This position paper, developed by an interdisciplinary expert group of neonatologists, paediatric infectious disease physicians, clinical pharmacists and specialists for the prevention and control of nosocomial infections, describes the "Good handling practice of medicines parenterally administered to patients on NICUs". It takes equal account of patient safety and the specialties of neonatal intensive care regarding feasibility and proportionality. The overall concept is perceived as a "learning system", in which open communication within the health-care team relating to medication errors and critical incidents enables continuous development and improvement to ensure patient safety. In our opinion, pharmacists, who are responsible for the supply of ready-to-administer parenteral medicinal products for neonatal intensive care patients, as well as the hygiene staff responsible on site are integral parts of the interdisciplinary treatment team. Risks of the current clinical practice of parenteral treatment of NICU patients are discussed in detail and recommendations for safety-relevant procedures are given. Das hier vorgelegte Positionspapier wurde von einer interdisziplinären Expertengruppe, bestehend aus Neonatologen, pädiatrischen Infektiologen, klinischen Pharmazeuten und Spezialisten für Prävention und Kontrolle nosokomialer Infektionen, erarbeitet und beschreibt die "Gute Praxis der parenteralen Arzneimitteltherapie". Diese trägt gleichermaßen der Sicherheit der Patienten und den besonderen Gegebenheiten einer neonatologischen Intensivstation Rechnung, wobei auch Machbarkeit und Verhältnismäßigkeit der praktischen Umsetzung berücksichtigt werden. Das Gesamtkonzept wird als ein "lernendes System" aufgefasst; das heißt Fehler (critical incidents) hinsichtlich der Patientensicherheit werden im Behandlungsteam offen kommuniziert, sodass eine stetige Weiterentwicklung und Verbesserung der Abläufe möglich wird. Apotheker, die für die Versorgung von neonatologischen Intensivpatienten mit parenteral zu verabreichenden Arzneimitteln verantwortlich sind, gelten nach unserem Verständnis (genau wie das vor Ort zuständige Hygienefachpersonal) als integraler Bestandteil des interdisziplinären Behandlungsteams. Die Risiken, die mit der etablierten klinischen Praxis der parenteralen Therapie von Früh-/Reifgeborenen auf Intensivstationen einhergehen, werden von der Expertengruppe im Einzelnen dargestellt und es werden Hinweise für sicherheitsrelevante Vorgehensweisen gegeben.

Meconium constitutes infants' first bowel movements postnatally. The consistency and microbial load of meconium are different from infant and adult stool. While recent evidence suggests that meconium is sterile in utero, rapid colonization occurs after birth. The meconium microbiome has been associated with negative health outcomes, but its composition is not well described, especially in preterm infants. Here, we characterized the meconium microbiomes from 330 very preterm infants (gestational ages 28 to 32 weeks) from 15 hospitals in Germany and in fecal samples from a subset of their mothers (N = 217). Microbiome profiles were compiled using 16S rRNA gene sequencing with negative and positive controls. The meconium microbiome was dominated by Bifidobacterium, Staphylococcus, and Enterococcus spp. and was associated with gestational age at birth and age at sample collection. Bifidobacterial abundance was negatively correlated with potentially pathogenic genera. The amount of bacterial DNA in meconium samples varied greatly across samples and was associated with the time since birth but not with gestational age or hospital site. In samples with low bacterial load, human mitochondrial sequences were highly amplified using commonly used, bacterial-targeted 16S rRNA primers. Only half of the meconium samples contained sufficient bacterial material to study the microbiome using a standard approach. To facilitate future meconium studies, we present a five-level scoring system ("MecBac") that predicts the success of 16S rRNA bacterial sequencing for meconium samples. These findings provide a foundational characterization of an understudied portion of the human microbiome and will aid the design of future meconium microbiome studies. We provide a characterization of the microbiome structure and composition of infant meconium and maternal feces from a large study cohort and propose a method to evaluate meconium samples for bacterial sequencing suitability. These findings provide a foundational characterization of an understudied portion of the human microbiome and will aid the design of future meconium microbiome studies.

The two-week block rotation in paediatrics (tenth semester) took place for 62 students purely as online teaching in the summer semester of 2020, at the time of the initial restrictions. As a teaching module, virtual patient presentations including debriefing took place as synchronous teaching. Patients and one parent were broadcast from the wards and outpatient clinics via video conference. Students were able to interact in small groups with 15-22 patients or their parents, respectively, via a doctor and both conduct the case history interview and brief the examination steps. Despite the limitation of not being able to perform the clinical examination themselves, participants rated the block rotation with good marks. They particularly appreciated the ability to interact with the children online as an indispensable compromise in times of suspended classroom teaching during the SARS-CoV-2 pandemic. Das zweiwöchige Blockpraktikum Pädiatrie (10. Semester) fand für 62 Studierende im Sommersemester 2020 zur Zeit der Ausgangsbeschränkung als reiner Online-Unterricht statt. Als ein Unterrichtsbaustein fanden virtuelle Patientenvorstellungen inkl. Nachbesprechung als synchrone Lehre statt. Patientinnen und Patienten sowie ein Elternteil wurden von den Stationen und Ambulanzen per Videokonferenz übertragen. Die Studierenden konnten in Kleingruppen über eine ärztliche Person mit jeweils 15-22 Patienten bzw. deren Eltern interagieren und sowohl das Anamnesegespräch führen, also auch Untersuchungsschritte instruieren. Die Teilnehmenden bewerteten das Blockpraktikum trotz der Limitierung, nicht selbst die klinische Untersuchung durchführen zu können, mit guten Noten und wertschätzten insbesondere die Möglichkeit zur Online-Interaktion mit den Kindern als unverzichtbaren Kompromiss in Zeiten der ausgesetzten Präsenzlehre während der SARS-CoV-2-Pandemie.
