Abstract

BackgroundWeb-based programs are a potential medium for supporting weight loss because of their accessibility and wide reach. Research is warranted to determine the shorter- and longer-term effects of these programs in relation to weight loss and other health outcomes. ObjectiveThe aim was to evaluate the effects of a Web-based component of a weight loss service (Imperative Health) in an overweight/obese population at risk of cardiovascular disease (CVD) using a randomized controlled design and a true control group. MethodsA total of 65 overweight/obese adults at high risk of CVD were randomly allocated to 1 of 2 groups. Group 1 (n=32) was provided with the Web-based program, which supported positive dietary and physical activity changes and assisted in managing weight. Group 2 continued with their usual self-care (n=33). Assessments were conducted face-to-face. The primary outcome was between-group change in weight at 3 months. Secondary outcomes included between-group change in anthropometric measurements, blood pressure, lipid measurements, physical activity, and energy intake at 3, 6, and 12 months. Interviews were conducted to explore participants’ views of the Web-based program. ResultsRetention rates for the intervention and control groups at 3 months were 78% (25/32) vs 97% (32/33), at 6 months were 66% (21/32) vs 94% (31/33), and at 12 months were 53% (17/32) vs 88% (29/33). Abstract

The Cavex® Double Effect (DE) hydrocyclone is a double-efficiency classification unit, working as a single stage without the need for additional intermediate equipment such as pumping. The aim of this work is to evaluate the physical response of design and operating variables of the Cavex® DE hydrocyclone on the classification performance of mine tailings. We conducted tests at a pilot level and the subsequent industrial scale, to know the real application potential of the DE hydrocyclones and scopes for future implementation. The result from the study reveals that double classification helps to significantly lower the fine bypass in the underflow to produce the desired quality of sand for tailing dam construction. It is also concluded that the DE hydrocyclones increase the recovery of sand (sand/slime ratio) and also improve the global water recovery in the process when compared with conventional single-stage hydrocyclones. Abstract

Background Epicardial ventricular tachycardia (VT) ablation is an established approach in patients with epicardial arrhythmogenic foci and is most commonly performed via percutaneous access. An alternative approach is via video-assisted thoracoscopic surgery (VATS), although reports of this technique are limited to the use of catheter-based technologies for radiofrequency ablation delivery. Case summary A 55-year-old man with non-ischaemic cardiomyopathy presented with recurrent VT despite medical therapy. Twelve-lead ECG and cardiac MRI were suggestive of an epicardial left ventricular lateral wall breakout. Epicardial ablation was successfully performed via VATS using a linear surgical bipolar ablation device with electroanatomical mapping (EAM) integration. Following ablation, VT was non-inducible. Other than six short episodes of non-sustained during initial follow-up (up to 6 weeks), the patient remained free of ventricular arrhythmias at 18 months with minimal anti-arrhythmic therapy. Discussion Epicardial VT ablation via VATS is feasible and allows for integration of a surgical ablation device within an EAM system. This may serve as an alternative approach in patients with a failed percutaneous ablation, or those with adverse features for a successful percutaneous procedure. [ABSTRACT FROM AUTHOR]

Abstract

Cushing’s disease is a rare condition that occurs due to an adrenocorticotrophin-producing corticotrophinoma arising from the pituitary gland. The consequent hypercortisolaemia results in multisystem morbidity and mortality. This study aims to report incidence, clinicopathological characteristics, remission outcomes and mortality in a regional pituitary neurosurgical cohort of patients diagnosed with Cushing’s disease in Northern Ireland (NI) from 2000 to 2019. Clinical, biochemical and radiological data from a cohort of patients operated for Cushing’s disease were retrospectively collected and analysed. Fifty-three patients were identified, resulting in an estimated annual incidence of Cushing’s disease of 1.39–1.57 per million population per year. Females accounted for 72% (38/53) of the cohort. The majority (74%, 39/53) of corticotrophinomas were microadenomas and in 44% (17/39) of these no tumour was identified on preoperative magnetic resonance imaging. Histopathological characterisation was similarly difficult, with no tumour being identified in the histopathological specimen in 40% (21/53) of cases. Immediate postoperative remission rates were 53% and 66% when considering serum morning cortisol cut-offs of ≤ 50 nmol/L (1.8 µg/dL) and ≤ 138 nmol/L (5 µg/dL), respectively, in the week following pituitary surgery. Approximately 70% (37/53) of patients achieved longer-term remission with a single pituitary surgery. Three patients had recurrent disease. Patients with Cushing’s disease had a significantly higher mortality rate compared to the NI general population (standardised mortality ratio 8.10, 95% CI 3.3–16.7, P < 0.001). Annual incidence of Cushing’s disease in NI is consistent with other Northern European cohorts. Functioning corticotrophinomas are a clinically, radiologically and histopathologically elusive disease with increased mortality compared to the general population. [ABSTRACT FROM AUTHOR]

