Introduction: An evidence-based approach is relevant and indispensable for occupational therapy practice. It combines three levels: research evidence, objective knowledge through scientific literature, clinical expertise, the knowledge of therapists working in practice and the patient's perspective. In order to treat children with Down syndrome with fine motor skill problems, it is necessary to bring these three levels together. This paper aims to shed light on this to create a basis for evidence-based decisions. This article asks about occupational therapy treatment methods that are useful to enhance fine motor skills in children with Down syndrome. Methods: In the first step, the two-stage approach combines, evaluates and summarizes research evidence on the topic by means of a rapid review. In a second step, three qualitative interviews with occupational therapists working in practice were conducted, transcribed, analysed, and narratively summarized. Results: Research evidence on the question is limited, with only six studies with different study designs explicitly addressing fine motor skills in children with Down syndrome and occupational therapy options. The quality of the studies is also inadequate. Regarding clinical expertise, the three evaluated interviews discuss occupational therapy options. Conclusion: Research evidence and clinical expertise on the research question result in three main recommendations for occupational work with children with Down syndrome in order to enhance fine motor skills. Background: The inpatient Parkinson's disease multimodal complex treatment (PD-MCT) was applied more than 15,000 times in 2022, in Germany. This number is increasing as is Parkinson's disease (PD), which affects more than 400,000 people in Germany and leads to 100,000 disability-adjusted life years. In recent years, several observational studies have been conducted on the effectiveness of this kind of multidisciplinary care. Objective: To summarize and discuss the evidence on the nature, benefits and potential of PD-MCT. Methods: A narrative review of selected empirical findings was carried out. Results: The PD-MCT frequently lasts for 2–3 weeks and aims to maintain the quality of life of people with PD. Disease symptoms and activities of daily living are jointly improved by pharmacological strategies and activating therapies (physiotherapy, occupational therapy, speech and language therapy, physical training, art therapy). The PD-MCT is a useful measure to avoid or mitigate crisis situations in the course of the disease. A total of eight observational studies (n = 1246) have shown good effectiveness with a total mean improvement of the International Parkinson and Movement Disorder Society unified Parkinson's disease rating scale III (MDS-UPDRS III) by 7.8 points. The transfer of effects into everyday life through intensive and specialized community-based care must be ensured in order to achieve sustained effects on the quality of life. Ideally, this transfer can be supported by integrated PD networks and digital technologies in the future. Conclusion: There is potential for development in the standardization, patient selection and quality assurance of PD-MCT as well as in the embedding in care structures such as PD networks. Open research questions include a precise definition of the target group and higher quality evidence of short-term and long-term effectiveness. [ABSTRACT FROM AUTHOR]

Introduction: The study examined opportunities and barriers for occupations that are meaningful to unaccompanied minors. The occupational repertoire of individual adolescents was analysed and described in the context of their living environment. Method: Based on a qualitative descriptive approach the participants' experience when engaging in subjectively meaningful occupations was gathered by participant observation and semi-structured interviews. The data collected was analysed with qualitative content analysis and condensed into occupational profiles. Findings: Participants described resources and barriers for occupational participation. They appreciated opportunities for (vocational) education and for the acquisition of everday skills (e.g. handling money, cooking, washing clothes). Barriers were experienced due to life changes, the uncertain situation within the asylum procedure, laws and regulations, and scarce financial resources. The impact of these restrictions on well-being and health were significant: The young people felt dependent, lonely, reduced in their self-efficacy and ill. Conclusion: This study highlights the need for legal and infrastructural framework supporting occupational justice and occupational well-being. The implementation of healthpromoting occupational therapy services for refugees appears to be particularly important. [ABSTRACT FROM AUTHOR]

