Abstract

As part of its Extremely Brilliant Source (EBS) upgrade project, the ESRF's BM29 BioSAXS beamline was subject to a significant upgrade and refurbishment. In addition to the replacement of the beamline's original bending magnet source by a two-pole wiggler, leading to an increase in brilliance by a factor of 60, the sample environment of the beamline was almost completely refurbished: a vacuum-compatible Pilatus3 X 2M with a sensitive area of 253.7 mm × 288 mm and frame rates up to 250 Hz was installed, increasing the active area available and thus the q-scaling of scattering images taken; the sample changer was replaced with an upgraded version, allowing more space for customizable sample environments and the installation of two new sample exposure units; the software associated with the beamline was also renewed. In addition, the layout and functionality of the BSXCuBE3 (BioSAXS Customized Beamline Environment) data acquisition software was redesigned, providing an intuitive `user first' approach for inexperienced users, while at the same time maintaining more powerful options for experienced users and beamline staff. Additional features of BSXCuBE3 are queuing of samples; either consecutive sample changer and/or SEC-SAXS (size-exclusion chromatography small-angle X-ray scattering) experiments, including column equilibration were also implemented. Automatic data processing and analysis are now managed via Dahu, an online server with upstream data reduction, data scaling and azimuthal integration built around PyFAI (Python Fast Azimuthal Integration), and data analysis performed using the open source FreeSAS. The results of this automated data analysis pipeline are displayed in ISPyB/ExiSAXS. The upgraded BM29 has been in operation since the post-EBS restart in September 2020, and here a full description of its new hardware and software characteristics together with examples of data obtained are provided.

Abstract

D’après un rapport de l’IGAS, la loi de 2009 ayant supprimé l’autorisation préalable pour l’ouverture d’un centre de santé a provoqué un accroissement de leur création de 25 % entre 2011 et 2016. Le nombre de chirurgiens-dentistes travaillant dans ces structures s’accroit lui aussi, encourageant l’offre d’une nouvelle façon d’exercer l’art dentaire. Le travail de cette thèse consiste, en premier lieu, à établir un état des lieux des centres de santé dentaires en 2020, en s’intéressant aux différents types existants et à leur mode de fonctionnement. Ensuite, l’intérêt est porté sur le statut salarié du chirurgien-dentiste exerçant dans ces structures, incluant le focus sur le contrat de travail ainsi que les avantages et les inconvénients que présente ce statut. Dans un second temps, une enquête a été réalisée auprès d’une population cible de chirurgiens-dentistes travaillant en centre de santé afin d’une part d’identifier les caractéristiques et la typologie du chirurgien-dentiste salarié en centre de santé, et d’autre part d’évaluer le devenir des centres de santé dentaires et du statut de salarié du chirurgien-dentiste en France, dans les prochaines années.

3. La thérapeutique étiologique parodontale : ressenti du patient et regard du praticien

Abstract

Les maladies parodontales sont associées à une rupture d’un état symbiotique hôte/bactéries, le facteur étiologique principal est donc bactérien et abouti à un ensemble de modifications inflammatoires et destructions affectant les tissus parodontaux. La gestion de ces maladies passe par un partenariat impliquant le patient et le praticien, dont l’objectif sera de contrôler l’infection de façon durable, rétablir un état symbiotique et secondairement de régénérer les structures parodontales perdues. Les deux maladies parodontales qui vont nous intéresser sont la gingivite d’origine bactérienne dont la prévalence dans le monde varie entre 50 et 100 % et la parodontite dont la prévalence dans le monde est encore à ce jour mal évaluée. Encore aujourd’hui, on sait que la perception et le ressenti du patient et du praticien au cours de la thérapeutique parodontale initiale est méconnue et n’a jamais été le sujet d’étude. Or, comprendre et appréhender ces deux notions conditionneraient la compliance du patient à poursuivre son traitement, et nous aideraient en tant que praticien à améliorer notre accompagnement et suivi thérapeutique. La présente thèse mettra en avant les résultats de deux questionnaires, adressés respectivement aux praticiens et aux patients et aura pour objectif de poursuivre la réflexion sur la méthode de traitement la plus adaptée pour prendre en compte le ressenti du patient tout en améliorant l’ergonomie de travail du praticien.

Abstract

Dans la population actuelle, le sport fait partie du quotidien de nos patients. En fonction du sport pratiqué par nos patients, la prévalence des traumatismes dentaires et tissus environnants est plus ou moins importante. Des recherches montrent que l’utilisation de protections (casque, plastron, épaulière, genouillère, protège -dent, etc…) baisse nettement la prévalence de ces accidents traumatiques. Malgré tout, selon des études épidémiologiques il n’est pas dans les mœurs de les utiliser. Ceci démontre un manque de prévention primaire à ce sujet, pourtant indispensable. Cette thèse cherche à améliorer le système de classification des protège-dents (commerciaux et professionnels). Les caractéristiques d'un protège-dents idéal ont été déterminées avec une revue de la littérature actuelle. Ceux-ci ont servi de base pour réaliser un tableau de classification en pondérant ces différents critères. Ce processus aboutit à une note permettant de classer le dispositif. Malgré les limites du tableau obtenu, de l’impact de cette thèse et de la législation actuelle sur les équipements de protection individuelle, l’intérêt de ce sujet reste d’importance. Cette thèse montre les changements importants à apporter à la classification actuelle pour améliorer la diffusion de l’information.

Abstract

International audience; To achieve the WHO's global strategy of eliminating hepatitis B virus (HBV) infection, it is essential to ensure adequate understanding of hepatitis B disease to facilitate uptake of screening, linkage to care, and adherence to antiviral therapy. Sub-Saharan Africa has the world's highest prevalence of HBV infection, yet less than 1% of HBV-infected people have been diagnosed. Lay populations do not widely recognize "hepatitis B." Hypothesizing that visible signs and symptoms of chronic HBV infection sequelae (jaundice, ascites, or cachexia) might be more easily recognizable, we conducted a qualitative study to determine how rural populations in Senegal understood these sequelae. We led six focus groups and 149 individual interviews with lay populations and formal and informal health workers in three rural study sites in Senegal. Only a third of lay populations (30/105) had ever heard of "hepatitis B," nor did they evoke local language diagnostic terms resembling this illness. Nevertheless, more than two-thirds (71/105) recognized signs and symptoms of end-stage liver disease but considered these physical manifestations to be a consequence of the manipulation of occult forces. Lay populations also contended that traditional healers, not formal medical structures, should treat such illnesses. Formal and informal health workers had limited knowledge about hepatitis B and difficulties identifying terms in local languages to explain the disease. Communication strategies based on the population's widespread recognition of the hepatitis sequelae may enhance awareness of hepatitis, which is a crucial element to increase screening uptake, linkage to care, and commitment to lifelong treatment in African communities.

Abstract

L’objectif de cette thèse est de déterminer les pathologies liées à la profession de chirurgien-dentiste et de s’intéresser à la procédure de reconnaissance en tant que maladies professionnelles, ainsi qu’à la prévoyance complémentaire spécifique à la profession. Cette étude a mené à de nombreux constats, notamment celui que les praticiens sont toujours plus nombreux à souffrir des mêmes pathologies, en particulier de troubles musculo-squelettiques, d’affections psychiques et de troubles du sommeil. Les facteurs sont connus, des solutions sont proposées, notamment des actions de prévention, pourtant les résultats sont mitigés. Nous avons réalisé une enquête auprès des chirurgiens-dentistes de France pour connaître leurs pathologies, évaluer leur comportement et tenter d’approfondir les raisons qui freinent la prise de conscience et la mise en œuvre des solutions exposées au cours de leur formation.

