Abstract

Summary: Background: People with diabetes are at increased risk of hospitalisation, morbidity, and mortality following SARS-CoV-2 infection. Long-term outcomes for people with diabetes previously hospitalised with COVID-19 are, however, unknown. This study aimed to determine the longer-term physical and mental health effects of COVID-19 in people with and without diabetes. Methods: The PHOSP-COVID study is a multicentre, long-term follow-up study of adults discharged from hospital between 1 February 2020 and 31 March 2021 in the UK following COVID-19, involving detailed assessment at 5 and 12 months after discharge. The association between diabetes status and outcomes were explored using multivariable linear and logistic regressions. Findings: People with diabetes who survived hospital admission with COVID-19 display worse physical outcomes compared to those without diabetes at 5- and 12-month follow-up. People with diabetes displayed higher fatigue (only at 5 months), frailty, lower physical performance, and health-related quality of life and poorer cognitive function. Differences in outcomes between diabetes status groups were largely consistent from 5 to 12-months. In regression models, differences at 5 and 12 months were attenuated after adjustment for BMI and presence of other long-term conditions. Interpretation: People with diabetes reported worse physical outcomes up to 12 months after hospital discharge with COVID-19 compared to those without diabetes. These data support the need to reduce inequalities in long-term physical and mental health effects of SARS-CoV-2 infection in people with diabetes. Abstract

Background The long-term outcomes of COVID-19 hospitalisation in individuals with pre-existing airway diseases are unknown. Methods Adult participants hospitalised for confirmed or clinically suspected COVID-19 and discharged between 5 March 2020 and 31 March 2021 were recruited to the Post-hospitalisation COVID-19 (PHOSP-COVID) study. Participants attended research visits at 5 months and 1 year post discharge. Clinical characteristics, perceived recovery, burden of symptoms and health-related quality of life (HRQoL) of individuals with pre-existing airway disease (i.e., asthma, COPD or bronchiectasis) were compared to the non-airways group. Results A total of 615 out of 2697 (22.8%) participants had a history of pre-existing airway diseases (72.0% diagnosed with asthma, 22.9% COPD and 5.1% bronchiectasis). Abstract

Summary: Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. Background: Most studies of immunity to SARS-CoV-2 focus on circulating antibody, giving limited insights into mucosal defences that prevent viral replication and onward transmission. We studied nasal and plasma antibody responses one year after hospitalisation for COVID-19, including a period when SARS-CoV-2 vaccination was introduced. Methods: In this follow up study, plasma and nasosorption samples were prospectively collected from 446 adults hospitalised for COVID-19 between February 2020 and March 2021 via the ISARIC4C and PHOSP-COVID consortia. IgA and IgG responses to NP and S of ancestral SARS-CoV-2, Delta and Omicron (BA.1) variants were measured by electrochemiluminescence and compared with plasma neutralisation data. Findings: Strong and consistent nasal anti-NP and anti-S IgA responses were demonstrated, which remained elevated for nine months (p < 0.0001). Nasal and plasma anti-S IgG remained elevated for at least 12 months (p < 0.0001) with plasma neutralising titres that were raised against all variants compared to controls (p < 0.0001). Of 323 with complete data, 307 were vaccinated between 6 and 12 months; coinciding with rises in nasal and plasma IgA and IgG anti-S titres for all SARS-CoV-2 variants, although the change in nasal IgA was minimal (1.46-fold change after 10 months, p = 0.011) and the median remained below the positive threshold determined by pre-pandemic controls. Samples 12 months after admission showed no association between nasal IgA and plasma IgG anti-S responses (R = 0.05, p = 0.18), indicating that nasal IgA responses are distinct from those in plasma and minimally boosted by vaccination. Interpretation: The decline in nasal IgA responses 9 months after infection and minimal impact of subsequent vaccination may explain the lack of long-lasting nasal defence against reinfection and the limited effects of vaccination on transmission. Background: The risk factors for recovery from COVID-19 dyspnoea are poorly understood. We investigated determinants of recovery from dyspnoea in adults with COVID-19 and compared these to determinants of recovery from non-COVID-19 dyspnoea. Methods: We used data from two prospective cohort studies: PHOSP-COVID (patients hospitalised between March 2020 and April 2021 with COVID-19) and COVIDENCE UK (community cohort studied over the same time period). PHOSP-COVID data were collected during hospitalisation and at 5-month and 1-year follow-up visits. COVIDENCE UK data were obtained through baseline and monthly online questionnaires. Dyspnoea was measured in both cohorts with the Medical Research Council Dyspnoea Scale. We used multivariable logistic regression to identify determinants associated with a reduction in dyspnoea between 5-month and 1-year follow-up. Findings: We included 990 PHOSP-COVID and 3309 COVIDENCE UK participants. We observed higher odds of improvement between 5-month and 1-year follow-up among PHOSP-COVID participants who were younger (odds ratio 1.02 per year, 95% CI 1.01–1.03), male (1.54, 1.16–2.04), neither obese nor severely obese (1.82, 1.06–3.13 and 4.19, 2.14–8.19, respectively), had no pre-existing anxiety or depression (1.56, 1.09–2.22) or cardiovascular disease (1.33, 1.00–1.79), and shorter hospital admission (1.01 per day, 1.00–1.02). Similar associations were found in those recovering from non-COVID-19 dyspnoea, excluding age (and length of hospital admission). Interpretation: Factors associated with dyspnoea recovery at 1-year post-discharge among patients hospitalised with COVID-19 were similar to those among community controls without COVID-19. Abstract

