Decision-making about cardiopulmonary resuscitation in nursing homes happens in an area of conflict between advance planning and ad hoc decision when cardiac arrest occurs. Responsibility is distributed among nursing home residents, nurses and family physicians. In advance planning, shared decision making is recommended, but it is restricted by different obstacles. In the reality of care, the responsibility for decisions about resuscitation attempts is often shouldered by nurses. To distinguish indication for resuscitation attempts from patients' wishes more clearly, family physicians could assess the medical indication ex ante. This would improve integration of medical expertise in decision-making about resuscitation attempts in both situations, when cardiac arrest occured and in advance planning. Exerting such ex ante assessment, family physicians would introduce their medical competence into advance planning, which could promote personal autonomy of nursing home residents. The result of an ex ante assessment can be integrated in an advance care planning process and can provide a beginning of such a process. An ex ante assessment of the medical indication for resuscitation attempts can be exerted by family physicians without further ado. Specific research could promote dissemination.
