Eine haploidente Stammzelltransplantation bietet aufgrund der schnellen Spenderverfügbarkeit und einem möglicherweise stärkeren GvL-Effekt eine vielversprechende Alternative zur Transplantation von Stammzellen eines Fremdspenders. Allerdings ist diese Transplantationsform aufgrund der HLA-Verschiedenheit und notwendigen Transplantatbearbeitung mit besonderen Risiken, wie einer erhöhten Rate an Abstoßungen, GvHD oder vermehrten Infektionen, assoziiert. In dieser Studie wurde die haploidente Stammzelltransplantation mit Depletion CD3 und CD19 positiver Zellen hinsichtlich ihrer Durchführbarkeit, Verträglichkeit und Effektivität untersucht. Die Ergebnisse bezüglich Engraftment, GvHD, Toxizität und TRM waren denen allogener Transplantationen passender Spender nicht unterlegen. Die Immunrekonstitution verlief verzögert, infektiologische Komplikationen ließen sich durch intensives Monitoring und präemptive Therapie weitgehend vermeiden. Die Zahl an Rezidiven ist weiterhin unbefriedigend, wobei das Patientenkollektiv mit einer hohen Zahl von Hochrisikopatienten beachtet werden muss.
