- Abstract
INTRODUCTION: Dementia is a multifactorial disease with Alzheimer's disease (AD) and vascular dementia (VaD) pathologies making the largest contributions. Yet, most genome-wide association studies (GWAS) focus on AD. METHODS: We conducted a GWAS of all-cause dementia (ACD) and examined the genetic overlap with VaD. Our dataset includes 800,597 individuals, with 46,902 and 8702 cases of ACD and VaD, respectively. Known AD loci for ACD and VaD were replicated. Bioinformatic analyses prioritized genes that are likely functionally relevant and shared with closely related traits and risk factors. RESULTS: For ACD, novel loci identified were associated with energy transport (SEMA4D), neuronal excitability (ANO3), amyloid deposition in the brain (RBFOX1), and magnetic resonance imaging markers of small vessel disease (SVD; HBEGF). Novel VaD loci were associated with hypertension, diabetes, and neuron maintenance (SPRY2, FOXA2, AJAP1, and PSMA3). DISCUSSION: Our study identified genetic risks underlying ACD, demonstrating overlap with neurodegenerative processes, vascular risk factors, and cerebral SVD. Highlights: We conducted the largest genome-wide association study of all-cause dementia (ACD) and vascular dementia (VaD). Known genetic variants associated with AD were replicated for ACD and VaD. Functional analyses identified novel loci for ACD and VaD. Genetic risks of ACD overlapped with neurodegeneration, vascular risk factors, and cerebral small vessel disease.

RIVM rapport:De Nederlandse maatregelen tijdens de coronacrisis werkten goed om het aantal besmettingen met het virus SARS-Cov-2 te verminderen. Dat blijkt uit onderzoek van het RIVM. Dit waren maatregelen die ervoor zorgden dat mensen het virus zo min mogelijk aan elkaar doorgaven. Bijvoorbeeld door anderhalve meter afstand van elkaar te houden en de sluiting van scholen. De resultaten van dit onderzoek helpen het RIVM om bij een volgende grote ziekte-uitbraak snel te adviseren over maatregelen en beleid.? Hoe goed de contactmaatregelen werkten, is niet per losse maatregel te onderzoeken. Dat komt omdat er vaak meerdere tegelijk golden. Wel kun je kijken naar de effecten van verschillende pakketten van maatregelen. Voor Nederland bleek: hoe zwaarder het pakket aan maatregelen als geheel, hoe effectiever. Ook is het moment waarop de maatregelen ingingen heel belangrijk. Hoe sneller ze na het begin van de eerste coronagolf werden ingevoerd, hoe meer sterfgevallen zijn voorkomen. ? Uit het onderzoek komen een aantal adviezen over maatregelpakketten. Een daarvan is om bepaalde gegevens van besmette personen te verzamelen. Zoals wie de meest waarschijnlijke besmetter is, wat het meest waarschijnlijke moment van de besmetting is en de eerste ziektedag. Met deze informatie kan worden bepaald hoeveel tijd er zit tussen besmet raken en ziek worden. En hoe lang het duurt voordat een besmet persoon iemand anders besmet. Een ander advies is om zo snel mogelijk goed inzicht te krijgen hoe vaak mensen contact hebben met anderen in een samenleving en hoe mensen zich verplaatsen. Dit is nodig om beter te kunnen bepalen welke maatregelen het beste de verspreiding van een virus kunnen tegengaan.?? Vanaf januari 2021 is in Nederland gestart met vaccinatie tegen COVID19. In binnen- en buitenland is onderzocht in hoeverre de vaccins mensen hebben beschermd tegen COVID-19, de ziekte die het virus veroorzaakt. Daaruit blijkt dat gevaccineerde mensen minder vaak besmet raken dan ongevaccineerde mensen, The measures implemented in the Netherlands during the COVID-19 crisis were effective in reducing infections with the SARS-CoV-2 virus. This is the outcome of a study conducted by RIVM. The measures were intended to minimise the risk of people transmitting the virus to others, for example by means of social distancing and by closing schools. The results of this study will help RIVM in providing rapid advice on measures and policy in the event of a new major outbreak of disease.? The effectiveness of contact measures cannot be determined at the level of individual measures. This is because often several measures were in force at the same time. However, it is possible to look at the effects of different packages of measures. In the Netherlands it was found that the more severe the package of measures as a whole, the greater the effect. The timing of the measures was also very important. The faster the measures were introduced after the start of the first wave of infections, the more deaths they were able to prevent. ? The study has also yielded a number of recommendations on such packages of measures. One of these recommendations is to collect specific data on people who are infected, such as data on the most likely source of infection, the most likely time of exposure, and the first day of illness. This information can be used to calculate the amount of time between the moment of exposure and the onset of illness, and the time it takes before an infected person can infect another person. Another recommendation is to obtain a clear picture, as soon as possible, of the frequency of contacts between individuals in society and of the ways in which people move about. This information will improve our ability to identify the measures that are most effective in curbing the spread of a virus.?? Introduction: Real-world vaccine effectiveness (VE) estimates are essential to identify potential groups at higher risk of break-through infections and to guide policy. We assessed the VE of COVID-19 vaccination against COVID-19 hospitalization, while adjusting and stratifying for patient characteristics. Methods: We performed a test-negative case-control study in six Dutch hospitals. The study population consisted of adults eligible for COVID-19 vaccination hospitalized between May 1 and June 28, 2021 with respiratory symptoms. Cases were defined as patients who tested positive for SARS-CoV-2 by PCR during the first 48 h of admission or within 14 days prior to hospital admission. Controls were patients tested negative at admission and did not have a positive test during the 2 weeks prior to hospitalization. VE was calculated using multivariable logistic regression, adjusting for calendar week, sex, age, comorbidity and nursing home residency. Subgroup analysis was performed for age, sex and different comorbidities. Secondary endpoints were ICU-admission and mortality. Results: 379 cases and 255 controls were included of whom 157 (18%) were vaccinated prior to admis-sion. Five cases (1%) and 40 controls (16%) were fully vaccinated (VE: 93%; 95% CI: 81 - 98), and 40 cases (11%) and 70 controls (27%) were partially vaccinated (VE: 70%; 95% CI: 50-82). A strongly protective effect of vaccination was found in all comorbidity subgroups. No ICU-admission or mortality were reported among fully vaccinated cases. Of unvaccinated cases, mortality was 10% and 19% was admitted at the ICU. Conclusion: COVID-19 vaccination provides a strong protective effect against COVID-19 related hospital admission, in patients with and without comorbidity. (C) 2022 The Authors. The paper presents the influence of mechanical surface damage on the physicochemical properties of plates after implantation made of CrNiMo stainless steel, used in the treatment of anterior surface deformity of the chest. Analysis of the data allowed us to investigate the effect of implant design and condition of their surface on the results of chest deformation treatment. Results of electrochemical, impedance and surface wettability tests and SEM observations were compared with clinical observations. When removing the plates we found only slight inflammatory-periosteal reactions. On the basis of obtained results, it can be stated that plates, in spite of mechanical damage of the surface, were characterized by good corrosion resistance, a fact which is confirmed by the results of clinical evaluation.

