Abstract Objective: Evaluate histological changes in testicular parameters after hormone treatment in transgender women. Methods: Cross-section study with patients who underwent gonadectomy at Hospital de Clínicas de Porto Alegre from 2011 to 2019. Hormone treatment type, route of administration, age at initiation and duration were recorded. Atrophy parameters were observed: testicular volume, tubular diameter, basal membrane length, presence of spermatogonia and spermatids (diploid and haploid spermatozoid precursors). Results: Eighty-six patients were included. Duration of hormone treatment is associated with testicular atrophy and spermatogenesis arrest. Other characteristics of hormone treatment such as age of initiation, route of administration and type of treatment were not associated with testicular histological changes. Testicular volume may predict spermatogenesis arrest. Basal membrane length and tubular diameter ratio is an interesting predictor of germ cell presence. Conclusion: Cross-sex hormone treatment affects testicular germ cell presence. Basal membrane length and tubular diameter ratio reduces inter variability of measurements and better exemplify how atrophic seminiferous tubules are. Fertility preservation should be addressed by healthcare providers in order to recognize gender affirming treatment impact on transgender health.

Abstract Objective: Seminal cryopreservation causes significant damage to the sperm; therefore, different methods of cryopreservation have been studied. The aim of the study was to compare the effects of density gradient processing and washing/centrifugation with seminal plasma removal for cryopreservation in semen parameters. Methods: Seminal samples of 26 normozoospermic patients were divided into 3 parts: with seminal plasma; after washing/centrifugation; and after selection through density gradient. The samples were cryopreserved for at least two weeks. Motility, sperm count, morphology and viability were evaluated before cryopreservation and after thawing. Abstract The TP53 3’UTR variant rs78378222 A>C has been detected in different tumor types as a somatic alteration that reduces p53 expression through modification of polyadenylation and miRNA regulation. Its prevalence is not yet known in all tumors. Herein, we examine tumor tissue prevalence of rs7837822 in Brazilian cohorts of patients from south and southeast regions diagnosed with lung adenocarcinoma (LUAD, n=586), sarcoma (SARC, n=188) and uterine leiomyoma (ULM, n=41). The minor allele (C) was identified in heterozygosity in 6/586 LUAD tumors (prevalence = 1.02 %) and none of the SARC and ULM samples. Additionally, next generation sequencing analysis revealed that all variant-positive tumors (n=4) with sample availability had additional pathogenic or likely pathogenic somatic variants in the TP53 coding regions. Among them, 3/4 (75 %) had the same pathogenic or likely pathogenic sequence variant (allele frequency C (p.Ile251Leu). Our results indicate a low somatic prevalence of rs78378222 in LUAD, ULM and SARC tumors from Brazilian patients, which suggests that no further analysis of this variant in the specific studied regions of Brazil is warranted. However, these findings should not exclude tumor molecular testing of this TP53 3’UTR functional variant for different populations.

Abstract Residency is still considered the gold standard for quality medical training, and acquiring a professional identity as a specialist is one of its central elements. Residents obtain this identity through both the educational environment and direct interaction with peers and supervisors. However, modifications in health care and educational routines during the recent coronavirus disease 2019 (COVID-19) pandemic have significantly impaired these channels. This study is part of a qualitative research project to analyze professional identity formation in a medical residency program in obstetrics and gynecology at a public hospital in southern Brazil. The authors conducted 28 semi-structured interviews with medical residents and preceptors, as well as a focus group with the residents, which was recorded, transcribed, and analyzed in an effort to construct major analytical categories. Restricted movement and physical contact have forced the use of alternative means of interpersonal interaction, such as communication through social media or instant messaging applications. This has also affected educational activities, such as morning rounds, lectures, and seminars. These changes represent a significant impact, especially in Brazil, where physical proximity is an important cultural feature, even in the work and school environments. We speculate that this new type of virtual interaction will also affect the formation of professional identity among obstetrician-gynecologists. These findings suggest that medical residency programs should be attentive to changes in resident training to ensure that the specialist profile and the expected skills, which are consolidated over many years, are not lost.

