7. Relation of peritumoral, prepectoral and diffuse edema with histopathologic findings of breast cancer in preoperative 3T magnetic resonance imaging

Amaç: Preoperatif meme manyetik rezonans görüntüleme (MRG), meme kanserinin prognozu konusunda zengin bilgiler sağlar. Bunun için özellikle morfolojik ve dinamik özellikler kullanılır. Çalışmamızda meme MRG’de peritümöral, prepektoral, diffüz ödemin meme kanserinin histopatolojik prognostic parametreler ile ilişkisin araştırmayı ve meme MRG’den prognoz ile ilgili edinilen bilgileri değerlendirmeyi amaçladık.Yöntemler: Ocak-Ağustos 2018 tarihleri arasında hastanemizde meme kanseri tanısı almış ve ameliyat öncesi değerlendirmede kontrastlı dinamik meme MRG çekilen 46 hasta çalışmaya dahil edildi. T2A sekansta tümör çevresinde, pektoral kas önünde ya da tüm memede izlenen su ile benzer sinyal ödem lehine değerlendirildi. Her bir ödem tipi; kanser tipi, tümör boyutu, histolojik evre, ER-PR reseptör pozitifliği, Her2 reseptör pozitifliği, Ki67 proliferasyon indeksi ve lenfovasküler invazyon gibi klinikopatolojik parametreler ile değerlendirildi.Bulgular: Ortalama yaş 53.15±11.75 (aralık 27-80) idi. Meme MRG’de 11 hastada diffüz, 27 hastada peritümöral, 5 hastada prepektoral ödem bulgusu izlenmekteydi. 19 lüminal A, 17 lüminal B, 9 üçlü negatif, 1 Her2 kanser mevcuttu.Peritümöral ödem LVI pozitifliği ile ilişkili bulundu (p=0.002). Tümör boyutu ve Ki-67 indexi peritümöral ödemle ilişkilidir (p=0.001, p=0.009).Prepektoral ödem ile değerlendirilen parametreler arasında ilişkili görülmedi. Diffüz ödem tümör boyutuna göre farklılık gösterir (p=0.026).Sonuç: Meme MRG’de görülen peritümöral ödem LVI pozitifliği açısından bilgi vericidir. Farklı ödem tipleri prognoz açısından bilgiler taşıdığından rutin raporlamalarda belirtilmelidir., Aim: Preoperative breast magnetic resonance imaging (MRI) findings can provide rich information about the prognosis of the disease. Morphologic and dynamic features are especially used for it. We aimed to compare peritumoral, prepectoral, and diffuse edema identified in MRI with histopathologic findings, and to show how prognostic information can be gathered from the identification of edema.Methods: We conducted a retrospective cohort study with forty-six women who underwent breast DCE-MRI as part of the pre-surgical evaluation between January and August 2018 were included in the study. Signal enhancements similar to water that were localized to the prepectoral or peritumoral areas or diffuse enhancements on T2A-weighted sequences were considered as edema. The presence of edema was compared with clinicopathologic parameters such as cancer type, tumor size, histologic grade, ER-PR receptor positivity, Her2 positivity, Ki-67 labelling index and lymphovascular invasion.Results: The mean age of the participants was 53.15±11.75 (range, 27-80) years. Eleven patients had diffuse edema, 27 patients had peritumoral edema, and 5 patients had prepectoral edema. Nineteen luminal A cancers, 17 luminal B, 9 triple-negative, and 1 Her2 cancer were seen. Peritumoral edema was associated with lymphovascular invasion positivity (p=0.002). Tumor size and the level of Ki-67 was associated with peritumoral edema (p=0.001, p=0.009). The odds of observing prepectoral edema showed no statistically significant difference in the presence of lymphovascular invasion positivity and other parameters. The presence of diffuse edema showed significant differences depending on tumor size measurements (p=0.026).Conclusion: Edema in breast MRI can provide information about histopathologic findings, particularly about lymphovascular invasion. The authors suggest that different edema types could be mentioned in radiology reports as a matter of routine given that such findings can provide information about the prognosis.

