1. Evidence for routine brain‐to‐pelvis imaging and antiplatelet therapy in patients diagnosed with fibromuscular dysplasia

Abstract: Abstract Fibromuscular dysplasia (FMD) is a disease of the musculature of arterial walls leading to stenoses, aneurysms, and dissections. The purpose of this report was to summarize the evidence for (1) one‐time routine imaging from brain‐to‐pelvis and (2) lifelong antiplatelet therapy, for example, aspirin, for patients diagnosed with FMD as suggested by an international consensus report from 2019. PubMed was systematically searched, and the evidence providing a basis for the current consensus points, as well as articles published since, were reviewed. In four registries evaluating patients with FMD, the prevalence of multivessel involvement, aneurysms, and dissections was reported to be 43.5%–66.3%, 21.6%–30.6%, and 5.6%–28.1%, respectively. Any antiplatelet drug was used in 72.9% of patients, and aspirin was prescribed in up to 70.2% of patients. Based on the high prevalence of vascular manifestations, their associated morbidity, and the potential for endovascular or surgical intervention, the suggestion of one‐time brain‐to‐pelvis screening with computed tomography angiography or magnetic resonance angiography is well supported. Contrarily, the evidence to support the consensus statement of lifelong antiplatelet therapy to all patients in the absence of contraindications is more uncertain since a beneficial effect has not been demonstrated specifically in patients with fibromuscular dysplasia. Therefore, until the efficacy and safety of primary thromboprophylaxis have been demonstrated in this patient group specifically, it may be equally appropriate to only use antiplatelet agents in patients with a clear indication after individual evaluation according to risk factors for thrombotic and thromboembolic complications.

Abstract: Purpose: To evaluate DCE-MRI and PET-CT at baseline for prediction of pathological response to neoadjuvant chemotherapy (NAC) in patients with locally advanced breast cancer. Materials and methods: 88 patients with locally advanced breast cancer treated with NAC followed in DCE-MRI at 1.5T and PET-CT were included in this retrospective study. Perfusion parameters (Ktrans, kep, ve) and SUVmax were measured at baseline of the NAC. Imaging datas were compared with tumoral and nodal pathologic response, prognostic factors (histologic grade, expression of hormonal receptors, human epidermal growth factor receptor 2, p53 and Ki-67) and subtypes of breast cancers. Results: Response were observed in 54 patients (61%) and non-response in 34 patients. Low ve and high SUVmax were significatively associated with tumoral response to NAC (ve, p=0,0035 ; SUVmax, p=0,0265). Other perfusion parameters were not significatively associated to pathological response. Low ve was significantly associated with high histologic grade (SBR III), negative estrogen receptor, p53 muted and Ki67 >15%). Triple negative cancers showed a lower ve than other subtypes. High SUVmax was significantly associated with high histologic grade (SBR III) and p53 muted. Triple negative and HER2 subtypes showed a higher SUVmax than the luminal type. Conclusion: ve and SUVmax before NAC were associated with tumoral response and histopathological aggressivity markers.; But : Établir un pronostic de réponse avant chimiothérapie néoadjuvante (CNA) dans les cancers du sein localement avancés en IRM de perfusion et TEP-TDM.Matériel et méthodes : Étude rétrospective monocentrique de patientes ayant bénéficié avant CNA d’une IRM perfusionnelle et d’un PET-TDM avec cartographie microcirculatoire (Ktrans, kep, ve) et mesure du SUVmax. Les paramètres d’imagerie ont été comparés avec la réponse tumorale et ganglionnaire après chirurgie selon Sataloff, les facteurs histo-pronostiques des tumeurs (grade SBR, récepteurs hormonaux, HER2, p53, Ki67) et les sous-types de cancers du sein.Résultats : 88 patientes âgées de 23 à 83 ans (moyenne 53 ans) ont été incluses dont 3 Luminal A, 52 Luminal B, 8 HER2 et 25 Basaloïde. Les répondeurs étaient 54 (Sataloff A + B) et les non-répondeurs 34 (Sataloff C +D). Un ve bas et un SUVmax élevé étaient associés significativement avec une réponse tumorale à la CNA (ve p=0,0035 ; SUVmax p=0,0265). Les autres paramètres microcirculatoires n’étaient pas significativement associés à la réponse histologique.Une valeur médiane de ve basse était associée significativement aux facteurs histo-pronostiques d’agressivité (SBR III, RE négatifs, p53 muté et Ki67>15%) et au sous-type basaloïde. Une valeur de SUVmax élevée était associée significativement aux facteurs histo-pronostiques d’agressivité (SBR III, p53 muté) et aux sous-types HER2 et Basaloïde. Conclusion : Une valeur basse de ve et élevée de SUVmax étaient associés à la réponse tumorale, aux facteurs histopronostiques d’agressivité et aux sous-types de cancers du sein de mauvais pronostic. ve et SUVmax pourraient donc être des biomarqueurs de l’agressivité tumorale et pourraient servir à prédire la réponse tumorale.