Objective: To analyze the factors associated with the birth of small for gestational age newborns. Methods: A cross-sectional study was performed with 15 years old or younger postpartum adolescents divided into small-for-gestational-age newborn (SGA) and non-small-for-gestational age newborn groups (NSGA). Socio-demographic, clinical, prenatal care, delivery, postpartum data and anthropometric measures were collected. Results: 8,153 women gave birth at the obstetric ward and 364 (4.46%) ≤ 15 years old adolescents were enrolled in the study. The proportion of SGA newborns was 34.61%. Abstract Objective To assess the daily dietary intake and energy contribution of ultraprocessed foods among women who are positive and negative for the human immunodeficiency virus (HIV) during pregnancy. Methods This case-control study included 77 HIV-positive and 79 HIV-negative puerperal women between 2015 and 2016. The socioeconomic and maternal demographic data were assessed, and a food frequency questionnaire (FFQ) adapted for pregnant women was applied. The Fisher exact test and the Mann-Whitney test were applied to detect differences between the groups. Linear regression was used to assess the associations between the intake of ultra-processed food and energy, macro- and micronutrients, with values of p < 0.05 considered significant. Results The HIV-positive group was older (p< 0.001) and had lower income (p= 0.016) and level of schooling (p< 0.001) than the HIV-negative group. Both groups presented similar average food intake: 4,082.99 Kcal/day and 4,369.24 Kcal/day for the HIV-positive and HIV-negative women respectively (p= 0.258).The HIV-positive group consumed less protein (p= 0.048), carbohydrates (p= 0.028) and calcium(p= 0.001), andmore total fats (p= 0.003). Ultra-processed foods accounted for 39.80% and 40.10% of the HIV-positive and HIV-negative groups’ caloric intake respectively (p= 0.893). The intake of these foods was associated with a higher consumption of carbohydrates (p < 0.001), trans fat (p= 0.013) and sodium (p< 0.001), as well as lower protein (p < 0.001) and fiber intake (p= 0.022). Conclusion These findings demonstrate that the energy consumption and ultraprocessed food intake were similar in both groups, which reinforces the trend toward a high intake of ultra-processed food in the general population. The intake of ultraprocessed food was positively associated with the consumption of carbohydrates, trans fat and sodium, and negatively associated with the consumption of protein and fiber.

A Síndrome dos Ovários Policísticos (SOP) é uma das disfunções endócrinas mais comuns em mulheres em idade reprodutiva, com uma prevalência que varia entre 6 e 16%, sendo motivo frequente de consulta em um ambulatório de Ginecologia e Obstetrícia. Esta revisão enfoca alguns dos principais riscos tardios para a saúde das pacientes com SOP na meia idade e após a menopausa. A busca da literatura foi realizada no Pubmed utilizando-se as seguintes palavras-chave: Polycystic Ovary Syndrome; time. Identificou-se 229 artigos relacionando SOP e suas consequências tardias que foram publicados em periódicos indexados no Pubmed entre 1º de janeiro de 2010 e 19 de junho de 2020. Desses, excluiu-se 3 por não estarem em língua inglesa (2 em chinês, 1 em russo), 177 pelo título e pelo abstract, e 27 com base na análise do texto. Os 22 artigos restantes estão contemplados nesta revisão sistematizada. Evidenciam-se complicações metabólicas, cardiovasculares, neoplásicas e psíquicas, bem como uma complexa, e ainda não totalmente elucidada, relação com a obesidade. Apesar das complicações cardiovasculares, não existem evidências de que estas levem a maior mortalidade em pacientes com SOP, tampouco de que a mortalidade por todas as causas seja maior nessas mulheres.

Benign cystic mesothelioma is a rare tumor originated from mesenchymal cells of the abdominal and pelvic peritoneum. This rare tumor should be considered in the differential diagnosis of young women presenting with abdominal pain and a multicystic abdominal mass. Abstract Aim To assess ovarian reserve (OVR) by means of follicle-stimulating hormone (FSH), anti-Müllerian hormone (AMH), and antral follicle count (AFC) measurement in eumenorrheic women with breast cancer, exposed to gonadotoxic chemotherapy. Method Fifty-two women (35.3 ± 3.8 years old) with breast cancer and undergoing cyclophosphamide-containing chemotherapy were enrolled. The assessment was performed before chemotherapy (T1) and after 2 (T2) and 6 months (T3). Results Six months after chemotherapy, the prevalence of regular cycles was 60%. Anti-Müllerian hormone decreased down to undetectable levels at T2 and T3 (T1: 2.53 [1.00–5.31]; T2 < 0.08; T3: < 0.08 [< 0.08–1.07] ng/mL), (p< 0.0001). Antral follicle count was 11 [8.0–13.5] follicles at T1 and lower at T2 (5.50 [3.75–8.0] and T3 (5.0 [2.5–7.0]) (p< 0.0001). In patients who remained with regular cycles during chemotherapy or resumed normal menses, FSH and estradiol levels remained unchanged. Conclusion Anti-Müllerian hormone and AFC are useful as markers of OVR decline in women exposed to chemotherapy. Follicle-stimulating hormone is only adequate in women who become amenorrheic.

