In the last two decades, the capacity of vessels has been trebled. Currently, vessels of 23,000 TEU (20-foot equivalent unit) sail the seas. Despite the exponential growth experienced in this sector, individuals question if it is possible to reach a peak capacity, as has occurred with bulk cargo vessels and, recently, aircraft. This paper aims to predict the possible size and dimensions of a new generation of mega container ships. Based on economy of scale, port infrastructure, demand and environmental trends and naval design criteria the limit ship size has been estimated. The results suggest that it is still possible additional increases of the ship size. This paper allow Port Authorities to understand the needs of shipping container industry and to figure out the expansion and investment necessary., This research has been conducted as part of a research grant from the Port of Barcelona. Giriş: Pulmoner tromboemboli (PTE) tanı ve tedavi algoritimleri belirlenmiş olmasına rağmen hala yüksek morbidite ve mortalite ile seyreden önemli bir klinik problemdir. Amaç: Çalışmamızda, Acil Servisten PTE ön tanısı ile yatan hastaların demografik özellikleri ve mortaliteye etkili faktörler belirlenmeye çalışıldı. Metod: Çalışmamız Acil Serviste PTE ön tanısı alan 260 hastayı içermektedir. Hastalar PTE(+) (n=208), PTE(-) (n=52) olmak üzere iki gruba ayrıldı. Ayrıca PTE (+) grubumuzu da yaşayanlar (n=187) ve ölenler (n=21) şeklinde bölündü. Bu gruplarda başvuru semptomları, laboratuar değerleri, radyolojik teknik ve raporları, vital değerleri, ek hastalıkları, klinik olasılık skorlamaları incelendi. Veriler SPSS programıyla yorumlandı. Bulgular: PTE’ nin, her iki cinsiyette eşit oranda görülebildiği, yaş arttıkça görülme sıklığının arttığı ve her mevsimde rastlanabildiği saptandı. Hastalıkta en sık görülen semptomlar nefes darlığı ve göğüs ağrısı olmasına rağmen bacak ağrısı şikayeti PTE (+) hastalarda anlamlı bulundu. PTE’li hastalarda segmenter emboli daha çok görüldü. Emboli lokalizasyonu açısından önemli farklılıklar saptanmadı. PTE’li hastaların özgeçmişlerindeki ek hastalıkların PTE oluşumunda etkili olmadığı saptandı. Modifiye Geneva Skorlaması ve Wells Skorlaması parametreleri PTE tanısı için anlamlı bulundu. Modifiye Geneva Skorlaması’ndaki yüksek risk grubu, Wells trikotimizedeki orta risk grubu ve Wells dikotimizedeki hem zayıf olasılık hem de kuvvetli olasılık grupları PTE tanısı konmasında anlamlı bulundu. PTE’ li hastalarda D-dimer ve troponin düzeylerinin yüksek olduğu, arteryel kan gazı incelemelerinde en sık hipoksi+hipokapni olduğu tespit edildi. En sık rastlanılan predispozan faktörler olarak operasyon ve immobilizasyon saptandı. Postpartum periyotta olmak PTE(-)’ liği için anlamlı bir predispozan faktör olarak bulundu. Hastalarımızın %34’ünde predispozan faktör tespit edilemedi. Genel durum bozukluğu ve şuur kaybı ile gelen hastalarda mortalite anlamlı derecede yüksek bulundu. Masif ve subsegmenter tutulumun mortalitede anlamlı olduğu görüldü. Ek hastalık olarak koroner arter hastalığı (KAH) ve serebrovasküler hastalığı (SVO) olanlarda mortalite anlamlı derecede yüksek saptandı. Modifiye Geneva ve Wells Skorlaması’ nın trikotimize değerlendirilmesinde risk grupları mortalite için anlamsızken, Wells Skorlaması’ nın dikotimize değerlendirilmesindeki zayıf ve kuvvetli olasılık durumları mortalitede anlamlı bulundu. Ortalama Modifiye Geneva Skor puanı ve ortalama Wells Skor puanı mortalitede anlamsız saptandı. Troponin değerlerinin yüksekliği mortalitede anlamlı bulunurken D-dimer düzeylerinin yüksekliğinin anlamsız olduğu görüldü. Hipotansiyon ve taşikardisi olan ve aynı zamanda yoğun bakıma yatan hastalarda mortalite anlamlı derecede yüksek bulundu. Predispozan faktör olarak immobilizasyonu olan hasta grubunda mortalite anlamlı derecede yüksek saptandı. Sonuç: Nefes darlığı ve göğüs ağrısı şikayeti ile acil servislere başvuran, özellikle ileri yaşta olan hastalarda PTE’ den şüphelenilmelidir. Özellikle genel durum bozukluğu ve şuur kaybı ile gelen, troponin değerleri yüksek olan hastalar ile hipotansiyon ve taşikardisi olup, yoğun bakıma yatan hastalarda mortalite açısından daha dikkatli olunmalıdır. Anahtar kelimeler: pulmoner tromboemboli, demografik özellik, mortalite İntroduction: Pulmonary thromboembolism(PTE) is an important clinical problem that