UMMARY Fifteen cases of lissencephaly were studied and the literature reviewed. The authors concluded that the clinical findings of lissencepaly infancy are not-specific, consisting of development delay and hypotonia. While the CT scan establishes the diagnosis, it may also be strongly suggested by an EEG showing ‘major fast dysrhythmia’, characterized by abnormally rapid, very high-voltage activity, predominantly in the alpha and beta frequency bands. Some possible mechanisms for this highly suggestive EEG pattern are proposed. RESUME Lissencephalie (agyrie-pachygyrie): donnees cliniques et etudes EEG en serie Quinze cas de lissencalie ont eteetudies, avec une analyse de la litterature. Les auteurs concluent que les donnees cliniques de la lissencephalie durant l'enfance ne sont pas specifiques, consistent en un retard de developpement et une hypotonic Tandis que la tomodensitometrie fait le diagnostic, celui-ci peut etre aussi fortement suggere par un EEG montrant “une dysrhytmie majeure rapide”, caracterisee par une activite anormalement rapide, de tres haut voltage, predominant dans les bandes de frequence alpha et beta. Quelques mecanismes possibles pour ces particularites EEG hautement suggestives sont proposes. ZUSAMMENFASSUNG Lissencephalie (Agyrie-Pachygyrie): Klinische Befunde und fortlaufende EEG Untersuchungen 15 Falle mit Lissencephalie wurden untersucht und die Literatur wurde gesichtet. Die Autoren sind der Meinung, das die klinischen Befunde der Lissencephalie im Kindesalter unspezifisch sind, sie bestehen in Entwicklungsverzogerung und Hypotonie. Obwohl die Diagnose durch das Computertomogramm gesichert wird, kann sie doch mit groser Wahrscheinlichkeit durch ein EEG mit vorwiegend hochfrequenter Arrhythmie, charakterisiert durch abnorm schnelle, sehr hohe Amplituden, vorwiegend in den Alpha und Beta Frequenzen, gestellt werden. Es werden einige mogliche Vorgange fur dieses hochst verdachtige EEG Muster diskutiert. RESUMEN Lisencefaiia (agiria-paquigiria): hallazgos cli'nicos y estudios EEG seriados Se estudiaron quince casos de lisencefaiia y se reviso la literatura. Los autores concluyen que los hallazgos clinicos de la lisencefaiia en la lactancia no son especificos y consisten en retraso en el desarrollo e hipotonia. Si bien la TAC establece el diagnostico, puede ser tambien sospechado por el EEG que muestra una disritmia rapida mayor, caracterizada por una actividad anormalmente rapida con voltaje muy alto, con predominio en las bandas de frecuencia alfa y beta. Se proponen algunos posibles mecanismos para este tipo de EEG tan sugestivo.

SUMMARY 37 children presenting with West's syndrome were studied by computerized tomography scanning. An abnormality consisting of diffuse cerebral atrophy, predominating in the fronto-temporal region, was demonstrated in 30 cases. The abnormality was sometimes associated with calcification (tuberous sclerosis) or malformation (agenesis of the corpus callosum). The aetiology of this atrophy and its relationship to clinical and electro-encephalographic signs is discussed. Computerized tomography scanning is recommended for children with West's syndrome in order to distinguish between primary and secondary cases, and for greater prognostic accuracy. RESUME Etude tomodensitometrique du syndrome de West 37 enfants avec syndrome de West ont ete etudies par la tomodensitometrie; celle-ci a revele dans 81 pour cent des cas une anomalie, representee le plus souvent par une atrophie cerebrale diffuse a predominance fronto-temporale, quelquefois associee a des calcifications (sclerose tubereuse) ou malformations (agenesie du corps calleux). L'eetiologie de cette atrophie tres particuliere et ses rapports avec l'importance des signes cliniques et eeg sont discutes. Un examen tomodensitometrique des enfants presentant un syndrome de West est recommande pour distinguer les cas primitifs des cas secondaires et donc pour mieux apprecier le pronostic de cette encephalopathie epileptogene. ZUSAMMENFUNG Komputertomographie bei der Untersuchung des West Syndrom Bei 37 Kindern mit einem West Syndrom wurde eine Komputertomographie durchge-fuhrt. In 30 Fallen stellte sich eine Fehlbildung in Form einer diffusen Hirnatrophie dar, die im Bereich der Frontalregion am ausgepragtesten war. Zusatzlich zu dieser Fehlbildung fand man in einigen Fallen Verkalkungen (tuberose Hirnsklerose) oder andere Missbildungen (Agenesie des Corpus callosum). Eine Diskussion uber die Aetiologie dieser Atrophie und ihre klinischen und elektro-encephalographischen Merkmale schliesst sich an. Die Komputertomographie wird als Untersuchung bei Kindern mit einem West Syndrom empfohlen, um zwischen sekundaren und primaren Fallen zu unterscheiden und um eien genauere Prognose stellen zu konnen.