Introduction: Depression among human immunodeficiency virus (HIV)-seropositive individuals has been associated with reduced quality of life. Objective: To evaluate the effect of depressive symptoms and HIV exposure on mean quality of life scores in HIV-seropositive and HIV-seronegative postpartum women. Methods: A cross-sectional study was conducted with two groups: 80 HIV-seropositive and 80 HIV-seronegative postpartum women. The Edinburgh Postnatal Depression Scale and the World Health Organization Quality of Life short-version scale were used to assess presence of depressive symptoms and quality of life scores. Two-way analysis of variance was used to compare the effects of depressive symptoms, HIV exposure and interaction between depressive symptoms and HIV exposure on mean quality of life scores, with p < 0.05 considered statistically significant. Results: Depressive symptoms were present in 35% (28) of HIV-seropositive and 17.5% (14) of HIV-seronegative participants (p = 0.02). The interaction between depressive symptoms and HIV exposure was not significant for any quality of life domain. The main effect of HIV exposure was also not significant. Depressive symptoms had a negative influence on quality of life scores in all domains (physical health, psychological health, social relationships and environment) (p < 0.001). Conclusion: The quality of life of pregnant women is negatively influenced by the presence of depressive symptoms. Keywords: Pregnancy; quality of life; depression and HIV

Objective: To evaluate factors that may interfere in mother-to-child transmission (MTCT) of human immunodeficiency virus (HIV). Method: A historical cohort study with a sample of 299 HIV-infected mothers and their newborns who delivered at the Obstetric Center of the Hospital de Clínicas de Porto Alegre, southern Brazil, from January 2010 to December 2014. Results: Of the 299 newborns of HIV-infected mothers, 3.7% (n = 11) were infected. Of those, 90.9% (n = 10) were born by cesarean section; 90.9% (n = 10) had ≥ 37 weeks; 54.6% (n = 6) received zidovudine starting within the first 4 hours after birth; and 45.4% (n = 5) received zidovudine and nevirapine. Four women whose newborns were infected with HIV had syphilis during pregnancy (36.4%). Poor adherence to highly active antiretroviral therapy (HAART) (p < 0.003), viral load ≥ 1000 copies/mL or ignored in the third trimester (p < 0.000), and CD4 count < 500 cells/mm3 in the third trimester (p < 0.046) were significantly associated with an increased risk of MTCT. Conclusion: Lack of control of risk factors may contribute to unfavorable rates of MTCT of HIV. OBJETIVO: observar o impacto da obesidade e de outros fatores de risco sobre a taxa de falha das pacientes submetidas à cirurgia de Burch para tratamento da incontinência urinária. MÉTODOS: estudo de casos de pacientes submetidas à cirurgia de Burch no período de 1992 a 2003. As pacientes foram avaliadas no momento da segunda consulta pós-operatória (66 dias em média) e com um ano de acompanhamento, e classificadas em dois grupos: Continentes e Não Continentes. As variáveis analisadas foram: idade, paridade, índice de massa corpórea (IMC), tempo de menopausa, tempo de terapia de reposição hormonal, avaliação urodinâmica, história de infecção do trato urinário, cirurgia prévia para incontinência urinária, diabetes, cistocele e prolapso uterino, tempo de internação, necessidade de autossondagem, micção espontânea no pós-operatório e ferida operatória. Os dados foram analisados com o pacote estatístico Statistical Package for Social Sciences 14.0. Foram utilizados o teste τ de Student ou Mann-Whitney, para comparação das variáveis contínuas, e os testes exato de Fisher e χ2, para variáveis categóricas (p30 foi 3,7 (IC95%=1,2-11,5). CONCLUSÕES: a obesidade mostrou-se um importante fator de risco para a falha da cirurgia no primeiro ano de acompanhamento. Os resultados demonstram que pacientes com IMC>30 têm chance 3,7 vezes maior de apresentarem-se não continentes após um ano da cirurgia de Burch em relação às não obesas.PURPOSE: to observe the impact of obesity and other risk factors on the rate of failure in patients submitted to Burch's surgery for the treatment of urinary incontinence. METHODS: cases study of patients submitted to Burch's surgery, from 1992 to 2003. Patients were evaluated at the second post-surgery appointment (average 66 days) and after one-year follow-up, and classified in two groups: Continent and Non-continent. Variables analyzed were: age, parity, body mass index (BMI), menopause duration, duration of hormonal therapy, urodynamic evaluation, history of urinary tract infection, previous urinary incontinence surgery, diabetes, cystocele and uterine prolapse, time spent in hospital, necessity of self-probing, post-surgical spontaneous micturition, and surgical wound. Data were analyzed with the Statistical Package for Social Sciences 14.0 statistical package. For the comparison of continuous variables, Student's t-test or Mann-Whitney test were used, and Fisher exact and χ2 tests, for the categorical variables (p30 Odds Ratio was 3.7 (CI95%=1.2-11.5). CONCLUSIONS: obesity has shown to be an important risk factor for the surgery failure in the first follow-up year. Results show that patients with BMI>30 have 3.7 times more chance of being non-continent one-year after Burch's surgery than non-obese patients.

