ZusammenfassungDie saisonale Influenza verursacht eine signifikante Krankheitslast in der deutschen Bevölkerung und geht mit hohen volkswirtschaftlichen Kosten einher. Personen ab 60 Jahren sind aufgrund von Immunseneszenz und chronischen Erkrankungen besonders gefährdet und machen einen Großteil der influenzaassoziierten Hospitalisierungen und Todesfälle aus. Adjuvantierter, Hochdosis-, rekombinanter und zellbasierter Influenzaimpfstoff wurden entwickelt, um die Effektivität im Vergleich zu herkömmlichen Impfstoffen zu verbessern. Neueste Beobachtungsstudien zeigen eine bessere Effektivität des adjuvantierten Impfstoffs gegenüber herkömmlichen Impfstoffen und eine ähnliche Effektivität wie der Hochdosis-Impfstoff bei älteren Erwachsenen. Einige Länder berücksichtigten die neue Evidenz bereits in ihren Impfempfehlungen für die aktuelle oder frühere Saisons. Auch in Deutschland sollte die Verfügbarkeit der Impfstoffe für ältere Erwachsene sichergestellt sein, um einen hohen Impfschutz zu garantieren.

Background HPV vaccination has been recommended and reimbursed for girls in Germany since 2007. In June 2018 the German Standing Committee on Vaccination (STIKO) recommended the gender-neutral vaccination of adolescents aged 9 to 14 years with catch-up through age 17. Objectives of this study were to describe the uptake of vaccination in boys before and during the COVID-19 pandemic. Methods The study used data from a proprietary electronic medical record database and a database with information on nationally dispensed vaccine doses. The monthly number of first doses of HPV vaccinations in boys and girls aged 9–17 years in the period from 01/2018 to 12/2021 was determined. In addition, for boys the cumulative vaccination rates were calculated for initiated and completed vaccination series. Results Four months after the introduction of mandatory reimbursement for boys, the monthly numbers of first doses were comparable to that of girls. Compared to the same month in 2019, the number of first doses declined by up to 49% (girls) in 2020 and 71% (boys) in 2021. At the end of 2021, the vaccination rate for 15-year-old boys (2006 birth cohort) reached 44.4% for initiated and 26.4% for completed series. Conclusion After an initial dynamic increase in HPV vaccinations in boys, the impact of COVID-19 was particularly strong in the second year of the pandemic. At the end of 2021 vaccination rates were still low. Efforts are needed to catch-up on adolescents that missed doses during the pandemic and to increase uptake.

Background A central role in the care of patients with osteoarthritis has the family practice. The aim of this analysis is to assess the association between the quality of family practice care from the osteoarthritis patients' perspectives and their health-related quality of life, disease-specific symptoms, and physical function. Method 111 patients suffering from cox- and/or gonosteoarthritis at the age of 65 or older had been recruited from family practices in Saxony and Bavaria. The participants received a paper-based questionnaire that assessed their sociodemographic data, the patients' level of depression (PHQ-9), physical activity, health-related quality of life (EQ-5D with EQ-VAS), as well as the quality of care in family practice from the patients' perspectives (PACIC short form). Furthermore, disease-specific symptoms (pain, stiffness) and physical function of patients with osteoarthritis were investigated (WOMAC). The analysis was performed with multiple linear regression models. Results A greater extent of depression was associated with stronger pain (beta=0,378; 95% CI [0,180; 0,576]), more stiffness of the joints (beta=0,223; 95% CI [0,135; 0,310]), and worse physical function (beta=1,628; 95% CI [0,908; 2,348]). The analysis also showed that older (beta=-0,901; 95% CI [-1,705; -0,097]), more depressive (beta=-1,654; 95% CI [-2,820; -0,488]) osteoarthritis patients were associated with a lower health-related quality of life. A statistically significant association between the quality of family practice care from the patients' perspectives and their health-related quality of life, disease-specific symptoms, and physical function could not be detected. Conclusions Previous literature provides evidence for an association between the quality of care of patients with a chronic illness and their quality of life or other health outcomes. The results of this research work did not align with these findings. Since depression was associated with decreased health-related quality of life and more severe complaints of patients suffering from osteoarthritis, potential depressive comorbidity should not be disregarded in the health care of this patient group and should be treated if necessary.

