- Abstract
The purpose of this work was to screen a set of isoquinoline alkaloids for their inhibitory potency toward CYP450 isoenzyme CYP2D6. Since the human cytochrome P450 plays a pivotal role in a metabolism of both xenobiotics and endogenous substances, studying its interactions is a basic task in early drug development. CYP2D6 is due to its high polymorphism monitored very often. The in vitro method with fluorometric detection was used to determine the inhibitory effect of 20 alkaloids towards the recombinant human CYP2D6. Isoquinoline alkaloids were divided into the groups based on their relative structures. It was determined that 11 out of 20 are potent inhibitors, 3 moderate, 5 weak and one is not inhibitor. In percentage representation it means that 70% of all measured compounds are at least moderate potent inhibitors to CYP2D6 in this assay. They create mainly the members of protoberberine alkaloid group. The structure of these compounds might contribute to the searching of new inhibitors for data-sets for in silico drug metabolism predicting methods.

- Abstract
Metoděj Jan Nepomuk Zavoral, Abbot from Strahov Metoděj Jan Nepomuk Zavoral is a eminent personality of Czech sacred history of the end of 19th and first half of the 20th centuries. He was born in Neveklov in 1862, in 1872- 1880 he studied at secondary school in Benešov and Prague, after graduation in 1880 he joined the Norbertines (Premonstratensians) and became a novice in the monastery in Prague- Strahov. A year later he took the first vows, starting to study theology, ordained in 1885. He became a priest first in Sepekov (1885-1888), then in Jihlava (1888-1905). In 1905 he was adjourned back to Strahov. In the same year, after death of abbot Zikmund Starý, Zavoral is elected an abbot, being of the age of 43 at that time. His main objective was to manage the regular community and operation of the monastery. At the first, rather peaceful stage (1906-1914) he was able to become deeply acquainted with his office. Owing to this, he successfully conducted the community a monastery through the difficult period of the 1st World War (1914-1918) as well as the period of establishment of the new Czechoslovak State after 1918. He was facing a number of problems related with the difficult position of the Catholic Church in the new republic, including of the period of "priest dissidence". Public activities...

- Abstract
8 ABSTRACT In the north-east part of the Třebíč district 108 water sites were repeatedly monitored to analyze breeding of amphibians where its 11 species were proven by this research. Bombina bombina, Bufo bufo, Bufo viridis, Hyla arborea, Pelobates fuscus were proven in line with previous studies. However, species Triturus cristatus, Triturus vulgaris, Rana temporaria, Rana dalmatina, Rana esculenta synklepton were monitored in the region for the first time ever. Distinctly high values of the species presence were reached by BufBuf (60,2 %), RanEscSyn (78,7 %) and HylArb (48,1 %). In contrast, species TriCri, TriVul, BufVir, RanArv, RanDal were found in the region with low frequency. There were found positive correlations in the proportion of littoral, natant and submerse vegetation and in the transparency of the water column and in contrast negative correlations were found in the extent of urban areas and with the number of amphibians breeding in the water site. The Redundancy analysis (RDA) helped to discover statistically significant influence of the environmental variables (proportion of littoral vegetation, transparency, water site total surface and lastly number of other water sites within the range of 500 metres) on the presence of amphibians on the water sites. Consequently, "ideal breeding sites"...

Morphological abnormalities occur frequently in wild amphibian populations. We analysed malformation and injuries in the hybrid zone of three crested newt species, in the Czech Republic. In total, 274 individuals from 35 localities in South Moravia (Czech Republic) were examined during the period 2010-2014. Malformations were found in eight newts (2.9%) from seven localities. Injuries were recorded on 59 newts (21.5%). Proportions of tail crest injuries was significantly higher (P < 0.1) in males than in females and the probability of being injured was significantly higher (P <0.01) for adult individuals. We discuss gene mutation, parasitism and predation as possible explanations for our observations. [ABSTRACT FROM AUTHOR]

- Abstract
In this paper we present a novel approach for stable interactive cutting of deformable objects in virtual environments. Our method is based on the extended finite elements method, allowing for a modeling of discontinuities without remeshing. As no new elements are created, the impact on simulation performance is minimized. We also propose an appropriate mass lumping technique to guarantee for the stability of the simulation regardless of the position of the cut. [ABSTRACT FROM AUTHOR]