Amaç: Sınıf-I grubu antiaritmik ilaçların elektrokardiyografik (EKG) etkisini bilinmektedir. Bilinen bu etkileri ileri sayısal bilgisayar analizleri uygulayarak araştırmak amacı ile bir fare deney hayvanı modeli oluşturarak da desteklene bilir mi? Bu çalışma da bu soruya cevap aranmıştır.Gereç ve Yöntem: Anestezi altında in-vivo hayvan deneylerini içeren bu tez çalışmasında 25-30 g ağırlığında 4-6- aylık 32 adet erkek BALB/C fareler kullanılmıştır. Nembutal (Na-pentobarbital) 75 mg/kg dozunda intraperitoneal (i.p) enjeksiyonla tüm deneylerde uygulanmıştır. Kontrol grubu SF veya kinidin (50 mg/kg) veya Lidokain (30 mg/kg) veya Flekainid (20 mg/kg) içeren dört grup oluşturulmuştur. İki kanallı EKG kayıtları acıyı en az seviyede tutabilmek için çok ince metal iğne elektrotlar kullanılarak yapılmıştır. Buna paralel olarak vücut sıcaklığı ve solunum sayısı gibi hayati parametrelerde kaydedilmiştir. Elde edilen EKG kayıtları bilgisayar programları kullanılarak dalgaların spektral güç yoğunluğu analizi yapılmıştır. Bulgular: Çalışmamızda kullanılan kinidin PR intervali, QRS kompleksi ve QJintervali sürelerini uzatmıştır.yapılan PSD analizinde de yüksek frekans bandında(120-300 Hz) güç değerlerinde anlamlı düşüşler gözlenmiştir. Lidokainin EKG parametreleri üzerine etkileri incelendiğinde PR intervalini etkilememiş, QRS kompleksi süresini uzatırken QJ mesafesinde kısalmaya sebepolmuştur. PSD analizinde yüksek frekans bandında (120-300 Hz) güç değerlerindebir düşüş olmuştur.Flekainidin etkilerine baktığımızda EKG'de PR intervali, QRS kompleksi ve QJintervalini önemli ölçüde etkilemiş olduğunu gördük. PSD analizinde de yüksek frekans bandında (120-300 Hz) düşen güç değerleri olarak karşımıza çıktı. Sonuçlar: Çalışmamızda anestezi altındaki farelerde Kinidin, Lidokain veFlekainid'in çeşitli EKG parametrelerinin üzerinde etkilerini yapılan ölçümlersonucunda gösterdik. Aim: Electrocardiographic (ECG) effect of class-I group antiarrhythmic drugs isknown. Is it also supported by creating a mouse experimental animal model toinvestigate these known effects by applying advanced computer analysis? In thisstudy, the answer to this question was sought.Material and Method: In this thesis study including in-vivo animal experimentsunder anesthesia, 32 male BALB / C mice with 4-6-month weight of 25-30 g wereused. Nembutal (Na-pentobarbital) was administered in all experiments with anintraperitoneal (i.p) injection at a dose of 75 mg / kg. Four groups consisting ofcontrol group SF or quinidine (50 mg / kg) or Lidocaine (30 mg / kg) or Flecainide(20 mg / kg) were formed. Two-channel ECG recordings were made using very thinmetal needle electrodes to minimize pain. Parallel to this, vital parameters such asbody temperature and respiratory rate were recorded. The spectral power densityanalysis of the waves was done by using the computer programs.Findings: The quinidine PR interval, QRS complex and QJ used in our studyextended PSD analysis in high frequency band significant decreases in power values(120-300 Hz) were observed. Examining the effects of lidocaine on ECG parametersdid not affect the PR interval, QRS complex prolonged the time and caused ashortening in QJ distance. PSD analysis showed a decrease in power values in thehigh frequency band (120-300 Hz). When we look at the effects of flekainid, wefound that the ECG has significantly affected PR interval, QRS complex and QJinterval. In the PSD analysis, we found the power values falling in the high frequencyband (120-300 Hz).Results: In our study, we demonstrated the effects of kinidine, lidocaine andflekainid on various ECG parameters in anesthetized mice. In addition, as a result ofPSD analyzes, we found that all of these drugs have significantly decreased powervalues in the range of 120-300 Hz, which we defined as high frequency band. Wecan explain the decrease in these power values with changes in the QRS complex inthe light of the fact that all drugs affect the QRS complex and we know that the QRScomplex is a high frequency component in the ECG. 71

14. Comparison of Qualitative and Volumetric Assessments of Breast Density and Analyses of Breast Compression Parameters and Breast Volume of Women in Bahcesehir Mammography Screening Project.