During the preparation period for the examination to become a specialist in general medicine, physicians in advanced training are often left alone. Since 2016, "Allgemeinmedizin Kompakt" has taken on the task of imparting exam-relevant knowledge to young family doctors. The course concept is characterized by independent pharma-free knowledge transfer and is tailored both to doctors in advanced training for exam preparation and to experienced doctors as a repetitorium. In order to increase the level of awareness and to give the course an identity-forming feature, a word/image brand was developed. This is intended to integrate the goals and features as well as create a recognition value. Overall, according to current evaluation, the course concept is appreciated by physicians in advanced training for exam preparation, as the degree of recommendation is 97%. But also experienced physicians are to be addressed by the DMP certifications, so that the course concept is in constant development.In der Vorbereitungszeit auf die Prüfung zum Facharzt für Allgemeinmedizin sind Ärzte in Weiterbildung oft allein gelassen. Seit dem Jahr 2016 stellt sich die Veranstaltungsreihe "Allgemeinmedizin Kompakt" der Aufgabe, jungen Allgemeinmedizinerinnen und -medizinern prüfungsrelevantes Wissen zu vermitteln.Das Kurskonzept zeichnet sich durch eine unabhängige pharmafreie Wissensvermittlung aus und ist sowohl auf Ärzte in Weiterbildung zur Prüfungsvorbereitung als auch auf erfahrenere Ärzte als Repetitorium zugeschnitten. Um den Bekanntheitsgrad zu erhöhen und dem Kurs ein identitätsstiftendes Merkmal zu verleihen, wurde eine Wort-Bild-Marke entwickelt. Diese soll sowohl die Ziele und Merkmale integrieren als auch einen Wiedererkennungswert schaffen. Insgesamt wird das Kurskonzept gemäß aktueller Evaluation bei Ärzten in Weiterbildung zur Prüfungsvorbereitung sehr geschätzt, der Weiterempfehlungsgrad liegt bei 97%. Currently, due to medical advances, more than 500 000 patients with congenital heart defects (CHD) are living in Germany, including more than 330 000 adults. In CHD, the aspect of transition from childhood and adolescence to adulthood is becoming increasingly important to ensure long-term success of the provided treatments. This is so important because despite primarily successful treatments, residua and sequels as well as (non-) cardiac comorbidities may influence the chronic course of the disease and lead to increased morbidity and mortality. Adults with congenital heart defects in Germany are insufficiently managed by existing specialized and accredited care structures, despite the great need for follow-up, and their care is largely provided by primary care physicians (general practitioners and internists) and non-specialized cardiologists.Optimized, future-oriented care requires regular follow-up and the possibility of interdisciplinary, integrated medical care of CHD in the long-term course.

BACKGROUND: The significant annual burden caused by seasonal influenza has led to global calls for increased influenza vaccination coverage rates (VCRs). We aimed to estimate the proportion of the German population at high risk of serious illness from influenza due to chronic conditions and to estimate age-specific VCRs of people with/without chronic conditions.; METHODS: Using health insurance claims data covering nine influenza seasons (2010-2019), we assessed up to 7 million insured individuals per season across all German regions. Individuals were classified according to age and presence of chronic health conditions. VCRs were estimated using outpatient healthcare utilization documentation.; RESULTS: In the 2018-2019 influenza season, 47.3% of individuals had ≥1 chronic condition. Most common were circulatory disorders, accounting for more than a third of individuals with ≥1 condition. Prevalence of chronic diseases, and therefore the proportion of high-risk individuals, increased slightly over time across most age groups. A downward trend in influenza VCRs was observed in all age groups until the 2017-2018 season, followed by a noticeable increase in the 2018-2019 season. Highest VCRs occurred among individuals of ≥60years, with a 38.5% VCR for this age group in the 2018-2019 season. Several factors, including age, chronic condition type, and geographical location, affected VCRs.; CONCLUSIONS: Influenza VCRs in individuals at high risk of severe complications from influenza infection are insufficient. Our results suggest that intensified public health efforts are necessary to reach the World Health Organization vaccination coverage target of 75%. © 2022 The Authors. Context Primary aldosteronism (PA) represents the most frequent cause of endocrine arterial hypertension. PA is also common in patients with mild forms of hypertension and normokalemia. Objective To identify the prevalence of PA in newly diagnosed hypertensive patients in primary care in Southern Germany. Patients and methods Newly diagnosed hypertensive patients in 27 primary care centers in Munich agreed to participate in the study. Patients were screened for PA using the aldosterone-to-renin ratio (ARR). In case of elevated ARR, confirmation testing was performed. After the diagnosis of PA, subtype differentiation and subsequent therapy of PA were initiated. Results A total of 235 patients with newly discovered arterial hypertension were initially screened for PA. Among these, 35 were excluded because the medication indicated pre-existing treated arterial hypertension or they were on interfering antihypertensive medication. At the first screening, 2.0% of the patients had hypokalemia. Of the 200 patients with newly discovered arterial hypertension, 42 had an elevated ARR. The incidence of the presence of hypokalemia did not differ according to normal or pathological ARR. Nine patients (21%) did not show up for further testing and were lost to follow-up, and 33 patients underwent a saline infusion test. Of these, 11 patients were diagnosed with PA, leading to at least 5.5% prevalence of PA in the collective. None of the diagnosed PA patients was hypokalemic at screening. Conclusion A 5.5% prevalence of PA was observed in our data of untreated newly diagnosed patients with hypertension.