Abstract: Intracranial cerebral aneurisms (ICA) are a frequent extra-renal manifestation of autosomal dominant polycystic kidney disease (ADPKD). Its main complication is subarachnoid haemorrhage, which is associated with a high morbidity and mortality. Aneurysms can be screened using time-of-flight magnetic resonance imagery (MRI) without gadolinium enhancement.A web-based survey was conducted among 420 French-speaking nephrologists to determine the determinants of screening for ICA in ADPKD patients. Patients consulting for ADPKD at Necker hospital had a targeted ICA screening using MRI. They were followed-up prospectively for subarachnoid haemorrhage, and the results of the screening MRI, if performed, were recorded. ICA screening practices were very heterogeneous among the nephrologists who participated to our study, apart from a few consensual indications such as in case of family history of subarachnoid haemorrhage. After targeted screening, the rate of aneurisms detected was high. However, subarachnoid haemorrhage occurred mostly in the population not targeted by this screening. Benefits of ICA screening in ADPKD have never been evaluated with a prospective, controlled study, and systematic screening is not recommended according to expert opinions. This study shows the necessity to evaluate the benefits of systematic versus targeted screening of ICA in this population using a randomised controlled trial. It will help to predict the rate of events and therefore to determine the number of subjects to include in such study.; Les anévrismes cérébraux sont des manifestations extra-rénales fréquentes dans la polykystose rénale autosomique dominante, dont la complication principale, la rupture anévrismale, présente une grande sévérité. Les anévrismes peuvent être dépistés à partir d’angio-IRM (ARM) cérébrales sans injection de produit de contraste. Une enquête des pratiques sur le dépistage des anévrismes a été conduite auprès de 420 néphrologues francophones qui ont répondu par email à un questionnaire sur leurs pratiques de prescription des ARM de dépistage. De plus, les patients polykystiques suivis en consultation à Necker ont fait l’objet d’une politique de dépistage ciblé et d’un suivi prospectif ; les résultats des ARM de dépistage et la survenue d’évènements neurologiques ont été surveillés. Les pratiques de dépistage paraissent très hétérogènes parmi les néphrologues interrogés, en dehors de quelques indications consensuelles, comme en cas d’antécédent familial d’hémorragie méningée. La réalisation d’un dépistage ciblé permet de mettre en évidence un taux élevé d’anévrisme sur les examens de dépistage. Toutefois, les ruptures d’anévrismes surviennent essentiellement dans la population non ciblée par ce dépistage dans notre étude. Le bénéfice du dépistage de ces anévrismes n’a jamais été étudié de façon prospective et contrôlée, et fait l’objet de recommandations basées sur des avis d’experts. Cette étude montre la nécessité d’évaluer à l’aide d’une étude contrôlée et randomisée le bénéfice d’un dépistage systématique par rapport à un dépistage ciblé des anévrismes dans cette population, et aidera à prévoir le taux d’événement attendu pour déterminer le nombre de sujets à inclure.

Abstract: International audience; BACKGROUND: Lateral, homonymous upper and lower field blind sectors sparing a horizontal zone define quadruple sectoranopia. This rare campimetric deficit involves ischemia or infarction of parts of the lateral geniculate body supplied by the distal anterior choroidal artery. METHODS: A 41-year-old patient presented with a complaint of blurred vision. Visual acuity was 20/20 on both sides. Visual field showed a left quadruple sectoranopia. CT scan, MRI, MRA and conventional angiography showed a right cerebral arteriovenous malformation supplied partly by the right anterior choroidal artery. Partial retinal nerve fiber layer atrophy matched to the campimetric deficit proved that some degree of infarction involved the lateral geniculate body. RESULTS: The arteriovenous malformation was treated with stereotactic radiosurgery. Two years after treatment, the nidus had completely disappeared. The campimetric deficit began improving from 19 months onwards after stereotactic radiosurgery. Twenty-eight months after treatment, the superior defect had completely reversed. CONCLUSIONS: Quadruple sectoranopia involves ischemia or infarction of parts of the lateral geniculate body supplied by the distal anterior choroidal artery. Following radio-induced angioma thrombosis, blood was no longer shunted away from the right lateral geniculate body, whose viable areas resumed their activity. Accordingly, a partial campimetric recovery occurred.

Abstract: Background and Purpose— To replace digital subtraction angiography (DSA) in carotid stenosis evaluation, noninvasive imaging techniques have to reach a high concordance rate. Our purpose is to compare the concordance rates of contrast-enhanced MR angiography (CEMRA) and CT angiography (CTA) with Doppler ultrasound (DUS) in clinical routine practice. Methods— We evaluated prospectively with DUS, CEMRA, and CTA 150 patients suspected of carotid stenosis. The overall concordance rates of the 3 techniques were calculated for symptomatic stenosis ≥50% and ≥70%, for asymptomatic stenosis ≥60%, and for occlusion. For the carotid arteries treated by surgery (n=97), the results of each method and combined techniques were recorded, and misclassification rates were evaluated from surgical reports. Results— The overall concordance rates of DUS-CEMRA, DUS-CTA, and CEMRA-CTA were not statistically different. However, the concordance rate of DUS-CEMRA (92.53%) was significantly higher than that for DUS-CTA (79.10%) in the surgical asymptomatic stenosis group ( P =0.0258). CTA considered alone would misclassify the stenosis in a significant number of cases (11 of 64) in the surgical asymptomatic group compared with CEMRA (3 of 67) and DUS (1 of 66) ( P =0.0186 versus MRA, P =0.0020 versus DUS). Conclusions— With the techniques as utilized in our study, the overall concordance rates of combined noninvasive methods are similar for measuring carotid stenosis in clinical routine practice, but in asymptomatic carotid stenosis, the decision making for surgery is significantly altered if DUS and CTA are considered in place of DUS and CEMRA.

