This article, titled "Association between deep endometriosis and clinical symptoms," explores the relationship between deep endometriosis and the symptoms reported by patients. The study was conducted at a University Hospital using ultrasound data collected between 2016 and 2023. The results showed that 40% of the 630 patients evaluated had deep endometriosis, with 88% of them having involvement of the posterior compartment and 12% having involvement of the anterior compartment. Symptoms reported by the patients included pelvic pain, dysmenorrhea, dyspareunia, heavy menstrual bleeding, irregular cycles, infertility, and cystitis. The study found a statistically significant association between the presence of heavy menstrual bleeding, irregular cycles, infertility, and cystitis with ultrasound findings suggestive of deep endometriosis. Disorders of mineral metabolism, including secondary hyperparathyroidism, are thought to contribute to extraskeletal (including vascular) calcification among patients with chronic kidney disease. It has been hypothesized that treatment with the calcimimetic agent cinacalcet might reduce the risk of death or nonfatal cardiovascular events in such patients.In this clinical trial, we randomly assigned 3883 patients with moderate-to-severe secondary hyperparathyroidism (median level of intact parathyroid hormone, 693 pg per milliliter [10th to 90th percentile, 363 to 1694]) who were undergoing hemodialysis to receive either cinacalcet or placebo. All patients were eligible to receive conventional therapy, including phosphate binders, vitamin D sterols, or both. The patients were followed for up to 64 months. The primary composite end point was the time until death, myocardial infarction, hospitalization for unstable angina, heart failure, or a peripheral vascular event. The primary analysis was performed on the basis of the intention-to-treat principle.The median duration of study-drug exposure was 21.2 months in the cinacalcet group, versus 17.5 months in the placebo group. The primary composite end point was reached in 938 of 1948 patients (48.2%) in the cinacalcet group and 952 of 1935 patients (49.2%) in the placebo group (relative hazard in the cinacalcet group vs. the placebo group, 0.93; 95% confidence interval, 0.85 to 1.02; P=0.11). Hypocalcemia and gastrointestinal adverse events were significantly more frequent in patients receiving cinacalcet.In an unadjusted intention-to-treat analysis, cinacalcet did not significantly reduce the risk of death or major cardiovascular events in patients with moderate-to-severe secondary hyperparathyroidism who were undergoing dialysis. Background— Patients with kidney disease have disordered bone and mineral metabolism, including elevated serum concentrations of fibroblast growth factor-23 (FGF23). These elevated concentrations are associated with cardiovascular and all-cause mortality. The objective was to determine the effects of the calcimimetic cinacalcet (versus placebo) on reducing serum FGF23 and whether changes in FGF23 are associated with death and cardiovascular events. Methods and Results— This was a secondary analysis of a randomized clinical trial comparing cinacalcet to placebo in addition to conventional therapy (phosphate binders/vitamin D) in patients receiving hemodialysis with secondary hyperparathyroidism (intact parathyroid hormone ≥300 pg/mL). The primary study end point was time to death or a first nonfatal cardiovascular event (myocardial infarction, hospitalization for angina, heart failure, or a peripheral vascular event). This analysis included 2985 patients (77% of randomized) with serum samples at baseline and 2602 patients (67%) with samples at both baseline and week 20. The results demonstrated that a significantly larger proportion of patients randomized to cinacalcet had ≥30% (68% versus 28%) reductions in FGF23. Among patients randomized to cinacalcet, a ≥30% reduction in FGF23 between baseline and week 20 was associated with a nominally significant reduction in the primary composite end point (relative hazard, 0.82; 95% confidence interval, 0.69–0.98), cardiovascular mortality (relative hazard, 0.66; 95% confidence interval, 0.50–0.87), sudden cardiac death (relative hazard, 0.57; 95% confidence interval, 0.37–0.86), and heart failure (relative hazard, 0.69; 95% confidence interval, 0.48–0.99). Conclusions— Treatment with cinacalcet significantly lowers serum FGF23. Treatment-induced reductions in serum FGF23 are associated with lower rates of cardiovascular death and major cardiovascular events. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Introducción: La infección por el virus de inmunodeficiencia humana constituye un grave problema sanitario. En el 2011, el Ministerio del Poder Popular para la Salud señaló una prevalencia del 0,91%, calculando 111.324 casos en mayores de 15 años. Los reportes ecográficos en pacientes con VIH son escasos. Sin embargo, la ecografía es un método seguro, económico y accesible que permite el estudio de un gran número de condiciones. Pacientes y Métodos: Estudio prospectivo, descriptivo y de corte transversal, que incluye pacientes portadores de infección de VIH, que acudieron al servicio de Gastroenterología del Hospital General del Oeste, entre mayo de 2012 a mayo de 2013. A los cuales se les realizó eco abdominal. Resultados: 80 pacientes. 