Inzidenz und Pravalenz eines Diabetes mellitus bei Mukoviszidose (zystische Fibrose, „cystic fibrosis“ [CF]) steigen mit zunehmender Lebenserwartung. Der CFRD („cystic fibrosis related diabetes“) unterscheidet sich von anderen bekannten Diabetestypen. Neben ihm konnen CF-Patienten auch unter Steroidtherapie bzw. im Rahmen akuter Infektionen oder wahrend der Schwangerschaft (Gestationsdiabetes), dann meistens vorubergehend, einen ggf. auch insulinpflichtigen Diabetes mellitus entwickeln. Frauen mit CF erkranken haufiger an einem Diabetes mellitus als Manner mit CF. In der Regel begleitet der Diabetes mellitus die Betroffenen lebenslang. Deshalb sind ein umfangreiches Wissen und eine gute Patientenschulung von groser Bedeutung. Da CF-Patienten mit einem Diabetes mellitus einen schlechteren Krankheitsverlauf und eine hohere Mortalitat aufweisen, ist die fruhzeitige Diagnosestellung sehr wichtig, um von Anfang an eine adaquate Behandlung einzuleiten. Ein gut therapierter Diabetes mellitus wirkt sich auf den weiteren Krankheitsverlauf positiv aus. Die inzwischen guten Kenntnisse uber die Pathogenese, den Verlauf und die Therapie erlauben eine gute Kontrolle des Diabetes mellitus. Dies fuhrt zu einer Verbesserung der Lebensqualitat und zu einer Erhohung der Lebenserwartung. Patienten mit CF und einem Diabetes mellitus mussen keine strenge Diat einhalten. Wichtig sind vielmehr bewusstes Essen, ausgewogene Mahlzeiten und eine gute Kenntnis der Nahrungsmittel in Bezug auf die Verstoffwechselung von Kohlenhydraten, Fetten und Eiweis. Die Therapie der Wahl ist die (intensivierte) Insulintherapie. In der Initialphase kann der Einsatz von Repaglinid temporar eine Alternative darstellen.

Background Although blood cultures (BCs) are the "gold standard" for detecting bacteremia, the utility of BCs in patients with community-acquired pneumonia (CAP) is controversial. This study describes the proportion of patients with CAP and afebrile bacteremia and identifies the clinical characteristics predicting the necessity for BCs in patients who are afebrile. Methods Bacteremia rates were determined in 4,349 patients with CAP enrolled in the multinational cohort study The Competence Network of Community-Acquired Pneumonia (CAPNETZ) and stratified by presence of fever at first patient contact. Independent predictors of bacteremia in patients who were afebrile were determined using logistic regression analysis. Results Bacteremic pneumonia was present in 190 of 2,116 patients who were febrile (8.9%), 101 of 2,149 patients who were afebrile (4.7%), and one of 23 patients with hypothermia (4.3%). Bacteremia rates increased with the CURB-65 score from 3.5% in patients with CURB-65 score of 0 to 17.1% in patients with CURB-65 score of 4. Patients with afebrile bacteremia exhibited the highest 28-day mortality rate (9.9%). Positive pneumococcal urinary antigen test (adjusted OR [AOR], 4.6; 95% CI, 2.6-8.2), C-reactive protein level > 200 mg/L (AOR, 3.1; 95% CI, 1.9-5.2), and BUN level ≥ 30 mg/dL (AOR, 3.1; 95% CI, 1.9-5.3) were independent positive predictors, and antibiotic pretreatment (AOR, 0.3; 95% CI, 0.1-0.6) was an independent negative predictor of bacteremia in patients who were afebrile. Conclusions A relevant proportion of patients with bacteremic CAP was afebrile. These patients had an increased mortality rate compared with patients with febrile bacteremia or nonbacteremic pneumonia. Therefore, the relevance of fever as an indicator for BC necessity merits reconsideration.

