Persistent human papillomavirus (HPV) infection is associated with the development of oropharyngeal squamous cell carcinoma (OPSCC), and increasing incidences of OPSCC are reported. The generally favorable treatment outcome in patients with HPV-driven OPSCC has brought de-escalation of treatment into discussion. Nevertheless, 13% to 25% develop a relapse within two years after current standard treatment. New biomarkers are urgently required to monitor therapy response, tumor burden, and minimal residual disease during follow-up. This observational study examined 50 patients with OPSCC to investigate plasma cell-free (cf) HPV-DNA derived from tumor cells before therapy and during follow-up. Real-time quantitative PCR was applied to quantify the DNA concentration of HPV oncogenes E6 and E7. A total of 85.7% of pretreatment samples from patients with HPV-driven OPSCC (n = 28) were positive for at least one marker, and cfHPV-DNA concentration increased with tumor size. Virtually no signals were detected in HPV-negative OPSCC patients (n = 20; P ≤ 0.001). Patients without clinical evidence of recurrence had significantly reduced cfHPV-DNA concentrations after therapy (P ≤ 0.001). Conversely, cfHPV-DNA levels increased or remained above threshold in five patients who had residual disease or developed recurrence. In conclusion, plasma cfHPV-DNA detection correlates with the clinical course of disease in patients with HPV-driven OPSCC. Consequently, extensive clinical investigation should be considered if cfHPV-DNA is detected during follow-up of patients with HPV-driven OPSCC.

Consequently, de-escalation of standard treatment has been suggested. Due to modest specificity rates, debates are ongoing, whether p16INK4a, a surrogate marker for HPV-driven OPSCC, is sufficient to correctly identify those tumours and avoid substantial HPV misattribution and thus undertreatment of patients by de-escalation. Robust data estimating the proportion of potentially undertreated patients are missing. Methods We assessed a large-scale cohort of consecutively included OPSCC diagnosed between 2000 and 2017 for HPV–DNA, HPV genotypes, p16INK4a expression and multiple tumour- and patient-related risk factors, and investigated their impact on patients’ survival in comprehensive uni- and multivariate analyses. Results Aetiological relevance of HPV (p16INK4a- and high-risk HPV–DNA-positivity) was detected in 27.1% (n = 192) of OPSCC, with HPV16 being the most abundant HPV type (94.6%). In 5.5% patients (n = 39), p16INK4a overexpression but no HPV–DNA was detected. Principal component and survival analyses revealed that 60.6% of these p16INK4a-positive OPSCC lacking HPV–DNA did not resemble HPV16-driven but HPV-negative OPSCC regarding risk-factor profile and overall survival. Notably, this group represented 10.6% of all p16INK4a-overexpressing OPSCC. Conclusions p16INK4a as a single marker appears insufficient to indicate OPSCC patients suitable for treatment de-escalation.

Zusammenfassung Hintergrund Eine Umfrage aus dem Jahr 2009 zeigte, dass lediglich etwa 24 % aller deutschen HNO-Kliniken die Sialendoskopie anboten. Durch die Wiederholung der Untersuchung im Jahr 2016 sollte die Entwicklung der Sialendoskopie an deutschen HNO-Kliniken reevaluiert werden. Material und Methoden Erneut wurde ein selbstentwickelter Fragebogen mit elf Fragen an alle deutschen HNO-Kliniken verschickt. Der gleiche Fragebogen wurde bereits im Jahr 2009 verwendet. Die aktuellen Umfrageergebnisse wurden mit den Ergebnissen von damals verglichen und ausgewertet. Ergebnisse Die Zahl der sialendoskopierenden HNO-Kliniken konnte sich mehr als verdoppeln. Die Anzahl der Interventionen verdreifachte sich. Die Gründe dafür, dass eine Klinik keine Sialendoskopien durchführte, waren verschieden. Ein Hauptargument war ein Mangel an Patienten. Kaum Veränderungen ergaben sich bei ambulanten und stationären Therapien und der Eingriffsdauer. Die Sonografie wird unverändert von allen Kliniken präoperativ durchgeführt. Schlussfolgerung Auffallend ist, dass bei zunehmender Anzahl an Kliniken, die die Sialendoskopie anbieten und steigenden Fallzahlen, ein Teil der Kliniken einen Patientenmangel angibt. Dies könnte für die Etablierung von spezialisierten Zentren sprechen. Inzwischen werden neben der obstruktiven Sialadenitis weitere Krankheitsbilder durch die Sialendoskopie therapiert. Die Sonografie bleibt die präoperative Diagnostik der Wahl. Die Auswertung zeigt, dass die Sialendoskopie in den vergangenen sieben Jahren eine steigende Akzeptanz an deutschen HNO-Kliniken erfahren hat.

