For masses of the spleen, which are mostly benign, accessory spleens, cysts and hemangiomas should be radiologically described; however, if confirmed further follow-up control is unnecessary. In the case of disseminated small masses, chronic inflammation and granulomatous diseases, such as tuberculosis and sarcoidosis should be considered in the differential diagnostics. Solid masses in the kidneys should always be further clarified, with the exception of a fat-rich angiomyolipoma. For cystic masses of the kidneys, the modified Bosniak classification for computed tomography or magnetic resonance imaging should be used. Masses of the adrenal glands greater than 10mm in size should be clarified further as well as those where fat is not detected, independent of the size and evidence of malignancy.Bei den zumeist benignen Raumforderungen der Milz sollten Nebenmilzen sowie Zysten und Hämangiome radiologisch beschrieben werden, bei sicherem Nachweis ist jedoch eine weitere Verlaufskontrolle unnötig. Bei disseminierten kleinen Raumforderungen sollten chronische Entzündungen sowie granulomatöse Erkrankungen wie Tuberkulose oder Sarkoidose differenzialdiagnostisch beachtet werden. Solide Raumforderungen der Niere sollten mit Ausnahme des fetthaltigen Angiomyolipoms unbedingt weiter abgeklärt werden. Bei zystischen Raumforderungen der Niere ist die modifizierte Bosniak Klassifikation für Computertomographie oder Magnetresonanztomographie zu verwenden. Raumforderungen der Nebennieren sollten ab einer Größe von 10 mm und bei fehlendem Nachweis von Fett abhängig von Größe und Malignomanamnese weiter abgeklärt werden.

Incidentalomas of the parenchymal organs of the abdomen, i.e. radiological findings in these organs that are not the primary focus of the clinical question, are frequent in this region of the body. In particular, findings presumed to be unimportant, such as cystic masses in the liver, the bile duct system or the pancreas, initially appear to be irrelevant in the diagnosis. For the liver we define the mostly clearly diagnosable simple cysts and hemangiomas as leave me alone lesions. Otherwise, we recommend a classification of incidentalomas into the three major categories (0.5 cm, 0.5-1.5 cm and1.5 cm) as well as an assessment with respect to clearly benign and suspect imaging characteristics in the context of a classification of patients into three different risk groups.Inzidentalome der parenchymatösen Organe des Abdomens, also radiologische Befunde dieser Organe, die nicht im primären Fokus der klinischen Fragestellung stehen, liegen in dieser Körperregion häufig vor. Gerade eher mutmaßlich unbedeutende Befunde, wie etwa zystische Raumforderungen der Leber, des Gallengangsystems oder des Pankreas, erscheinen zunächst bei der Befundung irrelevant. Für die Leber definieren wir die meist klar diagnostizierbaren einfachen Zysten und Hämangiome als Leave-me-alone-Läsion. Ansonsten empfehlen wir eine Einteilung der Inzidentalome in die 3 Größenkategorien (0,5 cm, 0,5–1,5 cm und1,5 cm) sowie eine Bewertung hinsichtlich eindeutig benigner und verdächtiger Bildcharakteristika im Kontext einer Einteilung der Patienten in 3 verschiedene Risikogruppen.

