Zusammenfassung Hintergrund Kardiovaskuläre Erkrankungen sind die häufigste Todesursache in Europa mit relevanter sozioökonomischer Belastung. Ein Screeningprogramm für Gefäßerkrankungen bei asymptomatischen Personen mit definierter Risikokonstellation kann zu einer frühen Diagnose führen. Ziel der Arbeit Die Studie untersucht ein Screeningprogramm auf Karotisstenosen, periphere arterielle Verschlusskrankheit (pAVK) und abdominelle Aortenaneurysmen (AAA) bei Menschen ohne anamnestisch bekannte arterielle Gefäßerkrankungen hinsichtlich demografischer Daten, Risikofaktoren, Vorerkrankungen, Medikamenteneinnahme sowie Detektion und Therapie pathologischer Befunde. Material und Methoden Probanden wurden durch verschiedene Informationsmaterialien eingeladen, ein Fragebogen erfasste kardiovaskuläre Risikofaktoren. Das Screening erfolgte mit ABI-Messung und Duplexsonographie als monozentrische, prospektive, einarmige Studie innerhalb eines Jahres. Endpunkte waren die Prävalenz von Risikofaktoren, pathologische und/oder behandlungsbedürftige Befunde. Ergebnisse Es nahmen 391 Personen teil, bei 36,0 % bestand mindestens ein kardiovaskulärer Risikofaktor, 35,5 % wiesen zwei und 14,4 % drei oder mehr auf. Aus der Sonographie der Karotiden resultierte bei 9 % ein kontrollbedürftiger Befund mit einer 75%igen Stenose oder eines Verschlusses. Ein AAA mit einem Durchmesser von 3,0–4,5 cm wurde bei 0,9 % nachgewiesen, ein pathologischer ABI 1,3 bei 12,3 %. Bei 17,0 % bestand befundbasiert die Indikation einer Pharmakotherapie, bei keinem die einer Operation. Diskussion Es zeigte sich die Durchführbarkeit eines Screeningprogramms auf das Vorliegen einer Karotisstenose, einer pAVK und eines AAA bei definierten Risikopersonen. Es wurden kaum behandlungsbedürftige Gefäßpathologien im Einzugsgebiet der Klinik nachgewiesen, sodass ein Einsatz des Programms in Deutschland in dieser Form derzeit auf Basis der Daten nicht empfohlen werden kann.

ZusammenfassungEin 70-jähriger adipöser Patient (Body-Mass-Index [BMI] 46,7 kg/m2) stellte sich mit ausgeprägter Schmerzsymptomatik beider Beine bei klinischem Vollbild einer tiefen Venenthrombose vor. Duplexsonographisch bestätigte sich eine 4‑Etagen-Thrombose. Nach frustranem venösem Rekanalisationsversuch erfolgte CT-angiographisch der Nachweis eines abdominellen Aortenaneurysmas von 15 cm maximalem Querdurchmesser mit vollständiger Kompression der V. cava inferior. Zur venösen Dekompression sowie der Rupturprophylaxe wurde eine offen-chirurgische Aneurysmaausschaltung durchgeführt.

Objective: The aim of this study was to report 10-year real-world single-center experience with the GORE TAG conformable thoracic aortic graft (CTAG), focusing on rupture-free survival, aortic-related reintervention, and device-related complications during midterm and long-term follow-up (FU). Methods: This retrospective study analyzes results of thoracic endovascular aortic repair (TEVAR) performed between January 2009 and December 2018. Out of 419 TEVAR procedures within this period, 194 patients (male 57.2%, 111/194), with a mean age of 65 ± 13 years, were treated with the CTAG device. Indication for TEVAR was a thoracic aortic aneurysm in 24.7% (48/194), type B aortic dissection in 32.5% (63/194), penetrating aortic ulcer 15.5% (30/194), and miscellaneous 27.3% (53/194). Emergently were operated 43.8% (85/194) patients. Median follow-up (FU) including computed tomography imaging was 43.5 months (Q1-Q3: 8.6–67.0) and was completed in 91.2% (177/194) of patients. Results: Overall survival rates were 75.8% (95% confidence interval [CI] = [0.76–0.70]) and 56.6% (95% CI = [0.57–0.50]) at 12 and 60 months, respectively. Cumulative incidence for aortic rupture was 11.9% (95% CI = [0.07–0.17]) at 60 and 90 months, respectively. Cumulative incidence for aortic-related reintervention was 27.5% (95% CI = [0.21–0.34]) at 60 and 90 months. Cumulative incidence for migration was 2.8% (95% CI = [0.004–0.05]) and 3.9% (95% CI = [0.007–0.07]) at 60 and 90 months, respectively. New endograft infections or material fatigue were not observed. Conclusions: The herein reported 10-year real-world single-center experience with the CTAG observed favorable long-term outcome. Thus, the device demonstrates appropriate persistent safety, efficacy, and clinical durability up to long-term FU in the treatment of diverse thoracic aortic pathologies.

OBJECTIVES The aim of this study was to evaluate technical/clinical outcomes of the Gore TAG Conformable Thoracic Stent Graft with Active Control System (CTAG with ACS) in thoracic aortic disease since its introduction in Europe in 2017. METHODS Patients undergoing thoracic endovascular aortic repair (TEVAR) with the device between 2017 and 2020 were identified from a prospectively maintained single-centre database and retrospectively analysed. Predominant indications were aortic dissection (n = 46), thoracic/thoraco-abdominal aneurysm (n = 20/n = 7), penetrating ulcer (n = 14) and intramural haematoma (n = 12). Emergency procedures were performed in 47% (54/115). The primary outcome was technical success. Secondary outcomes were clinical success, serious adverse events and procedure-related reintervention. ACS was assessed regarding angulation, accuracy and apposition. The median follow-up was 8.46 months (interquartile range: 3.18–16.89 months). RESULTS A total of 115 consecutive patients (82 males, median age 63; interquartile range: 53–74 years) have been included. Technical success was achieved in 95.7% (110/115). Clinical success rate was 80.9% (93/115); mostly due to procedure-related death (n = 14). Overall mortality was 19.1% (22/115), with significant differences between elective/emergency procedures (log-rank: P CONCLUSIONS This single-centre study shows encouraging performance of the CTAG with ACS in an array of aortic pathologies. Although longer-term data must be awaited, ACS leads to overall favourable device placement. Despite ongoing advances in device technology, TEVAR remains challenging and is associated with significant burden inherent to the underlying disease.