Hintergrund Das mobile (m) Health-Telemonitoring beschreibt eine Form der Telemedizin, bei der Smartphones, entsprechende mobile Apps und Sensoren zur Fernüberwachung von PatientInnen genutzt werden. Diese Technologie könnte besonders bei BewohnerInnen von Alters- und Pflegeeinrichtungen vorteilhaft sein. Aufgrund von Alter, Grunderkrankungen und eingeschränkter Mobilität sind diese BewohnerInnen gegenüber pulmonalen Infekten, inkl. COVID-19, besonders empfindlich. Ziel Entwicklung eines Konzepts zur Implementierung von mHealth Telemonitoring des kardiopulmonalen Systems bei BewohnerInnen von Alters- und Pflegeeinrichtungen. Studie Bei den BewohnerInnen werden über einen Zeitraum von 2 bis 12 Wochen mit den zur Verfügung gestellten digitalen Sensoren (Pulsoxymeter, Blutdruckmessgerät, Spirometer, Aktivitätstracker) tägliche Messungen durchgeführt. Aufgrund der erwarteten geringen digitalen Kenntnis der BewohnerInnen, wird das Pflegepersonal in die Messungen mit involviert und geschult. Bei Bedarf werden den BewohnerInnen Smartphones auf Leihbasis (eingelegt über Scheinaccounts) zur Verfügung gestellt. Die Akzeptanz des mHealth-Telemonitorings wird bei BewohnerInnen, Pflegepersonal und zuständigen Allgemein- bzw Fachärzten über eine Mixed-Methods Studie evaluiert, angelehnt an das Technologieakzeptanzmodell. Erwartete Ergebnisse und Schlussfolgerung Aufgrund der mangelnden digitalen Kenntnisse können ProbandInnen Probleme im Umgang mit den digitalen Sensoren haben. Der geistige bzw. körperliche Gesundheitszustand kann eine Herausforderung darstellen. Die Pflegekräfte könnten aufgrund Arbeitsbelastung nicht in der Lage sein, eine ausreichende Unterstützung zu leisten. Das betreuende Medizinpersonal könnte es schwierig finden, die Telemedizin mit ihrer üblichen Versorgung zu verbinden, nicht zuletzt wegen eines aktuell unklaren Kostenerstattungsmodells. Durch Beobachtung und Lösung der Hürden, sowie durch die Mixed-Methods Untersuchung, wird ein Konzept zur Implementierung vom mHealth Telemonitoring des kardiopulmonalen Systems in Alters- und Pflegeeinrichtungen vorangebracht.

The present guideline provides a new and updated concept of the management of adult patients with community-acquired pneumonia. It replaces the previous guideline dating from 2016.The guideline was worked out and agreed on following the standards of methodology of a S3-guideline. This includes a systematic literature search and grading, a structured discussion of recommendations supported by the literature as well as the declaration and assessment of potential conflicts of interests.The guideline has a focus on specific clinical circumstances, an update on severity assessment, and includes recommendations for an individualized selection of antimicrobial treatment.The recommendations aim at the same time at a structured assessment of risk for adverse outcome as well as an early determination of treatment goals in order to reduce mortality in patients with curative treatment goal and to provide palliation for patients with treatment restrictions.Die vorliegende Leitlinie umfasst ein aktualisiertes Konzept der Behandlung und Prävention von erwachsenen Patienten mit ambulant erworbener Pneumonie und löst die bisherige Leitlinie aus dem Jahre 2016 ab.Sie wurde entsprechend den Maßgaben zur Methodologie einer S3-Leitlinie erarbeitet und verabschiedet. Hierzu gehören eine systematische Literaturrecherche und -bewertung, die strukturierte Diskussion der aus der Literatur begründbaren Empfehlungen sowie eine Offenlegung und Bewertung möglicher Interessenskonflikte.Die Leitlinie zeichnet sich aus durch eine Zentrierung auf definierte klinische Situationen, eine aktualisierte Maßgabe der Schweregradbestimmung sowie Empfehlungen zu einer individualisierten Auswahl der initialen antimikrobiellen Therapie.Die Empfehlungen zielen gleichzeitig auf eine strukturierte Risikoevaluation als auch auf eine frühzeitige Bestimmung des Therapieziels, um einerseits bei kurativem Therapieziel die Letalität der Erkrankung zu reduzieren, andererseits bei palliativem Therapieziel eine palliative Therapie zu eröffnen.