Leiomiomas são tumores benignos. Eles surgem no miométrio e contêm quantidade variável de tecido conjuntivo fibroso. Cerca de 75% dos casos são assintomáticos, encontrados ocasionalmente durante exame abdominal, pélvico bimanual ou ultra-sonografia. Os sintomas são relacionados diretamente ao tamanho, ao número e à localização dos miomas. Nessa revisão, são apresentadas as abordagens terapêuticas atuais clínicas (anticoncepcionais orais, progestágenos e antiprogestágenos, análogos do hormônio liberador das gonadotrofinas (GnRH), e antiinflamatórios não esteróides) e cirúrgicas (histerectomia, miomectomia e embolização) para o tratamento de leiomiomas.Leiomyomas are benign tumors. They appear in the myometrium and present a variable amount of fibrous conjunctive tissue. About 75% of the cases are not symptomatic and are usually found during abdominal, bimanual pelvic examination or during ultrasonography. The symptoms are directly related to the size, number and localization of the myomas. In the present review, the current clinical therapeutic procedures (oral anti-conceptive drugs, progestins and anti-progestins, analogues of the gonadothrophins’ releasing hormone (GnRH), and non-steroid anti-inflammatory drugs), and also the surgical procedures (hysterectomy, myomectomy, embolization) are presented for the treatment of leiomyomas.

OBJETIVO: Comparar a expressão gênica (mRNA) e protéica dos protooncogenes c-fos, c-myc e c-jun em miométrio normal e mioma humanos. MÉTODOS: Foi realizado um estudo do tipo caso-controle. O material foi coletado de 12 pacientes submetidas a histerectomia no Hospital de Clínicas de Porto Alegre. A expressão do mRNA específico para c-myc, c-fos, c-jun e beta-microglobulina foi avaliada pela técnica de RT-PCR, utilizando primers específicos para cada gene. A expressão protéica destes protooncogenes foi avaliada através de Western blot com anticorpos específicos. RESULTADOS: Não houve diferença significativa para expressão gênica desses protooncogenes entre miométrio normal e mioma (c-myc: 0,87 ± 0,08 vs 0,87 ± 0,08, p = 0,952; c-fos: 1,10 ± 0,17 vs 1,01 ± 0,11, p = 0,21; c-jun: 1,03 ± 0,12 vs 0,96 ± 0,09, p = 0,168, respectivamente). Não houve diferença significativa para expressão protéica desses protooncogenes entre miométrio normal e mioma (c-myc: 1,36 ± 0,48 vs 1,53 ± 0,29, p = 0,569; c-fos: 8,85 ± 5,5 vs 6,56 ± 4,22, p = 0,434; e c-jun: 6,47 ± 3,04 vs 5,42 ± 2,03, p = 0,266, respectivamente). CONCLUSÃO: A expressão gênica (transcrição) e a expressão protéica (tradução) dos protooncogenes c-myc, c-fos e c-jun em mioma e miométrio normal são semelhantes.Uterine myomas are common benign tumors of the female genital tract. The expression of growth factor signal transduction cascade components including the protooncogenes c-myc, c-fos, and c-jun seem to be involved in the development of myomas. PURPOSE: To compare the gene (mRNA) and protein expression of the protooncogenes c-fos, c-myc, and c-jun in human normal myometrium and leiomyoma. METHOD: A case-control study was performed. Samples were collected from 12 patients submitted to hysterectomy at the Hospital de Clínicas at Porto Alegre. The expression of the specific mRNA for c-myc, c-fos, c-jun, and beta-microglobulin was assessed through the RT-PCR technique, using specific primers to each gene. The protein expression of these protooncogenes was evaluated through the Western blot technique with specific antibodies. RESULTS: No statistically significant difference was observed in the gene expression for these protooncogenes between normal myometrium and leiomyoma (c-myc: 0,87 ± 0,08 vs 0,87 ± 0,08, p = 0,952; c-fos: 1,10 ± 0,17 vs 1,01 ± 0,11, p = 0,21; c-jun: 1,03 ± 0,12 vs 0,96 ± 0,09, p = 0,168, respectively). A carcinogênese é um processo de múltiplas etapas. Alterações no equilíbrio citogenético ocorrem na transformação do epitélio normal a câncer cervical. Numerosos estudos apoiam a hipótese de que a infecção por HPV está associada com o desenvolvimento de alterações malignas e pré-malignas do trato genital inferior. Neste trabalho são apresentadas as bases para a compreensão da oncogênese cervical. O ciclo celular é controlado por proto-oncogenes e genes supressores. Quando ocorrem mutações, proto-oncogenes tornam-se oncogenes, que são carcinogênicos e causam multiplicação celular excessiva. A perda da ação de genes supressores funcionais pode levar a célula ao crescimento inadequado. O ciclo celular também pode ser alterado pela ação de vírus, entre eles o HPV (Human Papiloma Virus), de especial interesse na oncogênese cervical. Os tipos de HPV 16 e 18 são os de maior interesse, freqüentemente associados a câncer cervical e anal. O conhecimento das bases moleculares que estão envolvidas na oncogênese cervical tem sido possível devido a utilização de técnicas avançadas de biologia molecular. A associação destas técnicas aos métodos diagnósticos clássicos, poderão levar a uma melhor avaliação das neoplasias cervicais e auxiliar no desenvolvimento de novas terapias, talvez menos invasivas e mais efetivas.Carcinogenesis involves several steps. Disorders of the cytogenetic balance occur during the evolution from normal epithelium to cervical cancer. Several studies support the hypothesis that the Human Papiloma Virus (HPV) infection is associated to development of premalignant and malignant lesions of cervical cancer. In this review we show the basis to understand cervical oncogenesis. The cell cycle is controlled by protooncogenes and supressive genes. This orchestrated cell cycle can be affected by virus such as HPV. Of special interest in the cervical carcinogenesis are the HPV subtypes 16 and 18. How HPV immortalizes cervical cells is not fully understood. Advances have been made in the application of molecular biology techniques in the understanding of this mechanism. Once established, these techniques will lead to a better assessment of cervical neoplasias and help the development of new therapies, hopefully less invasive and more effective.