Nach Stos mit dem Fahrradlenker gegen den Hals entwickelt ein Kleinkind ein massives Hautemphysem mit Atemnot. Die orotracheale Intubation ist notwendig. Im Computertomogramm (CT) von Hals und Thorax finden sich ein Pneumomediastinum sowie Pneumothoraces beidseits, aber keine Atemwegsverletzung. Mittels Thoraxdrainagen und kontrollierter Beatmung kann das Kind stabilisiert werden. Die Endoskopie der oberen Trachea zeigt einen Einriss der Pars membranacea. Die Verletzung wird erfolgreich konservativ behandelt. Krankheitsbild sowie Notfall- bzw. Atemwegsmanagement werden anhand einer Literaturrecherche diskutiert.

Im Verlauf der Erkrankung kommt es in vielen Fällen zu rezidivierenden Sialadenitiden. Durch verbesserte Diagnostik besteht neben der Submandibulektomie auch die Möglichkeit einer drüsenerhaltenden Therapie, z. B. mittels Miniaturendoskopie (Sialendoskopie).

In 2009 a nationwide survey revealed that only 24 % of the German ENT-hospitals performed sialendoscopy. In 2016 the survey was repeated to reevaluate the actual ranking of sialendoscopy in Germany.Again, the same questionnaire as in 2009 was sent to all German ENT-hospitals. It is a self-developed questionnaire including eleven questions. The results from 2009 and the new results from 2016 were matched with each other.The amount of hospitals performing sialendoscopy doubled and the number of interventions tripled. There were various reasons for denying sialendoscopy. Main reason was a lack of patients. No differences were seen in ambulant vs. inpatient interventions and the duration of sialendoscopy. Preoperative ultrasound was performed in all hospitals.The survey reveals an increasing number of hospitals performing sialendoscopy and an increasing number of sialendoscopies. Simultaneously, some hospitals alleged a lack of patients. These facts could explain a development of specialized centers for obstructive sialadenitis and sialendoscopy. Meanwhile, further salivary gland diseases are treated with sialendoscopy. The preoperative diagnostic of choice is ultrasound. LACRIMAL SYSTEMS ENDOSCOPY WITH A NEW ENDOSCOPE SYSTEMBACKGROUND: Successful therapy of postsaccal stenosis in the lacrimal system can be done by an ENT Surgeon by endonasal dacryocystorhinostomy. It is desirable to have a good diagnostic method of the lacrimal system before undertaking surgery. This study describes the first experiences of endoscopic diagnostics of the lacrimal system with a new endoscope. MATERIAL AND METHODS: 24 patients (10 male and 14 female, between 3 and 65 years old) with epiphora and chronic lacrimal system infections were examined in the ENT Department Homburg/Saar with a 0.53 mm modular endoscope, between 11/2008 and 12/2009. The result were evaluated and analysed. RESULTS: The endoscopic lacrimal system examination was done in all 24 patients. 8 patients with high stenosis postsaccal and purulent epiphora had an endonasal dacryocystorhinostomie. 13 patients with prasaccal stenosis or stenosis in the ductus nasolacrimalis with clear epiphora were treated with irrigation, dilatation and U-intubation. The removal of the Endoscopic dilatation was undertaken in 3 children without using bicanicular intubation. CONCLUSION: Lacrimal system endoscopy with a modular endoscope with the possibility of interventional procedures is an efficient option in the widening diagnosis and therapy of the lacrimalm system disease. With this technique is it possible with selected patients to do interventional therapy. On the basis of the endoscopic findings an interventional therapy or an endonasal dacryocystorhinostomie can be done.

Kurzfassung Beim Planläppen hängen die erzielbare Abtragsrate und Oberflächengüte sehr stark von der Werkstückkinematik ab. Daher ist die genaue Kenntnis dieser Kinematik eine wichtige Voraussetzung wenn es gilt, die Effizienz und Qualität des Feinstbearbeitungsverfahrens zu steigern. Untersuchungen am Institut für Werkzeugmaschinen (IfW) der Universität Stuttgart befassen sich mit den Auswirkungen unterschiedlicher Werkstückgeometrien und Belegungen des Werkstückhalters auf die daraus resultierende Werkstückkinematik. Hierbei wird speziell das Einscheiben-Planläppen mit Reibungskopplung betrachtet. Die optische Erfassung der komplexen Bewegungsmuster der am Läppvorgang beteiligten Komponenten erfolgte mit Hilfe eines am IfW entwickelten Motion Tracking-Verfahrens. Hiermit konnte eine Abhängigkeit der Werkstückkinematik von der Werkstückgeometrie beobachtet werden.