This article investigates the effect of sucrose addition on the formation of casein gels by acidification and/or renneting of pure micellar casein. Gelation kinetics and gel properties were followed by rheological methods, and microscopy and syneresis measurements were used to obtain a more complete characterization of the structures formed. Sucrose content has been identified as a key parameter for controlling the kinetics of aggregation and the strength of the final gels. Results have shown that the effect of sucrose on gelation can vary such that effects can be completely reversed depending on the gelation route used. During acid gelation, addition of up to 30% (wt/wt) sucrose causes gels to form more rapidly and at higher pH values, and to have higher viscoelastic moduli and a more homogeneous microstructure than those without sucrose. By contrast, gels formed by renneting in the presence of sucrose are weaker and have longer gelation times. It is proposed that sucrose reduces solvent quality and causes the collapse of the "hairy" kappa-casein brush on the surface of the casein micelles. This may explain why sucrose increases the possibility of gel formation during acidification and reduces the degree of kappa-casein hydrolysis during renneting.

Phase equilibria of milk protein/locust bean gum systems in high levels of sucrose were investigated to understand the behaviour of these components in diary products and also to look at the particular effect of sucrose. Two milk protein systems were investigated, skimmed milk (SMP) and native phosphocaseinate (PCN). The main advantage of using this latter sample is that it is representative of the casein micelles of milk and allows understanding of what happens in a model system with controlled sugar and salt content. At a temperature of 5°C, pH 6.8, and at an ionic strength of 0.08 M, these mixtures tend to phase separate because of high incompatibility. The similarity between the behaviour of SMP/LBG and micellar casein/LBG systems is evident, suggesting that the incompatibility between skimmed milk and locust bean gum is largely attributable to the incompatibility of micellar casein and locust bean gum. Ternary phase diagrams, in the absence or presence of 20%, 30% or 40% (w/w) sucrose, were established and compared. Sucrose has only a slight influence on the thermodynamic compatibility. The addition of sucrose leads to a concentration effect on the protein phase and dilution of the locust bean gum phase. Some rheological and turbidity measurements, as well as observations using confocal laser scanning microscopy and electron microscopy, were carried out and demonstrated that for the concentrations, temperatures and times investigated, there is no interaction between the components. The microstructures of mixtures have also been investigated in order to study the concentration effect of each biopolymer as well as the sucrose effect. It was clearly demonstrated that the microstructure is driven by the phase volume ratio and this probably leads to a difference in mechanical properties.

Abstract

The stability of dairy emulsions containing both proteins and polysaccharides is mainly controlled by the nature of the protein–polysaccharide interaction. This work describes the behaviour of a micellar casein/locust bean gum (LBG) model system, and examines the effect of adding sucrose. For such systems a very high incompatibility has been demonstrated leading to thermodynamic separation into two distinct phases, one rich in protein and the other in polysaccharide. The second virial coefficient approach described by Edmond and Ogston was adopted to clarify the effect of sucrose on the micellar casein/LBG phase diagrams.

