The formation of primary multiple malignant neoplasms, such as breast cancer and prostate cancer, in one patient is a rather rare combination. Treatment and diagnostic strategies in such cases require a multidisciplinary approach. Currently, there are no approved recommendations for the management of patients with a combination of these diseases. This manuscript presents a clinical case of diagnosis and treatment of breast and prostate cancer in one patient.

BACKGROUND: Seasonal influenza vaccination is recommended for patients with cardiovascular diseases. Low vaccination coverage among these patients is due to insufficient knowledge about vaccine efficiency and its regular annual use. This work aimed to study the repeated 6-month changes in hemagglutinin antibody titers (AT) for 3 years in patients with cardiac pathology in a comparative study of influenza preventive vaccination. MATERIALS AND METHODS: Analysis of ATs obtained based on the hemagglutination-inhibition test (HAI) was performed in 235 of 817 participants in a prospective follow-up. Blood sampling was performed at baseline, before the vaccination, and 6 months after, and at the same term in unvaccinated patients in the 2012–2013 and 2014–2015 seasons, respectively. The seropositive and seronegative responses to vaccination or acute respiratory or influenza infection were used, according to the reference values of seroconversion and seroprotection and the fact of seroconversion. Multiple regression analysis with a logarithmic scale was used to assess the vaccine effectiveness indices. RESULTS : With vaccination coverage of at least 40% against seasonal influenza within 3 years, the trends of a decrease in seropositive and an increase in seronegative responses in the vaccination group and its reverse nature in the comparison group were determined by traditional analysis. Using logarithmic calculation, an increase in HAI AT seroconversion was revealed over a 3-year follow-up period. It was characterized by a homogeneous serological response at annual vaccination and heterogeneous with a higher serological response in cases without vaccination (p = 0.002 for H1N1 and p = 0.005 for H3N2, respectively). This trend can be determined by a higher and more stable prevaccination level of HAI AT than the same residual level of AT among unvaccinated patients. CONCLUSION : During long-term cardiac studies, the logarithmic calculation in interpreting the results of HAI AT overcomes the limitations of the traditional analysis of assessing the efficiency of the annual influenza vaccine. Further serological programs are required to better understand the role of routine seasonal influenza vaccination in preventing morbidity and mortality of patients with cardiovascular diseases.

The new international guidelines on hypertension management have been issued in the past few years. The AHA (2017) and ESH (2018) Guidelines are similar in many key aspects. However, they differ substantially in the area of blood pressure (BP) measurement methodology. In this article, we aim to explain the ESC Guidelines position, specifically the conservative BP thresholds and a wider use of ambulatory BP measurement methods. In our opinion, the main reason behind this position is the introduction and a relatively widespread use of automatic office BP measurement (AOBP). On one hand, this method has questioned the results of the SPRINT trial – the key evidence source for the AHA Guidelines. On the other hand, AOBP has challenged the concept of “traditional office BP measurement”. Therefore, it is important to be aware of the current multiplicity of BP measurement methods and respective threshold values recommended for each method. It is essential to perform the office BP measurement correctly and thoroughly. Throughout all stages of the hypertensive patient management (diagnosis, antihypertensive treatment choice, and long-term observation), the use of out-of-clinic BP measurement is strongly advisable.

The study present the results of the satellite monitoring of Arctic icebergs formed from the outlet glaciers of the Severnaya Zemlya, Novaya Zemlya, and Franz Josef Land archipelagos during the autumn-winter and spring periods. The algorithms for iceberg detection in satellite images under different meteorological and ice conditions are described.

The connection between the heating of power transformers and nonlinear distortion of the current curve is shown. Distortion is created by non-linear electrical loads. The heating is caused by the higher harmonics of the electric current flowing through the transformer. The necessity of solving the problem of heating transformers in the presence of a large share of nonlinear consumers of electricity in their load has been substantiated. The results of the analysis of voltage and current sinusoidal distortions of transformers with a predominance of nonlinear load are presented. It was revealed that the harmonic coefficients exceeded the values admissible by the standards. In the spectrum of harmonics, the third harmonic has the highest percentage. The simulation of an electrical network with a transformer and nonlinear load in the MATLAB environment has been performed. The results are obtained for the original network and the network with the installed passive filter. The filter is designed for the third harmonic of the electric current. As a result of the simulation, a significant reduction in the current harmonic coefficient was obtained after installing a passive filter. Analysis of other methods of reducing the temperature of the transformer from the influence of higher harmonics of the current showed the advantage for the substations under consideration of the method for reducing the load of transformers by installing an additional transformer.

