This is caused mainly by aging of populations and improvement in the treatment of coronary artery disease. As HF patients age, they tend to develop comorbidities, creating new problems for health-care professionals. Sarcopenia, defined as the loss of muscle mass and function, and cachexia, defined as weight loss due to an underlying illness, are muscle wasting disorders of particular relevance in the heart failure population, but they go mostly unrecognized. The coexistence of chronic HF and metabolic disorders facilitates the development of cachexia. Cachexia, in turn, significantly worsens a patient’s prognosis and quality of life. The mechanisms underlying cachexia have not been explained yet and require further research. Understanding its background is crucial in the development of treatment strategies to prevent and treat tissue wasting. There are currently no specific European guidelines or recommended therapy for cachexia treatment in HF (“cardiac cachexia”).Keywords: cardiac cachexia, heart failure, nutritional status

Abstract

The reaction pp -> K^+ + (Lambda p) has been measured at T_p = 1.953 GeV and \Theta = 0 deg with a high missing mass resolution in order to study the Lambda p final state interaction. Narrow S = -1 resonances predicted by bag model calculations are not visible in the missing mass spectrum. Small structures observed in a previous experiment are not confirmed. Abstract

The cross section for the reaction $p+^6\text{Li}\to\eta+^7\text{Be}$ was measured at an excess energy of 11.28 MeV above threshold by detecting the recoiling $^7$Be nuclei. A dedicated set of focal plane detectors was built for the magnetic spectrograph Big Karl and was used for identification and four momentum measurement of the $^7$Be. A differential cross section of $\frac{d\sigma}{d\Omega}=(0.69\pm{0.20}\text{(stat.)}\pm 0.20\text{(syst.)})\text{nb/sr}$ for the ground state plus the 1/2$^-$ was measured. Abstract

We have measured inclusive data on $K^+$-meson production in $pp$ collisions at COSY J\"ulich close to the hyperon production threshold and determined the hyperon-nucleon invariant mass spectra. The spectra were decomposed into three parts: $\Lambda p$, $\Sigma^0p$ and $\Sigma^+n$. The cross section for the $\Sigma^+n$ channel was found to be much smaller than a previous measurement in that excess energy region. Abstract

Abstract

Inclusive measurements of pion production in proton--proton collisions in the forward direction were undertaken at 400 and 600 MeV at COSY using the Big Karl spectrograph. The high resolution in the $\pi^+$ momentum ensured that there was an unambiguous separation of the $pp\to {\pi}^+d/\pi^+pn$ channels. Using these and earlier data, the ratio of the production cross sections could be followed through the $\Delta$ region and compared with the predictions of final state interaction theory. Deviations are strongly influenced by long-range terms in the production operator and the tensor force in the final $pn$ system. These have been investigated in a realistic $pp\to\pi^+d/\pi^+pn$ calculation that includes $S \rightleftharpoons D$ channel coupling between the final nucleons. A semi-quantitative understanding of the observed effects is achieved., Comment: 10 pages, 3 figures

Abstract

The angular distributions of the unpolarised differential cross section and tensor analysing power $A_{xx}$ of the $\vec{d}d\to\alpha \eta$ reaction have been measured at an excess energy of 16.6 MeV. The ambiguities in the partial-wave description of these data are made explicit by using the invariant amplitude decomposition. This allows the magnitude of the s-wave amplitude to be extracted and compared with results published at lower energies. In this way, firmer bounds could be obtained on the scattering length of the $\eta \alpha$ system. The results do not, however, unambiguously prove the existence of a quasi-bound $\eta \alpha$ state., Comment: 20 pages, 11 figures

Abstract

A large acceptance scintillator detector with wavelength shifting optical fibre readout has been designed and built to detect the decay particles of $\eta$-nucleus bound system (the so-called $\eta$-mesic nuclei), namely, protons and pions. The detector, named as ENSTAR detector, consists of 122 pieces of plastic scintillator of various shapes and sizes, which are arranged in a cylindrical geometry to provide particle identification, energy loss and coarse position information for these particles. A solid angle coverage of $\sim$95% of total 4$\pi$ is obtained in the present design of the detector. Monte Carlo phase space calculations performed to simulate the formation and decay of $\eta$-mesic nuclei suggest that its decay particles, the protons and pions are emitted with an opening angle of 150$^\circ \pm 20^\circ$, and with energies in the range of 25 to 300 MeV and 225 to 450 MeV respectively. The detailed GEANT simulations show that $\sim$ 80 % of the decay particles (protons and pions) can be detected within ENSTAR. Several test measurements using alpha source, cosmic-ray muons etc. have been carried out to study the response of ENSTAR scintillator pieces. The in-beam tests of fully assembled detector with proton beam of momentum 870 MeV/c from the Cooler synchrotron COSY have been performed. The test results show that the scintillator fiber design chosen for the detector has performed satisfactorily well. Abstract