The new German S3 guidelines on ductal pancreatic adenocarcinoma were published for the first time on the homepage of the Association of the Scientific Medical Societies in Germany (AWMF) in December 2021 as a new update and thus the second update (first update 2013) and contain a large number of innovations in terms of diagnostics and treatment. The guidelines were organized and led by the German Society for Digestive and Metabolic Diseases (DGVS). In this article we would like to present the central content and innovations related to the radiological diagnostics of ductal pancreatic adenocarcinoma. The most important innovations are the highest recommendation strength in favor of computed tomography (CT) when assessing tumor spread as well as the adaptation of the European guidelines for cystic tumors by magnetic resonance imaging (MRI) with magnetic resonance cholangiopancreatography (MRCP) as the method of first choice. A further innovation is the implementation of a structured reporting of CT findings for describing the anatomical tumor resectability on the basis of the publication presented by the Abdominal Imaging Working Group and the Oncological Imaging Working Group of the German Radiological Society. There is no evidence for the implementation of radiological imaging in the field of aftercare or in the field of screening.Die neue Deutsche S3-Leitlinie zum exokrinen Pankreaskarzinom wurde als neues Update und damit 2. Aktualisierung (1. Aktualisierung 2013) im Dezember 2021 erstmalig auf der Homepage der Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften (AWMF) veröffentlicht und beinhaltet eine Vielzahl an Neuerungen bezüglich Diagnostik und Therapie. Die Leitlinie wurde federführend durch die Deutsche Gesellschaft für Verdauungs- und Stoffwechselkrankheiten (DGVS) organisiert. Im vorliegenden Artikel möchten wir die zentralen Inhalte und Neuerungen bezogen auf die radiologische Diagnostik des exokrinen Pankreaskarzinoms vorstellen. Die wichtigsten Neuerungen sind dabei die höchste Empfehlungsstärke „soll“ zugunsten der Computertomographie (CT) bei der Beurteilung der Tumorausbreitung sowie die Adaptation der europäischen Leitlinie für zystische Tumoren mit der Magnetresonanztomographie (MRT) mit Magnetresonanzcholangiopankreatikographie (MRCP) als Methode der ersten Wahl sowie die Implementation einer strukturierten Befundung der CT zur Beschreibung der anatomischen Tumorresektabilität auf der Basis der Befundvorlage der Arbeitsgemeinschaft (AG) Abdominelle Bildgebung und der AG Onkologische Bildgebung der Deutschen Röntgengesellschaft. Weder im Bereich der Nachsorge noch im Bereich des Screenings gibt es eine Evidenz zur Durchführung radiologischer Bildgebung.

Due to the widespread use of computed tomography (CT) and magnetic resonance imaging (MRI) asymptomatic incidental findings of the gastrointestinal (GI) tract are increasingly being recognized. Incidental findings are frequently not part of the primary inquiry from referring physicians but it is obligatory to make a diagnosis and to stipulate the need for further clarification. A multitude of incidental findings in the intestinal lumen, in the intestinal wall and in the adjacent mesentery or subperitoneal space are shown in CT and MRI of the GI tract. The last part of the two-part review addresses the gastrointestinal incidental findings in the intestinal wall and the adjacent sections. Extramural incidental findings occur as mesenteric inflammation, tumors and cysts. Many of the findings can be classified as benign and as "leave me alone lesions" do not necessitate any further clarification but in contrast others need a definitive clarification. Imaging plays a major role to differentiate interstitial edematous from necrotic pancreatitis and to describe relevant local complications for proper patient management and decision-making in acute pancreatitis.The revised Atlanta classification 2012 of acute pancreatitis divides patients into mild, moderately severe and severe groups. Computed tomography (CT) and magnetic resonance imaging (MRI) are well established for diagnosis and follow-up of acute pancreatitis and its complications METHODOLOGICAL INNOVATIONS AND ASSESSMENT: Based on the revised Atlanta classification, CT and MRI allow for adequate categorization of interstitial edematous and necrotic pancreatitis. Imaging and standardized reporting nomenclature enable confident differentiation of early and late complications in acute pancreatitis.The aim of this review article is to present an image-rich overview of different morphologic characteristics of acute pancreatitis and associated local complications by CT or MRI using case examples.KLINISCHES PROBLEM: Bildgebend ist die Differenzierung einer interstitiellen ödematösen von einer nekrotisierenden Form der akuten Pankreatitis und ihren lokalen Komplikationen entscheidend für Therapiewahl und den Krankheitsverlauf.Patienten mit der Diagnose einer akuten Pankreatitis werden nach der revidierten Atlanta-Klassifikation von 2012 klinisch einem leichten, mittelschweren oder schweren Verlauf zugeordnet. Bildgebend sind die Computertomographie (CT) und die Magnetresonanztomographie (MRT) in der Diagnostik und Verlaufskontrolle der akuten Pankreatitis und ihrer Komplikationen etabliert.Im Rahmen der revidierten Atlanta-Klassifikation erlauben CT und MRT die sichere Abgrenzung einer interstitiell-ödematösen von einer nekrotisierenden Pankreatitis. Diese Differenzierung geht einher mit der nomenklatorisch korrekten Einordnung der lokalen Früh- und Spätkomplikationen in der CT und MRT. EMPFEHLUNG FüR DIE PRAXIS: Der folgende Beitrag adressiert anhand von Fallbespielen die entscheidenden bilddiagnostischen CT- und MRT-Kriterien für die korrekte Befundeinordnung und -beschreibung der akuten Pankreatitis und ihrer lokalen Komplikationen.

