Nia is a first-grade student with a history of trauma who was brought in by ambulance to the pediatric emergency department for “out of control behavior” at school. This is the first of multiple presentations to the emergency department for psychiatric evaluation, stabilization, and management throughout her elementary and middle school years. Several of the visits resulted in admission to the inpatient pediatric service, where she “boarded” while awaiting transfer to an inpatient psychiatric facility. At times, clinical teams used involuntary emergency medications and physical restraints, as well as hospital security presence at the bedside, to control Nia’s behavior. Nia is Black and her story is a case study of how structural racism manifests for an individual child. Her story highlights the impact of adultification bias and the propensity to mislabel Black youth with diagnoses characterized by fixed patterns of negative behaviors, as opposed to recognizing normative reactions to trauma or other adverse childhood experiences—in Nia’s case, poverty, domestic violence, and Child Protective Services involvement. In telling Nia’s story, we (1) define racism and discuss the interplay of structural, institutional, and interpersonal racism in the health care, education, and judicial systems; (2) highlight the impact of adultification bias on Black youth; (3) delineate racial disparities in behavioral health diagnosis and management, school discipline and exclusion, and health care’s contributions to the school-to-prison pipeline; and finally (4) propose action steps to mitigate the impact of racism on pediatric mental health and health care.

Assessment of learning outcomes is a challenging standard for journalism and mass communication programs accredited by the Accrediting Council on Education in Journalism and Mass Communications, typically accounting for half of all annual noncompliances. This study conducts the first content analysis on assessment plans from accredited programs using categories from a Council-generated guide as a best-practice document. Findings show that while most plans include many recommended components, and that every plan had direct learning measures, plans varied widely in component inclusion and assessment activities.

Objectives The relationship between blood group antigens and disease has been studied in humans. Blood types have been associated with both decreased and increased rates of various infections. In addition, blood group expression has been shown to vary with some cancers and gastrointestinal diseases. The objective of this study was to explore whether there is a relationship between blood type and retroviral infections in cats. Methods Case records from a veterinary research laboratory, veterinary teaching hospitals and veterinary blood banks were retrospectively searched for cats where both blood type and retroviral status (feline leukemia [FeLV], feline immunodeficiency virus [FIV] or both) were listed (part 1). In addition, a sample of 33 cats with confirmed FIV infection was genotyped to determine blood groups (part 2). Results In part 1, 709 cats were identified, 119 of which were positive for retroviral infection. Among all cases, 621 were type A (87.6%), 68 were type B (9.6%) and 20 were type AB (2.8%). There was no relationship between overall retroviral status (positive/negative) and blood type ( P = 0.43), between FeLV status and blood type ( P = 0.86) or between FIV status and blood type ( P = 0.94). There was no difference in the distribution of blood types between cats that were healthy and typed as possible blood donors vs sick cats that were typed prior to a possible transfusion ( P = 0.13). In part 2, of the 33 FIV-infected cats, all blood group genotypes were identified, although this test did not discriminate type A from type AB. Conclusions and relevance No relationship was identified between feline retroviral status and blood type in this study. The relationship between blood type and other disease states requires further study in veterinary patients.

In this article, an English professor and a sophomore-level English major explicate the singular difficulties of teaching and learning Virginia Woolf’s novel Mrs Dalloway mid-pandemic. These difficulties arise despite the fact that Mrs Dalloway would seem an ideal novel for our historical moment in the US. Woolf offers her readers searing insights into pandemic casualties, trauma, ruinous disillusionment with political systems, and radical isolation in a fragmented society. Working together, professor and student identify potent reasons why teaching and learning from this novel can be so difficult. We unpack a serious yet widely misunderstood gap between students’ and educators’ perspectives: a gap widened since 2020 by a combination of remote learning and social media consumption. We then recommend intellectual and pedagogical strategies that illuminate Woolf in ways not required before the pandemic, while also bridging perceptual gaps in the classroom between professors and students. Studying and interpreting Mrs Dalloway, a novel invested in illuminating myriad perspectives on PTSD, pandemic casualties, and political ruination, is difficult yet uniquely vital in this historical moment—though not for the reasons this professor expected.