A general nurse may, without clinical supervision, administer medicinal products with the exception of intravenous injections (IV) or infusions in newborns and children under 3 years of age and with the exception of radiopharmaceuticals. This activity is regulated in Decree No. 55/2011 Coll. by Ministry of Health of the Czech Republic. The preparation and administration of the medicinal product is an integral part of the everyday practice of a l nurse; however, the dimensions of this procedure are very broad, and it does not start and end with the administration of the prescribed medicinal product. In the wider context, this is a multi-disciplinary process, in which other health professionals can participate with their well-founded roles. Determining individual roles and collaboration in the multi-disciplinary team composed of a doctor, a nurse and a clinical pharmacist has an effect on the correct pharmacological treatment of the patient. Of all professions, nurses spend time with patients. 40 % of their time administering medications; therefore, they play a key role in the reduction of medication errors. (Miller et all, 2016). The aim of the research is to analyze the current role of the nurse in the administration of medicinal products from the perspective of a clinical pharmacist. The research...

Introduction: A nurse preceptorship is essential in the transition period (TP) of novice nurses. Especially in intensive care workplaces, where training for highly specialized and demanding care is provided. TP thus becomes a sensitive period in which the employee decides whether to remain in the workplace or even in the profession. Methodology: This is qualitative study with the main focus of the survey on the specific experience of preceptors with TP management. Data collection was performed using semi-structured interviews with preceptor nurses in one selected workplace of intensive care. Data analysis was performed in 4 steps, open coding and sorting of data into categories and subcategories were used. Results: Six main topics were identified: Preceptors' experience from their own transition, TP organization attributes, Evaluation and feedback, The role of the preceptor nurse, Critical moments and Factors influencing the TP. The experience of trainers from their own TP is a significant predictor for their individual way of training and leading novice nurses. Evaluation and feedback have a culture and should be performed regularly in cycles to achieve the purpose. The preceptor has many roles, participates in the creation of social relationships with the novice, he himself strives to establish...

Introduction: The shortage of nurses is a global problem of a multifactorial nature, to which demographic change in the world's population contributes greatly. In the future, a growing imbalance in the supply and demand of nursing staff can be expected, and according to WHO estimates (2015), the deficit of nurses and midwives will reach 7.6 million in 2030. The lack of nurses is largely due to their turnover, which is influenced by a number of interrelated factors and has a negative impact on the quality and safety of health care, the economy, health systems and the nurses themselves. The first step in solving this issue is to examine the causes of institutional and professional turnover of nurses and then create stabilization measures. Aim: Identify, characterize and analyse the factors that influenced the decision to change the current job position of general nurses with more than two years of experience. Methodology: Qualitative research was carried out through individual semi-structured interviews, phenomenological approach and method of purposeful selection was methodologically chosen. The research survey was attended by 14 general nurses from Prague hospitals aged 30 to 52 years. Data collection was terminated by reaching theoretical saturation. Data analysis was performed using open coding...

Introduction: The topic of burnout syndrome is a considerably up-to date topic worldwide and not completely thoroughly researched. The aim of the diploma thesis is to learn whether a higher rate of burnout syndrome has an effect on sexuality in non-medical male health workers in urgent care workplaces in teaching facilities. The topic of the syndrome has fascinated me as a healthcare worker in urgent care. The information obtained from the research work will be a partof an international study investigating the impact of the burnout syndrome on human health. In the Czech Republic, this research is conducted under the authority of PhDr. Šárka Dynáková, Ph.D. and doc. PhDr. Jana Kožnara, CSc. Methodology: The research was carried out using a quantitative method using two anonymous questionnaires. The research sample consisted of non-medical male health workers at urgent care facilities in teaching hospitals. The first standardized questionnaire is focused on the burnout syndrome according to the Czech version of the Shirom-Melamed scale (Shirom, Melamed, 2006 In Ptáček, 2013), the second questionnaire is focused on the sexual functions of men according to Mellan in Kratochvíl, S. The rapid development of the intensive care medicine with a large amount of highly specific information requires sufficiently educated nurses in the field. A nurse anesthetist is a nurse working in intensive care with a focus on anesthesiology. She provides highly specialized nursing care to patients during the course of general or local anesthesia. The work is divided into two parts, a theoretical part and a practical part. The theoretical part focuses on the development of anesthesiology and intensive care medicine. It also focuses on the education of nurses. The education of nurses is summarised from historical beginnings to the present time. This part also includes new requirements for the education of non-medical medical staff and the current supporting legislation. Higher education for intensive care is also mentioned. The next chapter of the theoretical part focuses on the competencies of nurses in anesthesiology and intensive care medicine. The practical part includes interviews with six nurses. Three interviews were conducted with nurses that witnessed the beginnings of the field of anesthesiology as we know it nowadays. The method of qualitative research, study of documents, analysis of primary archival sources of the Ministry of Health, NLK, ÚVN, legislation, periodicals, professional...