Die hier aufgefuhrten „Choosing-wisely“-Empfehlungen zur Behandlung der akuten und chronischen Typ-Stanford-B-Dissektion basieren auf den Leitlinien und Konsensusempfehlungen in der publizierten Literatur der letzten 10 Jahre. Es existieren neben ADSORB und INSTEAD keine randomisierten Studien zum Therapievergleich „best medical treatment“ (BMT) versus „TEVAR“ („thoracic endovascular aortic repair“). Die Literatur zur offenen Chirurgie der Aortendissektion ist mittlerweile historisch, da im Falle der Indikation zur invasiven Therapie TEVAR seit mehr als einem Jahrzehnt die Methode der Wahl ist. Die Positivempfehlungen adressieren insbesondere Differenzialdiagnose, Diagnostik und Indikationsstellung sowie TEVAR und Nachsorge. Die Negativempfehlungen fokussieren auf das State-of-the-Art-Management der konservativen Therapie, dem sog. „best medical treatment“, die Pravention der schwerwiegenden Komplikation der Paraplegie und die elektive Indikationsstellung der chronisch expandierenden Typ-B-Dissektion. Der Artikel stellt die Komplexitat der Therapieentscheidungen mit der anspruchsvollen Diagnosestellung und der komplikationsreichen invasiven Therapie am Beispiel der Aortendissektion Typ Stanford B ab. Ein Risiko der endovaskularen Uberversorgung besteht bei Patienten mit unkomplizierter Typ-B-Dissektion. Eine Gefahr der Unterversorgung besteht sowohl im Hinblick auf die primare Diagnosestellung und Diagnosesicherung als auch hinsichtlich der Nachsorge und Uberwachung wahrend des Follow-ups.

Die Angaben zu Komplikationsraten nach perkutanen Interventionen in der Behandlung der peripheren arteriellen Verschlusskrankheit (pAVK) variieren in der Literatur, zwischen 2,4 und 18,1 % sind beschrieben. Hierunter werden sowohl Zugangs- bzw. punktionsbedingte Komplikationen als auch systemische Komplikationen, wie z. B. kardiovaskulare Ereignisse erfasst. Aufgrund des hohen Stellenwerts interventioneller Techniken in der pAVK-Versorgung, zunehmend auch im ambulanten Sektor, sowie vor dem Hintergrund eines hoch komorbiden Patientenkollektivs ist die Kenntnis potenzieller Risikofaktoren sowie die Pravention moglicher Komplikationen von Relevanz. Fast 90 % der Komplikationen treten innerhalb des ersten postinterventionellen Tages auf. Dabei korreliert das Auftreten von Komplikationen mit einem schlechteren Outcome bezogen auf die 30-Tages- und 1‑Jahres-Mortalitat sowie den Amputationsraten. Jedoch erschwert die fehlende Anwendung einer einheitlichen Terminologie die Erfassung und insbesondere die Vergleichbarkeit der Ergebnisse. Der vorliegende Artikel soll daher eine Ubersicht uber mogliche Komplikationen perkutaner Interventionen in der pAVK-Therapie geben und diese unter Berucksichtigung moglicher Risikofaktoren in den klinischen Kontext einordnen.

BackgroundAn inverse association between the case volume per hospital and surgeon and perioperative mortality has been shown for many surgical interventions. There are numerous studies on this issue for the open treatment of infrarenal aortic aneurysms.AimTo present the available data on the association between the case volume per hospital and surgeon and perioperative mortality in the elective open repair of infrarenal aortic aneurysms in a systematic review.Materials and methodsUsing the PubMed, Cochrane Library, Web of Science Core Collection, CINAHL, Current Contents Medicine (CCMed), and ClinicalTrials.gov databases, a systematic search was performed using defined keywords. From the search results, all original papers were included that compared the elective open repair of an infrarenal aortic aneurysm in a “high volume” center with a “low volume” center or by a “high volume” surgeon with a “low volume” surgeon, as defined in the respective study.ResultsAfter deduplication, the literature search yielded 1021 hits of which 60 publications met the inclusion criteria. Of these, 37/43 studies showed a lower mortality in “high volume” compared to “low volume” centers and 14/17 comparisons showed a lower mortality for “high volume” compared to “low volume” surgeons. The effect measures, usually odds ratios, ranged from 0.37 to 0.99 for volume per hospital and 0.31 to 0.92 for volume per surgeon. Regarding the threshold values for the definition of “high volume” and “low volume,” a clear heterogeneity was shown between the individual studies.DiscussionThe available data on the association between the case volume per hospital and surgeon and perioperative mortality in the elective open repair of infrarenal aortic aneurysms show that interventions performed in “high volume” centers or by “high volume” surgeons are associated with lower mortality. To ensure the best possible outcome in terms of low perioperative mortality in the open repair of infrarenal aortic aneurysms, the aim should be centralization with high case volume per hospital and surgeon.