BACKGROUND The treatment of elderly patients with dizziness/vertigo/balance disorders (VDB) can be challenging for their general practitioner. Patient-centered care with a focus on self-management support could be a possible approach. Primary objective of this study was to investigate the correlations between quality of primary care and health-related quality of life (HRQOL) from the patient's perspective. METHOD Data had been collected in the area of Munich and in the area of Dresden between 2017 and 2019. Questionnaires of n = 157 elderly patients from primary care practices in Munich and Dresden were evaluated. Multiple linear regression was used to analyse quality of care, physical activity, depression, and their correlations with HRQOL. RESULTS In this cohort of patients aged 65 to 94 no significant correlation between quality of care and HRQOL could be detected. Depression correlated negatively with HRQOL, whereas physical activity showed a positive correlation with HRQOL. CONCLUSIONS Physical activity can be beneficial for elderly dizzy patients' HRQOL. Additionally, a potential depression should be taken into account when treating patients with dizziness to improve their HRQOL. The results can be well transferred to elderly patients with VDB. Analysing more patients however, could increase the significance of the results. ZUSAMMENFASSUNG Hintergrund: Die Behandlung von alteren Patienten mit Schwindelbeschwerden kann eine Herausforderung fur den Hausarzt darstellen. Ein moglicher Ansatz ist eine patientenzentrierte Versorgung mit Fokus auf der Unterstutzung des Selbstmanagements. Die Untersuchung des Zusammenhangs zwischen der Versorgungsqualitat durch den Hausarzt und der gesundheitsbezogenen Lebensqualitat (HRQOL) aus der Patientenperspektive war primares Ziel dieser Analyse. Methode: Die Datenerhebung fand zwischen 2017 und 2019 im Raum Munchen und im Raum Dresden statt. Insgesamt wurden Fragebogen von n = 157 Patienten mit Schwindel aus der hausarztlichen Versorgung analysiert. Die Versorgungsqualitat, korperliche Aktivitat, Depressivitat und deren Zusammenhang mit der HRQOL wurden mithilfe multipler linearer Regressionsmodelle im Quer- und Langsschnitt (nach 6 Monaten) betrachtet. Ergebnisse: In der beobachteten Kohorte von Patienten zwischen 65-94 Jahren ergaben sich keine signifikanten Zusammenhange zwischen der Versorgungsqualitat und der HRQOL. Es zeigte sich ein negativer Zusammenhang zwischen Depressivitat und HRQOL und ein positiver Zusammenhang zwischen korperlicher Aktivitat und HRQOL. Schlussfolgerungen: Fur die HRQOL alterer Patienten mit Schwindel kann korperliche Aktivitat von Vorteil sein. Auch sollte zur Verbesserung der HRQOL eine mogliche Depressivitat bei der Behandlung von Patienten mit Schwindel berucksichtigt werden. Die Ergebnisse lassen sich sehr gut auf altere hausarztliche Patienten mit Schwindel ubertragen. Die Untersuchung einer groseren Kohorte konnte die Aussagekraft der Ergebnisse noch erhohen. To identify correlates of quality of life (QoL) measured with the Quality of Life Scale (QOLS) in participants of a multidisciplinary day hospital treatment program for fibromyalgia (FM). In this cross-sectional, observational study, “real world” data from 480 FM patients including socio-demographics, pain variables and questionnaires such as the SF-36, Beck Depression Inventory (BDI), Multiphasic Pain Inventory (MPI), SCL-90-R and others were categorized according to the components (body structure and function, activities and participation, personal factors, environmental factors) of the International Classification of Functioning (ICF). For every ICF component, a linear regression analysis with QOLS as the dependent variable was computed. A final comprehensive model was calculated on the basis of the results of the five independent analyses. The following variables could be identified as main correlates for QoL in FM, explaining 56% of the variance of the QOLS (subscale/questionnaire and standardized beta in parenthesis): depression (− 0.22), pain-related interference with everyday life (− 0.19), general activity (0.13), general health perception (0.11), punishing response from others (− 0.11), work status (− 0.10), vitality (− 0.11) and cognitive difficulties (− 0.12). Pain intensity or frequency was not an independent correlate. More than 50% of QoL variance could be explained by distinct self-reported variables with neither pain intensity nor pain frequency playing a major role. Therefore, FM treatment should not primarily concentrate on pain but should address multiple factors within multidisciplinary therapy.

