1. Late-onset myasthenia gravis: Is it a different clinical entity?

Abstract: Objective: The increase in the average life expectancy of humans has resulted in an increased occurrence of late-onset myasthenia gravis (LOMG). It is noticeable that the clinical, immunologic, and prognostic features of patients with LOMG are different than those of the patients with early-onset myasthenia gravis (EOMG). The present study aimed to establish the dissimilar features of patients with EOMG and LOMG. Subjects and Methods: The study retrospectively compared 93 patients with EOMG and LOMG in terms of clinical and immunologic features and response to medical treatment and thymectomy. Results: Of the 93 patients with MG, 56 had EOMG and 37 had LOMG. The EOMG group was dominated by female patients and generalized MG, whereas male patients and ocular MG were predominant in the LOMG group. Both groups tested positive for acetyl choline receptors antibodies. The number of patients undergoing thymectomy was higher in the EOMG group than in LOMG group. Responses to thymectomy and immunosuppressive treatment in LOMG group were lower compared with the EOMG group. Conclusion: The present study concludes that the clinical prognosis of patients with LOMG is better, and thymectomy may not be quite necessary for patients in this group who do not have thymomas (nonthymomatous patients).

2. Role of thymus on prognosis of myasthenia gravis in Turkish population

Abstract: OBJECTIVE: Myasthenia gravis (MG) is an autoimmune disease that may cause a disorder in transmission at the neuromuscular junction. Antibodies directed against acetylcholine receptors are responsible. The thymus is the place that that production of these antibodies mainly occurs. The thymus gland abnormalities and abnormal production of these antibodies are associated with MG. Consequently, thymectomy is a common treatment for MG. The nature of the disease makes it difficult to plan prospective, controlled trials; therefore, there is no current consensus among clinicians on a single algorithm of treatment, and the approach is frequently based on the observations and experiences of experts. The contributions to the literature largely consist of retrospective studies examining an approach to treatment and the effects of thymectomy on prognosis. In this retrospective study, evaluation of Turkish patients with myasthenia gravis was carried out for the importance of thymectomy and effects on prognosis. METHODS: In this study, 93 patients with myasthenia gravis whose followed up at Neuromuscular outpatient clinic between 1998–2018 were evaluated retrospectively. Type of disease, antibody status, treatment, thymectomy, thymus pathology and prognosis were assessed. RESULTS: Thymectomy had been a positive effect on the prognosis of the disease independent of the duration of disease and thymic pathology. The best results had been obtained with early thymectomy with short disease duration, younger age and patients with thymic hyperplasia. Success of therapy was limited with thymoma. With advanced age need for thymectomy was decreased. CONCLUSION: In the present study, evaluation of 93 patients with myasthenia gravis was done retrospectively and it was concluded that thymectomy had a positive effect on prognosis, especially in young patients when performed as early as possible. The most successful results were obtained in cases with thymic hyperplasia.

3. Neurobrucellosis: clinical, diagnostic, therapeutic features and outcome. Unusual clinical presentations in an endemic region

