A total of 226 patients was studied in a phase-Ill multicenter trial to compare the results of stress/rest 99mTc-MIBI with 201TI scintigraphy and/or coronary arteriography. The evaluation of planar and SPECT imaging was based on a semiquantitative visual scoring. A congruent diagnosis was generated from all planar 201TI and 99mTc-MIBI scintigrams performed in 36 patients. In 62 of 63 patients, SPECT-studies with 99mTc-MIBI and 201TI led to an identical diagnosis. In one patient a positive result was obtained with 201TI-SPECT whereas 99mTc-MIBI was negative. Segmental agreement for 1509 SPECT segments was 85.7%, for 554 planar segments it was 86.6%. In comparison to coronary arteriography, performed in 180 patients, the overall sensitivity for the correct diagnosis of coronary artery disease (CAD) was 86% for planar MIBI scintigraphy or 92% for MIBI-SPECT. Overall specificity was 100% for planar imaging and 57% for SPECT. The low specificity of MIBI-SPECT was probably due to high prevalence of CAD in this study population. In a subgroup of 43 patients, who underwent coronary angiography, identical results were found with 99mTc-MIBI and 201TI. Only one patient showed a positive scintigraphic result with both 99mTc-MIBI and 201TI without angiographically proven stenosis greater than 50% or prior myocardial infarction. Vessel sensitivities in stenosed coronary arteries (>50% stenosis) were 54% for the LAD and 87% for the RCX/RCA areas with MIBI-SPECT, or 51 % or 79% for planar scintigraphy, respectively. Vessel specificities for SPECT were 65% in the U\D or 52% in the RCX/RCA, or 90% or 83% in planar imaging. According to the results of this study, 99mTc-MIBI is of the same diagnostic value as 201TI in nuclear cardiology. Scintigraphy with both radiopharmaceuticals correlates well with the diagnosis of CAD. Therefore, 99mTc-MIBI might be an effective substitute for 201TI for myocardial perfusion scintigraphy.

Gliquidone (a second generation sulphonylurea) was administered orally to normal rats 1 h before killing. Gliquidone treatment led to a decrease in plasma glucose, an increase in insulin and a diminution in glucagon concentration. Insulin binding to liver plasma membranes was enhanced by 40% in comparison with controls, whereas glucagon binding was slightly diminished. These findings indicate a greater sensitivity of liver cells to insulin during sulphonylurea treatment and support the view that sulphonylureas potentiate insulin action on the liver.

1. Bei einer Gruppe von 16 Miniaturschweinen, die unter Standardbedingungen gehalten wurde, wurde der Acetatspiegel im Blutplasma uber einen Zeitraum von 8 Monaten verfolgt. 2. Der mittlere Plasmaacetatspiegel betrug 195 nMol/ml Plasma, d. i. etwa doppelt so hoch wie beim Menschen. Verfutterung einer fettreichen Diat hatte keinen Einflus auf den Plasmaacetatspiegel. Ebenso ergaben sich keine Unterschiede bei Tieren, die im Stall gehalten wurden, oder die freien Auslauf hatten. 3. Die Enzyme Acetat-Thiokinase und Acetyl-CoA-Deacylase wurden in allen der untersuchten Gewebe angetroffen. Ihre Aktivitat war derjenigen von Rattengeweben vergleichbar. 4. Partikelfreie Extrakte aus Miniaturschweineaorten vermogen Acetat mit etwa gleich groser Geschwindigkeit in langkettige Fettsauren einzubauen wie Extrakte aus Rattenaorten. Die Bedeutung dieser Befunde fur die Entwicklung degenerativer Gefaswandveranderungen wird diskutiert.

The overall activity and distribution of pyruvate carboxylase and of phosphoenolpyruvate carboxykinase in rat liver was studied by applying an improved technique for the fractional extraction of hepatic enzymes. In the rat the mean activity of pyruvate carboxylase amounted to 10 units/g fresh liver (1 unit = 1 μmole pyruvate carboxylated/min at 30°). In the human liver the corresponding value was about 1 unit/g fresh wt. For phosphoenolpyruvate carboxy-kinase 2.5 and 10 units/g fresh wt. were measured in rat and human liver, respectively. During fractional extraction only negligible quantities of pyruvate carboxylase appeared in the soluble cytoplasmatic fraction. It is concluded that the bulk of enzyme activity is located within the mitochondria. By digitonin treatment of isolated rat liver mitochondria pyruvate carboxylase could be localized within the matrix space. Phosphoenolpyruvate carboxykinase was found to be located mainly in the soluble cytoplasmatic fraction. Our results strongly support the view that in rat liver gluconeogenesis pyruvate has to enter the mitochondria prior to its carboxy-lation to oxaloacetate.