25 páginas, 6 figuras, 2 tablas, Characterization of the genetic landscape of Alzheimer's disease (AD) and related dementias (ADD) provides a unique opportunity for a better understanding of the associated pathophysiological processes. We performed a two-stage genome-wide association study totaling 111,326 clinically diagnosed/'proxy' AD cases and 677,663 controls. We found 75 risk loci, of which 42 were new at the time of analysis. Pathway enrichment analyses confirmed the involvement of amyloid/tau pathways and highlighted microglia implication. Gene prioritization in the new loci identified 31 genes that were suggestive of new genetically associated processes, including the tumor necrosis factor alpha pathway through the linear ubiquitin chain assembly complex. We also built a new genetic risk score associated with the risk of future AD/dementia or progression from mild cognitive impairment to AD/dementia. The improvement in prediction led to a 1.6- to 1.9-fold increase in AD risk from the lowest to the highest decile, in addition to effects of age and the APOE ε4 allele., This work was funded by a grant (EADB) from the EU Joint Programme – Neurodegenerative Disease Research. De pneumokok is de belangrijkste veroorzaker van ernstige infecties als longontsteking, bloedvergiftiging en hersenvliesontsteking. Deze infecties komen het meest voor bij ouderen met minder weerstand en bij kinderen onder de vijf jaar. Bij ouderen veroorzaakt de bacterie vooral longontsteking. In Nederland worden ongeveer 10.000 patiënten per jaar in het ziekenhuis opgenomen vanwege een pneumokokkeninfectie. Vooral ouderen hebben een groter risico (10 tot 20 procent) om hieraan te overlijden. In februari 2018 heeft de Gezondheidsraad geadviseerd om mensen van 60-80 jaar elke vijf jaar een pneumokokkenvaccinatie (PPV23) aan te bieden. Het was de bedoeling dat hiervoor alle mensen van 60, 65, 70 of 75 jaar in de herfst van 2020 zouden worden uitgenodigd. Vanwege de COVID-19-pandemie heeft de Gezondheidsraad in april 2020 geadviseerd om de vaccinatie eerst aan te bieden aan de oudste leeftijdsgroepen (70-79-jarigen). Zij hebben zowel een grotere kans op ernstige pneumokokkeninfecties als op een ernstig verloop van COVID19. Aangezien er onvoldoende vaccins zijn om alle mensen van 70 tot 79 jaar in 2020 te vaccineren, worden in het najaar eerst mensen van 73 tot 79 jaar uitgenodigd. In december 2020 zal de Gezondheidsraad adviseren welke leeftijdsgroepen in 2021 voor de pneumokokkenvaccinatie in aanmerking komen. Als ondersteuning van dat advies heeft het RIVM nu de beschikbare en relevante wetenschappelijke informatie bijeengebracht. Het gaat onder meer om het aantal mensen dat in Nederland ziek wordt van pneumokokken vlak voor en tijdens de COVID-19 pandemie. Het aantal patiënten met pneumokokkenziekte is sinds april 2020 tot 80 procent lager dan voorgaande jaren. Dit komt waarschijnlijk doordat pneumokokken minder worden verspreid door de maatregelen tegen COVID-19. Ook staat er informatie in het rapport over nieuwe pneumokokkenvaccins die momenteel ontwikkeld worden.