Abstract: Brucellosis is a zoonotic infection and has endemic characteristics. Neurobrucellosis is an uncommon complication of this infection. The aim of this study was to present unusual clinical manifestations and to discuss the management and outcome of a series of 18 neurobrucellosis cases. Initial clinical manifestations consist of pseudotumor cerebri in one case, white matter lesions and demyelinating syndrome in three cases, intracranial granuloma in one case, transverse myelitis in two cases, sagittal sinus thrombosis in one case, spinal arachnoiditis in one case, intracranial vasculitis in one case, in addition to meningitis in all cases. Eleven patients were male and seven were female. The most prevalent symptoms were headache (83%) and fever (44%). All patients were treated with rifampicin, doxycycline plus trimethoprim-sulfamethoxazole or ceftriaxone. Duration of treatment (varied 3-12 months) was determined on basis of the CSF response. In four patients presented with left mild sequelae including aphasia, hearing loss, hemiparesis. In conclusion, although mortality is rare in neurobrucellosis, its sequelae are significant. In neurobrucellosis various clinical and neuroradiologic signs and symptoms can be confused with other neurologic diseases. In inhabitants or visitors of endemic areas, neurobrucellosis should be kept in mind in cases that have unusual neurological manifestations. Abstract: Introduction To investigate retinal nerve fiber layer (RNFL), macular, foveal and parafoveal thickness in patient with early stage Parkinson's (PD) and Alzheimer's disease (AD) by optical coherence tomography (OCT) and to compare results with healthy control group and between both disease. Methods Participants with AD dementia (n: 15) and PD (n: 15), besides 15 age-sex matched controls were enrolled in the study and received OCT assessments. Clinical disability grade in PD was determined by the Unified Parkinson's Disease Rating Scale and Hoehn Yahr (H-Y) Scale was used to determine the stage of PD. Standardized Mini Mental Test (SMMT) and Montreal Cognitive Rating Scale (MOCA) were used for neurocognitive evaluation of patients with AD. The relationship between OCT and test results was analyzed. Results OCT measurements did show significant decrease in temporal, nasal, inferiorR (R means examination of retina in two sections as superior and inferior instead of four quadrants) RNFL thickness and foveal, parafoveal, macular thickness of AD group compared to control group. Temporal, inferior and inferiorR RNFL thickness were thinner in patients with PD than those of control group but these differences were not significant. However the superiorR and superior RNFL thickness decreased significantly in the PD group as the disease duration increased. There was no relationship between SMMT, MOCA, UPDRS, H-Y scores and OCT results. Conclusion As several studies have reported different results so far, we thought that the use of OCT in early diagnosis and follow-up of the course of both diseases was not appropriate until many studies indicated the same result.

Abstract: Neurosyphilis (NS) has different clinical manifestations and can appear during any stage of syphilis. We aimed to identify the factors affecting poor outcome in NS patients. Patients with positive cerebrospinal fluid Venereal Disease Research Laboratory test, and positive serological serum treponemal or nontreponemal tests were classified as definite NS. The data of 141 patients with definite NS were submitted from 22 referral centers. Asymptomatic NS, syphilitic meningitis, meningovascular syphilis, tabes dorsalis, general paresis, and taboparesis were detected in 22 (15.6%), 67 (47.5%), 13 (9.2%), 10 (7%), 13 (9.2%), and 16 patients (11.3%), respectively. The number of HIV-positive patients was 43 (30.4%). The most common symptoms were headache (n = 55, 39%), fatigue (n = 52, 36.8%), and altered consciousness (50, 35.4%). Tabetic symptoms were detected in 28 (19.8%), paretic symptoms in 32 (22.6%), and vascular symptoms in 39 patients (27.6%). Eye involvement was detected in 19 of 80 patients (23.7%) who underwent eye examination and ear involvement was detected in eight of 25 patients (32%) who underwent ear examination. Crystallized penicillin was used in 109 (77.3%), procaine penicillin in seven (4.9%), ceftriaxone in 31 (21.9%), and doxycycline in five patients (3.5%). According to multivariate regression analysis, while headache was a protective factor in NS patients, double vision was significantly associated to poor outcome. We concluded that double vision indicated unfavorable outcome among NS patients. A high clinical suspicion is needed for the diagnosis NS. As determined in our study, the presence of headache in syphilitic patients can help in early diagnosis of central nervous system disease.