Glucose tolerance after abdominal operation was studied with and without infusion of bradykinin. With bradykinin a normalization of the postoperative decreased assimilation of glucose was found.

Ein Zusammenhang zwischen Diabetes mellitus und Lebererkrankungen ist in der Literatur gut dokumentiert [2, 8, 14]. Dieses betrifft in erster Linie Fettleber und Le-berzirrhose. Der Zusammenhang zwischen akuter Virushepatitis und Diabetes mellitus ist dabei nicht eindeutig geklart [17]. So sind die fruher haufig bei Diabetikern gefundenen Hepatitiden weitgehend durch Ubertragung einer Serumhepatitis zu erklaren, da Einmalspritzen und -kanulen fur Injektionen und Blutentnahmen noch nicht zur Verfugung standen. Mit der Fragestellung, ob eine diabetische Stoffwechsellage die Ausbildung einer akuten Hepatitis begunstigt, wurden stoffwechselgesunde sowie Streptozotocin-diabetische Ratten nach Induktion einer experimentellen Galaktosaminhepatitis [12, 15], die der akuten Virushepatitis des Menschen ahnelt, untersucht.

Nachdem bekannt ist, das der Stoffwechsel des Fettgewebes eine sehr grose Sensitivitat gegenuber Insulin besitzt [3], ware es naheliegend, ein entsprechendes Verhalten analog auch fur das periphere Organ Muskulatur anzunehmen. Dagegen sollte die Ansprechbarkeit der Leber auf Insulin wegen der anatomischen Lokalisation des Pankreas mit direkter Sekretion des Hormons in die Vena portae und deshalb sehr hohen portalen Insulin-konzentrationen eher gering sein.

Nuclear and cytosolic androgen receptor concentrations in tissues of human benign prostatic hypertrophy (BPH) were determined by use of methyltrienolone (R-1881) and 7 alpha,17 alpha-dimethyl-19-nortestosterone (DMNT) as radiolabeled ligands. Cytosolic R-1881-binding sites were 46.1 +/- 43 fmol/mg protein and nuclear R-1881-binding sites were 51.8 +/- 42 fmol/mg protein. DMNT-binding sites in cytosol were 44.3 +/- 38 fmol/mg protein and in nuclear extract 73.4 +/- 64 fmol/mg protein. No significant correlation was found between the number of R-1881- and DMNT-binding sites in either cytosol or nuclear extracts. Cytosolic or nuclear androgen receptor content was not significantly correlated with the percentage of epithelial or stromal cells as determined from the corresponding histological sections. In BPH tissue with marked cystic degeneration, very low androgen receptor levels were found.

Die Vielzahl der gegenwartig fur die Pankreastransplantation zur Behandlung des Insulinmangeldiabetes (Typ I) angegebenen Verfahren demonstriert die Unsicherheit bezuglich des geeignetsten chirurgischen Verfahrens. Die heterotope Transplantation eines Pankreassegmentes mit Occlusion des Pankreasganges durch ein Polymer gilt als die technisch sicherste Methode, sie fuhrt aber zu einer Fibrosierung des exokrinen Drusenanteils und langzeitig auch zu Veranderungen der Inselzellen mit Verlust der endokrinen Funktion. Deshalb wird in jungster Zeit die Drainage des exokrinen Sekrets in ein Hohlorgan, z.B. Magen, Jejunum oder Blase, angestrebt, um ein morphologisch moglichst intaktes Transplantat zu bewahren.