OBJECTIVESTo examine how outpatient mental health (MH) follow-up after a pediatric MH emergency department (ED) discharge varies by patient characteristics and to evaluate the association between timely follow-up and return encounters.METHODSWe conducted a retrospective study of 28 551 children aged 6 to 17 years with MH ED discharges from January 2018 to June 2019, using the IBM Watson MarketScan Medicaid database. Odds of nonemergent outpatient follow-up, adjusted for sociodemographic and clinical characteristics, were estimated using logistic regression. Cox proportional hazard models were used to evaluate the association between timely follow-up and risk of return MH acute care encounters (ED visits and hospitalizations).RESULTSFollowing MH ED discharge, 31.2% and 55.8% of children had an outpatient MH visit within 7 and 30 days, respectively. The return rate was 26.5% within 6 months. Compared with children with no past-year outpatient MH visits, those with ≥14 past-year MH visits had 9.53 odds of accessing follow-up care within 30 days (95% confidence interval [CI], 8.75-10.38). Timely follow-up within 30 days was associated with a 26% decreased risk of return within 5 days of the index ED discharge (hazard ratio, 0.74; 95% CI, 0.63-0.91), followed by an increased risk of return thereafter.CONCLUSIONSConnection to outpatient care within 7 and 30 days of a MH ED discharge remains poor, and children without prior MH outpatient care are at highest risk for poor access to care. Interventions to link to outpatient MH care should prioritize follow-up within 5 days of an MH ED discharge.

“Do you think she’ll be ready to go home tomorrow?” The team turns toward the father’s 6-year-old daughter. This is her tenth admission for status asthmaticus, and she is now on day 2 of continuous albuterol. “I know the roaches make her asthma worse, but our landlord is useless,” he explains. “I pick up her medications as often as I can, but the inhaler costs $200 since I lost my job and insurance.” The team explains the necessary steps before discharge. Outside the room, after a period of silence, the attending says, “Let’s space her albuterol and get her out of bed today.” Walking to the next room, the attending adds, “We should reach out to social work too.” As pediatric hospitalists, we encounter patients and families like this every day. Although we are confident in our ability to treat a patient’s acute illness and teach trainees evidence-based medical management, too often, we miss opportunities to both model and teach the role of the pediatrician-advocate in the hospital. Pediatrics was the first medical specialty to include “advocacy” in the Accreditation Council of Graduate Medical Education (ACGME) program requirements for residency training. However, the “advocacy” requirement is formally designated as an “ambulatory experience,” significantly limiting its scope and impact.1 Advocacy and advocacy training can, does, and should occur across all pediatric clinical settings and specialties. The inpatient setting, where pediatric residents spend the majority of their time, is particularly well-suited to advocacy training. Intentional advocacy training in the inpatient setting would improve the skills of future pediatrician-advocates while also elevating the quality of inpatient medical care. To characterize trends in pediatric mental health visit counts, including visits for prolonged length of stay (LOS), in a sample of emergency departments (EDs) from 29 states during COVID-19.We performed a secondary analysis of the Clinical Emergency Data Registry from January 2020 through December 2021. We reported trends in pediatric mental health visit counts overall and for those with prolonged ED LOS. We reported incident rate ratios (IRRs) for monthly counts compared to January 2020. Among visits with LOS 24 hours, we reported on the most common diagnostic categories.There were 107 EDs from 29 states with available complete data in 2020 and 2021. Pediatric mental health visit counts resulting in a LOS greater than 6, 12, and 24 hours were higher for much of 2021. At their peak, there were 604 visits with LOS 12 hours (IRR, 2.14; 95% confidence interval [CI], 1.86-2.47) and 262 visits (IRR, 2.46; 95% CI, 1.97-3.09) with LOS 24 hours in April 2021. Pediatric mental health visits with LOS 12 hours and 24 hours made up 20.9% and 7.3% of pediatric mental health visits overall, respectively. For visits with ED LOS 24 hours, the most common diagnostic categories were suicide or self-injury, depressive disorders, and mental health syndrome.In this sample of 107 EDs in 29 states, visit counts with prolonged LOS 24 hours more than doubled in some months since the arrival of COVID-19. These findings are indicative of an increasingly strained emergency and mental health system.