Objective: We aimed to compare visual and quantitative measurements of breast density and to reveal the density profile with compression characteristics. Materials and Methods: Screening mammograms of 1399 women between May 2014 and May 2015 were evaluated by using Volpara 4th and 5th version. First 379 mammograms were assessed according to ACR BI-RADS 4th edition and compared to Volpara. We categorized the breast density in two subgroups as dens or non-dens. Two radiologists reviewed the images in consensus. Agreement level between visual and volumetric methods and volumetric methods between themselves assessed using weighted kappa statistics. Volpara data such as fibroglandular volume (FGV), breast volume (BV), compression thickness (CT), compression force (CF), compression pressure (CP) were also analyzed with relation to the age. Results: 1399 mammograms were distributed as follows: 12.7% VDG1, 39.3% VDG2, 34.1% VDG3, 13.9% VDG4 according to the 4th edition of Volpara; 1.2% VDG1, 46% VDG2, 36.8% VDG3, 15.9% VDG4 according to the 5th edition of Volpara. The difference between two editions was 4.7% increase in dense category. 379 mammograms, according to ACR BI-RADS 4th edition, were distributed as follows: 25.9% category A, 50.9% category B, 19.8% category C, 3.4% category D. The strength of agreement between the Volpara 4th and 5th editions was found substantial (k=0.726). The agreements between visual assessment and both Volpara editions were poor (k=- 0.413, k=-0.399 respectively). There was a 142% increase in dense group with the VDG 4th edition and 162% with the VDG 5th edition when compared to visual assessment. Compression force decreased while compression pressure increased with increasing Volpara Density Grade (VDG) (p for trend <0.001 for both). Compression thickness and breast volume decreased with increasing VDG (p for trend <0.001 for both). The FGV decreases with age and the breast volume increases with increasing age (p<0.001). Conclusion: Visual assessment of breast density doesn't correlate well with volumetric assessments. Obtaining additional information about physical parameters and breast profile by the results of quantified methods is important for breast cancer risk assessments and prevention strategies. Objective: Gastrointestinal stromal tumors (GISTs) are the most common nonepithelial tumors of the gastrointestinal tract. GISTs occur anywhere along the gastrointestinal tract. Due to using radiological modalities often, we see the GİSTs more in our daily practice. We use radiologic modalities for not only making the diagnosis, also following the therapy and tumor progressions-relaps. Material and Method: In this study, 23 GISTs whose diagnosis and therapeutic surgical operations were made in our hospital between January 2007-January 2013, were evaluated retrospectively. Results: The ages of the cases ranged between 32 and 76 and the mean age were 58.70±13.67 years. 52.2% of the cases (n=12) were male and 47.8% of them (n=11) were female. 91.3% of the cases (n=21) were located at GIS and 8.7% of the cases (n=2) were located at non-GIS location. 