Abstract

The weaving culture is a local tradition owned by many local communities throughout Indonesia, including West Kalimantan. The culture, however, faces several challenges to its preservation due to modernization and cultural changes among younger generations. This community service program aims to preserve the weaving culture in West Kalimantan by delivering training and mentorship to a community of weaving artisans in Weaving Village Pontianak (Kanun), West Kalimantan, on the importance and skill of digitizing motifs and meanings of woven fabrics. This program is delivered by adopting the Community Development Method (CDM) consisting four stages: initiation, action planning, implementation of activities, and evaluation. From the training and mentorship activities, this program results in increasing knowledge and skills among Kanun’s youth regarding the importance of weaving tradition and digital skills to document and preserve the tradition. Therefore, it is recommended that more relevant sustainable programs are developed and integrated in the village development strategies, particularly in documenting and preserving rich histories and meanings related to the weaving tradition in Kanun. In the long term, this skill is hopefully beneficial for the archives of cultural heritage and for attracting new tourists and customers–realizing the “Tourism Village” goal for the targeted community.

Abstract

IntroductionCardiogenic shock (CS) is a severe syndrome with poor prognosis. Short-term mechanical circulatory support with Impella devices has emerged as an increasingly therapeutic option, unloading the failing left ventricle (LV) and improving hemodynamic status of affected patients. Impella devices should be used for the shortest time necessary to allow LV recovery because of time-dependent device-related adverse events. The weaning from Impella, however, is mostly performed in the absence of established guidelines, mainly based on the experience of the individual centres.MethodsThe aim of this single center study was to retrospectively evaluate whether a multiparametrical assessment before and during Impella weaning could predict successful weaning. The primary study outcome was death occurring during Impella weaning and secondary endpoints included assessment of in-hospital outcomes.ResultsOf a total of 45 patients (median age, 60 [51–66] years, 73% male) treated with an Impella device, 37 patients underwent impella weaning/removal and 9 patients (20%) died after the weaning. Non-survivors patients after impella weaning more commonly had a previous history of known heart failure (p = 0.054) and an implanted ICD-CRT (p = 0.01), and were more frequently treated with continuous renal replacement therapy (p = 0.02). In univariable logistic regression analysis, lactates variation (%) during the first 12–24 h of weaning, lactate value after 24 h of weaning, left ventricular ejection fraction (LVEF) at the beginning of weaning, and inotropic score after 24 h from weaning beginning were associated with death. Stepwise multivariable logistic regression identified LVEF at the beginning of weaning and lactates variation (%) in the first 12–24 h from weaning beginning as the most accurate predictors of death after weaning. The ROC analysis indicated 80% accuracy (95% confidence interval = 64%–96%) using the two variables in combination to predict death after weaning from Impella.ConclusionsThis single-center experience on Impella weaning in CS showed that two easily accessible parameters as LVEF at the beginning of weaning and lactates variation (%) in the first 12–24 h from weaning begin were the most accurate predictors of death after weaning.

Abstract

Recent years showed a gradual transition from terrestrial to aerial survey thanks to the development of UAV and sensors for it. Many sectors benefited by this change among which geological one; drones are flexible, cost-efficient and can support outcrops surveying in many difficult situations such as inaccessible steep and high rock faces. The experiences acquired in terrestrial survey, with total stations, GNSS or terrestrial laser scanner (TLS), are not yet completely transferred to UAV acquisition. Hence, quality comparisons are still needed. The present paper is framed in this perspective aiming to evaluate the quality of the point clouds generated by an UAV in a geological context; data analysis was conducted comparing the UAV product with the homologous acquired with a TLS system. Exploiting modern semantic classification, based on eigenfeatures and support vector machine (SVM), the two point clouds were compared in terms of density and mutual distance. The UAV survey proves its usefulness in this situation with a uniform density distribution in the whole area and producing a point cloud with a quality comparable with the more traditional TLS systems.

Abstract

Abstract Background It has been established that children with Autism Spectrum Disorders (ASD) are affected by oxidative stress, the origin of which is still under investigation. In the present work, we evaluated inflammatory and pro-oxidant soluble signature in non-syndromic ASD and age-matched typically developing (TD) control children. Methods We analyzed leukocyte gene expression of inflammatory cytokines and inflammation/oxidative-stress related molecules in 21 ASD and 20 TD children. Moreover, in another—comparable—group of non-syndromic ASD (N = 22) and TD (N = 21) children, we analyzed for the first time the protein expression of the four members of the antioxidant enzyme family of peroxiredoxins (Prx) in both erythrocyte membranes and in plasma. Results The gene expression of IL6 and of HSP70i, a stress protein, was increased in ASD children. Moreover, gene expression of many inflammatory cytokines and inflammation/oxidative stress-related proteins correlated with clinical features, and appeared to be linked by a complex network of inter-correlations involving the Aryl Hydrocarbon Receptor signaling pathway. In addition, when the study of inter-correlations within the expression pattern of these molecules was extended to include the healthy subjects, the intrinsic physiological relationships of the inflammatory/oxidative stress network emerged. Plasma levels of Prx2 and Prx5 were remarkably increased in ASD compared to healthy controls, while no significant differences were found in red cell Prx levels. Conclusions Previous findings reported elevated inflammatory cytokines in the plasma of ASD children, without clearly pointing to the presence of neuro-inflammation. On the other hand, the finding of microglia activation in autoptic specimens was clearly suggesting the presence of neuro-inflammation in ASD. Given the role of peroxiredoxins in the protection of brain cells against oxidative stress, the whole of our results, using peripheral data collected in living patients, support the involvement of neuro-inflammation in ASD, and generate a rational for neuro-inflammation as a possible therapeutic target and for plasma Prx5 as a novel indicator of ASD severity.

Abstract

Background: In many countries, gastric cancer is not diagnosed until an advanced stage. An Internet-based e-learning system to improve the ability of endoscopists to diagnose gastric cancer at an early stage was developed and was evaluated for its effectiveness. Methods: The study was designed as a randomized controlled trial. After receiving a pre-test, participants were randomly allocated to either an e-learning or non-e-learning group. Only those in the e-learning group gained access to the e-learning system. Two months after the pre-test, both groups received a post-test. The primary endpoint was the difference between the two groups regarding the rate of improvement of their test results. Findings: 515 endoscopists from 35 countries were assessed for eligibility, and 332 were enrolled in the study, with 166 allocated to each group. Of these, 151 participants in the e-learning group and 144 in the non-e-learning group were included in the analysis. For decades, lipid droplets have been considered as the main cellular organelles involved in the fat storage, because of their lipid composition. However, in recent years, some new and totally unexpected roles have been discovered for them: (i) they are active sites for synthesis and storage of inflammatory mediators, and (ii) they are key players in cancer cells and tissues, especially in cancer stem cells. In this review, we summarize the main concepts related to the lipid droplet structure and function and their involvement in inflammatory and cancer processes.