Abstract: International audience; Central nervous system (CNS) angiitis is a rare inflammatory disorder. To date, clinical data are lacking and treatment remains a matter of debate. The aim of this study is to analyse the main characteristics, response to therapy and outcome of 32 patients with CNS angiitis. Single-centre retrospective study in a tertiary centre was made. Diagnosis of CNS angiitis was made by cerebral angiography and/or magnetic resonance angiography and/or CNS biopsy. The main features and outcomes of primary and secondary CNS angiitis were compared and predictive factors of a favourable outcome were searched. Thirty-one patients (median age 45 Q1-Q3 37-54) sex ratio F/M 2.1) were included. Main clinical features were hemiparesis (35.5 %) and headache (29 %). The median CSF protein level was 0.64(0.52-0.81) g/L and was superior to 1 g/L in six cases. CNS magnetic resonance (MR) imaging findings were most frequently ischemic (96.8 %), bilateral (83.9 %), multiple (87.1 %) and supratentorial (96.8 %). The MR angiography was abnormal in all cases. Among the 31 patients in the study, 19 (61.3 %) were diagnosed with primary CNS angiitis. Systemic lupus erythematosus (n = 6) and vasculitis (n = 4) were the most frequent aetiologies of secondary CNS angiitis. No difference was evidenced between primary and secondary CNS angiitis. Steroids were administered in 79.2 % of treated patients and combined with immunosuppressants in 79.2 % of cases. Eight cases of CNS angiitis relapse were noted. CNS angiitis remains a severe illness. Treatment often associated steroids and immunosuppressants, and diagnosis delay is significantly associated with a poorer prognosis.

Abstract: Objectives: To establish the prevalence of intracranial atherosclerosis (ICAD) and symptomatic stenoses by MRangiography in a registry of patients hospitalized for ischemic stroke. To study its association with cardiovascular risk factors and other atherosclerotic locations. Materials and methods: Contrast-enhanced and TOF MRA sequences of 427 patients hospitalized for TIA or cerebral infarction were analyzed, blind from clinical datas and brain MRI. Stenoses were rated according to their severity (50-69%, 70-99%, occlusion), location, and number. Symptomatic nature of the lesions was established secondarily. These data were analyzed in relation to cardiovascular risk factors and extracranial locations of atherosclerotic disease. Results: The prevalence of ICAD was 27.4%, and half of the stenoocclusive lesions (13.6%) were symptomatic. ICAD prevalence increased significantly with age, blood pressure and atherogenic dyslipidemia regardless of the symptomatic character of stenosis. There was no association with diabetes mellitus. We found a strong association between ICAD and ischemic stroke (p=0.023), and between ICAD and CAD (p=0.023). Conclusion: ICAD is present in a quarter of our patients and is responsible for 13% of ischemic strokes. ICAD is strongly associated with coronary artery disease, and should not be ignored from radiologists during etiological assessment of ischemic stroke.; Objectifs : Établir la prévalence de l'athérosclérose intracrânienne (ICAD) et des sténoses symptomatiques en ARM au sein d’un registre de patients hospitalisés pour un accident ischémique cérébral. Matériels et méthodes : Les ARM sans et avec injection de 427 patients hospitalisés pour AIT ou infarctus cérébral ont été analysées en aveugle de la clinique et de l'IRM encéphalique. Les sténoses étaient cotées selon leur sévérité (50-69%, 70-99%, occlusion), localisation, et nombre. Le caractère symptomatique des lésions était établi secondairement. Ces données ont été analysées par rapport aux facteurs de risque cardiovasculaire et aux localisations extracrâniennes d'athérosclérose. Résultats : La prévalence de l'ICAD était de 27,4%, et la moitié des lésions (13,6%) étaient symptomatiques. Le risque d’ICAD augmentait significativement avec l’âge, la pression artérielle et la dyslipidémie athérogène indépendamment du caractère symptomatique de la sténose, et sans association au diabète. On retrouvait une forte association entre ICAD et constitution d’un accident ischémique (p=0,023), et d’autre part entre ICAD et coronaropathie (p=0,023). Conclusion : L'ICAD est présente chez un quart de nos patients et responsable de 13% des AVC ischémiques. L’ICAD est fortement associée à la coronaropathie et ne doit pas être méconnue des radiologues lors du bilan étiologique d’un infarctus cérébral.

Abstract: International audience; OBJECTIVE: The quantification of cerebral blood flow using pulsed arterial spin labeling could be erroneous with a nonoptimal inversion time (TI). We suggest that the optimal individual TI is dependent on the hemodynamic values of the supra-aortic vessels. METHODS: Twenty-two healthy volunteers (mean age, 31.8 years) underwent 7 pulsed arterial spin labeling scans at 3 T with TI ranging from 1200 to 1800 milliseconds every 100 milliseconds. The quantitative hemodynamic values of carotids were computed using a cine phase-contrast sequence. Optimal TIs were determined using visual and quantitative criteria. A correlation was sought between each optimal individual TI criterion and the hemodynamic parameter values. RESULTS: The optimal group TI was 1700 milliseconds. The individual analysis showed a significant correlation between the optimal TI of white matter signal-to-noise ratio and stroke distance, and the optimal TI of contrast-to-noise ratio with mean velocity. CONCLUSIONS: A correlation was found between optimal individual TIs and carotid hemodynamic parameters.