Dyspnoea is a frequent and compromising symptom in patients with advanced and metastatic lung cancer. Exogenous as well as endogenous factors contribute to development of shortness of breath. Knowledge of these influences is essential for effective treatment of this important symptom. In our study, we evaluated the influence of cofactors and comorbidity on development of dyspnoea in lung cancer patients for the purpose of effective therapy of shortness of breath in this target group. In this prospective monocentric study, we registered severity of dyspnoea in advanced lung cancer patients using the modified Medical Research Council-Scale (mMRC-scale). Patients' history of COPD and cardiopulmonary comorbidity was recorded using a standardized questionnaire. Moreover, cofactors such as pain or cancer-induced mental stress were documented by visual rating scale. 25 (48,1 %) of 52 recruited lung cancer-patients reported moderate or severe dyspnoea. In logistic regression analysis history of COPD or cardiopulmonary comorbidity, severe pain, airway obstruction or pleural effusion were associated with severe dyspnoea (mMRC-scale ≥ 3). Furthermore, in our study cohort lower cancer level III UICC and absence of metastasis correlated with severe dyspnoea. Our findings confirm the relevance of dyspnoea in patients with advanced lung cancer. Probability of occurrence is influenced by comorbidity and cofactors. The knowledge of these factors contributes to better understanding of occurrence and treatment of dyspnoea.ZIEL: Dyspnoe ist ein häufiges und schwer beeinträchtigendes Krankheitssymptom bei Patienten mit fortgeschrittenem Lungenkarzinom. Exogene wie endogene Faktoren tragen zum subjektiven Symptom Luftnot bei. Um eine effektive Therapie einleiten zu können, ist die Kenntnis von beeinflussbaren Faktoren essenziell. Mit dem Ziel, die Atemnot gezielter behandeln zu können, wurde in dieser Arbeit die Auswirkung von Begleitfaktoren und Komorbiditäten auf den Schweregrad von Luftnot bei Patienten mit Lungenkarzinom untersucht. Im Rahmen einer prospektiven, monozentrischen Beobachtungsstudie wurde der Schweregrad der Atemnot bei Patienten mit fortgeschrittenem Lungenkarzinom anhand des mMRC-Scores abgefragt und standardisiert nach kardialer oder pulmonaler Komorbidität befragt. Zudem wurden Schmerzsymptomatik und psychische Belastung der Patienten durch die Tumorerkrankung anhand numerischer Ratingskalen (NRS) abgefragt. 25 (48,1 %) von 52 eingeschlossenen Lungenkarzinompatienten gaben mäßige bis starke Dyspnoe an. Vorbekannte COPD, kardiopulmonale Begleiterkrankungen, starke Schmerzen, eine obstruktive Ventilationsstörung in der Lungenfunktion sowie nachweisbare Pleuraergüsse korrelierten in der logistischen Regressionsanalyse mit dem Auftreten von Dyspnoeschweregraden ≥ 3 in der mMRC-Skala. In dieser Untersuchung war auch das niedrigere UICC-Stadium III bzw. eine M0-Situation mit einer höheren Wahrscheinlichkeit einer schweren Dyspnoe korreliert als ein höheres Stadium IV nach UICC. Unsere Untersuchung bestätigt die klinische Relevanz von Dyspnoe bei Lungenkarzinompatienten. Die Wahrscheinlichkeit für das Auftreten dyspnoeischer Beschwerden wird durch Begleitsymptome wie starke Schmerzen und insbesondere durch das Vorhandensein von kardialen oder pulmonalen Komorbiditäten wesentlich beeinflusst. Eine effektive Behandlung von Luftnot sollte daher diese begleitenden Faktoren in der Therapie berücksichtigen.

Here, however, by directly monitoring the photoinduced collapse of the spectral gap in a canonical charge-density-wave material, the blue bronze Rb$_{0.3}$MoO$_3$, we find that ultrafast (∼60 fs) vibrational disordering due to efficient hot-electron energy dissipation quenches the gap significantly faster than the typical structural bottleneck time corresponding to one half-cycle oscillation (∼315 fs) of the coherent charge-density-wave amplitude mode. This result not only demonstrates the importance of incoherent lattice motion in the photoinduced quenching of electronic order, but also resolves the perennial debate about the nature of the spectral gap in a coupled electron-lattice system., Published by APS, College Park, Md.