COVID-19 challenges GP practice and emergency rooms across Germany. In addition to hygiene, the correct assignment of patients to outpatient, inpatient or intensive care management is difficult. This article provides an overview of aspects of initial care, management and risk assessment in COVID-19 patients. The care of corona infected patients can be improved at the interface between outpatient and inpatient care. There can be no “business as usual” after the crisis! Age, male sex and overweight are among the most important risk factors for serious corona disease. Poor oxygen saturation ( 97 mg/l and/or IL-6 > 80 pg/ml) indicate a critical course and should be determined in symptomatic patients. Only through regular dialogue between hospital and practice can meaningful decisions be made to slowly move from individual cases to a basic care structure.

In einigen Fallen sollte der Patient jedoch an einen Teilgebietsarzt uberwiesen werden.

Die Ultraschalluntersuchung ist aus der Allgemeinarztlichen Praxis nicht mehr wegzudenken. Die meisten Hausarzte sind qualifiziert, solche Untersuchungen durchzufuhren. Dieser Beitrag widmet sich folgenden Fragen: Bei welchen Symptomen/Erkrankungen soll die Sonografie des Abdomens eingesetzt werden? Welche Erkrankungen konnen zweckmasigerweise und im Regelfall vom Allgemeinmediziner alleine behandelt werden? Wann ist die Uberweisung zu einem Spezialisten sinnvoll?

Zusammenfassung Ziel der Studie Der Anteil an erwachsenen Bluthochdruckpatienten wurde 2010 weltweit auf 31,1% geschatzt. Ziel dieser Studie war es die Effekte von Lebensstilanderungen bei Patienten mit arterieller Hypertonie (AH) in der Primarversorgung zu untersuchen. Methodik Systematische Literaturrecherche in den Online-Datenbanken PubMed, Embase, Cochrane und Opengrey. Es wurden nur randomisierte kontrollierte Studien der Jahre 2005-2017 in deutscher oder englischer Sprache berucksichtigt. Ergebnisse Von 458 identifizierten Texten, wurden 11 Studien ausgewertet. Die untersuchten Patientengruppen waren sehr heterogen und unterzogen sich verschiedenen Interventionstypen. Besonders haufig waren Schulungen zum Krankheitsbild, regelmasige Selbstmessungen des Blutdrucks oder Patientenerinnerungen zur Einhaltung eines gesunden Lebensstils angewandt worden. Schlussfolgerung Es bedarf weiterer Studien, welche gezielt die hausarztliche Versorgung betrachten. Viele Lebensstilinterventionen scheinen aber sehr gute Effekte bei Patienten mit bereits bestehender AH zu zeigen (Sekundarprophylaxe), so dass diese Masnahmen die Basis einer antihypertensiven Therapie bei allen Patienten mit AH bilden sollten. Zudem ist es wichtig die Beratung zu einem gesunden Lebensstil wahrend der medikamentosen Therapie aufrecht zu erhalten.

