1. Clinical Observation of Surgical Treatment of Recurrent Dislocation of the Forearm Bones

Abstract

AIM OF STUDY. To describe a rare clinical case of surgical treatment of chronic recurrent dislocation of the forearm bones using the method of circular plastic surgery of the elbow joint ligaments. MATERIAL AND METHODS. In the conditions of the Department of Traumatology and Orthopedics on the basis of the Department of Traumatology and Orthopedics of the Federal State Autonomous Educational Institution of Higher Education “Russian Peoples’ Friendship University” in the V. M. Buyanov City Clinical Hospital of the Moscow Health Department, a comprehensive diagnosis and treatment of a 38-year-old patient with recurrent dislocation of the forearm bones was carried out. Clinical and radiological examinations including computed tomography and magnetic resonance imaging of the elbow joint were performed to verify the diagnosis. After a diagnosis of chronic posterior instability of the elbow joint was established, simultaneous plastic surgery of the medial and lateral ulnar collateral ligaments was performed using a single-loop circular autograft from the peroneus longus tendon. RESULTS. At a follow-up period of 12 months, a successful clinical result of surgical treatment with restoration of stability and function of the elbow joint was observed. Pain syndrome on the VAS scale decreased to 1 cm (before surgery 0 cm) with physical activity. The amplitude of active movements in the elbow joint after surgery was: flexion 140° (before surgery 135°), extension 5° (before surgery 5°), pronation-supination 150° (before surgery 160°). After surgery: QuickDASH score 26 (before surgery 44 points), OES score 72 (before surgery 45), MEPS score 85 (before surgery 55). MEPS was score 55, OES was score 45, QuickDASH was 44 points, QuickDASH (work section) was score 75. CONCLUSION. The method of tendon circular plasty of the elbow joint is an effective method of surgical treatment of multidirectional recurrent instability of the elbow joint, helps restore stability and function of the joint and does not lead to limitation of movements.

Abstract

The subject. Sports legal relations, as having a private legal nature, and assuming “horizontal” inter-subject relations by default, do not imply the extension to them the guarantees provided by international acts in the field of human rights protection in order to protect the individual in “vertical” interaction with public subjects. However, this doctrinal approach is still questioned by sports actors, who make attempts to refute it through the involvement of certain norms of the European Convention for the Protection of Human Rights and Fundamental Freedoms (ECHR) (Articles 6 and 8, mostly) as arguments in sports disputes. An additional incentive for this is the location of international and continental sports federations, their jurisdictional bodies and the Court of Arbitration for Sport (CAS) in the national legal orders that recognize the application of the mentioned ECHR.The purpose of the study. The prospects of arguments in the resolution of sports disputes based on the requirements of the literal use of the norms of the ECHR are considered in this article.Methodology. Methods of analysis and comparison based on the CAS practice.The main results of research and the field of their application. The use of only certain meanings of part 1 of Article 6 of the ECHR in sports justice is forced to be taken into account by CAS due to the prospect of appealing decisions to the Swiss Federal Tribunal.Conclusions. The norms of the ECHR generally do not apply in sports justice.

3. Biocomposite materials based on polyethylene and amphiphilic polymer-iron metal complex

Abstract

Objectives. To obtain and study the properties including degradability of polymer composite materials (PCM) based on low-density polyethylene (LDPE) obtained by introducing an environmentally friendly additive comprising an oxo-decomposing additive (ODA) based on an amphiphilic polymer-iron metal complex, which accelerates the process of polymer degradation.Methods. PCMs based on LDPE and ODA were produced by processing in laboratory extruders in the form of strands, granules, and films. Thermodynamic properties were determined by differential scanning calorimetry in the temperature range 20-130 °C. In order to assess the performance characteristics (physical and mechanical properties) of the PCM, tensile strength and elongation at break were determined. The biodegradability of PCM was evaluated by Sturm's method, with the biodegradation index being determined by the amount of CO2 gas released as a result of microorganism activity, as well as composting by placing the PCMs for six months in biohumus. Changes in physical and mechanical properties and water absorption of the films during storage were evaluated. The photochemical degradability of the PCM was determined by exposing it to ultraviolet radiation for 100 h (equivalent to approximately one year of exposure of the films under natural conditions). The appearance of the composite samples following removal from the biohumus was determined using an optical microscope with ×50 magnification in transmitted and reflected light.Results. Following biodegradation by composting, the physical and mechanical properties of PCMs decrease by an average of 40.6%. This is related to the structural changes that occur in composites during storage in biohumus, i.e., the formation of a looser structure due to the development of large clusters of microorganisms that affect the formation of microcracks. It leads to the stage of fragmentation of the polyethylene matrix and indicates the progress of biological degradation of composites. In this case, the water absorption of the composite samples was 63% after 96 h of exposure. The biodegradability index determined by the Sturm method after 28 days of bubbling had changed by 82%, indicating an intensive biodegradation process. Exposure to ultraviolet radiation for 96 h resulted in the complete destruction of the PCMs, which turned into small “flakes.” This method is the most effective for the degradation of LDPE- and ODA-based PCMs.Conclusions. According to the results of the study of ODA-containing PCMs based on an amphiphilic polymer-iron metal complex, the tested filler-modifier can be recommended for the production of PCMs offering an accelerated degradation period.

Abstract

The aim. To analyze the results of surgical treatment of sacral schwannomas with their extension into lower pelvis.Materials and methods. We analyzed the clinical results of surgical treatment of 25 patients with sacral schwannomas, including those with ventral extension and growth into pelvic cavity. All patients were operated at the Clinic of Neurosurgery of the Novosibirsk Research Institute of Traumatology and Orthopedics named after Ya.L. Tsivyan for a 10-year period. The analysis of literature describing prevalence, classification, diagnosis and surgical treatment of sacral tumors and schwannomas in particular was carried out. The features of the course, diagnosis and surgical treatment of sacral schwannomas and accepted surgical methods for their treatment are described.Results. The average follow-up period of the patients in our study was 61.6 ± 2 months. Compared with the initial indicators of patients in the postoperative period (1 year after the surgery), a good clinical result with the regression of symptoms was registered in 17 (68 %) out of 25 patients, and 10 (40 %) of them had a significant regression of neurological disorders. 3 (12 %) operated patients had a deterioration of neurological disorders in the form of aggravation of sensitive disorders; one patient had an aggravation of pelvic organ dysfunction. Radical removal of the tumor was achieved in 24 (96 %) out of 25 cases; subtotal resection was performed in 1 (4 %) patient. The number of relapses and continued growth of tumors in our series was 12 % (3 out of 25 patients). The mortality rate was 0 %. The complications were registered in 3 (12 %) cases and manifested as wound liquorrhea in cases of using posterior approach.Conclusion. The degree of radicality of tumor resection and clinical outcomes is directly related to the selected surgical approach.