A straightforward and rapid high-performance liquid chromatography–tandem mass spectrometry (HPLC–MS/MS) assay allowing the sensitive and selective quantitation of finerenone (BAY 94–8862) in lithium heparin human plasma is described. Finerenone is a novel, selective, nonsteroidal mineralocorticoid receptor antagonist that is in phase III clinical trials for the treatment of chronic kidney disease. Finerenone quantitation is performed after addition of its stable isotope-labelled internal standard (ISTD) by protein precipitation with acidified acetonitrile followed by HPLC–MS/MS separation and detection. The determination of finerenone concentrations was validated for a plasma volume of 0.100 mL and subsequently also for a lower plasma volume of 0.010 mL, collected e.g. in paediatric studies. The analytical range was from 0.100 µg/L (lower limit of quantification) to 200 µg/L (upper limit of quantification). Inter-day accuracy was 99.7–105.0% for the plasma volume of 0.100 mL and 101.1–104.5% for the plasma volume of 0.010 mL. Inter-day precision was ≤ 7.0%, independent of the extracted plasma volume. A moderate, concentration-independent matrix effect on ionisation was observed for both finerenone and its ISTD of 0.535–0.617, which is fully compensated by the ISTD (ISTD-normalised matrix factors were 0.98–1.03). The assay was successfully applied with both validated plasma volumes to a clinical phase I study in which the pharmacokinetics of 20 mg finerenone were compared in capillary plasma (0.010 mL) and venous plasma (0.100 mL) in a concentration range from the lower limit of quantification to 310 µg/L (capillary plasma) and 252 µg/L (venous plasma). The area under the plasma concentration versus time curve was similar in both matrices, while maximum concentrations were 37% higher in capillary plasma. In conclusion, capillary sampling should not bias pharmacokinetic exposure estimates compared with venous plasma values, if limited to sampling times in the distribution and elimination phases of finerenone.

A significant proportion of the current technological developments in pneumology originate from the various areas of information technology. The spectrum ranges from smartphone apps to be used in daily life or in patient care to the use of artificial intelligence in screening and early detection of diseases. The diagnostic accuracy of apps for symptom analysis is currently very limited. Research projects are performed on the integration of symptoms and functional parameters into early detection, but also on mobility measurements as a prognostic marker in COPD. Lung cancer screening using computed tomography represents a major challenge. Here, artificial intelligence can help radiologists to cope with huge amounts of data. However, the quality of the software depends on the sufficient training of the system. Technological developments shape all fields of pneumology. For diagnostic and interventional endoscopy, they offer improved biopsy techniques and microstructural imaging. Advances in lung function measurements allow the differentiated analysis of respiratory mechanical disorders, and they could be transferred to ventilation technology. The translation of basic findings about the lung microbiome into patient care may perspectively help to better understand and treat COPD exacerbations.Ein wesentlicher Anteil der aktuellen technologischen Entwicklungen in der Pneumologie liegt in den verschiedenen Bereichen der Informationstechnologie. Das Spektrum reicht dabei von Smartphone-Apps, die im täglichen Leben oder der Praxis von Patienten oder Ärzten angewandt werden sollen, bis hin zum Einsatz der künstlichen Intelligenz in der Früherkennung. Die Diagnose-Genauigkeit von Apps zur Symptomanalyse ist dabei zurzeit noch sehr limitiert. Forschungsprojekte beschäftigen sich mit der Integration von Symptomen und Funktionsparametern in der Früherkennung, aber auch mit der Mobilitätserfassung als prognostischem Marker bei der COPD. Eine große Herausforderung stellt das Lungenkrebs-Screening mittels Computertomografie dar. Hier kann künstliche Intelligenz helfen, riesige Datenmengen zu bewältigen. Die Qualität hängt jedoch vom suffizienten Training der Systeme ab. Technologische Entwicklungen prägen alle Felder der Pneumologie. Sie erlauben in der diagnostischen und interventionellen Endoskopie die verbesserte Biopsietechnik und mikrostrukturelle Bildgebung. Methoden der Lungenfunktionsdiagnostik ermöglichen die differenzierte Analyse von atemmechanischen Störungen und können in die Beatmungstechnologie überführt werden. Die Translation von Grundlagenerkenntnissen zum Mikrobiom kann perspektivisch helfen, COPD-Exazerbationen besser zu verstehen und zielgerichteter zu behandeln.