Abstract

Oxidation of hydrogen (0.1–2.0% v/v) in air over supported palladium catalysts at 300–625 K and atmospheric pressure has been investigated using a microcalorimetric method. The kinetics are influenced by reactor design, with activated processes at low temperatures and diffusion-limited rates at high temperatures. The mechanism involves a surface layer of PdO and weakly adsorbed H 2 . Water causes inhibition and reversible deactivation, particularly at low temperatures. The reaction is strongly inhibited by bromomethane (0.1–1.0% v/v) which competes with hydrogen for adsorption sites.

Abstract

The four LEP collaborations, ALEPH, DELPHI, L3 and OPAL, have collected a total of 2461 pb−1 of e+ e− collision data at centre-of-mass energies between189 and 209 GeV. The data are used to search for the Standard Model Higgs boson. The search results of the four collaborations are combined and examined in a likelihood test for their consistency with two hypotheses: the background hypothesis and the signal plus background hypothesis. The corresponding confidences have been computed as functions of the hypothetical Higgs boson mass. A lower bound of114.4 GeV/c2 is established, at the 95% confidence level, on the mass of the Standard Model Higgs boson. The LEP data are also used to set upper bounds on the HZZ coupling for various assumptions concerning the decay of the Higgs boson. © 2016, Elsevier. All rights reserved.

Abstract

A modified direct volume rendering algorithm is presented suppaning simultaneous display of MO source modalities via a spatially-variant Region-oflnterest func- tion that weights the combination of individually-specified opaciry transfer functions. The algorithm is applied within a focus + context paradigm. Examples are presented using computed tomography and colour histological image data from the Visible Human Project. The algorithm is imple- mented as a two stage process. During thefirst, computa- tionally expensive, stage global raycasting generates an ar- ray ofpanial projection segments for each displayed pixel; during the second, image generation supplements segment combination with local raycasting amund the ROI - this can be much more rapid. The visual data fusion approach is compared wirh convenrional rendering and shown to pro- vide an effective method of exploring complex 3-0 multi- modality data.

Abstract

Polysaccharides are widely used in the dairy industry as stabilising, thickening and gelling agents and locust bean gum (LBG) is one of the most commonly used. Together with the milk proteins, they form a pseudoternary milk protein-polysaccharide-water solution. The aim of this study was to build up a more detailed thermodynamic and kinetic understanding of molecular interactions between milk protein and LBG and to look especially at the effect of sucrose. Ternary phase diagrams in absence or in presence of sucrose (20, 30 or 40% w/w) were established and compared. For such a system, a very high degree of incompatibility has been demonstrated leading to thermodynamic separation into two distinct phases, one rich in protein and the other in polysaccharide. The similarity of a Skimmed milk (SMP/LBG and a micellar casein/LBG system suggests also that the incompatibility between skimmed milk and LBG is largely attributable to the incompatibility of micellar casein and LBG. Sucrose appears to have only a modest influence on the thermodynamics of these mixed systems, leading to a concentration effect within the protein phase and a dilution within the locust bean gum phase. Data on the effect of sucrose on the individual components suggests, however, an improved solvent effect for LBG. It has been clearly demonstrated that the microstructure of these systems is driven by the phase volume ratio.

Abstract

For process speed and throughput, high average power is always required in materials processing with lasers. However, in certain applications, such as the drilling of small-diameter or high-aspect-ratio holes, the brightness of the laser, which is proportional to the power and inversely proportional to the square of the beam quality, becomes equally important for the quality of the processed parts. Beyond the capabilities of conventional rod lasers, face pumped zigzag slab lasers provide the answer to the simultaneous requirements of high average power and good beam quality, and they have demonstrated spectacularly improved capabilities for percussion drilling and noncontact machining of aerospace alloys for over 10 years in free-running (gain-switched) mode with pulses of millisecond duration and several tens of kilowatts peak power.