There is currently no S3 guideline that contains statements on the diagnosis and treatment of acute, chronic and autoimmune pancreatitis in a single guideline.This article presents the most relevant recommendations relating to radiological imaging from the new "S3 guideline for pancreatitis".The guideline was developed by the DGVS (German Society for Digestive and Metabolic Diseases). After an online-based Delphi survey, the statements were discussed by an interdisciplinary expert team at a 3-day consensus conference and then agreed upon. The level of evidence scheme (version 2) of the Oxford Centre for Evidence-based Medicine was used.The new German "S3 guideline pancreatitis" comprises for the first time all entities of pancreatitis, acute, chronic and autoimmune pancreatitis and contains a large number of innovations with regard to diagnostic imaging and therapy. In addition to the evidence-based recommendations for diagnosis and therapy, a German-language glossary of the revised Atlanta classification on acute pancreatitis was adopted by consensus of the interdisciplinary guideline expert team. Furthermore, protocol recommendations for the performance of computed tomography (CT) are provided for the first time. In this article we introduce the key recommendations and innovations in the field of diagnostic radiologic imaging for acute, chronic and autoimmune pancreatitis.In the future, every radiologist should have deep knowledge of the recommended time intervals for radiological imaging in acute pancreatitis and know the German terms for the description of acute pancreatitis; furthermore they should be able to classify the morphologies to the appropriate stages.HINTERGRUND: Aktuell existiert keine S3-Leitlinie, die Statements zu Diagnostik und Therapie der akuten, chronischen und autoimmunen Pankreatitis in einer Leitlinie beinhaltet.In diesem Artikel werden die wichtigsten Empfehlungen aus der neuen, gemeinsamen „S3-Leitlinie Pankreatitis“ vorgestellt.Die Leitlinie wurde federführend organisiert durch die DGVS (Deutsche Gesellschaft für Verdauungs- und Stoffwechselkrankheiten). Nach einer onlinebasierten Delphi-Umfrage wurden die Statements auf einer 3‑tätgigen Konsensuskonferenz interdisziplinär diskutiert und anschließend konsentiert. Dabei wurde das Evidenzlevelschema in Version 2 des Oxford Centre for Evidence-based Medicine angewendet.Die neue Deutsche „S3-Leitlinie Pankreatitis“ führt erstmalig alle Entitäten der Pankreatitis, die akute, chronische und Autoimmunpankreatitis, in einer Leitlinie zusammen und beinhaltet eine Vielzahl an Neuerungen bezüglich Diagnostik und Therapie. Darüber hinaus wurden erstmals Protokollempfehlungen für die Durchführung der Computertomographie (CT) gegeben. Im vorliegenden Artikel werden die zentralen Inhalte und Neuerungen bezogen auf die radiologische Diagnostik der akuten, chronischen und autoimmunen Pankreatitis vorgestellt.Zukünftig sollte jeder Radiologe Kenntnisse über die empfohlenen Zeitintervalle zur radiologischen Bildgebung bei der akuten Pankreatitis haben sowie die deutschen Begrifflichkeiten für die Befundung kennen und die zur Anwendung kommenden Klassifikationen mit den jeweiligen Morphologien zu den entsprechenden Stadien zuordnen können.