Mass balance and biotransformation of finerenone, a nonsteroidal mineralocorticoid receptor antagonist, were investigated in four healthy male volunteers following a single oral administration of 10 mg (78 μCi) of [14C]finerenone and compared with data from studies in dogs and rats. The total recovery of the administered radioactivity was 101% in humans, 94.7% in dogs, and 95.2% in rats. In humans, radioactivity was mainly excreted renally (80%); in rats, it was primarily the biliary/fecal route (76%); and in dogs, excretion was more balanced. Finerenone was extensively metabolized in all species by oxidative biotransformation, with minor amounts of unchanged drug in excreta (humans: 1%; dogs, rats: 79%) of each metabolite predominated in all three species. In summary, the present study demonstrates that finerenone is cleared by oxidative biotransformation, mainly via naphthyridine derivatives.

Purpose To determine the pharmacokinetics of radiolabeled copanlisib (BAY 80-6946) in healthy male volunteers and to investigate the disposition and biotransformation of copanlisib. Methods A single dose of 12 mg copanlisib containing 2.76 MBq [14C]copanlisib was administered as a 1-h intravenous infusion to 6 volunteers with subsequent sampling up to 34 days. Blood, plasma, urine and feces were collected to monitor total radioactivity, parent compound and metabolites. Results Copanlisib treatment was well tolerated. Copanlisib was rapidly distributed throughout the body with a volume distribution of 1870 L and an elimination half-life of 52.1-h (range 40.4–67.5-h). Copanlisib was the predominant component in human plasma (84% of total radioactivity AUC) and the morpholinone metabolite M1 was the only circulating metabolite (about 5%). Excretion of drug-derived radioactivity based on all 6 subjects was 86% of the dose within a collection interval of 20–34 days with 64% excreted into feces as major route of elimination and 22% into urine. Unchanged copanlisib was the main component excreted into urine (15% of dose) and feces (30% of dose). Excreted metabolites (41% of dose) of copanlisib resulted from oxidative biotransformation. Conclusions Copanlisib was eliminated predominantly in the feces compared to urine as well as by hepatic biotransformation, suggesting that the clearance of copanlisib would more likely be affected by hepatic impairment than by renal dysfunction. The dual mode of elimination via unchanged excretion of copanlisib and oxidative metabolism decreases the risk of clinically relevant PK-related drug–drug interactions.

We employ time- and angle-resolved photoemission spectroscopy to study the spin- and valley-selective photoexcitation and dynamics of free carriers at the $\overline{\mathit{K}}$ and ${\overline{\mathit{K}}}^{\ensuremath{'}}$ points in singly oriented single-layer ${\mathrm{WS}}_{2}/\mathrm{Au}(111)$. Our results reveal that in the valence band maximum an ultimate valley polarization of free holes of 84% can be achieved upon excitation with circularly polarized light at room temperature. Notably, we observe a significantly smaller valley polarization for the photoexcited free electrons in the conduction band minimum. Clear differences in the carrier dynamics between electrons and holes imply intervalley scattering processes into dark states being responsible for the efficient depolarization of the excited electron population.

Changes of the electronic structure of FeTe1−xSex (x = 0, 0.3) upon ultrafast excitation with near-infrared laser pulses are investigated using time- and angle-resolved extreme ultraviolet photoemission spectroscopy. At different temperatures and for different doping levels we observe a global oscillation of energy bands near the Fermi energy driven by the coherent excitation of the A1g(Te) phonon mode in FeTe1−xSex. Prominently, photo-induced band shifts and a spectral weight reduction persist for at least 300 ps, long after the decay of the photo-excited carriers and the damping of the coherent phonons. We argue that the system escapes from the original equilibrium state and is transiently trapped in a possibly metastable state envisioning a purely optical means of manipulating the electronic structure in an iron-based superconductor.