The article reviews the assessment methods of energy supplied to the surface of solar panels. The need of increase in accuracy of calculation of the solar energy entering on the panel is proved due to the accounting of additional influencing factors. A technique for estimating solar energy to the surface of panels is developed. The degree of cloudiness influence on the solar energy input for the area under consideration is identified. The comparison of the results of modeling with the use of the developed technique and specialized computer programs is carried out. It showed satisfactory results. Modeling of incoming solar energy for the existing solar power station showed a higher quality of the developed technique compared with specialized software. The optimal angle of inclination of the solar panel is calculated and the reduction of the incoming energy is analyzed when the angle deviates from the optimal value. The application of modeling of incoming solar energy intake to obtain daily graphs for the electricity generation of a solar power station and comparing them with daily graphs of the electrical load of the consumer is considered. The authors draw the conclusions by the results of research.

An express assay for screening of potato transformants by their GFP fluorescence intensities is developed. In comparison to the widely used methods of transgenic plant screening by PCR, Real-Time RTPCR or Northern-blotting, the GFP fluorescence assay needs no expensive reagents and takes less time. This approach may also be used for nondestructive screening of the T0 transgenic regenerants which can be further grown and used. To prove this assay reliability, the expression of the hGFP gene in the leaves of transgenic potato (cv. Skoroplodny) plants, determined by its mRNA accumulation, was compared to GFP fluorescence intensity in the micro-samples of aseptic plant leaves. The strong correlation between the results of these two methods is the evidence of positive dependence of GFP fluorescence intensity on the target mRNA content.

An alternative to reequipping metalworking enterprises is to modernize outdated CNC machines. In modernization, the aggregation principle permits the introduction of up-to-date high-speed cutting and multiaxial machining technology.

By modernizing outdated machine tools for specific functions on modular principles, manufacturing and, in particular, metalworking may be brought to current standards without huge capital expenditures. As an example, the modernization of the 6М610F11-23 longitudinal milling machine and its utilization in the manufacturing of inexpensive molds offers a valuable means of import substitution for RUSAL.

Recently, more and more attention has been paid to the chronotherapy of arterial hypertension (HT). This method provides an opportunity to improve the treatment efficacy by non-traditional regimes of antihypertensive drug intake. The new findings concerning a high prognostic value of the night-time blood pressure (BP) and night BP fall stimulate the interest in chronotherapy of HT. This method is very easy to use, and the evidences of its high efficacy are growing. The advantages and limitations of two potential chronotherapy approaches are discussed in detail. MAPEC was the largest project which investigated the possibility of cardiovascular complications risk reduction by normalizing the circadian rhythm of BP due to the bed-time administration of antihypertensive drugs. The authors obtained impressive results – more than a double reduction of cardiovascular complications risk in the chronotherapy group. However, the study had serious limitations: ethnically homogeneous population, the lack of independent verification of endpoints and adverse events, and inconvenience of 48-hour BP monitoring. The currently available scientific information concerning advantages of HT chronotherapy needs further confirmation in the carefully designed multi-centre prospective studies. According to the authors’ opinion, illustrated by their original data, the relatively small and short studies are still important. Such trials allow using more nuanced methodological approaches, such as a double blind or cross-over design and comparison of the pharmacokinetics of antihypertensive drugs in the morning and evening administration.

The analysis of electricity tariffs for consumers of the Republic of Khakassia of the Russian Federation has been performed. Tariffs for the population are quite low. The use of small solar power plants for private residential buildings is inexpedientl due to the long payback period. Tariffs for legal entities and organizations are much higher than for the population and have a steady upward trend. The main consumption of electricity by such consumers occurs during daylight hours. That is much in line with the energy generation by solar panels. It is concluded that calculations are necessary to determine the expedience of using small solar power plants for this group of consumers. The classification of the types of low-power solar power plants is given, their advantages and disadvantages are highlighted, and the area of application is shown. The results of the comparative analysis of three types of small solar power plants for a state facility of the Republic of Khakassia are presented. The expediency of using small network solar power plants without electric storage batteries with a limitation of the power supplied to the network was substantiated.