95% presentó alteraciones ecográficas, 52% cursaba con síntomas gastrointestinales, de ellos, el 100% presentaba alteraciones ecográficas. 42,5% pertenecían a la Categoría C de VIH, de los cuales el 100% tuvo alteraciones ecográficas. Se identificaron alteraciones hepáticas en el 85% de los casos, vesiculares en el 65%, retroperitoneales 51,25 %, renales 37,5%, esplénicas 25,25% y pancreáticas 3,75%. Conclusiones: La ecografía abdominal constituye una herramienta útil y accesible para el médico gastroenterólogo en la detección y/o diagnóstico de alteraciones intra-abdominales en pacientes VIH. Introduction: Human Immunodeficiency Virus Infection (HIV) is a serious health problem.In 2011, the Ministry of People's Power for Health estimated a prevalence of 0.91% with 111,324 cases over 15 year’s population. Ultrasound reports in patients with HIV are scarce. However, ultrasound is safe, non-invasive, affordable and accessible to study a large number of conditions. Patients and methods: A Prospective, descriptive and cross-sectional study was performed. HIV infected patients who underwent the Gastroenterology Service ofWestern General Hospital, from May 2012 to May 2013, to an abdominal echo were included. Results: 80 patients. 95% had abnormal ultrasound, 52% had gastrointestinal symptoms and of these, 100% had ultrasound abnormalities. 42.5% were C category, of which 100% had ultrasound abnormalities. We identified lesions in the liver (85%), gallbladder (65%), retroperitoneum (51,25%), Kidneys (37,5%), spleen (26,25%) and pancreas (3,75%) of the patients. Conclusions: Abdominal ultrasound is a useful and accessible diagnostic tool for the gastroenterologist to detect abdominal abnormalities in HIV patients.

Con la finalidad de diseñar un programa de vigilancia y pronóstico para la enfermedad conocida como sigatoka negra en pequeños sistemas de producción de plátano (Musa AAB cv Hartón) del Asentamiento Campesino Macagua-Jurimiquire, municipio Veroes, estado Yaracuy; durante 1998-2000, se determinó quincenalmente: el promedio ponderado de infección (PPI), posición de la hoja más joven manchada (PHJM) y emisión foliar (EF); diariamente: precipitación (PP), humedad relativa (HR), evaporación (Eva), temperatura máxima y mínima (Tmax y Tmin), nubosidad (Nub), insolación (Ins) y radiación (Rad). Se correlacionó el PPI con las variables climáticas registradas desde una hasta ocho semanas previas y consecutivas a la evaluación de la severidad de la enfermedad en campo. Las variables con mejor coeficiente de correlación se analizaron por regresión múltiple. Los promedios mensuales de las variables climáticas fueron: PP: 143,5 mm; Tmax: 29°C; Tmin: 21°C y HR: 78,5%. Los mayores valores de PPI se observaron en enero, mayo, septiembre, octubre y diciembre, con valores de 1,31; 1,14, 1,32; 1,20 y 1,21; respectivamente. El valor promedio de PHJM fue 5,38; con valor máximo de 6,88 y mínimo de 4,40; mientras que el valor promedio de PPI fue de 1,13, con valor mínimo de 0,64 y máximo de 1,32. Se observó correlación negativa entre el PPI y PHJM (r = -0,7132**) y entre PPI y EF (r = -0,2556*). La ecuación que permitió explicar el comportamiento de la enfermedad en un 33,32% en relación con el clima fue: PPI = 3,7241** - 0,0047**Nub - 0,0015**Tmin + 2,019**HR - 0,0051**Eva, con valores acumulados durante seis semanas consecutivas y previas a la última semana de observación del PPI. To establish a survey and forecast program for black sigatoka disease in Musa AAB cv Harton plantain stablished on growing in small production system. The severity average index (SAI), youngest spotted leaf position (YSLP), foliar emission (FE), daily precipitation (PP), relative humidity (RH), evaporation (EVA), maximun and minimun temperature (Tmax and Tmin), cloudiness (CL), insolation and radiation were determined every 2 weeks. The averages per month for some of these climatic variables were: PP 143.5 mm; Tmax 29°C; Tmin 21°C and RH 78.5%. The SAI was correlated to the climatic variables during 8-consecutive weeks, previous to evaluate the disease severity. The variable with the best correlated coeficients were analized by using multiple regression method. The higher values of SAI were observed in January 1.31; May 1.14; September 1.32; October 1.20 and December 1.21 during this study. The YSLP average was 5.38 with a maximun of 6.88 and minimum of 4.40; while the SAI average was 1.13 with a minimum of 0.64 and a maximum of 1.32. A negative correlation were observed between SAI and YSLP (r = - 0.7132), and between SAI and FE (r = - 0.2556). The disease behavior is 33,32% explained by the following equation: SAI = 3.7241** - 0.0047**CL 0.0015**Tmin + 2.019**RH 0.0051**EVA, according to acumulated values during 6-consecutive weeks previous to the last week of SAI observation.