30.4% of the cases (n=7) were metastatic. When the cases were evalulated in terms of their pathologic risk stratification systems, 39.1% of the cases (n=9) were in high risk group, 34.8% of the cases (n=8) were in intermediate risk group, 26.1% of the cases (n= 6) were in low risk group. The maximum diameter of the masses was 10.22 cm. 69.1% of the cases (n=13) showed luminal growth pattern and 38.1% (n=8) showed extraluminal growth pattern. 86.9% of the cases (n=20) had smooth borders, 13.1% of the cases (n=3) had irregular-invasive borders. Conclusion: Imaging is important for diagnosis, theraphy and follow -up. Computed tomography is the first choice for the radiologic evaluation. Also ultrasonography (US), magnetic resonance (MRI) and Flor18-deoxy-glucose positron emission tomography (FDG-PET) can be used in some cases., Amaç: Gastrointestinal stromal tümör (GİST) gastrointestinal kanalın en sık görülen mezenkimal tümörüdür. Özofagustan rektuma kadar tüm gastrointestinal kanal boyunca izlenebilir. Günlük pratikte hastalara uygulanan radyolojik tetkiklerin artışıyla birlikte GİST'lere daha sıklıkla rastlanmaktadır. Sadece tümör tanısını koyarken değil tedavi etkilerini izlemede ve tümör progresyon-nükslerini takip etmede radyolojik modalitelere sıkça ihtiyaç duyulmaktadır. Gereç ve Yöntem: Bu çalışmada Ocak 2007- Ocak 2013 yılları arasında hastanemizde tanı konulan ve cerrahi tedavileri yapılan 23 GİST olgusu retrospektif olarak değerlendirildi. Bulgular: Olguların yaş dağılımları 32 ile 76 arasında değişmekte olup ortalama yaş 58.70±13.67'dir. Çalışmaya katılan hastaların % 52.2'si (n=12) erkek, % 47.8'i (n=11) kadındır. Lokalizasyon yeri incelendiğinde; 21 (% 91.3) olguda GİS, 2 (% 8.7) olguda ise GİS dışı olduğu görülmektedir. En sık lokalizasyon midedir. Araştırmaya katılan hastaların % 30.4'ünde (n=7) metastaz saptanmıştır. Omentum en sık metastaz yeri olarak izlenmiştir. Olguların risk grupları incelendiğinde; % 39.1'inin (n=9) yüksek, % 34.8'inin (n=8) orta ve % 26.1'inin (n=6) ise düşük risk grubunda olduğu saptanmıştır. Kitlelerin maksimum çapı 10.2 cm'dir. Kitlelerin % 69.1'i (n=13) lüminal büyüme paterni gösterirken, % 38.1'i (n=8) ekstralüminal büyüme paterni göstermiştir. Kitlelerin % 86.9'u (n=20) düzgün kenarlı iken % 13.1'i (n=3) düzensiz ve invaziv kenarlı izlenmiştir. Sonuç: Görüntüleme yöntemleri GİST'lerin tanı, tedavi ve takibinde önemlidir. Bilgisayarlı tomografi radyolojik değerlendirmede ilk seçenektir. Ayrıca ultrasonografi (US), manyetik rezonans (MRG) ve flor18-deoksi-glukoz pozitron emisyon tomografinin (FDG-PET) bazı durumlarda kullanılabilir.

18. GRAF METODUNA GÖRE TİP 2A KALÇALARDA TAKİP BULGULARIMIZ VE ULTRASONOGRAFİK İLERLEME GÖSTEREN OLGULARDA RİSK FAKTÖRLERİ

Objective: Development of the hip joint is a dynamic process beginning before the delivery and continuing after it. Ultrasound (US) examination is preferred diagnostic method. US examination is performed with static method described by Dr. Graf and dynamic method described by Dr. Harcke. In both of these methods, bone and cartilage structure composing the hip joint is evaluated and typed according to Dr.Graf’s classification system. US examination is also used during the follow-up and treatment periods together with this