Little is known about pediatric psychotropic medication use in the emergency department (ED), despite a rise in mental and behavioral health visits. This study describes psychotropic medication use in a nationally representative sample of pediatric mental and behavioral health ED visits over a 14-year period.We conducted a cross-sectional analysis of pediatric (6-17 years) mental and behavioral health ED visits using the National Hospital Ambulatory Medical Care Survey, 2006-2019. We describe administration of psychotropic medications by medication type, diagnosis, and over time. Using multivariable survey weighted logistic regression, we examine associations between medication administration and sociodemographics.A psychotropic medication was administered in 11.4% of the estimated 11,792,860 pediatric mental and behavioral health ED visits in our sample. Benzodiazepines were administered most frequently (4.9% of visits). Visits with anxiety disorders had the highest frequency of psychotropic medication use (26.7%). Visits by Black non-Hispanic patients had a 60% decreased odds of medication administration compared to visits for White non-Hispanic patients. Visits with public compared to private insurance had a 3.5 times increased odds of psychotropic polypharmacy. The proportion of visits in which a psychotropic medication was administered did not change statistically over time.A psychotropic medication was administered in one in ten pediatric mental and behavioral health ED visits. Use differed by sociodemographics but did not change over time. As more youth seek mental and behavioral healthcare in the ED, we must better understand appropriate medication use to ensure quality and equitable care.

Objective To systematically review available evidence to develop guidelines for the prevention of transfusion reactions and monitoring of transfusion administration in dogs and cats.Design Evidence evaluation of the literature (identified through Medline searches through Pubmed and Google Scholar searches) was carried out for identified transfusion reaction types in dogs and cats. Evidence was evaluated using PICO (Population, Intervention, Comparison, Outcome) questions generated for each reaction type. Evidence was categorized by level of evidence (LOE) and quality (Good, Fair, or Poor). Guidelines for prevention and monitoring were generated based on the synthesis of the evidence. Consensus on the final recommendations and a proposed transfusion administration monitoring form was achieved through Delphi-style surveys. Draft recommendations and the monitoring form were made available through veterinary specialty listservs and comments were incorporated.Results Twenty-nine guidelines and a transfusion administration monitoring form were formulated from the evidence review with a high degree of consensusConclusions This systematic evidence evaluation process yielded recommended prevention and monitoring guidelines and a proposed transfusion administration form. However, significant knowledge gaps were identified, demonstrating the need for additional research in veterinary transfusion medicine.

Background: Quality measures for accreditation, reimbursement and care monitoring have long focused on the delivery of effective care within health care institutions. As calls for greater health equity have emerged alongside understandings of the limited role that health care plays in health outcomes, researchers and practitioners seek quality measures that reflect critical components of health quality while also helping to address inequities in resources, access and risks that present with diverse patient populations. Our research sought to identify quality measures of importance to people living in the catchment area of our comprehensive cancer center and to determine community priorities for the development of a cancer health equity dashboard. Methodology: In-depth interviews were conducted with 15 community members and/or patients as part of a mixed methods project. A modified rapid analysis framework was used to review, analyze, categorize, and reduce data from interview transcripts. The research team also met with health care leadership whose responsibilities entailed quality measurement or reporting. Proxy member verification, through use of the hospital’s Patient and Family Advisory Council, helped refine reporting priorities. Results: Although most participants appreciated the health care quality and health equity definitions used by the hospital, thematic analysis revealed nine concepts that community members believed were missing from the definitions as well as perceived misalignments in the operationalization of definitions when aiming to provide quality cancer care. Community members believed more attention should be paid to communications (i.e. provider-patient/family and health system-community), costs (i.e. affordability, transparency, insurance navigation) and patient context (i.e. home life, work conditions, history with health care systems). Workforce diversity was also frequently identified as an important quality-related metric that could be reported and monitored over time. Finally, participants suggested using the cancer care continuum as a comprehensible way to organize quality monitoring efforts. Most respondents had difficulty recalling cancer center efforts focused on cancer prevention and early detection yet believed there were educational and prevention interventions that could help create more equitable conditions for future cancer patients. Conclusion: Demonstrating and addressing differences in quality perspectives among community members and health systems may improve perceptions of shared interest among catchment area stakeholders while also highlighting paths to address cancer disparities. Qualitative findings will be combined with quantitative data to create a roadmap for design of a cancer health equity dashboard to facilitate quality monitoring for different audiences. Grouped by implementation difficulty, researchers hope this roadmap will highlight opportunities for cancer leadership to better incorporate equity into existing measurement and operations demands. This cross-sectional study explores the use of physical restraint on youth at risk of harming themselves or others in the emergency department, stratified by race and ethnicity.