Durch Verbesserung der Techniken der Pankreaschirurgie konnten die Mortalitat und die Morbiditat fur das Pankreaskarzinom deutlich gesenkt werden. Fur den Radiologen ist die Kenntnis der Operationstechniken von entscheidender Bedeutung. Nur so konnen postoperative Komplikationen sicher aufgedeckt und auch Lokalrezidive fruhzeitig detektiert werden. Die Komplikationen in der Folge eines pankreaschirurgischen Eingriffs konnen dabei schwerwiegend sein und erfordern oft einen radiologisch-interventionellen Eingriff. Da das Pankreaskarzinom selbst eine schwerwiegende Erkrankung mit schlechtem Funfjahresuberleben darstellt, welches nur durch eine R0-Resektion verbessert werden kann, ist es von entscheidender Bedeutung, diejenigen Patienten bildgebend zu identifizieren, die primar operabel sind oder potenziell durch eine neoadjuvante Chemotherapie in einen operablen Zustand versetzt werden konnen („borderline“), sowie inoperable Patienten nicht zusatzlich durch postoperative Komplikationen zu schwachen und sie stattdessen ohne unnotige Verzogerung einer palliativen Therapie zuzufuhren. Hierfur sind die Kenntnisse der Kriterien der Resektabilitat sowie der Inoperabilitat bei einem Pankreaskarzinom fur jeden onkologischen Radiologen heutzutage essenzielles Basiswissen.

IgG4-assoziierte Autoimmunerkrankungen sind seltene systemische Multiorganerkrankungen und konnen jedes Organsystem betreffen. Ihre Diagnosehaufigkeit nimmt mit zunehmendem Bekanntheitsgrad deutlich zu. Im Abdomen bietet das pankreatikohepatobiliare System ein wesentliches Organsystem fur die Auspragung IgG4-assoziierter Autoimmunerkrankungen. Im Vordergrund steht die Autoimmunpankreatitis Typ 1, weniger bekannt sind die IgG4-assoziierte Cholangitis und die IgG4-assoziierte Hepatopathie, die auch in Kombination auftreten konnen. Fur die Diagnostik der pankreatikohepatobiliaren IgG4-assoziierten Autoimmunerkrankungen existieren verschiedene, uberwiegend asiatische, histologisch zentrierte Diagnosesysteme wie HISORt oder ICDC (International Consensus Diagnostic Criteria), in welchen die Bildgebung jedoch einen wichtigen Stellenwert einnimmt. Neben der generalisierten Organschwellung sind weitere morphologische, aber auch funktionelle bildgebende Kriterien besonders fur die Autoimmunpankreatitis zunehmend bekannt geworden wie das „late enhancement“ oder das bildgebende Ansprechen auf eine Steroidtherapie. Auch die Magnetresonanzcholangiopankreatikographie (MRCP) und die endoskopische retrograde Cholangiopankreatikographie (ERP) konnen bei IgG4-assoziierten Autoimmunerkrankungen im pankreatikohepatobiliaren System wertvolle Hinweise auf deren Vorliegen liefern. IgG4-assozierte Autoimmunerkrankungen des pankreatikohepatobiliaren Systems sind eine seltene Erkrankungsgruppe, deren radiologisches Erscheinungsbild zu kennen auch zu einer zunehmenden Diagnosehaufigkeit fuhrt. Davon abzugrenzen sind die nicht nekrotisierende Pankreatitis und das Pankreaskarzinom, was oft schwierig ist, fur den Patienten aber eine wesentliche therapeutische Konsequenz nach sich zieht.

