本研究は、山梨県の早産（多胎を除く）の実態を明らかにすることであり、実態を明らかにすることで看護への示唆を得ることを目的とした。山梨県内の２病院において平成１２年１～12月の間に早産した単胎妊婦４２名（Ａ病院24名、Ｂ病院18名）を対象とし、 retro-spective に外来および入院診療録からデータ抽出した。各々の病院の早産の占める割合は年間分娩数でみるとＡ病院3.2％、Ｂ病院10.0％であった。分娩週数、児出生体重の平均はＡ病院34.8週、2016.3ｇ、Ｂ病院31.7週、1581.0ｇであり有意差があった。また、早産の入院理由は、分娩開始50.0％、前期破水28.6％、妊娠中毒症16.7％、胎児側問題4.8％であった。また、入院後分娩に至る期間は24時間以内が38.1％、３日以内が71.4％であった。看護内容は、分娩に関する看護は全例にされていたが、入院の理由に対応した特徴的な看護ケアは抽出できなかった。進行してしまう早産に関する看護ケアを開発することが求められると共に、予防レベルでの看護（外来）が検討されることが示唆された。

初めて子育てをする11名の母親を対象に、出産後から２か月の間に体験していることを明らかにするため質的帰納的な研究を行い以下の結果を得た。母親は子ども中心の生活を送る中【手探りの状態】で、子どもの泣きの意味を考えたり、子どもの世話の基準を探し、【子どもを知る】ようになっていた。【子どもを知る】ことは、子どもの特徴や、子どもの成長を実感することである。【子どもを知る】ことは、子どもに接する時の力加減や、すぐに対応することか否かという子育てにおける【手加減が分かる】こと、【育児に慣れる】という感覚を持つことに関係していた。そして子どもの反応を見ながら【子どもと自分に合った方法を探す】状態になっていた。また、母親は家族の支援を得ることや、子どもの成長に伴う生活リズムの形成によって【自分の時間を持てる】ようになっていた。そして産後１～２か月頃の母親は【子どもの世話を引き受ける】状態になっていた。

本研究の目的は、産褥母子訪問において「子どもの泣き」を訴える母親の傾向および援助の方向性を明らかにすることである。母子保健システムを推進するために行われている産褥母子訪問の検討をもとに、訪問の実態の中で母親が困難を感じている「子どもの泣き」に注目した。平成12年７月から平成13年１月に山梨県看護協会立訪問看護ステーション助産婦が産褥母子訪問を行った母親97名の実態を分析対象とした。その結果、初経産、家族構成、退院後の生活の場、訪問時期に関わらず母親は「子どもの泣き」を訴えていた。その内容は、泣く理由がわからない、時間帯が気になる、授乳との関連といったものであった。「子どもの泣き」を訴えない母親の方が育児技術を習得できていた。これらをふまえ、助産婦は退院後早期に家庭訪問を行い、母親を支援する方向性が示唆された。

訪問看護ステーション子育て支援事業による助産婦の新生児訪問指導を受けた母親を対象に、退院後の母子の生活の実態と、受けた訪問指導の評価から、今後の出産の母子を支援するあり方を明らかにした。母親は退院後の生活の中で、心配なこと・困ったことに応じて、支援者を選びながら生活をしていた。訪問指導時に母親が相談する事柄は、子どものこと、子どもの栄養方法に関することが多い。訪問指導は、母親の退院後の生活上の心配事、不安なことを減少、消失することに役立っている。訪問時のかかわりとして、助産婦は指導的役割を取るだけではなく、母親の子育てにまつわる思いを聴くことが重要である。出産後、実家で生活する母親が6割程度おり、行政の枠を越えた退院後の母子を支援するシステム作りの必要性がある。そのための方略として、訪問看護ステーションに訪問対象を母子専門とする助産婦を配置することの有効性が示唆された。

本研究は、平成6年1月1日～平成10年12月31日の間に、A病院で分娩した双胎妊婦の妊娠中の入院に関して、入院治療が妊婦および胎児の予後にどのようになっているのか実態を明らかにすることを目的とした調査研究である。対象者は58名で、54名（93.1％）は妊娠中から入院していた。入院開始時期は平均31週であり、約40日平均入院していた。分娩は平均36週１日で、35週以前の早産は14名（24.1％）であった。妊娠合併症は切迫早産47名（80.1％）、貧血38名（65.5％）、妊娠中毒症６名（10.3％）であった。胎児の出生体重は１卵性2105.4g、２卵性2355.9gと有意差がみられた。アプガ－スコア、discordant twinに卵性の差はなかった。死産・新生児死亡は３児（2.6％）であった。また、臍帯付着異常は卵膜付着６名、辺縁付着９名に認められた。以上のような結果は、本邦の双体胎妊娠、分娩の平均的な姿を呈しており、入院の効果とは断定できない。今後、妊婦管理には一定の検査入院と外来管理のプロトコールを検討していくことが示唆された。