BackgroundMany surgical interventions show an inverse association between case volume per hospital/surgeon and perioperative mortality. In the first part of this systematic review it was shown that such an association also exists for the open treatment of infrarenal aortic aneurysms. The second part now examines a possible association with endovascular treatment of infrarenal aortic aneurysms.ObjectiveIn a systematic review, the data available on the association between the case volume per hospital/surgeon and perioperative mortality in elective endovascular treatment of infrarenal aortic aneurysms are presented.Materials and MethodsSystematic research using defined keywords was carried out. All original works comparing elective endovascular treatment of an infrarenal aortic aneurysm in a “high volume” center with a “low volume” center or by a “high volume” surgeon with a “low volume” surgeon, as defined in each study, were included.ResultsAfter deduplication, the literature search produced 1,021 hits. Of these, 16 publications fulfilled the inclusion criteria. With regard to the thresholds for the definition of high volume and low volume, there was marked heterogeneity between individual studies. Twelve of the 15 studies showed a significantly lower mortality in high volume than in low volume centers. The effect measures, usually odds ratios, were between 0.43 and 0.91. In the comparison between high volume and low volume surgeons, there was no difference in mortality in any of the five studies included.DiscussionThe available data on the association between case volume per hospital and surgeon and the perioperative mortality in elective endovascular treatment of infrarenal aortic aneurysms consistently show that patients operated on in high volume centers have a lower mortality. The volume per surgeon seems to have no influence on perioperative mortality. To achieve the lowest perioperative mortality possible in endovascular treatment of infrarenal aortic aneurysms, centralization with high volume per hospital should be aimed for, taking into consideration the context of the health care system.

Eine Vielzahl chirurgischer Eingriffe weist eine inverse Assoziation zwischen Fallzahl pro Krankenhaus und Operateur und perioperativer Mortalitat auf. Im ersten Teil dieser systematischen Ubersichtsarbeit konnte gezeigt werden, dass eine solche Assoziation auch fur die offene Versorgung infrarenaler Aortenaneurysmen besteht. Der zweite Teil untersucht nun eine mogliche Assoziation fur die endovaskulare Versorgung. In einer systematischen Ubersichtsarbeit soll die verfugbare Datenlage zur Assoziation zwischen Fallzahl pro Krankenhaus und Operateur und perioperativer Mortalitat bei der elektiven endovaskularen Versorgung infrarenaler Aortenaneurysmen dargestellt werden. Eine systematische Literaturrecherche wurde mittels definierter Schlusselworter durchgefuhrt. Es wurden alle Originalarbeiten eingeschlossen, die die elektive endovaskulare Versorgung eines infrarenalen Aortenaneurysmas in einem „high volume“-Zentrum mit einem „low volume“-Zentrum oder durch einen „high volume“-Chirurgen mit einem „low volume“-Chirurgen, wie in der jeweiligen Studie definiert, verglichen. Nach Deduplizierung erbrachte die Literatursuche 1021 Treffer. Von diesen erfullten 16 Publikationen die Einschlusskriterien. Hinsichtlich der Schwellenwerte zur Definition von „high volume“ und „low volume“ zeigte sich eine deutliche Heterogenitat zwischen den einzelnen Studien. So zeigten 12/15 Studien eine signifikant niedrigere Mortalitat in „high volume“- verglichen mit „low volume“-Zentren. Die Effektmase, in aller Regel Odds Ratios, lagen zwischen 0,43 und 0,91. Fur den Vergleich von „high volume“- mit „low volume“-Chirurgen zeigte sich in keiner der funf eingeschlossenen Studien ein Mortalitatsunterschied. Die verfugbare Datenlage zur Assoziation zwischen Fallzahl pro Krankenhaus und Operateur und perioperativer Mortalitat bei der elektiven endovaskularen Versorgung infrarenaler Aortenaneurysmen zeigt ubereinstimmend, dass in „high volume“-Zentren operierte Patienten eine niedrigere Mortalitat aufweisen. Die Fallzahl pro Chirurg scheint keinen Einfluss auf die perioperative Mortalitat zu haben. Um eine moglichst niedrige perioperative Mortalitat bei der endovaskularen Versorgung infrarenaler Aortenaneurysmen zu erreichen, sollte unter Berucksichtigung des Kontextes des Gesundheitssystems eine Zentralisierung mit hohen Fallzahlen pro Krankenhaus angestrebt werden.

In den Datenbanken PubMed, Cochrane Library, Web of Science Core Collection, CINAHL, Current Contents Medizin (CCMed) und ClinicalTrials.gov wurde eine systematische Recherche mittels definierter Schlusselworter durchgefuhrt. Um ein moglichst gutes Outcome im Sinne einer niedrigen perioperativen Mortalitat bei der offenen Versorgung infrarenaler Aortenaneurysmen zu gewahrleisten, sollte eine Zentralisierung mit hohen Fallzahlen pro Krankenhaus und Chirurg angestrebt werden.

OBJECTIVES Optimal treatment for patients with diseased proximal landing zones in acute/subacute Stanford type B dissection and intramural haematoma remains unclear. This study describes the preliminary outcomes of a localized endovascular treatment [spot-stent grafting (SSG)] of main entries/intramural blood pooling located downstream (aortic zones 4 and 5) using one single short device comprising diseased landing zones, looking particularly at the technical and morphological outcomes. METHODS Patients undergoing thoracic endovascular aortic repair (TEVAR) for acute/subacute aortic dissection Stanford type B/intramural haematoma Stanford type B between 1997 and 2018 were identified from a prospectively maintained institutional database. In a total of 183 cases, 22 patients (7 women; median age 62 years; range 35–79 years) received SSG. The primary study end point was technical success. The primary morphological end point was false lumen thrombosis/aortic remodelling. Secondary end points were TEVAR-related mortality/morbidity and reinterventions. The median follow-up was 28.5 months (5 days–15.6 years). RESULTS The primary technical success rate was 100% (22/22). During follow-up, false lumen thrombosis was seen in 21 patients (95.5%) at a median of 6 days (0 days to 2.7 years) after the index procedure (limited/extended false lumen thrombosis: n = 9 vs 12). Aortic remodelling was achieved in 15 of 22 patients (68.2%) at a median of 360 days (3 days to 7.2 years). Limited/extended remodelling was observed in 8/15 and 7/15, respectively. Retrograde dissection or stent graft-induced new entry was not observed. No stroke or spinal cord injury occurred. Reinterventions were performed in 4/22 cases. The in-hospital mortality and 30-day mortality were 0%. Overall mortality during the follow-up period was 22.7% (5/22). CONCLUSIONS This study shows favourable technical and morphological results for SSG in selected patients with acute/subacute aortic dissection Stanford type B/intramural haematoma Stanford type B. Patient allocation to SSG remains individual. Prospective large-scale long-term data may allow refinement of the application.