Introduction: This study investigates the dating and romantic relationship-building experience of adults with autism spectrum disorders in Germany. The study aims to examine the occupations and their meaning within the context of occupational science, to question the fulfillment of occupational justice and integrate findings into practical recommendations. Method: A qualitative research approach was conducted to capture the subjective experience in eleven guideline-based interviews. The statements were evaluated based on content analysis according to Mayring (2016). Results: The insights gained from the interviews were assigned to four categories. As a result the participants' feelings in the dating process and development of a partnership were identified. Participants reported on their individual importance of their occupations, obstacles such as pressure and lack of support, and resources such as trust and honest communication. Conclusion: The participants are facing difficulties through a lack of knowledge, communication difficulties and pressure during dating and relationship building. There is a desire for support, which occupational therapy could address through further research and development of intervention approaches. [ABSTRACT FROM AUTHOR]

Against the background of increasingly complex healthcare needs, well-trained healthcare professionals are urgently needed. New technologies in the field of digital education can both improve access to education and support the development of transformative skills. This raises the question of the didactic use of digital education in the healthcare professions, which acquire professional knowledge, practical skills, and analytical skills as part of their training. Therefore, a scoping review was conducted to investigate what is known in the literature about digital education in occupational therapy, physical therapy, and speech and language therapy. Following a systematic literature search, 100 studies in the past ten years were identified. After prescreening, 16 studies were identified to answer the research question, and for this summarized numerically and then analyzed in a qualitative synthesis. The results provide evidence of positively evaluated teaching concepts and techniques, analytical and practical skill acquisition, and proven technologies. In addition, the results highlight the diversity of digital training options and provide evidence of their effects.

Introduction: Neurofeedback is increasingly used in occupational therapy practice but is discussed controversially. Up to now, there are no studies in Germany on the effect of neurofeedback under practical conditions, as a basis for expanding the range of activities of occupational therapists with neurofeedback in a targeted and well-founded way. Accordingly, the research question of the study was: Is neurofeedback significantly effective in the context of occupational therapy practice with children with ADHD? Method: A retrospective, monocentric longitudinal study was conducted on the influence of neurofeedback in children with ADHD in an occupational setting. The ten symptoms distractibility, concentration, listening, finishing tasks, maintaining concentration, short-term memory, following instructions, organizing time and space, impulsivity, and hyperactivity were evaluated by the children or parents (n = 96) in the course of the therapy at two times of measurement. Results: The most relevant symptoms at the beginning of therapy were found for distractibility, concentration and maintaining concentration. From the beginning to the end of therapy, symptoms improved significantly individually and in the total sum score. A worsening of symptoms by at least one unit occurred at 7.29% (concentration) to 19.79% (short-term memory). Conclusions: The analysis confirms neurofeedback therapy as a promising intervention for children with ADHD and shows that boys and girls benefit comparably, with less effect in children with additional mental and behavioural disorders. Introduction: People with multiple sclerosis have a disease with a variety of symptoms, severities and courses. Regardless of the different manifestations of the disease, quality of life and participation are affected in every case. The study investigates strengthening factors described by affected persons aged 18 to 40 years in relation to quality of life and participation. Implications for occupational therapy practice are derived. Method: Participants in this exploratory study were recruited through key informants and invited to participate in problemcentered interviews. Data evaluation was carried out deductively and inductively by means of individual case analyses and comparative thematic case analysis. Findings: 14 factors strengthening quality of life and five factors supporting participation have been identified, which particularly can be assigned to psychosocial aspects. From the participants' point of view, the strengthening factors affect both quality of life and participation. Conclusion: Quality of life and participation influence each other through coping strategies. Occupational therapists should facilitate and consider coping strategies especially in vulnerable phases of the disease. Counseling and educational components with reference to objectives that focus on individual everyday life and occupational skills, coping with illness and self-determined life design should be part of occupational therapy interventions. Background: Model-guided occupational therapy allows the systematization and professionalization of professional action, but is taught to learners in Germany very heterogeneously due to different and partly outdating educational conditions. Since little is known about the implementation of model-guided occupational therapy in Germany, the aim of the study was to determine the theoretical understanding and practical application of modelguided practice among learners and newcomers to occupational therapy. Method: A Germany-wide cross-sectional study was conducted among occupational therapy students (= 3 years education) and young professionals by means of an online survey. The questionnaire contained open and closed questions as well as the processing of a short case study. Data were analysed descriptively. The analysis of the case processing was done by structuring content analysis following Kuckartz. Results: 87 questionnaires (women: n=82; < 25 years: n=71; academic education: n=52) could be included. According to the analysis occupational therapy models are only applied selectively and situationally in the participants' practice. Deficits and lack of clarity in the theoretical basis were found to inhibit the application of model-guided occupational therapy. A comprehensive, continuous application could not be shown. Conclusion: The results indicate that implementation of modelguided occupational therapy is currently only selective and uncertain which clearly illustrate the challenges in this regard. Physiotherapy and occupational therapy in stroke rehabilitation has developed over the last decades into an evidence-based therapeutically science and includes forms of therapeutic treatment in which patient-oriented decisions are to be made explicitly on the basis of proven effectiveness. The starting point of this book are observations that indicate contradictions between everyday practice and the findings of the therapeutic sciences. This results in considerable difficulties in the transfer of theory to practice. The aim of this book was to present the problem from a psychological point of view, to discuss the consequences and to think about possible ways to overcome the gap between theory and practice. The current state of research on transfer of theory to practice in neurorehabilitation and the results of problem-centered interviews with experts will be presented. In summary, the results show that therapeutic behavior is influenced by a complex interaction of opportunities, capabilities and motivation. On all three levels, there are a variety of possible approaches for improvement. There are many indications that a change in behavior is possible, but that this change requires comprehensive approaches at all levels (abilities, opportunities and motivation). The results also show that appropriate actions must be tailored to the respective settings and target groups.

