Wir stellen einen Patienten mit einer ungewöhnlichen dermatologischen Manifestation auf dem Boden eines neu beschriebenen hämatologischen Krankheitsbildes vor. Im Dezember 2020 erfolgte dessen stationäre Aufnahme mit seit Jahren bestehendem Hautausschlag, einhergehend mit rezidivierenden Fieberschüben und Leukopenien, zur Diagnostik und Therapie. Unter der Verdachtsdiagnose eines Sweet-Syndroms mit der Hauptdifferenzialdiagnose eines adulten Morbus Still erfolgten bereits im Vorfeld multiple Systemtherapien in anderen Kliniken, unter anderem mit Methotrexat, Anakinra, Canakinumab, Tocilizumab und zuletzt mit Dapson im Jahr 2020. Die Leukopenien wurden im Vorfeld am ehesten als medikamentenassoziiert gewertet (MTX, Metamizol). Im Rahmen eines weiteren stationären Aufenthaltes wurde bei einem erneuten Leukozyten- sowie Neutrophilenabfall einhergehend mit einem Fieberschub eine Umkehrisolation erforderlich. Medikamente als potenzielle Auslöser sowie ein infektiöser Fokus konnten ausgeschlossen werden. In Anbetracht der Neutropenien, dem Fieber und der mit einem Sweet Syndrom zu vereinbarenden Histologie erfolgte zum Ausschluss einer hämatoonkologischen Erkrankung eine Knochenmarksbiopsie. Bei fehlendem Nachweis einer Blastenvermehrung, zeigte sich ein am ehesten zu einer Myelodysplasie passender Befund. Die Arbeitsdiagnose lautete: atypisches Sweet-Syndrom in Assoziation mit einer nicht näher klassifizierbaren myelodysplastischen Krankheit. Es erfolgte eine Anbindung an die Hämatoonkologie der Charité, Campus Mitte, Berlin zur Einleitung einer Therapie mit Azacitidin (Vidaza®). Zu einem späteren Zeitpunkt erreichte uns die Information, dass mittels Genmutationsanalyse bei unserem gemeinsamen Patienten ein erst im Oktober 2020 neu beschriebenes Syndrom diagnostiziert werden konnte. Hierbei handelt es sich um die Erstbeschreibung des VEXAS-Syndroms, einer monogenen, hochfieberhaften, autoinflammatorischen Erkrankung des Erwachsenenalters, basierend auf einer somatischen Mutation resultierend in einen x-chromosomal lokalisierten E1-Enzym Defekt., We present the case of a patient treated in the Department of Dermatology of the Bundeswehrkrankenhaus Berlin with an unusual dermatological manifestation based on a newly described hematological condition. In December 2020, a patient with a skin rash that had been present for years, accompanied by recurrent fevers and leukopenia, was admitted to the hospital for diagnosis and therapy. Under the suspected diagnosis of a Sweet syndrome with the main differential diagnosis of an adult Still's disease, multiple systemic therapies had already been carried out in other hospitals, including Methotrexate, Anakinra, Canakinumab, Tocilizumab and most recently Dapsone in 2020. The leukopenias were considered most likely associated with prior medication (MTX, Metamizole). In the course of treatment, a reversal isolation was required due to a renewed leukocyte as well as neutrophil drop, also accompanied by a febrile episode. This time, no drugs were considered as potential triggers and an infectious focus was ruled out. Considering neutropenia, fever, and histology consistent with Sweet syndrome, a bone marrow biopsy was performed to exclude hemato-oncologic diseases. In the absence of blastic cyte proliferation, these findings were best consistent with myelodysplasia. Therefore, the working diagnosis was atypical Sweet syndrome associated with an unclassifiable myelodysplastic disease. We referred the patient to the Hematooncology Department of the Charité, Campus Mitte Berlin, to initiate a therapy with azacitidine (Vidaza®). Later on, we received the information that by means of gene mutation analysis, a syndrome newly described only in October 2020, could be diagnosed in our common patient. This is the first description of the VEXAS syndrome, a monogenic, highly febrile, autoinflammatory disease of adulthood, based on a somatic mutation resulting in an x-localized E1 enzyme defect.

