La ventriculomegalia (VMG) és una malaltia rara que afecta entre 0,3 i 22 de cada 1000 embarassos. D'aquests, entre el 25 i el 60% són casos aïllats, és a dir, que no tenen una altra patologia associada. L'evolució de l'afectació i l'efecte sobre el neurodesenvolupament no és previsible. La detecció de la VMG s'efectua a través de la mesura de dos únics paràmetres: la mida de l'Atrium Esquerre i de l'Atrium Dret. L'objectiu és usar una base de dades de 81 participants (41 d'ells afectats i 40 no afectats) per a millorar la caracterització de l'afectació i de la prognosi i per a fer una detecció precoç per poder predir els efectes post-natals en base a les dades pre-natals disponibles. La reduïda mida de la base de dades ha suposat un repte que ha impedit trobar noves caracteritzacions de l'afectació tot i que s'ha detectat certa correlació entre la VMG i altres dos paràmetres: la Banya Anterior Esquerra i la Banya Anterior Dreta. S'ha aconseguit entrenar un model de classificació binari amb una valor de F1 al voltant del 90%. S'ha aconseguit amb xarxes neuronals o amb una cascada de xarxes neuronals amb els models d'aprenentatge supervisat més potents. Es conclou que un nombre reduït de dades inclús desbalancejat permet oferir models amb suficients prestacions però que un major coneixement del context podria millorar els resultats. Ventriculomegaly (VMG) is a rare disease that affects between 0.3 and 22 out of every 1000 pregnancies. 25-60% of these are isolated cases, which means that it has no other associated pathology. The evolution of the disease and the effect on neurodevelopment is not predictable. The detection of VMG is done through the measurement of only two parameters: the size of the Left Atrium and the Right Atrium . The aim is to use a database of 81 participants (41 affected and 40 unaffected) to improve characterisation of the condition and the prognosis and to provide an early detection to predict postnatal effects based on the available prenatal data. The small size of the database has been a challenge to find new characterisations of the disease, although some correlation has been detected between VMG and two other parameters: the Left Anterior Horn and the Right Anterior Horn. A binary classification model has been trained with a value of F1 of about 90%. This has been achieved applying neural networks or by cascading neural networks with the most powerful supervised learning models. The conclusion is that a small number of even unbalanced data allows to provide models with sufficient performance but more knowledge on the context might improve the results.

Background: Muscles from patients with cerebral palsy (CP) are often spastic and form contractures that limit the range of motion. Injections of botulinum toxin A (BTX) into the calf muscles are an important treatment for functional equinus; however, improvement in gait function is not always achieved. BTX is also used to test muscle weakening for risk evaluation of muscle lengthening surgery. Our aim was to assess the effect of BTX over time on calf muscle properties in pediatric CP patients with MRI. Material and Methods: Six toe-walking CP patients (mean age 11.6 years) with indication for lengthening surgery were prospectively enrolled and received BTX injections into the gastrocnemius and soleus muscles. MRI scans at 3T of the lower legs and clinical examinations were performed pre-BTX, 6 weeks (6w), and 12 weeks (12w) post-BTX. A fat-suppressed 2D multi-spin-echo sequence was used to acquire T2 maps and for segmentation. Fat fraction maps were calculated from 3D multi-echo Dixon images. Diffusion tensor imaging (DTI) with a 2D echo-planar imaging (EPI) sequence yielded maps of the mean apparent diffusion coefficient (ADC) and of the fractional anisotropy (FA). Hyperintense regions of interest (ROIs) on the T2-weighted (T2w) images at 6w were segmented in treated muscles. Mean values of T2, fat fraction, ADC, and FA were calculated in hyperintense ROIs and in reference ROIs in non-treated muscles. Results: Hyperintensity on T2w scans and increased T2 (group mean ± standard deviation: 35 ± 1 ms pre-BTX, 45 ± 2 ms at 6w, and 44 ± 2 ms at 12w) were observed in all patients at the injection sites. The T2 increase was spatially limited to parts of the injected muscles. FA increased (0.30 ± 0.03 pre-BTX, 0.34 ± 0.02 at 6w, and 0.36 ± 0.03 at 12w) while ADC did not change in hyperintense ROIs, indicating a BTX-induced increase in extracellular space and a simultaneous decrease of muscle fiber diameter. Fat fraction showed a trend for increase at 12w. Mean values in reference ROIs remained unchanged. Conclusion: MRI showed limited spatial distribution of the BTX-induced effects in pediatric CP patients. It could be a promising non-invasive tool for future studies to test BTX treatment protocols.