The emergence of the Omicron variant was accompanied by an acute increase in COVID-19 cases and hospitalizations in New York City. An increased incidence of COVID-19-associated croup in children during the Omicron wave has been recognized, suggesting that there may be other changes in clinical symptoms and severity. To better understand clinical outcomes and health care utilization in children infected with SARS-CoV-2 during the Omicron wave, we performed a cross-sectional study in pediatric patients aged ≤18 years who were tested for SARS-CoV-2 in pediatric emergency departments within a large medical system in New York City from 2 December 2021 to 23 January 2022. We described the clinical characteristics and outcomes of pediatric patients who presented to the pediatric emergency department and were hospitalized with SARS-CoV-2 infection during the Omicron wave in New York City. There were 2515 children tested in the ED for SARS-CoV-2 of whom 794 (31.6%) tested positive. Fifty-eight children were hospitalized for a COVID-19-related indication, representing 7.3% of all COVID-19-positive children and 72% of hospitalized COVID-19-positive children. Most (64%) children hospitalized for a COVID-19-related indication were less than 5 years old. Indications for hospitalization included respiratory symptoms, clinical monitoring of patients with comorbid conditions, and exacerbations of underlying disease. Eleven (19%) hospitalized children were admitted to the ICU and six (10%) required mechanical ventilation. Children infected with COVID-19 during the Omicron wave, particularly those less than 5 years old, were at risk for hospitalization. A majority of hospitalizations were directly related to COVID-19 infection although clinical indications varied with less than a half being admitted for respiratory diseases including croup. Our findings underscore the need for an effective COVID-19 vaccine in those less than 5 years old, continued monitoring for changes in clinical outcomes and health care utilization in children as more SARS-CoV-2 variants emerge, and understanding that children are often admitted for non-respiratory diseases with COVID-19.

BACKGROUND AND OBJECTIVES: Children seeking care in the emergency department (ED) for mental health conditions are at risk for prolonged length of stay (LOS). A more contemporary description of trends and visit characteristics associated with prolonged ED LOS at the national level is lacking in the literature. Our objectives were to (1) compare LOS trends for pediatric mental health versus non–mental health ED visits and (2) explore patient-level characteristics associated with prolonged LOS for mental health ED visits. METHODS: We conducted an observational analysis of ED visits among children 6 to 17 years of age using the National Hospital Ambulatory Medical Care Survey (2005–2015). We assessed trends in rates of prolonged LOS and the association between prolonged LOS and demographic and clinical characteristics (race and ethnicity, payer type, and presence of a concurrent physical health diagnosis) using descriptive statistics and survey-weighted logistic regression. RESULTS: From 2005 to 2015, rates of prolonged LOS for pediatric mental health ED visits increased over time from 16.3% to 24.6% (LOS >6 hours) and 5.3% to 12.7% (LOS >12 hours), in contrast to non–mental health visits for which LOS remained stable. For mental health visits, Hispanic ethnicity was associated with an almost threefold odds of LOS >12 hours (odds ratio 2.74; 95% confidence interval 1.69–4.44); there was no difference in LOS by payer type. CONCLUSIONS: The substantial rise in prolonged LOS for mental health ED visits and disparity for Hispanic children suggest worsening and inequitable access to definitive pediatric mental health care. Policy makers and health systems should work to provide equitable and timely access to pediatric mental health care.

