt passage of the Bishops Falls hydro facility in the order of 85%. A total of seven kelts (6.8%) passed through the turbines and were not detected 1.5 km downstream suggesting they did not survive turbine passage. Smolt spent on average 39.8 h in the forebay before exiting in 2003 and forebay residency averaged 26 h in 2004. In both years, most smolt selected their passage route, actively or passively, within the first 10 h with secondary peaks at 25–30 h and 50–55 h, corresponding to evening passage in the second and third night, after release. Few smolt were bypassed or entrained into turbines during daylight hours. In both years turbine passed smolt spent more time in the forebay suggesting the longer fish reside in the forebay the greater the likelihood of turbine entrainment. Kelt were either bypassed or turbine entrained relatively quickly, within 2 h of release, and virtually all kelts were bypassed/turbine entrained during the hours of 18:00 and 01:00. These data on fish behaviour and residency in the forebay will assist further refinement of operations of the bypass facility to optimize survival.

International audience; The aim of this study was to evaluate the osseointegration of titanium implants (Ti-6Al-4V, noted here TA6V) and poly(etheretherketone) PEEK implants induced by a BMP-2-delivering surface coating made of polyelectrolyte multilayer films. The in vitro bioactivity of the polyelectrolyte film-coated implants was assessed using the alkaline phosphatase assay. BMP-2-coated TA6V and PEEK implants with a total dose of 9.3 μg of BMP-2 were inserted into the femoral condyles of New Zealand white rabbits and compared to uncoated implants. Rabbits were sacrificed 4 and 8 weeks after implantation. Histomorphometric analyses on TA6V and PEEK implants and microcomputed tomography on PEEK implants revealed that the bone-to-implant contact and bone area around the implants were significantly lower for the BMP-2-coated implants than for the bare implants. This was confirmed by scanning electron microscopy imaging. This difference was more pronounced at 4 weeks in comparison to the 8-week time point. However, bone growth inside the hexagonal upper hollow cavity of the screws was higher in the case of the BMP-2 coated implants. Overall, this study shows that a high dose of BMP-2 leads to localized and temporary bone impairment, and that the dose of BMP-2 delivered at the surface of an implant needs to be carefully optimized. Statement of Significance The presentation of growth factors from material surfaces currently presents significant challenges in academia, clinics and industry. Applying osteoinductive factors to different types of implants, made of metals or polymers, may improve bone repair in difficult situations. Here, we show the effects of an osteoinductive coating made of polyelectrolyte multilayer films on two widely used materials, titanium TA6V alloys and PEEK implants, which were implanted in the rabbit femoral condyle. We show that a too high dose of BMP-2 delivered from the screw surface has a negative short-term effect on bone regeneration in close vicinity of the screw surface. In contrast, bone formation was increased at early times in the empty spaces around the screw. These results highlight the need for future dose-dependence studies on bone formation in response to osteoinductive coatings.

Context The primary goal of traditional treatment and rehabilitation programs is to safely return athletes to full functional capacity. Nontraditional activities such as rock climbing or rodeo are typically less training structured and coach structured; individualism, self-determination, and autonomy are more prevalent than observed in athletes in National Collegiate Athletic Association (NCAA)-sponsored sports. The limited research available on nontraditional athletes has provided the athletic trainer little insight into the coping skills and adaptations to stressors that these athletes may bring into the clinical setting, especially among the growing number of women participating in these types of activities. A better understanding of the pain-coping traits of nontraditional competitors would enhance insight and triage procedures while heading off potential athlete-related risk factors in the clinical setting. Objective To quantify and compare pain-coping traits among individual-sport women athletes participating in nontraditional versus traditional NCAA-structured competition, with relevance to optimal treatment and rehabilitation. Design Cross-sectional study. Setting Data collected during each participant's respective group meeting before seasonal activity. Participants or Other Participants A total of 298 athletes involved in either nontraditional, non-NCAA individual sports (n = 152; mean age = 20.2 ± 1.3 years; downhill skiing, martial arts, rock climbing, rodeo, skydiving, telemark skiing) or traditional NCAA sports (n = 146; mean age = 20.3 ± 1.4 years; equestrian, golf, swimming/diving, tennis, track). Main Outcome Measure(s) All participants completed the Sports Inventory for Pain, a sport-specific, self-report instrument that measures pain-coping traits relevant to competition, treatment, and rehabilitation. Trait measures were direct coping, cognitive, catastrophizing, avoidance, body awareness, and total coping response. Data were grouped for analyses by type of athlete (nontraditional, traditional). Results We found a significant main effect for type of athlete (Wilks' λ F6,291 = 12.922; P = .0001). Nontraditional sport athletes scored lower on direct coping (P = .0001), cognitive (P = .0001), catastrophizing (P = .0001), and total coping response (P = .0001) than traditional athletes. Conclusions Women participating in nontraditional individual-sport activity revealed less pronounced pain-coping traits than women participating in more coach-structured, traditional NCAA sports. Sport and medical personnel should consider the type of athlete when prescribing training, treatment, and rehabilitation for optimal performance and return to play.