Kenntnisse uber Stoffwechselstorungen als Ursache des Harnsteinleidens, ermoglichen eine metabolische Klassifikation der Harnsteinbildner in diagnostische Untergruppen. Albright (1) pragte 1953 erstmalig den Begriff der idiopathischen Hyperkalzurie, die weiter unterteilt wurde in eine absorptive und eine renale Form. Der von Pak (9) 1975 inaugierte orale Kalziumbelastungstest soil eine Differenzierung dieser Hyperkalzurieformen ermoglichen. Die Haufigkeit, mit der diese Diagnosen gestellt werden, ist jedochbei verschiedenen Untersuchernunterschiedlich (2, 5,9,10,11).

99mTc-HMPAO-SPECT and SPECT with the 123I-labelled benzodiazepine (Bz) receptor ligand Ro 16-0154 were performed in 10 patients suffering from partial epilepsy, without cerebral lesion in MRT or CT.2 h p.i. of Ro 16-0154 the distribution of activity correlated with the known distribution of Bz-receptors in the human brain. Perfusion and receptor-binding were found decreased in 7 patients of each study in the suspicious brain-area. 123I-labelled Ro 16-0154 is suitable for Bz-receptor mapping by SPECT. The decrease of Bz-receptor binding in epileptic foci, as described in PET-studies, was also detected by SPECT in 7 of 10 patients.

Untersuchungen von Felig et al. [1] haben gezeigt, das bei Verdrei- bis Vervierfachung des Glucagonspiegels eine Verdoppelung der Glucoseabgabe der Leber auftritt, die trotz fortgesetzter Glucagoninfusion nach 30 min wieder auf den Normalwert zuruckkehrt. Dabei stieg jedoch der Plasmainsulinspiegel geringfugig von 18 auf 26 μU/ml an, so das die Reduktion der hepatischen Glucoseabgabe bei der hohen Insulinsensitivitat des Lebergewebes dadurch erklart werden konnte. Untersuchungen von Grill et al. [2], die nach Beginn unserer Studie erschienen, ergaben bei gleicher Dosierung eine Verdoppelung der Glucoseabgabe und registrierten dabei in der Lebervene keinen Anstieg von Insulin und zyklischem AMP (cAMP). Allerdings hatten sie nicht innerhalb der ersten 5 min gemessen, in der normalerweise die hochsten Spiegel zu erwarten sind. Unsere Untersuchungen wurden deshalb so angelegt, das wir (a) noch weniger Glucagon verabreichten, um sicher keinen Insulinanstieg zu bekommen und (b) sowohl fur Insulin- als auch fur cAMP in den ersten 3 min Blutabnahmen durchfuhrten. Die vorliegenden Untersuchungen wurden im Rahmen einer groseren Studie uber den molekularen Wirkungsmechanismus von Glucagon auf Zellatmung sowie cytosolisches und mitochondriales Redoxpotential des Lebergewebes vorgenommen. Bisher wurden nur vier Probanden untersucht, so das die Ergebnisse als praliminar zu betrachten sind.

Polymorphonucleare Neutrophile (PMN) erzeugen als Folge der Phagocytose von Bakterien und Tumorzellen eine Chemolumineszenz. Nach Ingestion des Fremdpartikels werden die mikrobiziden Funktionen der PMN aktiviert, wobei hochreaktive Zwischenprodukte der Sauerstoffreduktion entstehen (1). Die Chemolumineszenz der Granulocyten korreliert direkt mit der metabolischen Aktivierung der PMN wahrend der Phagocytose und kann somit als Grundlage einer Messung der Lyseaktivitat von Granulocyten gegen Tumorzellen gemacht werden (2).