has high morbidity and mortality though the algorithms of diagnosis and treatment strategies were determined. Objective: The aim of this study was to evalute the demographic characteristics of patients who were diagnosied as PTE at emergency department and the factors which affect the mortality. Methods: This study included 260 patients who were dignosied as PTE at emergency department. The patients were allocated into two groups; 208 patients with PTE(+) and 52 patients with PTE(-). The PTE(+) patients were subgrouped as alive (187 patients) and dead (21 patients). All the groups were evaluted for the symptoms onset, laboratory parameters, radiological techniques and reports, vital parameters, accompanying disease and scores of clinical probability. The data were analysied with SPSS programme. Results: PTE was determined in both sexes with no significant differences, the incidence increased by age and no significant difference was noted by the season. Although the most common complaints were dyspnea and chest pain in PTE(+) patients , pain in the leg was found significant. Segmental embolism was more common in PTE(+) patients. There was no significant differences in terms of the location of embolism. It was found that accompanying diseases had no effects on the occurrence of PTE. Scoring methods of Modified Geneva’s and Wells’ parameters were found significant for PTE. High risk group of Modified Geneva Scoring, medium risk group of Wells’ tricotimized risk assessment, unlikely and likely risk groups of Wells’ dicotimized risk assesment were significant for PTE. D-dimer and troponin levels were determined to be high in patients with PTE. Hypoxia+hypocapnia was the most common value determined in the analysis of the arterial blood gas. Surgery and immobilization were the most frequently predisposing factors for PTE(+). It was found significant to be at postpartium period for PTE(-). Because no prediposing factors were detected in 34% of PTE(+) patients, other possible predisposing factors might be investegated. The mortality was found significantly high in patients who were admitted to hospital with general situation disorder and loss of conscience. Massive and subsegmental eclipse were significant for mortality. Mortality was determined to be significantly high in patients with coronary arterial disease and cerebrovascular disease as accompaying disease. All the risk groups of Scoring Modified Geneva and scoring tricotimized Wells had no significant effect on mortality but risk groups as likely and unlikely of scoring dicotimized Wells were significant for mortality. Points of Scoring Modified Geneva and Wells had no significant effect on mortality. Although the levels of troponin were determined to be significant for mortality, the levels of D-dimer were not. Mortality was significantly high in patients who were admitted to intensive care unit due to hypotension and tachycardia. Mortality was significantly high in patients with immobility as prediposing factor. Conclusions: Suspicion should be raised for PTE in patients, especially elderly, who were admitted to emergency department with the complaints of dyspne and chest pain. In particular, extra care is required in terms of mortality in patients with high troponin values, general situation disorder and loss of consience, and those that were admitted to intensive care unit due to hypotension and tachycardia. Toplantı ve gösteri yürüyüşü hakkı ve özgürlüğünü konu alan çalışmamızın amacı,toplantı ve gösteri yürüyüşü hakkı ve özgürlüğünü, tarihsel süreci içerisinde değerlendirmek,yer aldığı uluslararası temel belge ve sözleşmeler kapsamında ve Türk hukuk sistemimiziçerisindeki mevzuat hükümleriyle anlatmak; AİHM kararları ışığında ve Türkiye'dekiuygulamalar çerçevesinde izah etmektir.Tezimizin birinci bölümünde toplantı ve gösteri yürüyüşü hakkı ve özgürlüğününtarihsel gelişimine yer verilmiştir. Tez çerçevesinde toplantı ve gösteri yürüyüşü hakkı veözgürlüğünün geçmiş dönemlerde ele alınış şekline değinilmiş olup bu hak ve özgürlük Türkhukuk sistemindeki anayasal gelişmeler de temel alınarak irdelenmiştir.Tezin ikinci bölümünde 1982 Anayasası'na göre bu hak ve özgürlüğün hukuki açıdanirdelenmesine yer verilmiş olup toplantı ve gösteri yürüyüşünün diğer hak ve özgürlüklerleilişkisi de ele alınarak konu, 1982 