Abstract

Background: eosinophilic esophagitis is an esophageal disorder characterized by esophageal and/or upper gastrointestinal tract symptoms, and by dense esophageal eosinophilia associated with a normal gastric and duodenal mucosa. Prevalently reported in children, eosinophilic esophagitis has recently been reported with increased frequency also in adults. Aims: the purpose of this study was to report our experience with eosinophilic esophagitis in Italy, since there are only very few series of such patients in our country. Patients and methods: we retrospectively reviewed the histological data of consecutive patients with a diagnosis of esophagitis or reflux disease in the period September 2004-September 2008. Eosinophils were counted where they appeared most numerous in the biopsy, with a cutoff > 15 eosinophils in more than one high-power field as diagnostic of eosinophilic esophagitis. Patients were excluded if gastric or duodenal biopsies showed a prominent eosinophilic infiltrate. Results: twenty two patients (14 adults, 8 children, age range 2-59 years) were identified according to the above criteria. The average eosinophil count was 86/ high-power field (range 31-150), associated with other pathologic features (eosinophilic microabscesses eosinophil degranulation, basal zone hyperplasia, papillary elongation). The main clinical complaints were dysphagia, food impaction, and heartburn, and endoscopic findings consisted of mucosal thickening and inelasticity, longitudinal shearing, rings, and white specks, without difference between adults and children for both clinical and endoscopic variables. Conclusions: eosinophilic esophagitis is not rare in Italy, and displays clinical, endoscopic, and pathologic features similar to those described in other countries.

9. How physical exercise changes rat myotendinous junctions: an ultrastructural study

Abstract

Myotendinous junctions can be easily injured by overloading or trauma, and exercise training may be a way of increasing their resistance to mechanical stress. To this end, we examined herein the morphological changes induced by moderate exercise training in the myotendinous junctions of extensor digitorum longus and gastrocnemius muscles in rats. Twelve Sprague-Dawley rats were used in this investigation. Six of them were trained to run on a treadmill for 1 h/day, 3 days/week over 10 weeks in order for them to achieve a running rate of 25 m/min at the end of the training period. Six age-matched sedentary rats were used as controls. The rats were sacrificed 24 h after the final training session, and the extensor digitorum longum (EDL) and the gastrocnemium were excised; the myotendinous junctions (MTJ) were then prepared and observed with electron microscopy. Digitation branching was evaluated by counting the bifurcations in the MTJ protrusions. Our observations indicate that exercise does indeed induce changes in MTJ morphology. In both muscles the number of bifurcated interdigitations increased significantly, as well as, in gastrocnemius, the branching of the finger-like processes. It was demonstrated that the MTJ is able to adapt to an increase in tensile force by enlarging the muscle-tendon contact area and, consequently, mechanical resistance.

The half-linear difference equations with the deviating argument Δ(an|Δxn|αsgn Δxn)+bn|xn+q|αsgn xn+q=0 , q ∈ ℤ are considered. We study the role of the deviating argument q, especially as regards the growth of the nonoscillatory solutions and the oscillation. Moreover, the problem of the existence of the intermediate solutions is completely resolved for the classical half-linear equation (q = 1). Some analogies or discrepancies on the growth of the nonoscillatory solutions for the delayed and advanced equations are presented; and the coexistence with different types of nonoscillatory solutions is studied.

Abstract

Evaluamos los resultados de la aplicación de técnicas combinadas de cirugía abierta y endovascular en el tratamiento de patología compleja de arco y aorta torácica. Material y métodos: Entre 2006–2012, 15 pacientes (80% varones, edad media 62,2 ± 11,7 años, EuroSCORE logístico medio 21,1 ± 15,8% y 100% ASA III-IV) fueron sometidos a procedimientos híbridos: 4 derivaciones extraanatómicas de troncos supraaórticos; 4 reemplazos de arco completo con reimplante troncos supraaórticos; 4 traslocaciones de troncos supraaórticos; 2 accesos anterógrados mediante injerto en aorta ascendente; 1 derivación extraanatómica de troncos viscerales desde aorta ascendente con injerto cuatrifurcado transdiafragmático. Dos casos fueron cirugía emergente y el resto cirugía electiva. En 13 casos se efectuó el procedimiento en una única fase. Resultados: No hubo mortalidad intrahospitalaria. Las complicaciones postoperatorias fueron: respiratorias, 3 casos; insuficiencia renal, 1 caso, y accidente cerebrovascular (ACV), 1 caso. La mediana de estancia en unidad de cuidados intensivos (UCI) fue 3 días (rango 1–61); estancia postoperatoria global 7 días (rango 4–84), y de seguimiento 17 meses (rango 2–61). No hubo complicaciones aórticas durante seguimiento con una reducción media del diámetro aórtico máximo de 20,7 ± 16,9 mm. La supervivencia actuarial fue del 100% a 12 y 24 meses, y del 75% a 60 meses. Conclusiones: Nuestro estudio destaca cómo las estrategias híbridas incorporando un abordaje quirúrgico abierto y endovascular pueden ser empleadas con éxito en el tratamiento de patología compleja de arco y aorta torácica. Este abordaje combinado expande potencialmente el uso de las técnicas endovasculares aórticas y es una solución atractiva para pacientes quirúrgicos de alto riesgo.

Abstract

Desarrollar una escala de riesgo para la identificación precoz de lesiones traumáticas de aorta (LTA) en politraumatismos basada en datos clínicos y de radiografía de tórax. Pacientes y métodos: Estudio multicéntrico retrospectivo efectuado con 640 pacientes con traumatismo torácico mayor cerrado divididos en: muestra para el desarrollo de escala (76 pacientes consecutivos con LTA y 304 traumatizados sin LTA de un mismo centro) y muestra para validación externa de escala (52 pacientes consecutivos con LTA y 208 traumatizados sin LTA). El análisis bivariante identificó variables como potenciales factores predictores de LTA que, tras ser confirmadas por regresión logística, recibieron una puntuación de 1–4 según su coeficiente β. Resultados: Predictores de LTA identificados: ensanchamiento mediastínico (odds ratio [OR]: 30,82; intervalo de confianza [IC]: 12,05–78,81); hipotensión inferior a 90 mmHg (OR: 5,85; IC: 2,26–15,15); fractura huesos largos (OR: 8,60; IC: 2,15–34,31); contusión pulmonar (OR: 4,12; IC: 1,11–15,20); fractura escápula izquierda (OR: 3,81; IC: 1,24–11,69); hemotórax (OR: 3,47; IC: 1,19–10,09), y fractura pélvica (OR: 2,96; IC: 1,15–7,60). La prueba se consideró positiva para un valor de corte igual o superior a 4 puntos, proporcionando un área bajo la curva ROC de 0,96, sensibilidad 93,42%, especificidad 85,85%, índice Youden 0,79, razón verosimilitud positiva 6,60 y razón verosimilitud negativa 0,08. En muestra de validación externa, la sensibilidad fue 92,31% y especificidad 85,1%. Conclusiones: Dada la baja frecuencia de la LTA, que a menudo asocia un retraso diagnóstico, la aplicación de esta escala predictiva permite identificar al politraumatismo de alto riesgo de LTA, favoreciendo así la instauración precoz de un manejo diagnóstico y terapéutico específico.