Abstract: Background: Patients with minor acute ischemic stroke or transient ischemic attack are at high risk for subsequent stroke, and more potent antiplatelet therapy in the acute setting is needed. However, the potential benefit of more intense antiplatelet therapy must be assessed in relation to the risk for major bleeding. The SOCRATES trial (Acute Stroke or Transient Ischemic Attack Treated With Aspirin or Ticagrelor and Patient Outcomes) was the first trial with ticagrelor in patients with acute ischemic stroke or transient ischemic attack in which the efficacy and safety of ticagrelor were compared with those of aspirin. The main safety objective was assessment of PLATO (Platelet Inhibition and Patient Outcomes)–defined major bleeds on treatment, with special focus on intracranial hemorrhage (ICrH). Methods: An independent adjudication committee blinded to study treatment classified bleeds according to the PLATO, TIMI (Thrombolysis in Myocardial Infarction), and GUSTO (Global Use of Strategies to Open Occluded Coronary Arteries) definitions. The definitions of ICrH and major bleeding excluded cerebral microbleeds and asymptomatic hemorrhagic transformations of cerebral infarctions so that the definitions better discriminated important events in the acute stroke population. Results: A total of 13 130 of 13 199 randomized patients received at least 1 dose of study drug and were included in the safety analysis set. PLATO major bleeds occurred in 31 patients (0.5%) on ticagrelor and 38 patients (0.6%) on aspirin (hazard ratio, 0.83; 95% confidence interval, 0.52–1.34). The most common locations of major bleeds were intracranial and gastrointestinal. ICrH was reported in 12 patients (0.2%) on ticagrelor and 18 patients (0.3%) on aspirin. Thirteen of all 30 ICrHs (4 on ticagrelor and 9 on aspirin) were hemorrhagic strokes, and 4 (2 in each group) were symptomatic hemorrhagic transformations of brain infarctions. The ICrHs were spontaneous in 6 and 13, traumatic in 3 and 3, and procedural in 3 and 2 patients on ticagrelor and aspirin, respectively. In total, 9 fatal bleeds occurred on ticagrelor and 4 on aspirin. The composite of ICrH or fatal bleeding included 15 patients on ticagrelor and 18 on aspirin. Independently of bleeding classification, PLATO, TIMI, or GUSTO, the relative difference between treatments for major/severe bleeds was similar. Nonmajor bleeds were more common on ticagrelor. Conclusions: Antiplatelet therapy with ticagrelor in patients with acute ischemic stroke or transient ischemic attack showed a bleeding profile similar to that of aspirin for major bleeds. Abstract: WOS: 000379252100027, PubMed ID: 27085724, There have been many studies pertaining to the management of herpetic meningoencephalitis (HME), but the majority of them have focussed on virologically unconfirmed cases or included only small sample sizes. We have conducted a multicentre study aimed at providing management strategies for HME. Overall, 501 adult patients with PCR-proven HME were included retrospectively from 35 referral centres in 10 countries; 496 patients were found to be eligible for the analysis. Cerebrospinal fluid (CSF) analysis using a PCR assay yielded herpes simplex virus (HSV)-1 DNA in 351 patients (70.8%), HSV-2 DNA in 83 patients (16.7%) and undefined HSV DNA type in 62 patients (12.5%). A total of 379 patients (76.4%) had at least one of the specified characteristics of encephalitis, and we placed these patients into the encephalitis presentation group. The remaining 117 patients (23.6%) had none of these findings, and these patients were placed in the nonencephalitis presentation group. Abnormalities suggestive of encephalitis were detected in magnetic resonance imaging (MRI) in 83.9% of the patients and in electroencephalography (EEG) in 91.0% of patients in the encephalitis presentation group. In the nonencephalitis presentation group, MRI and EEG data were suggestive of encephalitis in 33.3 and 61.9% of patients, respectively. However, the concomitant use of MRI and EEG indicated encephalitis in 96.3 and 87.5% of the cases with and without encephalitic clinical presentation, respectively. Considering the subtle nature of HME, CSF HSV PCR, EEG and MRI data should be collected for all patients with a central nervous system infection. Abstract: WOS: 000384527500014, PubMed ID: 27625226, Aims: There is no report on the factors affecting the resolution of symptoms related to meningitis during treatment of tuberculous meningitis (TBM). Thus, we examined the factors associated with early therapeutic responses. Materials and Methods: This multicenter study included 507 patients with microbiologically confirmed TBM. However, 94 patients eligible for the analysis were included in this study from 24 centers. Six out of 94 patients died and the statistical analysis was performed with 88 survivors. Early and late responder groups were compared in the statistical analysis. P < 0.05 were considered to show a significant difference. Results: In the multivariate analysis, the presence of vasculitis (P = 0.029, OR = 10.491 [95% CI, 1.27u86.83]) was found to be significantly associated with a delayed fever response whereas hydrocephalus was associated with altered mental status for 9 days duration (P = 0.005, OR = 5.740 [95% CI, 1.68u19.57]). According to linear regression analysis, fever was significantly persisting (7 days) in the presence of vasculitis (17.5 vs. 7, P< 0.001) and hydrocephalus (11 vs. 7, P = 0.029). Hydrocephalus was significantly associated with persisting headache (21 vs. 12, P = 0.025), delayed recovery of consciousness (19.5 vs. 7, P = 0.001), and a delay in complete recovery (21 vs. 14, P = 0.007) in the linear regression analysis. Following institution of treatment, the complaints seemed to disappear in up to 2 weeks among TBM survivors. Conclusions: In the absence of hydrocephalus or vasculitis, one week of anti-tuberculosis treatment seems to be adequate for the resolution of TBM symptoms. Hydrocephalus and vasculitis delay the resolution of TBM symptoms in response to antimycobacterial treatment.