第３子を出産した母親に焦点をあて、第３子出産後２年間の縦断的な育児体験の記述から、第３子育児の特徴を導き出すことを目的に、1995年７～９月に第３子を出産した母親39名に、出産後１、６、12、24ヵ月の計４回、家庭訪問を行った。その結果、妊娠の計画性、妊娠希望の有無による出産後の育児に違いがみられるのは、出産直後の母親の反応のみであり、その後の育児において差異はみられなかった。また、第３子の育児の特徴は、上の子の育児体験が活かされ、熟達した育児技術がとれている点にあった。しかし、育児の大変さは３人の子どもに同時に対応する時間や空間的な余裕のなさであり、その時期は６ヵ月頃から始まり、24ヵ月まで続くことが明らかになった。, Home visits were conducted a total four times(1,6,12 and 24 months after birth) on 39 mothers who had given birth to a third child during July-September 1995; longitudinal descriptions of the child-caring experiences of these mothers in the two-year period following the birth of their third child were used to identify any special features in the bringing up of a third child. The results were follows. Difference were observed in post-natal upbringing according to the degree to which the pregnancy was planned or desired, but these differences were found to be due only to the mother's response immediately following the birth and to significant difference was observed later in the child-caring process. Further, one of the characteristic features apparent in the upbringing of a third child was the way in which the experiences gained from the elder children were put to good use and proficient child-caring techniques were adopted. However what made child-caring difficult was the lack of time and spatial freedom in dealing simultaneously with three children and it was clear that this difficult period began around the 6 month point and continued until around 24 months.

This study is part of the research on acculturation of foreigners in Japan, reporting on the accul-turation problems of 30 Chinese‘trainees’, who are involved in job training in Yamanashi Prefecture. The discussions focus mainly on their health problems and identity crisis.Recently, foreigners are seen even in the far corners of Japan; long-stay foreigners have rapidly in-creased in rural as well as urban areas, and they are more likely to have some stress in life than short-stay foreigners. While their stress, or problems, are considered to be caused by language differences, social conventions, and their own thoughts, it should be taken as our problem, which means, their problems raise a question as to what we consider to be self-evident. Living together is clearly beyond diplomacy.Hoping that relative evalution of our own culture, however little, may be carried on, we report herein the results of this study.

本研究は, 早産児の経口哺乳準備期間における口腔刺激プログラムが, 経管栄養から経口哺乳への移行において, その期間を短縮できるかなどの影響について検討することを目的とした. 在胎34週未満で出生した合併症のない早産児を対象に, 口腔刺激プログラムを行った群(以下, 口腔刺激群)とNon-Nutritive Suckingを行った群(以下, NNS群)の2群に分けた。両群の経口哺乳開始から経口哺乳成功・胃管抜去・コット移床・退院までの日数その時の日齢・修正週数・体重の有意差, および, 経口哺乳開始から7日目までの吸畷緩慢・むせ・飲みこぼし・努力呼吸・酸素飽和度の低下の症状が出現した回数の有意差を検討した. 8回/日の経口哺乳を成功するまでにかかった日数は, 口腔刺激群9±8日, NNS群6±2日, そのときの日齢は, 口腔刺激群24±6, NNS群33±12, 修正週数は口腔刺激群35週6日±1週0日, NNS群35週6日±0週3日, 体重は口腔刺激群1802±207g, NNS群1630±297gであり有意差は認められなかった. 他の項目においても2群間で有意差はなかった. 本研究の対象は在胎週数が比較的進んだ事例が多かったため, 出生時にある程度哺乳行動の準備が進んでいた可能性があり, 経管栄養から経口哺乳への移行において特別な援助を必要としなかったと考えられた., The purpose of this study was to assess whether an oral stimulation program, during the preparation period for bottle feeding, enhances the bottle feeding performance of preterm infants. Preterm infants born at less than 34 weeks' gestational age were randomized into an experimental and a control group. Infants in the experimental group received an oral stimulation program (table 1.). Infants in the control group received non-nutritive sucking. We compared the number of days from starting bottle feeding, days of life, gestational age and weight between the two groups at successful bottle feeding as well as duration of nasogastric intubation, time in an incubator, and length of hospital stay. We also compared the groups with respect to inactive sucking, choking, spilling from the mouth, difficulty breathing, and low saturation of oxygen in the 7 days from starting bottle feeding. The number of days until 8 successful bottle feedings/day was 9 ± 8 days in the oral stimulation group and 6 ± 2 days in the non-nutritive sucking group, this difference was non-significant. There were no significant intergroup differences in days of life (24 ± 6 days in the oral stimulation group vs. 33 ± 12 days in the non-nutritive sucking group), gestational age (35 weeks 6days ± 1 week, vs. 35 weeks 6 days ± 3 days), and weight (1802 ± 207 g vs. 1630 ± 297 g) at successful bottle feeding. None of the other variable measured differed significantly between the two groups. Because the subjects had almost normal growth for gestational age, their bottle feeding behavior was relatively well developed, and it was not necessary to offer specific assistance in the transition from tube to bottle feeding.