Ziel der Forschungsarbeit ist, den Zusammenhang der hausärztlichen Versorgungsqualität aus Perspektive der Patientinnen und Patienten mit deren gesundheitsbezogener Lebensqualität, arthrosespezifischen Symptomen und körperlicher Funktionsfähigkeit zu untersuchen.Methode: Rekrutiert wurden 111 Cox- und/oder Gonarthrosepatientinnen und -patienten ab 65 Jahren aus Hausarztpraxen in Sachsen und Bayern. Die Studienteilnehmer erhielten einen papierbasierten Fragebogen. Dieser untersuchte die soziodemografischen Daten, Depressivität (PHQ-9), körperliche Aktivität und gesundheitsbezogene Lebensqualität (EQ-5D mit EQ-VAS) sowie die von den Patientinnen und Patienten subjektiv bewertete hausärztliche Versorgungsqualität (PACIC short form). Je älter (beta = -0,901; 95%-KI [-1,705; -0,097]) und je depressiver (beta = -1,654; 95%-KI [-2,820; -0,488]) der Arthrosepatient war, desto niedriger war dessen gesundheitsbezogene Lebensqualität. Es konnte kein signifikanter Zusammenhang zwischen der hausärztlichen Versorgungsqualität aus Perspektive der Patienten und deren Lebensqualität, Symptomen oder körperlicher Funktionsfähigkeit festgestellt werden.Schlussfolgerungen: In der Literatur konnte mehrfach ein Zusammenhang zwischen der Qualität der Versorgung von chronisch Kranken und deren Lebensqualität sowie anderen gesundheitlichen Aspekten festgestellt werden. The number of adults with congenital heart disease (ACHD) is steadily increasing. The present cross-sectional study was conceived to investigate ACHD care from the perspective of patients and family practitioners (specialists for general medicine and internal medicine, general practitioners).Questionnaire-based cross-sectional study to analyze the real care situation of ACHD in Germany from the perspective of patients and primary care physicians (PCP).The questionnaire was completed by 4493 ACHD (53.7% female; 41.3 ± 16.9 years) and 1055 PCP. The majority of ACHD (79.8%) visited their PCP for noncardiac health problems but also for cardiac problems. Almost all ACHD had substantial needs for medical consultation (performance, employment etc.). Of the patients 2014 (44.8%) did not know of any certified ACHD specialists or specialized centers and 2816 (62.7%) respondents were not aware of any ACHD patient organization. Of the PCPs 87.5% had cared for ACHD of all severities due to defect-typical residual and resulting symptoms. Many were not aware of any certified ACHD specialists. Only 28.5% consulted an ACHD specialist. Only 23.5% were aware of ACHD patient organizations.General practitioners are a mainstay of ACHD care in Germany. The present study shows that ACHD and their general practitioners are largely uninformed about the specialized care structures available nationwide, despite the high level of need for specialist care. In order to keep the morbidity and mortality of affected patients low, solutions must be elaborated in future to involve and integrate primary care physicians more intensively into the already existing dedicated ACHD care structures, in cooperation with specialized pediatric cardiologists, cardiologists and centers.HINTERGRUND: Die Zahl der Erwachsenen mit angeborenen Herzfehlern (EMAH) steigt stetig. Die Mehrheit der EMAH (79,8 %) besuchten ihren Hausarzt wegen nichtkardialer Gesundheitsprobleme, aber auch wegen kardialer Probleme. Fast alle EMAH hatten erheblichen medizinischen Beratungsbedarf (Leistungsfähigkeit, Berufsfindung etc.). 2014 Patienten (44,8 %) kannten keine zertifizierten EMAH-Spezialisten oder -Zentren. 2816 Befragte (62,7 %) waren nicht über EMAH-Patientenorganisationen informiert. 87,5 % der Hausärzte hatten EMAH aller Schweregrade wegen vitientypischer Rest- und Folgeerscheinungen betreut. Viele kannten keine zertifizierten EMAH-Spezialisten. Nur 28,5 % zogen einen EMAH-Spezialisten konsiliarisch hinzu. Nur 23,5 % kannten EMAH-Patientenorganisationen.Hausärzte sind eine tragende Säule der EMAH-Versorgung in Deutschland. Die vorliegende Studie zeigt, dass EMAH und ihre Hausärzte, trotz des großen Bedarfs an spezialisierter Betreuung, weitgehend uninformiert sind über die bundesweit vorhandenen EMAH-Versorgungsstrukturen. Um die Morbidität und Mortalität der Betroffenen niedrig zu halten, müssen Lösungen entwickelt werden, um in Kooperation mit den auf EMAH spezialisierten Kinderkardiologen, Kardiologen und Zentren die Hausärzte stärker in die vorhandenen EMAH-Versorgungsstrukturen einzubinden bzw. zu integrieren.