Zusammenfassung Hintergrund Mausmodelle zur Erforschung der Pathogenese von Aortenerkrankungen sind seit Jahren in der vaskulären Forschung etabliert. Aufgrund der komplexen multifaktoriellen Prozesse der Entstehung von Aortenaneurysmen, die bis heute noch nicht vollständig verstanden sind, wurden verschiedene Mausmodelle entwickelt. Der vorliegende Artikel stellt eine Übersicht der verfügbaren Modelle sowie der Vor- und Nachteile des jeweiligen Modells vor dem Hintergrund spezifischer Fragestellungen dar. Methodik Es erfolgte eine selektive Literaturrecherche zum Thema. Ergebnisse Die verfügbaren Mausmodelle lassen sich in drei Kategorien einteilen: genetisch induzierte, chemisch-topisch induzierte und medikamentös-systemisch induzierte Modelle. Die einzelnen Mausmodelle bieten jeweils spezifische Vor- und Nachteile, sodass die Auswahl des Modelles anhand der zugrunde liegenden Fragestellung erfolgen muss. Idealerweise werden mehrere Modelle kombiniert, um auszuschließen, dass den Ergebnissen modellspezifische Effekte zugrunde liegen. Medikamentös-systemisch induzierte Modelle mittels Angiotensin II und chemisch-topisch induzierte Modelle mittels Kalziumchlorid oder Elastase stellen dabei aufgrund ihrer Flexibilität die am häufigsten verwendeten Mausmodelle dar. Sie erlauben die Untersuchung einer Vielzahl verschiedener Einflussfaktoren oder medikamentöser Therapieansätze. Schlussfolgerung Mausmodelle liefern grundlegende Erkenntnisse zur bislang noch unzureichend verstandenen komplexen und multifaktoriellen Pathophysiologie der Entstehung und Progression von Aortenaneurysmen. Tiermodellbasierte Forschungsansätze stellen dabei eine ideale Grundlage für translationale Forschungsprojekte dar, welche prospektiv für die Entwicklung konservativer medikamentöser Therapieansätze für Aortenaneurysmen wesentliche Erkenntnisse beitragen können.

This article presents the case of a 70-year-old obese patient with severe bilateral leg pain due to deep vein thrombosis. After unsuccessful venous recanalization, computed tomography angiography revealed an abdominal aortic aneurysm 15 cm in diameter with total compression of the inferior vena cava. Zusammenfassung Hintergrund Interdisziplinäre Aortenkolloquien (iAK) gelten als wertvolle Instrumente in der Konzepterstellung für die Therapie aortaler Pathologien im Bereich der Krankenhausmaximalversorgung. Ziel der Arbeit ist die Evaluation eines universitären iAK (Universitätsklinikum Heidelberg; beteiligte Kliniken: Gefäßchirurgie und Endovaskuläre Chirurgie [GCH]/Kardiochirurgie [KCH]/Radiologie) unter den Gesichtspunkten Gesamtpatientenvolumen, fachspezifische Vorstellungen, konservativer/operativer Therapieentscheid und interdisziplinäre fachrichtungswechselnde Therapieallokation (GCK nach KCH und vice versa). Methodik Es erfolgte eine retrospektive monozentrische Datenerfassung aller im iAK vorgestellten Patienten im Zeitraum 01/2014 – 12/2018. Ergebnisse Insgesamt wurden in 5 Jahren 344 Patienten (Median: 71 Patienten/Jahr) im iAK evaluiert. Davon wurden 262 von der GCH (76%), 63 von der KCH (18%) und 19 (6%) von anderen Disziplinen vorgestellt. Bei 153 von 344 Patienten (45%) wurde eine OP-Indikation gestellt. Entsprechend erfolgte eine konservative Therapieempfehlung in 191 von 344 Fällen (55%). In den Fällen mit gestellter OP-Indikation handelte es sich in 117 von 153 (76%) um originär GCH-geführte, in 29 von 153 (19%) um originär KCH-geführte und in 7/153 (5%) um anderweitig geführte Patienten. Eingriffe aus dem Fachbereich GCH wurden in 85/153 Fällen (55%) indiziert, davon entfielen 25% (22/85) auf fachfremde Patienten. Eingriffe aus dem Fachbereich KCH wurden in 68/153 Fällen (45%) indiziert. Der Anteil der fachrichtungswechselnden Therapieallokation betrug hier 84% (57/68). In 59 von 153 Fällen (39%) erfolgte die im iAK indizierte Therapie nicht. Schlussfolgerung Die Frequentierung des iAK zeigt sich über die Jahre konstant. Nicht zuletzt aufgrund der ambulanten Präsenz des Faches stellt die GCH den größten Teil der evaluierten Patienten. Die hohe Rate an konservativen Therapieentscheiden unterstreicht die Verantwortung/Selektionsaufgabe eines iAK. In Bezug auf die gestellten OP-Indikationen zeigt die vergleichbare relative Verteilung auf GCH/KCH eine gute Interdisziplinarität. Die hohe Rate fachfremder Fälle im Indikationsvolumen der KCH spiegelt den nach wie vor hohen Stellenwert des Faches wider. Kritisch zu hinterfragen ist die Effizienz des iAK in Bezug auf die tatsächliche Umsetzung der gestellten OP-Indikationen.