classification system.Material and Methods: In our study, we compared follow-up sonographic worsening of the hips which were evaluated to be in favor of type 2a at the first US investigation between January 2008 and December 2012 with the first presenting symptoms and the parameters predisposing to developmental dysplasia of the hip (DDH).Results: In the study, 80 female (n=99, [69%]) and 36 male (n=43, [31%]) babies were evaluated. There were 73 babies with type 2a (+) (n=88, [62%]) and 43 babies with type 2a (-) (n=54, [38%]). There were 13 babies with family history (n=14, [9.9%]) and 40 babies of primiparae mothers (n=48, [33.8%]). One hundred and eleven babies (n=135, [95.1%]) gained physiologic appearance up to 3 months.Risk factors and sonographic evaluation findings were evaluated according to ultrasonographic worsening. There is no statistically significant difference between gender and family history, being right and/or left hip, being Type 2a (-) or (+) with ultrasonographic worsening.Conclusion: In our study, ultrasonographic worsening was determined in 4.9% of the cases. No statistical significance was determined between DDH risk factors and sonographic worsening. The cases determined to have physiological immaturity at the first US investigation should be followed-up carefully during first 3 months regardless of risk factors., Amaç: Kalça eklemi gelişimi doğum öncesi başlayıp doğum sonrası devam eden dinamik bir süreçtir. Ultrasonografi (US) tercih edilen ve tanı koydurucu yöntemdir. US inceleme, Dr. Graf’ın tanımladığı statik yöntem ile Dr. Harcke’nin tanımladığı dinamik yöntem ile yapılır. Her iki yöntemde de kalça eklemini oluşturan kemik ve kıkırdak çatı değerlendirilir ve Dr. Graf’ın sınıflama sistemine göre tiplendirilir. US inceleme, bu sınıflandırma sistemi ile birlikte takip ve tedavi sürecinde de kullanılır. Gereç ve Yöntemler: Çalışmamızda Ocak 2008 - Aralık 2012 tarihleri arasında ilk US değerlendirmede tip 2a lehine değerlendirilen kalçalarda, takip sonografik kötüleşme durumunu, ilk geliş bulguları ve gelişimsel kalça displazisi(GKD)’ne yatkınlık oluşturacak parametreler ile karşılaştırdık. Bulgular: Çalışmada 80 kız (n=99, [% 69]) , 36 erkek (n=43, [% 31]) bebek değerlendirilmiştir. Tip 2a (+) olan 73 bebek (n=88, [% 62]) , Tip 2a (-) olan 43 bebek (n=54, [% 38]) bulunmaktadır. Aile hikayesi bulunan 13 bebek (n=14, [% 9.9]); primipar 40 bebek (n=48, [% 33.8]) mevcuttur. Olgularda ilk US incelemede 4-6 haftalarda yapılmış olup takip değerlendirmeler 4 hafta aralıklarla yapılmıştır. 3 aya kadar 111 bebek (n=135, [ %95.1]) fizyolojik görünüme kavuşmuştur. Risk faktörleri ve sonografik değerlendirme bulguları, sonografik ilerlemeye göre değerlendirilmiştir. Sonografik ilerleme ile cinsiyet, aile hikayesi, primiparite , sağ ve/veya sol kalça olma, Tip 2a (-) veya (+) olma arasında istatistiksel olarak anlamlı farklılık bulunmamaktadır. Sonuç: Çalışmamızda olguların % 4.9’unda US ile ilerleme tespit edilmiştir. GKD risk faktörleri ile sonografik kötüleşme arasında istatistiksel anlamlılık saptanmamıştır. İlk US incelemede fizyolojik immatürite saptanan olgular, risk faktörleri gözetilmeksizin ilk 3 ay dikkatlice takip edilmelidir.