The formation and 29Si NMR spectroscopic characterization of silicon cations that are intramolecularly stabilized by a dialkyl thioether are described. The chemical stability of the silicon–sulfur Lewis pair and, hence, the viability of the approach, were probed with a 2-[(alkylthio)methyl]phenyl-substituted hydrosilane as a proxy before three different motifs with chiral binaphthyl backbones were prepared in multistep sequences. The degree of shielding of the silicon atom in these cations was found to depend on the substitution pattern at the silicon atom and the ring size generated by the silicon–sulfur interaction. These sulfur-stabilized silicon cations are sufficiently reactive to promote Diels–Alder reactions of cyclohexa-1,3-diene with various dienophiles; the same set of reactions with cyclopentadiene is also reported. One of the three chiral Lewis acids induces low, but promising, enantioselectivity, and 24% ee is the highest value so far obtained with a cationic tetracoordinate silicon catalyst.

Nosocomial pneumonia (HAP) is a frequent complication of hospital care. Most data are available on ventilator-associated pneumonia. However, infections on general wards are increasing. A central issue are infections with multidrug resistant (MDR) pathogens which are difficult to treat in the empirical setting potentially leading to inappropriate use of antimicrobial therapy.This guideline update was compiled by an interdisciplinary group on the basis of a systematic literature review. Recommendations are made according to GRADE giving guidance for the diagnosis and treatment of HAP on the basis of quality of evidence and benefit/risk ratio.This guideline has two parts. First an update on epidemiology, spectrum of pathogens and antimicrobials is provided. In the second part recommendations for the management of diagnosis and treatment are given. New recommendations with respect to imaging, diagnosis of nosocomial viral pneumonia and prolonged infusion of antibacterial drugs have been added. The statements to risk factors for infections with MDR pathogens and recommendations for monotherapy vs combination therapy have been actualised. The importance of structured deescalation concepts and limitation of treatment duration is emphasized.Die nosokomiale Pneumonie bleibt eine häufige Komplikation von Krankenhausaufenthalten. Die meisten Daten liegen zur beatmungsassoziierten Pneumonie vor, aber auch auf Normalstationen ist vermehrt mit dieser Erkrankung zu rechnen. Problematisch ist die Zunahme von Infektionen mit multiresistenten Erregern (MRE). Diese erschwert die adäquate Initialtherapie und kann zu unkritischem Einsatz von Breitspektrumantibiotika führen.Das vorliegende Update der S3-Leitlinie von 2012 wurde von einer interdisziplinären Arbeitsgruppe auf der Basis einer systematischen Literaturrecherche erarbeitet. Die Empfehlungen zu Diagnostik und Therapie wurden nach dem GRADE-System abgefasst. Graduierung und Empfehlungsstärke entsprechen der Methodik der Nationalen Versorgungsleitlinien.Die Leitlinie besteht aus zwei Teilen. Der allgemeine Teil vermittelt einen aktuellen Überblick zu Epidemiologie, Erregerspektrum und Therapieoptionen der nosokomialen Pneumonie. Handlungsorientierte Empfehlungen zu Diagnostik und Therapie werden im zweiten Teil abgegeben. Neu in diesem Update sind detailliertere Empfehlungen zur bildgebenden Diagnostik, zur Diagnostik nosokomialer Virusinfektionen sowie zur prolongierten Applikation von Antiinfektiva. Die Angaben zur Bedeutung von Risikofaktoren für Infektionen mit MRE und die Empfehlungen zum Einsatz einer antibakteriellen Kombinationstherapie bei derartigen Infektionen wurden angepasst. Strukturierte Deeskalationskonzepte und eine konsequente Begrenzung der Therapiedauer zur Verminderung des Selektionsdrucks werden besonders betont.

Within the first 50 fs after excitation, a sequence of time frames is resolved that are characterized by different energy and momentum exchange processes among the involved photonic, electronic, and phononic degrees of freedom. The results reveal experimentally the complexity of the transition from a nascent nonthermal towards a thermal electron distribution due to the different timescales associated with the involved interaction processes., Published by APS, College Park, Md.

The ultrafast spectral responses of a charge-ordered state in 1 T -TiSe 2 to the photo-excitation with laser-pulses of 790 nm and 395 nm center wavelength are compared. Time- and angle-resolved photoemission reveals pronounced differences in the energy-momentum distribution of the nascent electron gas; a distinct effect of the distribution character on the destruction dynamics of the charge-ordered state is, however, not observed.