Abstract

La santé orale des personnes en situation de handicap reste une problématique majeure de santé publique en 2019. Les personnes dites à besoins spécifiques rencontrent aujourd’hui encore des difficultés à se faire soigner. Le défi découlant de ce constat est multiple : il s’agit d’une part d’identifier ces patients, d’autre part de mettre en place des solutions pour faciliter et valoriser leur prise en charge. Cette thèse est une étude pilote visant à évaluer une échelle de complexité de prise en charge des patients à besoins spécifiques. L’échelle se veut être un outil à l’usage des chirurgiens-dentistes pour identifier les patients nécessitant une prise en charge spécifique et prenant en compte tous les paramètres de difficulté de celle-ci. Elle est constituée de sept items, chacun se référant à un facteur de difficulté : communication, nécessité de technique facilitatrice, coopération, complexité de l’état de santé, risque de pathologie dentaire, autonomie d’accès aux soins et nécessité d’une coordination administrative. Chaque item est accrédité d’un certain nombre de points selon que la difficulté est mineure, modérée ou majeure. L’addition de ces points permet d’obtenir un score total de complexité. Au cours de l’année 2018, au service d’odontologie de l’hôpital Louis Mourier (Colombes), nous avons réalisé cent questionnaires. L’objectif de ce travail est de présenter cette étude ainsi que les résultats préliminaires obtenus.

Abstract

International audience; Coincident with the tsunami of COVID-19–related publications, there has been a surge of studies using real-world data, including those obtained from the electronic health record (EHR). Unfortunately, several of these high-profile publications were retracted because of concerns regarding the soundness and quality of the studies and the EHR data they purported to analyze. These retractions highlight that although a small community of EHR informatics experts can readily identify strengths and flaws in EHR-derived studies, many medical editorial teams and otherwise sophisticated medical readers lack the framework to fully critically appraise these studies. In addition, conventional statistical analyses cannot overcome the need for an understanding of the opportunities and limitations of EHR-derived studies. We distill here from the broader informatics literature six key considerations that are crucial for appraising studies utilizing EHR data: data completeness, data collection and handling (eg, transformation), data type (ie, codified, textual), robustness of methods against EHR variability (within and across institutions, countries, and time), transparency of data and analytic code, and the multidisciplinary approach. These considerations will inform researchers, clinicians, and other stakeholders as to the recommended best practices in reviewing manuscripts, grants, and other outputs from EHR-data derived studies, and thereby promote and foster rigor, quality, and reliability of this rapidly growing field.

Introduction : les violences faites aux femmes sont considérées comme un problème majeur de santé publique. Les médecins généralistes méconnaissent la plupart des violences que subissent leurs patientes. La pratique d’un repérage systématique est de plus en plus envisagée afin de mieux identifier ces situations. Au même titre que leurs confrères civils, les médecins militaires semblent être peu confrontés à ce phénomène, alors que les femmes militaires françaises peuvent aussi y avoir été exposées. Ce travail a pour objectif de dresser un état des lieux des connaissances, expériences professionnelles et opinions des praticiens des armées concernant cette thématique. Méthodes : une étude transversale par questionnaire a été menée du 14 mai au 18 juin 2018, auprès des médecins d’unité des Centres Médicaux des Armées de Paris et de Versailles et de deux promotions d’Internes des Hôpitaux des Armées de médecine générale et d’urgence. Au total, sur 226 personnes sollicitées, 59 ont été inclues. Résultats : 81,4% des participants ne connaissent pas la pratique du repérage systématique des violences faites aux femmes. Au cours de la dernière année, ils sont 40,7% à avoir été confrontés de manière très occasionnelle à cette problématique. Une très grande majorité des répondants estiment qu’un médecin d’unité pourrait avoir un rôle central dans le repérage et la prise en charge des femmes militaires victimes de violences, sous couvert d’une formation. Conclusion : ce travail souligne l’intérêt de réfléchir au développement de formations spécifiques visant à sensibiliser les médecins d’unité au repérage et à l’orientation des femmes militaires victimes de violences.

Abstract

International audience; The concept of health-related quality of life (HRQoL) considers patient's perspective as an essential component of the health care relationship. HRQoL is often assimilated to a set of existing tools aimed at measuring the perspective of patient, more particularly the consequences of ill-health condition on patient's everyday life. Patients' reports of their health is however diverse, as social factors such as age, gender, professional status are likely to impact on health self-perception and reporting. Social aspects of HRQoL are somehow underexplored. This paper presents a secondary analysis of epidemiological HRQoL data issued from two national surveys (Baromètre Santé 2005; Enquête Décennale Santé 2002-2003). The data analysis showed the existence of social determinants of HRQoL. It more specifically demonstrated that four social indicators are determinants of HRQoL namely living in couple, level of education, occupational status and net income per household, independently of age and gender known effect. Social mechanisms governing the impact of social determinants on quality of life could be further explored by adopting a multidisciplinary and mutilevel research approach of HRQoL as well as considering the ability of individuals to engage with social aspect of their health conditions.

Abstract

Objective To review current practice and update guidelines for the methodology of shortening composite measurement scales (CMSs). Study Design and Setting A literature review gathered data on 91 shortening processes from 1995 to 2009. The validity of the initial CMS, the shortening methods, and the validity of the derived short-form scales were examined. The results were compared with those from a previous literature review (articles from 1985 to 1995) to develop updated guidelines for CMS shortening. Results The literature review revealed a persisting lack of use of rigorous methodology for CMS shortening. Of the 91 cases of CMS shortening, 36 combined a content approach and a statistical approach; 45 used only a statistical approach and 10 (11%) only a content approach. The updated guidelines deal with the validity and conceptual model of the initial CMS, the preservation of content and psychometric properties during shortening, the selection of items, and the validation of the short form. Conclusion Item reduction based on a rigorous methodology is necessary if the short-form instrument aims to maintain the validity and other measurement properties of the parent instrument, which in turn supports application in research and clinical practice.

Abstract

International audience; The Gambia National Cancer Registry (GNCR) is one of the few nationwide population-based cancer registries in sub-Saharan Africa. Most registries in sub-Saharan Africa are limited to cities; therefore, the GNCR is important in providing estimates of cancer incidence in rural Africa. Our study assesses the quality of its data. The methods proposed by Bray and Parkin, and Parkin and Bray (Eur J Cancer 2009;45:747-64) were applied to the registry data from 1990 to 2009 to assess comparability, validity and completeness. The system used for classification and coding of neoplasms followed international standards. The percentage of cases morphologically verified was 18.1% for men and 33.1% for women, and that of death certificate only cases was 6.6 and 3.6%, respectively. Incidence rates in rural regions were lower than in the urban part of the country, except amongst young male adults. Comparison with other West African registries showed that the incidences of liver and uterine cervical cancer were comparable, but those of prostate and breast in The Gambia were relatively low. The overall completeness was estimated at 50.3% using the capture-recapture method. The GNCR applies international standard practices to data collection and handling, providing valuable data on cancer incidence in sub-Saharan Africa. However, the data are incomplete in the rural and elderly populations probably because of health care access and use.