Abstract: Brucellosis is a zoonotic infection and has endemic characteristics. Neurobrucellosis is an uncommon complication of this infection. The aim of this study was to present unusual clinical manifestations and to discuss the management and outcome of a series of 18 neurobrucellosis cases. Initial clinical manifestations consist of pseudotumor cerebri in one case, white matter lesions and demyelinating syndrome in three cases, intracranial granuloma in one case, transverse myelitis in two cases, sagittal sinus thrombosis in one case, spinal arachnoiditis in one case, intracranial vasculitis in one case, in addition to meningitis in all cases. Eleven patients were male and seven were female. The most prevalent symptoms were headache (83%) and fever (44%). All patients were treated with rifampicin, doxycycline plus trimethoprim-sulfamethoxazole or ceftriaxone. Duration of treatment (varied 3-12 months) was determined on basis of the CSF response. In four patients presented with left mild sequelae including aphasia, hearing loss, hemiparesis. In conclusion, although mortality is rare in neurobrucellosis, its sequelae are significant. In neurobrucellosis various clinical and neuroradiologic signs and symptoms can be confused with other neurologic diseases. In inhabitants or visitors of endemic areas, neurobrucellosis should be kept in mind in cases that have unusual neurological manifestations. Abstract: The etiologies of well-known cavernous sinus syndrome are many. It is necessary to identify clinical and radiologic findings of various cavernous sinus syndromes to avoid major mistakes in treatment. As an example, pitfalls in the diagnosis of Tolosa-Hunt syndrome (THS) are not rare. In this study, we report a case series of THS and review and discuss the mistakes in the diagnosis of cavernous sinus syndromes. This study was conducted during a 10-year-period between 1997 and 2007. There were 10 patients (6 males, 4 females) in this series. Patients were included in this series only if they fulfilled the International Headache Society criteria for Tolosa-Hunt syndrome. Follow-up duration of the patients ranged from 6 months to 9 years (mean=3.8 y). Age of the patients (n=10) ranged from 23 to 70 years (mean=44 y). According to magnetic resonance scans, the lesion was located in the cavernous sinus only in 6 cases; in the orbit only in 1 case; and in cavernous sinus as well as superior orbital fissure and ipsilateral orbit in 1 case; and in both the cavernous sinus and ipsilateral superior orbital fissure in 1 patient. In 1 patient, there was no magnetic resonance imaging abnormality. Nine out of 10 patients completely recovered after steroid therapy. In conclusion, good knowledge of clinical presentation of orbital pseudotumor and THS is very helpful in clinical decision-making in a patient with cavernous sinus syndrome.