Abstract: International audience; BACKGROUND AND PURPOSE: Our aim was to compare 3D TOF-MRA sequences at 3T and 1.5T in the follow-up of coiled aneurysms. The follow-up of coiled intracranial aneurysms is mandatory to depict potential recanalization. 3D-TOF MRA is an appropriate tool for this purpose. MATERIALS AND METHODS: DSA and 3D TOF-MRA at 1.5T and 3T were performed in a prospective series of 126 aneurysms in 96 patients (58 women, 38 men; age, 25-75 years; mean, 51.3 ± 11.3 years). DSA was the reference standard to which the accuracy of 3D TOF-MRA was compared. The quality of aneurysm occlusion was assessed independently and anonymously by a core lab by using a 3-grade scale (total occlusion, neck remnant, and aneurysm remnant). Adequate occlusion was defined as total occlusion or neck remnant and used in a 2-grade scale: adequate occlusion/aneurysm remnant. RESULTS: With DSA, total occlusion was depicted in 58 aneurysms (46.0%); neck remnant, in 33 aneurysms (26.2%); and aneurysm remnant, in 35 aneurysms (27.8%). Adequate occlusion was seen in 91 cases (72.2%). A remnant (aneurysm or neck) was depicted in 68 cases (54.0%). For the 3 imaging techniques and regardless of scale used, the interobserver agreement was always greater at 3T than at 1.5T. SE and NPV for the prediction of aneurysm remnant versus adequate occlusion were higher at 3T than at 1.5T (SE 3T, 0.74; SE 1.5T, 0.54; NPV 3T, 0.90; NPV 1.5T, 0.85). CONCLUSIONS: In this large prospective series of patients, 3D TOF-MRA was superior at 3T to 1.5T for the evaluation of coiled intracranial aneurysms.

Abstract: To compare 3D-TOF magnetic resonance angiography (MRA) and contrast-enhanced MRA (CE-MRA) sequences at 3T in the follow-up of coiled aneurysms with digital subtracted angiography (DSA) as the gold standard.DSA, 3D-TOF and CE-MRA were performed in a prospective series of 126 aneurysms in 96 patients (57 female, 39 male; age: 25-75 years, mean: 51.3 ± 11.3 years). The quality of aneurysm occlusion was assessed independently and anonymously by a core laboratory.Using DSA (gold standard technique), total occlusion was depicted in 57 aneurysms (45.2%), neck remnant in 34 aneurysms (27.0%) and aneurysm remnant in 35 aneurysms (27.8%). Sensitivity, specificity, positive predictive value and negative predictive value were very similar with 3D-TOF and CE-MRA. Visibility of coils was much better with 3D-TOF (95.2%) than with CE-MRA (23.0%) (P 0.001). Also, substantial artefacts were less frequent with 3D-TOF (4.0%) than with CE-MRA (11.9%; P = 0.012).In this large prospective series of patients with coiled aneurysms, at 3T 3D-TOF MRA was equivalent to CE-MRA for the evaluation of aneurysm occlusion, but coil visibility was superior at 3D-TOF. Thus the use of 3D-TOF at 3T is recommended for the follow-up of coiled intracranial aneurysms.• Different Magnetic Resonance (MR) imaging techniques are used to evaluate intracranial aneurysms. • At 3T MR, 3D-TOF and CE-MRA appear equivalent for evaluating coiled aneurysms.. • Coils are better visualised on 3D-TOF than on CE-MRA. • Combined analysis of 3D-TOF and CE-MRA does not seem helpful. • At 3T, 3D-TOF techniques are recommended for monitoring patients with coiled aneurysms.

Abstract: International audience; PURPOSE: We describe and validate a novel hybrid nonlinear vessel registration algorithm for intra-operative updating of preoperative magnetic resonance (MR) images using Doppler ultrasound (US) images acquired on the dura for the correction of brain-shift and registration inaccuracies. We also introduce an US vessel appearance simulator that generates vessel images similar in appearance to that acquired with US from MR angiography data. METHODS: Our registration uses the minimum amount of preprocessing to extract vessels from the raw volumetric images. This prevents the removal of important registration information and minimizes the introduction of artifacts that may affect robustness, while reducing the amount of extraneous information in the image to be processed, thus improving the convergence speed of the algorithm. We then completed 3 rounds of validation for our vessel registration method for robustness and accuracy using (i) a large number of synthetic trials generated with our US vessel simulator, (ii) US images acquired from a real physical phantom made from polyvinyl alcohol cryogel, and (iii) real clinical data gathered intra-operatively from 3 patients. RESULTS: Resulting target registration errors (TRE) of less than 2.5 mm are achieved in more than 90 % of the synthetic trials when the initial TREs are less than 20 mm. TREs of less than 2 mm were achieved when the technique was applied to the physical phantom, and TREs of less than 3 mm were achieved on clinical data. CONCLUSIONS: These test trials show that the proposed algorithm is not only accurate but also highly robust to noise and missing vessel segments when working with US images acquired in a wide range of real-world conditions.