Abstract

Context: Head and neck paragangliomas (HNPGLs) are rare, usually benign, slow‐growing tumours arising from neural crest‐derived tissue. Definitive management pathways for HNPGLs have yet to be clearly defined. Objective: To review our experience of the clinical features and management of these tumours and to analyse outcomes of different treatment modalities. Methods: Demographic and clinical data were obtained from The Northern Ireland Electronic Care Record (NIECR) as well from a prospectively maintained HNPGL database between January 2011 through December 2023. Results: There were 87 patients; 50 females: 37 males with a mean age of 52.3 ± 14.2 years old (range 17–91 years old). 58.6% (n = 51) of patients had carotid body tumours, 25.2% (n = 22) glomus vagal tumours, 6.8% (n = 6) tumours in the middle ear, 2.2% (n = 2) in the parapharyngeal space and 1.1% (n = 1) in the sphenoid sinus. 5.7% (n = 5) of patients had multifocal disease. The mean tumour size at presentation was 3.2 ± 1.4 cm (range 0.5–6.9 cm). Pathogenic SDHD mutations were identified in 41.3% (n = 36), SDHB in 12.6% (n = 11), SDHC in 2.2% (n = 2) and SDHA in 1.1% (n = 1) of the patients. Overall treatment modalities included surgery alone in 51.7% (n = 45) of patients, radiotherapy in 14.9% (n = 13), observation in 28.7% (n = 25), and somatostatin analogue therapy with octreotide in 4.5% (n = 4) of patients. Factors associated with a significantly higher risk of recurrence included age over 60 years (p =.04), tumour size exceeding 2 cm (p =.03), positive SDHx variants (p =.01), and vagal and jugular tumours (p =.04). Conclusion: The majority of our patients underwent initial surgical intervention and achieved disease stability. Our results suggest that carefully selected asymptomatic or medically unfit patients can be safely observed provided lifelong surveillance is maintained. We advocate for the establishment of a UK and Ireland national HNPGL registry, to delineate optimal management strategies for these rare tumours and improve long term outcomes. 11. High-pressure computational and experimental studies of energetic materials