15. 312. Tratamiento endovascular de lesiones traumáticas de aorta: resultado a corto y medio plazo

Abstract

Analizar los resultados a corto y medio plazo de la reparación endovascular (TEVAR) de lesiones traumáticas de aorta torácica en pacientes politraumatizados. Pacientes y métodos: Revisión retrospectiva de un registro prospectivo de lesiones traumáticas de aorta. Entre los años 2003–2011 se trataron mediante TEVAR 16 pacientes politraumatizados (86,7% varones, edad media 49,5 ± 16,1 años) con lesiones traumáticas a nivel del istmo aórtico: 4 disecciones focales superiores a 10 mm; 5 seudoaneurismas saculares, y 6 transecciones completas/roturas contenidas. Nueve casos fueron cirugía urgente-emergente y el resto cirugía diferida tras seguimiento extrahospitalario. Todos los pacientes diferidos siguieron un estricto control radiológico para decidir el momento óptimo de intervención. Resultados: Todos los pacientes sufrieron politraumatismo cerrado grave con puntuación media Injury Severity Score 39,1 ± 171,1 y mortalidad media estimada según Trauma Injury Severity Score 26,9% ± 34,6%. Tres pacientes operados emergentes fallecieron debido a fracaso multiorgánico por lesiones asociadas. Todos los procedimientos fueron técnicamente exitosos. No se produjeron complicaciones neurológicas ni vasculares asociadas al procedimiento. Sólo un caso diferido requirió cobertura de arteria subclavia izquierda con revascularización carotidosubclavia asociada. Se implantaron 5 endoprótesis de aorta torácica Talent®, 4 Valiant® y 5 Valiant Captivia® (Medtronic, World Medical Manufacturing Corp, Sunrise, FL). La mediana de seguimiento fue 22 meses (rango 0–95) con supervivencia actuarial a 1 y 5 años del 80%. No se detectaron endofugas durante seguimiento postoperatorio. Conclusiones: Los resultados a corto y medio plazo del tratamiento endovascular de lesiones traumáticas aórticas son prometedores, especialmente cuando se puede diferir la cirugía y realizar un seguimiento radiológico estricto.

Abstract

Evaluar los resultados a corto y medio plazo del tratamiento endovascular o híbrido (abierta y endovascular) de lesiones complejas de arco aórtico y/o aorta torácica en pacientes de alto riesgo quirúrgico. Métodos: Se analizaron 35 pacientes de alto riesgo quirúrgico operados entre mayo de 2001 y febrero de 2010 mediante técnicas endovasculares y/o híbridas de lesiones de arco aórtico y/o aorta torácica, incluyendo 12 disecciones de aorta tipo B, 8 aneurismas fusiformes, 8 roturas traumáticas de aorta, 4 úlceras penetrantes y 3 aneurismas saculares. Todos los pacientes fueron clasificados como clase IV de la American Society of Anaesthesiologists. La mediana de la escala de riesgo quirúrgico EuroSCORE logístico fue 16,5% (rango 10,5–70,34%), mientras que del Parsonnet 95 fue 20 puntos (rango 14–60). Resultados: Doce casos fueron cirugía emergente. No se produjo ninguna reconversión a cirugía abierta. La mortalidad intrahospitalaria fue 8,5%. Se efectuó seguimiento clínico y radiológico a todos los pacientes (n = 32) con mediana de 24 meses (rango 1–107 meses). La supervivencia global fue del 97,1% a 1 año y del 93,2% a 5 años. Se produjeron endofugas tardías en dos pacientes. La supervivencia libre de reintervención aórtica fue del 97,2% a 1 y 5 años y del 77,7% a 7 años. Conclusiones: Las técnicas endovasculares y la cirugía híbrida permiten abordar la enfermedad compleja de arco aórtico y aorta torácica con una aceptable morbimortalidad. Los pacientes de «alto riesgo», con múltiples comorbilidades que limitan su esperanza de vida, son potencialmente los que más se beneficiarán de estas técnicas en el tratamiento de lesiones de aorta torácica.

Abstract

El manejo de la rotura traumática de aorta (RTA) ha evolucionado desde la cirugía emergente abierta para todos a incorporar tratamientos conservadores y endovasculares. Métodos: Entre enero de 1980 y enero de 2010 se incluyeron 68 pacientes con RTA que fueron asignados a tratamiento conservador (n = 37) o quirúrgico (total n = 31; clamp-and-sew n = 9; bypass izquierdo n = 14; endovascular n = 8). Se realizó cirugía emergente (< 24 h) en 26 casos. Ambos grupos eran similares en edad, sexo y mortalidad predicha por puntuaciones Injurity Severity Score (ISS) y Trauma and injury severity score (TRISS). Resultados: La mortalidad intrahospitalaria global fue del 25%, 28,9% en el grupo conservador y 20% en el quirúrgico (p = 0,4). Sólo se produjeron complicaciones aórticas en el grupo conservador (0 vs 52,9%; p < 0,001). La mediana de seguimiento fue de 75 meses (rango 2-360 meses). La supervivencia del grupo conservador fue 76,2% a 1 año, 73% a 5 años y 67,8% a 10 años. La supervivencia del grupo quirúrgico fue 79,2% a 1, 5 y 10 años (p = 0,42). Ningún paciente del grupo quirúrgico requirió reintervención, mientras que un 37,8% del grupo conservador necesitó cirugía aórtica durante el seguimiento. La supervivencia libre de complicación aórtica en el grupo conservador fue 93,2% a 1 año, 85,4% a 5 años y 49,3% a 10 años. La regresión Cox confirmó el tipo de lesión aórtica (hazard ratio [HR]: 2,94; p = 0,002) y puntuación TRISS > 50% (HR: 1,49; p = 0,042) al ingreso como factores de riesgo para el desarrollo de complicaciones aórticas tardías. Conclusiones: El manejo conservador de la RTA puede ser una opción terapéutica en pacientes altamente seleccionados, y requiere un estricto seguimiento clinicorradiológico. El tratamiento quirúrgico, abierto o endovascular, tiene unos excelentes resultados tanto a corto como a largo plazo.

21. A review of horses sent to slaughter for human consumption: impact of horsemeat consumption, residual banned drugs, and public health risks

Abstract

Nearly all of the American horses exported to Mexico and Canada are slaughtered for human consumption, and their meat is either exported around the world or consumed locally. Previous work showed that 18 Thoroughbred racehorses purchased by rescues that would have otherwise been sold for export for the sole purpose of slaughter to produce meat for human consumption were administered phenylbutazone. We report the number of American horses exported to Canada and Mexico from 2016 to 2021, the presence of contaminated horsemeat from Canadian slaughterhouses, and the human use and idiosyncratic effects of veterinary phenylbutazone and side effects of clenbuterol, 2 of the drugs that were found in contaminated Canadian horsemeat. The number of live American horses exported to Canada declined precipitously from 2016 to 2017, and a second decline occurred in 2020. All food-producing animals are under strict regulatory control to prevent animals administered banned drugs to enter the food chain. A major principle of this program is zero tolerance for banned drugs and testing for compliance. No regulatory process is in place to remove horses administered banned drugs such as phenylbutazone. The efficacy lasts for more than 24 hours as a result of the irreversible binding to cyclooxygenase, slow elimination, and long elimination half-life of its metabolite oxyphenbutazone. High or frequent doses of phenylbutazone result in disproportionately increased plasma concentrations, which result in the residual presence in tissues. It is this fact that underlies the ban of this drug in food-producing animals. No human clinical surveillance program is in place to monitor individuals on the possible short- and long-term consequences of banned drugs in contaminated horsemeat. If the United States is unable to put in place a regulatory program to remove horses administered banned drugs as exists for all food-producing animals, the exportation of American horses across both borders for the sole purpose of slaughter for human consumption must end.