Abstract

The majority of healthy individuals exposed to Mycobacterium tuberculosis will not develop tuberculosis (TB), though many may become latently infected. More precise measurement of the human immune response to M. tuberculosis infection may help us understand this difference and potentially identify those subjects most at risk of developing active disease. Gamma interferon (IFN-γ) production has been widely used as a proxy marker to study infection and to examine the human immune response to specific M. tuberculosis antigens. It has been suggested that genetically distinct M. tuberculosis strains may invoke different immune responses, although how these differences influence the immune responses and clinical outcome in human tuberculosis is still poorly understood. We therefore evaluated the antigen-specific IFN-γ production responses in peripheral blood mononuclear cells from two cohorts of subjects recruited in Antananarivo, Madagascar, from 2004 to 2006 and examined the influence of the infecting M. tuberculosis strains on this response. The cohorts were sputum-positive index cases and their household contacts. Clinical strains isolated from the TB patients were typed by spoligotyping. Comparison of the IFN-γ responses with the spoligotype of the infecting clinical strains showed that “modern” M. tuberculosis strains, like Beijing and Central Asian (CAS) strains, tended to induce lower IFN-γ responses than “ancient” strains, like East African-Indian (EAI) strains, in index cases and their household contacts. These results suggest that new strains may have evolved to induce a host response different from that of ancient strains. These findings could have important implications in the development of therapeutic and diagnostic strategies.

Abstract

Objective To survey rheumatologists' preferences for the choice of a second-line disease-modifying antirheumatic drug (DMARD) after inadequate response with methotrexate (MTX) therapy in rheumatoid arthritis (RA). Methods Thirty-six rheumatologists stated their preferences for RA treatment after inadequate response with MTX therapy (optimal dose at least 6 months). From the initial scenario, we derived 54 vignettes varying by rheumatoid factor or anti–cyclic citrullinated peptide antibody presence, swollen joint count, Disease Activity Score in 28 joints, and structural damage. Respondents stated their preference among 5 therapeutic options: MTX continuation, switch to another conventional DMARD, addition of another conventional DMARD, addition of anakinra, or addition of a tumor necrosis factor (TNF) blocker. Presentation by pairs yielded 10 combinations of strategies for each variant, totaling 540 vignettes; participants evaluated a random sample of 180 vignettes. Determinants of each top-ranked option were analyzed by logistic regression. The compilation of these data served to define a therapeutic algorithm. Results The responses of 33 rheumatologists were analyzable. Therapeutic preferences corresponded to the top-ranked options. For patients with mild or moderately active RA, either a switch or step-up strategy to another conventional DMARD was top ranked. TNF blockers were preferred for RA patients with high disease activity or progressive structural damage. On the basis of these preferences, we developed a simple decision tree for use in daily clinical practice. Conclusion Our simple, easy-to-use decision tree developed from rheumatologists' preferences for therapy after failure of MTX therapy in RA treatment may guide rheumatologists in daily practice to choose a second-line DMARD.

Abstract

International audience; BACKGROUND: Because no effective curative approaches are available, preventive approaches in the field of Alzheimer's disease (AD) are needed. We present the design of the ongoing Multidomain Alzheimer Preventive Trial (MAPT) Study. Several previous studies suggested that many factors may be involved in the occurrence of AD at late ages. Because of the probable multifactorial nature of AD, it seems logical to initiate multidomain interventions to examine their potential synergistic effects. The MAPT Study aims to evaluate the efficacy of a multidomain intervention (nutritional, physical, and cognitive training) and omega 3 treatment in the prevention of cognitive decline in frail elderly persons aged 70 years or over. The study also collects imaging and biological data that could be used in future AD prevention and treatment trials. METHODS: The MAPT Study is a 3-year, randomized, controlled trial conducted by university hospital practitioners specializing in memory disorders in four French cities (Bordeaux, Limoges, Montpellier, and Toulouse). The study plans to enroll 1200 frail elderly subjects on the basis of at least one of the following criteria: subjective memory complaint spontaneously expressed to a general practitioner, limitation in one instrumental activity of daily living (IADL), and slow walking speed. To demonstrate the protective effect of interventions, subjects are randomized into one of the following four groups: omega 3 alone, multidomain intervention alone, omega 3 plus multidomain intervention, or placebo (n = 300 each). The principal outcome measure is a change in cognitive function at 3 years, as determined by the Grober and Buschke Test. CONCLUSIONS: The MAPT Study is the first preventive trial involving multidomain interventions. Final results should be available in 2013.

Abstract

Background The Global Activity Limitation Indicator (GALI), the measure underlying the European indicator Healthy Life Years (HLY), is widely used to compare population health across countries. However, the comparability of the item has been questioned. This study aims to further validate the GALI in the adult European population. Methods Data from the European Health Interview Survey (EHIS), covering 14 European countries and 152,787 individuals, were used to explore how the GALI was associated with other measures of disability and whether the GALI was consistent or reflected different disability situations in different countries. Abstract

International audience; Despite the potential for patient-reported outcome measures (PROMs) and experience measures (PREMs) to enhance understanding of patient experiences and outcomes they have not, to date, been widely incorporated into renal registry datasets. This report summarizes the main points learned from an ERA-EDTA QUEST-funded consensus meeting on how to routinely collect PROMs and PREMs in renal registries in Europe. In preparation for the meeting, we surveyed all European renal registries to establish current or planned efforts to collect PROMs/PREMs. A systematic review of the literature was performed. Publications reporting barriers and/or facilitators to PROMs/PREMs collection by registries were identified and a narrative synthesis undertaken. A group of renal registry representatives, PROMs/PREMs experts and patient representatives then met to (i) share any experience renal registries in Europe have in this area; (ii) establish how patient-reported data might be collected by understanding how registries currently collect routine data and how patient-reported data is collected in other settings; (iii) harmonize the future collection of patient-reported data by renal registries in Europe by agreeing upon preferred instruments and (iv) to identify the barriers to routine collection of patient-reported data in renal registries in Europe. In total, 23 of the 45 European renal registries responded to the survey. Two reported experience in collecting PROMs and three stated that they were actively exploring ways to do so. The systematic review identified 157 potentially relevant articles of which 9 met the inclusion criteria and were analysed for barriers and facilitators to routine PROM/PREM collection. Thirteen themes were identified and mapped to a three-stage framework around establishing the need, setting up and maintaining the routine collection of PROMs/PREMs. At the consensus meeting some PROMs instruments were agreed for routine renal registry collection (the generic SF-12, the disease-specific KDQOL™-36 and EQ-5D-5L to be able to derive quality-adjusted life years), but further work was felt to be needed before recommending PREMs. Routinely collecting PROMs and PREMs in renal registries is important if we are to better understand what matters to patients but it is likely to be challenging; close international collaboration will be beneficial.