This diploma thesis deals with current pharmacotherapeutic care with a focus on activities that nurses take part in. The aim was to collect and evaluate the views and experiences of selected nurses regarding their role in pharmacotherapy team, particularly what activities they perform in such team and how their pharmacotherapy teams work in general. SWOT analysis was then used firstly to evaluate the current role of nurses and secondly to suggest a way of implementation of possible extension of nurses' competencies in pharmacotherapy. In order to collect the required data, semi-structured interviews were conducted. These were recorded, so they could be easily processed afterwards. During transcription of the interviews into written text, all the collected data was briefly analysed. The data was further analysed using the open coding method, when three main categories with specific subcategories and subcodes were identified. All the selected men and women, which were asked for cooperation, are people working as a nurse without supervision and mentoring. Based on their career history all of them were expected to have a more specific experience with the issue. The respondents were mainly nurses with long-term working experience, often working in middle or senior management. When selecting the future...

Introduction to the issue: This paper examines the impact of burnout syndrome, which is common among caring professionals, on the sexuality of male paramedics. The term "burnout" was coined in the US 25 years ago. Burnout is defined as a mental state resulting from constant and long-term exposure to stress, especially in relation to psychosocial work factors (Melamed and Shirom, 2006). The effect of burnout syndrome on sexuality is a topic that has received very little attention. That is why I decided to cooperate with PhDr. Šárka Dynáková, Ph.D. and doc. PhDr. Jan Kožnar, CSc, who are working on an international study in the Czech Republic on the effects of burnout syndrome on human health. Methodology: For the theoretical part of this diploma thesis, use was made of monographs and databases. For the practical part, quantitative data was collected using standardized questionnaires distributed among male paramedics. The first questionnaire focused on burnout syndrome using a validated questionnaire for the Czech Republic, one that applies the Shirom-Melamed scale from 2006 (Ptáček, 2013). An extended Kroměříž version of the Sexual Functions of Men questionnaire was used for collecting the data on sexuality (Kratochvíl, 1999). (v AJ) I chose the topic for my bachelor thesis: Nursing care of a patient with lymphoma. I consider this topic as a very actual because the occurence of lymphoma is increasing (In: Linkos, 2019). Lymphomas refer to tumors which originate from lymphocytes. The theoretical part deals with the issue of lymphomas. I focus mainly on their diagnostics and treatment. And I deal with nursing care and the Lymphoma Help patient organization. Before I started to work on my bachelor thesis I had made a research in the National Medical Library. Furthermore, I used information from the databases: BMC, PubMed, EBSCOhost and Medline. I also used therapeutic recommendations - Cooperative lymphoma groups. Methology: I chose a case study of a patient with B-cell lymphoma from large cells, diffuse for the preparation of this thesis. I used Marjory Gordon's functional health model. Nursing standard procedures, potential risks of nursing care and their prevention are mentioned and described in a specific case. Objective:. I focus on general nursing care of a patient diagnosed with B-cell lymphoma from large cells, diffuse, who has undergone chemotherapy. Results: Nursing procedures are determined and their compliance is monitored. The information leaflet for patients is being created upon this study. Conclusion: New...