Study population drawn from consecutive patients aged ≥18 admitted with COVID-19 from March 29thto August 1st, 2020. Patients excluded from the analysis if not their first admission with COVID-19, if they did not have SOFA score recorded within 24 hours of admission, if race and ethnicity data were not Non-Hispanic Black, Non-Hispanic White, or Hispanic, or if they had other missing data. The primary outcomes was SOFA score, with peak score within 24 hours of admission dichotomized as ResultsOf 2982 patients admitted with COVID-19, 2320 met inclusion criteria and were analyzed, of whom 1058 (45.6%) were Non-Hispanic White, 645 (27.8%) were Hispanic, and 617 (26.6%) were Non-Hispanic Black. Median age was 65.0 and 1226 (52.8%) were female. In univariate logistic screen and in full multivariate model, Non-Hispanic Black patients but not Hispanic patients had greater odds of an elevated SOFA score ≥6 when compared to Non-Hispanic White patients (OR 1.49, 95%CI 1.11-1.99).InterpretationCrisis standards of care utilizing the SOFA score to allocate medical resources would be more likely to deny these resources to Non-Hispanic Black patients.

BackgroundThe COVID-19 pandemic has had a devastating impact in the United States, particularly for Black populations, and has heavily burdened the healthcare system. Hospitals have created protocols to allocate limited resources, but there is concern that these protocols will exacerbate disparities. The sequential organ failure assessment (SOFA) score is a tool often used in triage protocols. In these protocols, patients with higher SOFA scores are denied resources based on the assumption that they have worse clinical outcomes. The purpose of this study was to assess whether using SOFA score as a triage tool among COVID-positive patients would exacerbate racial disparities in clinical outcomes.MethodsWe analyzed data from a retrospective cohort of hospitalized COVID-positive patients in the Yale-New Haven Health System. We examined associations between race/ethnicity and peak overall/24-hour SOFA score, in-hospital mortality, and ICU admission. Other predictors of interest were age, sex, primary language, and insurance status. We used one-way ANOVA and chi-square tests to assess differences in SOFA score across racial/ethnic groups and linear and logistic regression to assess differences in clinical outcomes by sociodemographic characteristics.ResultsOur final sample included 2,554 patients. Black patients had higher SOFA scores compared to patients of other races. However, Black patients did not have significantly greater in-hospital mortality or ICU admission compared to patients of other races.ConclusionWhile Black patients in this sample of hospitalized COVID-positive patients had higher SOFA scores compared to patients of other races, this did not translate to higher in-hospital mortality or ICU admission. Results demonstrate that if SOFA score had been used to allocate care, Black COVID patients would have been denied care despite having similar clinical outcomes to white patients. Therefore, using SOFA score to allocate resources has the potential to exacerbate racial inequities by disproportionately denying care to Black patients and should not be used to determine access to care. Healthcare systems must develop and use COVID-19 triage protocols that prioritize equity.

Objective To use a systematic, evidence-based consensus process to develop definitions for transfusion reactions in dogs and cats.Design Evidence evaluation of the literature was carried out for identified transfusion reaction types in dogs and cats. Reaction definitions were generated based on synthesis of human and veterinary literature. Consensus on the definitions was achieved through Delphi-style surveys. Draft recommendations were made available through industry specialty listservs and comments were incorporated.Results Definitions with imputability criteria were developed for 14 types of transfusion reactions.Conclusions The evidence review and consensus process resulted in definitions that can be used to facilitate future veterinary transfusion reaction research.