Abstract

Background This systematic literature review aimed to summarize evidence for the added value of drug sales data analysis for the surveillance of infectious diseases. Methods A search for relevant publications was conducted in Pubmed, Embase, Scopus, Cochrane Library, African Index Medicus and Lilacs databases. Retrieved studies were evaluated in terms of objectives, diseases studied, data sources, methodologies and performance for real-time surveillance. Most studies compared drug sales data to reference surveillance data using correlation measurements or indicators of outbreak detection performance (sensitivity, specificity, timeliness of the detection). Results We screened 3266 articles and included 27 in the review. Most studies focused on acute respiratory and gastroenteritis infections. Nineteen studies retrospectively compared drug sales data to reference clinical data, and significant correlations were observed in 17 of them. Four studies found that over-the-counter drug sales preceded clinical data in terms of incidence increase. Five studies developed and evaluated statistical algorithms for selecting drug groups to monitor specific diseases. Another three studies developed models to predict incidence increase from drug sales. Conclusions Drug sales data analyses appear to be a useful tool for surveillance of gastrointestinal and respiratory disease, and OTC drugs have the potential for early outbreak detection. Their utility remains to be investigated for other diseases, in particular those poorly surveyed. Companion of the pharmacy since the introduction of the system SESAM-vitale in 1998, management softwares (aka Logiciels de gestion d'Officine) of pharmacy are becoming over the years, a key element of the French informatics tool. This work proposes an exhaustive list of softwares commercialized in France in 2014 and a list of their functionalities classified by order needs: obligatory, essential and optional functions and services given by editors. After that, this work deals with a development of the different doers and institutions that interact with those softwares, especially by describing organization where the pharmacists had an active participation. A brief table comparing editors is proposed. A generalist explanation of free software is given because they are used inside some of this professionalssoftwares. Then, analyses of teletransmission data are undertaken, before a series of tables ranking the various LGO softwares of the market by the functionalities, the operating system, the software of database used. Finally, a discussion concludes this work, Compagnon de l'Officine depuis la mise en place du système SESAM-Vitale en 1998, les logiciels de gestion d'officine (LGO) sont devenus au fil des années, un élément clé de l'outil informatique du pharmacien officinal. Abstract

Introduction: In France, one in three pregnancies is unintended. Among these pregnancies, two‐thirds occur despite the use of contraception. The intrauterine device (IUD) is a safe, effective and well-tolerated mean of contraception. This work aimed to identify the reticence and knowledge of women about the use of an IUD. Method: Descriptive survey from self-administered questionnaires completed by 150 patients younger than 45 years consulting at a Parisian university hospital and at the family planning center of the small city of Provins (population 12,000). Results: The main concerns voiced by order of frequency were: fear of the laying (39%), fear of feeling the IUD(36%), fear that the body does not accept the IUD as a foreign body (35%), fear of discomfort during sexual intercourse (29%). Most patients knew that the IUD is safe and effective. 65% of patients knew it could be used in nulliparous patients. 96% of patients said they had ever been informed about the IUD. 48% of patients were favorable to the IUD. 16 patients (14%) were using the IUD. The factors that influenced the rate of IUD use were the number of children, age, education level and ethnicity. Conclusion: The IUD is well known by women, but it remains reticence to which any doctor should be able to respond to improve contraceptive efficacy in order to prevent the occurrence of unintended pregnancies.; Introduction : En France, une grossesse sur trois est non désirée. Parmi ces grossesses, les deux tiers surviennent malgré l'utilisation d'un moyen de contraception. Le dispositif intra-utérin (DIU) est un moyen de contraception sûr, efficace et bien toléré. Ce travail avait pour objectif d'identifier les réticences et les connaissances des patientes sur le DIU. Méthode : Enquête descriptive réalisée à partir d'auto-questionnaires remplis par 150 patientes âgées de moins de 45 ans consultant un centre hospitalo-universitaire parisien et le centre de planification et d’éducation familiale de Provins (ville moyenne de 12000 habitants). Résultats : Les principales réticences exprimées étaient par ordre de fréquence: la peur de la pose (39%), la peur de le sentir (36%), la peur que le corps ne l'accepte pas comme un corps étranger (35%), la peur d'une gêne lors des rapports sexuels (29%). Les patientes savaient dans leur quasi‐totalité que le DIU est sûr et efficace. 65% des patientes savaient qu'il peut être utilisé chez les patientes nullipares. 96% des patientes disaient avoir déjà été informées sur le DIU. 48% des patientes étaient favorable au DIU. 16 patientes (14%) utilisaient le DIU. Les facteurs qui influençaient le taux d'utilisation du DIU étaient le nombre d'enfant, l'âge, le niveau d'étude et l'origine ethnique. Conclusion : Le DIU est bien connu des patientes, mais fait l'objet de réticences auxquelles tout médecin devrait pouvoir répondre afin d'améliorer l'efficacité contraceptive pour éviter la survenue de grossesses non désirées.

Abstract

International audience; The goal of this study was to evaluate the interest of the members of the French society of young radiation oncologists (SFjRO) for brachytherapy as well as their theoretical and practical level in this radiation technique. An anonymous survey was conducted regarding practical and theoretical teaching of brachytherapy before the French national courses on brachytherapy. Among the 106 residents attending this teaching course, 99 (93%) answered the survey. Most of them were interested in brachytherapy but 82% considered they had not received sufficient teaching. Relevant indications of brachytherapy were known by 76% of the residents for gynaecological malignancies and 70% for prostate. Seventy-one percent of the residents have seen at least one gynecological brachytherapy but only 12% knew how to deal with this technique. Fifty-six percent have seen vaginal high dose rate brachytherapy and 21% had acquired the technique. For prostate brachytherapy, 65% had seen and done an implant and only 4% had acquired the technique. Fifty percent have performed at least one brachytherapy treatment during their residency. Residents expressed a strong wish for more courses about dosimetry (82%), technique (75%) and treatment planning (90%). Our study shows the interest of French residents for brachytherapy but suggests that practical teaching courses and an evaluation of the existing theoretical courses are warranted.

Abstract

International audience; Preliminary results of an empirical study of human experimentation practices are presented and contrasted with those of a survey conducted a hundred years ago when clinical research, although tolerated, was culturally deviant. Now that biomedical research is both authorized and controlled, its actors (sponsors, committees, investigators, subjects) come out with heterogeneous rationalities, and they appear to be engaged in a transactional process of negotiating their rationales with one another. In the European context "protective" of subjects, surprisingly the subjects we interviewed (and especially patient-subjects) were creative and revealed an aptitude for integrating experimental medicine into common culture.

Abstract

International audience; In BOLD fMRI a detailed analysis of the MRI signal time course sometimes shows time differences between different activated regions. Some researchers have suggested that these latencies could be used to infer the temporal order of activation of these cortical regions. Several effects must be considered, however, before interpreting these latencies. The effect of a slice-dependent time shift (SDTS) with multi-slice acquisitions, for instance, may be important for regions located on different slices. After correction for this SDTS effect the time dispersion between activated regions is significantly decreased and the correlation between the MRI signal time course and the stimulation paradigm is improved. Another effect to consider is the latency which may exist between perception and stimulus presentation. It is shown that the control of perception can be achieved using a finger-spanning technique during the fMRI acquisition. The use of this perception profile rather than an arbitrary waveform derived from the paradigm proves to be a powerful alternative to fMRI data processing, especially with chemical senses studies, when return to baseline is not always correlated to stimulus suppression. This approach should also be relevant to other kinds of stimulation tasks, as a realistic way of monitoring the actual task performance, which may depend on attention, adaptation, fatigue or even variability of stimulus presentation.