5. Aims of disciplinary sanctions for the spectators’ behavior: controversial UEFA practice concerning football clubs

The subject. The objectives of strict club liability for spectators’ behavior are not mentioned in the provisions of the UEFA Disciplinary Regulations. Strict liability implies the responsibility of clubs, regardless of the presence of fault for the actions of third parties – their spectators. Therefore, the question of the purpose of sanctions acquires additional actuality: a sanction cannot only have a punitive effect in the absence of the subject’s fault.The purpose of the study. The variety of sporting sanctions and the wide range of their application creates risks of excessive coercion against football clubs. It is necessary to consider the preventive and deterrent purposes of sports sanctions, without which sports liability is deprived of the sign of certainty for the subjects of sport and turns into the arbitrariness of the soccer authorities.Methodology. In an attempt to find references to sanctions targets under strict liability we analyzed the available practice of the UEFA bodies from 2013 to 2021 (a massive of several hundred decisions of the UEFA Control, Disciplinary and Ethics Commission, the UEFA Appeals Commission). Due to the dispute resolution system existing in European football our research could not be carried out without referring to the decisions of the Court of Arbitration for Sport (CAS) for the period 2002-2020.The main results of research and the field of their application. A serious obstacle to the consistent practice of disputes about the responsibility of clubs for the behavior of spectators is the ambiguity of the terminology used and the doctrinal approaches of law enforcement officers. In decisions we can come across a mention of a preventive effect; preventive and deterrent effect; preventive and educational effect of sanctions. The study found that the current sports justice’s practice of applying strict liability to football clubs has two main problems. Firstly, the UEFA bodies have not established an understanding of who is the subject of the focus of the sanctions. In the disputes examined, two target audiences for sanctions under strict liability are named: clubs and spectators. Secondly, different disputes have emphasized different goals of sports sanctions. The combined approach has not yet been formulated. We have tried to fix these problems.Conclusions. The goals of strict liability and applied sports sanctions in the UEFA perimeter should not differ: preventive and deterrent, and only in the last – punitive. The need for an unambiguous choice of the football entity targeted by sports sanctions will be the first step to take into account the set of aims of the sports sanctions applied to clubs: preventive, deterrent and punitive. Despite decades of UEFA practice in the application of strict liability, there is still uncertainty as to how a sanction will have the expected effect on the spectators. Limiting UEFA to private prevention in determining the sanction and its size in club competitions does not fully fulfill the mission of sports justice. Even if the sanctions imposed on clubs under strict liability have not been verified by a UEFA jurisdictional body to take into account a set of objectives, CAS is entitled to carry out such verification. An additional difficulty arises due to the ambiguity of terminology (and ideology) regarding the objectives of sanctions in sports justice’s practice.

6. Biodegradable packaging materials based on low density polyethylene, starch and monoglycerides

Abstract

Objectives. To investigate the production and biological degradation of biodegradable hybrid compositions (BHCs), dispersed-filled with starch-containing products of various origins and distilled monoglycerides, along with the biodegradation of compositions based on low density polyethylene and thermoplastic starch (TPS) of various origins: corn, pea, and rice.Methods. Thermoplastic starch was obtained based on native starches of several types, which were processed in Brabender and MashkPlast (Russia) laboratory extruders. BHCs in the form of strands, granules, and films were obtained by mixing thermoplastic starches with polyethylene in extruders. Structural BHC parameters were studied by optical and electron scanning microscopy. The biodegradability of the composite films was evaluated by placing them in biohumus for six months; during storage, the change in water absorption of the films was determined. Before and after the biodegradation process, tensile fracture stress and elongation at rupture were determined to evaluate BHC performance (physical and mechanical characteristics of films). Changes in the chemical structure during biodegradation were determined by Fourier infrared spectroscopy.Results. The positive effect (acceleration of the biodegradation process) of using a novel type of starch plasticizer—monoglycerides distilled in TPS–polyethylene compositions—was confirmed. After six months, intensive sporulation of active microorganisms was observed on the surface of the samples. At the same time, water absorption by the samples reached 30%. The observed 60% decrease in strength and deformation properties indicates an intensive process of biodegradation.Conclusions. The biodegradation rate was shown to depend on the concentration and even distribution of the natural biodegradable filler in the synthetic polymer composition.

7. The principle of proportionality and strict liability of football clubs for the behavior of spectators at UEFA competitions

Abstract

The subject. This article is devoted to the content of the principle of proportionality in disputes about the strict liability of football clubs for the behavior of spectators. The proportionality means that the sanction corresponds to the offense and it has two dimensions. Firstly, the more serious the offense is the higher the sanction should be. Secondly, proportionality protects sport from unreasonably low sanctions while the violation is serious.The purpose of the study is the content of the principle of proportionality: the use of related principles of sports jurisprudence, exceptional circumstances (mitigating and aggravating) in the practice of applying clubs` strict liability for spectators` behavior in UEFA competitions over the period 2007-2021. Liability without fault increases the value of investigating the factual circumstances of a dispute. The broad discretion of the bodies raises the question of the validity of the choice of aggravating circumstances or the refusal of mitigating circumstances. Therefore, the jurisdictional authority in each specific dispute must search for exceptional circumstances thereby fulfilling the principle of proportionality. The second important nuance of strict liability in the UEFA regulations is the difference in the interconnection between violations and sanctions. In some articles, the sanction is predetermined. It is possible to reduce such a sanction only in the presence of an exceptional circumstance and to increase it in the presence of an aggravating circumstance. Separately considered, in conjunction with the principle of proportionality, other principles: principles of predictability of sanctions, equal treatment, the precedent value of decisions on similar disputes (stare decisis).Methodology. The methodological basis of the stated research involves the generalization and analysis of the practice of two institutions of sports jurisprudence. Firstly, the jurisdictional bodies of UEFA are publicly available, as well as available to the author, but currently not available for free download on the UEFA website. Secondly, the relevant decisions of the Court of Arbitration for Sport are in the public domain. Turning to the approaches of law enforcement officers regarding the content of the principle of proportionality meant comparing positions that did not differ in inconsistency. As a result of the analysis of the practice were systematized and identified typical exceptional circumstances, unique exceptional circumstances, and specific enforcement of the principle of proportionality.The main results of research and the field of their application. The article examined the normative limits of sanctions in the UEFA Disciplinary Regulations; exceptional circumstances affecting the choice of sanction; search by the law enforcement officer of the content of exceptional circumstances; principles of predictability of sanctions, equal treatment, the precedent value of decisions on similar disputes (stare decisis) in connection with the verification of sanctions for proportionality. Compliance with the principle of proportionality, in this case, should protect the club from an unreasonably harsh and grossly disproportional sanction. Therefore, it is important to analyze the factual circumstances: which of them are mitigating and which are aggravating. In other categories of offenses, the sanction remains at the discretion of the jurisdictional authority. In such violations, the principle of proportionality takes on a special value. The more flexibility in the choice of sanction is, the higher is the risk of abuse by the jurisdictional bodes. UEFA`s enforcement practice is seeking exceptional circumstances that are not consistent enough to be predictable. Some consistency exists only concerning aggravating circumstances. There is an unreasonably strict approach to mitigating circumstances. The practice of CAS does not differ from the practice of UEFA in terms of strict liability compositions. The principle of proportionality in sports jurisprudence can be interconnected with other legal concepts. Such concepts are equal treatment, predictability, and so-called stare decisis.Conclusions. For the slightly undisputed observance of the principle of proportionality, several requirements must be fulfilled. First, analyze the factual circumstances to find exceptional circumstances among them. Secondly, always choose the minimum sanction in the absence of aggravating circumstances, since strict liability is a forced legal institution. Thirdly, indicate in the decisions what circumstances are mitigating, what aggravating circumstances have been established, and how they both affect the choice of a sanction. Fourth, use the previous decisions of the UEFA`s jurisdictional bodies and CAS of the strict liability offenses when the actual circumstances are close.