Abstract: International audience; PURPOSE: The purposes of this study were to (1) develop a high-resolution 3-T magnetic resonance angiography (MRA) technique with an in-plane resolution approximate to that of multidetector coronary computed tomography (MDCT) and a voxel size of 0.35 × 0.35 × 1.5 mm³ and to (2) investigate the image quality of this technique in healthy participants and preliminarily in patients with known coronary artery disease (CAD). MATERIALS AND METHODS: A 3-T coronary MRA technique optimized for an image acquisition voxel as small as 0.35 × 0.35 × 1.5 mm³ (high-resolution coronary MRA [HRC]) was implemented and the coronary arteries of 22 participants were imaged. These included 11 healthy participants (average age, 28.5 years; 5 men) and 11 participants with CAD (average age, 52.9 years; 5 women) as identified on MDCT. In addition, the 11 healthy participants were imaged using a method with a more common spatial resolution of 0.7 × 1 × 3 mm³ (regular-resolution coronary MRA [RRC]). Qualitative and quantitative comparisons were made between the 2 MRA techniques. RESULTS: Normal vessels and CAD lesions were successfully depicted at 350 × 350 μm² in-plane resolution with adequate signal-to-noise ratio (SNR) and contrast-to-noise ratio. The CAD findings were consistent among MDCT and HRC. The HRC showed a 47% improvement in sharpness despite a reduction in SNR (by 72%) and in contrast-to-noise ratio (by 86%) compared with the regular-resolution coronary MRA. CONCLUSION: This study, as a first step toward substantial improvement in the resolution of coronary MRA, demonstrates the feasibility of obtaining at 3 T a spatial resolution that approximates that of MDCT. The acquisition in-plane pixel dimensions are as small as 350 × 350 μm² with a 1.5-mm slice thickness. Although SNR is lower, the images have improved sharpness, resulting in image quality that allows qualitative identification of disease sites on MRA consistent with MDCT.

Abstract: International audience; Uterine arteriovenous malformations (AVM) may be responsible for vaginal bleeding potentially life-threatening. They are most often acquired following uterine trauma (curettage, cesarean section, artificial delivery/uterus examination) in association with pregnancy or gestational trophoblastic disease. We report three cases of patients having uterine AVM after curettage. The diagnostic management is important to avoid differential diagnoses (intra-uterine retention, hemangioma, gestational trophoblastic disease). It is based on serum hCG measurement and Doppler ultrasound, then confirmed on dynamic angio-MRI, which tends to replace angiography as first-line. The therapeutic management in cases of symptomatic AVMs is mostly embolization which offers the possibility for childbearing. Current data on subsequent pregnancies is reassuring even if they remain limited.

Abstract: International audience; PURPOSE: To evaluate the agreement and diagnostic accuracy of Contrast enhanced magnetic resonance angiography (CE-MRA), Doppler ultrasound (DUS) and Digital subtraction angiography (DSA) in the assessment of carotid stenosis. METHODS: DUS, CE-MRA and DSA were performed in 56 patients included in the Carotide-angiographie par résonance magnétique-échographie-doppler-angioscanner (CARMEDAS) multicenter study with a carotid stenosis ≥ 50%. Three readers evaluated stenoses on CE-MRA and DSA (NASCET criteria). Velocities criteria were used for stenosis estimation on DUS. RESULTS: CE-MRA had a sensitivity and specificity of 96-98% and 66-83% respectively for carotid stenoses ≥ 50% and a sensitivity and specificity of 94% and 76-84% respectively for carotid stenoses ≥ 70%. The interobserver agreement of CE-MRA was excellent, except for moderate stenoses (50-69%). DUS had a sensitivity and specificity of 88 and 75% respectively for carotid stenoses ≥ 50% and a sensitivity and specificity of 83 and 86% respectively for carotid stenoses ≥ 70%. Combined concordant CE-MRA and DUS had a sensitivity and specificity of 100 and 85-90% respectively for carotid stenoses ≥ 50% and a sensitivity and specificity of 96-100% and 80-87% respectively for carotid stenoses ≥ 70%. The positive predictive value of the association CE-MRA and DUS for carotid stenoses ≥ 70% is calculated between 77 and 82% while the negative predictive value is calculated between 97 and 100%. CE-MRA and DUS have concordant findings in 63-72%, and the overestimations cases were recorded only for carotid stenosis ≤ 69%. CONCLUSION: Combined DUS-CE-MRA is excellent for evaluation of severe stenosis but remains debatable in moderate stenosis (50-69%) due to the risk of overestimations.