Günümüzde geleneksel ayırma yöntemlerine alternatif olarak çeşitli yöntemler geliştirilmiştir. Bu tez çalışmasında en yeni yöntemlerden biri olan tepkimeli özütleme ile organik asitlerin sulu çözeltilerinden ayrıştırılması konusunda çalışılmıştır. Bu amaçla; nikotinik asit, fumarik asit ve glioksilik asitlerle birlikte çeşitli amin ve çözücüler kullanılmıştır. Nikotinik asit, niyasin veya B3 vitamini suda ve alkolde çözünebilen monokarboksilik bir asittir. Geniş bir pH aralığında ısı ve ışığa karşı yüksek stabilite göstermesi nedeniyle en dayanıklı vitamindir. B vitamini karbonhidrat metabolizması için çok önemlidir. Fumarik asit sentetik olarak üretilen ve çeşitli bitkilerin içerisinde bulunan renksiz kristal yapıda bir bileşiktir. Çoğunlukla reçine, vernik, boya ve mürekkeplerde kullanılır. Etilen zincirinin karşı tarafına iki karboksil grubuyla bağlanan maleik asidin geometrik izomeridir. Glioksilik asit doğal olarak oluşan renksiz katıdır ve endüstride kullanışlıdır. Ürünün temeli olan okzalik asit, sıcak nitrik asit ile glioksalın organik oksidasyonuyla elde edilir. Deneysel çalışmada, nikotinik asit, fumarik asit ve glioksilik asidin triisooktilamin, tridodesilamin, tributilamin ve trioktilamin kullanılarak tepkimeli ekstraksiyon işlemi gerçekleştirilmiştir. Seyreltici olarak; 3 farklı alkol (oktanol, dekanol, izo amil alkol), 3 farklı keton (izo butil metil keton (IBMK), heptanon ve oktanon) ile n-oktan, n-heptan ve n-hekzan olmak üzere toplamda kullanılan 9 farklı seyreltici kullanılarak, her bir amin için 10 set deney çalışılmıştır.Yapılan denemeler sonucunda amin konsantrasyonu arttıkça dağılma katsayısının (KD ) arttığı, yükleme değerinin (Z) azaldığı gözlenmiştir. Denemeler sonucunda görülmüştür ki nikotinik asit için trioktilamin ve tridodesilamin kullanıldığında en iyi seyreltici izoamil alkoldür. Fumarik asit ile yapılan denemede tridodesilamin ile izoamil alkol, tributil amin ile dekanol en iyi sonucu vermiştir. Glioksilik asit ile yapılan denemelerde tridodesilamin ile dekanol, trioktilamin ile izo amil alkol en iyi sonucu vermiştir Nowadays, several methods are developed alternatively to traditional methods. The aim of this study is the separation of carboxylic acids with reactive extraction which is the newest seperation methods. For this purpose; nicotinic acid, fumaric acid and glyoxylic acid and also several amines and solvents are used.Nicotinic acid, niacin or vitamin B3, is a carboxylic acid that can be soluble in water and alcohol-soluble. It's the most stable vitamin because of high stability for heat and light at a large pH range. The vitamin B complex is very important for carbohydrate metabolism.Fumaric acid is a colorless crystalline compound found in various plants and also produced synthetically. It is used mainly in resins, paints, varnishes, and inks. Fumaric acid is a geometric isomer of maleic acid, having two carboxyl (COOH) groups attached on opposite sides of an ethylene chain.Glyoxylic acid is a colourless solid that occurs naturally and is useful in industry. The compound is formed by organic oxidation of glyoxal with hot nitric acid, the main side product being oxalic acid. In the experimental study, the reactive extraction of nicotinic acid, fumaric acid and glyoxylic acid were studied with triisooctylamine, tridodecylamine, tributhylamine and trioctiylamine. Totally nine solvents were used, which consisted of 3 different alcohols (octanol, decanol and iso amyl alcohol), 3 different ketones (iso buthyl methyl ketone, heptanone, octanone) and n-octane, n-heptane, n-hexane. Ten set experiments were studied for each amine.As a result of, the best solvent for nicotinic acid was found to be isoamyl alcohol, if triisooctyl amine and tridodecyl amine were used. Also, the experiments with fumaric acid, the best solvent was decanol with tributyl amine and isoamyl alcohol with tridodecyl amine. The experiments with glyoxylic acid, the best solvent was isoamyl alcohol with trioctylamine and decanol with tridodecyl amine. 137

Objective: The aim of this study was to evaluate the differences between shear wave elastography findings responders and non-responders in the follow-up of breast cancer patients receiving neoadjuvant chemotherapy (NAC). Materials and Methods: Between September 2015 and August 2018, 47 patients were evaluated retrospectively. The patients were measured by tru-cut biopsy before and after the diagnosis and at the end of the treatment with shear wave elastography (SWE). Measurements were taken from the hardest part of the mass and from the environment normal parenchyma. In addition, the ratio of the mass and parenchyma to each other (R) was recorded. Patients were followed up in the interim period. Results: The SWE value of the mass was higher in the patients who responded to NAC, whereas the SWE value decreased after the treatment (p=0.00). In patients without response, SWE values of the mass did not decrease significantly at the end of NAC. After the NAC, parenchymal SWE values were found to be increased in all patients. In patients without response, SWE values of the masses were significantly higher than those in patients with no response (p=0.029). When the values at the end of NAC treatment were examined, the patients with unresponsive response had higher SWE values than the patients with response (respectively; p=0.05, p=0.02). Conclusion: There is a statistically significant relationship between the response of NAC to breast tumors and the SWE values measured before and after treatment. [ABSTRACT FROM AUTHOR]