The synthesis and spectroscopic characterization of previously unprecedented sulfur-stabilized silicon cations are reported. Several 1,3-dithiolan-2-yl- and 1,3-dithian-2-yl-substituted silanes were prepared and successfully transformed into the corresponding silicon cations by hydride abstraction. The silicon–sulfur interaction creates three consecutive stereocenters at three different elements. It is remarkable that the present stereocenter at the silicon atom determines the stereochemical outcome at the formerly prochiral sulfur and carbon atoms with excellent diastereoselectivity. All sulfur-stabilized silicon cations are shown to be potent catalysts in a challenging Diels–Alder reaction. Moreover, structurally related oxazoline-stabilized silicon cations were generated and characterized but found to be unreactive.

The 29Si NMR chemical shifts of ferrocene-stabilized silicon cations span a wide range depending on the substituents at the silicon atom. These pronounced differences in deshielding of the silicon atom do not translate into significant differences in their catalytic activity in Diels–Alder reactions. It was shown by Lewis pair formation with Lewis base probes (Et3PO and pyridine-d5) that there is hardly any difference between these silicon cations after coordination to a Lewis base. This finding not only thwarts experimental quantification of the Lewis acidity of the free Lewis acids but also demonstrates that the reactivity differences are largely due to steric effects for a given counteranion. These observations are further verified by a ReactIR kinetic analysis. The Lewis acidity of silicon cations and their performance as catalysts cannot be correlated with 29Si NMR chemical shifts as well as resonances of adducts with Lewis base probes, not even for a subset of silicon Lewis acids.

The ultrafast dynamics of excited electrons in 1T-TiSe2 after absorption of a 390 nm light pulse is probed by time- and angle-resolved photoemission spectroscopy using femtosecond XUV pulses. It is demonstrated that the experimental approach can provide a comprehensive view of hot carrier motion in momentum space during relaxation back to equilibrium. This capability opens a new avenue in the investigation of energy dissipation processes in solids after intense optical excitation.

The layered transition-metal dichalcogenide MoS${}_{2}$ as well as related compounds, such as WS${}_{2}$ and WSe${}_{2}$, undergo a transition from an indirect to a direct band-gap semiconductor when thinned down to a single layer. This unexpected observation, which goes along with substantial changes in electronic and optical properties, has stimulated numerous recent studies on this compound class, including pump-probe time-domain studies addressing fundamental aspects of hot carrier relaxation. So far, mainly all-optical methods have been applied to probe the ultrafast dynamics in a rather comprehensive manner. Due to intermixing of different processes and a lack of momentum-resolution, the interpretation of such data is, however, not necessarily straightforward. In order to complement these studies, the authors present here results of a time- and angle-resolved photoemission (trARPES) experiment on bulk MoS${}_{2}$, a technique that provides the most direct view onto hot electron dynamics in energy and momentum space. Surprisingly, the new data agree well with past results of monolayer MoS${}_{2}$ as opposed to bulk MoS${}_{2}$. The authors interpret this finding in terms of the high density of surface defects in MoS${}_{2}$ samples, which substantially affect hot carrier relaxation due to trapping. In contrast to all optical studies, trARPES is very efficient in probing such processes owing to the extreme surface sensitivity of photoemission.

The present guideline provides a new and updated concept of treatment and prevention of adult patients with community-acquired pneumonia. It replaces the previous guideline dating from 2009.The guideline was worked out and agreed on following the standards of methodology of a S3-guideline. This includes a systematic literature search and grading, a structured discussion of recommendations supported by the literature as well as the declaration and assessment of potential conflicts of interests.The guideline has a focus on specific clinical circumstances, an update on severity assessment, and includes recommendations for an individualized selection of antimicrobial treatment as well as primary and secondary prevention.