8. Development of technology for producing biodegradable hybrid composites based on polyethylene, starch, and monoglycerides

Abstract

Objectives. This work aimed to develop technology to produce biodegradable hybrid composite (BHC) films based on low-density polyethylene (LDPE) 115030-070 and thermoplastic starches (TPS) of various origins (corn, pea, and rice), with distilled monoglycerides as the plasticizer. The properties of the produced BHC films were studied and the optimal native starch : glycerol : monoglycerides ratio is proposed.Methods. TPS and BHC films based on this material were produced from different types of native starches in laboratory extruders (Brabender and MashPlast, Russia), and the extruded melts were subjected to ultrasonic vibrations. The structure and appearance of the BHC films were studied using scanning electron microscopy and rheology. Their biodegradability was assessed by immersing them in biocompost for three months. To evaluate the mechanical performance of the BHC films produced with and without ultrasound, the changes in tensile stress and elongation at break were determined during the biodegradation process.Results. The BHC films had a homogeneous structure, except small agglomerates (non-melted starch grains), which did not reduce their quality. The films with monoglycerides had high tensile strength, which was comparable with low-density polyethylene. After removing samples of the BHC films from the biocompost, their tensile strength decreased by 20%, which shows their biodegradability.Conclusions. The produced biodegradable composite films and the technology used to produce them will be applicable for the packaging industry to reduce environmental impact.

Abstract

The article describes the treatment of a 20-year-old patient with a spinal hourglass schwannoma (neurinoma) in the thoracic spine, growing from the right Th3 spinal root, spreading into the thoracic cavity and compressing the apex of the right lung. The introduction presents the frequency of occurrence of such neoplasms among all spinal cord tumors. Complaints, neurological symptoms, neoplasm diagnosis methods and its clinical and radiological manifestations are presented. The article describes stages and techniques of total surgical removal of the spinal hourglass schwannoma through the posterior paravertebral minimally invasive approach without opening the pleural cavity and with using neurophysiological monitoring of spinal cord function performed at the Neurosurgical Clinic of the Novosibirsk Research Institute of Traumatology and Orthopaedics named after Ya.L. Tsivyan. The data of histological examination and MRI control in the early postoperative period are presented. The follow-up was carried out at 6 and 12 months after surgical treatment. The publications describing the surgical treatment of paravertebral tumors through different approaches, the technique of their removal, possible complications and methods of their prevention are analyzed, and the classification according to Eden (1941), which characterizes the type of tumor growth, is presented.

Abstract

Aim. To approve of diagnostic effectiveness of SWAN (SWI) images in revealing of calcium containing epileptogenic substrates in children with resistant focal epilepsy.Materials and methods. The results of MRI in children with refractory focal epilepsy obtained in the Radiology Department of the Russian State Children Hospital in the period from 2018 to 2020 were observed retrospectively. High-resolution epileptological MR protocol used for investigation of 67 children. SWAN was applied in all cases for identification of calcium containing epileptogenic substrates, including cavernomas, DVA syndrome, cortical gangliogliomas, Sturge-Weber syndrome and tuberous sclerosis complex. All images were received by using MRI 3T 750 W Discovery GE.Results. In 17 cases (25%) SWAN provided important diagnostic information about the nature of the of epileptogenic lesion, its prevalence and borders. Additional earlier invisible structural changes were revealed in 2 cases of SWS and 1 cases of FCD; and in 13 cases SWAN gave us possibility to avoid CT for approving calcium in epileptogenic focus.Conclusion. We believe that adding SWAN in to the epileptological MR protocol is the necessary step for optimizing calcium and blood degradation products identification in the structure of potential epileptogenic focuses. Moreover, it would be very effective instrument for differential diagnosis of cerebral structural changes, specifying its etiology and, hence, would have influence on the therapeutic tactic and surgical strategy in children with focal epilepsy.

Abstract

The results of recent studies indicate a significant role of gut microbiota in the pathogenesis of inflammatory bowel diseases (IBD). The aim of the study was to study the taxonomic and functional composition of the gut microbiota in ulcerative colitis (UC) and Crohn's disease (CD) patients to identify key markers of dysbiosis in IBD. Materials and methods. Fecal samples obtained from 95 IBD patients (78 UC and 17 CD) as well as 96 healthy volunteers were used for whole-genome sequencing carried out on the SOLiD 5500 W platform. Taxonomic profiling was performed by aligning the reeds, not maped on hg19, on MetaPhlAn2 reference database. Reeds were mapped using the HUNAnN2 algorithm to the ChocoPhlAn database to assess the representation of microbial metabolic pathways. Short-chain fatty acids (SCFA) level were measured in fecal samples by gas-liquid chromatographic analysis. Results and discussion. Changes in IBD patients gut microbiota were characterized by an increase in the representation of Proteobacteria and Bacteroidetes phyla bacteria and decrease in the number of Firmicutes phylum bacteria and Euryarchaeota phylum archaea; a decrease in the alpha-diversity index, relative representation of butyrate-producing, hydrogen-utilizing bacteria, and Methanobrevibacter smithii; increase in the relative representation of Ruminococcus gnavus in UC and CD patients and Akkermansia muciniphila in CD patients. Reduction of Butyryl-CoA: acetate CoA transferase gene relative representation in CD patients, decrease of absolute content of SCFA total number as well as particular SCFAs and main SCFAs ratio in IBD patients may indicate inhibition of functional activity and number of anaerobic microflora and/or an change in SCFA utilization by colonocytes. This article provides a detailed description of clinical and electroanatomical characteristics of epilepsy in patients suffering from epileptic trances – a rare type of focal motor seizures with ambulatory automatism manifesting as an unplanned travel. We reviewed the currently available data on the nature of psychical seizures in patients with epilepsy and the role of social and legal expertise for this disease. We developed the criteria for differential diagnosis between epileptic trances and other conditions with similar manifestations (for example, epileptic fugues) and identified a diagnostic minimum. We also proposed a hypothesis of ictal spatial migration. We emphasized the efficiency of surgical treatment in the case of compliance with a proper diagnostic algorithm.