Abstract

International audience; Methods for analyzing and displaying EEG signals are discussed. The increasing availability and affordability of powerful computer equipment makes possible the use of ever more sophisticated signal processing techniques, which extract relevant (but not readily discernible) information from long-term EEG recordings and can easily identify important features in the EEG. Whether these techniques are actually taken up in clinical practice is heavily dependent on how well they match clinical requirements. This article concentrates on requirements set in the context of long-term recordings in the ICU that demand the ability to process short-term discrete events as well as long-term trend information. A huge range of potentially useful signal processing techniques exists. This article illustrates the value of some of these techniques for ICU signals using the EEG recordings collected during the IMPROVE project

Abstract

International audience; OBJECTIVE:To assess the prevalence of systemic sclerosis (SSc) in the Lorraine region, France.METHODS:Data from three sources - general practitioners and community and hospital specialists, medical records departments, and regional and national laboratories-and a capture-recapture method with log-linear models were used to estimate SSc prevalence in the region. Double recording was checked, and reported cases were validated after a review of medical records.RESULTS:We identified 560 records of suspected SSc cases corresponding to 327 unique suspected SSc cases existing on June 30, 2006, in Lorraine. On the basis of the 193 validated cases (22 [11.4%] with diffuse disease, 136 [70.5%] with limited disease, 31 [16.1%] with limited involvement and 4 unknown), the observed overall crude prevalence of SSc was 105.4 cases per million adult inhabitants (95% confidence interval [CI]: 91.0; 121.4). With the capture-recapture method, the estimated number of SSc cases was 233 (95% CI: 217.3; 260.0), so an estimated 40 cases were not identified by the three sources. The estimated overall prevalence was 132.2 cases per million adult inhabitants (95% CI: 115.8; 154.0).CONCLUSIONS:Our study provides the first estimate of SSc prevalence in the Lorraine region. The capture-recapture method allowed us to estimate an additional 21% of unobserved cases and is a good alternative to the community-based study design for estimating the prevalence of rare diseases.

Abstract

International audience; BACKGROUND: Data on the prognosis of non-infectious mixed cryoglobulinaemia vasculitis (CryoVas) in the era of hepatitis C virus screening are lacking. METHODS: The French multicentre and retrospective CryoVas survey included 242 patients with non-infectious mixed CryoVas. Causes of death and prognostic factors of survival were assessed and a prognostic score was determined to predict survival at 5 years. RESULTS: After a median follow-up of 35 months, 42 patients (17%) died. Causes of death were mainly serious infections (50%) and vasculitis flare (19%). One-, 2-, 5- and 10-year overall survival rates were 91%, 89%, 79% and 65%, respectively. A prognostic score, the CryoVas score (CVS), for the prediction of survival at 5 years was devised. Pulmonary and gastrointestinal involvement, glomerular filtration rate 65 years were independently associated with death. At 5 years the death rates were 2.6%, 13.1%, 29.6% and 38.5% for a CVS of 0, 1, 2 and ≥3, respectively. At 1 year the death rates were 0%, 3.2%, 18.5% and 30.8% for a CVS of 0, 1, 2 and ≥3, respectively. The CVS was strongly correlated with the Five Factor Score (FFS) 2009, another prognostic score validated in primary necrotising vasculitis (r=0.82; p65 years, pulmonary and gastrointestinal involvement and renal failure. A score including these variables is significantly associated with the prognosis.

Abstract

CONTEXTE : En France, la prévalence de l’obésité est passée de 5 % en 1981 à 10 % en 2003 avec une augmentation plus rapide chez les agriculteurs et ouvriers que dans les catégories socio-professionnelles supérieures, aboutissant à des inégalités sociales de santé. Les campagnes d’éducation nutritionnelle n’ont cependant pas permis d’enrayer cette progression. Les trois-quarts des généralistes considèrent que c’est leur rôle de mettre en place une prise en charge préventive sur le plan nutritionnel mais rapportent de nombreuses difficultés à le faire.OBJECTIF : Nous voulons comprendre comment les médecins identifient ces difficultés, s’ils en rapportent, et leurs façons de faire, en pratique quotidienne, pour mettre en place un suivi nutritionnel.MÉTHODE : Nous profitons d’une enquête qualitative plus générale sur les pratiquespréventives d'une centaine de médecins généralistes franciliens, pour analyser 21entretiens de médecins portant sur la nutrition.RÉSULTATS : Des difficultés peuvent survenir à tous les niveaux de la prise en charge, à débuter par ce qui se passe en amont : histoire personnelle et trajectoire professionnel du médecin, héritage socio-culturel et rapport de chaque patient avec son alimentation et son poids, contraintes matérielles imposées par le système de soins primaires en France (consultations rémunérées à l’acte). Il s’avère que les médecins, même lorsqu’ils sont spécialisés dans ce domaine (de façon formelle ou pas) identifient presque tous la prévention nutritionnelle comme un soin difficile à mettre en place mais le sentiment d’efficacité découle de l’attitude du praticien vis-à-vis des échecs probables et donc de sa conception de l’éducation thérapeutique (accompagnement vs. combat, dialogue vs. paternalisme).

Abstract

International audience; Census and disposal data provide a multipurpose source of information on cattle mortality. The retrospective analyses we conducted on the data gathered in the National Cattle Register produced relevant information for describing and modelling the cattle mortality baseline and evaluating the impact of the 2007-2008 Blue Tongue epidemic on the French cattle population. This work was conducted retrospectively but showed that monitoring cattle mortality near real time could help detecting unexpected events. We are thus currently working on a timely and automated system to monitor cadaver disposal requests received by rendering plants, thanks to a data interchange system recently implemented between the Ministry of Agriculture and the fallen stock companies. Besides technical and methodological challenges, using these data for surveillance purposes raises epidemiological questions that still need to be answered. The question remains notably as to whether an abnormal increased mortality is a sensitive and timely signal for detecting unexpected health events. It appears also very challenging to identify the most adequate surveillance scale (time, space and population) and the most adequate anomaly detection algorithms to apply when the characteristics of the signals to be detected (shape, amplitude, etc.) are not known a priori. In Human health, similar systems have not yet proven their ability to detect unexpected events earlier than classical surveillance systems currently in place, but they have already demonstrated their value for real time assessment of identified and potentially dangerous events. Combined with traditional surveillance systems, we think that monitoring routinely collected data could improve the surveillance of the animal population health. Even if not used for detection purposes, cattle mortality monitoring could be used to rapidly produce information on the impact and evolution of identified events, what would facilitate decision-making regarding management measures and improve the communication.

Abstract

International audience; In protein crystallography experiments, only two critical steps remain manual: the transfer of crystals from their original crystallization drop into the cryoprotection solution followed by flash-cooling. These steps are risky and tedious, requiring a high degree of manual dexterity. These limiting steps are a real bottleneck to high-throughput crystallography and limit the remote use of protein crystallography core facilities. To eliminate this limit, the Robotic Equipment for Automated Crystal Harvesting (REACH) was developed. This robotized system, equipped with a two-finger micro-gripping device, allows crystal harvesting, cryoprotection and flash-cooling. Using this setup, harvesting experiments were performed on several crystals, followed by direct data collection using the same robot arm as a goniometer. Analysis of the diffraction data demonstrates that REACH is highly reliable and efficient and does not alter crystallographic data. This new instrument fills the gap in the high-throughput crystallographic pipeline.

