Purpose: To evaluate the test–retest reliability of isokinetic leg-press power–force–velocity profile (P–F–v) parameters in male and female elite athletes. In addition, we determined the concurrent validity of leg-press against squat-jump (SJ) P–F–v parameters in task-experienced athletes. Methods: For test–retest reliability, 22 female and 23 male elite athletes (22.3 [4.1] y) with different sporting backgrounds conducted 3 isokinetic leg-press test sessions over 3 consecutive weeks. The testing consisted of bilateral leg extensions at isokinetic velocities of 0.1, 0.3, 0.7, and 1.2 m·s−1. For concurrent validity, 13 ski jumpers (20.3 [3.3] y) were recruited to perform the isokinetic leg-press and SJ P–F–v profile tests using 5 predefined loading conditions of 0%, +20%, +40%, +60%, and +80% of body mass. Results: Relative and absolute reliability were acceptable for female (intraclass correlation coefficient ≥.87 and coefficient of variation ≤6.5%) and male (intraclass correlation coefficient ≥.89 and coefficient of variation ≤5.7%) elite athletes. In contrast, concurrent validity was insufficient, with correlations ranging from −.26 to .69 between isokinetic and SJ P–F–v parameters. Conclusion: Irrespective of sex, isokinetic leg-press P–F–v profiles provide reliable parameters. However, leg-press P–F–v profiles do not serve as a valid substitute for SJ P–F–v profiles. P–F–v parameter magnitudes are likely dependent on the constraints of the tested movement and testing device.

Objective Realistic, safe, and efficient modalities for simulation-based training are highly warranted to enhance the quality of surgical education, and they should be incorporated in resident training. The aim of this study was to develop a patient-specific virtual cerebral aneurysm-clipping simulator with haptic force feedback and real-time deformation of the aneurysm and vessels. Methods A prototype simulator was developed from 2012 to 2016. Evaluation of virtual clipping by blood flow simulation was integrated in this software, and the prototype was evaluated by 18 neurosurgeons. In 4 patients with different medial cerebral artery aneurysms, virtual clipping was performed after real-life surgery, and surgical results were compared regarding clip application, surgical trajectory, and blood flow. Results After head positioning and craniotomy, bimanual virtual aneurysm clipping with an original forceps was performed. Blood flow simulation demonstrated residual aneurysm filling or branch stenosis. The simulator improved anatomic understanding for 89% of neurosurgeons. Simulation of head positioning and craniotomy was considered realistic by 89% and 94% of users, respectively. Most participants agreed that this simulator should be integrated into neurosurgical education (94%). Our illustrative cases demonstrated that virtual aneurysm surgery was possible using the same trajectory as in real-life cases. Both virtual clipping and blood flow simulation were realistic in broad-based but not calcified aneurysms. Virtual clipping of a calcified aneurysm could be performed using the same surgical trajectory, but not the same clip type. Conclusions We have successfully developed a virtual aneurysm-clipping simulator. Next, we will prospectively evaluate this device for surgical procedure planning and education.

The present study was conducted to assess test-retest reproducibility of explosive strength measurements during single-joint isometric knee extension using the IsoMed 2000 dynamometer. Thirty-one physically active male subjects (mean age: 23.7 years) were measured on two occasions separated by 48–72 h. The intraclass correlation coefficient (ICC 2,1) and the coefficient of variation (CV) were calculated for (i) maximum torque (MVC), (ii) the peak rate of torque development (RTDpeak) as well as for (iii) the average rate of torque development (RTD) and the impulse taken at several predefined time intervals (0–30 to 0–300 ms); thereby explosive strength variables were derived in two conceptually different versions: on the one hand from the MVC-trial (version I), on the other hand from the trial showing the RTDpeak (version II). High ICC-values (0.80–0.99) and acceptable CV-values (1.9–8.7%) could be found for MVC as well as for the RTD and the impulse taken at time intervals of ≥100 ms, regardless of whether version I or II was used. In contrast, measurements of the RTDpeak as well as the RTD and the impulse taken during the very early contraction phase (i.e. RTD/impulse0–30ms and RTD/impulse0–50ms) showed clearly weaker reproducibility results (ICC: 0.53–0.84; CV: 7.3–16.4%) and gave rise to considerable doubts as to clinical usefulness, especially when derived using version I. However, if there is a need to measure explosive strength for earlier time intervals in practice, it is, in view of stronger reproducibility results, recommended to concentrate on measures derived from version II, which is based on the RTDpeak-trial.