OBJECTIVES In pediatric emergency departments (EDs), racial/ethnic minorities are less likely to receive needed and timely care; however, clinical protocols have the potential to mitigate disparities. Neonatal fever management is protocolized in many EDs, but the timeliness to antibiotic administration is likely variable. We investigated the timeliness of antibiotic administration for febrile neonates and whether timeliness was associated with patients' race/ethnicity. METHODS Retrospective cross-sectional study of febrile neonates evaluated in one pediatric ED that uses an evidence-based guideline for the management of neonatal fever between March 2010 and December 2015. Primary outcome was time from ED arrival to antibiotic administration. Analysis of variance tests compared mean time with antibiotic administration across race/ethnicity. Multivariable linear regression investigated racial/ethnic differences in time to antibiotic administration after adjusting for patient demographics, timing of visit, the number of physicians involved, and ED census. RESULTS We evaluated 317 febrile neonates. Of the 269 patients with racial/ethnic data (84.9%), 54% were white non-Hispanic, 13% were black non-Hispanic, and 23% were Hispanic. The mean time to antibiotic administration was 204 minutes (range = 51-601 minutes). There was no significant association between patient race/ethnicity and time to first antibiotic administration. Emergency department census was significantly associated with timeliness. CONCLUSIONS There was a 10-hour range in the time to antibiotic administration for febrile neonates; however, variability in timeliness did not differ by race or ethnicity. This study demonstrates the need to further examine the role of protocols in mitigating disparities as well as factors that influence timeliness in antibiotic administration to febrile neonates.

Introduction The medical community recognizes the importance of confronting structural racism and implicit bias to address health inequities. Several curricula aimed at teaching trainees about these issues are described in the literature. However, few curricula exist that engage faculty members as learners rather than teachers of these topics or target interdisciplinary audiences. Methods We developed a longitudinal case conference curriculum called Health Equity Rounds (HER) to discuss and address the impact of structural racism and implicit bias on patient care. The curriculum engaged participants across training levels and disciplines on these topics utilizing case-based discussion, evidence-based exercises, and two relevant conceptual frameworks. It was delivered quarterly as part of a departmental case conference series. We evaluated HER's feasibility and acceptability by tracking conference attendance and administering postconference surveys. We analyzed quantitative survey data using descriptive statistics and qualitatively reviewed free-text comments. Results We delivered seven 1-hour HER conferences at our institution from June 2016 to June 2018. A mean of 66 participants attended each HER. Most survey respondents (88% or more) indicated that HER promoted personal reflection on implicit bias, and 75% or more indicated that HER would impact their clinical practice. Discussion HER provided a unique forum for practitioners across training levels to address structural racism and implicit bias. Our aim in dissemination is to provide meaningful tools for others to adapt at their own institutions, recognizing that HER should serve as a component of larger, multifaceted efforts to decrease structural racism and implicit bias in health care.

Background Pediatric mental and behavioral health disorders are a growing public health concern with immediate and long-term impacts for children, families, and society. Pediatricians are uniquely positioned, but often inadequately trained, to identify and address childhood mental and behavioral health needs. Design/Methods Two major learning domains were identified for a 2-week rotation in senior year: 1) screening, diagnosis, and treatment of depression, anxiety, and ADHD in outpatient pediatric settings; and 2) evaluation, de-escalation, and triage of children in acute mental health crises. Residents participate in repeated sessions in integrated behavioral health clinics, conduct screenings with social workers, perform intakes supervised by a child psychiatrist, and evaluate patients with acute mental health concerns in the emergency department with child psychiatrists, in the community with Boston Emergency Services Team, and in inpatient settings with the hospital's Behavioral Response Team. Self-directed learning activities provide individualized experiences tailored to residents’ interests. By random selection, half of senior residents will participate in the rotation during its first year. All senior residents were invited to complete a baseline survey at the beginning of the year evaluating their satisfaction with their behavioral health training thus far. Results Fourteen of 29 (48%) eligible seniors completed the baseline assessment, showing that residents were least satisfied with their training in titrating/initiating medications to treat depression and anxiety, and most satisfied with their training in agitation management (Figure). Thus far, 11 senior residents have participated in the rotation. Conclusions Many senior residents have received limited training in behavioral and mental health competencies. We developed a novel 2-week rotation that leverages hands-on experiences with child psychiatrists, social workers, and crisis response teams, as well as individualized self-directed learning, which we will follow-up with surveys to assess efficacy as our next steps.