L'objectiu del projecte és elaborar un mòdul de consulta de camins i publicar-lo al servidor de mapes del Consell Comarcal del Vallès Occidental. Des del visor del servidor web, aquest mòdul permet realitzar consultes clares i intuïtives sobre les característiques i situació dels camins. El projecte es diferencia en dues parts bàsiques: la construcció de la base de dades, gestionada a través PostgreSQL/PostGIS i, l'altra, el desenvolupament de l'aplicació de consulta, programada en els llenguatges HTML, Javascript i PHP, i que construeix i visualitza els mapes a través de la xarxa web amb la llibreria MapScript PHP de MapServer.

L’estudi de la morfologia embrionària usant mètodes convencionals de microscòpia, ha estat històricament l’eina utilitzada pels embriòlegs per estudiar la viabilitat dels embrions. Ara, aquesta disciplina es suma als nous mètodes d’avaluació aportats pels sistemes time-lapse que s’han incorporat recentment als laboratoris de fecundació in vitro (FIV) de tot el món. Cada cop està més estandarditzat l’ús del time-lapse com a mètode d’avaluació i selecció embrionària conjuntament amb l’avaluació morfològica convencional, però més enllà del seu ús clínic, aquesta nova tecnologia aporta una nova perspectiva dels embrions en cultiu que obre un ventall de possibilitats a l’hora d’estudiar el desenvolupament embrionari preimplantacional, el coneixement del qual és fonamental per assolir l’èxit en els cicles de FIV. En aquest context, la present tesi doctoral ha volgut aprofundir en l’avaluació dels embrions des del punt de vista morfocinètic amb l’objectiu d’ampliar els coneixements sobre el desenvolupament embrionari preimplantacional, sempre prenent com a referència l’avaluació morfològica. Per a valorar la fiabilitat d’aquestes avaluacions basades només en la morfologia embrionària, existeixen estudis que evidencien la fiabilitat de les avaluacions morfològiques realitzades entre diferents embriòlegs. En la mateixa direcció i amb l’objectiu de demostrar la fiabilitat de les mesures preses amb els sistemes time-lapse, així com de donar solidesa als dos treballs següents, aquesta tesi doctoral inclou un primer estudi sobre la concordança entre les mesures preses amb els sistemes time-lapse per diferents observadors. En aquest treball es conclou que l’ús de la tecnologia time-lapse és fiable a l’hora de valorar el patró morfocinètic dels embrions en desenvolupament, així com que aquesta fiabilitat és extensible i independent del grau de formació o experiència dels embriòlegs, exceptuant la valoració de tPB2 que és fiable només si la duu a terme un sol observador (concordança intra-observador). Per altra banda, donada l’evidència científica que afirma que els primers temps de divisió cel·lular estan relacionats amb la viabilitat embrionària, en la present tesi doctoral s’inclou un segon estudi en el qual s’ha usat la tecnologia time-lapse per avaluar com afecta la vitrificació d’oòcits al patró morfocinètic dels embrions resultants i per tant sobre llur qualitat i viabilitat. En ell es conclou que la vitrificació d’oòcits no afecta al patró morfocinètic dels embrions en desenvolupament fins a dia 3. Finalment la present tesi doctoral inclou un tercer treball on amb el mateix objectiu que l’anterior s’ha utilitzat la tecnologia time-lapse per avaluar com afecta l’activació oocitària assistida (AOA) sobre la morfocinètica dels embrions i per tant sobre la seva viabilitat. En ell es conclou que l’aplicació de AOA accelera el desbloqueig meiòtic i l’extrusió del segon corpuscle polar però que, exceptuant les mesures de tPB2 i t3, el patró morfocinètic dels embrions derivats de cicles d’ICSI amb AOA no es mostra significativament diferent al dels embrions procedents de cicles ICSI sense AOA, estant això en concordança amb els resultats reproductius, els quals només mostren diferències significatives en comparar la taxa de fecundació entre les dues cohorts estudiades. En conjunt, els resultats d’aquesta tesi mostren com l’ús de la tecnologia time-lapse al laboratori de FIV, no només és útil per a realitzar