Abstract

Since the spread of multidrug-resistant Klebsiella pneumoniae (MDRKP) strains is considered as a challenge for patients with weakened or suppressed immunity, the emergence of isolates carrying determinants of hypervirulent phenotypes in addition may become a serious problem even for healthy individuals. The aim of this study is an investigation of the nonoutbreak K. pneumoniae emergence occurred in early 2017 at a maternity hospital of Kazan, Russia. Ten bacterial isolates demonstrating multiple drug resistance phenotypes were collected from eight healthy full-term breastfed neonates, observed at the maternity hospital of Kazan, Russia. All the infants and their mothers were dismissed without symptoms or complaints, in a satisfactory condition. Whole-genome shotgun (WGS) sequencing was performed with the purpose to track down a possible spread source(s) and obtain detailed information about resistance determinants and pathogenic potential of the collected isolates. Microdilution tests have confirmed production of extended-spectrum β-lactamases (ESBL) and their resistance to aminoglycoside, β-lactam, fluoroquinolone, sulfonamide, nitrofurantoin, trimethoprim, and fosfomycin antibiotics and Klebsiella phage. The WGS analysis has revealed the genes that are resistant to aminoglycosides, fluoroquinolones, macrolides, sulfonamides, chloramphenicols, tetracyclines, and trimethoprim and ESBL determinants. The pangenome analysis had split the isolates into two phylogenetic clades. The first group, a more heterogeneous clade, was represented by 5 isolates with 4 different in silico multilocus sequence types (MLSTs). The second group contained 5 isolates from infants born vaginally with the single MLST ST23, positive for genes corresponding to hypervirulent phenotypes: yersiniabactin, aerobactin, salmochelin, colibactin, hypermucoid determinants, and specific alleles of K- and O-antigens. The source of the MDRKP spread was not defined. Infected infants have shown no developed disease symptoms.

Currently, there are no doubts about the relevance of surgery as a part of integral treatment. However, minimally invasive surgeries for treatment of pulmonary tuberculosis are rarely used due to post-inflammatory changes in the pleural space and lung root. And outcomes of robot-assisted lobectomies in pulmonary tuberculosis patients have never been investigated.The objective of the study: to investigate the efficiency and safety of robot-assisted surgeries in pulmonary tuberculosis patients.Subjects and methods. Since May 2013, 56 patients suffering from focal unilateral pulmonary tuberculosis were enrolled into a prospective study, after having an adequate course of anti-tuberculosis chemotherapy. At the moment of surgery, bacillary excretion persisted in 32% of patients, and 90.5% of patients had cavities.Results. All patients had robot-assisted lobectomies using the surgical system of Da Vinci Si. The average time of surgery made 174 minutes (90-380 minutes), the blood loss made 82 ml (10-500 ml). In 2 (3%) patients, a robot-assisted access was converted into lateral thoracotomy. The frequency of post-operative surgical complications made 25% [6].Conclusion. High clinical efficiency and safety are associated with robot-assisted lobectomies as a part of the integral treatment of pulmonary tuberculosis patients.

18. Results of surgical treatment of the thoracic spinal nerve tumors: a retrospective analysis

Objective. To analyze clinical and morphological features of neurogenic hourglass tumors in the thoracic spine and their impact on outcomes of treatment through posterior surgical approaches.Material and Methods. The results of surgical treatment of 295 patients with tumors growing from the nerve roots of the thoracic spinal cord were studied. In 63 (21 %) of them, tumors of the spinal nerves were diagnosed. The vast majority of neoplasms were represented by Grade 1 neuromas – in 57 (90 %) patients, Grade 1 neurofibromas were found in 3 (5 %) patients, and High-grade malignant tumors – in 3 (5 %). Intracanal neoplasms were found in 42 (66.7 %) cases and intraextravertebral (hourglass) – in 21 (33.3 %).Results. Microsurgical removal of tumor was performed using two types of low-traumatic surgical approaches: 1) posterior median approach – in 56 (89.0 %) cases, of which hemilaminectomy was performed in 36 (64.4%) cases, interlaminectomy – in 15 (26.8 %) cases, and laminectomy – in 5 (8.1 %) cases; 2) paravertebral approach – in 7 (11.0 %) out of 63 cases with partial facetotomy or facetectomy and resection of part of the head and upper or lower edge of the rib at the same level. Tumors were removed totally in 56 (88.9 %) patients and subtotally – in 7 (11.1%). Intracanal tumors were removed totally in 40 (95.2 %) patients. A similar totality was achieved in removal of 16 (76.2 %) of hourglass neurinomas. Surgical interventions performed in the early postoperative period improved the functional state of patients: the Karnofsky Performance Scale (KPS) index increased from 70–80 to 90 %, the VAS pain score decreased from 5–6 to 2 points. Good clinical outcomes were achieved in 42 (66.7 %) patients, satisfactory – in 17 (27.0 %), and unsatisfactory – in 6 (6.3 %). Twenty nine patients had symptoms of myelopathy, complete regression of which occurred in 3 (10.3 %) cases, partial – in 9 (34.6 %), in 13 (50.0 %) cases they remained at the preoperative level, and in 4 (15.4 %) – worsened.Conclusion. The use of modern neurointroscopy, microsurgical techniques and low-traumatic posterior surgical approaches for resection of tumors of the spinal nerve roots in the thoracic spine provides good clinical outcomes of treatment in the early postoperative period. The existing hourglass tumor in the thoracic spine reduces the likelihood of its total removal when performing a low-traumatic posterior approach.