INTRODUÇÃO: pacientes em amenorréia primária com disgenesia gonadal têm níveis elevados de gonadotrofinas e necessitam de avaliação cromossômica. O estudo citogenético (cariótipo) pode ser realizado na gônada ou no sangue periférico. Nos casos de amenorréia primária sem sinal de virilização, a necessidade de investigação adicional do cariótipo da gônada não está estabelecida. OBJETIVO: revisar os cariótipos de gônadas (ovários) de mulheres com amenorréia primária e compará-los com os resultados do cariótipo no sangue periférico, relacionando-os às características fenotípicas das pacientes. MÉTODOS: foram analisados retrospectivamente os dados clínicos e os cariótipos de doze pacientes atendidas no período de janeiro de 1997 a dezembro de 2003 no Hospital de Clínicas de Porto Alegre. RESULTADOS: nas pacientes incluídas, quando o motivo da investigação foi amenorréia primária sem sinal de virilização, o cariótipo da gônada foi concordante com o cariótipo do sangue periférico nos oito casos avaliados (sete pacientes com cariótipo 46XX e uma paciente com cariótipo 46XY). A paciente oito foi a única com sinal de virilização (hipertrofia de clitóris) e o único caso de cariótipos discordantes. CONCLUSÃO: este estudo sugere que o cariótipo de gônada não traz informação adicional ao cariótipo do sangue periférico nas pacientes com amenorréia primária sem sinais de virilização. Até o momento todos os trabalhos publicados tiveram número pequeno de pacientes. A análise da relação custo-benefício pode permitir redução de estresse psicológico para paciente e familiares, bem como redução de custos para as instituições.BACKGROUND: patients with primary amenorrhea and gonadal dysgenesia have higher serum gonadotrophins and should be submitted to chromosome studies. Karyotype studies may be performed in gonadal tissue or peripheral blood however, it is not yet established if cases of primary amenorrhea without signs of virilization need additional investigation of gonadal karyotype. PURPOSE: to analize the gonadal karyotypes (ovaries) from patients with primary amenorrhea and compare them to their respective peripheral blood karyotypes. METHODS: clinical and karyotype data of 12 patients were retrospectively analyzed from January 1997 to December 2003. RESULTS: when the investigation was indicated for primary amenorrhea without signs of virilization, the gonadal and peripheral blood karyotypes were concordant in 8 cases (7 cases 46XX and 1 case 46XY). One patient with virilization signs was the only case of discordant karyotype. CONCLUSION: the present study suggests that the gonadal karyotype does not bring additional information to peripheral blood karyotype in patients with amenorrhea and no signs of virilization. Although all previous studies had a small number of patients, it seems advisable to investigate the gonadal karyotype in patients with signs of virilization. The cost-benefit analysis could allow cost and stress reduction for patients, family and institutions.