Anayasası, 2911 sayılı Toplantı ve Gösteri YürüyüşleriKanunu ve AİHS çerçevesinde değerlendirilmiştir.Son olarak sonuç bölümünde ise; toplantı ve gösteri yürüyüşü hakkı ve özgürlüğününgenel bir değerlendirmesi yapılarak konuyla ilgili görüşler yer almaktadır.Anahtar Kelimeler: Hak, Özgürlük, Toplantı ve Gösteri Yürüyüşü Hakkı ve Özgürlüğü, 2911 sayılı Toplantı ve Gösteri Yürüyüşleri Kanunu The main purpose of this study which subjects the right and the freedom of assemblyand demonstration is to evaluate this right and freedom in its historic process, to express it inthe scope of basic international documents and conventions and within the legistation ofTurkish legal system, and to clarify it in the light of the decisions of European Court ofHuman Rights (ECHR), interpret the practice of sentences passed by those organizations.In the first part of thesis, the historical development of the right and the freedom ofassembly and demonstration is mentioned. Also, the way how right and freedom of assemblyand demonstration are approached in the past, and the examination of these rights based onconstitutional progress in Turkish legal system are discussed in the first part.The second part of this work comprised of the legal examination of this right andfreedom according to 1982 constitution, and the relation between the right and freedom ofassembly and demonstration, and other rights and freedoms are approached. Samet HAŞILOĞLU Geleneksel kontrol stratejileri, işlemlerin en ince detayına kadar tanımlanabildiği durumlar için uygun sistemlerdir. Ancak, sistem davranışını karakterize etmede güçlüklerin olduğu ya da tahmin edilemeyen çevresel etkilerin olduğu durumlarda bu stratejileri uygulamak son derece zordur. Bu tür sistemler için neuro-fuzzy yöntemleri alternatif bir yaklaşımdır. Neuro-fuzzy sistemlerinin kurulmasında, uygun bulanık kuralları bulmak gibi bazı zorluklar vardır. Bu zorluklan aşmak için öğrenme kabiliyeti olan sistemler kullanılmaktadır. Atıksu arıtımı ve geri dönüşümü günümüzün en önemli problemlerinden biridir. Sistem içinde bulunan ve kontrol edilmesi gereken bir çok değişkenin kompleks yapıda olması ve atıksu arıtım tesisleri girişinde sürekli bir değişime uğraması, atıksu arıtma tesislerinin yapay zeka modelleri kullanılarak kontrol edilmesini ön plana çıkarmaktadır. Bu çalışmada yapay zeka modellerinden neuro-fuzzy kontrol metodu kullanarak bir atıksu arıtım tesisinin kontrol edilmesine çalışıldı. Değişik öğrenme metodları kullanılarak bilgisayarda simulasyonu yapıldı ve sonuçlan karşılaştmldı. However, if there are difficulties to characterize the behaviour of the system or unpredictable environmental effects, it is very difficult to apply these strategies. For these types of systems, fuzzy logic method is an alternative. In construction of fuzzy logic systems, there are some difficulties such as finding suitable fuzzy rules. To cope with these difficulties, systems having learning ability are used. Wastewater treatment and recycling is one of the today's most important problems. Due to the complex structures of the variables in the system and their continuously changing at the entrance of the wastewater treatment plants, to use artificial intelligence methods is becoming a matter of primary importance to control wastewater treatment plants. In this study, neuro-fuzzy method which is one of the articial intelligence models, is used to control a waste water treatment plant. By using different learning methods, these models are simulated on the computer and results are compared. Mesanenin karsinosarkom veya sarkomatoid karsinomu kötü prognozlu olup, agresif cerrahi tedaviye rağmen hastalar kısa sürede kaybedilmektedir. Bu makalede patolojisi sarkomatoid ürotelyal karsinom gelen 73 yaşında erkek hastada, mesane koruyucu yaklaşımla yapılan tedavi ve hastalıksız olarak geçen 27 aylık süreç sonrası beyin damar hastalığı nedeniyle ölen olgu sunulmuştur, Carcinosarcoma or sarcomatoid carcinoma of the urinary bladder has a poor prognosis with a short time from diagnosis to death despite aggressive treatment. In this case, we report a 73 years-old man with the pathological diagnosis of sarcomatoid urothelial carcinoma of urinary bladder who survived without recurrence for 27 months after bladder sparing treatment and died due to cerebrovascular disease