Abstract: WOS: 000358623200015, PubMed ID: 25779579, Data in the literature regarding the factors that predict unfavorable outcomes in adult herpetic meningoencephalitis (HME) cases are scarce. We conducted a multicenter study in order to provide insights into the predictors of HME outcomes, with special emphasis on the use and timing of antiviral treatment. Samples from 501 patients with molecular confirmation from cerebrospinal fluid were included from 35 referral centers in 10 countries. Four hundred thirty-eight patients were found to be eligible for the analysis. Overall, 232 (52.9%) patients experienced unfavorable outcomes, 44 died, and 188 survived, with sequelae. Age (odds ratio [OR], 1.04; 95% confidence interval [CI], 1.02 to 1.05), Glasgow Coma Scale score (OR, 0.84; 95% CI, 0.77 to 0.93), and symptomatic periods of 2 to 7 days (OR, 1.80; 95% CI, 1.16 to 2.79) and >7 days (OR, 3.75; 95% CI, 1.72 to 8.15) until the commencement of treatment predicted unfavorable outcomes. The outcome in HME patients is related to a combination of therapeutic and host factors. This study suggests that rapid diagnosis and early administration of antiviral treatment in HME patients are keys to a favorable outcome.

Abstract: WOS: 000353295400011, PubMed ID: 25634680, Predicting unfavorable outcome is of paramount importance in clinical decision making. Accordingly, we designed this multinational study, which provided the largest case series of tuberculous meningitis (TBM). 43 centers from 14 countries (Albania, Croatia, Denmark, Egypt, France, Hungary, Iraq, Italy, Macedonia, Romania, Serbia, Slovenia, Syria, Turkey) submitted data of microbiologically confirmed TBM patients hospitalized between 2000 and 2012. Unfavorable outcome was defined as survival with significant sequela or death. In developing our index, binary logistic regression models were constructed via 200 replicates of database by bootstrap resampling methodology. The final model was built according to the selection frequencies of variables. The severity scale included variables with arbitrary scores proportional to predictive powers of terms in the final model. The final model was internally validated by bootstrap resampling. A total of 507 patients' data were submitted among which 165 had unfavorable outcome. Eighty-six patients died while 119 had different neurological sequelae in 79 (16 %) patients. The full model included 13 variables. Age, nausea, vomiting, altered consciousness, hydrocephalus, vasculitis, immunosuppression, diabetes mellitus and neurological deficit remained in the final model. Scores 1-3 were assigned to the variables in the severity scale, which included scores of 1-6. The distribution of mortality for the scores 1-6 was 3.4, 8.2, 20.6, 31, 30 and 40.1 %, respectively. Altered consciousness, diabetes mellitus, immunosuppression, neurological deficits, hydrocephalus, and vasculitis predicted the unfavorable outcome in the scoring and the cumulative score provided a linear estimation of prognosis.