Hintergrund. Vor dem Hintergrund eines steigenden Nachwuchsmangels in chirurgischen Fachern kommt der zeitgemasen Nachwuchsforderung in der Aus- und Weiterbildung eine zunehmende Rolle zu. Hierzu ist eine Anpassung der Lehrstrukturen an die Anforderungen einer technisierten und spezialisierten Medizin essenziell. Der vorliegende Ubersichtsartikel widmet sich den Hintergrunden von Simulation in der Chirurgie, stellt Simulation in den Kontext von Nachwuchsmangel und eruiert simulationsbasierte Strategien in der gefaschirurgischen Aus- und Weiterbildung. Methodik. Es erfolgte eine selektive Literaturrecherche zum Thema. Ergebnisse. Simulationsbasiertes Lernen hat in der medizinischen Aus- und Weiterbildung einen hohen Stellenwert. Trotz guter Datenlage und bereits zunehmender Anwendung von Skills-Trainings in der Weiterbildung ist die Anwendung in der Pragraduiertenausbildung bisher marginal. Schlussfolgerung. Die Implementierung simulationsbasierter Lehreinheit in die Ausbildung ist nicht nur im Sinne der Nachwuchsforderung des eigenen Fachbereichs sinnvoll. Skills-Trainings bieten die Moglichkeit, Chirurgie und die damit assoziierten Aspekte in einem angemessenen Umfeld zu lehren und den jungen und motivierten Nachfolgegenerationen naher zu bringen. Unterstutzung finden Konzepte simulationsbasierter Lehreinheiten ganz aktuell durch die im Masterplan Medizinstudium 2020 geforderte Verschiebung der traditionellen Curriculumsinhalte zugunsten von Kompetenzen und klinisch-praktischen Fertigkeiten. Dies ist insbesondere in der klinikinternen Curriculumsplanung zu beachten. Prospektiv denkbare Ansatze fur die Weiterbildung waren auserdem eine verpflichtende Aufnahme von Simulationstrainings als Facharztstandard.

Der erste Teil dieser Ubersichtsarbeit „Intraoperatives Monitoring in der konventionellen und endovaskularen Aortenchirurgie“ befasst sich mit drei etablierten Monitoringverfahren, welche im Rahmen der konventionell-offenen Aortenchirurgie selektiv Anwendung finden. Diese sind (1) die transosophageale Echokardiographie (TEE), (2) die Ableitung motorisch-evozierter Potenziale (MEP) und (3) die zerebrospinale Liquordrainage (CSFD). Jedes Verfahren wird einzeln hinsichtlich seiner Methodik und Evidenz diskutiert. Abschliesend erfolgt eine zusammenfassende Bewertung.

Limb occlusion following endovascular aortic repair (EVAR) is one of the most common complications requiring secondary interventions and is associated with a relevant morbidity and mortality. This review article describes the risk factors for limb occlusion, highlights strategies for prevention and provides a treatment algorithm involving current open and endovascular modalities. A selective literature search for risk factors and treatment modalities of limb occlusion after EVAR was carried out. A total of five risk factors for limb occlusion after EVAR were identified: (1) iliac angulations >60°/high iliac tortuosity (odds ratio, OR 5.76), (2) severe iliac calcification (OR 5.8), (3) excessive oversizing >15% (OR 5.54), (4) graft placement in the external iliac artery (EIA) with (5) EIA diameter

Objective In the past years, a paradigm shift from best medical therapy to early thoracic endovascular aortic repair (TEVAR) has been observed in the treatment of uncomplicated type B aortic dissection (uTBAD). We aimed to analyze outcomes after TEVAR in the acute and subacute phase of uTBAD, focusing on aortic remodeling. Methods Retrospective analysis of consecutive patients who underwent TEVAR for acute or subacute uTBAD in two tertiary referral centers from 2008 – 2017. Two assessors per center reviewed computed tomography scans of each patient at presentation, at one year and at the last follow-up using post-processing software. Results Forty-nine patients were treated with TEVAR for uTBAD. The indication for TEVAR was the presence of multiple morphologic predictors of adverse aortic outcome. The most common predictors were a false lumen diameter of > = 22mm in 76% of patients, a primary entry tear of > = 10mm in 69% and a total aortic diameter of > = 40mm in 67%. There were no in-hospital deaths and no deaths at 1 year. The median follow-up was 40.6 months. Three-year cumulative survival was 94 % (46/49). Fourteen secondary interventions were performed in 10 patients (20 %) after a median of 4.2 months. TEVAR lead to remodeling of the descending thoracic aorta with a median reduction of the total aortic diameter of 4.5 mm within one year and stable diameters after three years. The median maximum false lumen diameter diminished from 26 mm to 15 mm in one year (at 3 years: 14.8 mm). Conclusion In this cohort of selected patients with uTBAD and multiple morphologic predictors of worse aortic outcome, elective acute or subacute TEVAR was associated with a low mortality and positive aortic remodeling in the mid-term follow-up.

Summary: Background: Isolated femoral artery revascularisation (iFAR) represents a well-established surgical method in the treatment of peripheral arterial disease (PAD) involving common femoral artery disease. Data for iFAR in multilevel PAD are inconsistent, particularly in patients with critical limb ischemia (CLI). The aim of the study was to evaluate the outcome of iFAR in CLI regarding major amputation and reintervention and to identify associated risk factors for this outcome. Patients and methods: The data used have been derived from the German Registry of Firstline Treatment in Critical Limb Ischemia (CRITISCH). A total of 1200 patients were enrolled in 27 vascular centres. This sub-analysis included patients, which were treated with iFAR with/without concomitant iliac intervention. For detection of risk factors for the combined endpoint of major amputation and/or reintervention, selection of variables for multiple regression was conducted using stepwise forward/backward selection by Akaike’s information criterion. Results: 95 patients were included (mean age: 72 years ± 10.82; 64.2% male). Of those, 32 (33.7%) participants reached the combined endpoint. Risk factor analysis revealed continued tobacco use (odds ratio [OR] 2.316, confidence interval [CI] 0.832–6.674), TASC D-lesion (OR: 2.293, CI: 0.869–6.261) and previous vascular intervention in the trial leg (OR: 2.720, CI: 1.037–7.381) to be associated with reaching the combined endpoint. Conclusions: iFAR provides a reasonable, surgical option to treat CLI. Lesion length (TASC D) seems to have a negative impact on outcome. Further research is required to better define the future role of iFAR for combined femoro-popliteal lesions in CLI – best in terms of a randomised controlled trial.