Several processes of meson production in proton-deuteron collisions have been measured simultaneously using a calibrated magnetic spectrograph. Among these processes, the $\eta$ meson is seen clearly as a sharp missing--mass peak on a slowly varying background in the $p+d\to ^3\textrm{He} +X$ reaction. Knowing the kinematics of the other reactions with well determined masses, it is possible to deduce a precise mass for the $\eta$ meson. The final result, $m(\eta)=547.311\pm 0.028 \textrm{(stat)} \pm 0.032 \textrm{(syst) MeV/c}^2$, is significantly lower than that found by the recent NA48 measurement, though it is consistent with values obtained in earlier counter experiments., Comment: 10 pages, 6 figures, Fig. 3 changed

Abstract

Objectives: The aim of this study was to identify independent predictors of procedural success after retrograde chronic total occlusion (CTO) percutaneous coronary intervention (PCI). Background: Retrograde CTO PCI is an established technique, but predictors of success remain poorly understood. Methods: A multivariable logistic regression model was used to analyze potentially important demographic, clinical, anatomical, and technical aspects of retrograde CTO PCI cases uploaded to the multicenter European CTO (ERCTO) Club Registry. Results: In calendar years 2018 and 2019, 2,364 retrograde CTO PCI cases constituted the primary analysis cohort. A primary retrograde strategy was used in 1,953 cases (82.6%), and an initial antegrade approach was converted to retrograde in 411 cases (17.4%). Procedural success was achieved in 1,820 cases (77.0%) and was more likely to occur after a primary retrograde attempt versus conversion from an initial antegrade approach (80.9% vs 58.4%; P < 0.0001). After multivariable analysis, an absence of lesion calcification (OR: 1.86; 95% CI: 1.37-2.51; P < 0.0001), a higher degree of distal vessel opacification (OR: 2.47; 95% CI: 1.72-3.55; P < 0.0001), little or no proximal target vessel tortuosity (OR: 1.84; 95% CI: 1.28-2.64; P = 0.001), Werner collateral connection CC1 (OR: 4.87; 95% CI: 2.90-8.19; P < 0.0001) or CC2 (OR: 5.33; 95% CI: 3.02-9.42; P < 0.0001), and the top tertile of operator volume (>120 cases over 2 years) (OR: 1.88; 95% CI: 1.26-2.79; P = 0.002) were associated with the greatest chance of achieving angiographic success. Conclusions: Less calcification with good distal vessel opacification, little or absent proximal vessel tortuosity, and visible collateral connections, along with high-volume operator status, were all independently predictive of angiographically successful retrograde CTO PCI.

Abstract

The worldwide incidence of abnormally invasive placenta is rapidly rising, following the trend of increasing cesarean delivery. It is a heterogeneous condition and has a high maternal morbidity and mortality rate, presenting specific intrapartum challenges. Its rarity makes developing individual expertise difficult for the majority of clinicians. The International Society for Abnormally Invasive Placenta aims to improve clinicians’ understanding and skills in managing this difficult condition. By pooling knowledge, experience, and expertise gained within a variety of different healthcare systems, the Society seeks to improve the outcomes for women with abnormally invasive placenta globally. The recommendations presented herewith were reached using a modified Delphi technique and are based on the best available evidence. The evidence base for each is presented using a formal grading system. The topics chosen address the most pertinent questions regarding intrapartum management of abnormally invasive placenta with respect to clinically relevant outcomes, including the following: definition of a center of excellence; requirement for antenatal hospitalization; antenatal optimization of hemoglobin; gestational age for delivery; antenatal corticosteroid administration; use of preoperative cystoscopy, ureteric stents, and prophylactic pelvic arterial balloon catheters; maternal position for surgery; type of skin incision; position of the uterine incision; use of interoperative ultrasound; prophylactic administration of oxytocin; optimal method for intraoperative diagnosis; use of expectant management; adjuvant therapies for expectant management; use of local surgical resection; type of hysterectomy; use of delayed hysterectomy; intraoperative measures to treat life-threatening hemorrhage; and fertility after conservative management.