Abstract: The purpose of this paper was to report a case of central serous chorioretinopathy following pulse corticosteroid therapy in multiple sclerosis. The patient was a 40-year-old female patient admitted to neurology clinic with muscle weakness and difficulty in moving. Having the diagnosis of multiple sclerosis, she was hospitalised for the treatment of the acute attack. Pulse intravenous methylprednisolone therapy was introduced and on the third day of the therapy, patient reported acute visual loss. Magnetic resonance imaging was performed to exclude optic neuritis. Complete eye examination and imaging of the fundus revealed retinal pigment epithelial detachment as the cause of the visual loss. Corticosteroids (endogenous or exogenous) are known risk factors for central serous chorioretinopathy. As retinal pigment epithelial detachment is a component of central serous chorioretinopathy, systemic corticosteroid was thought to be responsible for acute visual loss in this case. Abstract: Objective: We aimed to evaluate the relationship between low back pain (LBP) in patients with Parkinson's disease (PD) and its effects on the activities of daily living of the patients. We also observed the correlation between LBP and spinal mobility tests. Materials and Methods: The study included 55 patients with PD, 39 males and 16 females, in Hoenh Yahr stage I-IV with an average age of 63+-5 (range, 38- 82). We included age- and sex- matched 20 healthy controls who had no orthopedic disease, with a mean age of 65+-7 (range, 35-87). All participants had lomber magnetic resonance imaging to exclude disc or other degenerative pathologies. The modified-modified schober test (MMST), lumbar flexion and extension range of motion (ROM) measurement were applied for assessment of spinal mobility. Lomber flexion and extension ROM was assessed by using a universal goniometry. In addition, Roland Morris disability questionnaire (RMDQ), the Oswestry disability index (ODI), visual analogue scale for pain (VAS), the unified Parkinson's disease rating scale (UPDRS), activities of daily living scale (ADLs), fatigue severity scale and Beck's depression inventory were applied to patients. Abstract: To investigate the F wave parameters (F duration, F minimum latency, F maximum latency, F mean latency, F chronodispersion, and F persistence) of the tibial nerve with unilateral S1 radiculopathy. We evaluated the differences of these parameters between the affected and unaffected sides and also with the control group.The study was performed from September 2007 to January 2008 in the Electrophysiology Laboratory of Marmara University Medical Faculty, Istanbul, Turkey. Bilateral tibial F waves were obtained from 20 normal control subjects (control group) and 20 patients with unilateral S1 radiculopathy (patient group). Minimum, maximum, and mean F latency values were corrected by the subject`s height (F min/H, F max/H, F mean/H). Needle electromyography was performed in the patient group. The patients with a history of diabetes, alcoholism, or other abnormality known to affect peripheral nerves were excluded.In the control group, no significant differences were found in any of the F-wave parameters between the 2 sides. In the patient group, there were significant prolongations of F duration, F min/H, F max/H, F mean/H, and F chronodispersion on the lesion side. Patients` F durations of the affected and unaffected side were significantly longer than the control group. The F chronodispersion also showed significant prolongation on the affected side in the patient group compared with the control group. Among 20 patients, 15 had evidence of denervation or polyphasic potentials on needle electromyography.The F wave study can be clinically useful in the evaluation of S1 radiculopathies, especially in patients with mild and early stage of the disease. Both F duration and F chronodispersion have a higher diagnostic value as compared to F min in the diagnosis of lumbosacral radiculopathy, especially in cases with normal findings on needle electromyography.

21. Electrodiagnosis of ulnar nerve entrapment at the elbow

Abstract: To evaluate the different localizing electrodiagnostic techniques of ulnar nerve entrapment at the elbow (UNE), particularly, comparison of the sensitivities of long segment stimulation across the elbow, versus short segment stimulation.Patients who were referred to the Neurophysiology Laboratory of Haydarpasa Numune Training and Research Hospital, Istanbul, Turkey between 2000-2004 with a preliminary diagnosis of UNE were retrospectively evaluated. We compared the sensitivity of studying long segments (8-12 cm) versus short segments (3 cm) for the diagnosis of UNE in 93 limbs.The study group consisted of 55 females and 31 males. Slowing of the conduction velocity (50 m/sn) across the elbow was recorded in 48.4% of the limbs with long segment studies, and 73% of the limbs with short segment studies. In 82% of cases, an amplitude drop of the compound muscle action potential (CMAP) was also recorded. A CMAP amplitude drop of 10-30% between the wrist and elbow was recorded in 35 limbs (37.6%), while a drop of more than 50% was only recorded in 5 limbs (5.4%).Short segment studies are sensitive for the electrodiagnosis of UNE, and although a CMAP amplitude drop is recorded in most patients, an amplitude drop consistent with a conduction block (50%) is rare.

23. Neurofibromatosis Type 1-related Multiple Plexiform Neurofibromas: A Case Report

Abstract: Neurofibromatosis type 1 (NF1) and type 2 (NF2) are autosomal dominant diseases. Patients with NF1 or NF2 have the potential to develop benign and malignant tumors. We present a 17-year-old patient with undiagnosed NF1-related multiple plexiform neurofibromas (PNs). The patient was admitted to our department for his left limb paresthesia. He had café-au-lait spots spread over his body and his cervical, thoracic, and lumbosacral magnetic resonance imaging (MRI) of the spine showed multiple PNs. Among his family members, only his father had café-au-lait spots. He refused to have genetic analysis and biopsy of the lesions. Even though clinical symptoms of neurofibromas vary according to the localization of lesions, it is known that PNs could be asymptomatic throughout life. Surgical treatment for PNs is essential. Phase 2 clinical trials have been performed for the medical treatment of PNs and further research on this subject is ongoing. Extensive growth of the tumors and invasion of surrounding tissues made resection of PNs difficult in the patient. Therefore, he had annual MRI follow-ups to assess the growth rate of the neurofibromas and transformation to malignant peripheral nerve-sheath tumors. This case was demonstrative to show multiple PNs. The presence of as much multiple PNs as in our patient is rare.