Summary The different types of epilepsy encountered during the various phases of life are presented and the distinction is made between benign and, essentially if not exclusively, functional epilepsy related to a genetically-transmitted predisposition to epilepsy: (1) the primary generalized epilepsies and (2) the benign partial epilepsies with functional foci. In the case of primary generalized epilepsy, seizures are the result of a discharge, generalized from the onset, of the entire cortex in individuals genetically predisposed to epilepsy. The seizures present (1) in older children, adolescents and young adults, the well-known characteristics of Petit Mal and/or Grand Mal; (2) in neonates, infants and young children, a different type of generalized (or hemigeneralized) seizures, the semeiology of which remains unclear. In the case of benign partial epilepsy, the seizures result from an epileptic discharge in a cortical area having a low convulsant threshold in patients genetically predisposed to epilepsy. The semeiology of the seizures varies as a function of the patient's age and the site of the epileptic focus: u - in neonates: benign partial epilepsy with an erratic EEG focus, e.g. ‘Fifth day seizures’; - in infants: ‘cryptogenic’ benign partial epilepsy, which remains poorly defined; - in children: (a) benign partial epilepsy with nocturnal motor seizures and a central EEG focus; (b) benign partial epilepsy with affective seizures and a temporal focus; (c) benign partial epilepsy with visual seizures (often coupled with signs of basilar migraine) and an occipital focus; - in adolescents; benign partial motor epilepsy (often versive) often without an interictal EEG focus or with generalized spike-waves; benign partial epilepsies do not occur after adolescence.

Resume Les auteurs etudient les relations existant entre la sclerose tubereuse de Bourneville et le syndrome d'encephalopathie infantile avec hypsarythmie (E.M.I.H.). Ils notent que la sclerose tubereuse de Bourneville represente dans 4% des cas d'E.M.I.H., une etiologie de ce syndrome et ils illustrent cette constatation de deux observations personnelles et de sept autres retrouvees dans la litterature. Ils analysent ensuite 137 observations de sclerose tubereuse afin de verifier si les crises epileptiques de la premiere enfance, decrites dans le tableau clinique de cette affection, n'ont pas ete en fait l'expression clinique d'une E.M.I.H. Une observation personnelle et cinq autres de la litterature semblent confirmer cette maniere de voir. Les relations entre l'E.M.I.H. et l'arrieration mentale de la sclerose tubereuse de Bourneville sont egalement etudiees. Il semble bien que l'arrieration mentale grave soit la regle chez les sujets atteints d'epiloia et qui ont presente en outre une E.M.I.H. dans leur enfance. Le deficit intellectuel est encore important si l'epilepsie au cours de l'evolution d'une sclerose tubereuse est apparus entre un mois et un an. Il est moins grave chez les malades pourtant atteints de ce type de phacomatose mais qui ne sont pas epileptiques ou dont l'epilepsie a debute soit avant un mois soit avant un an. Il existe donc d'indeniables relations entre l'E.M.I.H. et la sclerose tubereuse de Bourneville, cette derniere etant un facteur etiologique important de la premiere. Inversement, le syndrome E.M.I.H. parait etre responsable de l'importance de l'oligophrenie qui accompagne, dans certains cas, cette phacomatose.

Somato-inhibitory status epilepticus is a rare condition which may pose some problems of differential diagnosis. The authors describe the case of a girl who suffered from episodes of prolonged ictal paralysis lasting from some minutes to some hours. One seizure was described 2 hours from onset. The EEG showed a continuous spike activity on the right side, predominantly on the parieto-rolandic areas. Status was stopped by the administration of clobazam per os. The physiopathological mechanism of this rare form of status epilepticus is discussed.

Cranial computerized tomography (CCT) in permitting visualization of cerebral edema in live patients, allows for the first time valid studies concerning the role of reactional edema in the generation of EEG abnormalities related to expanding processes. The authors analyze the results of EEG and CCT in 127 patients presenting cerebral tumor(s), 84 of which were accompanied by reactional edema. The study leads to the conclusion (also demonstrated by certain animal work) that edema per se is only rarely responsible (9.5%) for the EEG abnormalities.