27. Diffusion Weighted MR Imaging of Breast and Correlation of Prognostic Factors in Breast Cancer.

- Abstract
Background: Through Diffusion Weighted Imaging (DWI), information related to early molecular changes, changes in the permeability of cell membranes, and early morphologic and physiologic changes such as cell swelling can be obtained. Aims: We investigated the correlation between the prognostic factors of breast cancer and apparent diffusion coefficient (ADC) in DWI sequences of malignant lesions. Study Design: Retrospective cross-sectional study. Methods: Patients who were referred to our clinic between September 2012 and September 2013, who underwent dynamic breast MRI before or after biopsy and whose biopsy results were determined as malignant, were included in our study. Before the dynamic analysis, DWI sequences were taken. ADC relationship with all prognostic factors was investigated. Pearson correlation test was used to compare the numerical data, while Spearman correlation and Fisher exact tests were used to compare the categorical data. The advanced relationships were evaluated with linear regression analysis and univariate analysis. The efficiency of the parameters was evaluated using ROC analysis. The significance level (P) was accepted as 0.05. Results: In total, 41 female patients with an average age of 49.4 years (age interval 21-77) and 44 lesions were included into the study. In the Pearson correlation test, no statistically significant difference was determined between ADC and the patient's age and tumor size. In the Spearman correlation test, a statistically significant difference was determined between nuclear grade (NG) and ADC (r=-0.424, p=0.04); no statistically significant correlation was observed between the other prognostic factors with each other and ADC values. In the linear regression analysis, the relationship of NG with ADC was found to be more significant alone than when comparing all parameters (corrected r2=0.196, p=0.005). Further evaluations between the NG and ADC correlation were carried out with ROC analysis. A statistically significant difference was determined when NG 1 separately was compared with NG 2 and 3 (p=0.03). A statistically significant difference was also determined (p=0.05) in the comparison of NG 1 with only NG 3. No statistically significant difference was determined when NG 2 separately was compared with NG 1 and NG 3 and when NG 3 separately was compared with NG 1 and 2 (p=0.431, p=0.097), Conclusion: We found that ADC values obtained by breast DWI showed a higher correlation with the NG of breast cancer, which is an important factor in the patient's treatment. Predictions can be made about NG by analyzing the ADC values. Additional studies are needed, however, and the ADC value of the lesion can be used as a prognostic factor proving the aggressiveness. Inthis work, the equilibrium distribution of (E)-butenedioicacid (fumaric acid) between water and tridodecylamine(TDDA) or tributylamine (TBA) was carried out at 298 K. The organicphase was prepared using both TDDA and TBA in three nontoxic diluents(canola oil, sesame oil, and almond oil). The batch equilibrium experimentaldata are presented by calculation of the loading factor (Z), extraction efficiency (E), and distribution coefficient(KD). The highest extraction efficienciesof 92.59 % and 92.50 % with values of KDequal to 12.50 and 12.40 were obtained with 0.789 mol·kg–1TDDA in canola oil and 2.080 mol·kg–1TBA in canola oil. The extraction abilities of TDDA and TBA in differentnontoxic natural diluents in terms of KDand Ewere found to be in the order of canola oil> almond oil > sesame oil. Partition coefficients (P= 0.3819, 0.4739, and 0.7535) and dimerization constants (D= 83.64, 50.28, and 43.51) were calculated according tophysical extraction for sesame oil, almond oil, and canola oil, respectively. Objective: Gastrointestinal stromal tumors (GISTs) are the most common nonepithelial tumors of the gastrointestinal tract. GISTs occur anywhere along the gastrointestinal tract. Due to using radiologicaI modalities often, we see the GtSTs more in our daily practice. We use radiologic modalities for not only making the diagnosis, also following the therapy and tumor progressions-relaps. Material and Method: In this study, 23 GISTs whose diagnosis and therapeutic surgical operations were made in our hospital between January 2007-January 2013, were evaluated retrospectively. Results: The ages of the cases ranged between 32 and 76 and the mean age were 58.70±13.67 years. 52.2% of the cases (n=12) were male and 47.8% of them (n=l 1) were female. 91.3% of the cases (n=21) were located at GIS and 8.7% of the cases (n=2) were located at non-GIS location. 30.4% of the cases (n=7) were metastatic. When the cases were evalulated in terms of their pathologic risk stratification systems, 39.1% of the cases (n=9) were in high risk group, 34.8% of the cases (n=8) were in intermediate risk group, 26.1% of the cases (n= 6) were in low risk group. The maximum diameter of the masses was 10.22 cm. 69.1% of the cases (n=13) showed luminal growth pattern and 38.1% (n=8) showed extraluminal growth pattern. 86.9% of the cases (n=20) had smooth borders, 13.1% of the cases (n=3) had irregular-invasive borders. Conclusion: Imaging is important for diagnosis, theraphy and follow -up. Computed tomography is the first choice for the radiologic evaluation. Also ultrasonography (US), magnetic resonance (MRI) and Florl8-deoxy-glucose positron emission tomography (FDG-PET) can be used in some cases. [ABSTRACT FROM AUTHOR]

31. Evaluation of Mutation Types with Clinicopathological and MRI Features in Breast Cancer Patients with BRCA1 and BRCA2 Mutations.

Objective: We aimed to evaluate the difference in clinicopathological and MRI findings in patients with breast cancer diagnosed with one of the BRCA1-2 mutations. Materials and Methods: Six BRCA1 gene mutant and 7 BRCA2 gene mutant cases diagnosed with breast cancer were included in the study. Age, tumor size, hormone receptors, histopathologic stage, Ki-67 value were recorded. Two radiologists evaluated MRI findings retrospectively according to the 5th edition of ACR BI-RADS. Consensus was achieved in case of non-compliance. Dynamic contrast-enhanced breast MRI examinations were obtained with 3.0 Tesla MRI (Verio, Siemens, Germany). SPSS25 program was used. The analysis was evaluated at 90% confidence interval and 10% significance level. Results: Mean age was 38.5 (SD: 5.4) and mean tumor size was 4.29 cm (SD: 2.5). BRCA1 mutant patients were younger than BRCA2 mutant patients (p=0.05). Ki-67 index was higher in BRCA1 mutant group (p<0.10). Four triple negative and 1 Her2 tumor were assessed in BRCA1 group; Of the BRCA2 group, 5 had luminal B, 1 had Her2 tumor, 1 had triple negative tumor and the distribution was statistically different (p=0.024). Estrogen receptor (ER) was negative in 100% of BRCA1 group while ER was negative in 71.4% of BRCA2 group. The difference was statistically significant (p=0.021). Also there were statistically significant differences in tumor shape, margin, contrast enhancement pattern and location on MRI exams (p<0.10). Round shape, circumscribed margin and peripheral enhancement in noted in BRCA1 group. Inversly, irregular shape, non-circumscribed margin, homogeneous-heterogeneous contrast enhancement were noted in BRCA2 group. The tumors were located posteriorly in BRCA1 group, whereas no posterior localization was found in BRCA2 group. Conclusion: In BRCA1 group, benign morphologic features are observed on MRI, but they might be clinically aggressive. The difference in tumor localization between two genetic subtypes is remarkable and might be important in screening. [ABSTRACT FROM AUTHOR]