Les recommandations des sociétés savantes comme la Société française de rhumatologie pour la polyarthrite rhumatoïde et le rhumatisme psoriasique mettent en avant l'ergothérapie dans une prise en charge non pharmacologique pluridisciplinaire. Elle associe la rééducation des mains et l'usage des orthèses, avec un besoin moins marqué du fait des nouvelles stratégies thérapeutiques et traitements qui réduisent destructions et déformations. Les orthèses de fonction assurent une diminution des douleurs ressenties lors des tâches quotidiennes, et les orthèses de repos nocturne de poignets et doigts assurent une antalgie appréciable. La rééducation des mains participe à l'amélioration de la qualité de vie dans les activités quotidiennes. L'avenir est aux orthèses sur mesure grâce aux imprimantes 3D. The recommendations of Scientific societies such as the French Society for Rheumatology for rheumatoid arthritis and psoriatic arthritis emphasize occupational therapy as part of a multidisciplinary, non-pharmacological approach. It combines hand rehabilitation and the use of orthoses, with a lesser need due to new therapeutic strategies and treatments that reduce destruction and deformity. Functional orthoses ensure a reduction in the pain felt during daily tasks, and night rest orthoses for the wrists and fingers provide appreciable pain relief. Hand rehabilitation helps to improve the quality of life in daily activities. The future is in custom-made orthoses thanks to 3D printers. [ABSTRACT FROM AUTHOR]