Abstract

The current study included an examination of social factors that mitigate or exacerbate insomnia symptoms among older adults who are married or living with a partner. We first examined the unique effects of spousal support and strain on insomnia symptoms and then evaluated the degree to which extramarital social factors (e.g., friend support) moderated spousal influences. Data came from Waves 2 and 3 of the National Social Life, Health, and Aging Project. Our sample consisted of 495 participants who were either married or cohabitating with a partner ( M age in years = 69.84, SD = 8.08). Spousal strain—but not support—predicted higher insomnia symptoms 5 years later. Spousal influences on sleep, however, were moderated by extramarital factors in nuanced ways. Findings highlight the importance of taking into account older adults’ wider social context when examining the ways in which sleep is sensitive to positive and negative aspects of marital quality.

23. The days add up: Daily marital discord and depressive reactivity linked to past-month depressed mood and marital risk across 10 years

Abstract

Marital discord fuels depression, according to decades of research. Most prior studies in this area have focused on macro-longitudinal change in depression over the course of years, and on global ratings of marital satisfaction. Less work has examined fluctuations in depressed mood and marital discord in daily life, and none has investigated associations of short-term patterns with longer-term depressed mood and marital outcomes. Using data from participants in the Midlife in the U.S. project, the current study examined daily associations between marital discord and depressed mood, as well as their links to concurrent and prospective patterns of past-month depressed mood and marital risk. Results showed that, on average, depressed mood rose on days when individuals had an argument or tension with their spouse (i.e., marital discord), after accounting for the roles of other stressors. More frequent daily marital discord was also associated with greater past-month depressed mood and marital risk, above and beyond prior levels. Those with larger depressive mood responses to discord in daily life (i.e., greater reactivity) exhibited higher concurrent past-month depressed mood and greater 10-year increases in depressed mood. As the first study to link daily marital patterns to concurrent and prospective changes in depressed mood and marital outcomes, this investigation uncovered two novel processes—daily marital discord and depressive reactivity—as important for understanding long-term patterns of marital risk and depression.

Abstract

Although the marital relationship is often the primary source of emotional support in adulthood, sole reliance on the spouse to discuss health-related issues may be harmful to the well-being of both partners. The first aim of this study was to examine whether declines in health during later life would be associated with poorer psychological well-being in self and partner. We further investigated whether declining health would have a stronger impact on own and partner psychological well-being in the absence of non-spousal health confidants. Longitudinal actor-partner interdependence models (APIMs) were used to test both hypotheses with dyadic data from Wave 2 (2010–2011) and Wave 3 (2015–2016) of the National Social Life, Health, and Aging Project (NSHAP). Contrary to prediction, increased anxiety following spousal declines in gait speed and cognitive function occurred for those whose spouse did (rather than did not) have additional health confidants. A much fuller understanding is needed in regard to whether close relationships provide resources or present unwanted complications to dyadic coping, and the processes by which effects occur.

Funding Acknowledgements Type of funding sources: None. Introduction Left atrial appendage (LAA) is the most common source of thrombi in patients with atrial fibrillation (AF). The Totally Thoracoscopic (TT)-LAA exclusion with epicardial clip is an effective procedure, however the effects on left atrial (LA) function remain unknown. Purpose The aim of this study was to assess the impact of TT-LAA exclusion on the LA function in patients undergoing to standalone TT-LAA exclusion and patients received combined procedure, namely: TT-LAA exclusion and Thoracoscopic Epicardial Ablation (TEA). Methods 39 AF Patients underwent to TT-LAA exclusion with the clip. Indication to standalone TT-LAA exclusion was: high risk for ischemic stroke or contraindication to long-term anticoagulant therapy. All patients were screened preoperatively with 3D CT scan, transthoracic and trans-esophageal echocardiography. Intraoperative clip positioning and LAA exclusion were guided by trans-esophageal echo. To evaluate LA function, standard transthoracic echocardiography and 2D strain of LA were performed before surgery, at discharge (3-4 days after the procedure), at 3-month and at 1 year. Routine blood tests and BNP levels were also measured. Results The study enrolled 39 consecutive patients, 25 underwent to standalone TT-LAA exclusion (mean age 78.6 ± 5.7 years, 79% males), and 14 LAA exclusion during TEA (mean age 64.5 ± 8.2 years, 82% males). The mean CHA2DS2-VASc and HASBLED scores were 3.5 and 2.1 respectively. Baseline characteristics are summarized in Figure 1. There were no major complications during the procedure. At median follow-up of 13 months, 1 non cardiovascular death, 1 ischemic stroke and 4 cardiovascular hospitalizations occurred. At 2D strain of LA, the reservoir function decreased significantly after clip implantation and recovered at 3-months follow-up; at 1 year it remained stable. Furthermore, NT-proBNP increased significantly after LAA exclusion with a return to baseline after 3 months. Changes in E/A occurred at predischarge and did not persist at follow up. Echocardiographic findings are summarized in Figure 2. Patients with indication to TEA and LAA exclusion presented a lower LA volume and a lower strain value. Conclusion The main findings of this study can be summarized as follows: 1) The amputation of LAA is a safe procedure at long term follow up. 2) LAA amputation with the clip is associated with acute elevation of BNP levels the days after the procedure with return to baseline at 3 months and one year. 3) LAA amputation impairs the LA reservoir function after the procedure, with recovery after 3 months and remains preserved at one-year. This occurs either when LA strain is measured in sinus rhythm or in AF. 4) LAA exclusion results in a change in E/A that does not persist at three months follow-up and at 1 year. 5) Patients with indication to epicardial AF ablation and LAA exclusion present a lower LA volume with a lower strain value than patients undergoing standalone TT-LAA exclusion.

Background The assessment of the ventricular myocardial substrate critically depends on the size of mapping electrodes, their orientation with respect to wavefront propagation, and interelectrode distance. Objective we conducted a dual-center study to evaluate the impact of microelectrode mapping in patients undergoing catheter ablation (CA) of ventricular tachycardia (VT). Methods We included 21 consecutive patients (median age, 68 [12], 95% male) with structural heart disease undergoing CA for electrical storm (n=14) or recurrent VT (n=7) using the QDOT Micro catheter and a multipolar catheter (PentaRay, n=9). The associations of peak-to-peak maximum standard bipolar (BVc) and minibipolar (PentaRay, BVp) with microbipolar (BVμMax) voltages were respectively tested in sinus rhythm with mixed effect models. Furthermore, we compared the features of standard bipolar (BE) and microbipolar (μBE) electrograms in sinus rhythm at sites of termination with radiofrequency energy. Results BVμMax was moderately associated with both BVc (β=0.85, p Conclusions Microelectrode mapping is more sensitive than standard bipolar mapping in the identification of viable myocytes in SR, and may facilitate recognition of targets for catheter ablation.