Currently, due to medical advances, more than 500 000 patients with congenital heart defects (CHD) are living in Germany, including more than 330 000 adults. In CHD, the aspect of transition from childhood and adolescence to adulthood is becoming increasingly important to ensure long-term success of the provided treatments. This is so important because despite primarily successful treatments, residua and sequels as well as (non-) cardiac comorbidities may influence the chronic course of the disease and lead to increased morbidity and mortality. Adults with congenital heart defects in Germany are insufficiently managed by existing specialized and accredited care structures, despite the great need for follow-up, and their care is largely provided by primary care physicians (general practitioners and internists) and non-specialized cardiologists.Optimized, future-oriented care requires regular follow-up and the possibility of interdisciplinary, integrated medical care of CHD in the long-term course.

Fur ausreichenden Impfschutz von Jung und Alt zu sorgen, ist eine zentrale Aufgabe des Hausarztes. Doch in der Praxis gibt es Schwachstellen, z. B. wenn Jugendliche gesund sind und nicht in die Praxis kommen. Dann werden Impftermine leicht mal vergessen.

In ihren Empfehlungen geht die Standige Impfkommission (STIKO) im Jahr 2018 auf viele praxisrelevante Ablaufe ein, so auch auf das richtige Impfmanagement. Dieses orientiert sich an den etablierten Standards des Qualitatsmanagements nach DIN EN ISO 9000 ff. und kann durch die Implementierung einer Impfsoftware sinnvoll unterstutzt werden. Im vorliegenden Beitrag sollen die Wichtigkeit und Durchfuhrbarkeit des standardisierten Impfmanagements in der taglichen Praxis niedergelassener Arztinnen und Arzte veranschaulicht werden. Dazu erfolgte eine selektive Literaturrecherche, in die verschiedene offentliche Empfehlungen, klinische Studien, Beobachtungsstudien und systematische Reviews einbezogen wurden. Es werden im Wesentlichen vier Bereiche unterschieden, in denen das Impfmanagement ansetzt: Patient, Impfstoff, Personal und Lagerung. Mehrere Arbeiten zeigen, dass in Deutschland eine Steigerung der Impfquoten moglich ist, wenn das Praxispersonal bzw. die Patientinnen und Patienten durch eine Software an fehlende Impfungen erinnert werden. Zudem kann eine entsprechende Software auch bei der Dokumentation des Lagerbestands, regelmasigen Nachbestellungen oder Abrechnungsmodalitaten eine grose Unterstutzung darstellen. Obwohl viele niedergelassene Arztinnen und Arzte einen qualitativ hochwertigen Impfprozess anstreben, wird die Bedeutung des Impfmanagements in Deutschland eher unterschatzt und eine Impfsoftware relativ selten eingesetzt. Um den taglichen Arbeitsablauf in der Arztpraxis zu verbessern und langfristig eine Anhebung der Impfquoten in Deutschland zu erreichen, sollte das Impfmanagement gefordert werden. Besonders durch den Einsatz einer passenden Impfsoftware konnten Verbesserungen erreicht werden.

COVID-19 continues to pose major challenges for GP practice and emergency rooms across Germany. Even if there is now a certain routine, the optimal treatment of patients is still difficult. This article provides an overview of the aspects of caring for COVID-19 patients in GP practice and emergency rooms and the changes since the beginning of the pandemic.

Background The number of adults with congenital heart disease (ACHD) is steadily rising due to increased survival rate and improved medical resources. Accordingly, more than 330,000 ACHD are currently living in Germany. Almost all of them require lifelong specialized medical follow-up for their chronic heart disease, often accompanied by residua, sequelae, or comorbidities. Primary care physicians (PCPs) are a crucial factor in directing patients to ACHD specialists or specialized institutions, but despite all efforts, the number of ACHD under specialized care is low, the lost-to-follow-up rate is high, and the morbidity and mortality is substantial. The present cross-sectional study was designed to systematically characterize the health care of ACHD from a patient perspective, including (I) use of medical care by ACHD, (II) medical counselling needs, and (III) perceived satisfaction with health care. Methods The German-wide analysis was based on a 25-item questionnaire designed to address different aspects of medical status and health issues of ACHD from their own perspective, performed between May of 2017 and July of 2020. Results A total of 4,008 ACHD (52% female; mean age 41.9±17.2 years) completed the questionnaire. The majority of ACHD (3,524, 87.9%) reported, that they consulted their PCP for non-cardiac health problems, and 49.7% (n=1,991) consulted their PCP also for medical problems associated with the underlying CHD. Almost all ACHD reported a need for medical advice concerning exercise capacity and daily life activities, occupational skills, pregnancy, rehabilitation, genetic counselling, insurance, and retirement. A total of 1,840 (45.9%) patients were not aware of the existence of certified ACHD specialists or centers. Moreover, 2,552 (67.6%) of those surveyed were uninformed about patient organizations for ACHD. Conclusions The present study demonstrates that ACHD are largely uninformed about the ACHD care structures available nationwide, although the patients have a great need for specialized follow-up, advice, and care.

