7. Brain activity monitoring by compressed spectral array during deep hypothermic circulatory arrest in acute aortic dissection surgery

Abstract

Monitoring the central nervous system during aortic dissection repair may improve the understanding of the intraoperative changes related to its bioactivity.The aim of the study was to evaluate the influence of deep hypothermia on intraoperative brain bioactivity measured by the compressed spectral array (CSA) method and to assess the influence of the operations on postoperative cognitive function.The study enrolled 40 patients (31 men and 9 women) at the mean age of 60.2 ± 8.6 years, diagnosed with acute aortic dissection. They underwent emergency operations in deep hypothermic circulatory arrest (DHCA). During the operations, brain bioactivity was monitored with the compressed spectral array method.There were no intraoperative deaths. Electrocerebral silence during DHCA was observed in 31 patients (74%). The lowest activity was observed during DHCA: it was 0.01 ± 0.05 nW in the left hemisphere and 0.01 ± 0.03 nW in the right hemisphere. The postoperative results of neurological tests deteriorated statistically significantly (26.9 ± 1.7 points vs. 22.0 ± 1.7 points; p0.001), especially among patients who exhibited brain activity during DHCA.The compressed spectral array method is clinically useful in monitoring brain bioactivity during emergency operations of acute aortic dissections. Electrocerebral silence occurs in 75% of patients during DHCA. The cognitive function of patients deteriorates significantly after operations with DHCA.Monitorowanie czynności mózgu podczas operacji rozwarstwień aorty może przyczynić się do lepszego zrozumienia śródoperacyjnych zmian jego czynności bioelektrycznej.Ocena śródoperacyjna czynności bioelektrycznej mózgu za pomocą badania skompresowanego rozkładu widma (Badaniem objęto 40 chorych (31 mężczyzn i 9 kobiet) w średnim wieku 60,2 ± 8,6 roku, z rozpoznaniem ostrego rozwarstwienia aorty, poddanych leczeniu chirurgicznemu w trybie nagłym, z zastosowaniem DHCA. Śródoperacyjnie czynność bioelektryczną mózgu monitorowano za pomocą badania CSA.W okresie śródoperacyjnym nie zmarł żaden pacjent. U 31 chorych (74%) podczas DHCA obserwowano ciszę bioelektryczną mózgu. Najniższą moc całkowitą aktywności bioelektrycznej mózgu stwierdzono podczas DHCA, odpowiednio, dla półkuli lewej 0,01 ± 0,05 nW oraz dla półkuli prawej 0,01 ± 0,03 nW. Wyniki testów neurologicznych uległy istotnie statystycznie pogorszeniu przede wszystkim u chorych, u których w okresie DHCA obserwowano aktywność bioelektryczną mózgu.Podczas operacji w głębokiej hipotermii do całkowitego zaniknięcia czynności bioelektrycznej mózgu dochodzi u ok. 75% chorych. Funkcje poznawcze u chorych operowanych w głębokiej hipotermii ulegają istotnemu pogorszeniu we wczesnym okresie pooperacyjnym szczególnie u chorych, u których podczas DHCA obserwowano aktywność bioelektryczną mózgu.

8. Evaluation of exercise capacity with cardiopulmonary exercise testing and BNP levels in adult patients with single or systemic right ventricles

Abstract

Introduction The aim of the study was to evaluate exercise capacity using cardiopulmonary exercise test (CpET) and serum B-type natriuretic peptide (BNP) levels in patients with single or systemic right ventricles. Material and methods The study group included 40 patients (16 males) – 17 with transposition of the great arteries after Senning operation, 13 with corrected transposition of the great arteries and 10 with single ventricle after Fontan operation, aged 19–55 years (mean 28.8 ±9.5 years). The control group included 22 healthy individuals (10 males) aged 23–49 years (mean 30.6 ±6.1 years). Results The majority of patients reported good exercise tolerance – accordingly 27 were classified in NYHA class I (67.5%), 12 (30%) in class II, and only 1 (0.5%) in class III. Cardiopulmonary exercise test revealed significantly lower exercise capacity in study patients than in control subjects. In the study vs. control group VO2max was 21.7 ±5.9 vs. 34.2 ±7.4 ml/kg/min (p = 0.00001), maximum heart rate at peak exercise (HRmax) 152.5 ±32.3 vs. 187.2 ±15.6 bpm (p = 0.00001), VE/VCO2 slope 34.8 ±7.1 vs. 25.7 ±3.2 (p = 0.00001), forced vital capacity (FVC) 3.7 ±0.9l vs. 4.6 ±0.3 (p = 0.03), forced expiratory volume in 1 s (FEV1) 3.0 ±0.7 vs. 3.7 ±0.9l (p = 0.0002) respectively. Serum BNP concentrations were higher in study patients than in control subjects; 71.8 ±74.4 vs. 10.7 ±8.1 (pg/ml) respectively (p = 0.00001). No significant correlations between BNP levels and CpET parameters were found. Conclusions Patients with a morphological right ventricle serving the systemic circulation and those with common ventricle physiology after Fontan operation show markedly reduced exercise capacity. They are also characterized by higher serum BNP concentrations, which do not however correlate with CpET parameters.