This study investigated the effect of a 12-week recreational skiing intervention on lower limb muscle strength and cardiorespiratory fitness in participants with unilateral total knee arthroplasty (TKA). Twenty-seven older adults (70 ± 5 years) were assigned to the intervention (n = 13) or control group (n = 14) after surgery (2.5 ± 1 years). Leg muscle strength was measured using an IsoMed 2000 dynamometer and cardiorespiratory fitness was determined by cycle ergometry before and after the intervention as well as after an 8-week retention period. Burn depth and burn size are crucial determinants for assessing patients suffering from burns. Therefore, a correct evaluation of these factors is optimal for adapting the appropriate treatment in modern burn care. Burn surface assessment is subject to considerable differences among clinicians. This work investigated the accuracy among experts based on conventional surface estimation methods (e.g. "Rule of Palm", "Rule of Nines" or "Lund-Browder Chart"). The estimation results were compared to a computer-based evaluation method. Survey data was collected during one national and one international burn conference. The poll confirmed deviations of burn depth/size estimates of up to 62% in relation to the mean value of all participants. In comparison to the computer-based method, overestimation of up to 161% was found. We suggest introducing improved methods for burn depth/size assessment in clinical routine in order to efficiently allocate and distribute the available resources for practicing burn care.

Hintergrund: Isokinetische Gerate nehmen im Bereich der Kraftdiagnostik einen hohen Stellenwert ein. Das breite Angebot entsprechender Produkte am Markt wurde in den letzten Jahren um das IsoMed 2000 erweitert. Bevor ein Gerat in der Diagnostik Verwendung finden kann, mussen Informationen uber die Reliabilitat fur spezifische Anwendungen vorliegen. Wahrend es fur das IsoMed 2000 diesbez. bereits einzelne Studien hinsichtlich der Maximalkraft gibt, fehlen diese bislang ganzlich fur den Bereich der Kraftausdauer. Das Ziel dieser Studie bestand darin, die Reliabilitat mehrerer Methoden der Quantifizierung der Kraftausdauer am IsoMed 2000 zu untersuchen. Methode: Eine Stichprobe von 33 sportlich aktiven gesunden Probanden (Alter: 23,8 ± 2,6 J.) partizipierte an einem Gewohnungs- und zwei Testterminen, mit 3–4 Tagen Abstand zwischen den Terminen. Der Test umfasste eine Serie von 30 maximal durchgefuhrten konzentrischen Beuge- und Streckbewegungen des rechten Knies bei einer Winkelgeschwindigkeit von 180 °/s. Ausgehend von den Parametern maximales Drehmoment und Arbeit fur jede Einzelwiederholung wurden Indizes der absoluten (KADabs) und relativen (KADrel) Kraftausdauer abgeleitet. Die KADabs wurde anhand des Mittelwerts uber alle Testwiederholungen, die KADrel auf zwei Arten berechnet: zum einen als prozentueller Abfall zwischen den ersten und letzten 5 Wiederholungen (KADrelA), zum anderen als negative Steigung der linearen Regressionsgleichung uber alle Wiederholungen (KADrelB). Systematische Fehler wurden anhand gepaarter t-Tests gepruft, die relative und absolute Reliabilitat anhand des Intraklassenkorrelationskoeffizienten (ICC 2.1) bzw. des Standardmessfehlers (SEM%). Ergebnisse / Schlussfolgerung: Zusammenfassend wurden fur die Extension sowohl hinsichtlich der Messung der KADabs als auch – in etwas abgeschwachter Form – fur die KADrel durchweg hohe ICCWerte von 0,76–0,89 verbunden mit akzeptablen SEM%-Werten von 1,2–5,9 % gefunden. Fur die Flexion gilt dies lediglich fur die KADabs, wahrend die Ergebnisse fur die KADrel mit ICC- und SEM%-Werten von 0,42–0,62 bzw. 9,6–17,7 % deutlich schlechter ausfielen und Zweifel an der klinischen Verwertbarkeit aufwerfen. Sollte die KADrel fur die Flexion dennoch erhoben werden, so empfiehlt es sich – angesichts der jeweils etwas hoheren Reliabilitat – (i) die KADrelB und (ii) den Parameter Arbeit heranzuziehen sowie – angesichts der festgestellten betrachtlichen Lernund Gewohnungseffekte von ≈10 % – (iii) eine Gewohnungsphase durchzufuhren, die uber den in dieser Studie praktizierten Gewohnungstermin hinausgeht.