- Abstract
his article studies the coupling between prescribed thermal oscillations in the surface of a micro-cylinder and vortex shedding. We deal with the unsteady, laminar, compressible flow regime where the aerodynamics forces have a periodic behavior. It is shown that appropriate spatial and time-dependent temperature oscillations on the surface of the micro-cylinder create a resonance that controls the amplitude and frequency of both lift and drag coefficients. In practice, what we study is a mechanism to modulate the amplitude and frequency of mechanical loads of aerodynamics origin in a micro-structure by using surface temperature fluctuations as the control parameter.

Introduction: Acute leukemia is the most common neoplasm in children, accounting for approximately 25% of all tumors in childhood. In Colombia the cure proportion, around 50%, are lower than reported in developed countries. Objective: The main bjective is to determine the global and event-free survival of children with diagnosis of lymphoblastic leukemia, all treated with the ACHOP Protocol 2006, from 2007 to 2012. The secondary objectives are to describe mortality, abandonment, relapse and major complications related to treatment. Material and methods: A descriptive cohort study of 183 patients under 18 years of age, with a confirmed diagnosis of acute lymphoblastic leukemia, who were treated at the Foundation of the Misericordia (HOMI) from 2007 to 2012, was performed. The survival dates were obtained by analysis with Kaplan-Meier curves. Results: We analyzed overall survival at 2, 3 and 5 years with results of 89%, 87.3% and 74.7 % (95% CI 67 - 80.9) respectively. At 5 years survival for the standard risk group was 78.6 % (95 % CI 68.3-85.1) and 61.9 % (95 % CI 50.7-73) for the high risk group. The event-free survival, considering the abandonment and transfer to another institution as an event, was 56.3 % (95% CI 45.5 - 65.8) at 5 years. Mortality in induction was 3.8 %, mortality related to treatment was 3.4 %, 32 patients (17 %) relapsed, bandonment was 16.4 % and transfers 10.4 %. The main complication of the treatment was infections. Conclusions: Overall survival is acceptable for a country with limited resources, the events of abandonment and transfers are very high. Efforts should be made to reduce these situations that worsen the prognosis of the disease.

Abstract Background Salamanca, Mexico occupied fourth place nationally in contaminating emissions. The aim of the study was to determine the impact of air pollution on the frequency of pulmonary function alterations and respiratory symptoms in school-age children in a longitudinal repeated-measures study. Methods We recruited a cohort of 464 children from 6 to 14 years of age, from two schools differing in distance from the major stationary air pollution sources. Spirometry, respiratory symptoms and air pollutants (O3, SO2, NO, NO2, NOx, PM10,) were obtained for each season. Mixed models for continuous variables and multilevel logistic regression for respiratory symptoms were fitted taking into account seasonal variations in health effects according to air pollution levels. Results Abnormalities in lung function and frequency of respiratory symptoms were higher in the school closer to major stationary air pollution sources than in the distant school. However, in winter differences on health disappeared. The principal alteration in lung function was the obstructive type, which frequency was greater in those students with greater exposure (10.4% vs. 5.3%; OR = 1.95, 95% CI 1.0-3.7), followed by the mixed pattern also more frequent in the same students (4.1% vs. 0.9%; OR = 4.69, 95% CI, 1.0-21.1). PM10 levels were the most consistent factor with a negative relationship with FVC, FEV1 and PEF but with a positive relationship with FEV1/FVC coefficient according to its change per 3-month period. Conclusions Students from the school closer to major stationary air pollution sources had in general more respiratory symptoms than those from the distant school. However, in winter air pollution was generalized in this city and differences in health disappeared. PM10 levels were the most consistent factor related to pulmonary function according, to its change per 3-month period.