19. Network form of realization of universities’ educational programs: has a certainty appeared?

Abstract

In the article, the authors consider the latest by-law regulation of the institute of the network form of the implementation of educational programs in the higher education system of Russia. Despite the emergence of normative certainty on several issues, there are still quite ambiguous provisions. Thanks to this study, key ones have been identified and commented on. The popularity of the network form among universities is still relatively low. The authors have systematized several topical reasons for the conclusion of agreements on the network form by universities. Qualitative changes in the popularity of the online form can occur both thanks to new state programs (Priority-2030) and the renewal of development programs for leading Russian universities (for example, St. Petersburg State University). A separate issue is proposed at the sub-legal level as an approximate form of an agreement on the network form. The authors see the legal nature of such an agreement as undoubtedly administrative. But the conclusion of the agreement is accompanied by many nuances discussed in the article, and which must be kept in mind by universities. After the study, the authors highlighted priority issues for continuing to give the network form the expected normative certainty.

The main purpose of an integrated monitoring research is the analysis of the main directions of research activity of faculty, staff and young scientists of the university.Methods. Scientific and theoretical analysis of publications on the researched topic are used as basic methods; sociological and diagnostic data collection methods; the method of statistical processing and classification of documentary and empirical data; the methods of content analysis and quantification of documentary and sociological information.Scientific novelty. The research is characterized by an integrated approach to the study of the problem: the basic provisions are analyzed; conclusions and recommendations of reports on research projects made by members of temporary research teams (or, university scientists and teaching staff). The classification (depending on the translation vectors results) of dissertation works of graduate students, doctoral candidates is carried out in the course of the present study. Documentary information about the publication and presentation of scientific and pedagogical staff of the university is systematized; the report and information cards on the activities of innovative platforms are analyzed. The research team, with the direct participation of the author, after studying a few scientific publications on the subject, has developed an original method of complex research of the main directions of research activity of university scientists.Results. The presented research has allowed to note publication and innovative activity of the research and educational personnel, along with other its types, can act as the indicators characterizing the main directions of research activity both of higher education institution in general, and its concrete educational and scientific divisions. At the same, time the author emphasizes that efficiency of research process is caused not so much by quantitative as qualitative characteristics of concrete research staff, its vocational, scientific and creative potential.Practical significance. This approach can be used while studying the issues of pragmatic use of the main directions of research activity results in higher education in various fields. At the same time, the conclusions and recommendations as a result of this integrated monitoring study can serve as a basis for the development and management decisions aimed at improving the scientific and practical potential of university research in general and its specific areas.

Abstract

Quality is the key problem of the Russian education system as a whole and higher vocational training in particular, the level and quality of the later determining the growth of intellectual potential of the state, andconsequently its effective economic development and competitiveness. In the last years, Russian higher schools have lost their position in the international educational service market as the result of several premature reforms. The paper denotes the findings of sociological research, conducted in Moscow pedagogical universities, and aimed at analyzing the factors adversely affecting the quality of educational institutions, students’ satisfaction, and transparency of educational management. The survey involved the total respondent number of 881, including both the academic staff and full-time students. Sociological diagnostics based on the above monitoring research revealed the multi-factorial determinant of the high quality educational institution. The research findings emphasize the complexity of problem solving related to fostering the country’s intellectual potential, and the need for informal and systematic quality control. The research data can be of interest to sociology experts, educational management and academic staff.

A liquid-scintillator γ-ray detector has been investigated. The electronics, which consists of a preamplifier and a shaping amplifier of the detector signals, is described. A method for determining the start pulse for the time-of-flight technique has been developed based on the detection of several (three or more) γ rays from a neutron source in a narrow time interval. This method is insensitive to the timing instability of the sync pulse of the proton beam. The energy resolution of the γ-detector section is compared to the simulation results for the γ-ray detection. The potential of the γ detector for measuring cross sections for radiative neutron capture by a nucleus is shown.

Abstract

Aim. To approve of diagnostic effectiveness of SWAN (SWI) images in revealing of calcium containing epileptogenic substrates in children with resistant focal epilepsy.Materials and methods. The results of MRI in children with refractory focal epilepsy obtained in the Radiology Department of the Russian State Children Hospital in the period from 2018 to 2020 were observed retrospectively. High-resolution epileptological MR protocol used for investigation of 67 children. SWAN was applied in all cases for identification of calcium containing epileptogenic substrates, including cavernomas, DVA syndrome, cortical gangliogliomas, Sturge-Weber syndrome and tuberous sclerosis complex. All images were received by using MRI 3T 750 W Discovery GE.Results. In 17 cases (25%) SWAN provided important diagnostic information about the nature of the of epileptogenic lesion, its prevalence and borders. Additional earlier invisible structural changes were revealed in 2 cases of SWS and 1 cases of FCD; and in 13 cases SWAN gave us possibility to avoid CT for approving calcium in epileptogenic focus.Conclusion. We believe that adding SWAN in to the epileptological MR protocol is the necessary step for optimizing calcium and blood degradation products identification in the structure of potential epileptogenic focuses. Moreover, it would be very effective instrument for differential diagnosis of cerebral structural changes, specifying its etiology and, hence, would have influence on the therapeutic tactic and surgical strategy in children with focal epilepsy.

Abstract

The Mu2e experiment at Fermilab will search for the charged lepton flavor violating neutrino-less conversion of a negative muon into an electron in the field of an aluminum nucleus [1] , [2] . The Mu2e detector is comprised of a tracker, an electromagnetic calorimeter, and an external veto for cosmic rays. The calorimeter plays an important role in providing excellent particle identification capabilities, a fast and online trigger filter while aiding the track reconstruction capabilities. The calorimeter requirements are to provide a large acceptance for 100-MeV electrons and reach: 1) a time resolution better than 0.5 ns at 100 MeV; 2) an energy resolution O(10%) at 100 MeV; and 3) a position resolution of about 1 cm. The calorimeter consists of two disks, each one made of 674 pure CsI crystals readout by two large-area $2 \times 3$ array of UV-extended silicon photomultipliers (Mu2e SiPMs) of $6\,\,\times6$ mm2 dimensions. A large-scale prototype has been constructed with 51 preproduction crystals readout by 102 Mu2e SiPMs. It has been tested at the beam test facility in Frascati, demonstrating satisfying results compared to the Mu2e requirements. At the moment of writing, the crystals production phase is halfway through the work, while the SiPM production has been completed. An overview of the characterization tests is also reported, together with a description of the final calorimeter design.