Abstract: International audience; AIMS: Intracoronary administration of autologous bone marrow cells (BMCs) leads to a modest improvement in cardiac function, but the effect on myocardial viability is unknown. The aim of this randomized multicentre study was to evaluate the effect of BMC therapy on myocardial viability in patients with decreased left ventricular ejection fraction (LVEF) after acute myocardial infarction (AMI) and to identify predictive factors for improvement of myocardial viability. METHODS AND RESULTS: One hundred and one patients with AMI and successful reperfusion, LVEF ≤45%, and decreased myocardial viability (resting Tl201-SPECT) were randomized to either a control group (n = 49) or a BMC group (n = 52). Primary endpoint was improvement of myocardial viability 3 months after AMI. Baseline mean LVEF measured by radionuclide angiography was 36.3 ± 6.9%. Bone marrow cell infusion was performed 9.3 ± 1.7 days after AMI. Myocardial viability improved in 16/47 (34%) patients in the BMC group compared with 7/43 (16%) in the control group (P = 0.06). The number of non-viable segments becoming viable was 0.8 ± 1.1 in the control group and 1.2 ± 1.5 in the BMC group (P = 0.13). Multivariate analysis including major post-AMI prognostic factors showed a significant improvement of myocardial viability in BMC vs. control group (P = 0.03). Moreover, a significant adverse role for active smoking (P = 0.04) and a positive trend for microvascular obstruction (P = 0.07) were observed. CONCLUSION: Intracoronary autologous BMC administration to patients with decreased LVEF after AMI was associated with improvement of myocardial viability in multivariate-but not in univariate-analysis. A large multicentre international trial is warranted to further document the efficacy of cardiac cell therapy and better define a group of patients that will benefit from this therapy. Abstract: International audience; OBJECTIVE: The purpose of this study was to evaluate retrospectively whether cerebral blood volume measurement based on pretreatment perfusion MRI is a prognostic biomarker for survival in patients with oligodendroglioma or mixed oligoastrocytoma. PATIENTS AND METHODS: Between 1998 and 2004, 54 patients (23 females and 31 males), aged 21-73 years, with oligodendroglioma (or mixed tumour) were examined prior to beginning treatment with dynamic susceptibility-weighted contrast (DSC) perfusion MRI during gadolinium first-pass. The relative cerebral blood volume (rCBV) was calculated by dividing the measurement within the tumour by the measurement of the normal-appearing contralateral region. Patients were classified in two groups, grade A and grade B, according to the Saint-Anne Hospital classification and followed-up clinically and by means of MRI until their death or for a minimum of 5 years. Patients were also classified in grade II and grade III-IV, according to the World Health Organisation (WHO) classification, and were analysed with the same methods. Age, sex, treatment, tumour grade, contrast agent uptake, and rCBV were tested using survival curves with Kaplan-Meier's method, and their differences were analysed using the log-rank test. RESULTS: In this population, median survival was 3 years. A rCBV threshold value of 2.2 was validated as a prognostic factor, for survival in these patients with oligodendrogliomas. Age, sex, contrast uptake, and maximum rCBV were found to be prognostic factors in univariate analysis. Multivariate analysis revealed that tumour grade (grade A/grade B), rCBV, age, and sex were prognostic factors independent of the other factors. The tumour grade according to the WHO classification (II versus III-IV) was also detected as an independent prognostic factor. CONCLUSION: Pretreatment rCBV measured by DSC perfusion MRI was found to be a prognostic factor for survival in patients with oligodendroglioma or mixed tumour, by using the Saint-Anne Hospital classification, which separate the IIB from the IIA.

Abstract: International audience; BACKGROUND AND PURPOSE: No practical tool has been reported in the literature to evaluate the quality of cerebral TR-3D-CE-MRA techniques. Our study assessed a large list of parameters used to propose a quality-evaluation scheme for TR-3D-CE-MRA. MATERIALS AND METHODS: A large list of visual and quantitative parameters used to study the quality of images was collected from the literature and evaluated in 19 healthy patients and 11 patients with arteriovenous shunts who had undergone both CENTRA keyhole TR-3D-CE-MRA at 3T and CCA. Several observers evaluated the visual parameters, such as the diagnostic confidence index, artifacts, maximum vascular signal intensity, arterial-to-venous separation, and visibility of 17 arteries and 7 veins; and quantitative parameters, such as maximum arterial SI, arteriovenous transit time, arteriovenous contrast curve, and ADW. A statistical analysis was used to determine interobserver reproducibility of the visual parameters, to calculate the sensitivity of TR-3D-CE-MRA for detecting each vessel (with CCA as standard of reference), and to compare the results of the visual and quantitative evaluations. RESULTS: Diagnostic confidence index, artifacts, arterial-to-venous separation, and 4 vessels-the PICA, ophthalmic and occipital arteries, and the ISS-demonstrated high reproducibility and sensitivity. The ADW was the most reliable dynamic quantitative parameter and was correlated with arterial-to-venous separation. CONCLUSIONS: The image quality of TR-3D-CE-MRA can be effectively evaluated with a scheme of 1 quantitative and 7 visual parameters.

Abstract: International audience; OBJECTIVES: To describe an optimized 3D time-resolved contrast-enhanced MR angiography (3D TR-CE-MRA) at 3T in diagnosing head and neck paragangliomas and assessing their morphology and relation to neighboring vessels. METHODS: In a prospective study, eight consecutive patients presenting cranial cervical masses suspected to be 10 paragangliomas were examined with 3D TR-CE-MRA at 3T. Two neuroradiologists evaluated the overall image quality, the presence of a paraganglioma, the maximum diameter, as well as the vessel invasion. RESULTS: In all of the cases, the overall image quality was scored as good. The tumors (n=10) were all visualized and localized. The mean maximum diameter was 32.7mm [range 7-80]. Vessel invasion was assessed as uncertain in one case and improbable in nine cases. CONCLUSION: 3D TR-CE-MRA at 3T associated with conventional sequences facilitates a comprehensive investigation of paragangliomas, thus providing the anatomical and functional information.