Background andobjectivesThecalcimimeticcinacalcet reduced therisk of death or cardiovascular (CV) events in older, but not younger, patients with moderate to severe secondary hyperparathyroidism (HPT) who were receiving hemodialysis. To determine whether the lower risk in younger patients might be due to lower baseline CV risk and more frequent use of cointerventions that reduce parathyroid hormone (kidney transplantation, parathyroidectomy, and commercial cinacalcet use), this study examined the effects of cinacalcet in older ($65 years, n=1005) and younger (,65 years, n=2878) patients. Design, setting, participants, & measurements Evaluation of Cinacalcet HCl Therapy to Lower Cardiovascular Events (EVOLVE) was a global, multicenter, randomized placebo-controlled trial in 3883 prevalent patients on hemodialysis, whose outcomes included death, major CV events, and development of severe unremitting HPT. The age subgroup analysis was prespecified. ResultsOlderpatients hadhigher baselineprevalenceof diabetesmellitusandCV comorbidity. Annualizedrates of kidney transplantation and parathyroidectomy were .3-fold higher in younger relative to older patients and were more frequent in patients randomized to placebo. In older patients, the adjusted relative hazard (95% confidence interval) for the primary composite (CV) end point (cinacalcet versus placebo) was 0.70 (0.60 to 0.81); in younger patients, the relative hazard was 0.97 (0.86 to 1.09). Corresponding adjusted relative hazards for mortality were 0.68 (0.51 to 0.81) and 0.99 (0.86 to 1.13). Reduction in the risk of severe unremitting HPT was similar in both groups. Conclusions In the EVOLVE trial, cinacalcet decreased the risk of death and of major CV events in older, but not younger, patients with moderate to severe HPT who were receiving hemodialysis. Effect modification by age may be partly explained by differences in underlying CV risk and differential application of cointerventions that reduce parathyroid hormone. Antecedentes: La insuficiencia suprarrenal no ha sido en cuenta en los niños que reciben dosis altas de esteroides, como los de la quimioterapia de inducción para Leucemia Linfoide Aguda (LLA). Algunos autores han reportado una incidencia de disfuncion suprarrenal entre 46% a 81,5% en este grupo de pacientes. Objetivos: Evaluar la frecuencia de insuficiencia suprarrenal en pacientes pediátricos con LLA después de recibir quimioterapia de inducción que incluye esteroides a dosis altas, en una unidad de oncología pediátrica en Bogotá-Colombia. Diseño / Método: Estudio prospectivo. Pacientes con edad 18 años con diagnóstico nuevo de LLA. Los pacientes recibieron 5 semanas de tratamiento con esteroides incluyendo nueve días de descenso de dosis. Se midieron cortisol basal y Adrenocorticothropin (ACTH) en todos los pacientes y debía ser normal antes de iniciar el estudio (cortisol basal 6 -30 mcg / dl y ACTH 4,4 - 22 pmol / l); después de 3 días fuera de la terapia con esteroides se hizo una prueba de estimulación con ACTH a dosis baja de 1 mcg (LD-ACTH); En los pacientes con cortisol anormal (18 mcg / dl) se obtuvieron nuevas pruebas de estimulacion con LD-ACTH los días 7, 14 y 30 hasta que los niveles de cortisol se normalizaron ( 18 mcg / dl). Resultados: 40 pacientes, edad media 8,5 años (rango de 2 -17 años). Tres días después de la última dosis de esteroides 29/40 (72,5%) pacientes tenían niveles bajos de cortisol después del estimulo con LD-ACTH. En los días 7, 14 y 30: 12/29, 03/29 y 0 / 29, respectivamente, tenían niveles anormales de cortisol después del estimulo con LD-ACTH. Todos los pacientes con insuficiencia suprarrenal eran mayores de 5 años con un HR de 4.69 (IC 95% 1,44-15,32) p 0.011. No hubo diferencias de la frecuencia de insuficiencia suprarenal entre los pacientes que recibian prednisona o dexametasona. Un paciente falleció después de la quimioterapia de inducción, debido a la tuberculosis sistémica. Conclusión: Las dosis de esteroides utilizada en todos los protocolos de LLA pueden causar insuficiencia suprarrenal prolongada. Los pacientes de más de 5 años están en alto riesgo de desarrollar disfunción suprarrenal. Se sugiere seguir a los pacientes con episodios de estrés después de la quimioterapia de inducción ya que es posible que tengan indicacion de suplencia de esteroides. / Abstract. Background: Adrenal insufficiency is not always considered in children who receive high dose steroids. Such could be the case of patients during chemotherapy for Acute Lymphoid Leukemia (ALL). Authors have reported an incidence of adrenal dysfunction in this group of patients ranging from 46 to 81.5%. Objective: To assess the frequency of adrenal insufficiency in children with ALL, after induction chemotherapy with steroids, in a pediatric oncology unit in Bogota, Colombia. Design/Methods: In a case control study nested in a cohort, patients aged 18 years, newly diagnosed as suffering ALL, were included. They received 