Introduction Remote monitoring (RM) technologies have the potential to improve patient care by increasing compliance, providing early indications of heart failure (HF) decompensation, and potentially allowing for optimization of therapy to prevent HF admissions. The aim of this observational, retrospective study was to assess the clinical and economic consequences of RM vs standard monitoring (SM) through in-office cardiology visits, in patients carrying an implantable cardioverter defibrillator (ICD) or a cardiac resynchronization therapy defibrillator (CRTD). Methods Clinical and resource consumption data of this retrospective analysis were extracted from the Electrophysiology Registry of our Cardiology Unit, which has been systemically collecting patient information from January 2011 to February 2022. From a clinical standpoint, survival analysis was conducted, and incidence of CV-related hospitalizations was measured. From an economic standpoint, direct costs of RM and SM patients were collected to compare the cost per treated patient over a 2-year time horizon. Propensity score matching (PSM) was used to reduce the effect of confounding biases and the unbalance of patient characteristics at baseline. Results In the enrollment period, N=402 patients carrying ICD/CRTD met the inclusion criteria and were included in the analysis (N=189 patients -47.0%- followed through SM; N=213 patients -53.0%- followed through RM). After PSM, comparison was limited to N=191 patients in each arm (Figure 1). After a follow-up of 2 years since ICD/CRTD implantation, mortality rate for any cause was 1.6% in the RM group and 19.9% in the SM group (log-rank test, p Conclusion RM of patients carrying ICD/CRTD improves short-term (2-year) morbidity and mortality risks, compared to SM (based on the traditional in-office visit approach) and finally reduces direct management costs for both hospitals and healthcare services

Background Catheter ablation is an increasingly used treatment option for patients with recurrent ventricular tachycardia (VT) or electrical storm (ES). The QDOT Micro (QDOT) Catheter (Biosense Webster) is a novel radiofrequency ablation catheter that was found to allow rapid and effective pulmonary vein isolation among patients with atrial fibrillation. There is no published data on the safety and efficacy of catheter ablation for VT using the QDOT catheter, and few data allow a direct comparison of different technologies in this setting. Purpose to assess outcomes of CA for VT among patients with structural heart disease using the QDOT catheter, and compare them to those observed with the Thermocool Smarttouch (TC-ST, Biosense Webster) catheter. Methods we conducted a dual-center, observational, prospective study enrolling patients with structural heart disease and recurrent VT/ES undergoing catheter ablation with the QDOT catheter. For comparison, we performed a 1:1 propensity score matching to identify patients who underwent VT CA with the TC-ST catheter, using the nearest neighbor method. Propensity score was based on gender, age, etiology of structural heart disease, left ventricular ejection fraction, electrical storm, and number of previous CAs for VT. The primary efficacy outcome was freedom from recurrent VT, as assessed by implantable defibrillator (ICD) interrogation; the primary safety outcome was in-hospital complications. Results Forty propensity-matched patients were included in the analysis (QDOT, n=20; TC-ST, n=20); baseline clinical characteristics are resumed in Table. After a median follow-up of 7 months, survival free from any recurrent VT measured 74 (95% CI, 57-97)% and 69 (95% CI, 51-93)% in the QDOT and TC-ST groups, respectively (log-rank p=0.54, Figure A), while survival free from ICD shocks was 95 (95% CI, 86-100)% versus 84 (95% CI, 69-100)% (log-rank p=0.26, Figure B). There were no major complications in the QDOT group, while three patients (15%) in the TC-ST had a primary safety event (p=0.23; femoral pseudoaneurysm, n=1; cardiac tamponade, n=1; aortic cusp perforation, n=1). Conclusion In a cohort of patients with structural heart disease and recurrent VT or ES, the novel QDOT ablation catheter had similar efficacy and safety compared to the TC-ST catheter. Interestingly, no major procedural complications were observed with the QDOT catheter, suggesting that this device may be safe even in most challenging patients.

Background An increasing number of patients need cardiac implantable electronic devices (CIED) for bradycardia or to prevent sudden cardiac death. At the same time many patients require radiotherapy for oncologic reasons. Interaction between CIEDs and ionizing radiations may cause several dysfunctions: electromagnetic interferences due to oversensing, CIED reset, drainage of the battery, hardware damage to the pulse generator and to the lead-tissue interface. Purpose An adequate planning is expected to guarantee the safety of CIED patients, reducing the potential complications. We describe our protocol in use and its application. Methods In 2016 an internal protocol was written to standardize the approach to CIED patients undergoing radiotherapy. Cardiology Department, Radiotherapy Unit and Medical Physics were involved in the draft. We stratified patients in high and low risk groups according to the CIED characteristics (ICD vs pacemaker), electro-dependency (yes vs no), and the estimated radiation dose (< 2 Gy: 2-10 Gy, > 10 Gy). Low risk group: - Non pacemaker (PM) dependent patients, estimated dose < 10 Gy - PM dependent patients, estimated dose < 2 Gy High risk group: - PM dependent patients, estimated dose > 2 Gy - Non-PM dependent patients estimated dose >10 Gy - ICD patients Two EHRA certificated nurses followed up the patients before, during and after the radiotherapy. Low risk patients received weekly in office evaluation during radiotherapy course. High risk patients were scheduled for daily in office check plus monitoring during each radiotherapy session, ICD therapy was temporarily switched off during each radiotherapy session. An in office evaluation was planned 1, 3, 6 and 12 months after the end of radiotherapy course until 2019, since 2020 all patients were given remote monitoring for accurate follow up. Results Between 2016 and November 2022 101 patients with CIED underwent radiotherapy in our facility. Clinical characteristics of the patients are described in Table 1. EHRA certificated nurses managed 1282 radiotherapy sessions without any complication. A transient underestimation of ICD longevity was the only software error occurred. Conclusions The reported protocol assured a safe radiotherapy course for all our CIED patients. Given the limited data available about the interaction between the newest CIEDs and radiotherapy, we opted for a high safety profile. The EHRA certified nurse’s involvement allowed to optimize personnel resources.

32. Military couples' experiences in the aftermath of a cancelled deployment

Abstract

Deployment requires considerable preparation for military families and changes to these plans may create notable stress. The current study leveraged data from a sample of military couples who experienced the cancellation of an overseas deployment to learn more about their experiences as they adjusted to this change. Guided by family stress and anticipatory stress perspectives, we analyzed qualitative data from 28 service members and their significant others (i.e., spouses or cohabitating partners) to understand their overall reactions to the deployment cancellation. We identified three overall reactions (positive, negative, and ambivalent) that were based on participants' appraisals of-and preparations for-deployment, as well as ambiguity about family roles and relationships. Further, participants across groups experienced uncertainty about whether or not the deployment would occur, and altered timelines for other life events. Together, our findings highlight the post-cancellation period as a significant time of stress and transition for military families. However, our findings also signify the need to help all military families cope with uncertainty about when or if deployments will occur given that the military's priorities are often in flux. We therefore describe coping efforts that may be particularly adaptive for families to engage in as they prepare for uncertain, anticipated stressors.La comisión militar exige una preparación considerable para las familias de militares, y los cambios en estos planes pueden generar bastante estrés. En el presente estudio se utilizaron datos de una muestra de parejas de militares a quienes se les suspendió una comisión fuera del país para averiguar más acerca de sus experiencias mientras se adaptaban a este cambio. Guiados por las perspectivas de estrés familiar y de estrés anticipatorio, analizamos datos cualitativos de 28 miembros del servicio militar y sus parejas (p. ej.: cónyuges o concubinos) para comprender sus reacciones generales a la suspensión de la comisión militar. Identificamos tres reacciones generales (positiva, negativa y ambivalente) que se basaron en las apreciaciones de la comisión y en los preparativos para esta que hicieron los participantes, así como en la ambigüedad acerca de los roles y las relaciones familiares. Además, los participantes de los distintos grupos sintieron incertidumbre acerca de si se produciría o no la comisión y cambiaron programas por otros acontecimientos de la vida. Juntos, nuestros resultados destacan el periodo posterior a la suspensión como un momento de estrés considerable y de transición para las familias de militares. Sin embargo, nuestros resultados también indican la necesidad de ayudar a todas las familias de militares a afrontar la incertidumbre acerca de cuándo ocurrirán las comisiones o de si estas ocurrirán, dado que las prioridades de los militares generalmente cambian constantemente. Abstract