The fluctuation of nursing staff in the medical facilities is not just current problem in the Czech Republic. Aging population will present higher demand for number of nursing staff. It is assumed that the insufficient number of nursing staff will remain and even rise (also due to aging of the staff) if appropriate actions are not taken (Simoens et al., 2005). This thesis discusses the decision to migrate amongst the nursing staff - general nurses, practical nurses, midwives, and emergency paramedics within the medical facility. It analyses the reasons for changing their workplace. The investigation was executed using the questionnaire method. It was conducted in January and February of 2020 in the selected medical facility in the Czech Republic. Total of 701 questionnaires were distributed to all inpatient wards, outpatient departments and intensive care units. The investigation targeted nursing medical staff. A statistical analysis of the results was performed Mann-Whitney U test was used to monitor the differences for decision to migrate based on various characteristics and responses. Total of 415 completed questionnaires were analysed in this investigation. 61/415 (14.7 %) responders consider the change of workplace. Considering the rise of tourism and the number of immigrants in the CR, the increasing number of patients-foreigners can be expected. The mutual communication and the impact of ethno-cultural influence on nursing is becoming a subject of the global interest. Patients' life threatening, stress, and a necessity of prompt and swift decision-making both by patients and health care professionals play a role in the intensive care. The theoretical part of the theses explains the basic concepts associated with multicultural nursing, the legislative framework of the nursing process and the topic of communication with foreigners in an intensive care. The language barriers and ways of overcoming the barriers are defined in this part. Methodology: I chose the case study method for my bachelor theses. A case report of a French patient hospitalized in a critical condition in an intensive care unit was processed from the point of view of multicultural nursing. The basis of the work was the application of theoretical knowledge of the Transcultural Nursing Society and deep insight and understanding my own cultural values and...

This Bachelor thesis deals with problems of home care. At present, the whole population is aging in the Czech Republic, increasing the number of people with limited self-sufficiency. These individuals often seek help from home care services, so it is important that they know the opportunities that this service can provide. The main aim of the bachelor thesis is to find out the awareness among patients with limited self-sufficiency about the possibilities of home care. In line with the bachelor thesis topic and goal setting, I defined the following hypotheses: Hypothesis H1: I presume that more than 50 % of respondents evaluate their awareness about the possibilities of home care as insufficient. Hypothesis H2: I suppose that more than 50 % of respondents do not know who is competent to indicate home care. Hypothesis H3: I think that more than half of the respondents are not informed about the possibility of reimbursing some home care services by the health insurance company. For the bachelor thesis, the method of quantitative research was chosen. I obtained the required data using a non-standardized questionnaire of my own design. I filled out the questionnaire personally with each respondent. Respondents were patients from internal departments and hospices from unnamed Prague hospitals. All...

Juvenile idiopathic arthritis is one of the most common rheumatic diseases that affects children less than 16 years of age. The illness is considered irrecoverable because of its unclear aetiology. My work is focused on nursing care of a child with juvenile idiopathic arthritis due to the very low awarness of this disease. The main objective was to describe the nursing care of a child with a given disease and to increase awareness of juvenile idiopathic arthritis. Another objective was to create information material for the public about the illness. I choose a case study method to meet these goals. I described the nursing process of the patient with the disease in the practical part of the bachelor thesis, supplemented by Mary Henderson model. Information was drew mainly from medical records, interviews with parents, medical staff, and by providing nursing care to the chosen patient. The specifics of nursing care of a child with juvenile idiopathic arthritis were described on the basis of the information and knowledge gained during the hospitalization of the patient. These informations were used also for creation the information material for the general public. keywords: juvenile idiopathic arthritis, nursing care, rheumatology, paediatrics nursing, arthritis

This thesis deals with methods used for childhood obese patient education. It is further devoted to the creation of new educational resource to help fight the childhood obesity. This educational material has been prepared on a similar basis as the educational material for diabetic patients called Conversation maps TM. The theoretical part is dedicated to three main topics. First of all it analyzes risk factors influencing progression of the child obesity, prevention and treatment of this disease. Secondly, the theme of education is further elaborated. Finally, the third main topic is focused specifically on education in childhood obesitology and educational materials dealing with this issue. Quality of childhood obesity prevention and its specific impact on children aged 11-15 years is assessed in practical part. This part was realized using quantitative research, where I used an anonymous non-standardized questionnaire (which contained 14 questions) to gather the necessary data. The aim of the questionnaire was to find two facts. First, I was wondering where the children got accurate information about a healthy lifestyle. This finding has influenced the formation of educational material. Second, I was wondering what the real dietary habits of children aged 11-15 years are. The survey shows that...