Hintergrund Akute und chronische Pankreatitis sind in der westlichen Welt zunehmende schwere Krankheitsbilder mit tiefgreifenden Konsequenzen fur den einzelnen Patienten sowie soziookonomisch. Der Beitrag radiologischer Bildgebung zur Diagnostik und Therapie beider Erkrankungsformen soll im vorliegenden Ubersichtsbeitrag diskutiert werden.

Das Pankreas- (PDAC) und Cholangiokarzinom (CCA) sind schwierig zu diagnostizierende gastrointestinale Tumoren, die haufig erst im spaten Stadium erkannt werden. Initial sind die Beschwerden unspezifisch, was die Diagnosestellung verzogert. Fast immer sind mehrere diagnostische Methoden notwendig, um sicher die Erkrankung und ihr Stadium festzustellen. Ziel der Arbeit ist darzustellen, wie das PDAC und das CCA leitliniengerecht diagnostiziert werden. Zudem werden neuere Techniken der Diagnosefindung diskutiert. Ubersicht uber gangige und zukunftige diagnostische Verfahren, ihre Moglichkeiten und Grenzen auch in Hinblick auf differenzialdiagnostisch infrage kommende andere Erkrankungen. Die Diagnose eines PDAC ist laborchemisch nicht moglich, sondern erfordert einen multimodalen diagnostischen Ansatz. Am Anfang steht fast immer die abdominelle Sonographie, dann folgt oft eine Computertomographie (CT), ein endoskopischer Ultraschall (EUS) und/oder eine Magnetresonanztomographie (MRT), in Abhangigkeit von der Verfugbarkeit der apparativen Diagnostik und der Expertise der Untersucher. Neuere Verfahren wie die transiente Elastographie bedurfen einer weiteren Evaluation. Die Diagnose des CCA beruht ebenfalls auf bildgebenden und endoskopischen Verfahren, v. a. der MRT und der endoskopisch-retrograden Cholangiographie (ERC). Die einzelnen diagnostischen Methoden weisen spezifische Starken und Schwachen auf, sodass zur sicheren Diagnose des PDAC und CCA meist eine Kombination der Verfahren eingesetzt wird. Aufgrund der insgesamt schlechten Prognose der Tumoren sind neue diagnostische Methoden zur fruhzeitigen Diagnose von besonderem Interesse.

By improving the techniques of pancreatic surgery, the mortality and morbidity for pancreatic carcinoma could be significantly reduced. For radiologists a profound knowledge of the surgical techniques is of decisive importance. Based on this knowledge postoperative complications can be reliably uncovered and local recurrences can be detected at an early stage. The complications resulting from pancreatic surgery can be severe and often necessitate a radiological intervention. As pancreatic cancer itself is a severe disease with a poor 5‑year survival, which can only be improved by an R0 resection, it is crucial to identify using imaging those patients who are primarily operable or who can potentially achieve an operable condition through neoadjuvant chemotherapy (borderline) and inoperable patients to avoid postoperative complications which would additionally weaken them and result in unnecessary delays in initiating palliative therapy. Thus, familiarity with the clinical criteria of resectability and also inoperability in pancreatic cancer nowadays represents an essential basic knowledge for every oncological radiologist.