Abstract

Data on the clinical spectrum and therapeutic management of noninfectious mixed cryoglobulinemia vasculitis (CryoVas) in the era of hepatitis C virus screening are lacking. We analyzed data from 242 patients with noninfectious mixed CryoVas included in the French multicenter CryoVas survey. Baseline manifestations were purpura (75%), peripheral neuropathy (52%), arthralgia or arthritis (44%), glomerulonephritis (35%), cutaneous ulcers (16%), and cutaneous necrosis (14%). A connective tissue disease was diagnosed in 30% and B-cell non-Hodgkin lymphoma in 22%, whereas the CryoVas was considered to be essential in 48%. With the use of Cox-marginal structural models, rituximab plus corticosteroids showed the greater therapeutic efficacy compared with corticosteroids alone and alkylating agents plus corticosteroids to achieve complete clinical, renal, and immunologic responses and a prednisone dosage < 10 mg/d at 6 months. However, this regimen was also associated with severe infections, particularly when high doses of corticosteroids were used, whereas death rates did not differ between the therapeutic regimens. The role of each of these strategies remains to be defined in well-designed randomized controlled trials.

Abstract

International audience; OBJECTIVE: To study trends in the main indicators of health, medical practices and risk factors in France. POPULATION AND METHOD: We collected data from samples of all births in France during one week in 1995 (n=13318), 1998 (n=13718), 2003 (n=14737) and 2010 (n=14903) and have compared them. RESULTS: Between 1995 and 2010, maternal age and body mass index increased steadily, but tobacco use decreased. In 2010, 39.4% of pregnant women had a visit with a midwife in a maternity unit, versus 26.6% in 2003. Deliveries occurred in large public hospitals more and more frequently. The increase in caesarean sections was no longer significant between 2003 and 2010. In general, medical decisions during pregnancy and delivery were closer to professional recommendations in 2010 than in earlier years. Live births before 37 weeks increased steadily from 5.4% in 1995 to 6.6% in 2010, but the proportion of birth weights below 2500g or the 10th percentile stopped increasing after 2003. CONCLUSION: Routine national perinatal surveys highlight major trends in maternal characteristics, obstetric practices, organisation of services, and perinatal health.

Abstract

International audience; PURPOSE: Estimate the prevalence of epilepsy in the 15 years and older in Benin. METHODS: We used a random multistage sampling design to select a representative sample of the 15 years and older in Benin. From March to May 2010, people were screened door-to-door in the twelve regions of Benin. Screening and data collection were performed using a validated standardised questionnaire of epilepsy in tropical regions. A neurologist examined all people suspected of epilepsy. RESULTS: We identified 174 suspected epilepsy cases from 13,046 screened people; 105 were confirmed by the neurologist (54 men and 51 women). The mean age of PWE was 28.9±14.3 years. The estimate of crude prevalence of epilepsy in the 15 years and older in Benin was 8.05/1000 (95% CI: 6.52-9.58/1000). The crude prevalence of epilepsy among men was 9.77/1000 (95% CI 7.35-12.73/1000) and 6.79/1000 (95% CI 5.06-8.91/1000) for women. The age-adjusted prevalence of epilepsy on sub-Saharan Africa population was 8.25/1000 and 7.33/1000 on world population. Substantial heterogeneity was noted, with differences from one region to another. The most common seizure types were generalised tonic-clonic (80.0%), partial secondary generalised seizures (14.3%) and partial seizures (5.7%). SIGNIFICANCE: This nationwide study is the first in West Africa. It provides a low prevalence of epilepsy in Benin compared to previous studies performed in this country and in neighbouring countries. Restricted-area studies are often motivated by the presence of specific risk factors and could overestimate the prevalence, while large-scale studies could underestimate other subtle forms of epilepsy.

Abstract

International audience; BACKGROUND: There is presently a lack of information on the role of healthcare in suicidal ideation in adults. AIMS: To assess the frequencies, patterns, and factors associated with the communication of suicidal ideation toward a health professional. METHODS: Participants stem from a French cross-sectional survey of 22,133 randomly selected adults. Lifetime suicidal behaviors and 12-month mental disorder patterns were assessed using the short form of the Composite International Diagnostic Interview. Participants with suicidal ideation were asked whether they had talked about it and, if they had, to whom. RESULTS: Around 20% of people with suicidal ideation had talked about this distress to a health professional. It was more frequent for people with more severe suicidal behaviors (plan or a prior attempt), among women, those aged 30 or more, those suffering from major depressive episode, panic disorder, or drug use disorder. Above all, it was more frequent among those who had also talked to friends or relatives. CONCLUSIONS: Prevention strategies that encourage suicidal persons to seek help for their distress, whoever that is, may be the more important strategies to develop.

Abstract

International audience; OBJECTIVE: To study trends in the main indicators of health, medical practices and risk factors in France. POPULATION AND METHOD: We collected data from samples of all births in France during one week in 1995 (n=13318), 1998 (n=13718), 2003 (n=14737) and 2010 (n=14903) and have compared them. RESULTS: Between 1995 and 2010, maternal age and body mass index increased steadily, but tobacco use decreased. In 2010, 39.4% of pregnant women had a visit with a midwife in a maternity unit, versus 26.6% in 2003. Deliveries occurred in large public hospitals more and more frequently. The increase in caesarean sections was no longer significant between 2003 and 2010. In general, medical decisions during pregnancy and delivery were closer to professional recommendations in 2010 than in earlier years. Live births before 37 weeks increased steadily from 5.4% in 1995 to 6.6% in 2010, but the proportion of birth weights below 2500g or the 10th percentile stopped increasing after 2003. CONCLUSION: Routine national perinatal surveys highlight major trends in maternal characteristics, obstetric practices, organisation of services, and perinatal health.

Abstract

International audience; PROBLEM:The revision of the International Health Regulations (IHR) and the threat of influenza pandemics and other disease outbreaks with a major impact on developing countries have prompted bolstered surveillance capacity, particularly in low-resource settings.APPROACH:Surveillance tools with well-timed, validated data are necessary to strengthen disease surveillance. In 2007 Madagascar implemented a sentinel surveillance system for influenza-like illness (ILI) based on data collected from sentinel general practitioners.SETTING:Before 2007, Madagascar's disease surveillance was based on the passive collection and reporting of data aggregated weekly or monthly. The system did not allow for the early identification of outbreaks or unexpected increases in disease incidence.RELEVANT CHANGES:An innovative case reporting system based on the use of cell phones was launched in March 2007. Encrypted short message service, which costs less than 2 United States dollars per month per health centre, is now being used by sentinel general practitioners for the daily reporting of cases of fever and ILI seen in their practices. To validate the daily data, practitioners also report epidemiological and clinical data (e.g. new febrile patient's sex, age, visit date, symptoms) weekly to the epidemiologists on the research team using special patient forms.LESSONS LEARNT:Madagascar's sentinel ILI surveillance system represents the country's first nationwide "real-time" surveillance system. It has proved the feasibility of improving disease surveillance capacity through innovative systems despite resource constraints. This type of syndromic surveillance can detect unexpected increases in the incidence of ILI and other syndromic illnesses.

Abstract

International audience; PURPOSE: The need for comparable epidemiologic data on epilepsy from various locations in tropical areas has led in 1994 to the creation of a questionnaire able to standardize information. The Limoges' questionnaire was created to collect information independently of the objectives of each survey performed, and since it has been employed in various continents under tropics latitude. In Africa between 1994 and 2004, 13 epidemiologic surveys in 12 countries were performed by this means. Authors of these works were solicited to communicate their raw data on people with epilepsy (PWE). METHODS: Information collected was aggregated in a database upon which operations of data management were processed. Undernutrition status was determined using an anthropologic method, according World Health Organization (WHO) recommendations. Factors associated with undernutrition and absence of treatment by phenobarbitone were searched for by using multivariate logistic regression. KEY FINDINGS: Information about 2,269 PWE was collected. Mean treatment gap and undernutrition were determined, respectively, to be 30.6% [95% confidence interval (95% CI) 28.7-32.6] and 25.4% (95% CI 22.7-28.2). Factors significantly associated with undernutrition and not being treated with phenobarbitone were determined. SIGNIFICANCE: Despite the different purposes of each study, we were able to pool information in order to characterize and study particular traits of PWE in Africa. Some items of particular importance should be collected systematically and will be highlighted in a newer version of this questionnaire. Because many surveys were undertaken using this tool in tropical areas, a backward compatibility should be ensured.