avaluacions embrionàries a nivell clínic, sinó que també és una tecnologia fiable i útil per aprofundir en l’estudi de diferents aspectes del desenvolupament embrionari preimplantacional. El estudio de la morfología embrionaria usando métodos convencionales de microscopía, ha sido históricamente la herramienta utilizada por los embriólogos para estudiar la viabilidad de los embriones. Ahora, esta disciplina se suma a los nuevos métodos de evaluación aportados por los sistemas time-lapse que se han incorporado recientemente a los laboratorios de fecundación in vitro (FIV) de todo el mundo. Cada vez está más estandarizado el uso del time-lapse como método de evaluación y selección embrionaria juntamente con la evaluación morfológica convencional, pero más allá de su uso clínico, esta nueva tecnología aporta una nueva perspectiva de los embriones en cultivo que abre un abanico de posibilidades para estudiar el desarrollo embrionario preimplantacional, cuyo conocimiento es fundamental para alcanzar el éxito en los ciclos de FIV. En este contexto, la presente tesis doctoral ha querido profundizar en la evaluación de los embriones desde el punto de vista morfocinético con el objetivo de ampliar los conocimientos sobre el desarrollo embrionario preimplantacional, siempre tomando como referencia la evaluación morfológica. Para valorar la fiabilidad de estas evaluaciones basadas sólo en la morfología embrionaria, existen estudios que evidencian la fiabilidad de las evaluaciones morfológicas realizadas entre diferentes embriólogos. En la misma dirección y con el objetivo de demostrar la fiabilidad de las medidas tomadas con los sistemas time-lapse, así como dar solidez a los dos trabajos siguientes, esta tesis doctoral incluye un primer estudio sobre la concordancia entre las medidas tomadas con los sistemas time-lapse por diferentes observadores. En este trabajo se concluye que el uso de la tecnología time-lapse es fiable para valorar el patrón morfocinético de los embriones en desarrollo, así como que esta fiabilidad es extensible e independiente del grado de formación o experiencia de los embriólogos, exceptuando la valoración de tPB2 que es fiable sólo si la lleva a cabo un único observador (concordancia intra-observador). Por otro lado, dada la evidencia científica que afirma que los primeros tiempos de división celular están relacionados con la viabilidad embrionaria, en la presente tesis doctoral se incluye un segundo estudio en el que se ha usado la tecnología time-lapse para evaluar cómo afecta la vitrificación de ovocitos al patrón morfocinético de los embriones resultantes y por lo tanto a su calidad y viabilidad. En él se concluye que la vitrificación de ovocitos no afecta al patrón morfocinético de los embriones en desarrollo hasta el día 3. Finalmente, la presente tesis doctoral incluye un tercer trabajo donde con el mismo objetivo que la anterior se ha utilizado la tecnología time-lapse para evaluar cómo afecta la activación ovocitaria asistida (AOA) sobre la morfocinética de los embriones y por lo tanto sobre su viabilidad. En él se concluye que la aplicación de AOA acelera el desbloqueo meiótico y la extrusión del segundo corpúsculo polar pero que, exceptuando las medidas de tPB2 y t3, el patrón morfocinético de los embriones derivados de ciclos de ICSI con AOA no se muestra significativamente diferente al de los embriones procedentes de ciclos de ICSI sin AOA, estando ello en concordancia con los resultados reproductivos, los cuales sólo muestran diferencias significativas al comparar la tasa de fecundación entre las dos cohortes estudiadas. En conjunto, los resultados de esta tesis muestran como el uso de la tecnología time-lapse en el laboratorio de FIV, no sólo es útil para realizar evaluaciones embrionarias a nivel clínico, sino que también es una tecnología fiable y útil para profundizar en el estudio de diferentes aspectos del desarrollo embrionario preimplantacional. The study of embryonic morphology using conventional methods of microscopy has historically been the tool used by embryologists to study the