International audience; Adolescent idiopathic scoliosis is a 3D spine deformity that worsens during the whole growth. The treatments available are bracing during the whole growth to avoid progression of the deformity and surgical fusion of the spine at skeletal maturity for the most severe curves. Recent advances in flexible spinal implants have given rise to new expectations in the management of progressive scoliosis with growth modulation without fusion. For the first time, treatment allows us to expect a correction of progressive curves without fusion. This review discusses the recent developments in the field of spinal growth modulation techniques and discusses the pros and cons of the medical devices used in this indication.

Elbow injuries are frequent but occult fractures are difficult to diagnose on x-rays. However, any delay in the diagnosis may severely impair the prognosis of some fractures. Simple tips may help the clinician read x-rays properly and avoid the classical pitfalls of elbow injuries in children. The chronology of appearance of ossification nuclei around the elbow is important to distinguish normal features from abnormality. Drawing simple geometric constructions on the x-rays may clarify most occult elbow fractures in children.

Burst is an ultra-rapid imaging sequence that works by excitation of mag- netization using a series of regularly spaced, low flip-angle pulses, applied at the same time as a gradient in the read direction. In the original concept, this gradient is constant, and multiple echoes are refocused under a constant-read gradient. This article brings together the previous literature on the subject, placing the various developments within a common framework. The potential of the technique is discussed, with the aid of a comparison of the signal-to-noise ratio for the various Burst sequences previously proposed and a detailed case study of one of the more promising variants. © 2005 Wiley Periodicals, Inc. This paper analyzes the effects of intra-scan motion and demonstrates the possibility of correcting them directly in k-space with a new automatic retrospective method. The method is presented for series of 2D acquisitions with Cartesian sampling. Using a reference k-space acquisition (corrected for translations) within the series, intra-scan motion parameters are accurately estimated for each trajectory in k-space of each data set in the series resulting in pseudo-random sample positions. The images are reconstructed with a Bayesian estimator that can handle sparse arbitrary sampling in k-space and reduces intra-scan rotation artefacts to the noise level. The method has been assessed by means of a Monte Carlo study on axial brain images for different signal-to-noise ratios. The accuracy of motion estimates is better than 0.1 degrees for rotation, and 0.1 and 0.05 pixel, respectively, for translation along the read and phase directions for signal-to-noise ratios higher than 6 of the signals on each trajectory. An example of reconstruction from experimental data corrupted by head motion is also given.

Objectives: The primary objective was to document the relationship between the Balanced Inventory of Desirable Responding (BIDR) indices of impression management (IM) and self-deception (SDE) response styles and the Athletic Coping Skills Inventory (ACSI-28) subscales. It was hypothesized that the ACSI-28 subscales would be significantly related to BIDR IM and SDE. Design: Cross-sectional. The relationship between ACSI-28 subscales and the BIDR was assessed using SAS Multiple Regression Analyses procedures. A combination of IM and SDE served as predictors of the ACSI-28 subscale outcome variables. Method: Undergraduate volunteers (n=468) completed a test booklet consisting of the BIDR and the ACSI-28. Results: The ACSI-28 was found to be relatively free of the more traditional IM response bias, however all ACSI-28 subscales were strongly affected by SDE bias. Conclusions: Additional studies are needed to determine whether self-deceptive responding is a source of psychometric confounding or is perhaps an adaptive psychological coping skill. It is suggested that both the IM and SDE constructs be considered in the design of sport psychology research involving self-report measures.

L’analyse par des methodes numeriques du comportement des ouvrages renforces par inclusions est difficile et couteuse lorsque l’on veut modeliser explicitement les renforcements. Une approche alternative consiste a remplacer le massif renforce par un materiau equivalent constitue par la superposition de plusieurs milieux continus. Contrairement aux theories d’homogeneisation « classiques », cette methode permet la prise en compte de cinematiques differentes pour chacun des milieux qui constituent le materiau equivalent, et donc de representer le glissement relatif du sol et des inclusions de renforcement. Elle est en outre plus simple et plus rapide a mettre en œuvre.L’intregration de ce modele dans le progiciel CESAR-LCPC a ete entreprise. La version actuelle du code permet le calcul elastoplastique d’ouvrages bidimensionnels et tridimensionnels, dans le cas ou seuls les efforts de traction-compression dans les inclusions sont pris en compte, et ou la cinematique de chaque materiau constituant est supposee identique. On presente ici les resultats obtenus pour l’etude de deux ouvrages renforces (analyse tridimensionnelle du tassement d’une fondation carree reposant sur des pieux, etude de la convergence d’un tunnel boulonne), mettant en evidence la pertinence et l’efficacite de la methode.