Abstract

On account of the high temperatures and pressures experienced by energetic materials during deflagration and detonation, it is important to know not only the physical properties of these materials at ambient temperatures and pressures, but also to understand how their structure and properties are affected by extreme conditions. Combined computational and experimental investigations of the effects of high pressures on the structure and properties of several energetic materials are described herein. A comparison of the performances of different pseudopotentials and density functional theory (DFT) dispersion correction schemes in calculating crystal geometries and vibrational frequencies of crystalline ammonium perchlorate at high pressure is described. The results highlight the fact that care must be taken when choosing pseudopotentials for high-pressure studies. A comprehensive comparison of calculated vibrational modes (including symmetry) with experiment has been performed, with the frequencies of all internal modes predicted to lie within 5% of experimental values. This study established that no significant improvements in the calculation of crystal geometries of ammonium perchlorate are obtained by employing DFT-D corrections. The enthalpy of fusion (ΔHfus) of the highly metastable β-form of RDX (cyclotrimethylenetrinitramine) was determined to be 12.63 ± 0.28 kJ mol-1. DFT-D calculations of the lattice energies of the α- and β-forms of RDX are described. Furthermore, the response of the lattice parameters and unit-cell volumes to pressure for the α-, γ- and ε-forms of RDX calculated using DFT-D are in very good agreement with experimental data. Phonon calculations provide good agreement with vibrational frequencies obtained from Raman spectroscopy, and a predicted inelastic neutron scattering (INS) spectrum of α-RDX shows excellent agreement with experimental INS data recorded as part of this study. The results of the high-pressure phonon calculations have been used to show that the heat capacities of the α-, γ- and ε- forms of RDX are only weakly affected by pressure. DFT-D calculations have been utilised to describe accurately the structure and properties of both β-HMX (Cyclotetramethylenetetranitramine) and α-FOX-7 (1,1-Diamino-2,2-dinitroethylene) as a function of pressure. This work presents data for the experimental hydrostatic compression of both deuterated β-HMX and α-FOX-7 performed using neutron powder diffraction at the ISIS Neutron and Muon facility, in addition to experimental determinations of the INS spectra of both β-HMX and α-FOX-7. The DFT-D hydrostatic compression studies for both materials reproduce the experimental compression trends. Furthermore, the calculated vibrational properties as a function of pressure were in very good agreement with available experimental data. The results of the phonon calculations were then used to predict the effect of pressure on the heat capacities of β-HMX and α-FOX-7. These predictions suggest a very weak pressure dependence of heat capacities (approximately -1 J K-1 mol-1 GPa-1) for these materials. This work demonstrates that the DFT-D model performs extremely well over a range of conditions, and is able to describe accurately intramolecular and intermolecular interactions, and thus the structure and properties of organic molecular nitramine crystals. The computational model was therefore used to predict the high-pressure hydrostatic compression behaviour of a related nitramine, CL-20 (2,4,6,8,10,12-Hexanitro-2,4,6,8,10,12-hexaazaisowurtzitane), the results of which highlighted possible discrepancies in the experimental high-pressure X-ray diffraction data recorded for ε-CL-20. This prompted a high-pressure neutron powder diffraction study, which showed good agreement with the computational results, thereby highlighting radiation damage in the X-ray experiments.

Abstract

Background Increasing fruit and vegetable (FV) consumption is associated with reduced cardiovascular disease risk in observational studies but with little evidence from randomised controlled trials (RCTs). The impact of concurrent pharmacological therapy is unknown. Objective To pool data from six RCTs to examine the effect of increasing FV intake on blood pressure (BP) and lipid profile, also exploring whether effects differed by medication use. Design Across trials, dietary intake was assessed by diet diaries or histories, lipids by routine biochemical methods and BP by automated monitors. Linear regression provided an estimate of the change in lipid profile or BP associated with a one portion increase in self-reported daily FV intake, with interaction terms fitted for medication use. Results The pooled sample included a total of 554 participants (308 males and 246 females). Meta-analysis of regression coefficients revealed no significant change in either systolic or diastolic BP per portion FV increase, although there was significant heterogeneity across trials for systolic BP (I 2 = 73%). Neither adjusting for change in body mass index, nor analysis according to use of anti-hypertensive medication altered the relationship. There was no significant change in lipid profile per portion FV increase, although there was a significant reduction in total cholesterol among those not on lipid-lowering therapy (P  < 0.05 after Bonferroni correction). Conclusion Pooled analysis of six individual FV trials showed no impact of increasing intake on BP or lipids, but there was a total cholesterol-lowering effect in those not on lipid-lowering therapy. Abstract

OBJECTIVE: The optimal approach to the surveillance of non-functioning pituitary microadenomas (micro-NFPAs) is not clearly established. Our aim was to generate evidence on the natural history of micro-NFPAs to support patient care. DESIGN: Multi-centre, retrospective, cohort study involving 23 endocrine departments (UK NFPA consortium). METHODS: Clinical, imaging, and hormonal data of micro-NFPA cases between 1/1/2008 and 21/12/2021 were analysed. RESULTS: Data for 459 patients were retrieved [median age at detection 44 years [interquartile range (IQR) 31-57) - 152 males/307 females]. 419 patients had more than two MRIs [median imaging monitoring 3.5 years (IQR 1.71-6.1)]. One case developed apoplexy. Cumulative probability of micro-NFPA growth was 7.8% (95%CI 4.9%-8.1%) and 14.5% (95%CI 10.2%-18.8%) at 3 and 5 years, respectively, and of reduction 14.1% (95%CI 10.4-17.8%) and 21.3% (95%CI 16.4-26.2%) at 3 and 5 years, respectively. Median tumour enlargement was 2 mm (IQR 1-3) and 49% of micro-NFPAs that grew became macroadenomas (nearly all >5 mm at detection). Eight (1.9%) patients received surgery (only one had visual compromise with surgery required >3 years after micro-NFPA detection). Sex, age, size at baseline were not predictors of enlargement/reduction. At time of detection, 7.2%, 1.7% and 1.5% patients had secondary hypogonadism, hypothyroidism and hypoadrenalism, respectively. Two (0.6%) developed hypopituitarism during follow-up (after progression to macroadenoma). CONCLUSIONS: Probability of micro-NFPA growth is low and development of new hypopituitarism is rare. Delaying first follow-up MRI to three years and avoiding hormonal re-evaluation in absence of tumour growth or clinical manifestations is a safe approach for micro-NFPA surveillance.