5 week steroid therapy, including 9 days for dosage tapering. Basal cortisol and adrenocorticothropin (ACTH) were determined in all the patients. Levels should be normal at the start of the study (cortisol 6 -30 mcg/dL and ACTH 4.4–22 pmol/L). Three days after stopping steroid therapy, a stimulation test with low dose ACTH (1 mcg) was performed. Patients with low cortisol (18 mcg/dL) had repeated stimulation tests at days 7, 14 and 30, until cortisol levels were found normal (18 mcg/dL). Results: Forty patients, mean age 8.5 years, were enrolled. Three days after the last steroid dose, 29/40 (72.5%) patients had low cortisol levels after LD-ACTH stimulation. At days 7, 14 and 30: 12/29, 3/29 and 0/29, respectively, had abnormal cortisol levels after LD-ACTH. All patients with adrenal suppression had ages over 5 years (HR 4.69; CI 95%; 1.44-15.32; p= 0.011). There were no differences when patients received prednisone or dexamethasone. One patient died during the study, after induction chemotherapy, due to systemic tuberculosis. Conclusions: Steroid doses used in ALL protocols may cause prolonged adrenal insufficiency. Patients aged 5 years are at high risk of developing adrenal suppression. We suggest follow-up of patients with stress episodes after induction chemotherapy, as steroid supply could be needed. Otra

This column provides an overview of the Virtual Health Library (VHL) Regional Portal, a resource provided by Pan American Health Organization and others with the purpose of communicating and exchanging health experiences and information for Latin American and Caribbean countries. The content is organized in four sub-portals: the VHL Model, which describes health information and knowledge management to structures and guides; LILACS, the Latin American and Caribbean Index Health Science Literature; DeCS, Descriptors Health Sciences, a controlled vocabulary; and Training, which gathers and organizes information for librarians, developers, and users who are interested in construction and use of VHL methodologies. An overview of the resource and sample search is discussed.

Emerging data have suggested that circulating tumor DNA (ctDNA) can be a reliable biomarker for minimal residual disease (MRD) in CRC patients. Recent studies have shown that the ability to detect MRD using ctDNA assay after curative-intent surgery will change how to assess the recurrence risk and patient selection for adjuvant chemotherapy. We performed a meta-analysis of post-operative ctDNA in stage I-IV (oligometastatic) CRC patients after curative-intent resection. We included 23 studies representing 3568 patients with evaluable ctDNA in CRC patient post-curative-intent surgery. Data were extracted from each study to perform a meta-analysis using RevMan 5.4. software. Subsequent subgroup analysis was performed for stages I-III and oligometastatic stage IV CRC patients. Results showed that the pooled hazard ratio (HR) for recurrence-free survival (RFS) in post-surgical ctDNA-positive versus -negative patients in all stages was 7.27 (95% CI 5.49-9.62), p < 0.00001. Subgroup analysis revealed pooled HRs of 8.14 (95% CI 5.60-11.82) and 4.83 (95% CI 3.64-6.39) for stages I-III and IV CRC, respectively. The pooled HR for RFS in post-adjuvant chemotherapy ctDNA-positive versus -negative patients in all stages was 10.59 (95% CI 5.59-20.06), p < 0.00001. Circulating tumor DNA (ctDNA) analysis has revolutionized non-invasive cancer diagnostics and monitoring, with two primary forms of analysis emerging: tumor-informed techniques and tumor-agnostic or tumor-naive techniques. Tumor-informed methods involve the initial identification of somatic mutations in tumor tissue, followed by the targeted sequencing of plasma DNA using a personalized assay. In contrast, the tumor-agnostic approach performs ctDNA analysis without prior knowledge of the patient's tumor tissue molecular profile. This review highlights the distinctive features and implications of each approach. Tumor-informed techniques enable the precise monitoring of known tumor-specific mutations, leveraging the sensitivity and specificity of ctDNA detection. Conversely, the tumor-agnostic approach allows for a broader genetic and epigenetic analysis, potentially revealing novel alterations and enhancing our understanding of tumor heterogeneity. Both approaches have significant implications for personalized medicine and improved patient outcomes in the field of oncology. The subgroup analysis based on the ctDNA method showed pooled HRs of 8.66 (95% CI 6.38-11.75) and 3.76 (95% CI 2.58-5.48) for tumor-informed and tumor-agnostic, respectively. Our analysis emphasizes that post-operative ctDNA is a strong prognostic marker of RFS. Based on our results, ctDNA can be a significant and independent predictor of RFS. This real-time assessment of treatment benefits using ctDNA can be used as a surrogate endpoint for the development of novel drugs in the adjuvant setting.