The treatment of elderly patients with dizziness/vertigo/balance disorders (VDB) can be challenging for their general practitioner. Patient-centered care with a focus on self-management support could be a possible approach. Primary objective of this study was to investigate the correlations between quality of primary care and health-related quality of life (HRQOL) from the patient's perspective.Data had been collected in the area of Munich and in the area of Dresden between 2017 and 2019. Questionnaires of n = 157 elderly patients from primary care practices in Munich and Dresden were evaluated. Multiple linear regression was used to analyse quality of care, physical activity, depression, and their correlations with HRQOL.In this cohort of patients aged 65 to 94 no significant correlation between quality of care and HRQOL could be detected. Depression correlated negatively with HRQOL, whereas physical activity showed a positive correlation with HRQOL.Physical activity can be beneficial for elderly dizzy patients' HRQOL. Additionally, a potential depression should be taken into account when treating patients with dizziness to improve their HRQOL. The results can be well transferred to elderly patients with VDB. Analysing more patients however, could increase the significance of the results.Hintergrund: Die Behandlung von älteren Patienten mit Schwindelbeschwerden kann eine Herausforderung für den Hausarzt darstellen. Ein möglicher Ansatz ist eine patientenzentrierte Versorgung mit Fokus auf der Unterstützung des Selbstmanagements. Die Untersuchung des Zusammenhangs zwischen der Versorgungsqualität durch den Hausarzt und der gesundheitsbezogenen Lebensqualität (HRQOL) aus der Patientenperspektive war primäres Ziel dieser Analyse. Methode: Die Datenerhebung fand zwischen 2017 und 2019 im Raum München und im Raum Dresden statt. Insgesamt wurden Fragebögen von n = 157 Patienten mit Schwindel aus der hausärztlichen Versorgung analysiert. Die Versorgungsqualität, körperliche Aktivität, Depressivität und deren Zusammenhang mit der HRQOL wurden mithilfe multipler linearer Regressionsmodelle im Quer- und Längsschnitt (nach 6 Monaten) betrachtet. Ergebnisse: In der beobachteten Kohorte von Patienten zwischen 65-94 Jahren ergaben sich keine signifikanten Zusammenhänge zwischen der Versorgungsqualität und der HRQOL. Es zeigte sich ein negativer Zusammenhang zwischen Depressivität und HRQOL und ein positiver Zusammenhang zwischen körperlicher Aktivität und HRQOL. Schlussfolgerungen: Für die HRQOL älterer Patienten mit Schwindel kann körperliche Aktivität von Vorteil sein. Auch sollte zur Verbesserung der HRQOL eine mögliche Depressivität bei der Behandlung von Patienten mit Schwindel berücksichtigt werden. Die Ergebnisse lassen sich sehr gut auf ältere hausärztliche Patienten mit Schwindel übertragen. Die Untersuchung einer größeren Kohorte könnte die Aussagekraft der Ergebnisse noch erhöhen. Zusammenfassung Hintergrund Die aktuelle SARS-CoV-2-Pandemie erfordert hohe Durchimpfungsraten von chronisch Kranken gegen Influenza, um das Gesundheitssystem nicht zusatzlich zu belasten. Trotz klarer Evidenz fur Sicherheit und Wirksamkeit der Influenza-Impfung sind die Impfquoten in den vergangenen Jahren international auf insuffizientem Niveau verblieben. Der Hausarzt hat eine zentrale Bedeutung fur die Versorgung dieser Population. Ziel dieser systematischen Ubersichtsarbeit war daher die Evaluation verschiedener Masnahmen in der Allgemeinmedizin zur Steigerung der saisonalen Influenza-Impfquoten bei chronisch Kranken unter Berucksichtigung verschiedener internationaler Gesundheitssysteme. Methoden Eine systematische Literaturrecherche wurde in MEDLINE, CENTRAL, EMBASE und ERIC sowie manuell in Studienregistern und Literaturlisten durchgefuhrt. Dabei wurden ausschlieslich randomisierte kontrollierte Studien berucksichtigt. Die Methodik wurde im Vorfeld in einem Studienprotokoll festgelegt (PROSPERO CRD42018114163). Ergebnisse Insgesamt wurden 14 Studien im Rahmen eines nationalen Gesundheitsdienstes (Grosbritannien), einer staatlichen (Australien) und sozialen (Schweiz) Krankenversicherung und eines privaten Gesundheitssystems (USA) in unsere Ubersichtsarbeit eingeschlossen. Analoge Patientenerinnerungen und automatisierte Arzterinnerungen sowie Veranderungen der beruflichen Rollen fuhrten ausschlieslich im privaten Gesundheitssystem zu einem deutlichen Anstieg der Influenzaimpfquoten. Im nationalen Gesundheitsdienst konnte keine der analysierten Interventionen einen signifikanten Anstieg der Impfquoten erreichen, wobei im nationalen Gesundheitsdienst Grosbritanniens verhaltnismasig gute Basisimpfraten gegen Influenza bereits vor Durchfuhrung der Studien interventionsunabhangig vorlagen. Fortbildungsveranstaltungen fur Praxisteams und Erinnerungs-SMS zeigten in den sozialen und staatlichen Krankenversicherungssystemen der Schweiz und in Australien gute Resultate. Schlussfolgerungen In Deutschland konnten vor allem Fortbildungsveranstaltungen fur medizinische Teams und sowie zentral organisierte Einladungs- und Monitoringsysteme zur Verbesserung der Impfquoten bei chronisch Kranken geeignet sein. Eine staatliche Kostenubernahme der Impfkosten scheint in verschiedenen Gesundheitssystemen fur eine gute Basisimpfquote gegen Influenza bei Indikationspatienten zu sorgen.