Background The conventional approach for MRI procedures in very young children is to use general anesthesia which comes with inherent risks. Non-pharmacological strategies to reduce anxiety in children have also been described, but they all require patient cooperation. The purpose of the study was to evaluate the ability to complete diagnosis using temporary spica cast immobilization (TSCI) in children less than 3 years old undergoing MRI procedures for lower limb disorders. Materials and methods A retrospective review identified 14 children under 3 years old that had required an MRI for a lower limb disorder, using TSCI. The MRI procedure was performed for evaluation of hip dysplasia, bone infections, limping, evaluation of soft tissue tumor and femoral head osteonecrosis. A spica cast was fitted by the pediatric orthopedic team. The MRI procedure was subsequently performed. Results Diagnosis was achieved in all cases. The radiologist identified movement artifacts (14 %) that did not impair the image quality enough to prevent interpretation. Conclusion TSCI is a safe, effective and costless procedure avoiding general anesthesia for young patients under 3 years old who require MRI for pelvis or lower limb disorders. Level of evidence IV.

Marine exploitation rates were estimated for nine Newfoundland Atlantic salmon (Salmo salar L.) populations, separately for small and large salmon size components. Estimates were derived using counts of salmon returning to fish counting facilities rather than from tagging studies and thus adjustments were not required to account for tag loss, handling or tagging mortality, or tag reporting rates. For all stocks combined, the overall marine exploitation rate during the period 1984-1991 averaged 45.3% (29.6-57.1%) on small salmon and 74.2% (57.7-83.7%) on large salmon. These estimates are considered minimum values. Concerns related to declining salmon abundance resulted in the closure of the Newfoundland commercial salmon fishery in 1992. Results are discussed in relation to previous estimates derived from tagging, and highlight the importance of accounting for marine exploitation when examining trends in salmon survival and return data even when commercial fisheries have been closed.

This investigation examined the relationship of group cohesion to mood disturbance, stress, and athletic performance in a female collegiate basketball team. Twenty players from a Division I college basketball team were administered a battery ofself-reportquestionnaires, including the Group Environment Questionnaire (GEQ), the Profile of Mood States (POMS), and the Social and Athletic Readjustment Rating Scale (SARRS). Significant results were observed in relation to the Group Integration-Task (GI-T) and Group Integration-Social (GI-S) subscales of the GEQ. Participants perceiving high GI-T were less depressed, less confused, and showed less total mood disturbance on the POMS. Individuals reporting high GI-S experienced less personal, academic, coaching, sport, and injury-related stress, and subjects scoring high on GI-T or GI-S subscales reported lower levels of total stress as measured by the SARRS.

The consolidation of a saturated porous medium submitted to a vertical load is a classical problem, and has been given a complete analytical solution within the framework of linear poroelasticity. For a layer constituted by several homogeneous strata, the problem can be dealt with by means of analytical as well as numerical methods, provided that the contrast between the properties of the strata remain moderate. In this paper, we consider a layer constituted by two strata and discuss the situation in which the contrast between the properties of the strata becomes very large and fluid conduction exhibits very different characteristic times in each stratum. The method used consists in introducing a double time scale, and gives an analytical solution, which makes it possible to discuss the influence of the ratio between strata thicknesses on the overall consolidation time of the layer.

The numerous works devoted to the phenomena involved in the compaction of sediments have led to a good understanding and to mechanically consistent modellings of this process. During compaction, large reduction of porosity occurs and the sediments undergo large strains : in this context, the usual linearized Green-Lagrange strain tensor is no longer relevant. Using the constitutive law of a poroelastic medium (made of incompressible constituents) at large strains, we discuss the influence of geometrical nonlinearities in the study of purely mechanical compaction.; Les contributions successives de différents auteurs ont permis d'identifier les phénomènes mis en jeu dans la compaction des sédiments, puis de proposer des modélisations mécaniques cohérentes de ce processus. On s'intéresse ici aux non-linéarités géométriques dues à la forte diminution de la porosité: elles imposent de renoncer à employer la déformation de Green-Lagrange linéarisée. On adopte une loi de comportement adaptée aux grandes transformations d'un milieu poreux élastique dont les constituants sont incompressibles pour étudier la diminution de porosité liée aux phénomènes purement mécaniques intervenant dans la compaction.

A system is described for the large-scale stocking of unfed salmon fry over large distances (200–300 km). The system is based on the principle of a box within a box and uses recirculated water. Fry are transported by helicopter, and the system can be powered by the helicopter's power supply (24 V) or by a battery (12 V). A maximum of 150,000 fry can be transferred per trip. Individual containers, stocked at required densities, allow for control of stocking density.