Neben Insulin fuhrt Muskelarbeit zu einer Steigerung der Glukoseaufnahme in den Skelettmuskel [1,2], weshalb fruher ein „muscle activity factor“ postuliert wurde [3]. Heute weis man jedoch, das auch fur diesen Effekt unter Muskelarbeit geringe Mengen Insulin notwendig sind [4]. Neueste Untersuchungen lassen vermuten, das endogene Kinine, welche wahrend Muskelarbeit und Hypoxie freigesetzt werden [19], an dieser Regulation des Kohlenhydratstoffwechsels beteiligt sind. Sie scheinen dabei uber eine Verbesserung der Insulinwirksamkeit zum Effekt zu fuhren [5–8]. So rief intraarteriell infundiertes, exogenes Bradykinin in physiologischen Dosen (10−11 M/1) bei niedrigen Insulinspiegeln im ruhenden Muskel eine Steigerung der Glukoseaufnahme sowie einen korrespondierenden Abfall der muskularen Laktatund Fettsaurenabgabe hervor [22]. ahnliche Ergebnisse wurden unter Muskelarbeit, Hypoxie und operativen Streszustanden an Gesunden und Diabetikern erzielt. Da auch der Aminosaurestoffwechsel durch Insulin und Muskelarbeit gleichartig beeinflust wird [9–13], stellte sich die Frage, ob Kinine auch an diesem Insulineffekt beteiligt sind. Hierzu wurden 21 gesunde, klinisch vergleichbare Probanden untersucht. Die Experimente fanden nach einem ubernachtfasten ohne Pramedikation statt. Die Katheterisierung einer tiefen Muskelvene sowie der Brachialarterie wurden an anderer Stelle ausfuhrlich beschrieben [5, 7]. Arterielle und tiefvenose Blutproben wurden alle 10 min wahrend einer 30minutigen Basalperiode und einer 30–40minutigen Testperiode simultan entnommen.

Beim Mammacarcinom besteht eine gute Korrelation zwischen Steroidreceptorgehalt und dem Ansprechen auf hormonelle Therapie [1, 2]. Eine vergleichbare Beziehung wurde im Fall des Prostatacarcinoms vermutet.

Cytotoxic activity of natural killer (NK) cells in peripheral blood was studied in 60 patients with renal cell carcinoma (RCC) by means of a 51Cr release assay against K 562 cells, homologous and autologous RCC. All patients were tested prior to surgery and thereafter followed up for 1 year. Immunostimulating agents as interferon, levamisole, OMPI, and various bacterial extracts were tested for their boosting capacity on NK cells. Patients with localized tumor showed increased activity in comparison to age-matched controls, whereas NK cell activity and interferon-boosted activity in patients with advanced disease and tumor spread was decreased. Augmentation of NK activity could only be achieved by interferon, while application of levamisole, OMPI, and bacterial extracts resulted rather in suppression of cytotoxic activity.

Six to 12 hr after IP injection of 400 mg/kg of D-galactosamine in rats a 5-fold increase in plasma insulin was observed. In addition, impaired glucose assimilation was present after an IV Load in spite of unchanged fasting glucose levels. In streptozotocin-diabetic rats (100 mg/kg IV) plasma insulin remained diminished 12 h after induction of D-galactosamine hepatitis. Under identical conditions of preparation and incubation, the liver plasma membranes of D-galactosamine-treated rats, in both normal and diabetic states, bound only 40--60% as much insulin per mg of membrane protein as those of the control rats. Scatchard analysis suggested that this was due to a decrease in the number of receptor sites in the membranes of the D-galactosamine-injected rats. No difference in the insulin degrading capacity and in insulin-receptor dissociation of the plasma membranes between control and D-galactosamine-treated groups was found. These data suggest that a reduction in the number of hepatic insulin receptors in galactosamine hepatitis can lead to insulin resistance and hyperinsulinaemia.

The concentration of substances with glucagon-like immunoreactivity (GLI) was determined in arterial blood plasma of rats up to 4 weeks after partial hepatectomy. 10 h following surgery a more than 18-fold increase in GLI + glucagon immunoreactivity could be observed. About 2/3 of the maximum activity seems to be of non pancreatic origin. The levels returned to normal about 22 h after surgery.