Abstract

BackgroundThe practical application of ‘virtual’ (computed) fractional flow reserve (vFFR) based on invasive coronary angiogram (ICA) images is unknown. The objective of this cohort study was to investigate the potential of vFFR to guide the management of unselected patients undergoing ICA. The hypothesis was that it changes management in >10% of cases.MethodsvFFR was computed using the Sheffield VIRTUheart system, at five hospitals in the North of England, on ‘all-comers’ undergoing ICA for non-ST-elevation myocardial infarction acute coronary syndrome (ACS) and chronic coronary syndrome (CCS). The cardiologists’ management plan (optimal medical therapy, percutaneous coronary intervention (PCI), coronary artery bypass surgery or ‘more information required’) and confidence level were recorded after ICA, and again after vFFR disclosure.Results517 patients were screened; 320 were recruited: 208 with ACS and 112 with CCS. The median vFFR was 0.82 (0.70–0.91). vFFR disclosure did not change the mean number of significantly stenosed vessels per patient (1.16 (±0.96) visually and 1.18 (±0.92) with vFFR (p=0.79)). A change in intended management following vFFR disclosure occurred in 22% of all patients; in the ACS cohort, there was a 62% increase in the number planned for medical management, and in the CCS cohort, there was a 31% increase in the number planned for PCI. In all patients, vFFR disclosure increased physician confidence from 8 of 10 (7.33–9) to 9 of 10 (8–10) (p<0.001).ConclusionThe addition of vFFR to ICA changed intended management strategy in 22% of patients, provided a detailed and specific ‘all-in-one’ anatomical and physiological assessment of coronary artery disease, and was accompanied by augmentation of the operator’s confidence in the treatment strategy.

40. National improvements in quality of prenatal healthcare across states and territories in India from 2006 to 2021: a systematic analysis

Abstract

Aim: This study aimed to assess changes in India’s prevalence of low birth weight (LBW) infants relating to the quality of prenatal healthcare, coverage of the Integrated Child Development Services Scheme (ICDS) and maternal factors from 2006 to 2021. Subject and methods: Data from India’s demographic health survey (DHS) were analysed at three times points – 2006, 2016 and 2021. States were classified as ‘low healthcare spending and resource (HSR)’ or ‘high HSR’. Chi-square analysis was conducted to identify differences between quality of prenatal healthcare, coverage of pregnancy benefits, prevalence of LBW infants and changes in maternal factors across three time points and between low HSR and high HSR states. Factor analysis was subsequently used to devise a quality of pregnancy healthcare index, while multi-level analysis was conducted to identify the influence of quality of pregnancy healthcare on LBW and the influence of adolescent pregnancies on quality of prenatal healthcare at both individual and state levels. Results: A significant reduction in LBW infants was found from 2006 to 2021, while significant improvements in quality of prenatal healthcare, coverage of ICDS pregnancy benefits and maternal factors were identified. Significantly poorer quality of pregnancy health was consistently identified across low HSR states, with mothers reporting having received fewer aspects of standard and advanced prenatal care. Prevalence of adolescent (under 20 years) pregnancies remained above 30%, and adolescent pregnancies significantly predicted worse scores in quality of prenatal care. Conclusion: Significant disparity in quality of prenatal healthcare is still evident between low and high HSR states. Adolescent pregnancies also remain as a critical issue that needs to be addressed in India, predicting poor access to prenatal healthcare, yielding a lower score on standard and advanced prenatal care subscales. Further government and community-based efforts are required to bridge the gap in the accessibility and quality of prenatal care between low HSR and high states in India.