OBJECTIVE To assess the effect of decreased platelet and WBC counts on platelet aggregation as measured by a multiple-electrode impedance aggregometer in dogs. ANIMALS 24 healthy dogs. PROCEDURES From each dog, 9 mL of blood was collected into a 10-mL syringe that contained 1 mL of 4% sodium citrate solution to yield a 10-mL sample with a 1:9 citrate-to-blood ratio. Each sample was then divided into unmanipulated and manipulated aliquots with progressively depleted buffy-coat fractions such that 2 to 3 blood samples were evaluated per dog. The Hct for manipulated aliquots was adjusted with autologous plasma so that it was within 2% of the Hct for the unmanipulated aliquot for each dog. All samples were analyzed in duplicate with a multiple-electrode impedance aggregometer following the addition of ADP as a platelet agonist. The respective effects of platelet count, plateletcrit, Hct, and WBC count on platelet aggregation area under the curve (AUC), aggregation, and velocity were analyzed with linear mixed models. RESULTS WBC count was positively associated with platelet AUC, aggregation, and velocity; blood samples with leukopenia had a lower AUC, aggregation, and velocity than samples with WBC counts within the reference range. Platelet count, plateletcrit, and Hct did not have an independent effect on AUC, aggregation, or velocity. CONCLUSIONS AND CLINICAL RELEVANCE Results indicated that WBC count was positively associated with platelet aggregation when ADP was used to activate canine blood samples for impedance aggregometry. That finding may be clinically relevant and needs to be confirmed by in vivo studies.

The purpose of this study was to evaluate this effect related to emergency and essential surgical care in central Haiti. Methods We conducted a retrospective review of operative logbooks from the Clinique Bon Sauveur in Cange, Haiti, from 2008 to 2010. We used Geographic Information Systems to map the home locations of all patients. Spearman's correlation was used to determine the relationship between surgical utilization and distance, and a multivariate linear regression model identified characteristics associated with differences in distances traveled to care. Results The highest annual surgical utilization rate was 184 operations/100 000 inhabitants. We found a significant inverse correlation between surgical utilization rate and distance from residence to hospital (r(s) = -0.68, p = 0.02). Although millions of women receive injuries from intimate partner violence (IPV) each year in the United States alone, there has been only limited research of acute injury patterns and the types, locations, and mechanisms of IPV injuries. The mechanism of being punched to the face with a fist resulting in blunt trauma—related injuries is most commonly reported. Strangulation, especially manual strangulation, is a frequently cited mechanism of injury; however, less is known about the types of injuries that result from strangulation. In general, clinicians should assess all patients who present for treatment of head, neck, and face injuries for IPV. There is little consistency between and much inaccuracy with medical terms used to describe types of injuries. To increase the accuracy and generalizability of findings from studies of acute IPV injuries, researchers need to use more standardized medical forensic terminology.

In the course of arguing "Against Interpretation," Susan Sontag claims that "[i]n place of a hermeneutics we need an erotics of art" (14). This challenge appears for the first time, without gloss or elaboration, as the last line of her essay. It is a com pelling but vague call for literary scholars?and good readers in general?to do something outside their ken: take a work on its own terms without constructing a paratext in which all the symbols are unpacked, all the meanings laid bare. Sontag identifies "interpretation" as "presupposing] a discrepancy between the clear mean ing of the text and the demands of (later) readers.... The modern style of interpreta tion excavates, and as it excavates, destroys; it digs 'behind' the text, to find a subtext which is the true one" (6). She also notes that "interpretation of this type indicates a dissatisfaction (conscious or unconscious) with the work, a wish to replace it by something else" (10). Many authors whom John Cowper Powys particularly admired, such as James Joyce and Henry James, wrote novels that invite and reward careful hermeneutic ex plication. Sontag names Joyce and James in her short list of authors "around whom thick encrustations of interpretation have taken hold" (8). Their novels operate on several levels of significance simultaneously, developing intricate, intellectual rela tions between implied author and authorial audience, prompting the latter's interpre tation largely through their progressive exploitation and resolution of various tensions and instabilities.1 But in response to the influence of his contemporaries, Powys composed several digressive, improbable, ecstatic fictions that employ a con spicuously unfamiliar system of narrative progression, baffling his authorial audi ence's efforts at interpretation. The neglect shown to Powys's work by most scholars and teachers alike may be seen as a symptom of this bafflement.

