Over the past Ten years more than 500 Lumbar discectomies have been performed by the senior author through all standard methods, but only ten patients underwent Transdural disc excision. Seven males and three females in age group of 18 - 58 yrs were treated by this method. Six patients had PIVD at L4-L5 level, 3 at L5-S1 and one patient had PIVD at two levels L2-L3 and L3-L4. Results were evaluated by postoperative Frankel's grading, relief from pain, sensory improvement, return of bowel and bladder functions and complications. We achieved excellent results as for as relief of sciatica was concerned. Full recovery in motor and sensory symptoms was observed in 80% (n = 8) cases. One patient of cauda-equina syndrome did not recover and one patient developed headache due to CSF leak. Spontaneous regression of a herniated nucleus pulposus (HNP) occurs where intervertebral disc herniation loses its volume partly or totally without surgical interventions. In 1985, Teplick and Haskin first documented the spontaneous resolution of lumbar disc by computerized tomography and they reported that 9 out of their 55 patients harboring lumbar disc herniation showed spontaneous regression. Aims: Lumbar disc prolapse in elderly patients at L4-L5 level presenting as bilateral foot drop without other features of cauda equine syndrome has been rarely reported. Methods: This is a report of an elderly lady with presenting with sudden backache and radiating pain to both legs with bilateral foot drop. There was no urinary or bowel involvement. Clinical examination showed a bilateral L5 radiculopathy with normal perianal sensation. Investigations excluded other causes of bilateral foot drop. MRI revealed a L4-L5 central disc prolapse causing severe canal stenosis at that level. Results: Patient underwent L4-L5 laminectomy and discectomy. At follow up a month later her bilateral foot drop had improved to normal and she was independent. Conclusion: This case highlights the fact that though very rare, L4-L5 disc prolapse can manifest with bilateral foot drop and surgery can produce a good result. A 26 year old, male professional, right-handed sustained an injury after a fall during a football game. He had pain on neck, right shoulder, right scapula medial side and right arm dorsal-lateral side. The space C6-C7, right 7th nerve root is affected according to patient complains and physical examination. Plain cervical radiographs and MRI of cervical spine confirmed the diagnosis. After underwent conservative treatment for six weeks the patient is back to his normal duties.