OBJECTIVES The aim of this study was to evaluate the effects of the coronavirus 2019 pandemic on elective and acute thoracic aortic surgery in the Netherlands. METHODS The Netherlands Heart Registration database was used to compare the volume of elective and acute surgery on the thoracic aorta in 2019 and 2020, starting from week 11 in both years. A sub-analysis was done to assess the impact of the pandemic on high-volume and low-volume aortic centres. RESULTS During the pandemic, the number of elective thoracic aortic operations declined by 18% [incidence rate ratio (IRR) 0.82 [0.73–0.91]; P < 0.01]. The decline in volume of elective surgery was significant in both high-volume (IRR 0.82 [0.71–0.94]; P < 0.01) and low-volume aortic centres (IRR 0.81 [0.68–0.98]; P = 0.03). The overall number of acute aortic operations during the pandemic remained similar to that in 2019 (505 vs 499; P = 0.85), but an increased share of these operations occurred at high-volume centres. The number of acute operations performed in high-volume centres increased by 20% (IRR 1.20 [1.01–1.42]; P = 0.04), while the number of acute operations performed in low-volume centres decreased by 17% (IRR 0.83 [0.69–1.00]; P = 0.04). CONCLUSIONS The coronavirus 2019 pandemic led to a significant decrease in elective thoracic aortic surgery but did not cause a change in the volume of acute thoracic aortic surgery in the Netherlands. Moreover, the pandemic led to a centralization of care for acute thoracic aortic surgery., The coronavirus disease 19 (COVID-19) pandemic has impacted the delivery of health care around the world.

To evaluate the significance of perioperative changes in ankle-brachial index (ABI) with regard to extremity-related outcome in non-diabetic patients with critical limb ischemia (CLI) following revascularization. The study represents a subanalysis of the multicentric Registry of First-line Treatment in Patients with CLI (CRITISCH). After exclusion of diabetic patients, conservative cases, and primary major amputation, 563 of 1200 CRITISCH patients (mean age 74 ± 10.7 years) were analyzed. This population was divided into two groups regarding perioperative ABI changes ∆ + 0.15 (Group 1) or ∆ − 0.15 (Group 2). Study endpoints were reintervention and major amputation during a mean follow-up of 14.6 ± 9 months. Logistic regression was performed in order to identify factors for ABI group affiliation. There were 279 patients in Group 1 (49.5%) and 284 in Group 2 (51.5%). ABI sensitivity and specificity regarding vessel patency were calculated to be 54 and 87%. A preoperative ABI ≤ 0.4 [odds ratio (OR) 7.7], patent vessels at discharge (OR 12.2), and secondary interventions (OR 2.4) were identified as factors for Group 1 affiliation. Contrariwise, previous revascularization (OR 0.6), a glomerular filtration rate ≤ 15 ml/min/1.73 m2 (OR 0.3), and TASC A lesions (OR 0.2) were associated with Group 2 affiliation. No statistical difference was found with regard to the need of reintervention. However, time to reintervention was significantly shorter in Group 2 compared to that in Group 1 (10.0 ± 9.5 months vs 12.1 ± 9.1 months; p = 0.005). Dissecting aneurysms of the internal carotid artery were considered as very rare disorders before the seventies. Undiagnosed carotid-artery dissections, however, may have gone hidden behind earlier reports of delayed "apoplexy" due to "traumatic carotid thrombosis". Here, we present a case report of delayed stroke after trauma, published by Aristide Verneuil in 1872 in the Bulletin de l'Academie de Medecine and cited under the heading of vascular rupture and dissecting aneurysm by Heinrich Quincke in 1876. Verneuil's case report represents, to our knowledge, the first detailed clinical description of a patient with a traumatic carotid dissection confirmed at autopsy. The author highlighted the diagnostic challenges of this case, head injury followed by delayed hemiplegia suggesting an intracranial bleeding.

Objective To report procedural results and mid-term follow-up outcomes of patients treated with endovascular aneurysm sealing (EVAS) for abdominal aortic disease. Methods In this retrospective observational study, all patients treated with EVAS between March 2013 and January 2018 for abdominal aortic aneurysm (AAA) or abdominal penetrating aortic ulcer were included. The datasets included demographics, aneurysm morphology, and procedural and clinical surveillance outcomes. Furthermore, patients treated within the original instructions for use (IFU-group) were compared with patients treated outside the IFU (non-IFU-group) with regard to survival, reintervention-free survival, freedom from type I endoleak, and freedom from stent graft migration. Results Seventy patients were included (67 male; median age, 72.5 years). Sixty-five patients were treated for AAA and 5 patients for abdominal penetrating aortic ulcer. Sixty-nine cases were treated electively (98.6%). Technical success was achieved in 68 cases (97.1%). The median clinical follow-up was 50.5 months (interquartile range, 29.3-62.7 months) with a median computed tomography angiographic follow-up of 38.5 months (interquartile range, 17.1-60.2 months). There were five deaths during the study period (7.1%), four of which were aneurysm related (5.7%). Five secondary AAA ruptures were detected (7.1%). Overall, 25 of 70 patients (35.7%) underwent 35 reinterventions, mostly owing to thrombotic complications (18.6%), stent graft migration (17.1%), and type I endoleak (12.9%). Fifteen patients were treated outside of the IFU (non-IFU-group) (21.4%). The estimated reintervention-free survival for the entire cohort at 30 days and 1, 3, and 5 years was 94.3%, 88.5%, 72%, and 56.9%, respectively. Freedom from stent graft migration at 1, 3, and 5 years was 98.6%, 82.0%, and 47.3%, respectively. The estimated freedom from type I endoleak at 30 days and 1, 3, and 5 years in the IFU-group was 100%, 100%, 94.9% and, 91.1% and significantly different when compared with the non-IFU-group with 79.5%, 72.2%, 72.2%, and 72.2% (P = .012). Conclusions Although the technical and initial results were satisfying, the mid-term results were disappointing. The enforcement of a close follow-up protocol for all patients treated with EVAS, especially vigilant for stent graft migration to prevent secondary type I endoleak and rupture, is strongly recommended.