Die fur den Diabetes mellitus typischen Veranderungen im Kohlenhydrat-Stoffwechsel der Leber fuhren insgesamt zu einer vermehrten Glucoseneubildung, v. a. durch Steigerung der Gluconeogenese und der Glykogenolyse. Diese Stoffwechselveranderungen sind erklabar durch eine erhohte Konzentration des zyklischen AMP in der Leber, wie sie im Tierexperiment sowohl beim Alloxan- [7,10], als auch beim Streptozotocin-Diabetes [14] nachgewiesen wurde. Eine Erhohung des hepatischen zyklischen AMP kann durch vermehrte Neubildung — bedingt durch erhohte Aktivitat des Adenylatzyklasesystemsoder durch verminderten Abbau durch Hemmung der Phosphodiesterase erfolgen. Als Schlusselsubstanz des Intermediarstoffwechsels wird die intrazellulare Konzentration des zyklischen AMP zum einen akut reguliert uber die Aktivitatsanderung der membrangebundenen Adenylatzyklase in Abhangigkeit vom aktuellen Verhaltnis von Insulin zu Glucagon im Blut. Zum anderen mus jedoch auch eine langer anhaltende Anderung der Membranfunktion bei chronisch verandertem Verhaltnis von Insulin zu Glucagon, wie es z. B. beim Insulinmangeldiabetes der Fall ist, angenommen werden. Als Parameter dafur kann die Empfindlichkeit der Adenylatzyklase gegenuber stimulierenden Hormonen herangenommen werden. Das solche chronischen Veranderungen erfolgen, wurde bereits am Beispiel der Insulinrezeptoren gezeigt: diese sind beim Insulinmangeldiabetes sowohl in der Leber [2, 8], als auch im Fettgewebe oder in Monozyten vermehrt.

Patients with recurrent non-infectious calcium urolithiasis were classified metabolically (122 patients). When the magnesium excretion was measured in the metabolic subgroups, a subset of patients (21.6%) could be identified with marked hypomagnesuria as the only metabolic abnormality. A significantly reduced rate of magnesium excretion was found in these normocalciuric stone formers while assessing the overall 24-h urine magnesium excretion or the 24-h urine and fasting urine magnesium to calcium ratio. These differences were apparently not due to factors that might modify renal magnesium excretion, such as parathyroid function, hypercalcemia, hypophosphatemia, alimentary sodium load, age and sex.

Die Rolle der Prostata als Zielorgan fair Prolactin ist durch eine Reihe experimenteller Untersuchungen belegt [1]. Die Arbeiten von Grayhack und Farnsworth zeigten, das Prolactin und Androgene eine synergistische Wirkung auf das Wachstum der Prostata ausuben [2, 3]. Aufgrund dieser Untersuchungen stellte sich die Frage, ob erhohte Prolactinspiegel in der Pathogenese des Prostatacarcinoms eine Rolle spielen konnten. Wegen der bekannten, erheblichen Schwankungen der Prolactinausschuttung ist die Einzelbestimmung des basalen Prolactinspiegels mit Vorbehalt zu beurteilen [4, 5]. Unser Ziel war es deshalb, die Prolactinsecretion unter Stimulationsbedingungen zu prufen.

Up to 4 weeks after partial hepatectomy the concentrations of immunoreactive insulin and glucagon in the blood plasma of rats were determined. Furthermore we measured the activity of acid phosphatase in the serum, the concentration of cyclic AMP in liver cells, the activities of acid phosphatase in whole liver cells and in the cytosol of liver cells after partial hepatectomy. 2 h after partial hepatectomy there was a decline of the concentration of insulin in the plasma to about 13% of the initial value. 12 h after surgery a 5-fold increase of glucagon was found in the plasma. Shortly after this cyclic AMP reached its highest concentration. The activity of acid phosphatase in the whole liver cells and in the cytosol decreases slightly in the first 24 h after surgery whereas there is an increase of the activity of acid phosphatase in the serum.

Da sichi n verschiedenen Teilen der Welt eine gehaufte Inzidenz von Harnsteinkoliken im Sommer und Herbst gezeigt hat (1, 2, 9), wird eine Steinsaison mit erhohtem Steinbildungsrisiko postuliert.

Plasma concentrations of T4, FT4, T3, rT3 and TBG, as well as of TSH before and after stimulation with TRH were studied in 25 patients, who had been treated with amiodarone for up to nine years. At the beginning of therapy, all the parameters mentioned above were found to be in the normal range in all patients. After two months of therapy, T4 had increased from 100 nmol/l +/- 24 nmol/l to 155 nmol/l +/- 32 nmol/l (p less than 0.01), and FT4 from 22 pmol/l +/- 10.5 pmol/l to 32 pmol/l +/- 8 pmol/l (p less than 0.01). T3 had decreased to the lower normal range (n.s.). TBG showed no significant changes. The TRH-tests had been normal in the beginning, but they remained positive in only 20% of the cases. At the end of the study, rT3 exceeded the normal range in all 25 patients. Two patients developed definite hyperthyroidism with elevations of T3 up to 4.7 nmol/l and 7.5 nmol/l, respectively. In one of them, we decided to discontinue amiodarone. Testing of thyroid function under antithyroid drug therapy revealed a hyperfunctioning autonomous adenoma, which was successfully eliminated by radioactive iodine therapy. In the other patient, it was not possible to withdraw amiodarone, so we initiated long-term treatment with antithyroid drugs. Our data support the assumption that amiodarone causes an impairment of the peripheral conversion of T4 to T3. As a result, one finds elevated serum concentrations of T4, which, in combination with the mainly negative TRH-test, must not be interpreted as proof of a hyperthyroid metabolic state being present. Hyperthyroidism is confirmed only if serum concentrations of T3 are also elevated.