International audience; The purpose of this paper is to highlight the difficulties of applying neuroepidemiological methods in low income countries or developing countries, which are mostly tropical countries, taking advantage of the experience of the Institute of Neuroepidemiology and Tropical Neurology, which was created in Limoges in 1982. These difficulties could be related to several aspects: methodological, logistical, political or economical, linked to ethical issues, even difficulties to publish the studies. However, concept and neuroepidemiological methods should stay the same worldwide, even if their translation into practice could sometimes raise some problems in developing countries. Study protocol should be more detailed. Some specific epidemiological methods could be useful. Collection of data should be standardized. True cooperation at every level is needed for these researches to be valid.

Abstract

Cytotoxic CD8+ T cells (CTLs) play a critical role in controlling viral infections. HIV-infected individuals develop CTL responses against epitopes derived from viral proteins, but also against cryptic epitopes encoded by viral alternative reading frames (ARF). We studied here the mechanisms of HIV-1 escape from CTLs targeting one such cryptic epitope, Q9VF, encoded by an HIVgag ARF and presented by HLA-B*07. Using PBMCs of HIV-infected patients, we first cloned and sequenced proviral DNA encoding for Q9VF. We identified several polymorphisms with a minority of proviruses encoding at position 5 an aspartic acid (Q9VF/5D) and a majority encoding an asparagine (Q9VF/5N). We compared the prevalence of each variant in PBMCs of HLA-B*07+ and HLA-B*07- patients. Proviruses encoding Q9VF/5D were significantly less represented in HLA-B*07+ than in HLA-B*07- patients, suggesting that Q9FV/5D encoding viruses might be under selective pressure in HLA-B*07+ individuals. We thus analyzed ex vivo CTL responses directed against Q9VF/5D and Q9VF/5N. Around 16% of HLA-B*07+ patients exhibited CTL responses targeting Q9VF epitopes. The frequency and the magnitude of CTL responses induced with Q9VF/5D or Q9VF/5N peptides were almost equal indicating a possible cross-reactivity of the same CTLs on the two peptides. We then dissected the cellular mechanisms involved in the presentation of Q9VF variants. As expected, cells infected with HIV strains encoding for Q9VF/5D were recognized by Q9VF/5D-specific CTLs. In contrast, Q9VF/5N-encoding strains were neither recognized by Q9VF/5N- nor by Q9VF/5D-specific CTLs. Using in vitro proteasomal digestions and MS/MS analysis, we demonstrate that the 5N variation introduces a strong proteasomal cleavage site within the epitope, leading to a dramatic reduction of Q9VF epitope production. Our results strongly suggest that HIV-1 escapes CTL surveillance by introducing mutations leading to HIV ARF-epitope destruction by proteasomes., Author Summary In addition to the classical open reading frames encoding for the well characterized HIV proteins, HIV exhibits a vast number of alternative reading frames that have the potential to encode proteins or polypeptides. We have previously shown that such reading frames within gag, pol and env genes express T cell epitopes. In the present work, we further characterized the role of T-cell responses targeting the gag-overlapping reading frame in the selection of HIV variants in vivo. We demonstrate that under CD8+ T cell immune pressure, HIV escapes by introducing mutation that affects T-cell recognition of HIV-infected cells. We characterized the mechanism of CTL-escape and demonstrate that HIV manipulates antigen processing and presentation. Our results highlight the importance of CTL targeting these alternative reading frame-encoded antigens in the control of HIV replication.

International audience; The approach to the diagnosis of acute pulmonary embolism (PE) is under constant revision with advances in technology, noninvasive approaches, and increasing awareness of the risks of ionizing radiation. Optimal approaches in some categories of patients are controversial. Data are insufficient for evidence-based recommendations. Therefore, this survey of investigators in the field was undertaken. Even among experts there were marked differences of opinion regarding the approach to the diagnosis of acute PE. Although CT pulmonary angiography was usually the imaging test of choice, the respondents were keenly aware of the dangers of ionizing radiation. In view of advances in scintigraphic diagnosis since the Prospective Investigation of Pulmonary Embolism Diagnosis (PIOPED) trial, ventilation/perfusion (V/Q) lung scans or perfusion scans alone and single photon emission computed tomography (SPECT) V/Q lung scans are often recommended. The choice depends on the patient's age, gender, and complexity of the findings on the plain chest radiograph.

Abstract

International audience; In France, free faecal occult blood testing is offered to individuals aged between 50 and 74 years every 2 years as a method of screening for colorectal cancer (CRC). To assess how a proposed organized programme of CRC screening would be perceived among a representative sample of individuals living in France, aged between 40 and 75 years, and by a representative sample of general practitioners, two nationwide observational telephone surveys were carried out in 2005 (EDIFICE 1; 1601 individuals) and 2008 (EDIFICE 2; 1801 individuals). In 2008, 38% of individuals aged between 50 and 74 years reported undergoing screening for CRC; this corresponded to a statistically significant 13% increase in CRC screening rate compared with 2005 (P=0.01). When asked whether it was possible to screen for CRC, 94% of individuals who had undergone screening (N=350) responded positively compared with 87% of individuals who had not been screened. The main reason for individuals not to undergo screening was a lack of awareness (35% of men and 37% of women, P=not significant); the second reason was lack of advice and referral from their general practitioner (21% of women versus 15% of men, P=0.03). The French population is aware of the potential benefit of CRC screening; however, many do not undergo regular screening. It is therefore important to identify the hurdles associated with CRC screening.

International audience; Regular mammography screening has been available in France, free of charge, for all women aged 50-74 years since 2005. Two nationwide surveys [EDIFICE 1 (507 women interviewed; age 50-74 years) and EDIFICE 2 (488 women)] recently collected data on individuals' access to cancer screening procedures. Two further surveys interviewed 600 general practitioners (GPs) each to determine their attitudes towards screening in general and screening for breast cancer in particular. The method of quotas ensured the populations surveyed were representative. In 2005, 93%, and in 2008, 94% of women reported having undergone a screening mammography. Anage-related effect was apparent in the 70-74 year subgroup in 2005, whereas in 2008, over 90% of women aged 50-74 years had had a mammography in their lifetime. In 2008, 66% of women had undergone a mammography as a part of an organized screening programme, versus 45% in 2005. In 2005, 15% of women reported having received no recommendation from their GP, whereas this figure fell to 4% in 2008. In both EDIFICE surveys, two thirds of GPs systematically recommended breast cancer screening to female patients aged 50-74 years; however, female GPs were more likely to systematically recommend this screening. In 2008, 77% of the GPs questioned were aware of the recommended frequency for breast cancer screening, compared with 71% in 2005. Results of studies such as EDIFICE may help increase the proportion of women who undergo breast cancer screening. Successful implementation of organized screening relies heavily on treating physicians. The impact of mammography in breast cancer screening may be further improved by systematic organized screening, by encouraging regular examination and by targeting women who give up screening.