Einleitung: Die Larynxpapillomatose ist der häufigste gutartige Tumor des Larynx im Kinderalter. Daher ist bei Heiserkeit, Stridor und Dyspnoe eines Kindes neben einer Laryngomalazie, subglottischer Stenose oder Stimmlippenparese auch die Larynxpapillomatose als eine wichtige Differenzialdiagnose in Betracht zu ziehen. Kasuistik: Wir berichten über den Fall eines 2jährigen Mädchens, bei dem alio loco bei Dysphonie eine Laryngoskopie durchgeführt wurde. Hier zeigte sich intraoperativ der Befund einer ausgeprägten Larynxpapillomatose. Es erfolgte die Verlegung des intubierten Kindes in unsere Klinik. In der Mikrolaryngoskopie zeigte sich die papillomatöse Veränderung auf der linken Seite von der Taschenfalte bis zur Stimmlippe über die vordere Kommissur reichend mit einem rechtsseitigen fast kompletten Befall der Stimmlippe bis zum Sinus Morgagni. Zunächst wurde die rechtsseitige Abtragung des Befundes vollzogen inklusive Stimmlipppenstripping mit Hilfe eines Mikroscherchens. Die sechs Wochen später durchgeführte Kontroll-Mikrolaryngoskopie zeigte eine papillomfreie gut verheilte rechte Seite. Im selben Eingriff wurde die Papillomabtragung links durchgeführt. Bei positivem HPV-6-Status erfolgte eine Vakzinierung. Kontrollmikrolaryngoskopien 6 Wochen sowie 6½ Monate nach der 2.OP zeigen bei guter Stimmfunktion einen unauffälligen Larynxbefund. Schlussfolgerung: Die Abtragung einer ausgedehnten Larynxpapillomatose kann mit Mikroinstrumenten im Kindesalter erfolgen. Die zweizeitige operative Behandlung ermöglicht das Erreichen einer guten Stimmfunktion. Weitere Erfahrungen müssen gewonnen werden, um den Stellenwert einer Vakzinierung bei differenziertem HPV-Nachweis bestimmen zu könne. Fistulas of the cerebrospinal fluid are often repaired by insertion of grafts of various kinds. However, current knowledge of wound healing after graft insertion is limited, and only a few animal studies are available. The objective of this study is to test whether an in vitro model is suited to analyze cellular healing aspects after duraplasty and to assess dura substitutes in such conditions in regard to their surface attractiveness for cellular migration from the dura margins. Harvested dura pieces from minipigs were perforated to mimic central dura lesions, placed on various coated surfaces (collagen, laminin, poly-L-lysine) or grafts, and investigated in a cell culture for cellular closure of the perforation. Cellular migration from the dura into the central perforation was noted on collagen-coated surfaces and when defects were filled with collagen gels, but there was no cell growth on surfaces with poly-L-lysine or laminin coating. Immunocytochemistry identified the migrating cells mainly as fibroblasts with some intermingled epithelial cells. Scanning electron microscopy proved cellular closure of defects after dura placement on allogenic non-crosslinked collagen transplants. Less cellular migration was observed on poly-p-dioxanon sheets, while no cells migrated into the central dura perforation after placement on a cartilage substitute. Cell counting indicated enhanced cellular closure of the dura opening after introduction of insulin or fibroblast growth factor (sign test for both: 0.031). Our study succeeded in establishing a cell culture model for duraplasty and indicated cellular migration from the dura borders at the site of the defect during the wound healing process. The cell culture model presented in this report shows that collagen grafts are best suited for duraplasty. In accordance with the immunocytological finding of fibroblast migration from the dura borders additional application of fibroblast-stimulating growth factors accelerated cellular defect closure.

This study set out to delineate the Scandinavian Approach to the development of computer-based systems. We aimed to help derive new ideas for human-oriented technology design in other countries. The study is based on the relevant literature, scientific contacts, and two field trips, and covers work in Denmark, Norway, and Sweden. The study focuses on methodological questions and their theoretical foundations, on explicit strategies for social implementation, and on innovative design illustrated by reference to concrete projects. Though it makes no claim to present a sociopolitical analysis of Scandinavian technology design, the sociocultural background is given due consideration. There is no general agreement among Scandinavians as to whether or not there is a well-defined Scandinavian Approach. We have come to identify such an approach in certain common features shared by the different schools of thought. These include efforts toward humanization and democratization as overriding design goals, in keeping with the aim of building an egalitarian society.