Introduction: In Switzerland, a steadily increasing number of people with mental illness receive a disability pension. People with (severe) mental illnesses are often excluded from the primary labour market or restricted in their choice of activity. As a particular method of Vocational Rehabilitation (VR), Supported Employment (SE) offers promising evidence-based possibilities, especially when applied according to the Individual Placement and Support (IPS) model. There is still considerable scope for development of SE in Switzerland. It is not yet clear what role occupational therapists (OT) in Switzerland play or could play in this regard. Study objectives were to determine how many OT in Switzerland use SE in professional (re)integration of people with (severe) mental illnesses and how these OT use SE integrating their occupational therapy skills. Method: A mainly quantitative online questionnaire was developed. OT in 268 Swiss mental health institutions/practices were asked to participate in the survey on VR in the primary labour market. Results: 82 OTs participated in the survey. 90% (74) of the participants consider the work of their clients as a field of competence in occupational therapy. 36% (30) have experience with interventions aiming at the clients' (re)integration into the primary labour market. 4% (3) use SE in their daily work, one of them uses IPS. These 4% report that SE/IPS fits well with occupational therapy, but that it can also contribute to successful and satisfying (re)integration and recommend promoting SE among OTs in Switzerland. Conclusion: There is mutual enrichment between the role as OT and the role as SE specialist in terms of resources, competences, and expertise to successfully (re)integrate clients into the labour market. Introduction: Unfortunate life events are a central part of many clients' narrations of life transitions. The Lost Possible Selves (LPS) approach offers an opportunity to take a closer look at the impact of drastic life events on the clients' self-perception and their everyday life. Since previous research on LPS has been limited to adults of advanced age, the study explores LPS and their impact on life perception in young adults and discusses potential benefits of integrating LPS in occupational therapy practice. Method: Using a questionnaire study with a mixed-methods approach, the participants answered open questions about drastic life events and the resulting LPS. The impact on life-perception was measured using standardised scales capturing life satisfaction, belief in a just world and the sense of coherence. Results: Participants experience drastic life events regarding their education, social life, and self, leading to a development of LPS in educational, social, career-oriented, and self-related contexts. Additionally, changes in life satisfaction and in the sense of coherence are shown. Conclusion: LPS are related to central aspects of occupation. In addition to the established concept of narrative reasoning, LPS offer the opportunity to explain changes in the clients' self-perception with reference to their experience of drastic life events. Introduction: Children with autism spectrum disorder (ASD) are often limited in social participation. Peer interventions to support children with ASD in interactions are widely described. However, their implementation in everyday life is rarely integrated into occupational therapy practice in German-speaking countries. The article delineates how to include peers to enhance the social participation of children between 3 and 12 years of age and how occupational therapists can enhance the cooperation between children, teachers, and parents. Methods: By performing a literature review, six studies were systematically selected and critically appraised. Relevant results were compared and embedded in the Partnering for Change model, which stands for a universal design approach in inclusive school settings. Results: Peer interventions in children with ASD are diverse. Eligibility criteria for peers are various, as are the training of peers and the implementation of peer interventions into daily practice. Common to all evaluated programs is the close collaboration of different stakeholders. Outside of the school setting, this collaboration rarely happens. Conclusion: The presented study delineates in detail how peer interventions can be planned and implemented. Introduction: Occupational therapy is part of the racist system. The purpose of this research was to examine white occupational therapists' critical reflection on racism and whiteness in the context of their occupational therapy work. Method: Qualitative interviews were conducted online with nine white occupational therapists and analysed using qualitative content analysis according to Mayring (2015). Results: The intensity of the occupational therapists' individual critical reflection on racism varies. Criticism of racism is integrated more consistently into professional action, the more it has been addressed. This is most visible on an individual level (micro level), e.g. in contact with clients, less so on an institutional (meso level) or structural level (macro level). In particular, political, ethical and interactive reasoning turns out to be significant. Conclusion: Based on the results, there is still a clear need for action at the micro, meso and macro level. A critical reflection on racism should therefore be further promoted in this professional group. [ABSTRACT FROM AUTHOR]

Introduction: Group interventions in occupational therapy (OT) are widely common in the field of adult mental health. This exploratory study examines which groups are actually offered in Austria, which competences OTs subjectively see in themselves, which further training needs they perceive, and which ideas they have for improving group interventions in their field of work. Method: As the first step in a mixed methods design, a scoping review on the topic of mental health group concepts in OT was conducted. With the aim of an explorative analysis, the answers of 92 OTs from Austria who work in this field of mental health were collected using an online questionnaire. Results: The majority of participants offer a wide variety of group interventions such as arts and crafts, cooking, or cognitive training. Concerning their skillsets, OTs see their strengths in relationship building and communication. Potential for development is perceived, among others, in conflict resolution. OTs wish to be heard more when it comes to decision making about group interventions in their respective places of work, for example concerning the balance of individual or group therapy, and the size of groups. Conclusion: Group interventions are an important tool for OT in mental health. OT students could be better prepared for conflict management and group dynamics in their basic training.