IgG4-related diseases are rare systemic multiorgan diseases and can thus affect any organ system. The incidence of diagnosis has significantly increased due to increasing awareness.In the abdomen the hepatopancreaticobiliary system provides an essential organ system for the expression of IgG4-associated autoimmune diseases. The focus here is autoimmune pancreatitis type 1 but IgG4-associated sclerosing cholangitis and IgG4-associated hepatopathy, which can also occur in combination are less well-known.Various mostly Asiatic, histologically-based diagnostic systems, such as HISORt or international consensus diagnostic criteria (ICDC) are available for the diagnostics of hepatopancreaticobiliary IgG4-related autoimmune diseases, in which imaging techniques playing an increasingly important role.In addition to generalized organ swelling further morphological and also functional imaging criteria have become increasingly well-known for autoimmune pancreatitis, such as late enhancement or the imaging response to steroid therapy. Magnetic resonance cholangiopancreatography (MRCP) and endoscopic retrograde cholangiopancreatography (ERP) can provide valuable information for the diagnostics of IgG4-related diseases in the hepatopancreaticobiliary system.IgG4-related autoimmune diseases of the hepatopancreaticobiliary system are a rare group of diseases in which increasing knowledge of the radiological appearance also leads to an increasingly frequency of diagnosis. IgG4-related diseases must be distinguished from non-necrotizing pancreatitis and pancreatic cancer, which is often difficult but has significant therapeutic consequences for the patients.

Acute and chronic pancreatitis are becoming increasingly more severe diseases in the western world with far-reaching consequences for the individual patient as well as the socioeconomic situation. This article gives an overview of the contribution of radiological imaging to the diagnostics and therapy of both forms of the disease.Acute pancreatitis can be subdivided into severe (20%) and mild manifestations. The diagnostics should be performed with computed tomography (CT) or magnetic resonance imaging (MRI) for assessing necrosis or potential infections only in severe forms of pancreatitis. In chronic pancreatitis transabdominal ultrasound should initially be adequate for assessment of the pancreas. For the differential diagnosis between pancreatic carcinoma and chronic pancreatitis, MRI with magnetic resonance cholangiopancreatography (MRCP) followed by an endoscopic ultrasound-guided fine needle aspiration is the method of choice.For the primary diagnosis for acute and chronic pancreatitis ultrasound examination is the modality of first choice followed by radiological CT and MRI with MRCP examinations.

Involvement of the liver is one of the most common injuries in addition to those of the pancreas following blunt force abdominal trauma. Due to the even now high mortality radiological imaging must provide a rapid, definitive and exact assessment of the extent of the damage. Despite conflicting study results ultrasound has become established as a rapid and relatively simple method in emergency room treatment and is irreplaceable for initial orientation diagnostics. The use of contrast medium-assisted examinations promises to be an advantage for diagnostics in the secondary phase. Due to the high sensitivity and specificity modern multidetector computed tomography is the most effective examination modality for the detection of liver damage and hepatobiliary complications and plays a central role in non-operative management following abdominal trauma. Shorter and shorter scan times even allow the examination of metastable patients and a rapid assessment even of large body volumes.

Die Computertomographie, v. a. mit Mehrzeilen-Technologie, hat ihren festen Platz im Staging von Malignomen des Magens. Grose, wanduberschreitend wachsende Tumoren werden am besten mit Hilfe der sog. „Hydro-CT“ klassifiziert. Dabei wird unter medikamentoser Spasmolyse der Magen durch Gabe von 1–1,5 l Wasser als „negatives“ Kontrastmittel maximal distendiert und so die Magenwand zur optimalen Diagnostik artefaktfrei aufgespannt. Die Hydro-CT hat eine Nachweisrate von Magenkarzinomen von 89–94% und Sensitivitaten im Nachweis von Lebermetastasen von 85–92%. Im T-Staging erreicht die CT nur Sensitivitaten zwischen 43 und 65%, im Lymphknoten-Staging 64–88%. Die Multislice-CT (MSCT) kann aber zuverlassige morphologische Hinweise auf die Entitat (Polypen, Gastritis, Lymphom, GIST, Karzinom) liefern. Der Einsatz von koronaren Rekonstruktionen (MPR), wie sie bei der MSCT erstellt werden konnen, ist fur die Lokalisationsdiagnostik und Zuordnung zu den Gefasen sehr hilfreich. Die MRT spielt eine eher untergeordnete Rolle bei der Diagnostik im oberen GI-Trakt.