Abstract

International audience; Quality evaluation in the field of Augmented Surgery is strategic for public health policies. It implies to be able to effectively perform evaluation of Quality in term of Expected Medical Benefit (EMB). The notion of EMB is complex and not standardized in this field. To define and to evaluate EMB, it is necessary to discover the knowledge on the domain targeted by the device and to structure it. This paper presents first parts of this work. Focused on navigated knee surgery, it led us to obtain two main results: the identification of a new criterion for evaluating EMB obtained thanks to the formalization of a new kind of metadata. These encouraging results seem to offer new perspectives for the evaluation of devices from the field of augmented surgery.

Abstract

International audience; The Inforare project aims to set up a system for the sharing of clinical and familial data, in order to study how genes are related to the severity of sickle cell disease. While the computerisation of clinical records represents a valuable research goal, an ethical framework is necessary to guarantee patients' protection and their rights in this developing field. Issues relating to patient information during the Inforare study were analysed by the steering committee. Several major concerns were discussed by the committee and formalized in the patients' information letter: educating patients to aid the recruitment of family members, rules of confidentiality and the disclosure of aggregate, individual and unexpected research results. This paper presents the main issues addressed.

Abstract

International audience; PURPOSE: Stigma is a major burden of epilepsy. In sub-Saharan Africa the few studies that addressed epilepsy stigma emphasize enacted, rather than perceived, stigma. This inattention may compromise clinical management and delay help seeking, thereby contributing to the treatment gap. We assessed perceived stigma and identified sociocultural and psychological factors explaining greater stigma among people with epilepsy (PWE) in Benin. METHODS: PWE included in this study were ascertained using a door-to-door survey in the general population in a Beninese rural area. We applied both qualitative and quantitative research methods to assess stigma and patient's experience and beliefs. An Explanatory Model Interview Catalogue (EMIC) and verbally administered questionnaires provided data for demographic, clinical, and sociocultural features. Sociocultural features were evaluated in terms of illness-related experience and sociocultural representations of epilepsy. Depression and anxiety were also screened. RESULTS: Eighty PWE were included. About 68.7% reported feeling stigmatized. Multivariate regression revealed that factors independently associated with perceived stigma were experience of social isolation (p < 0.001), experience of marital problems (p < 0.01), and presence of anxiety disorder (p < 0.01). DISCUSSION: Perceived stigma is an important issue in epilepsy in Benin. Social factors seem to be more influential than sociocultural representation of epilepsy. Insofar as research is needed in other African countries to determine the nature and relevant features of stigma to improve treatment and control.

Abstract

International audience; Average physical stature has increased dramatically during the 20th century in many populations across the world with few exceptions. It remains unclear if social inequalities in height persist despite improvements in living standards in the welfare economies of Western Europe. We examined trends in the association between height and socioeconomic indicators in adults over three decades in France. The data were drawn from the French Decennial Health Surveys: a multistage, stratified, random survey of households, representative of the population, conducted in 1970, 1980, 1991, and 2003. We categorised age into 10-year bands, 25-34, 35-44, 45-54 and 55-64 years. Education and income were the two socioeconomic measures used. The slope index of inequality (SII) was used as a summary index of absolute social inequalities in height. The results show that average height increased over this period; men and women aged 25-34 years were 171.9 and 161.2 cm tall in 1970 and 177.0 and 164.0 cm in 2003, respectively. However, education-related inequalities in height remained unchanged over this period and in men were 4.48 cm (1970), 4.71 cm (1980), 5.58 cm (1991) and 4.69 cm (2003), the corresponding figures in women were 2.41, 2.37, 3.14 and 2.96 cm. Income-related inequalities in height were smaller and much attenuated after adjustment for education. These results suggest that in France, social inequalities in adult height in absolute terms have remained unchanged across the three decades under examination.

Abstract

International audience; Linkages between animal models of diseases and human data enable the development of translational research hypotheses. The objective of this study is to investigate two approaches to integrating phenotype and clinical information. On the one hand, we develop a terminology mapping between phenotypes from the Mammalian Phenotype Ontology (MPO) and Online Mendelian Inheritance in Man (OMIM) through the Unified Medical Language System (UMLS). On the other, we associate MPO phenotypes with OMIM manifestations through annotations made to orthologous genes. 1,469 MPO concepts (22%) were mapped successfully to some disease concept in the UMLS, of which 869 were present in OMIM. Among the 16,764 distinct MGI genes associated with human orthologs, 1,968 distinct genes were associated with both MPO and OMIM annotations. The UMLS is a valuable resource for linking phenotype terms to clinical terminologies, and these mappings between terminologies can help enrich gene annotation databases and unify phenotype representation.

Abstract

Background Thorough knowledge of the regulatory requirements is a challenging prerequisite for conducting multinational clinical studies in Europe given their complexity and heterogeneity in regulation and perception across the EU member states. Methods In order to summarise the current situation in relation to the wide spectrum of clinical research, the European Clinical Research Infrastructures Network (ECRIN) developed a multinational survey in ten European countries. However a lack of common classification framework for major categories of clinical research was identified, and therefore reaching an agreement on a common classification was the initial step in the development of the survey. Results The ECRIN transnational working group on regulation, composed of experts in the field of clinical research from ten European countries, defined seven major categories of clinical research that seem relevant from both the regulatory and the scientific points of view, and correspond to congruent definitions in all countries: clinical trials on medicinal products; clinical trials on medical devices; other therapeutic trials (including surgery trials, transplantation trials, transfusion trials, trials with cell therapy, etc.); diagnostic studies; clinical research on nutrition; other interventional clinical research (including trials in complementary and alternative medicine, trials with collection of blood or tissue samples, physiology studies, etc.); and epidemiology studies. Our classification was essential to develop a survey focused on protocol submission to ethics committees and competent authorities, procedures for amendments, requirements for sponsor and insurance, and adverse event reporting following five main phases: drafting, consensus, data collection, validation, and finalising. Conclusion The list of clinical research categories as used for the survey could serve as a contribution to the, much needed, task of harmonisation and simplification of the regulatory requirements for clinical research in Europe.

Abstract

International audience; OBJECTIVE: To estimate the probability of medical consultation for infertility during the course of a pregnancy attempt and to study its determinants. DESIGN: Pregnancy-based retrospective telephone survey analyzed with a discrete time Cox model. SETTING: Two rural counties in Brittany and Normandy, France. PATIENT(S): A random sample of 901 women from the general population aged 18-60 years reporting 1,460 pregnancy attempts resulting in a live birth between 1985 and 2000 (participation rate, 73%). INTERVENTION(S): None. MAIN OUTCOME MEASURE(S): Probability of medical consultation for involuntary infertility cumulated over time. RESULT(S): The cumulative probability of medical consultation for involuntary infertility among nulligravid women was 45% after 12 months of involuntary infertility and 75% after 24 months. The probability of medical consultation at any time was half that for parous women (odds ratio 0.4, 95% confidence interval 0.2-0.6). More highly educated women were more likely to have sought medical help for infertility. Only 45% of women who had sought medical advice received infertility treatment. CONCLUSION(S): Our survival approach provides a description of infertility service use during the course of a pregnancy attempt, and confirms that parity and educational level are strong predictors of medical help-seeking behaviors.
