Summary Background/Objectives The aim of this study was to evaluate large field of view (FoV) subjective image quality of three cone-beam computed tomography (CBCT) machines to assess the threshold for reliable diagnostic perceptibility when lowering exposure settings. Materials and methods One entire cadaver’s head was scanned using three CBCT scanners. The largest available FoV of each device, imaging orthodontic relevant structures, was applied. CBCT datasets with different image quality based on standard to minimum scanning parameters were acquired. Five dentists evaluated the visibility of selected anatomical structures of the upper and lower jaw using a 5-point rating scale. Results Image quality depends on parameters such as a minimum voxel size of 0.2 to 0.4mm or a minimum of 4mA. A reduction in number of images (scanning protocol) reduces image quality. Visualization of different anatomical structures for orthodontic treatment planning requires distinct scanning protocols to support adequate perception of these structures. Limitations This study does not account for the evaluation of paediatric anatomical structures due to the availability of cadaver’s head. Conclusions CBCT scans performed for orthodontic purposes using a large FoV with reduced parameters (400 µm, 2 to 4 mA and low dose protocols) are acceptable for visualization of large anatomical structures. Further lowering these parameters will not be sufficient to view small anatomical structures. Orthodontic indications will have to define specific anatomical structures to choose adequate scanning protocols to reduce dose and ensure reliable diagnostic visibility.

Purpose: The design and performance of a novel head coil setup for (Formula presented.) P spectroscopy at ultra-high field strengths (7T) is presented. The described system supports measurements at both the (Formula presented.) H and (Formula presented.) P resonance frequencies. Methods: The novel coil consists of 2, actively detunable, coaxial birdcage coils to give homogeneous transmit, combined with a double resonant 30 channel receive array. This allows for anatomical imaging combined with (Formula presented.) P acquisitions over the whole head, without changing coils or disturbing the subject. A phosphate buffer phantom and 3 healthy volunteers were scanned with a pulse acquire CSI sequence using both the novel array coil and a conventional transceiver birdcage. Four different methods of combining the array channels were compared at 3 different levels of SNR. Results: The novel coil setup delivers significantly increased (Formula presented.) P SNR in the peripheral regions of the brain, reaching up to factor 8, while maintaining comparable performance relative to the birdcage in the center. Conclusions: The new system offers the potential to acquire whole brain (Formula presented.) P MRSI with superior signal relative to the standard options.

The purpose of this study was to determine the detection rate of distant metastasis and synchronous cancer, comparing clinically used imaging strategies based on chest x-ray 1 head and neck MRI (CXR/MRI) and chest CT 1 head and neck MRI (CHCT/MRI) with 18F-FDG PET/CT upfront in the diagnostic workup of patients with oral, pharyngeal, or laryngeal cancer. Methods: This was a prospective cohort study based on paired data. Consecutive patients with histologically verified primary head and squamous cell carcinoma at Odense University Hospital from September 2013 to March 2016 were considered for the study. Included patients underwent CXR/MRI and CHCT/MRI as well as PET/CT on the same day and before biopsy. Scans were read masked by separate teams of experienced nuclear physicians or radiologists. The true detection rate of distant metastasis and synchronous cancer was assessed for CXR/MRI, CHCT/MRI, and PET/CT. Results: A total of 307 patients were included. CXR/MRI correctly detected 3 (1%) patients with distant metastasis, CHCT/MRI detected 11 (4%) patients, and PET/CT detected 18 (6%) patients. The absolute differences of 5% and 2%, respectively, were statistically significant in favor of PET/CT. Also, PET/CT correctly detected 25 (8%) synchronous cancers, which was significantly more than CXR/MRI (3 patients, 1%) and CHCT/MRI (6 patients, 2%). The true detection rate of distant metastasis or synchronous cancer with PET/CT was 13% (40 patients), which was significantly higher than 2% (6 patients) for CXR/ MRI and 6% (17 patients) for CHCT/MRI. Conclusion: A clinical imaging strategy based on PET/CT demonstrated a significantly higher detection rate of distant metastasis or synchronous cancer than strategies in current clinical imaging guidelines, of which European ones primarily recommend CXR/MRI, whereas U.S. guidelines preferably point to CHCT/MRI in patients with head and neck squamous cell carcinoma.

OBJECTIVE: Head and neck ultrasonography (HNUS) increasingly is used as a point-of-care diagnostic tool by otolaryngologists. However, ultrasonography (US) is a very operator-dependent image modality. Hence, this study aimed to explore the diagnostic accuracy of surgeon-performed HNUS and to establish validity evidence for an objective structured assessment of ultrasound skills (OSAUS) used for competency-based assessment.STUDY DESIGN: A prospective experimental study.METHODS: Six otolaryngologists and 11 US novices were included in a standardized test setup for which they had to perform focused HNUS of eight patients suspected for different head and neck lesions. Their diagnostic accuracy was calculated based on the US reports, and two blinded raters assessed the video-recorded US performance using the OSAUS scale.RESULTS: The otolaryngologists obtained a high diagnostic accuracy on 88% (range 63%-100%) compared to the US novices on 38% (range 0-63%); P CONCLUSION: Strong validity evidence supported the use of the OSAUS scale to assess HNUS competence with good reliability, significant discrimination between US competence levels, and a strong correlation of assessment score to diagnostic accuracy.