Thiazide werden bei der rezidivierenden Kalziumurolithiasis zur Senkung der Kalziumausscheidung eingesetzt. Ihre Wirksamkeit soil unabhangig von der Grose der Kalziumexkretion und der Art der Hyperkalzurie sein (1). Nůr gelegentlich wird in der Literatur uber unterschiedliche Wirkungen der Thiazide in den metabolischen Untergruppen der idiopathischen Kalziumurolithiasis berichtet (2,3,4,5).

A marked discrepancy between a high ovulation rate (70-80%) and a low pregnancy rate (30-35%) is generally observed during clomiphene therapy in female infertility. Disturbances in follicular maturation, inadequate secretory transformation of the endometrium and influence on the quality of cervical mucus are discussed as cause for this phenomenon. The purpose of this study was to investigate whether changes in endocrine patterns might lead to these changes thus limiting the success of clomiphene therapy. In 7 patients gonadotropins, prolactin, ovarian and adrenal androgens, sexual hormone binding globuline (SHBG) and plasma levels of clomiphene were measured at short intervals during a stimulated cycle. The increase of testosterone and androstendione promoted by clomiphene during the follicular phase is in accordance with previous observations. The decline in SHBG due to an antioestrogenic effect on its hepatic production leads to an additional increase in free, biologically active androgens. These androgens interfere with follicular maturation, causing atresia and premature luteinisation. As a direct effect of clomiphene on the adrenal gland, we observed an elevation of DHEAS. The significance of this finding remains unclear. During the follicular phase prolactin remained suppressed inspite of supraphysiological oestrogen levels. In the late luteal phase the diminished antioestrogenic influence on lactotrophic cells causes a 80% rise in prolactin that in some cases may impair luteal function. The average plasma half-life of clomiphene was found to be 12 days. This is in accordance with the half-life of the closely related compound tamoxifen. Clomiphene, also present in the luteal phase of the cycle, may cause inadequate secretory transformation of the endometrium by blocking endometrial oestrogen receptors.(ABSTRACT TRUNCATED AT 250 WORDS)

Immunstimulantien wie Interferon (IFN), Levamisole, OMPI und bakterielle Extrakte, konnen NK-Zellen aktivieren. Mit Hilfe eines 51Cr-release-Testes wurde die zytotoxische Aktivitat von NK-Zellen bei Patienten mit Adenokarzinom der Niere getestet. Als Zielzellen wurden K-562-Lymphomazellen, homologe (Caki 1) und autologe Nierentumorzellen verwendet. Die NK-Aktivitat von nephrektomierten Patienten wurde unmittelbar vor und eine Woche nach der Operation getestet. Alle Patienten wurden 6-9 Monate spater erneut untersucht. Patienten mit oder ohne Metastasen zeigten eine deutlich verschiedene zytotoxische Aktivitat von nicht aktivierten und interferonaktivierten NK-Zellen. Levamisole, OMPI und bakterielle Extrakte wiesen keinen aktivierenden Effekt in vitro auf.

1959 berichteten Lamberg und Kuhlback erstmals uber die hypocalciurische Wirkung der Thiaziddiuretika [1]. Diese Beobachtung wurde 1966 von Yendt in die Behandlung der idiopathischen Hypercalciurie und rezidivierenden Urolithiasis eingefuhrt [2]. 1975 schlug Pak den heute weitverbreiteten oralen Calciumbelastungstest zur Klassifizierung der idiopathischen Hypercalciurie vor [3].