Acute occlusion of the descending thoracic aorta (DTA) is rare and associated with high morbidity and mortality. In the case described here, rescue thoracic endovascular aortic repair (TEVAR) was successful in a 59-year-old man with acute occlusion of the DTA accompanied by lower body hypoperfusion after two previous open repairs for aortic coarctation.

Genetically linked connective tissue diseases lead to degeneration, aneurysm formation and spontaneous dissection or rupture of arteries. The most well-known are Marfan syndrome, vascular Ehlers-Danlos syndrome (type IV), Loeys-Dietz syndrome and familial aortic aneurysms and dissections.This review article addresses the current status of endovascular treatment options for important connective tissue diseases.Evaluation of currently available randomized studies and registry data.The treatment of choice for patients that are mostly affected at a young age is primarily conservative or open repair. There is only limited evidence for endovascular aortic repair (EVAR) of abdominal aneurysms or thoracic endovascular aortic repair (TEVAR).The progression of the disease with dilatation leads to secondary endoleaks and high reintervention rates with uncertain long-term results. Die Kasuistik beschreibt die erfolgreiche endovaskulare Therapie einer Pelottierung der A. thoracica descendens durch einen Knochenzementsporn bei Zustand nach Kyphoplastie. Die Intervention erfolgte im Sinne der Ruptur-/Embolieprophylaxe und verdeutlicht den Stellenwert der Gefaschirurgie im interdisziplinaren Netzwerk der Orthopadie.

Die Versorgung von Aortenpathologien mit Beteiligung des Bogens wird an spezialisierten Zentren mittlerweile vornehmlich endovaskular durchgefuhrt. Gemas der Literatur stellt die offene Rekonstruktion zwar noch den Goldstandard dar, allerdings gilt dies lediglich fur ein hochselektioniertes Patientenkollektiv. Dementsprechend ist der Anteil von Versorgungen in Notfallsituationen und bei alteren bzw. vorerkrankten Patienten im TEVAR-Kollektiv besonders hoch. Insgesamt liegt die technische Erfolgsrate der TEVAR im Aortenbogen unabhangig von der Landungszone bei ca. 90 % mit einer Mortalitat von 0–14 % und einer Morbiditat von bis zu 55 %. Durch die anatomische Beziehung zu den supraaortalen Asten und den diversen Atiologien mit oft komplexen morphologischen Konfigurationen ist neben der kritischen Indikationsstellung die technische Herangehensweise anspruchsvoll und fur den Erfolg masgebend. Die angemessene Apposition des Stentgrafts im Bereich der proximalen Landungszone ist der wesentliche Aspekt fur das klinische Ergebnis. Ein thrombogener Aortenbogen bzw. die Positionierung in Landungszone 1 gehen mit erhohter neurologischer Morbiditat einher. Wird eine Proximalisierung in die Zonen 2–0 notwendig, erfordert dies eine Uberstentung der linken A. subclavia bzw. ein Debranching der supraaortalen Gefase. Durch sog. „Chimney“-Techniken und seit Kurzem eingesetzten fenestrierten bzw. verzweigten („gebranchten“) TEVAR-Stentgraftprothesen speziell fur den Bogen erweitert sich das rein endovaskulare Repertoire. TEVAR im Aortenbogen ist ein etabliertes Verfahren auch fur komplexe Pathologien. Aufgrund schwieriger Indikationsstellung und technischer Komplexitat ist aber fur die erfolgreiche Umsetzung ein spezialisiertes und zudem erfahrenes Team mit entsprechender Infrastruktur notwendig.

Das isolierte Iliakalaneurysma ist selten und vorrangig atherosklerotisch bedingt. Der klinische Verlauf ist zumeist asymptomatisch. Die Ruptur ist aufgrund des langjahrigen unbemerkten Wachstums im Becken relativ haufig und mit einer hohen Mortalitat vergesellschaftet; etwa ein Drittel der Patienten ist betroffen. Die Diagnostik der Wahl im klinischen Alltag besteht v. a. wegen der anatomischen Lage, insbesondere von A.-iliaca interna-Aneurysmen, in der kontrastmittelgestutzten Schnittbildgebung. Eine chirurgische Versorgung ist im asymptomatischen Stadium ab einem Durchmesser von 3 cm indiziert und erfolgt – analog zum abdominellen Aortenaneurysma – im Sinne der Ruptur- und Embolieprophylaxe. Zu den therapeutischen Optionen zahlen neben der konventionell-offenen Chirurgie (Interponat, Bypass, Ligatur) die endovaskularen Verfahren („bell-bottom technique“, iliakale „Sidebranch“-Technologie, Embolisation). Insbesondere bei Risikopatienten wird endovaskularen Techniken bei geeigneter Morphologie der Vorzug gegeben. Die Nachsorge erfolgt bei beiden Methoden mittels duplexsonographischen bzw. CT-angiographischen Verlaufskontrollen.