Non-linear excitation microscopy offers several advantages for in-vivo imaging compared to conventional confocal techniques. However, tissue penetration can still be an issue due to scattering and spherical aberrations induced on focused beams by the tissue. The use of low numerical aperture objectives to pass through the outer layers of the skin, together with high dioptric power microlenses implanted in-vivo close to the observation volume, can be beneficial to the reduction of optical aberrations. Here, we develop and test on fibroblast cell culture plano-convex microlenses to be used for non-linear imaging of biological tissue. The microlenses can be used as single lenses or multiplexed in an array. A thorough test of the lenses wavefront is reported together with the modulation transfer function and wavefront profile. We could retrieve magnified fluorescence images through the microlenses coupled to commercial confocal and two-photon excitation scanning microscopes. The signal-to-noise ratio of the images is not substantially affected by the use of the microlenses and the magnification can be adjusted by changing the relative position of the microlens array to the microscope objective and the immersion medium. These results are opening the way to the application of implanted micro-optics for optical in-vivo inspection of biological processes.

Abstract

Background Acute myocarditis (AM) is thought to be a rare cardiovascular complication of COVID–19, although minimal data are available beyond case reports. We aim to report the prevalence, baseline characteristics, in–hospital management, and outcomes for patients with COVID–19–associated AM on the basis of a retrospective cohort from 23 hospitals in the United States and Europe. Methods A total of 112 patients with suspected AM from 56963 hospitalized patients with COVID–19 were evaluated between February 1, 2020, and April 30, 2021. Inclusion criteria were hospitalization for COVID–19 and a diagnosis of AM on the basis of endomyocardial biopsy or increased troponin level plus typical signs of AM on cardiac magnetic resonance imaging. We identified 97 patients with possible AM, and among them, 54 patients with definite/probable AM supported by endomyocardial biopsy in 17 (31.5%) patients or magnetic resonance imaging in 50 (92.6%). We analyzed patient characteristics, treatments, and outcomes among all COVID–19–associated AM. Results AM prevalence among hospitalized patients with COVID–19 was 2.4 per 1000 hospitalizations considering definite/probable and 4.1 per 1000 considering also possible AM. The median age of definite/probable cases was 38 years, and 38.9% were female. On admission, chest pain and dyspnea were the most frequent symptoms (55.5% and 53.7%, respectively). Thirty–one cases (57.4%) occurred in the absence of COVID–19–associated pneumonia. Twenty– one (38.9%) had a fulminant presentation requiring inotropic support or temporary mechanical circulatory support. The composite of in–hospital mortality or temporary mechanical circulatory support occurred in 20.4%. At 120 days, estimated mortality was 6.6%, 15.1% in patients with associated pneumonia versus 0% in patients without pneumonia (P=0.044). During hospitalization, left ventricular ejection fraction, assessed by echocardiography, improved from a median of 40% on admission to 55% at discharge (n=47; P Conclusions AM occurrence is estimated between 2.4 and 4.1 out of 1000 patients hospitalized for COVID–19. The majority of AM occurs in the absence of pneumonia and is often complicated by hemodynamic instability. AM is a rare complication in patients hospitalized for COVID–19, with an outcome that differs on the basis of the presence of concomitant pneumonia.

Abstract

Background Cardiogenic shock (CS) after acute myocardial infarction (AMI) is a life-threatening condition with an high rate of in-hospital mortality. Purpose This study aims to 1) describe predictors of in-hospital mortality; 2) evaluate ten years mortality temporal trend in our Cardiac Intensive Care Unit (CICU); 3) assess the feasibility of CARDSHOCK risk score in our population; 4) elaborate a simpler version of CARDSHOCK risk score. Methods All consecutive patients with CS after AMI admitted at our CICU from March 2012 to July 2021 were included in this single-centre retrospective study. Results We included 167 patients [males 67%; age 71 (61–80) years] with ischemic CS. Patients had severe LV dysfunction in 66%. Baseline serum lactate was 5.2 (3.1–8.8) mmol/L. All patients required inotropes: 71% required dopamine [mean dose 5.6 (2.4–11,3) mcg/kg/min], 65% required noradrenaline [mean dose 0.10 (0.05–0.18) mcg/kg/min], 32% required dobutamine [mean dose 4.5 (2.2–15.9) mcg/kg/min]; 17.4% received levosimendan alone [mean dose 0.1 mcg/kg/min]. Mechanical cardiac support (MCS) was pursued in 91.1% [65% IABP, 23% Impella CP, 4% VA-ECMO]. From March 2012 to July 2021 we observed a significative temporal trend mortality reduction (OR=0.90, 95% CI: 0.84–0.96, p=0.0015), in particular in-hospital mortality has reduced from 57% of first time-quartile to 29% of the fourth quartile (Figure 1). In addition we also noted a significant increase in Impella catheter use (p=0.0005) with a consequent reduction of IABP (p=0.01), a reduction in dopamine administration (p=0.0007) and a greater use of dobutamine and levosimendan (p=0.015 and p=0.0001). In our population of AMI-CS patients CARDSHOCK risk score was a reliable in-hospital mortality predictor tool (OR 1.11; 95% CI, 1.06–1.17; p=0.00011). After the multivariate analysis only ejection fraction (EF) at baseline (OR=0.99, 95% CI: 0.98–0.99, p=0.009), lactate level at presentation (OR=1.03, 95% CI: 1.01–1.06, p=0.015) and presence of three-vessels coronary artery disease (OR=0.73, 95% CI: 0.59–0.90, p=0.0038) resulted to be in-hospital mortality predictors. For this reason, a prediction model composed by those three variables was created which exhibited better predictive performance for in-hospital mortality than Cardshock risk score (AUC of 0.94 vs AUC of 0.72 respectively, p=0.015) (Figure 2). Conclusions In our retrospective single-centre study a significant reduction of mortality through the years is observed, probably due to more extensive use of micro axial pumps and better manipulation of inotropic drug therapies. The use of Cardshock risk score has been proven to be a feasible tool in prediction on in-hospital mortality also in our sample composed only of AMI-CS patients. In addition, a more simplified risk score made up of only three clinical variables demonstrates at least the same predictive performance. Abstract

Objectives Although the adverse link between rumination and sleep quality is well established, much of the literature neglects the role of social factors. This study examined the role of older adults’ perceived social support from spouses and from family/friends in modifying the association between trait rumination and sleep quality. Existing hypotheses suggest that social support may play 3 unique roles, each tested within the current study: (H1) support may act as a protective factor that buffers negative effects of rumination on sleep quality, (H2) support may curtail rumination and, in turn, promote sleep quality, and (H3) rumination may erode support and, in turn, undermine sleep quality. Method Data came from 86 partnered older adults in independent-living or retirement communities (Mage = 75.70 years). We utilized 3 waves of interview data collected annually between 2017 and 2019. The first hypothesis was tested using moderation in multilevel models; the second 2 hypotheses were evaluated with prospective associations using multilevel mediation. Results Negative effects of high-trait rumination on time-varying sleep quality were attenuated among those who reported high, stable levels of support from their spouses. Perceived family/friend support did not yield the same protective effect. There was no evidence that support preempted, or was eroded by, rumination. Discussion Perceived spousal support may act as a psychosocial resource that mitigates negative effects of trait rumination on older adults’ sleep quality. Interventions aimed at mitigating maladaptive outcomes of rumination on sleep quality for older adults should consider spousal support as a key target.