Abstract

The pectinesterases in tomato pericarp were fractionated by cation exchange into four forms (A–D). Form A was the most abundant and C the second most abundant. Forms A–C were further purified by gel filtration, and antibodies were raised against A and C. Comparison of the different forms by dot blots and Western blots showed that although all three forms shared common immunological characteristics, there were also differences in their behaviour, indicative of structural differences. Form A had an N -terminal sequence identical to that published previously for the major pectinesterase in tomato fruit. In contrast, form C had a completely novel N -terminal sequence. Form A was absent from hypocotyls and roots. Forms B and C were present in comparable amounts in hypocotyls, while form C predominated in roots.

SPE Member Abstract There is a clear recognition within Shell Exploration and Production (E&P) Companies that the next phase in improving environmental stewardship is the development and implementation of an Environmental Management System (EMS) within the context of a Health, Safety and Environment Management System (HSE MS). while many of the system's components such as inventories, plans, training and auditing have been implemented in established environmental management programs, there is now a need to integrate and build upon these tools with a formal, structured management system. The appearance of Safety Management Systems (SMS) is seen as a milestone event and the vision of an HSE MS is seen as the objective. The move to management systems in the environmental field recognises the need for accountability, measurement, hazard management, feedback and priority of resource allocation. A practical look is taken at both the challenges and the solutions and how they can be introduced in company operations in many countries. Introduction Shell companies have had a long history of care for the environment through commitment to policy and applied management practices including environmental assessment, inventorisation of waste streams, waste management programs, training and auditing. With the recent emergence of the management system approach, especially in safety, there has been strong impetus to move environment in the same direction to fully capitalise on the collective synergy, and indeed to utilise a similar model. In parallel with Shell E&P a number of organisations and agencies have also been developing and promoting specific management systems for the environment. A review of the systems available has shown commonalities as well as differences in emphasis and approach. The systems were examined with the intention of creating a "fit for purpose" model for Shell E&P operations. This has taken into account the flexibility needed to address companies in various stages of health, safety and environment management evolution. This evolution recognises the needs of the small single string exploration venture, where HSE MS is already being implemented, and the fully integrated E&P company where safety systems are leading and environment and health systems are coming on behind but with the clear opportunity to converge into one overall approach. There is also recognition that the components already in place in individual companies would differ in scope and degree of implementation with the result that companies will wish to emphasise differing aspects of EMS during the early days of implementation. The focus of this paper is EMS and it has been written within the context and components of environmental management. However it fully recognises that it is not a stand alone approach but an aspect of fully integrated HSE management. THE CHALLENGE The challenge to implement the Environmental component of HSE MS in Shell E&P companies is seen as fivefold. P. 771^

Abstract

The response to hydrogen (0.1 – 2.0% in air) of flammable gas sensors consisting of pellistors coated with supported palladium has been investigated. An optimum diffusion-limited response of 15 – 30 mV/(% H2), depending on reactor geometry, was obtained with pellistors operated at ≈200 °C under atmospheric pressure. Both ambient and generated water reduced catalytic activity and response, particularly at low temperature. Activity was also greatly reduced by overheating of the pellistor. The influence of these factors on recommended operating procedures is considered.

Explicit solutions are obtained for the dynamic die loads generated during high-speed plane strain and axisymmetric compression with a free-flight impact forging device in mind. The influences of both axial and radial inertial forces are included and the die loads are evaluated for zero, full sticking, intermediate and mixed frictional boundary conditions. The manner in which inertial effects operate is exemplified for a range of impact velocities and interface frictional conditions. The onset of appreciable inertial effects can be easily assessed by considering the final dimensionless forging loads, i.e. ω F = 1 [1+( ∞ h 3 f )] for plane strain and ω F = 1 [1+( ∞ h 3 f )] for axial symmetry. A comparison is drawn between three types of impact forming devices on the basis of the effect of inertia on the die loads in a “real” forging operation.

α-Galactosidase I from Vicia faba seeds binds to potato starch and sheep erythrocytes. With the aid of fluorescence microscopy and using 4-methylumbelliferyl α- D -galactoside as the substrate it has been demonstrated that the binding is via the lectin sites of the enzyme leaving catalytic sites free and detectable. The lectin site is specific for D -glucose/ D -mannose residues.