OBJETIVOS: analisar evidências da literatura médica que avaliam a relação entre miomas uterinos e infertilidade, assim como as vantagens e desvantagens dos diferentes tipos de tratamentos disponíveis. MÉTODOS: foi realizada uma revisão da literatura utilizando os termos (MeSh terms) "miomas", "infertilidade", "tratamento", "miomectomia", "gestação", na base de dados "Pubmed" e na "The Cochrane Library 2003". O período analisado foi de 1989 a 2003. RESULTADOS: apesar de alguns resultados contraditórios, a maioria dos autores relata uma possível relação de causa-conseqüência entre determinados tipos de miomas e distúrbios reprodutivos. Em relação à terapêutica, observa-se claramente que a miomectomia representa a técnica cirúrgica com melhores resultados, não havendo diferença significativa entre as vias de acesso utilizadas, com exceção da miomatose submucosa, situação em que a via histeroscópica é preferida. As opções não-cirúrgicas de tratamento existentes não parecem ter indicação em pacientes inférteis. CONCLUSÕES: a possível associação entre miomas e distúrbios reprodutivos necessita de melhores investigações. O incremento nas taxas de gestação após ressecção cirúrgica de miomas submucosos leva-nos a acreditar que esses tumores tenham papel na etiologia da infertilidade. Em miomas de outras localizações, entretanto, a relação com infertilidade não é tão clara. A miomectomia é o procedimento de escolha para as pacientes que ainda desejam engravidar.OBJECTIVES: to analyze evidences in medical literature concerning the possible relation between uterine leiomyomas and infertility, and the advantages /disadvantages of the different treatments available. METHODS: literature review using the key words (MeSh terms) "leiomyomas", "infertility", "treatment", "myomectomy", "pregnancy" in "Pubmed" and "The Cochrane Library 2003" database. The search period was from 1989 through 2003. RESULTS: although there are contradictory results, the majority of authors determined a possible cause-consequence relation between some types of leiomyomas and reproductive impairement. Concerning therapeutics, we were able to clearly place myomectomy as being the surgical technique with the best results. Surgical approaches used for this procedure did not show significant differences from each other, except for submucosal myomas, where histeroscopy is more effective. Other existent treatment options do not seem to be indicated for infertile patients with uterine leiomyomas. CONCLUSIONS: the possible association between myomas and reproductive impairment needs to be further investigated. Success indicated by gestational rates following surgical resection, mainly in patients with submucosal myomas, are an indication that they do have a role in infertility etiology. Concerning the treatment, it became clear that myomectomy is the procedure of choice for all patients desiring to become pregnant.

OBJETIVOS: comparar as características fonoarticulatórias de mulheres no menacme e na pós-menopausa. MÉTODOS: foram verificados parâmetros acústicos em tons de voz, sua estabilidade, modulações de entonações, ritmo e velocidade de fala. Quarenta e cinco mulheres na pós-menopausa há três anos ou mais e sem terapia de reposição hormonal e, quarenta e cinco mulheres no menacme, com ciclos menstruais regulares e sem medicação hormonal responderam um questionário e gravaram produções verbais. As análises acústicas foram realizadas com o programa Motor Speech Profile da Key Elemetrics. Utilizou-se o teste t de Student para resultados com distribuição gaussiana e o teste Mann-Whitney quando houve distribuição assimétrica. RESULTADOS: no grupo pós-menopausa houve maior consistência na lentificação do ritmo e redução de velocidade fonoarticulatória. CONCLUSÕES: o melhor conhecimento nesta área permitirá a elaboração de estratégias para oferecer melhor qualidade de vida para mulheres na pós-menopausa, em especial para aquelas que utilizam a voz profissionalmente.OBJECTIVES: to compare phono-articulatory characteristics between women in reproductive age and postmenopause women. METHODS: acoustic variations in vocal intonation, speed of the speech and the pause pattern were measured. Fourty five reproductive age women with regular menstrual cycles and taking no hormonal contraceptives and 45 postmenopause women receiving no hormonal replacement therapy for at least three years were interviewed and their verbal productions were recorded. Acoustic analyses were performed using the Kay Elemetrics Motor Speech Profile. Student's t test was employed to compare data between the two groups when they presented normal distribution, and Mann-Whitney test when they were asymmetrical. RESULTS: the reduction on the rhythm and phonoarticulatory speed was more constant in the postmenopause group. CONCLUSIONS: a better understanding in this field will make possible to elaborate strategies to offer a better life quality for postmenopausal women, in special for those who use their voice professionally.