Introduction The implantable cardioverter-defibrillator (ICD) is an established therapy for the prevention of sudden cardiac death in selected patients. Although current guidelines apply to both women and men, there is a growing awareness that the incidence of cardiac arrhythmias and device interventions is influenced by sex. Purpose To investigate sex-specific risk of sustained ventricular arrhythmias (SVAs) and device therapies, using remote monitoring data from ICDs and cardiac resynchronization therapy defibrillators (CRT-Ds). Methods Study endpoints were time to the first appropriate SVA, time to the first appropriate device therapy for SVA, and time to the first ICD shock. Appropriateness of device-detected SVAs was adjudicated by three expert electrophysiologists. Results were compared between women and a 1:1 propensity score (PS)-matched subgroup of men. Results In a cohort of 2,532 patients with an ICD or CRT-D (median age, 70 years), 488 patients (19.3%) were women. As compared to men, women more frequently had a CRT-D (51% vs. 40%, p Conclusion In our analysis of remote monitoring data, women exhibited a lower SVA risk profile than PS-matched men in the subgroup of patients with an ICD or/and ejection fraction ≥30%.

39. Relationship Talk With Partners and Friends During Emerging Adulthood: The Role of Relationship Satisfaction

Abstract

Research indicates that discussing one’s romantic relationship with one’s partner benefits individual well-being and reduces uncertainty about the future of the relationship. Implications of relationship talk with friends remain less clear, though talking with friends may actually increase uncertainty about the relationship (e.g., by making one’s partner jealous of these friends), particularly for emerging adults. Relationship talk with friends may be especially likely to promote relational uncertainty for couples who are already unsatisfied in their relationships. In this study, we explored whether relationship talk with one’s partner and one’s friends would each be uniquely associated with depressive symptoms and uncertainty about the relationship, specifically in the form of perceived partner jealousy of one’s friends and whether these associations would be moderated by relationship satisfaction. Results from a series of path models using data from 202 romantically involved emerging adults in the United States revealed that associations between relationship talk and outcomes were indeed moderated by relationship satisfaction. For example, only in unsatisfied relationships was relationship talk with friends positively associated with a partner's jealousy and negatively associated with depressive symptoms. This research expands our understanding of relationship talk by differentiating between talk with partners versus friends, while considering the contextual role of relationship satisfaction.

Abstract

Hyperexcitability is a major mechanism implicated in several neuropsychiatric disorders, such as organophosphate-induced status epilepticus (SE), primary epilepsy, stroke, spinal cord injury, traumatic brain injury, schizophrenia, and autism spectrum disorders. Underlying mechanisms are diverse, but a functional impairment and loss of GABAergic inhibitory neurons are common features in many of these disorders. While novel therapies abound to correct for the loss of GABAergic inhibitory neurons, it has been difficult at best to improve the activities of daily living for the majority of patients. Alpha-linolenic acid (ALA) is an essential omega-3 polyunsaturated fatty acid found in plants. ALA exerts pleiotropic effects in the brain that attenuate injury in chronic and acute brain disease models. However, the effect of ALA on GABAergic neurotransmission in hyperexcitable brain regions involved in neuropsychiatric disorders, such as the basolateral amygdala (BLA) and CA1 subfield of the hippocampus, is unknown. Administration of a single dose of ALA (1500 nmol/kg) subcutaneously increased the charge transfer of inhibitory postsynaptic potential currents mediated by GABAA receptors in pyramidal neurons by 52% in the BLA and by 92% in the CA1 compared to vehicle animals a day later. Similar results were obtained in pyramidal neurons from the BLA and CA1 when ALA was bath-applied in slices from naïve animals. Importantly, pretreatment with the high-affinity, selective TrkB inhibitor, k252, completely abolished the ALA-induced increase in GABAergic neurotransmission in the BLA and CA1, suggesting a brain-derived neurotrophic factor (BDNF)-mediated mechanism. Addition of mature BDNF (20 ng/mL) significantly increased GABAA receptor inhibitory activity in the BLA and CA1 pyramidal neurons similar to the results obtained with ALA. ALA may be an effective treatment for neuropsychiatric disorders where hyperexcitability is a major feature.

Abstract

A 66–year–old man was suddenly referred to the emergency department (ED) for an extra–hospital cardiac arrest due to ventricular fibrillation, promptly treated by a single DC shock. Four months prior, he had undergone aortic valve replacement with a biological valve prosthesis (BVP, Perimount Magna n. 23) for severe aortic stenosis. One month after cardiac surgery the patient was hospitalized for acute cholecystitis requiring antibiotics and percutaneous cholecystostomy. On arrival to the ED the patient was awake and completely asymptomatic; no fever during days before the episode. He had been complaining of constricting retrosternal chest pain during physical exertion for ten days. However, no signs of ischemia were seen on the EKG. TT echo showed a very large pulsatile anaechogenic area posteriorly to BVP. Transesophageal echocardiography (TEE 2D, Traditional 3D and TrueVue) highlighted a very large pseudoaneurysm of mitral–aortic interventricular fibrosa (P–MAIVF; Figures 1 and 2, long axis and short–axis views). TEE features included: 1) a very large cavity with thickened walls, extended for more than two–thirds of the prosthetic circumference and with multiple septa; 2) a large communication between the left ventricular outflow tract (LVOT) and P–MAIVF (see yellow dotted line in Fig.1A) with a to–and–fro flow into the cavity and a prominent systolic pulsation; 3) no paraprosthetic leak and absence of any attached mass or intraprosthetic aortic regurgitation. These findings suggested a prosthetic aortic valve endocarditis with an extensive periannular involvement. Moreover, the P–MAIVF may have led to “ab extrinseco” compression of the left coronary artery, with a potential role in the genesis of chest pain and ventricular arrhythmia (in absence of significant CAD at preoperative coronary angiography). An empiric antibiotic therapy with daptomycin and piperacillin–tazobactam was started. The patients subsequently underwent Bentall surgery with a subannular implantation technique (Perimount valvular prosthesis n.23 and Gelweave vascular prosthesis n.28). Figure 3 shows an intraoperative image and postoperative TEE. In conclusion, P–MAIVF is an ominous complication of prosthetic valve endocarditis. Presentation with exertional angina and cardiac arrest is unusual. TEE is sensitive for diagnosis and 3D echo provides better insights into the anatomy helping formulate an appropriate surgical strategy.