Zusammenfassung Vier kolostrumfrei aufgezogene Kalber wurden unter Prednisolon-Behandlung mit dem VD-MD-Virusstamm Osloss/2482 infiziert. Der Virus-nachweis in Sekreten, Exkreten und Organen (Sektionsmaterial) geschah mit Hilfe der Immunofluoreszenz unter Verwendung von Zellkulturen aus Kalbernierengewebe und fluoreszierender Antikorper gegen das Virus der Europaischen Schweinepest. Eine Viramie trat bei einem Tier am 2. Tag post inoc. auf und bestand mindestens 18 Tage lang. Das Virus war im Blut hauptsachlich an die Leukozytenfraktion gebunden und verschwand erst, nachdem schon 11 Tage lang neutralisierende Antikorper im Blut zugegen waren. Weiterhin wurde das Virus im Nasensekret, im Speichel und im Kot festgestellt, doch trat es hier meist einige Tage spater als im Blut auf. Im Nasensekret lies sich das Virus bis zu 22 Tagen nach der Inokulation nachweisen. Besonders hohe Virustiter wurden in der Nebenniere, in der Milz und in den Speicheldrusen gefunden. Summary Studies on the pathogenesis of virus-diarrhoea-mucosal-disease in cattle using a heterotypic immuno-fluorescence technique For colostrum-deprived calves were infected experimentally with VD-MD virus of the strain Osloss/2482 while being conditioned by treatment with prednisolon. Demonstration of the virus in secretions, excretions and different organs was carried out by calf kidney cultures and fluorescent antibodies against swine fever virus. One animal developed viraemia two days after the injection and it persisted for at least 18 days. The virus was mainly bound to the leucocyte fraction in the blood and first disappeared after 11 days when neutralising antibodies were demonstrated. The virus was also detectable in nasal secretions, saliva and faeces but usually not until several days later than in the blood. In the nasal secretion the virus could still be detected up to 22 days after inoculation. Particularly high titres of virus were found in the adrenale, spleen and salivary glands. Virus was also found in the lung, mediastinal lymph nodes, heart muscle, pancreas, tonsils, mesenteric lymph nodes and the liver. Virus was isolated from the kidney in only one animal and in no case was it detected in the brain. En un animal se registro una viremia el 2° dia post inoc. y se mantuvo al menos durante 18 dias. El virus en sangre estaba ligado ante todo a la fraccion leucocitica y solo desaparecio cuando ya durante 11 dias se hallaban presentes en la sangre anticuerpos neutralizantes. Ademas se identifico el virus en la secrecion naritica, en saliva y en las heces, aunque aqui surgia casi siempre algunos dias mas tarde que en la sangre. En la destilacion naritica se pudo comprobar el virus hasta 22 dias despues de la inoculacion. Titulos singularmente elevados se encontraron en las adrenales, bazo y glandulas salivares. El virus se comprobo tambien en el pulmon, ganglios linfaticos intestinales e higado. En un animal nada mas se logro aislar el virus del rinon. En el cerebro siempre fallo la identificacion.

A high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) method allowing the sensitive and specific quantification of rivaroxaban (BAY 59-7939), a Factor Xa inhibitor in advanced development for the prevention and treatment of thromboembolic disorders, in human plasma is described. After precipitation of plasma proteins with methanol containing the internal standard followed by centrifugation, the plasma supernatant was injected directly onto the HPLC-MS/MS system. Concentrations could be determined between 0.50 and 500 microg/L. Inter-assay precision was < or = 7.4% and inter-assay accuracy was between 96.3 and 102.9% throughout the entire working range. The method was applied successfully in several clinical studies, which allowed an accurate determination of rivaroxaban pharmacokinetics in human plasma.

Aus Protoplasten von Candida lipolytica, Stamm 7-3, wurden nach osmotischem Schock durch Differentialzentrifugation bei 1500, 8000 und 16000 g subzellulare Partikeln gewonnen. Die Fraktionen katalysierten mit NADH als Cosubstrat den enzymatischen Abbau von n-Hexa-decan-1-14C. Bei Substratkonzentrationen von 0,46 bis 1,05 μmol/ml betrugen der Abbau 20 bis 75,8% und die Umsatzrate 3,5 bis 17,9 μmol · mg Protein−1 · h1. Die Reaktion war bereits nach 10 bis 15 min beendet. Die lipidloslichen Oxydationsprodukte wurden saulenchromatographisch vom Restalkan abgetrennt. In der wasrigen Phase des Inkubationsansatzes konnten hydrophile Reaktionsprodukte nachgewiesen werden. Auserdem wurden die von den subzellularen Partikeln absorbierten radioaktiv markierten Substanzen gemessen.