Abstract

Background: Pseudoaneurysm is a very rare and unusual form of myocardial rupture, with complications such as chest trauma, inflammation, acute myocardial infarction, and infection. Although this rare complication has already been reported, it has never been found in a transplanted patient., Case Report: We present the case of a 54-year-old women waiting on the urgent list who underwent heart transplantation. The donor of the organ died in a car accident. Although preoperative echocardiography had not revealed any signs of heart injury, a superficial small (3 × 3 mm hematoma) was detected on harvesting. After implantation, intraoperative echocardiography was satisfactory, with no signs of wall motion disturbances, and left ventricle ejection fraction was estimated at 50%. The postoperative period was uneventful. Three weeks after surgery, a left ventricle pseudoaneurysm was found on routine MRI. The aneurysm wall consisted of only an epicardial layer. There was an 8-mm-wide gap in the myocardial wall next to the endocardium and with the width of 4 mm beneath the epicardium. On repeated MRI performed 3 months thereafter, the pseudoaneurysm was filled by thrombus., Conclusions: The presented case illustrates the necessity of careful inspection of the organ reported for transplantation from a donor who died from high-speed motor vehicle crash injuries. Additional diagnostic steps like MRI imaging are obligatory after transplantation, especially when the organ was harvested from a motor vehicle crash victim.

Abstract

Ebstein anomaly is a congenital tricuspid valve and right ventricle malformation characterised by posterior and/or septal leaflet deformation that are malpositioned. The target of surgical intervention is to restore tricuspid valve function with right ventricle preservation but also to decrease the risk of cardiac arrhythmias. We present a case of a 56-year-old female patient with type C Ebstein anomaly. She underwent successful procedure of tricuspid valve repair with biodegradable ring.

20. Predictors of mortality in adult patients with congenital heart disease.

Abstract

Background: Most patients with congenital heart disease (CHD) reach adulthood thanks to the successful efforts of cardiac surgeons. However, sudden cardiac deaths are significantly more prevalent in this population, and survival is reduced when compared to the general population. The aim of this study is to define the prognostic value of selected clinical parameters to predict mortality in adult CHD patients referred to the specialist outpatient centre. The following parameters were analyzed as potential predictors of long-term survival: complexity of heart defect, past surgical intervention, heart failure (functional class according to NYHA > I), cyanosis, age and gender., Methods: We analyzed data gathered from 1,304 patients (568 male) aged 18 to 72 (mean 29.4 +/- 10.6) between 1995 and 2004. Mean duration of follow-up was 3.52 +/- 1.83 years., Results: During follow-up, 29 deaths were recorded (2.2%). Higher mortality was found in the group of patients with complex as oppposed to simple CHD (28 [6.7%] vs. 1 [0.1%]; p = 0.00001), in subjects without surgical correction as opposed to those operated on (21 [6.1%] vs. 8 [0.8%], p = 0.00001). General survival was 99.1% at two years and 96.6% at five years. In univariate survival analysis, all single clinical variables except patient gender were associated with increased risk of death (p = 0.00001 for all). All patients who died presented with heart failure. In multivariate analysis, the independent predictor of mortality was cyanosis (heart rate 38.1). Complexity of lesion (heart rate 6.4) represented a relative risk factor., Conclusions: Heart failure and cyanosis are negative predictors of survival in adult patients with CHD. Complexity of the lesion increases the relative risk of mortality. Past cardiac surgery is associated with better survival, but, as with age and gender, it is not a significant prognostic factor.