Abstract

Tuberous sclerosis is one of the genetically determined neurodermal syndromes, characterized by focal epilepsy, which is resistant to drug therapy in most cases. Tuberous sclerosis has not been surgically treated for a long time due to the multifocal nature of brain damage. However, using new instrumental methods the surgeon can isolate tuber – “drivers”, responsible for epileptic induction, which, therefore, can be a target for surgical removal. The article presents a clinical case of the treatment of pharmacoresistant epilepsy in a 17-year-old girl with tuberous sclerosis and normal neuropsychiatric and cognitive status; the surgeon managed to achieve a complete regression of seizures after the surgical removal of one of the tubers. Using a multimodal complex of examinations, including high-resolution magnetic resonance imaging and invasive electroencephalographic monitoring, the surgeon managed to identify the “inductor” tuber associated with the epileptic initiation zone among multiple cortical tubers. This section of the cerebral cortex was subjected to surgical resection. For the entire postoperative period (16 months) there were no seizures and antiepileptic drugs were canceled; cognitive processes, memory were improved. Thus, active surgical tactics may be effective in more than 50% of patients with tuberous sclerosis and pharmacoresistant epilepsy. This case confirms the need for a multidisciplinary examination prior to the surgery in patients with tuberous sclerosis.

Abstract

The shape of pulses produced in a 3He counter by thermal neutrons has been investigated. The electronics, which consist of a charge-sensitive preamplifier and a shaping amplifier, have been developed. The measured pulse shape is compared to the result of simulation of the 3He counter signals under neutron irradiation. A new method for determining the pressure in a 3He counter is proposed that is based on measurements of the total charge-collection efficiency and the maximum effective pulse width in the counter. This method is in good agreement with the direct measurement of the pressure in a 3He counter using the transmission method.

Abstract

Introduction. Cortical dysplasias (CDs) encompass a wide variety of disorders that in most cases lead to epilepsy, especially in infants and young children. MRI diagnosis of CDs is a major part of presurgical examination of pediatric patients with resistant focal epilepsy.Aim. To identify MR markers of CD in the immature brain and develop an MRI protocol for early diagnosis of CDs.Materials and methods. Children aged Results. The rate of detection of small-size cortical malformations, such as nodular heterotopies or focal cortical dysplasias was significantly higher in groups of patients whose brains (according to MR images) were at the infantile or adult phases of myelination. In children with the isointensive phase myelination, only large cortical dysplasias could be identified. In the first phase, the focal malformations had low amplitude signals in T2-weighted images and high amplitude signals in T1, unlike those in adult patients. In the isointensive phase, the quality of visualization was significantly reduced and provided poor diagnostic information.Conclusion. The results confirm the diagnostic significance of early (before age of 5 months) MRI testing in cases with suspected CD-associated focal epilepsy. However, at the period between 5 and 12 months of age, MR imaging was ineffective for CD diagnosing. Later, in the period from 12 to 15 months, the MRI ability to identify the CDs gradually increased. We consider the standard T2 weighted images with high TR values, the most effective MR modality for diagnosing CDs in young children.

According to the results of phenotype studies, Kl. pneumoniae in 92,3% of cases were ESBL producers and showed resistance to ampicillin, amoxiclav, co-trimoxazole, less to ciprofloxacin and nitrofurantoin. 90% of intestinal Kl. pneumoniae had multiple resistance to antimicrobial drugs. According to a genome-wide study, beta-lactamase genes were found in all strains. blaOXA-1 was found in 60%, blaTEM-30 in 70%, blaCTX-M-15 in 70%, blaSHV in 100% of the strains. Genes for resistance to aminoglycosides were found in 90%, fluoroquinolones — in 40%, sulfonamides — in 80%, chloramphenicol — in 70%, tetracycline — in 50%, and trimethoprim – in 70% of Kl. pneumoniae strains.

Abstract

Background: insulinoma is a neuroendocrine tumor characterized by hyperproduction of insulin and hypoglycemia that decrease patient's quality of life. The only curative method of treatment is surgery. Aim: studying the quality of life of patients after various options for surgical treatment of insulin-producing pancreatic tumors. Materials and methods: 48 patients operated in the First Moscow State Medical University were included in the quality of life study. Patients were divided into two groups: those who underwent tumor enucleation (n 28), and patients who underwent resection of the pancreas (n 20). The study was performed using the questionnaire SF-36, not earlier than 3 months after surgery. Results: in the study of quality of life, a significant difference was only in a scale of role-physical functioning, which was higher in patients that underwent tumor enucleation, as opposed to patients after resection procedures. Conclusion: the quality of life in both groups remained at a high level; the differences were noted only in the scale of role-physical functioning, which was higher in patients after tumor enucleation.

Abstract

Background: pancreaticneuroendocrine tumors occur in almost all patients with multiple endocrine neoplasia type 1 (MEN1) and are a major cause of death. Surgical resection is the only potentially curative therapy for pNETs, otherwise various endocrine syndromesdevelop threatening the patient's life. Aim: to evaluate the short-term results of surgical management of patients with pancreatic neuroendocrine tumors with MEN1. Materials and methods: by last 20 years, 40 patients with pancreatic neuroendocrine tumors within MEN1 were operated. Results: in the early postoperative period,according to the classification of Clavien-Dindo, complications occurred in 12 (26%) patients. The following types of complications were diagnosed: IIIa in 6 patients; IIIb - in 2; type V in 4 patients. Complications of types I, II and IV were not identified. Conclusion: less percentage of postoperative complications is achieved by performing organ-preserving operations, after precise imaging diagnose, due to minimal intraoperative pancreatic injury.

Abstract

Background: Surgical resection is considered to be the only potentially curative option for patients with pancreatic neuroendocrine tumors (pNETs). High risk rates of perioperative complications make minimally invasive ablative techniques a novel perspective and alternative treatment option for pancreatic neuroendocrine tumors. This study aims to present the first experience of using a microwave ablation in management of pNETs. Methods: Sechenov University has an experience of treating more than 400 patients with hormone-producing tumors of the pancreas, 7 of which were treated by microwave ablation (MWA). Results: In all patients that underwent MWA, a regression of hormonal symptomatic was achieved. Two patients required readmission a month later for draining of pseudocyst and abscess. Conclusions: The exact role of microwave ablation in the treatment of non-metastatic pancreatic neuroendocrine tumors has not been defined yet. There is a lack of large prospective randomized studies and the reason for this is that local tumor destruction is indicated in selected cases only, thus making it difficult to analyze a large group of patients and assess long-term results of the treatment. However, microwave ablation allows to take a better control of symptoms in patients with hormone overproduction and in those with high risk of postoperative complications.