Introduction: The use of a postcoital hormonal contraception regimen has been described and is known as emergency contraception (EC) or “the morning-after pill”. The aim of this study was to evaluate the use and level of knowledge about emergency oral contraception (EC) among women attending the gynecology outpatient clinic of the Hospital Fundação Santa Casa de Misericórdia of the State ofPará, the second largest state in the Brazilian Amazon. Methods: a cross-sectional study was conducted with 316 sexually active women, aged 18 to 50 years, who attended the gynecology outpatient clinic. Participants were included based on spontaneous demand between June and July 2012. Patients answered a questionnaire with 29 questions, including: age in years, educational attainment, knowledge about EC, and previous use of the method. The primary outcome was knowledge on EC use. Results: Participants' mean age was 31.84 years (SD ± 8.00). As for their educational level, 46.84% of them had completed high school, and only 8.55% had higher education. Most of the women obtained information about EC through friends (48.61%, n = 152), and only 7.30% from their doctors. Although most participants (83.54%) reported to be aware of the method, only 0.63% reported that EC could be used up to 5 days after unprotected intercourse; 57.59% did not know how to use it; and 76.58% (n = 242) had never used the method. Conclusion: The women in our study seem to have a high level of knowledge and prevalence of use of emergency contraception, although few of them knew about the time limit for its use. They should receive more information about emergency contraception.

Existem diferenças entre a morfologia das células somáticas que constituem o folículo ovariano. As células murais da granulosa e do cumulus oophorus foram coletadas de pacientes que realizaram tratamento de Reprodução Assistida no Núcleo de Reprodução Humana Gerar e foram cultivadas, por 5 dias, no Laboratório de Ginecologia e Obstetrícia Molecular do Hospital de Clínicas de Porto Alegre. Como ilustrado na Figura, as células murais apresentam morfologia alongada, são uninucleadas com bordos delimitados e proliferam em clusters (grumos de células).

Introdução: A doença cardiovascular (DCV) é a principal causa de morte em mulheres nos países desenvolvidos. Identificar os fatores de risco para doença cardiovascular modificáveis é imprescindível para atuação dos médicos e demais trabalhadores da saúde. Objetivo: Avaliar os fatores de risco para doença cardiovascular em funcionárias do Hospital de Clínicas de Porto Alegre. Método: Trata-se de estudo de prevalência no qual foram avaliadas 172 mulheres. Foi aplicado questionário sobre atividade física e fatores de risco para DCV e aferido peso, altura, pressão arterial (PA) e circunferência da cintura (CC). Resultados: A média do IMC foi 27,3 kg/m2. Setenta mulheres (40,7%) apresentavam IMC 30. A média da CC (89 cm) estava acima do limite superior da normalidade para mulheres. O número de fumantes foi 24(14%). Em relação à atividade física, 79(60%) funcionárias relataram atividade moderada à vigorosa por pelo menos 4 horas semanais. A atividade física mostrou relação inversa com a medida da cintura – aproximadamente a cada 78 minutos de atividade física semanal há redução de 1 cm na circunferência da cintura (P

We report the case of a nulliparous 33-year-old woman who had a uneventful insertion of levonorgestrel-releasing intrauterine device (LNG-IUD; Mirena, Schering, Brazil) inserted on august/2005. She was amenorrheic from august 2005 to may 2006, when she referred the beginning of an intermittent spotting. On september/2006, she had a normal transvaginal ultrasound, showing a well-placed device. One year later, she informed the occurrence of regular menstrual cycles, but with menorrhagia. At that time, during the physical examination, the retrieval thread was not visible at the cervix. A pré-eclâmpsia é uma doença da gestação que pode determinar restrição no crescimento fetal, prematuridade e, em casos mais graves, morte da mãe e do feto. Caracteriza-se por hipertensão arterial materna, proteinúria significativa (? 0,3g/24h), edema, vaso-constrição do leito vascular materno e conseqüente aumento da resistência vascular. Muitos estudos discutem fatores de risco, patogê-nese e critérios para o diagnóstico da pré-eclampsia, porém as variações na forma de apresentação e de evolução clínica dessa doença dificultam o entendimento dos resultados obtidos, freqüentemente conflitantes. A padronização diagnóstica e as pesquisas de base genética e molecular podem trazer, em um futuro próximo, maior compreensão dessa patologia. Neste artigo apresentamos uma revisão da literatura, com destaque para a relação entre pré-eclâmpsia e resistência à insulina.