Abstract: Objectives:We aimed to review the clinical, neuroradiological and electroencephalographic (EEG) findings of patients with epilepsy and malformation of cortical development (MCD).Methods:A retrospective analysis of the medical records of epilepsy outpatients was done. Patients with epilepsy, MCD, and ≥18 years of age were included and classified according to the Barkovich classification (BC).Results:The files of 2027 patients were assessed, and 17 out of 1395 epilepsy patients who were followed-up were found to have MCD. The mean age was 26.5 years. Ten patients were male and 7 were female. The mean time of seizure onset was 13 years. In terms of seizure type, 11 patients had focal, 10 had generalized tonic-clonic, 1 had absence, 1 had tonic-atonic, and 1 had myoclonic. MRI showed 7 patients with cortical dysplasia, 4 with heterotopia, 2 with periventricular nodular heterotopia, 2 with schiezencephaly, 2 with lissencephaly, 1 with agyria, 1 with pachygyria, 1 with macrogyria, 1 with polymicrogyria, and 1 with tuberous sclerosis. EEG revealed focal epileptiform discharges in 5 patients, high-frequency activity in 2, generalized epileptiform discharge in 1, diffuse disorganized background activity in 1, and focal disorganized background activity in 1. EEG was normal in 7 patients. Four of them were on monotherapy and 13 on polytherapy.Conclusion:The MCDs classified in group 1 and 2 according to BC are more frequent. Treatment-resistant or good responsive cases can be seen and seizures can begin at any age. We concluded that the prevalence of MCD is 1.21%. The spectrum of epilepsy seen is extensive and the most common type of MCD is focal cortical dysplasia.

Abstract: Parkinson's disease (PD) is a frequent neurodegenerative process in old age. Accumulation and aggregation of the lipid-binding SNARE complex component α-synuclein (SNCA) underlies this vulnerability and defines stages of disease progression. Determinants of SNCA levels and mechanisms of SNCA neurotoxicity have been intensely investigated. In view of the physiological roles of SNCA in blood to modulate vesicle release, we studied blood samples from a new large pedigree with SNCA gene duplication (PARK4 mutation) to identify effects of SNCA gain of function as potential disease biomarkers. Downregulation of complexin 1 (CPLX1) mRNA was correlated with genotype, but the expression of other Parkinson's disease genes was not. In global RNA-seq profiling of blood from presymptomatic PARK4 indviduals, bioinformatics detected significant upregulations for platelet activation, hemostasis, lipoproteins, endocytosis, lysosome, cytokine, Toll-like receptor signaling and extracellular pathways. In PARK4 platelets, stimulus-triggered degranulation was impaired. Strong SPP1, GZMH and PLTP mRNA upregulations were validated in PARK4. When analysing individuals with rapid eye movement sleep behavior disorder, the most specific known prodromal stage of general PD, only blood CPLX1 levels were altered. Validation experiments confirmed an inverse mutual regulation of SNCA and CPLX1 mRNA levels. In the 3′-UTR of the CPLX1 gene we identified a single nucleotide polymorphism that is significantly associated with PD risk. In summary, our data define CPLX1 as a PD risk factor and provide functional insights into the role and regulation of blood SNCA levels. The new blood biomarkers of PARK4 in this Turkish family might become useful for PD prediction.