43. Workloads of Different Soccer-Specific Drills in Professional Players.

- Abstract
Soccer is a predominantly tactical sport and, therefore, tactical training has become the most widely used strategy to improve players' performance. The objective of the present study was to assess the workload of soccer-specific drills in professional players over a two-season period in an established context. GPS technology was used to record the data. One hundred and thirty-two (n = 132) soccer-specific drills were studied and grouped by categories. The individual demands of each task were related to the individual competitive profile of each player. The level of physical demand was significantly different in relation to the specific soccer drills analysed. Total distance covered, high-speed running, and the total number of high accelerations and decelerations were significantly higher in competition than in drills used for training sessions (p < .001). The Large-Sided Games (LSG), Big-Position Games (BPG) and Position Games (PG) showed higher maximum running speed values than the rest of the exercises (p < .01). The sum of high accelerations and decelerations values was greater in the Small-Sided Games (SSG) than in BPG (p < .001), Small-Position Games (SPG) (p < .001) and Physical-Technical Circuits (PTC) (p < .001). Significant differences were observed in the exercises analysed according to the player's position. - Abstract
Focal adhesions (FAs) are adhesive organelles that attach cells to the extracellular matrix and can mediate various biological functions in response to different environmental cues. Reduced FAs are often associated with enhanced cell migration and cancer metastasis. In addition, because FAs are essential for preserving vascular integrity, the loss of FAs leads to hemorrhages and is frequently observed in many vascular diseases such as intracranial aneurysms. For these reasons, FAs are an attractive therapeutic target for treating cancer or vascular diseases, two leading causes of death world-wide. FAs are controlled by both their formation and turnover. In comparison to the large body of literature detailing FA formation, the mechanisms of FA turnover are poorly understood. Recently, autophagy has emerged as a major mechanism to degrade FAs and stabilizing FAs by inhibiting autophagy has a beneficial effect on breast cancer metastasis, suggesting autophagy-mediated FA turnover is a promising drug target. Intriguingly, autophagy-mediated FA turnover is a selective process and the cargo receptors for recognizing FAs in this process are context-dependent, which ensures the degradation of specific cargo. This paper mainly reviews the cargo recognition mechanisms of FA-phagy (selective autophagy-mediated FA turnover) and its disease relevance. 47. Optimizing Nutritional Status in Patients Undergoing Radical Cystectomy: A Systematic Scoping Review.