viability of embryos. Now, this discipline is being added to the new assessment methods provided by time-lapse systems that have recently been incorporated into in vitro fertilization (IVF) laboratories around the world. The use of time-lapse as a method of embryo evaluation and selection in conjunction with conventional morphological evaluation is increasingly standardized, but beyond its clinical use, this new technology provides a new perspective on the embryos in culture, which opens a range of possibilities while studying the preimplantation embryonic development, the knowledge of which is fundamental to achieving success in IVF cycles. In this context, the present doctoral thesis has sought to deepen the evaluation of embryos from a morphokinetic point of view with the aim of expanding knowledge on preimplantation embryonic development, always taking as a reference the morphological evaluation. To assess the reliability of these assessments, based only on embryonic morphology, existing studies demonstrate the reliability of morphological assessments performed among different embryologists. In the same direction and with the aim of demonstrating the reliability of the measures taken with time-lapse systems, as well as giving solidity to the next two works, this doctoral thesis includes a first study on the concordance between the measures taken with time-lapse systems by different observers. This thesis concludes that the use of time-lapse technology is reliable in assessing the morphokinetic pattern of embryos in development, as well as that this reliability is extensible and independent of the degree of training or experience of embryologists, except for the assessment of tPB2 which is reliable only if it is carried out by a single observer (intra-observer agreement). On the other hand, given the scientific evidence that states that the first times of cell division are related to embryonic viability, this doctoral thesis includes a second study in which time-lapse technology has been used to evaluate how oocyte vitrification affects the morphokinetic pattern of the resulting embryos and therefore on their quality and viability. It concludes that oocyte vitrification does not affect the morphokinetic pattern of developing embryos until day 3. Finally, the present doctoral thesis includes a third study where, with the same objective as the previous, time-lapse technology has been used to evaluate how assisted oocyte activation (AOA) affects on the morphokinetics of embryos and therefore on their viability. It concludes that the application of AOA accelerates meiotic unblocking and extrusion of the second polar corpuscle but that, except for tPB2 and t3 measurements, the morphokinetic pattern of embryos derived from ICSI cycles with AOA is not significantly shown different from the one of embryos from ICSI cycles without AOA, this being in agreement with the reproductive results, which only show significant differences when comparing the fertilization rate between the two cohorts studied. Taken together, the results of this thesis show how the use of time-lapse technology in the IVF laboratory is not only useful for conducting embryonic evaluations on a clinical level, but is also a reliable and instrumental technology for deepening the study of different aspects of preimplantation embryonic development. Die Alzheimer-Krankheit (AD) ist die häufigste Form der Demenz und eine fortschreitende neurodegenerative Störung, die durch zwei pathologische Hauptmerkmale gekennzeichnet ist: 1) senile Plaques, die aus Ablagerungen von -Amyloid (A) im Gehirnparenchym bestehen, und 2) neurofibrilläre Tangles (NFTs), die aus gepaarten helikalen Filamenten (PHF) von pathologisch phosphoryliertem Tau-Protein bestehen. Die synergistische Beziehung zwischen A und Tau-Pathologie ist noch unbekannt. Über Jahrzehnte wurden mehrere transgene A- und Tau-doppelt-transgene Mausmodelle entwickelt, um ihre Interaktion zu untersuchen und neue Therapeutika zu testen. Trotzdem bleibt AD heutzutage noch immer unheilbar. Die verfügbaren Tierlinien