Abstract

The basalt stratigraphy and structure of a 175-square kilometer area (the Nine Canyon Map Area) along the southern margin of the Pasco Basin have been studied to help assess the feasibility of a nuclear waste terminal storage facility. Detailed mapping shows that uplift of the Horse Heaven Hills began prior to extrusion of the Priest Rapids Member of the Wanapum Basalt, Columbia River Basalt Group. Both the Pomoma and the Elephant Mountain members (Saddle Mountains Basalt, Columbia River Basalt Group) are wide-spread throughout the basin, but thin considerably along the Horse Heaven Hills in the vicinity of Wallula Gap. The Ice Harbor Member is present only along the northern margin of the map area and possibly occupies a paleo-channel. The Rattlesnake Hills-Wallula Gap Lineament trends north 60 degrees west and intersects the older Horse Heaven Hills anticline in Wallula Gap. Four faults of short length and small vertical displacement are located along this structure. Within the map area, the intensity of folding increases, and the style of faulting changes from normal to reverse with proximity to the Wallula Gap area. No evidence for Quaternary deformation was found.

Abstract

SUMMARY 1) Pulmonary resection has a definite place in the surgical therapy of pulmonary tuberculosis in salvaging otherwise hopeless cases. 2) With the exceptions cited, we prefer thoracoplasty to lobectomy and pneumonectomy in the treatment of pulmonary tuberculosis. This point of view is based on the fact that the mortality from thoracoplasty is considerably lower than pulmonary resection and to date, the late results of resection seem to offer no better chance of permanent cure. 3) When thoracoplasty fails, the following measures are considered before performing pulmonary resection: (1)Revision thoracoplasty. (2)Creation of extrapleural space and packing. (3)Cavity drainage. (4)In some cases, no further surgery. 4) Primary pulmonary resection is preferred to thoracoplasty for: (1)Lower and middle lobe cavities. (2)Tuberculomata and blocked cavities. (3)Persistent bronchostenosis. (4)Symptomatic bronchiectasis. 5) From the point of view of the pathological pulmonary changes, the type of disease most amenable to lobectomy and pneumonectomy is that characterized by fibroid and caesating lesions of the lung in which the mode of spread is principally by way of the bronchus. Since the prognosis following thoracoplasty in this type of lesion is also good, it is unusual that the surgeon is confronted with the problem of primary pulmonary resection. 6) Primarily, hematogenous or lymphogenous disseminated pulmonary tuberculosis is a contra-indication to resection, as these types of disease are progressive and removal of one part of the lung does not alter the course of the lesions in the remaining part of the lung. 7) If thoracoplasty is unsuccessful, then revision thoracoplasty, plombage, and cavity drainage should be considered before lobectomy or pneumonectomy is performed. Pulmonary resection is to be preferred to cavity drainage, if the pulmonary capacity of the patient permits. 8) In selecting any case for lobectomy or pneumonectomy, the surgeon must be certain that the patient has sufficient pulmonary reserve. Contralateral pleural thickening and diaphragmatic paralysis, along with fibrosis, calcification, and emphysema of the lung are the most important conditions in lowering the patient's breathing capacity. 9) In the surgical management of over 1,000 cases of pulmonary tuberculosis, we have performed lobectomy and pneumonectomy in 40 cases with early and late mortality of 27.5 per cent. In approximately an equal number of cases, resection was indicated but not performed for various reasons. This represents a very hopeless group of cases where for the most part, thoracoplasty had failed, and no other treatment would cure. 10) Lobectomy and pneumonectomy are indicated in many patients where thoracoplasty has failed or obviously will not succeed. The actual number of patients upon whom it is possible to perform pulmonary resection is greatly reduced because of these factors: (1) contralateral tuberculosis; (2) instability of the contralateral lesion, and (3) low pulmonary reserve. 11) We are by no means satisfied that the indications we have presented will stand the test of time. Our point of view lies somewhere between the eager advocates for pulmonary resection, whose enthusiasm has not yet been supported by satisfactory long-time results, and those who roundly condemn lobectomy and pneumonectomy for pulmonary tuberculosis. 12) It is becoming steadily more evident that it is not technical skill alone that will lead to a higher percentage of permanent arrests in pulmonary tuberculosis. The basic tendency of the disease in each particular case is variable. Thus, the more able we become in recognizing the type of pulmonary involvement, the more accurately we will place lobectomy and pneumonectomy in their proper relationship with other therapeutic procedures.