Impfungen gehoren zu den wichtigsten vorbeugenden Masnahmen gegen Infektionserkrankungen. Hohe Durchimpfungsraten schutzen vor allem diejenigen in der Bevolkerung, die selbst nicht geimpft werden durfen. Unsere Vision ist es, dass jeder Absolvent des Medizinstudiums in Deutschland eine grundlegende Impfkompetenz mit dem entsprechenden Wissen und dazugehorigen praktischen Fertigkeiten aufweisen kann. Dazu gehort auch, wissenschaftliche Ergebnisse in einer fur Laien verstandlichen Form kommunizieren zu konnen, um im Sinne der partizipativen (geteilten) Entscheidungsfindung zu den relevanten Impfthemen beraten zu konnen. Deshalb war die Entwicklung und Implementierung eines innovativen, strukturierten und kompetenzbasierten Impfcurriculums an der Medizinischen Fakultat der Ludwig-Maximilians-Universitat (LMU) Munchen das Ziel unseres Multiplikatoren-Projektes.

BACKGROUND: Marfan syndrome (MFS) is a genetically determined multiorgan disease that leads to severe physiological and psychological impairments in adult life. Little consensus exists regarding quality of life (QOL) in individuals with MFS. The present study sought to investigate QOL in a representative cohort of adults with MFS. METHODS: Patient-reported outcome measures from a representative sample of 102 adults with MFS (39.3±13.1 years of age; 40.2% female) were retrospectively analyzed and compared with those from adults with different congenital heart defects (CHD), at the German Heart Center Munich. QOL was assessed using the updated five-level version of the EQ-5D. RESULTS: Differences between both populations were analyzed. Subjects affected by MFS reported an overall reduced QOL. COVID-19 challenges GP practice and emergency rooms across Germany. In addition to hygiene, the correct assignment of patients to outpatient, inpatient or intensive care management is difficult. This article provides an overview of aspects of initial care, management and risk assessment in COVID-19 patients. The care of corona infected patients can be improved at the interface between outpatient and inpatient care. There can be no "business as usual" after the crisis! Age, male sex and overweight are among the most important risk factors for serious corona disease. Poor oxygen saturation ( 88 %) and increased signs of inflammation (CRP 97 mg/l and/or IL-6 80 pg/ml) indicate a critical course and should be determined in symptomatic patients. Only through regular dialogue between hospital and practice can meaningful decisions be made to slowly move from individual cases to a basic care structure.