Präsentiert wird eine multimorbide vorerkrankte Patientin aus dem Wundzentrum des Bundeswehrkrankenhauses Berlin mit einer chronisch venösen Insuffizienz und einem komplexen sowie langwierigem Heilungsprozess eines chronischen Ulcus cruris postthromboticum des linken Unterschenkels. Seit 2021 erfolgten insgesamt fünf stationäre Aufenthalte in unserer Klinik für Dermatologie. Neben dem klassischen Wundmanagement, bestehend aus antiseptischen und antibiotischen Therapien nach mikrobiologischen Resistogramm sowie einer gefäßstatusadaptierten Kompressionsbehandlung, erweiterten wir das Behandlungsspektrum nicht nur um Spalthaut-, sondern auch um Fischhauttransplantationen. Zudem wurde mit dem Ziel einer schnelleren Wundheilung eine lokale Kaltplasmatherapie im Bundeswehrkrankenhaus Berlin erstmalig angewendet. Wegen vorliegender HIV-Infektion mit assoziierter Panzytopenie war eine enge interdisziplinäre Zusammenarbeit mit den Kollegen der Infektiologie und Hämato-Onkologie in domo erforderlich. Darüber hinaus wurde die Patientin während einer der stationären Aufenthalte aufgrund einer neutropenie-assoziierten Sepsis mit Fieber und unklaren Fokus isolationspflichtig. Desweiteren präsentierte sich eine Exazerbation der Wundverhältnisse aufgrund einer SARS-CoV-2-Infektion. Rückblickend verdeutlichte dieser Fall die Notwendigkeit einer konsequenten Kompressionstherapie sowie die Adhärenz und Patienten-Compliance zur langfristigen Aufrechterhaltung von Therapieerfolgen bei Patienten mit chronisch venöser Insuffizienz assoziierten Ulzerationen., We present the case of a multimorbid patient from the wound center of the Bundeswehrkrankenhaus Berlin with a chronic venous insufficiency and a complex and protracted healing process of a chronic ulcus cruris postthrombotic of the left lower leg. Since 2021, five inpatient treatments in our Department of Dermatology have proved necessary. In addition to classical wound management based on antiseptic and antibiotic therapies according to the antibiotic sensitivity pattern and compression therapy according to the vascular status, we extended the treatment spectrum by skin grafts and fish skin grafts as well. Additionally, atmospheric plasma therapy was performed. In this case, close interdisciplinary collaboration with colleagues from the Department of Infectious Diseases and Haemato-oncology in our hospital has been necessary due to therapy for HIV with associated pancytopenia. Further on in one of the inpatient stays, it was required to isolate the patient due to neutropenia-associated sepsis with a fever of unclear focus. In another inpatient stay, worsening of the wound conditions associated due to a Covid-19 infection was observed. Retrospectively, this case highlights the urgency of consistent compression therapy, adherence, and patient compliance for long-term maintenance of therapeutic success within patients with leg ulcerations triggered by chronic venous insufficiency.

Chronic infection with the Hepatitis C Virus (HCV) is associated with the risk of progressive liver disease. Although, HCV treatment options and viral cure rates have tremendously increased over the last decade, all currently licensed combination therapies contain inhibitors of the replication complex NS5A. Resistance-associated substitutions (RAS) in NS5A can limit the efficacy of therapy; however, resistance testing is routinely not recommended for all patients. Notably, pan-genotypic combinations have been approved, however the correct identification of the HCV genotype is still required for treatment decisions and is a good predictor for treatment success.The aim of this study was the establishment of a pan-genotypic NS5A amplification method for reliable genotyping and simultaneous resistance testing in a fast and cheap routine diagnostic setup.Pan-genotypic degenerated nested PCR primer were designed and tested in 262 HCV-patients. The collection included samples from genotypes 1-7 and the median viral load was 1.07 × 10Amplification of the expected 747bp fragment was successful in 257 of 262 (98.1%) samples including samples1000 IU/ml. The direct comparison of the genotype information obtained with core sequencing to those obtained by NS5A prediction showed high concordance (97.3%) and discrepancies occurred only for relatively rare subtypes. Resistance analysis using Geno2PhenoWe successfully developed a routine diagnostic method for pan-genotypic amplification of NS5A. This amplicon can be used for simultaneous genotyping and resistance testing for enhancing and improving routine HCV diagnostic.