The present study was conducted to evaluate the relative and absolute reproducibility of the IsoMed 2000-dynamometer in measuring Peak Torque (PT) during maximum isometric and isokinetic (60 and 120°/s) knee flexion and extension. Thirty four physically active male subjects (mean age: 23.2 years) were measured in three sessions (T1―T3), 48-72 h apart. Repeated measures analysis of variance (ANOVA) with Bonferroni post hoc adjustments revealed significant systematic errors for isokinetic flexion measurements with mean values on the second and third visit being around 3-5% higher than the corresponding values obtained in the first visit. Intraclass correlation coefficients (ICC 2,1) of 0.90-0.98 and 0.94-0.98 combined with values of standard error of measurement (SEM) of 5.5―9.1 and 4.0-9.0 Nm were found for T1―T2 and T2-T3, respectively. Based on the systematic errors found for flexor measurements and a clear improvement in reproducibility parameters from T1―T2 to T2―T3 for nearly all measurements, we recommend the use of a familiarisation session prior to actual testing.

Thirty five physically active male subjects (mean age: 23.7 years) with no prior experience of isokinetic exercise were measured on three occasions (T1-T3), separated by 48-72 h. Detection of systematic errors was performed using repeated measures analysis of variance (ANOVA) with Bonferroni post hoc adjustments. Further statistical calculations were done between consecutive pairs of sessions (T1-T2; T2-T3). Intraclass correlation coefficients (ICC 2,1) were calculated to assess relative reproducibility, whereas absolute reproducibility was determined by the standard error of measurement (SEM). A highly significant systematic error was found for theeccentric measurements withvalues on thefirst visit being around 5% lower than corresponding values on thesecond and third visit. ICCvalues of 0.907-0.955 and 0.968-0.984 withcorresponding values of SEMof 7.4-19.1 and 4.5-10.9 Nmwere found for T1-T2 and T2-T3, respectively. In conclusion, high reproducibility was shown for the IsoMed 2000-dynamometer in measuring PT during concentric and eccentric knee extension. However, due to a systematic practice-based improvement found for eccentric measurements, we suggest to do a familiarisation session prior the actual testing sessions in practice.

Systematic assessment of muscle strength of the lower extremities throughout the annual training cycle in athletes is crucial from a performance perspective for the optimization of the training process, as well as a health perspective with regard to injury prevention. The main aim of the present study was to determine isokinetic muscle strength of the knee flexors and extensors in female handball players at the beginning of a preparatory period and to assess whether there were any differences between players of different performance levels. The performance level was expressed by means of membership of the Women’s Junior National Handball Team (JNT, n=8) or the Women’s National Handball Team (NT, n=9). The isokinetic peak torque during concentric and eccentric single-joint knee flexion and extension was measured at angular velocities of 60, 180, 240°/s (concentric) and 60°/s (eccentric). The Mann- Whitney test showed no significant differences in the peak torques or ipsilateral ratios between the two groups. The bilateral force deficit (BFD) for concentric extension at 240°/s was significantly higher in the JNT compared with the NT (p=0.04; d=1.02). However, the results of individual evaluation show that the BFD was more frequent in the NT in most measurements. A high BFD was evident in the eccentric mode in both groups highlighting a need for particular strengthening. With regard to low strength ratios a prevention programme should be suggested for both observed groups of professional female handball players to reduce the risk of injury.

Medical research but also quality management is based upon medical data. The integration, validation, processing, and exploration of this data is known to be a technical obstacle for researching medical domain experts and a major pitfall to (bio‐)medical research projects. To overcome this pitfall and actively support the medical domain expert in these tasks, we present an ontology‐based clinical data warehouse for scientific research. It is completely generic and adapts itself at run‐time to the current domain‐ontology, which can be freely defined by the domain expert and describes the actual field of research. The whole system adapts is appearance and behavior to this central ontology and appears to the user like a custom made solution. Furthermore, the elaborate structural meta‐information from the ontology is used to actively support the user in tasks that usually require profound IT knowledge, such as defining complex search queries or data quality constraints, or applying advanced data visualization algorithms to the data. The proposed warehouse supports the domain expert trough the whole process of knowledge discovery from data integration to exploration.

Wound management, which is based on wound diagnostic, will become more and more important and will develop even more towards evidence-based medicine. Evidence-based medicine is the current state of the art in clinical research. The data quality is very much dependent on the transparency of the treatment and its objective evaluation. An objectification requires comparable patients with comparable wounds as well as comparable documentation and quality of the documentation.