Abstract

To evaluate an efficacy of lipoaspirate-based products in pathologic scarring management.There were 118 patients with external scars. Depending on scar type, localization and need for soft tissue augmentation, three different methods were used for lipoaspirate-based product preparation: 15-minute sedimentation, centrifugation at 1200 g for 3 minutes and emulsification with a channel diameter of 1.2 mm. Results were assessed using the Manchester Scar Scale (MSS) and photographing.According to MSS analysis, the following results were obtained: before treatment - 11.6 (9.3-13.3) scores, 3 months after treatment - 6.5 (5.1-7.2) scores, 6 months after treatment - 5.2 (4.5-6.1) scores. Significant differences were obtained for baseline values and both control points. Stable results were obtained in long-term follow-up period (12-24 months). No major adverse effects were observed. Minor complications were registered in 10.1% of patients.Injections of lipoaspirate-based products is an effective option for the treatment of patients with pathologic scarring. This approach is intermediate between conservative and conventional surgical treatment.Оценить эффективность инъекционной аутотрансплантации продуктов на основе липоаспирата в лечении пациентов с патологическими рубцовыми изменениями.В исследование включены 118 пациентов с рубцами наружной локализации. В зависимости от типа и размера рубца, его локализации и необходимости увеличения объема тканей в реципиентной зоне применялись три способа подготовки жирового трансплантата: седиментация в течение 15 мин, центрифугирование в режиме 1200 g в течение 3 мин и эмульсификация через переходник с диаметром канала 1,2 мм. Полученные результаты оценивались при помощи Манчестерской шкалы рубцов (Manchester Scar Scale — MSS) и цифрового фотографирования.При анализе данных по MSS получены следующие результаты: до лечения — 11,6 (9,3—13,3) балла, через 3 мес после окончания лечения — 6,5 (5,1—7,2) балла, через 6 мес после окончания лечения — 5,2 (4,5—6,1) балла. Статистически значимыми были различия между показателями до лечения и обеими контрольными точками (Инъекционная аутотрансплантация продуктов на основе жировой ткани — эффективный метод лечения пациентов с патологическими рубцовыми изменениями. Данный подход является промежуточным звеном между консервативными и хирургическими методами.

Abstract

We describe an analysis comparing the pp[over ¯] elastic cross section as measured by the D0 Collaboration at a center-of-mass energy of 1.96 TeV to that in pp collisions as measured by the TOTEM Collaboration at 2.76, 7, 8, and 13 TeV using a model-independent approach. The TOTEM cross sections, extrapolated to a center-of-mass energy of sqrt[s]=1.96 TeV, are compared with the D0 measurement in the region of the diffractive minimum and the second maximum of the pp cross section. The two data sets disagree at the 3.4σ level and thus provide evidence for the t-channel exchange of a colorless, C-odd gluonic compound, also known as the odderon. We combine these results with a TOTEM analysis of the same C-odd exchange based on the total cross section and the ratio of the real to imaginary parts of the forward elastic strong interaction scattering amplitude in pp scattering for which the significance is between 3.4σ and 4.6σ. The combined significance is larger than 5σ and is interpreted as the first observation of the exchange of a colorless, C-odd gluonic compound.

Abstract

In this paper, we report on the study of the optical properties of YAG:Ce and GGG:Ce garnet crystals after irradiation in a 660~MeV proton beam with a fluence up to 8.9$\times$10$^{14}$ protons/cm$^2$. We found that the transparency of both crystals fell by no more than 7\% in the region of their own luminescence. The light yield of a YAG:Ce sample, measured one year after irradiation, dropped by about 35\%.

Abstract

We report on the optical and structural properties of plastic scintillators irradiated with neutron beams produced by the IBR-2 reactor of the Frank Laboratory of Neutron Physics in JINR, Dubna. Blue UPS-923A and green plastic scintillators were irradiated with neutron fluence ranging from 10$^{13}$ to 10$^{17}$ n/cm$^2$. Discolouring in the plastic scintillators was observed after irradiation. The effects of radiation damage on the optical and structural properties of the samples were characterized by conducting light yield, light transmission, light fluorescence and Raman spectroscopy studies. The results showed that neutron radiation induced damage in the material. The disappearance of the Raman peak features in green scintillators at frequencies of 1165.8, 1574.7 and 1651.2 cm$^{-1}$ revealed significant structural alterations due to neutron bombardment. Losses in fluorescence intensity, light yield and light transmission in the plastic scintillators were observed., 19 pages, 9 figures

Abstract

The CDF and D0 experiments at the Fermilab Tevatron have measured the asymmetry between yields of forward- and backward-produced top and antitop quarks based on their rapidity difference and the asymmetry between their decay leptons. These measurements use the full data sets collected in proton-antiproton collisions at a center-of-mass energy of √s=1.96 TeV. We report the results of combinations of the inclusive asymmetries and their differential dependencies on relevant kinematic quantities. The combined inclusive asymmetry is At¯tFB=0.128±0.025. Abstract

This article considers poetry published in Ural periodicals between 1917 and 1919. It mainly comprises journalistic newspaper verses that are of interest from the point of view of how they reflect public opinion about the events of the 1917 revolutions and the Civil War in the Urals. The range of judgments and assessments of the events in question is determined not only by personal and political preferences, but primarily by the collective notions that are characteristic of mass consciousness. The author of the article concentrates on this aspect. He is interested in stable motifs and images that go back to the collective unconscious of the Ural public. To designate these universals immersed in tradition, he sometimes employs overlapping concepts: topos, archetype, and mythologeme. They assimilate various mental spheres — politics, religion, everyday consciousness, and establish a correlation of an image with timeless scales and thereby take the modeling of that image of the revolution to a higher level of figurative generalisations. The article examines individual figural details and motifs (parallels Revolution — Easter, connections with calendar rituals, seasons, agrarian mythology, colour and light symbols), as well as the meta-story of revolutionary poetry that included the modifying figures of the Hero, Enemy, Victim, depicted Revolution as a struggle and a difficult path to paradise (communism). As a result, the author of the article comes to the conclusion that reliance on common cultural experience based on archetypal principles promotes a profound and fruitful comprehension of life in the era of a historical crisis affecting the interests of the general public.