Prolonged atrial electromechanical delay (EMD) is an independent predictor for the development of AF. Aims. The aim of the present study was to evaluate the atrial conduction parameters in patients with PD and to assess their relation with the severity of PD. Study design. We prospectively enrolled 51 consecutive patients with newly diagnosed PD and 31 age- and sex-matched non-PD subjects. Methods. To assess atrial electromechanical coupling (PA), the time intervals from the onset of p wave on ECG to the late diastolic wave at the septal (PAs) and lateral (PAl) mitral annulus and lateral tricuspid annulus (PAt) were measured on Tissue Doppler Echocardiography (TDE). Abstract: OBJECTIVE: CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) is a systemic vasculopathy that causes various clinical pictures as recurrent ischemic attacks, migrainous headache, pseudo bulbar palsy, epileptic seizures and dementia. Mutations of notch3 gene on chromosome 19 are responsible for the disease. METHODS: The aim of this study is to draw attention to the family history and neuroradiological investigations of a genetically diagnosed CADASIL case. RESULTS: A forty-seven-years-old woman who presented with left hemiparesis was investigated for the cause of stroke as well as her family history. Her mother was reported to have chronic headache. Then inappropriate crying and laughing attacks, behavioral disturbances, urinary incontinency had contributed and she had died at the age of 51 following sudden loss of consciousness. Her father had had four stroke attacks after 45 and before he died at the age of 73 he was unresponsive for the last 10 years. First sibling, 58 years-old female had a history of three stroke attacks and reported forgetfulness. Second sibling, a female, suffered from headache and had died at the age of 37 succeeding two epileptic seizures. Third sibling, 53 years-old female was said to be living bedridden for the last 5 years. Fourth sibling was 51 years-old male and had had a stroke attack at the age of 38. Fifth sibling was 48 years-old female whose speech disorder started 10 years ago. Laughing and crying attacks, incontinency, childish behavior and forgetfulness were also added. Sixth sibling was our patient. Seventh sibling was 45 years-old, male and had no complaints. First, fourth, sixth and seventh siblings were evaluated in systematic, neurological, neuropsychological and neuroradiological perspectives. CONCLUSION: In addition to family history, hyperintensities in temporal polar region and external capsule on flair and T2-weighted MRI supported the diagnosis of CADASIL in our case. 31. Dropped Head and Parkinsonism

Abstract: Scientific BACKGROUND: Dropped head sing is characterized by severe neck flexion. It results from neck extensor weakness or increased tone of the flexor muscles. This sign is usually reported in neuromuscular diseases such as amyotrophic lateral sclerosis, myasthenia gravis, polymyositis and myopathy. Among extrapyramidal disorders, dropped head sign is not very common. CASE: Only one case with early dropped head and parkinsonism have been reported in our movement disorders department between 1998 and 2006 among our 330 parkinsonism patients. CONCLUSIONS: Existence of dropped head with extrapyramidal disorders makes one think of MSA. This sign has been reported in Parkinson’s disease recently. Its physiopathology, clinical features, and incidence have not been clarified yet. 32. Transcranial Magnetic Stimulation in Idiopathic Parkinson’s Disease

Abstract: OBJECTIVE: Activating the motor cortex via Transcranial Magnetic Stimulation (TMS) gives information about the central motor neuronal pathways. It has been used to assess the functional integrity of motor neurons and descending pathways in different neurological disorders including Idiopathic Parkinson’s Disease. There are different conclusions from different studies regarding TMS in IPD patients, but the most common finding is an increase in MEP amplitude. The aim of this study was to evaluate the functional integrity of corticomotor neuronal pathways in IPD patients, using TMS. METHODS: We studied 30 IPD patients (21 male, 9 female) and age and sex matched 30 healty controls with TMS. The MEP latency and amplitude was recorded and compared between the patients and controls. The effect of, age of onset, disease duration and disability degree was evaluated. RESULTS: We found no difference between the two groups for MEP latency but there was an increase in MEP ampltiude in patient group. This finding was correlated with disease duration and disability scales, and there was no change with dopaminergic treatment. CONCLUSION: PD is not only a disease of extrapyramidal system and the central motor neuronal pathways are affected early in the disease course