For successful treatment of burns one of the most important pillars is an adequate documentation. Otherwise, nobody in science, economics or quality control can comprehend this issue’s complexity [22]. Research, science and costing in burns are based on accurate assessment and documentation of burn injuries. Documentation required, is time consuming and labor intensive. For any scientific comparability of burns the exact and correct extent and depth of burns are essential.

Data acquisition and data mining are often seen as two independent processes in research. We introduce a meta-information based, highly generic data acquisition system which is able to store data of almost arbitrary structure. Based on the meta-information we plan to apply data mining algorithms for knowledge retrieval. Furthermore, the results from the data mining algorithms will be used to apply plausibility checks for the subsequent data acquisition, in order to maintain the quality of the collected data. So, the gap between data acquisition and data mining shall be decreased.

Our aim is to develop a physically correct simulation of blood flow through intracranial aneurysms. It shall provide means to estimate rupture risks by calculating the distribution of pressure and shear stresses in an intracranial aneurysm, in order to support the planning of clinical interventions. Due to the time-critical nature of the application, we are forced to use the most efficient state-of-the-art numerical methods and technologies together with high performance computing (HPC) infrastructures. The Navier-Stokes equations for the blood flow are discretized via the finite element method (FEM), and the resulting linear equation systems are handled by an algebraic multigrid (AMG) solver. First comparisons of our simulation results with commercial CFD (computational fluid dynamics) software already show good medical relevance for diagnostic decision support. Another challenge is the visualization of our simulation results at acceptable interaction response rates. Physicians require quick and highly interactive visualization of velocity, pressure and stress to be able to assess the rupture risk of an individual vessel morphology. To meet these demands, parallel visualization techniques and high performance computing resources are utilized. In order to provide physicians with access to remote HPC resources which are not available at every hospital, computing infrastructure of the Austrian Grid is utilized for simulation and visualization.

Understanding burns means knowing what is necessary for the successful treatment of burns. Nobody in science, economics, or quality control can comprehend this issue’s complexity without thorough documentation of the work involved. BurnCase 3D is a non-profit research project whose aim, achieved through software of the same name, is a thorough and accurate burn-treatment documentation schema, facilitated by three-dimensional digital models tracked over time. Adapting these models on the basis of gender, height, weight, and body shape avoids systemic errors. Superimposing photos of the burned areas on the model prevents individual error and can be combined with methods of burn-depth evaluation. The program includes automatic encoding of diagnostic and therapeutic procedures. Model resolution is 1 cm2 and finer, so that even small scars’ locations and extents can be documented, thus enabling registration of long-term results. The program’s status as a multilingual data-collection tool brings together multiple international efforts in data collection, and makes it suitable for e-medicine and disaster relief. In its basic form, it provides essential functions in burn documentation, photo documentation, and reporting. The four-dimensional database allows registration of interactions over time and can demonstrate the influence of location, timing, and intervention on outcome.

Rationale: The determination of the extent of a burn injury is a complex process. To aid this process, plenty methods and tools have been developed. The aim aim of this work is to show how accurate experts are able to estimate surfaces by using standard methods “palm-rule”, “rule-of-nine”, “lund-browder-chart” or others. The question we want to answer is how the results correlates when experts use those estimation methods. Methods: For this survey we visited a german-speaking conference in Austria this January 2010 dealing with burns. We provided a questionnaire with 3 pictures of burned patients where we marked the areas to estimate with a yellow line. The questionnaire asked the participants about their used estimation method and their profession. Additionally we provided sheets with graphical representations of the “palm-rule”, the “rule-of-nine” and a “lundbrowder-chart”. The participation of 51 persons has been voluntarily and without financial payment. Results: As we didn’t know the exact results our self, we calculated the mean value of all attendants per patient as a basis for statistical variance analysis in different type of professions as well as in different used methods. The usage of the “palm-rule” and the “rule-of-nine” has been almost equal; the “lund-browder-chart” has only been used by 7 persons. Concerning the standard deviations of the different used methods, the best results have been archived by the use of the “rule-of-nine”. Concerning the standard deviations of the different professions, the best results have been archived by the assistant physician and physicians could reach the best results. Conclusion: The standard deviations of all three estimations reached from 42 to 62 percent. The usage of existing methods shows significant deviations of their results, even used by experts. In order to overcome those anomalies, the usage of more objective methods should be considered in future.