Abstract

Importance It is unknown whether coronary revascularization, when added to optimal medical therapy, improves prognosis in patients with stable ischemic heart disease (SIHD) at increased risk of cardiovascular events owing to moderate or severe ischemia. Objective To describe baseline characteristics of participants enrolled and randomized in the International Study of Comparative Health Effectiveness With Medical and Invasive Approaches (ISCHEMIA) trial and to evaluate whether qualification by stress imaging or nonimaging exercise tolerance test (ETT) influenced risk profiles. Design, Setting, and Participants The ISCHEMIA trial recruited patients with SIHD with moderate or severe ischemia on stress testing. Abstract

The increased risk of non-cardiovascular death in patients receiving clopidogrel or prasugrel in comparison with the placebo group in the Dual Antiplatelet Therapy (DAPT) trial in contrast to the decreased risk of cardiovascular death and all-cause death seen in patients treated with low-dose ticagrelor in the EU label population of the PEGASUS-TIMI 54 trial, resulted in inclusion in the 2020 ESC NSTE-ACS guidelines the recommendation for use of clopidogrel or prasugrel only if the patient is not eligible for treatment with ticagrelor. The prevalence of the primary outcome composed of cardiovascular death, stroke, or myocardial infarction was lower in the low-dose rivaroxaban and acetylsalicylic acid (ASA) group than in the ASA-alone group in the COMPASS trial. Moreover, all-cause mortality and cardiovascular mortality rates were lower in the rivaroxaban-plus-ASA group. Comparison of the PEGASUS-TIMI 54 and COMPASS trial patient characteristics clearly shows that each of these treatment strategies should be addressed at different groups of patients. A greater benefit in post-acute coronary syndrome (ACS) patients with a high risk of ischemic events and without high bleeding risk may be expected with ASA and ticagrelor 60 mg b.i.d. when the therapy is continued without interruption or with short interruption only after ACS. On the other hand, ASA and rivaroxaban 2.5 mg b.i.d. seems to be a better option when indications for dual antithrombotic therapy (DATT) appear after a longer time from ACS (more than 2 years) and/or from cessation of DAPT (more than 1 year) and in patients with multiple vascular bed atherosclerosis. Thus, both options of DATTs com-plement each other rather than compete, as can be presumed from the recommendations. Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

METHODS: We analyzed 6170 AF patients from the Prevention of thromboembolic events - European Registry in Atrial Fibrillation (PREFER in AF), and categorized patients into: HF with reduced left-ventricular ejection fraction (HFrEF; LVEF60%), and no HF. Outcomes were ischemic stroke, major adverse cardiovascular and cerebral events (MACCE) and major bleeding occurring within 1-year. RESULTS: The annual incidence of stroke was linearly and inversely related to LVEF, increasing by 0.054% per each 1% of LVEF decrease (95% CI: 0.013%-0.096%; p=0.031). Patients with HFHpEF had the highest CHA2DS2-VASc score, but significantly lower stroke incidence than other HF groups (0.65%, compared to HFLpEF 1.30%; HFmrEF 1.71%; HFrEF 1.75%; trend p=0.014). The incidence of MACCE was also lower in HFHpEF (2.0%) compared to other HF groups (range: 3.8-4.4%; p=0.001). Age, HF type, and NYHA class were independent predictors of thromboembolic events. Conversely, major bleeding did not significantly differ between groups (p=0.168). CONCLUSION: Our study in predominantly anticoagulated patients with AF shows that, reduction in LVEF is associated with higher thromboembolic, but not higher bleeding risk. HFHpEF is a distinct and puzzling group, featuring the highest CHA2DS2-VASc score but the lowest residual risk of thromboembolic events, which warrants further investigation.

Abstract

Background: Overlapping implantation of bioresorbable scaffolds (BRSs) are frequent in long coronary lesions. Its impact on clinical outcomes is unknown. Objective: To compare the clinical outcomes of patients treated with overlapping BRS with those patients treated with no-overlap BRS. Methods: We analyzed the 1-year clinical outcomes of 1,477 patients treated with BRS in the GHOST-EU registry, according to the implantation of overlapping BRS. Primary endpoint was patient oriented composite endpoint (PoCE) of: all-cause death, any myocardial infarction (MI) and any repeated revascularization. Scaffold thrombosis, according to Academic Research Consortium definition, was also analyzed. Results: A total of 320 (21.7%) patients were treated with overlapping BRS (overlap group), whereas the remaining 1,157 (78.3%) received no-overlap BRS (no-overlap group). The overlap group had significantly higher frequency of male sex, diabetes mellitus, stable angina, B2/C lesion type, SYNTAX score ≥22, lesion length >34 mm, use of intracoronary imaging guidance, pre- and postdilatation. At 1-year, there were no differences in PoCE between the overlap versus no-overlap group (18.4% vs. 18.2%; HR 1.07, [0.80–1.44]; P = 0.636), even after adjustment (HR 1.05, [0.48–2.20]; P = 0.904). Scaffold thrombosis rate did not differ either at one-month (1.3% vs. 1.5%, P = 0.769) or at 1-year (1.9% vs. 2.1%, P = 0.823). Conclusions: In “Real-world” clinical practice, overlapping BRS does not appear to have an impact on clinical outcomes as compared to no-overlapping BRS. These preliminary data should be confirmed. © 2016 Wiley Periodicals, Inc.