Objective The aim of this study was to analyze the outcome of thoracic endovascular aortic repair (TEVAR) and medical therapy in patients with aortic intramural hematoma type B (IMHB). Methods Between January 2004 and January 2014, 41 IMHB patients were treated; 28 underwent TEVAR (16 male; median age, 69 years; group I) plus best medical therapy (BMT), whereas 13 had BMT alone (6 male; median age, 69 years; group II). Study end points were assessment of indications for TEVAR and BMT, clinical outcome, and evaluation of aortic morphology over time. Median follow-up was 32 months for group I and 40 months for group II. Results In group I, TEVAR was immediately performed in 7 of 28 cases because of pain and imaging results (penetrating aortic ulcers, n = 4; intramural blood pools, n = 3). In 21 of 28 cases, TEVAR was undertaken because of clinical or radiologic signs of progression at a median of 10 days (range, 2-223 days). The median number of stent grafts implanted was two (range, 1-3). The median length of covered aorta was 15 cm (range, 9.5-33.4 cm). Technical success was achieved in 25 of 28. In-hospital mortality was 1 of 28 in group I and 0 of 13 in group II. Survival in group I was 81.5%, 77.8%, and 67% at 1, 2, and 4 years. There was no death in group II during follow-up. Aortic reinterventions were performed in 6 of 28 group I cases, including 2 open conversions for retrograde type A dissection. Aortic diameter decreased during follow-up in 10% in group I (vs 3% in group II; P = .039). In group I, complete remodeling was seen in 7 of 27, regression in the remaining 20. In group II, complete remodeling was seen in 3 of 12; regression was seen in 9 of 12. No patient in group II required invasive treatment. Conclusions BMT is justified in uncomplicated IMHB. However, IMHB becomes complicated in the majority of patients within 20 days. TEVAR in complicated IMHB is feasible but associated with a substantial aortic reintervention rate, reflecting technical challenges and fragile aortic wall conditions.

BACKGROUND The aim of this study was to analyze device conformability in TEVAR of acute and chronic (a/c) type B aortic dissections (TBAD) using the Gore Conformable Thoracic Aortic Stent-graft (CTAG). MATERIAL AND METHODS From January 1997 to February 2014, a total of 90 out of 405 patients in our center received TEVAR for TBAD. Since November 2009, 23 patients (16 men; median age: 62 years) were treated with the CTAG. Indications were complicated aTBAD in 15 (65%) and expanding cTBAD in 8 (35%) patients. Primary endpoints were the assessment of device conformability by measuring the distance (D) from the radiopaque gold band marker (GM) at the proximal CTAG end to the inner curvature (IC) of the arch on parasagittal multiplanar reformations of CT angiography, as well as the evaluation of aortic diameter changes following TEVAR. Median follow-up was 13.3 months (range: 2 days to 35 months). RESULTS Primary and secondary success rates were 91.3% (21/23) and 95.6% (22/23), respectively. There was 1 type Ia endoleak, retrograde dissection or primary conversion was not observed. Median GM-IC-D was 0 mm (range: 0 mm to 10 mm). GM-IC-D was associated with zone 2 placement compared to zone 3 (P=0.036). There was no association between GM-IC-D formation and arch type. In aTBAD cases the true lumen significantly increased after TEVAR (P=0.017) and the false lumen underwent shrinkage (P=0.025). In cTBAD patients the false lumen decreased after TEVAR (P=0.036). CONCLUSIONS The CTAG shows favorable conformability and wall apposition in challenging arch pathologies such as TBAD.

Objective This study reports the long-term results after thoracic endovascular aortic repair (TEVAR) in thoracic aortic aneurysms (TAAs) and thoracoabdominal aortic aneurysms (TAAAs). Methods Between 1997 and 2010, 269 patients were treated with TEVAR, 100 of them (72 male; mean age, 68.3 years) for aneurysmal disease. An intact TAA (iTAA) was present in 49 patients and an intact TAAA (iTAAA) in 18. In 25 patients, there was a ruptured TAA or ruptured TAAA (rTAA). Eight patients were admitted with a post-traumatic TAA (pTAA). Retrospective analysis was performed from a prospectively maintained database. Primary end points were 5-year all-cause and TEVAR-related mortality. Secondary end points were causes of death, complications, and reinterventions (RIs). A 5-year follow-up was complete in all cases. Results The overall 5-year mortality rate was 50% (40.8% in iTAA, 80% in rTAA, 12.5% in pTAA, and 50% in iTAAA, respectively; log-rank test, P = .00012). The overall procedure-related mortality was 21% (10.2% [n = 5] in iTAA, 40% [n = 10] in rTAA, 33% [n = 6] in iTAAA, and 0 in pTAA, respectively; log-rank test, P = .00013). Freedom from complication was 52%, 47.2%, and 47.2% at 1, 3, and 5 years, respectively. There were a total of 30 RIs in 25 patients. Freedom from RI was 82%, 77.8%, and 71.2% at 1, 3, and 5 years. Stepwise forward logistic regression analysis revealed rTAA and occurrence of complications were risk factors for survival (odds ratios, 7.7 and 4.2, respectively). Conclusions Long-term results after TEVAR for aneurysmatic aortic disease demonstrate considerable overall and procedure-related mortality in both elective and urgent indications. Complications and RIs occur still as late events and emphasize the necessity for long-term follow-up.

Am bekanntesten sind das Marfan-Syndrom, das Ehlers-Danlos-Syndrom vom vaskularen Typ, das Loeys-Dietz-Syndrom sowie familiare Aortenaneurysmen und Dissektionen.

Background The aim of this study was to report the occurrence of a type IIIb endoleak after endovascular repair of a thoracic aortic aneurysm caused by endoanchor dislocation. Case Report An 84-year-old female patient underwent thoracic endovascular repair for aneurysmal disease of her thoracic aorta. The procedure included primary left subclavian artery revascularization and the placement of endoanchors to enhance fixation of the endograft within the aortic arch. Dislocation of one of the endoanchors resulted in a graft defect leading to a type IIIb endoleak and aortic diameter expansion. Conclusions Endoanchors represent a promising adjunct in endovascular repair settings. However, their use requires careful procedure planning and special attention during follow-up.

Endovascular repair has revolutionized the treatment of infrarenal aortic aneurysms. However, its application is still limited in patients with challenging access conditions such as small-caliber vessels, iliac tortuosity, excessive calcification or occlusive disease. The underlying manuscript addresses the crucial role of preoperative access vessel evaluation and summarizes recent developments in endograft manufacturing and surgical techniques allowing for coping with hostile access conditions. Furthermore, alternative access routes and complication management are discussed.