Abstract

The author emphasizes that the theoretical and methodological presuppositions of this study made a number of socio-economic factors, which he carried the formation of the social institution of education as one of the leading mechanisms of development and the most important conditions for the formation of an innovative society, and the fact that science is at the present stage of development society is transformed into a large and independent branch of the economy, acquiring the features of a direct productive force of society, becoming an essential tool not only spiritual, but also material reproduction of life. The article discusses the results of one of the parts of the Integrated Monitoring sociological research practical application problems of translation of scientific and research activity of the faculty and research staff of the university humanitarian and pedagogical profile in the education space. The study was completed in September 2015. The results of the analysis of scientific research (R&D), in which were allocated levels (spheres) of educational space in which the broadcast and/or could be broadcast results of specific research projects. The study was carried out a comparative analysis and summarizes the socio-professional composition of performing R&D actors. The author formulates a number of proposals aimed at improving the management of research activities of the university and modernization policies of university management science. It focuses on the possibilities of social science in matters of understanding the processes and interactions that are formed in the research field. The article suggests that the author’s technique and methods of research status and characteristics of the research activities at the university humanitarian and pedagogical profile may find their application in different high schools of specialization, provided that they adapt to the social and professional characteristics of the particular institution.

Abstract

Since the spread of multidrug-resistant Klebsiella pneumoniae (MDRKP) strains is considered as a challenge for patients with weakened or suppressed immunity, the emergence of isolates carrying determinants of hypervirulent phenotypes in addition may become a serious problem even for healthy individuals. The aim of this study is an investigation of the nonoutbreak K. pneumoniae emergence occurred in early 2017 at a maternity hospital of Kazan, Russia. Ten bacterial isolates demonstrating multiple drug resistance phenotypes were collected from eight healthy full-term breastfed neonates, observed at the maternity hospital of Kazan, Russia. All the infants and their mothers were dismissed without symptoms or complaints, in a satisfactory condition. Whole-genome shotgun (WGS) sequencing was performed with the purpose to track down a possible spread source(s) and obtain detailed information about resistance determinants and pathogenic potential of the collected isolates. Microdilution tests have confirmed production of extended-spectrum β-lactamases (ESBL) and their resistance to aminoglycoside, β-lactam, fluoroquinolone, sulfonamide, nitrofurantoin, trimethoprim, and fosfomycin antibiotics and Klebsiella phage. The WGS analysis has revealed the genes that are resistant to aminoglycosides, fluoroquinolones, macrolides, sulfonamides, chloramphenicols, tetracyclines, and trimethoprim and ESBL determinants. The pangenome analysis had split the isolates into two phylogenetic clades. The first group, a more heterogeneous clade, was represented by 5 isolates with 4 different in silico multilocus sequence types (MLSTs). The second group contained 5 isolates from infants born vaginally with the single MLST ST23, positive for genes corresponding to hypervirulent phenotypes: yersiniabactin, aerobactin, salmochelin, colibactin, hypermucoid determinants, and specific alleles of K- and O-antigens. The source of the MDRKP spread was not defined. Infected infants have shown no developed disease symptoms.

Abstract

The results of recent studies indicate a significant role of gut microbiota in the pathogenesis of inflammatory bowel diseases (IBD). The aim of the study was to study the taxonomic and functional composition of the gut microbiota in ulcerative colitis (UC) and Crohn's disease (CD) patients to identify key markers of dysbiosis in IBD. Materials and methods. Fecal samples obtained from 95 IBD patients (78 UC and 17 CD) as well as 96 healthy volunteers were used for whole-genome sequencing carried out on the SOLiD 5500 W platform. Taxonomic profiling was performed by aligning the reeds, not maped on hg19, on MetaPhlAn2 reference database. Reeds were mapped using the HUNAnN2 algorithm to the ChocoPhlAn database to assess the representation of microbial metabolic pathways. Short-chain fatty acids (SCFA) level were measured in fecal samples by gas-liquid chromatographic analysis. Results and discussion. Changes in IBD patients gut microbiota were characterized by an increase in the representation of Proteobacteria and Bacteroidetes phyla bacteria and decrease in the number of Firmicutes phylum bacteria and Euryarchaeota phylum archaea; a decrease in the alpha-diversity index, relative representation of butyrate-producing, hydrogen-utilizing bacteria, and Methanobrevibacter smithii; increase in the relative representation of Ruminococcus gnavus in UC and CD patients and Akkermansia muciniphila in CD patients. Reduction of Butyryl-CoA: acetate CoA transferase gene relative representation in CD patients, decrease of absolute content of SCFA total number as well as particular SCFAs and main SCFAs ratio in IBD patients may indicate inhibition of functional activity and number of anaerobic microflora and/or an change in SCFA utilization by colonocytes. Conclusion: the revealed changes can be considered as typical signs of dysbiosis in IBD patients and can be used as potential targets for IBD patients personalized treatment development.

Abstract

The Mu2e calorimeter is composed of two disks each containing 1348 pure CsI crystals, each crystal read out by two arrays of 6x6 mm2 monolithic SiPMs. The experimental requirements have been translated in a series of technical specifications for both crystals and SiPMs. Quality assurance tests, on first crystal and then SiPM production batches, confirm the performances of preproduction samples previously assembled in a calorimeter prototype and tested with an electron beam. Abstract

The Mu2e experiment at Fermilab will search for the charged-lepton flavour violating neutrino-less conversion of a negative muon into an electron in the field of an aluminum nucleus. The calorimeter plays an important role in providing a fast trigger filter, excellent particle identification and a seeding for track reconstruction. Its requirements are to provide an energy (timing) resolution better than 10% (0.5 ns) and a position resolution below 1 cm, for 100 MeV electrons. The calorimeter consists of two disks, each one made of 674 un-doped CsI crystals readout by two large area arrays of 2 × 3 UV-extended SiPMs of 6 × 6 mm^2 dimensions. A large scale prototype (Module-0) has been constructed and tested with an electron beam in the energy range between 60 and 120 MeV at the BTF of Frascati National Laboratories. Results demonstrated that this calorimeter satisfies the Mu2e requirements.

Abstract

The Mu2e electromagnetic calorimeter is made of two disks of un-doped parallelepiped CsI crystals readout by SiPM. There are 674 crystals in one disk and each crystal is readout by an array of two SiPM. The readout electronics is composed of two types of modules: 1) the front-end module hosts the shaping amplifier and the high voltage linear regulator; since one front-end module is interfaced to one SiPM, a total of 2696 modules are needed for the entire calorimeter; 2) a waveform digitizer provides a further level of amplification and digitizes the SiPM signal at the sampling frequency of $200\ \text{M}\text{Hz}$ with 12-bits ADC resolution; since one board digitizes the data received from 20 SiPMs, a total of 136 boards are needed. The readout system operational conditions are hostile: ionization dose of $20\ \text{krads}$, neutron flux of $10^{12}\ \mathrm{n}(1\ \text{MeVeq})/\text{cm}^2$, magnetic field of $1\ \text{T}$ and in vacuum level of $10^{-4}\ \text{Torr}$. A description of the readout system and qualification tests is reported.