Subcortical brain structures are integral to motion, consciousness, emotions and learning. We identified common genetic variation related to the volumes of the nucleus accumbens, amygdala, brainstem, caudate nucleus, globus pallidus, putamen and thalamus, using genome-wide association analyses in almost 40,000 individuals from CHARGE, ENIGMA and UK Biobank. We show that variability in subcortical volumes is heritable, and identify 48 significantly associated loci (40 novel at the time of analysis). Annotation of these loci by utilizing gene expression, methylation and neuropathological data identified 199 genes putatively implicated in neurodevelopment, synaptic signaling, axonal transport, apoptosis, inflammation/infection and susceptibility to neurological disorders. This set of genes is significantly enriched for Drosophila orthologs associated with neurodevelopmental phenotypes, suggesting evolutionarily conserved mechanisms. Our findings uncover novel biology and potential drug targets underlying brain development and disease.

Contains fulltext : 225469.pdf (publisher's version ) (Open Access), In 1993, a Haemophilus influenzae serotype b (Hib) conjugate vaccine was introduced in the Dutch national immunization program, resulting in a sharp decrease in invasive Hib disease. We used a population-based set of serum samples collected in the Netherlands in 2006-2007 (Pienter-II, 5696 sera) to assess the concentration of antibodies to the capsular polysaccharide of Hib, and compared the results with those obtained from a similar set collected in 1995-1996 (Pienter-I, 7837 sera). Post-primary vaccination serum samples from children aged 6-11 months from the Pienter-II study contained approximately 4-fold lower anti-Hib antibody concentrations than samples from children from the Pienter-I study. No such difference was found in post-booster samples from children older than 11 months of age. In Pienter-II, the proportion of children aged 6-11 months with anti-Hib antibody concentrations below the putative protective concentration of 0.15 µg/mL was 30%, which is significantly higher than in the Pienter-I study (12%). Fewer children in the Pienter-II group developed antibodies able to kill Hib in a serum bactericidal assay compared to the Pienter-I children. The cause of the lagged response in Pienter-II children remain uncertain, but lack of natural boosting, interference by the acellular pertussis vaccine, combining vaccines and acceleration of the schedule may have contributed.

49. Optimizing the use of the Men-ACWY conjugated vaccine to control the developing meningococcal W disease outbreak in the Netherlands, a rapid analysis

BackgroundThere is a developing outbreak of Neisseria meningitidis serotype W (MenW) in the Netherlands. In response, those aged 14 months and 14 years are vaccinated with the conjugated MenACWY vaccine. In the spring of 2018 we aimed to explore the impact of adding a one-off catch-up campaign targeting those aged 15-18 years on the transmission of MenW and the cost-effectiveness of such a campaign.MethodsWe estimated the growth rate of the MenW outbreak and quantified the impact of various targeted vaccination strategies on the reproductive number, and subsequently projected the future incidence with and without vaccination. Future cases were expressed in costs and QALYS and the incremental cost-effectiveness ratio was obtained.ResultsWe estimate a reproductive number of around 1.4 (95%CI 1.2-1.7) over the period February 2016-February 2018. Adding the catch-up campaign reduces the reproductive number five years earlier than without a catch-up campaign, to a level around 1.2. The vaccination campaign, including the catch-up, will prevent around 100 cases per year in our base case scenario. Given the projected impact and realistic assumptions on costs and QALYs, adding the catch-up can be considered cost-effective using a threshold of €20,000 per QALY.ConclusionAdding the catch-up campaign targeting those aged 15-18 brings the impact of vaccination on reducing transmission five years forward and directly prevents a high-incidence age group from carriage and disease. Such a campaign can be considered cost-effective. Our study did underpin the decision to introduce a catch-up campaign in spring 2019. Furthermore, our applied method can be of interest for anyone solving vaccine allocation questions in a developing outbreak.
