Introduction. This article presents a reflection on the need for a holistic view of the problem of premature births, so that healthcare teams can understand the multiplicity of factors present in premature births, and its consequences for the minor, the mother, the family members, and the healthcare staff involved. Topic of reflection. The topic is based on two investigations carried out with mothers, fathers, and children’s beneficiaries of the Mother Kangaroo Program in a State hospital, at two moments, 2012 and 2014, with some of the authors of this article. Conclusions. In addition to the review of the scientific literature related to the topic and the problem, it is concluded that, under a holistic view, the problem is understood in a comprehensive way and better communication with the parents is encouraged. With the healthcare team, cross-disciplinary handling is achieved, overcoming the objects of isolated studies. As such, there is an integrative and significant approach in the daily life of each actor, from the skin, with the Mother Kangaroo Program, to the rationality of science. Objective: to investigate the understanding of the professionals of the nursing team of a hospital in the interior of Mato Grosso about the role of the psychologist in the hospital environment. Method: descriptive-exploratory study with a qualitative approach. The sample was composed by convenience by nursing professionals from a medium-sized hospital in the south of Mato Grosso. Data were collected through semistructured interviews between June and July 2020, being transcribed and analyzed according to the theoretical precepts of Bardin's content analysis. Results: from the narratives of 24 nursing professionals, two thematic categories emerged, the place and function of the psychologist as a team member; needs and requests for psychological assistance. Conclusion: the participants understand the work of the psychologist focused on what they consider compromising nursing care or satisfactory clinical evolution for hospital discharge. This may be interfering with the request or limitation of psychological assistance, as hospital psychology routinely performs more care after the identification of demands by other professionals who are effectively in direct and prolonged contact with the patient and family. Objective: To understand healthcare professionals' perception of best practices in maternal and child health. Materials and methods: This qualitative exploratory-descriptive study was conducted between December 2020 and March 2021. The study corpus consisted of 23 healthcare professionals from southern Brazil. Data were collected using a semi-structured script and coded based on Minayo's thematic content analysis proposed. Results: Data analysis enabled the delimitation of two thematic categories: "best practices in maternal and child health: from idealization to accomplishment" and "strategies to qualify the maternal and child healthcare network." Conclusions: Healthcare professionals recognize the relevance of best practices in maternal and child health, although this process must be expanded and consolidated. Continuing education, hospitality, ambiance, improved access, and the professional-user connection stand out among the qualifying strategies. [ABSTRACT FROM AUTHOR]

Introduction: Problems related to patient safety in the pre-hospital context are little explored, but essential, given the vulnerability to incidents. Objective: To analyze the patient safety climate from the perspective of the multidisciplinary team that works in Mobile Pre-Hospital Care (PHC). Materials and Methods: Cross-sectional study, conducted in a Mobile Pre-Hospital Care. Data collection was performed through Safety attitudes Questionnaire (SAQ), with convenience sampling and participation rate of 94.3% of eligible professionals. Descriptive statistics and the Mann-Whitney test were used for data analysis. Results: Among the 151 participating professionals, males predominated (54.6%), nursing assistants and technicians (42.0%), working for 10 or more years (61.0%), in adult and pediatric care (93.4%). The SAQ Total had a median of 70, indicating a negative perception of the participants about patient safety in the APH. The domains with a negative perception were: Safety Climate, Stress Recognition, Management Perception and Working Conditions; while the Teamwork Climate and Job Satisfaction domains showed a positive perception. In the comparative analysis among professionals, differences were observed between some categories for the domains of Job Satisfaction, Stress Recognition and Working Conditions. Conclusions: This study presents the peculiarities of emergency medical services and the need to sensitize professionals and managers about patient safety, with a view to better understanding the current scenario and possibilities of reducing adverse events to improve the care offered. Introduction: The presence of physical, psychosocial, and spiritual signs and symptoms should be identified and managed through the palliative care that health care teams and professionals provide. Objective: To identify the palliative care needs of people with heart failure, their caregivers, and the multidisciplinary team of a heart failure unit. Materials and Methods: A mixed-method study with a sequential transformative design (DITRAS, for its acronym in Spanish) was conducted. It began with a quantitative phase in which the Edmonton Symptom Assessment Scale (ESAS), the Therapy-Spiritual Well-Being Scale (FACIT-Sp-12), and the Barthel Index were used. The qualitative phase was conducted with three focus groups involving seven patients, eight caregivers, and twelve health professionals from the multidisciplinary team. Elizabeth Lenz's Theory of Unpleasant Symptoms was used as a guideline for this study. Results: Physiological (edema, fatigue, and dyspnea) and psychological (attitude towards life and enjoyment of hobbies) palliative care needs and situational factors (caregiver dependence and support networks) were identified and could be understood through Lenz's theory. Conclusions: Palliative care needs in patients with heart failure are presented under a framework of symptoms that patients, caregivers, and the health care team perceive. Comprehensive approaches are required to improve symptom experience. [ABSTRACT FROM AUTHOR]