A incidência e a mortalidade de câncer cervical têm diminuído, em parte pelo diagnóstico precoce e tratamento de lesões precursoras do câncer cervical. Neste trabalho são apresentadas as bases para a compreensão da oncogênese cervical. Diversos estudos demonstraram que o maior risco para desenvolver câncer de colo uterino é a não realização de exames citopatológicos, rotineiramente. O ciclo celular é controlado por genes supressores e estimuladores da proliferação celular. Quando ocorrem mutações, proto-oncogenes tornam-se oncogenes, que são carcinogênicos e causam multiplicação celular excessiva. Os genes supressores, em contraste, contribuem para o desenvolvimento de câncer quando são inativados por mutações. A perda da ação de genes supressores funcionais pode levar a célula ao crescimento inadequado. O ciclo celular também pode ser alterado pela ação de vírus, entre eles o HPV (human papiloma virus), de especial interesse na oncogênese cervical. Os tipos HPV 16 e 18 são os de maior interesse, freqüentemente associados a câncer cervical e anal. O mecanismo pelo qual os tipos de HPV transformam as células ainda não é completamente compreendido. O conhecimento das bases moleculares que estão envolvidas na oncogênese cervical tem sido possível devido a utilização de técnicas avançadas de biologia molecular. Algumas destas técnicas permitem identificar grupos de HPV de alto ou baixo risco (captura híbrida) ou identificação de tipos virais específicos (PCR). São técnicas de fácil utilização em laboratórios equipados, embora ainda com custo elevado.

CONTEXT AND OBJECTIVE: Preeclampsia is a multi-systemic disease and one of the most frequent severe health problems during pregnancy. Binding of insulin triggers phosphorylation and activates cytoplasmic substrates such as phosphatidylinositol 3 kinase (PI3K). Phosphorylation of membrane phosphoinositide 2 (PIP2) to phosphoinositide 3 (PIP3) by PI3K starts Akt/PKB activation. Defects in phosphorylation of the insulin receptor and its substrates have an important role in insulin resistance. Studies have shown that insulin resistance is associated with preeclampsia and its pathophysiology. The aim here was to investigate insulin stimulation of the Akt/PKB pathway in the placenta, in normal and preeclampsia parturients. DESIGN AND SETTING: Cross-sectional study in a tertiary public university hospital. METHODS: Placentas were collected from 12 normal and 12 preeclampsia patients. These were stimulated and analyzed using Western blot to quantify the Akt/PKB phosphorylation. RESULTS: The insulin stimulation was confirmed through comparing the stimulated group (1.14 ± 0.10) with the non-stimulated group (0.91 ± 0.08; P < 0.001). The phosphorylation of Akt/PKB did not differ between the placenta of the normal patients (1.26 ± 0.16) and those of the preeclampsia patients (1.01 ± 0.11; P = 0.237). CONCLUSIONS: In vitro insulin stimulation of the human placenta has been well established. There was no difference in Akt/PKB phosphorylation, after stimulation with insulin, between placentas of normal and preeclampsia patients. Nevertheless, it cannot be ruled out that the Akt/PKB signaling pathway may have a role in the pathophysiology of preeclampsia, since the substrates of Akt/PKB still need to be investigated.

CONTEXT AND OBJECTIVE: Preeclampsia is a gestational disease that occurs mainly among nulliparous women after the 20th week of gestation, and frequently close to delivery. The effects of preeclampsia on women's blood pressure over the long term are still controversial. Patients with recurrent preeclampsia or preeclampsia in the early stages of pregnancy appear to present higher risk of hypertension. The aim of this study was to determine the risk factors for cardiovascular disease among women with preeclampsia 10 years earlier. DESIGN AND SETTING: Cross-sectional study at Hospital de Clínicas de Porto Alegre (HCPA). METHODS: Forty women with preeclampsia and 14 normotensive pregnant women followed up 10 or more years earlier at HCPA underwent clinical and laboratory examinations. Spearman's correlation coefficient was used to correlate body mass index (BMI) and systolic and diastolic pressures. The risk of developing hypertension was measured using the chi-square test. P < 0.05 was considered significant. RESULTS: The patients with preeclampsia 10 or more years earlier had significantly higher diastolic blood pressure (P = 0.047), BMI (P = 0.019) and abdominal circumference (P = 0.026). They presented positive correlations between BMI and diastolic blood pressure (0.341; P = 0.031) and between BMI and systolic blood pressure (0.407; P = 0.009). CONCLUSION: The patients with preeclampsia 10 or more years earlier had significantly higher diastolic blood pressure, BMI and abdominal circumference than did the control group. This emphasizes the importance of long-term follow-up assessment for cardiovascular risk factors among patients with preeclampsia.