Abstract: International audience; New MR imaging protocols enable visualization of brain structures. However, for dedicated clinical applications such as targeting deep brain stimulation (DBS), a more accurate localization requires the use of atlases. We developed a three-dimensional digitized mono-subject anatomical template of the human brain based on 3-T magnetic resonance images (MRI). By averaging 15 registered T1 image acquisitions, we have shown that the final image corresponds to an optimal image, limited by the performance of the 3-T MR machine. We compared different preprocessing workflows for template construction. With the optimal strategy, along with validated existing processing methods, one T1 template and one T1-T2 mixing template were created in order to improve visualization of spatially complex deep structures. Reduction of voxel size to 0.25 mm(3) was also advantageous to observe fine structures and white matter/gray matter intensity crossings. Results demonstrated that such a template also improved inter-patient registration for population comparison in DBS. These MR templates are made freely available to our community (http://www.vmip.org/mritemplate) to serve as a reference for neuroimage processing methods.

Abstract: International audience; OBJECTIVE: To compare 3D time-of-flight MR angiography (TOF-MRA) at 3 Tesla (3T) with digital subtraction angiography (DSA) for the evaluation of intracranial aneurysm occlusion after endovascular coiling. METHODS: In a prospective study, 51 consecutive patients (25 females, 26 males; median age, 51 years) with 51 saccular aneurysms treated with endovascular coiling underwent simultaneous DSA and 3T TOF-MRA at follow-up. DSA and TOF-MRA images were analyzed independently by two senior neuroradiologists. Findings were assigned to 1 of 3 categories in the Raymond classification: complete obliteration, residual neck or residual aneurysm. Agreement between observers and techniques was evaluated using kappa statistics. RESULTS: DSA images were not interpretable for one patient. Interobserver agreement was determined as excellent for DSA (kappa=0.86) and TOF-MRA (kappa=0.80). After reaching a consensus, DSA follow-up showed 26 (51%) complete obliterations, 20 (39%) residual necks and 4 (8%) residual aneurysms. TOF-MRA showed 23 (45%) complete obliterations, 22 (43%) residual necks and 6 (12%) residual aneurysms. Comparison between TOF-MRA and DSA showed excellent agreement between the techniques (kappa=0.86). In the four cases that were misclassified, TOF-MRA findings were assigned to a higher class than for DSA. CONCLUSION: TOF-MRA at 3T is at least as efficient as DSA for the evaluation of intracranial aneurysm occlusion after endovascular treatment with detachable coils. We suggest that TOF-MRA at 3T might be used as the primary method for imaging follow-up of coiled intracranial aneurysms.

Abstract: International audience; To enhance the clinical value of coronary magnetic resonance angiography (MRA), high-relaxivity contrast agents have recently been used at 3T. Here we examine a uniform bilateral shadowing artifact observed along the coronary arteries in MRA images collected using such a contrast agent. Simulations were performed to characterize this artifact, including its origin, to determine how best to mitigate this effect, and to optimize a data acquisition/injection scheme. An intraluminal contrast agent concentration model was used to simulate various acquisition strategies with two profile orders for a slow-infusion of a high-relaxivity contrast agent. Filtering effects from temporally variable weighting in k-space are prominent when a centric, radial (CR) profile order is applied during contrast infusion, resulting in decreased signal enhancement and underestimation of vessel width, while both pre- and postinfusion steady-state acquisitions result in overestimation of the vessel width. Acquisition during the brief postinfusion steady-state produces the greatest signal enhancement and minimizes k-space filtering artifacts.

Abstract: International audience; We reviewed the literature about the usefulness of screening for carotid artery lesions in patients with coronary artery disease (CAD). The use of duplex ultrasonography to detect carotid lesions has a two-fold interest in this situation. First, it enables the detection of severe carotid artery stenosis (CS), a potentially reversible cause of stroke. Stroke is frequent in patients with CAD, accounting for at least in one-fourth of all cardiovascular events during follow-up. The mean prevalence of CS > 60% in patients with CAD is 9%, too low to justify routine carotid screening. However, older age, history of a cerebrovascular event, and peripheral arterial disease are useful factors for focusing screening on a high-risk subset of patients with CAD. Trials assessing the interest of prophylactic carotid surgery in asymptomatic CS of 60% or more found similar benefits in the subset of patients with CAD. Screening for CS in patients undergoing coronary artery bypass grafting is also important because CS is a marker, and sometimes the source, of perioperative stroke. Unfortunately, randomized studies to determine the best strategy in this situation are lacking. Second, while measurement of the carotid intima-media thickness does not provide substantial prognostic information for patients with CAD, the presence of plaque, especially hypoechogenic or echolucent (lipid-rich) plaque, is associated with an unstable or high-risk cardiovascular status. In this situation, specific studies are warranted to compare carotid imaging with biomarkers such as C-reactive protein and stratify the cardiovascular risk.

Abstract: International audience; PURPOSE: To demonstrate the added diagnostic value of time-resolved imaging of contrast kinetics (TRICKS) in the evaluation of lower limb arteries compared to standard 3 level MRA with stepping table method. PATIENTS AND METHODS: Forty patients (30% diabetics) with lower extremity peripheral arterial disease (87.5% with chronic ischemia) underwent standard contrast MRA including TRICKS of the distal arteries. Five arterial segments were defined per leg, and 395 arterial segments were compared (one patient with amputation). Two reviewers evaluated the quality of arterial imaging, presence of venous return and degree of stenosis per segment. The degree of interobserver agreement for arterial stenosis measurement was calculated. RESULTS: More arterial segments could be analyzed on the TRICKS sequence (good or excellent analysis in 63.03%-66.32% of arterial segments compared to 41.51%-47.08% on routine MRA). There was less venous contamination on TRICKS images (25.57% to 27.60% gain). The degree of interobserver agreement was superior with TRICKS compared to standard MRA (kappa 0.85 vs 0.69). CONCLUSION: The TRICKS sequence can be added to standard MRA for pre-therapeutic evaluation of distal arteries in patients with peripheral arterial disease, especially with chronic ischemic with rest pain and/or trophic changes.