Chronic renal disease (CRD) is an important public health problem. There is no a national renal health program in Peru. There is evidence that the multidisciplinary model of care in patients with CRD delays progression and reduces all-cause mortality. Although there is no agreement on certain topics, such as at which glomerular filtration rate should the multidisciplinary team work, it is clear that outcomes are improved. Human and financial resources are needed to implement this approach. [ABSTRACT FROM AUTHOR]

Background: at the end of life, the patient with cancer conditions presents various physical, emotional and spiritual symptoms. Palliative medicine allows a continuous and comprehensive treatment for the diagnosis and control of symptoms. Objective: to describe the incidence of symptoms according to the location of the initial tumor and its transition in the last stage of the disease. Method: a descriptive, longitudinal, prospective study of 100 terminally ill patients treated at the Institute of Oncology and Radiobiology was carried out between September 2017 and September 2019. The medical history record with two evaluations was taken as a source, using Edmonton Symptom Rating Scale, modified. A questionnaire was developed using the in-depth interview technique to collect information on symptoms. With the information, a database was made in Microsoft Excel 16.0 and they were processed using the SPSS-PC statistical package in version 19.0.1 for Windows, which made it possible to make tables and graphs. Results: the incidence of nine symptoms is described, the main ones: pain, fatigue, loss of appetite, anxiety and depression, independent of the anatomical structure affected by the primary tumor. A higher incidence of pain was found in general (78 %). During the final stage, the most frequent symptoms were: fatigue, anxiety, loss of appetite and dyspnea. Conclusion: the symptoms in terminal patients with cancer diseases are multiple and variable, sometimes closely related to the natural history of their disease. Symptomatic diagnosis and control requires recognizing needs and generating collective strategies to minimize suffering.

Objective: To identify the role of nursing staff on rapid response teams in cases of cardiorespiratory arrest on different continents. Methods: An integrative review of the literature was performed. Searches were conducted in the PubMed, Web of Science, Scopus, Lilacs and CINAHL databases for studies published in Portuguese, English or Spanish between 2010 and 2020 that investigated the role of nursing staff on rapid response teams. The following search terms were used: rapid response team, nursing care, multiprofessional team, cardiac arrest and nursing. Results: The sample included six articles, half of which were from North America. The role of nurses on rapid response teams involved two dimensions: direct care for critical patients until the arrival of medical staff and the administrative functions of continuing education activities, the development of clinical protocols, improvement of communication between different sectors and care management. Conclusions: Nursing staff on rapid response teams play the role of coordinator of multiple care and management activities for hospitalized patients with signs of clinical deterioration who require immediate intervention., Objetivo: Identificar na literatura científica o papel da equipe de enfermagem em equipes de resposta rápida no atendimento à parada cardiorrespiratória em diferentes regiões continentais. - Abstract
Introduction: Frailty is a condition that involves a progressive deterioration of multiple functional areas, affecting occupational performance and causing loss of independence and admission to care facilities. This is a pre-disability situation that requires a multidisciplinary approach from different professional perspectives, amongst which nursing and occupational therapy are included. Method: the objective is to ascertain whether the multidisciplinary intervention of occupational therapy and nursing has a positive impact on the frail elderly individuals admitted in care facilities. For that purpose, a comprehensive literature review of both disciplines was performed. Finally, the articles that were considered appropriate for the objective of this review were selected. Conclusion: following a review of the available literature, it was observed that the current approaches of occupational therapy and nursing to the frail elderly are carried out independently of each other. Both are essential disciplines to early recognise patients at risk, to allow them to remain in their homes for the longest time possible and reduce socio-sanitary costs. In the same manner, it has been observed that there is little evidence that addresses a multidisciplinary approach.

HEALTH care teams , *HYPERTENSION , *MEDICAL care costs , *BLOOD pressure , *MEDICAL screening - Abstract
Hypertension remains the leading cause of cardiovascular disease globally despite the availability of safe and effective treatments. Unfortunately, many barriers exist to controlling hypertension, including a lack of effective screening and awareness, an inability to access treatment and challenges with its management when it is treated. Addressing these barriers is complex and requires engaging in a systematic and sustained approach across communities over time. This analysis aims to describe the key elements needed to create an effective delivery system for hypertension control. A successful system requires political will and supportive leadership at all levels of an organization, including at the point of care delivery (office or clinic), in the health care system, and at regional, state and national levels. Effective screening and outreach systems are necessary to identify individuals not previously diagnosed with hypertension, and a system for follow up and tracking is needed after people are diagnosed. Implementing simple protocols for treating hypertension can reduce confusion among providers and increase treatment efficiency. Ensuring easy access to safe, effective and affordable medications can increase blood pressure control and potentially decrease health care system costs. Task-sharing among members of the health care team can expand the services that are delivered. Finally, monitoring of and reporting on the performance of the health care team are needed to learn from those who are doing well, disseminate ideas to those in need of improvement and identify individual patients who need outreach or additional care. Successful large-scale hypertension programs in different settings share many of these key elements and serve as examples to improve systems of hypertension care delivery throughout the world.