The patient's autonomy is very important part of medical and nursing practice and every patient should have a right to express their opinion about their treatment. Advance directives give person an opportunity to express even in situations when it is not possible to communicate with others. Although it is possible to write advance directives in the Czech Republic only negligible percentage of people take advantage of this possibility. Why is it so? I have set two goals in my thesis. My first goal is to determine the awareness of the general public about the advance directives in the Czech Republic and also I want to find out awareness about the possibility to appoint a guardian as a mediator of fulfilment of advance directives of an individual. It is also my goal to discover whether respondents would welcome greater awareness of this topic. My second objective is to determine the attitudes of respondents to the problematics of advance directives. I want to discover if the respondents have ever dealt with the idea of being in a situation which they could not communicate with their environment and if they would like to have an opinion to decide about their treatment and also if they would like to name their mediator. This bachelor's thesis deals with comparison of auxiliary nursing professions in Slovakia and the Czech Republic. After the split of Czechoslovakia in 1993, there were a number of changes in health legislation. This work provides a comprehensive view of the development and current situation of health care assistants, auxiliary nurses, nurses and health care workers. The objectives of this work were to compare the development of individual professions, the concept of education, the possibilities of lifelong learning and professional association. The aim of the work was also to describe the actual performances, performed by auxiliary nursing staff and the legal requirements for the competencies of these professions. Through the form of a questionnaire survey, a survey was carried out between the auxiliary nursing staff in both countries, which showed that there was a significant overrun of competencies in both Slovak and Czech respondents. The provision of high-quality nursing care is currently highlighted. An important factor in providing quality and safe care is the proper division of tasks between nursing team members and the pursuit of a profession in accordance with legally defined competencies. keywords: Regulation of profession, health-care profession, nursing, health-care assistent, auxiliary...

Increasing age of primigravidae is discussed topic nowdays. It's a trend that also affects the Czech Republic - The Czech Statistical Office shows that the process of increasing age of primigravidae progresses since 1990 till now and that is why I consider it important to talk about this issue. The main objective of this work is to determine the causes of the increasing age of primigravidae in the Czech Republic. Sub-objectives are: to determine if socioeconomic factors are reasons for delaying childbirth, to determine whether delaying childbirth is the cause of the emancipation of women and whether the decision of delaying childbirth is affected by low knowledge of risks associated with late childbirths (for both mother and child). The work consists of two parts - theoretical and practical. The theoretical part consists of three parts and each of them is related to one objective. The first part is dedicated to changes during pregnancy and pregnancy of old primigravidae. - Abstract
Marginal populations of animals are highly susceptible to environmental pressures associated with climatic changes. Understanding their distribution and ecological requirements is, thus, essential for the development of efficient conservation strategies. The dice snake, Natrixtessellata , is listed as critically endangered in the Czech Republic. In certain regions (Bohemia and Silesia), its populations are located beyond the northern border of the continuous range of the species, while the south Moravian populations are connected to it. Based on the statewide database of the Czech Nature Conservation Agency, we created a predictive model and determined key factors influencing the species distribution. The most relevant factors were: watercourses and bodies, average annual temperatures, altitude, slope inclination and precipitation seasonality. The model fits the presence records well and is applicable in both theory and practice of the species conservation - for example, focusing faunistic research to certain areas, critical analysis of controversial presence reports and as an input for species management in the form of repatriation and introduction., Competing Interests: No conflict of interest to declare Disclaimer: This article is (co-)authored by any of the Editors-in-Chief, Managing Editors or their deputies in this journal., (Jan Chmelař, Petr Civiš, David Fischer, Daniel Frynta, Lenka Jeřábková, Veronika Rudolfová, Ivan Rehák.)