Aim. To estimate an efficiency of influenza vaccination in patients with circulatory system diseases diseases (CSD) under 3-year follow-up in outpatient clinics. Methods. The efficiency of influenza vaccination was investigated in CSD patients followed up at 2Ivanovo outpatient clinics and 2Saratov ones. The investigation enrolled 817 people, including 367 patients who consented to Grippol Plus influenza vaccination and 450 who refused. Results. During 36-month follow-up after being included in the study the vaccinated group showed a significantly fewer influenza and acute respiratory viral infections than the non-vaccinated group (28 and 442; р

Aim. To compare enalapril (E) and diltiazem (D) efficacy in menopausal women with arterial hypertension (AH) using three methods of blood pressure (BP) measurement.Material and methods. There was a cross-over randomised study with E and D included 22 menopausal women with AH. E was given at a daily dose of 5-20 mg and D – at a daily dose of 180-360 mg. At baseline and at the end of each treatment period ambulatory blood pressure monitoring (ABPM) was carried out. Self blood pressure measurement (SBPM) was performed during all study period. To assess a steadiness of antihypertensive effect we calculated smoothness index and trough-to-peak ratio based on ABPM data.Results. Ambulatory 24-h systolic BP and diastolic BP were decreased in the E group (-15.2/-9.6 mm Hg) and in the D group (-10.9/-6.0 mm Hg). E demonstrated a greater antihypertensive effect on SBPM results in the evening. Values of smoothness index on E treatment were slightly higher than on D treatment (0.86±0.15/0.64±0.13 vs 0.67±0.17/0.62±0.15). Trough-to-peak ratio was inconclusive in this study.Conclusions. Results of the study have shown some advantages of antihypertensive therapy with E compared with these of D in slow release formulation (both drugs at a single daily dose) in menopausal women. A sustained antihypertensive effect without negative influence on circadian BP profile allows to consider E as one of perspective agents for the AH treatment in this type of patients.

The article proposes creation of small high-tech engineering enterprises, which, in the context of a complete transition to digital technologies, will be able to solve effectively specific technological problems of large industrial enterprises and corporations. An example of an existing small digital enterprise is considered, where the technology for manufacturing foundry equipment for producing flat ingots for UC RUSAL is developed and implemented. Digital technologies are described using aggregation elements that were introduced during the modernization of the 6M610 longitudinally milling machine.

In the secondary circuits of power system facilities geomagnetic variations cause geoinduced current which is, according to the nature of its influence, a conducted electromagnetic disturbance; so, it can result in the incorrect operation of microprocessor protection relays. The geoinduced current values are determined by the intensity and duration of variations of the geomagnetic power and angle components. The fuzzy cluster analysis was carried out to define the influence of geomagnetic variations on the operation of microprocessor protection relays maintained in the power system facilities of the Republic of Khakassia. The authors also define time and amplitude intervals of the variation rate for geomagnetic power and angle components, which cause incorrect operation of microprocessor protection relays. The results of the fuzzy clustering show proper coincidence with the results obtained with numerical differentiation of functions in a tabular form, with the construction of Lagrange interpolation polynomials of the second order. The analysis outcomes are important for the prediction and prevention of possible incorrect operation of microprocessor protection relays at the power system facilities during geomagnetic storms.

The authors propose a model of fuzzy inference system (FIS) to determine the optimal consumption of hydro resources of hydroelectric reservoirs by the criterion of maximum generation of electrical energy. It is proposed to use a fuzzy controller that allows to develop optimal strategies to ensure the maximum energy output of hydroelectric power stations and passes to the downstream, to formulate proposals for creating a subsystem for calculating the regimes, schedules of drawdown and filling of reservoirs by the criterion of maximum generation of electrical energy. The use of fuzzy controllers and submission to their inputs of data on changes in the regimes of a cascade of hydroelectric stations in real time allows us to obtain a FIS system that will be an assistant for the operational management of hydroelectric reservoirs.