In AHF setting clinical trial and registries showed an improvement in systemic haemodynamics without a decrease in in–hospital mortality. Purpose: evaluate safety and efficacy of Levosimendan in an ischemic CS population. Methods All consecutive patients with CS after AMI admitted at our CICU from March 2012 to July 2021 were included in this single–centre retrospective study. 39 patients received Levosimendan alone or in combination with other inotropic drugs. All patients had an invasive monitoring of arterial pressure and continuous ECG–monitoring. Patients with severe renal failure were excluded. An echocardiogram was performed before drug infusion and 3 days after. We collected data from haemodynamics and from ECG monitoring looking for arrhythmic events. Results We enrolled 167 patients with CS after AMI, 39 patients received Levosimendan and 10 of them required association with low–dosage noradrenaline. 32 received Levosimendan at dosage of 0,1 mcg/kg/min. The drug was started on average 4 days after hospitalization. Statistical analysis showed a significant change in left ventricular EF assessed by echocardiogram from 25% to 31% (P = 0.018, fig 1). We observed also an improvement of symptoms with an overall relief of dyspnea and fatigue clinically evaluated. The ECG monitoring showed ventricular arrhythmias (VA) in 11 patients (NSVT, SVT, VF) but the most frequent arrhythmia after Levosimendan was atrial fibrillation (AF) which was significantly associated with in–hospital mortality (OR = 1,82, p = 0,002), whereas VA did not. Moreover patients with new onset AF after Levosimendan showed a lower clinical and instrumental response to the drug. No significant changes in electrolytes (sodium and potassium) have been documented during arrhythmic events. Conclusion Levosimendan is useful in CS after AMI setting because has demonstrated to improve systemic hemodynamics parameters and symptomatology. Overall Levosimendan is safe with a low rate of ventricular arrhythmias that, in our sample, are not associated with in–hospital mortality. Moreover we observed also a significant increase in AF that was associated with in–hospital mortality, but further studies in larger populations could be advisable to describe the real incidence of AF after Levosimendan infusion and its prognostic role in CS patients.

Abstract

Background Plasmatic procalcitonin (PCT) is an acute–phase inflammatory marker, its value tends to increase during inflammatory conditions such as septic shock, cardiogenic shock (CS) and bacterial infections. Patients with CS complicating acute myocardial infarction (AMI) and signs of systemic inflammatory response syndrome (SIRS) can show increased PCT values but its prognostic role in this setting is still unknown. Purpose: evalute prognostic role of plasmatic PCT in an ischemic CS population. Methods All consecutive patients with CS after AMI admitted at our CICU from March 2012 to July 2021 were included in this single–centre retrospective study. We evaluated PCT and C reactive protein (CRE) values in blood sample only in patients with clinical signs of SIRS or infection. Microbiological examination were also collected to exclude a bacterial infection. Results We included 167 patients [males 67%; mean age 71 years] with ischemic CS. Patients had severe LV dysfunction in 66%. Baseline serum lactate was 5,2 (3,1–8,8) mmol/L. All patients required inotropes while 91,1% required mechanical cardiac support (MCS). The overall in–hospital mortality rate was 46%. The PCT values resulted to be an in–hospital mortality predictor at multivariate analysis (p = 0,018) and we observed a generally lower PCT values in surviving CS patients (Fig 1). At the analysis the optimal maximum PCT cut–off value to predict in–hospital mortality was 5,77 ng/l (SE 0,82; SP 0,72, accuracy 0,78) with an area under the receiver operating characteristic curve of 0.80 (Fig 2). Moreover positivity to microbiological examination such as urine colture, blood colture, bronchoalveolar lavage and maximum CPR values were not associated with in–hospital death at univariable analysis. Conclusions in our retrospective study we found that PCT is an in–hospital mortality predictor and values greater than 5,77 ng/ml seem to be a good indicator of patients with poor prognosis in ischemic CS patients. High PCT values could have a role in indentifing CS patients with a intense SIRS or with a septic shock component which is associated with a even higher in–hospital mortality rate. The positivity to microbiological examination and maximum CPR values weren’t death predictors in our sample. Further studies in larger CS population could be advisable to better define prognostic role of PCT and evaluate its role in CICU daily clinical practice.

Abstract

A study was conducted to determine the effect of three levels of selenium (Se) supplementation on the libido, scrotal circumference, sperm quality, and testosterone levels in matured Boer bucks. The feeding trial was conducted at MARDI Bachok Station, Bachok, Kelantan, Malaysia, for over six months. A total of 18 matured Boer bucks aged two years with an initial body weight of 53.28±0.62 kg, and no experience of sexual activity was selected for the feeding trial. All animals were randomly divided into three dietary treatments based on a completely randomized design (CRD). Group A and B were supplemented with organic Se at doses of 0.18 mg/kg DM and 0.36 mg/kg DM, respectively, whereas group C acted as a control group (without Se supplementation). Their basal diet consisted of 60% formulated pellet and 40% Brachiaria dictyoneura grass. Daily feed offered (DM basis) was based on 3% of mean body weight. Libido, scrotal circumference, and sperm quality were measured every two months during the feeding trial. Testosterone levels were determined at the end of the feeding trial. The results indicated that the basal diet (control group) was deficient in Se, with the concentration of 0.004 mg/kg. Supplementation of 0.18 mg Se/kg DM only improved the libido and sperm quality (p<0.05). Supplementation of 0.36 mg Se/kg DM significantly (p<0.05) increased libido, sperm quality, and testosterone levels. It is summarized that supplementation of 0.36 mg Se/kg DM has the potential to improve the reproductive performance of bucks, although it occurred at the adult stage.

Abstract

Measurements of jet substructure describing the composition of quark- and gluon-initiated jets are presented. Proton-proton (pp) collision data at root s = 13 TeV collected with the CMS detector are used, corresponding to an integrated luminosity of 35.9 fb(-1). Generalized angularities are measured that characterize the jet substructure and distinguish quark- and gluon-initiated jets. These observables are sensitive to the distributions of transverse momenta and angular distances within a jet. The analysis is performed using a data sample of dijet events enriched in gluon-initiated jets, and, for the first time, a Z+jet event sample enriched in quark-initiated jets. The observables are measured in bins of jet transverse momentum, and as a function of the jet radius parameter. Each measurement is repeated applying a “soft drop” grooming procedure that removes soft and large angle radiation from the jet. Using these measurements, the ability of various models to describe jet substructure is assessed, showing a clear need for improvements in Monte Carlo generators.

Abstract

Inclusive and differential cross sections of single top quark production in association with a Z boson are measured in proton-proton collisions at a center-of-mass energy of 13 TeV with a data sample corresponding to an integrated luminosity of 138 fb(-1) recorded by the CMS experiment. Events are selected based on the presence of three leptons, electrons or muons, associated with leptonic Z boson and top quark decays. The measurement yields an inclusive cross section of 87.9(-7.3)(+7.5) (stat)(-6.0)(+7.3)(syst) fb for a dilepton invariant mass greater than 30 GeV, in agreement with standard model (SM) calculations and represents the most precise determination to date. The ratio between the cross sections for the top quark and the top antiquark production in association with a Z boson is measured as 2.37(-0.42)(+0.56)(stat)(-0.13)(+0.27) (syst). Differential measurements at parton and particle levels are performed for the first time. Several kinematic observables are considered to study the modeling of the process. Results are compared to theoretical predictions with different assumptions on the source of the initial-state b quark and found to be in agreement, within the uncertainties. Additionally, the spin asymmetry, which is sensitive to the top quark polarization, is determined from the differential distribution of the polarization angle at parton level to be 0.54 +/- 0.16 (stat)+/- 0.06 (syst), in agreement with SM predictions.