modellieren jedoch nicht die komplexe räumlich-zeitliche AD-Pathologie. Ziel dieser Doktorarbeit war es daher, ein neuartiges induzierbares Mausmodell zu generieren, das die beim Menschen beobachtete räumlich-zeitliche AD-Pathologie besser reproduziert. Zu diesem Zweck wurde ein Adeno-assoziierter Virus (AAV), der das P301L-Tau-Transgen enthält, intrazerebral in den entorhinalen Kortex (ERC) von mutiert Amyloid- Vorläuferprotein transgene Mäuse (APPSL) injiziert, denn in diesem Hirnareal beginnt die Tau-Pathologie bei AD. Tau war sowohl an der Injektionsstelle als auch im Hippocampus abnormal phosphoryliert und aggregiert, während sich senile Plaques wie erwartet im Gehirn der Tiere ablagerten. Neuroinflammation, neuronaler Zellverlust und synaptische Plastizitätsstörungen wurden nach 6 Monaten im ERC untersucht. Hervorzuheben ist, dass A die Tau-Pathologie zu verschlechtern schien, Tau aber keinen Einfluss auf die Amyloidose nahm. Verhaltensänderungen wurden für 12 Monate longitudinal untersucht. P301L Tau verschlimmerte die kognitiven Defizite im Morris water maze 12 Monate nach der Injektion. Im Gegensatz dazu löste P301L-Tau aber keine Angst oder Stress aus. Insgesamt wurde ein neuartiges viral induziertes AD Tiermodell entwickelt, das die räumlich-zeitliche Pathologie von AD gut modelliert. Dieses neue Tiermodell könnte daher die präklinische Forschung und die translationale Medizin verbessern. Alzheimers disease (AD), the most common form of dementia, is a progressive neurodegenerative disorder characterized by two main pathological hallmarks: 1) senile plaques consisting of -amyloid (A) deposits in the brain parenchyma and 2) neurofibrillary tangles (NFTs) composed of paired helical filaments (PHF) made of anomalously phosphorylated Tau protein. However, the synergistic relationship between A and Tau pathology still needs to be elucidated. Over decades, several A and Tau transgenic mouse models have been developed with the aim of investigating their interconnected roles and testing new therapeutic agents. Nevertheless, AD remains nowadays incurable. Available animal models are not mimicking complex spatio-temporal AD pathology. Thus, the aim of this PhD study was to generate a novel inducible mouse model closer reproducing spatio-temporal AD pathology observed in humans. To this end, adeno-associated virus (AAV) harboring the P301L tau transgene were intracerebrally injected in mice transgenic for human Amyloid precursor protein harboring the Swedish and London mutations (APPSL mice) targeting the entorhinal cortex (ERC), the starting point of tau pathology in AD. Tau was abnormally phosphorylated and aggregated both at the injection site and the hippocampus, while senile plaques deposited as expected in the APPSL mouse. Neuroinflammation, neuronal loss and synaptic plasticity dysfunction were evaluated in the ERC after 6 months. Importantly, A seemed to worsen Tau pathology, albeit Tau did not have an impact on amyloidosis. Behavioral abnormalities were monitored longitudinally for 12 months. P301L Tau aggravated cognitive decline as assessed in the Morris water maze 12 months p.i. By contrast, anxiety or stress induction by P301L Tau was not observed. Altogether, a novel virally induced AD animal model closer mimicking ADs spatio-temporal pathology was developed. This new tool could potentially improve preclinical research and translational medicine. Aquest projecte es basa en l'estudi de la base de dades WEANDB en dos vessants. Per una banda, es fa un estudi estadístic de les dades. Per una altra, es fa un estudi de viabilitat pel registre de la propietat intel·lectual de la mateixa. WEANDB recopila informació de pacients assistits mitjançant ventilació mecànica en procés d’extubació. Aquests pacients provenen dels Departaments de Cures Intensives de dos hospitals, d’acord amb els protocols aprovats pels comitès ètics. L'anàlisi estadístic s'ha realitzat mitjançant un software especialitzat. L'estudi de viabilitat ha estat fet per a la realització del tràmit al Registre de la Propietat Intel·lectual de Catalunya.