The authors described a model for predicting the improper operation of microprocessor relay protections caused by the flow of geo-induced currents in the secondary circuits during geomagnetic storms. The model is based on the mathematical method of Takagi-Sugeno. The simulation was performed using an adaptive neural fuzzy network. The ANFIS-editor of the Fuzzy Logic Toolbox extension package in Matlab was used. Since the parameters under study are probabilistic in nature, Gaussian functions were used in the prediction. Input parameters are the date, time and variations of the components of the geomagnetic field. The output data are predictions of malfunctioning microprocessor relay protection in the power system. Using the model, the dependencies between the studied parameters were revealed, the influence of these parameters on each other was evaluated.

New results of geomorphological, seismoacoustic, and lithological investigations on the upper continental slope off the Arkhipo-Osipovka Settlement are presented. Here, a large submarine slump was discovered by seismic survey in 1998. The assumed slump body, up to 200 m thick, rises 50–60 m above the valley floor that cuts the slope. Recent semiliquid mud that overlies laminated slope sediments with possible slump deformations flows down in the valley thalweg. Radiocarbon age inversion recorded in a Holocene sediment section of shelf facies recovered from the upper slope points to the gravity dislocation of sediments.

Amplification of high-power ultrashort pulses in gas-discharge amplifiers is investigated theoretically. The most essential features of amplification in this case are connected with an electron density wave (EDW) generated by laser pulse in gain medium due to the avalanche ionization. Traveling with amplified pulse, EDW results in the pulse amplitude and phase modulation. Also, it determines the ultimate pulse parameters (energy, peak intensity, duration) and features of pulse evolution in long amplifier. The laser operation with a variety of CO2 multi-isotope mixtures is investigated also. Such operation enables amplification of laser pulses with duration down to 100 fs. Aim.High-sensitive C-reactive protein (hsCRP) is currently regarded as an important marker of cardiovascular disease (CVD). The aim of the study was to assess the population characteristics of hsCRP in the sample of Moscow residents aged 55 years and older.Material and methods.The analysis included 1851 people (response rate 65 %), who participated in the SAHR (Stress, Aging, and Health in Russia) study.Results.The hsCRP levels were similar in men and women, with the right-skewed distribution. The hsCRP values ranged from 0,5 to 69,5 mg/dl in men (median 1,5 mg/l), and from 0,5 to 75 mg/l in women (median 1,5 mg/l). The mean hsCRP levels were slightly higher in men than in women (3,4±0,19 vs. 3,1±0,15, respectively;р=0,36). Overall, there was no marked age-related dynamics of hsCRP levels in men or women. The prevalence of high hsCRP levels (>3 mg/l) non-significantly increased from 31,2 % in the youngest age group to 36,2 % in those aged 75 years and older. Acute inflammation (hsCRP levels >10 mg/l) was registered in 85 participants (4,5 %), including 42 men and 43 women. After adjustment for age and gender, the lower education level remained a significant predictor of hsCRP elevation.Conclusion.No clear associations between hsCRP and age or gender were observed. However, participants with university and secondary education had significantly lower hsCRP concentrations, compared to their peers with education level lower than secondary.

Background. The elevation of blood pressure (BP) affects the development of vascular inflammation. At the same time, it has been suggested that inflammation itself could be an independent risk factor (RF) of arterial hypertension (AH) development. Aim. To investigate whether the association between increased levels of C-reactive protein (CRP) and AH is independent from classical RFs. Material and methods. The data were obtained during a crosssectional survey of 1876 Muscovites (47,9% men) aged ≥55 years, who participated in the prospective study “Stress, Ageing, and Health in Russia”. In all participants, socio-demographic characteristics, health behaviours, parameters of anthropometry and rest electrocardiography (ECG) were assessed. The levels of BP and blood lipids were also measured. The outcome variable was an increase in CRP levels (>3 mg/l). Statistical methods included logistic regression; the risk estimates were presented as odds ratios (OR) and 95% confidence intervals (CI). Results. There was a positive link between AH and CRP levels of >3 mg/l. After adjustment for age and sex, OR of increased CRP levels in hypertensive participants vs. their AH-free peers was 1,688 (95% CI 1,323-2,154; p=0,0001). In the final model (adjustment for age, sex, educational level, smoking, alcohol consumption, abdominal obesity, high atherogenicity index, and coronary heart disease, CHD), this effect remained statistically significant (OR 1,450; 95% CI 1,127-1,864; p=0,004). Conclusion. In elderly Muscovites, a positive association between increased CRP levels and the elevation of BP was independent from RFs and CHD.