- Abstract
Background: Nutrition is a modifiable risk factor for patients undergoing multimodal oncologic interventions and plays a major supportive role in the setting of bladder cancer. For patients undergoing radical cystectomy (RC), malnutrition is associated with increased postoperative complications and mortality., Objectives: The purpose of this scoping review is to characterize the role of nutritional interventions for patients undergoing RC for bladder cancer., Methods: A multi-database systematic scoping review based on the Preferred Reporting Items for Systematic Reviews extension for Scoping Reviews (PRISMA-ScR) guidelines was performed. Search terms were developed a priori to identify clinical trials that focused on nutritional interventions for patients with bladder cancer undergoing RC. Eligible articles were original research articles or abstracts from clinical trials evaluating nutritional interventions in adult patients undergoing RC. Articles were excluded if they did not focus on a nutritional intervention, if patients did not carry a diagnosis of bladder cancer, or if RC was not performed. Articles were reviewed independently by the authors, and inclusion/exclusion were based on consensus agreement., Results: A total of 83 articles were identified, of which 17 were included in the final analysis. A total of 49 articles were excluded during abstract screening. An additional 17 articles were excluded based on the review of full-text articles. Results of the scoping review suggest that data on the use of nutritional screening, assessment, and intervention for patients undergoing RC are scarce. Although parenteral nutrition (PN) appears to be associated with greater complications after RC, early introduction of food postoperatively or feeding enterally offers benefit and immunonutrition supplements with a focus on a high protein diet has the potential to better optimize surgical outcomes., Conclusions: Although the prevalence and consequences of malnutrition among patients undergoing RC are well-established, there are limited data evaluating the use of nutritional screening, assessment, and interventions for this population. An interdisciplinary team of faculty and students developed the Hydrate for Health project to provide relevant and evidence-based information to community-dwelling older adults. Evidence-based factsheets on bladder health, nighttime urination, medication safety, and physical activity/exercise, as well as a fluid intake self-monitoring tool, were developed. Four focus groups were conducted and included older adults (N = 21) who participated in activities at two local senior centers to obtain their feedback about the relevance of the factsheets. Extensive revisions were required based on the feedback received. Older adults expressed a desire for pragmatic information (i.e., how to determine fluid sources from food, how to measure water, how to determine their own fluid needs). They also wanted information that could be easily incorporated into daily life. - Abstract
Background: Salamanca, Mexico occupied fourth place nationally in contaminating emissions. The aim of the study was to determine the impact of air pollution on the frequency of pulmonary function alterations and respiratory symptoms in school-age children in a longitudinal repeated-measures study., Methods: We recruited a cohort of 464 children from 6 to 14 years of age, from two schools differing in distance from the major stationary air pollution sources. Spirometry, respiratory symptoms and air pollutants (O3, SO2, NO, NO2, NOx, PM10,) were obtained for each season. Mixed models for continuous variables and multilevel logistic regression for respiratory symptoms were fitted taking into account seasonal variations in health effects according to air pollution levels., Results: Abnormalities in lung function and frequency of respiratory symptoms were higher in the school closer to major stationary air pollution sources than in the distant school. However, in winter differences on health disappeared. The principal alteration in lung function was the obstructive type, which frequency was greater in those students with greater exposure (10.4% vs. 5.3%; OR = 1.95, 95% CI 1.0-3.7), followed by the mixed pattern also more frequent in the same students (4.1% vs. 0.9%; OR = 4.69, 95% CI, 1.0-21.1). PM10 levels were the most consistent factor with a negative relationship with FVC, FEV1 and PEF but with a positive relationship with FEV1/FVC coefficient according to its change per 3-month period., Conclusions: Students from the school closer to major stationary air pollution sources had in general more respiratory symptoms than those from the distant school. However, in winter air pollution was generalized in this city and differences in health disappeared. PM10 levels were the most consistent factor related to pulmonary function according, to its change per 3-month period.

- Abstract
The use of diamond for advanced applications has been the dream of mankind for centuries. Until recently this dream has been realized only in the use of diamond for gemstones and abrasive applications where tons of diamonds are used on an annual basis. Diamond is the material system of choice for many applications, but its use has historically been limited due to the small size, high cost, and inconsistent (and typically poor) quality of available diamond materials until recently. The recent development of high quality, single crystal diamond crystal growth via the Chemical Vapor Deposition (CVD) process has allowed physcists and increasingly scientists in the life science area to think beyond these limitations and envision how diamond may be used in advanced applications ranging from quantum computing, to power generation and molecular imaging, and eventually even diamond nano-bots. Because of diamond's unique properties as a bio-compatible material, better understanding of diamond's quantum effects and a convergence of mass production, semiconductor-like fabrication process, diamond now promises a unique and powerful key to the realization of the bio-electronic devices being envisioned for the new era of medical science. The combination of robust in-the-body diamond based sensors, coupled with smart bio-functionalized diamond devices may lead to diamond being the platform of choice for bio-electronics. This generation of diamond based bio-electronic devices would contribute substantially to ushering in a paradigm shift for medical science, leading to vastly improved patient diagnosis, decrease of drug development costs and risks, and improved effectiveness of drug delivery and gene therapy programs through better timed and more customized solutions.