Little is known about pediatric surgical disease in resource-poor countries. This study documents the surgical care of children in central Haiti and demonstrates the influence of the 2010 earthquake on pediatric surgical delivery. METHODS: We conducted a retrospective review of operations performed at Partners in Health/Zanmi Lasante hospitals in central Haiti. RESULTS: Of 2,057 operations performed prior to the earthquake, 423 were pediatric (20.6%). Congenital anomalies were the most common operative indication (159/423 operations; 33.5%). Pediatric surgical volume increased significantly after the earthquake, with 670 operations performed (23.0% post-earthquake v. 20.6% pre-earthquake, p=.03). Acceptance of basic surgical care as an essential element of any properly functioning health system is growing. To justify investment in surgical interventions, donors require estimates of the economic benefit of treating surgical disease. The present study aimed to establish a methodology for valuing the potential economic benefit of surgical intervention using cleft lip and palate (CLP) in sub-Saharan Africa (SSA) as a model.Economic modeling of cleft lip and cleft palate (CLP) in SSA was performed with retrospective demographic and economic data from 2008. The total number of Disability-Adjusted Life-Years (DALYs) secondary to CLP in 2008 was calculated from accepted clefting incidence rates and disability weights taken from the Global Burden of Disease Project. DALYs were then converted to monetary terms ($US), using both a human capital approach and Value of a Statistical Life (VSL) approach.With the human capital approach, the potential economic benefit if all incident cases of CLP in SSA in 2008 were repaired at birth ranged from $252 million to $441 million. With VSL, the potential economic benefit of the same CLP repair would range from $5.4 billion to $9.7 billion.Cleft lip and cleft palate can have a substantial impact on the economic health of countries in the developing world. Further studies should be directed at quantifying the economic benefit of surgical interventions and quantifying their costs with an economically sound approach.

Forensic nursing experts and other health professionals often testify regarding the injuries women and men receive from violence. Bruise discoloration can provide an opportunity for health professionals to grossly estimate the age of impact and determine if it is consistent with a given reported history. However, there is much confusion in the literature and clinical practice as to the accuracy of dating bruises. This article will demonstrate that using the color of the bruise as the primary criteria to determine its age is not the best practice. The limited available research and related literature on the topic are flawed and inconsistent. Finally, recommendations will be made for future research and clinical practice.

Background Background rates of latent tuberculosis infection in low prevalence regions of Britain are unknown. These would be valuable data for interpreting positive IGRA results, and guiding cost-benefit analyses. The management of a large outbreak of tuberculosis occurring in a rural district hospital provided an opportunity to determine the background rates and epidemiology of IGRA-positivity amongst unselected hospital patients in a low-prevalence region of U.K. Methods As part of a public health surveillance project we identified 445 individuals exposed to the index cases for clinical assessment and testing by a TB-specific interferon-γ release assay (IGRA): T-Spot.TB. Uniquely, an additional comparator group of 191 age-matched individuals without specific recent exposure, but with a similar age distribution and demographic, were recruited from the same wards where exposure had previously occurred, to undergo assessment by questionnaire and IGRA. Results Rates of IGRA positivity were 8.7% (95%CI, 4.2-13, n=149) amongst unexposed patients, 9.5%(3.0-22, n=21) amongst unexposed staff, 22%(14–29, n=130) amongst exposed patients, 11%(6.1-16, n=142) amongst exposed staff. Amongst the individuals without history of recent exposure to the outbreak, IGRA-positivity was associated with prior TB treatment (OR11, P.04) and corticosteroid use (OR5.9, P.02). Background age-specific prevalences of IGRA-positivity amongst unexposed individuals were: age Conclusions Background rates of IGRA-positivity remain high amongst unselected white-Caucasian hospital inpatients in U.K. These data will aid interpretation of future outbreak studies. As rates peak in the 5th and 6th decade, given an ageing population and increasing iatrogenic immunosuppression, reactivation of LTBI may be a persistent hazard in this population for several decades to come.