We investigated the technical feasibility and clinical results of bone fixation techniques in combination with needle suspension for correction of female stress urinary incontinence. In our experience the screw-like bone anchor, which is drilled into the public tubercle, represents a minimally invasive but very stable and reliable technique. However, the needle suspension fixed to the bone anchor turned out to be critical. Even though the suspension was fixed in the paraurethral tissue with a deep Z-stitch between the bladder neck and the midurethra, the 1-year recurrence rate was 76%. Our data showed that the suspension sutures pull through the paraurethral tissue because there is no paravesical scar formation as in open procedures. Modifications of the suspension technique (four-point suspension, simultaneous laparoscopic or digital dissection of the paravesical space, combination with sling procedures) revealed significantly improved short-term results. Therefore we conclude that after improvement of the suspension technique the bone anchor will represent a valid option for minimally invasive fixation of a bladder neck suspension.

The new German S3 guideline regarding stomach cancer includes a variety of innovations with respect to the diagnosis and treatment of adenocarcinoma of the stomach and the esophagogastric junction. The guideline has been strongly supported by the "oncology" guidelines program consisting of the "Deutsche Krebshilfe" and the German Cancer Society and the AWMF (Dr. M. Follmann). This guideline contains evidence-based treatment recommendations and quality indicators for guideline implementation and evaluation in order to improve broad medical care and to facilitate development and subsequent adjustment. The purpose of this article is to introduce the innovations with regard to radiological diagnosis and to discuss the latest literature on the subject.

The compatibility of family and profession is especially difficult for employees in medical professions because of shift work and overtime. It seems that in the future women are going to represent the majority of medical professionals in Germany. Hence, with the forthcoming lack of physicians in Germany social aspects will also play a greater role in the choice of the place of employment. Therefore a statistical survey was made among employees on how they judge the compatibility of family and job and what they would like to improve.From autumn 2009 until spring 2010 a total of 115 questionnaires were distributed to 8 different academic radiology departments. The anonymous questionnaire with partially open, partially graded questions and partially multiple answer questions was designed with the help of an expert for statistics and analytics and included questions about the employment and family situation, plans for the future, requested flexible working hours and childcare models, as well as ideas for improvement. Of the questionnaires 87 were analyzed with a specially designed access database using, for example descriptive statistics and histogram analyses.Of the interviewees 68% were female and 31% were male (1% not significant n.s.), 46% had children and 49% were childless (5% n.s.), 63% were medical doctors, 33% radiographers (3% other) and 82% worked full-time. Of the male respondents with children 42% indicated that their spouse was at home, 18% of female respondents with children indicated that their spouse was at home and only mothers worked part-time. Of the male respondents 73% would like to take parental leave, 44% of all respondents (70% of the male respondents and 34% of the female respondents) agreed that radiology is more compatible with family than other medical disciplines and 87% would like to have a childcare possibility in close proximity to the working place.In most of the families the classic role model prevails, although women are well-educated and men also set a high value on the compatibility of family and profession and would like to take parental leave and work flexible working hours. This is a chance especially for radiology.

In addition to focal liver lesions, diffuse and vascular disorders of the liver represent a wide spectrum of liver diseases which are from the radiological point of view often difficult or nearly impossible to diagnose. Classical diagnostic methods are computed tomography and magnetic resonance imaging in addition to ultrasound. Diffuse parenchymal damage caused by diseases of various etiologies is therefore difficult to evaluate because it often lacks characteristic morphological features. For hepatic steatosis, hemochromatosis/siderosis as an example of a diffuse storage disease and sarcoidosis and candidiasis as infectious/inflammatory diseases, an image-based diagnosis is appropriate in some cases. For most diffuse liver diseases, however only nonspecific changes are visualized. Vascular pathologies of the liver, such as the Budd-Chiari syndrome and portal vein thrombosis, however, can usually be diagnosed very clearly using radiology and there is also a very effective interventional radiological treatment. Chronic diseases very often culminate in liver cirrhosis which is highly associated with an increased risk of liver cancer.