Abstract: International audience; The purpose of our study was to determine the main anatomical features of the M1 segment of middle cerebral artery (MRA) using a 3D TOF-MRA at 3 T. Reconstructed and post-processed MRA images were independently analysed by two anatomists in order to determine the course patterns, the division patterns and the early cortical branches patterns of the M1 segments. The division patterns were defined as bipode, tripode or other. The ECB were studied according to their number and their distance from the origin of the M1 segment. The interobserver agreement, to determine the division patterns of the M1 segment, was calculated. The division of the M1 segment was bipode in 73% of the MCAs, monopode in 17%, tripode in 9%, and fan-shaped in 1. In 46% of the cases no ECB was found. In the other cases, only 1 ECB was found and it arose from the medium part of the M1 segment. Our results confirm post-mortem microdissection studies and show that strict anatomical criteria may be applied to 3D TOF MRA at 3 T.

Abstract: Objective— The association of inflammatory cells and neovessels in atherosclerosis is considered a histological hallmark of high-risk active lesions. Therefore, the development and validation of noninvasive imaging techniques that allow for the detection of inflammation and neoangiogenesis in atherosclerosis would be of major clinical interest. Our aim was to test 2 techniques, black blood dynamic contrast enhanced MRI (DCE-MRI) and 18-fluorine-fluorodeoxyglucose (18F-FDG) PET, to quantify inflammation expressed as plaque neovessels content in a rabbit model of atherosclerosis. Methods and Results— Atherosclerotic plaques were induced in the aorta of 10 rabbits by a combination of 2 endothelial abrasions and 4 months hyperlipidemic diet. Six rabbits underwent MRI during the injection of Gd-DTPA, whereas 4 rabbits were imaged after injection of 18F-FDG with PET. We found a positive correlation between neovessels count in atherosclerotic plaques and (1) Gd-DTPA uptake parameters evaluated by DCE-MRI ( r =0.89, P =0.016) and (2) 18F-FDG uptake evaluated by PET ( r =0.5, P =0.103 after clustered robust, Huber-White, standard errors analysis). Conclusion— DCE-MRI and 18F-FDG PET may allow for the evaluation of inflammation in atherosclerotic plaques of rabbits. These noninvasive imaging modalities could be proposed as clinical tools in the evaluation of lesion prognosis and monitoring of anti–angiogenic therapies.

We present a case of intimal sarcoma arising from right pulmonary artery and left lower pulmonary vein observed in a 44-year-old man with a non-productive cough. Computed tomographic scans and magnetic resonance imaging showing filling defect enhancement contributed early, suggesting the diagnosis of primary vascular tumor, hypothesis confirmed by pathologist findings.

Abstract: International audience; BACKGROUND AND PURPOSE: The present study was undertaken to assess the ability of bedside transcranial color-coded sonography (TCCS) to define the course and the patterns of the division of the middle cerebral artery (MCA) M1 segment compared with 3D time-of-flight magnetic resonance angiography (MRA) at 3 T. METHODS: Because MRA is considered as a reliable noninvasive technique to depict the normal anatomy of the intracranial artery, we prospectively defined the course and the patterns of division of 100 MCA M1 segments using bedside TCCS and compared the results with MRA findings. RESULTS: In 68/85 cases (80%), the sonographer was able to define the division of the M1 segment, and classified these as bipode for 50/68 (73.5%), monopode for 16/68 (23.5%) and tripode for 2/68 (3%). A comparison of the TCCS and MRA findings showed a perfect correlation (100%) for the course of the M1 segment. As for the type of division, the correlation was good in 45/68 of cases (67%). TCCS had a sensitivity of 78%, a specificity of 67%, a positive predictive value of 87% and a negative predictive value of 52%, when used to determine whether the division was bipode or not. CONCLUSIONS: Our results show that bedside TCCS can now provide anatomical information on the M1 segment of the MCA.

Abstract: International audience; Atherosclerosis initially develops predominantly at the aortic root and carotid origin, where effective visualization in mice requires efficient cardiac and respiratory gating. The present study sought to first compare the high-resolution MRI gating performance of two digital gating strategies using: 1) separate cardiac and respiratory signals (double-sensor); and 2) a single-sensor cardiorespiratory signal (ECG demodulation), and second, to apply an optimized processing technique to dynamic contrast-enhanced (CE) carotid origin vessel-wall imaging in mice. High-resolution MR mouse heart and aortic arch images were acquired by ECG signal detection, digital signal processing, and gating signal generation modeled using Simulink (MathWorks, USA). Double-sensor gating used a respiratory sensor while single-sensor gating used breathing-modulated ECG to generate a demodulated respiratory signal. Pre- and postcontrast T(1)-weighted images were acquired to evaluate vessel-wall enhancement with a gadolinium blood-pool agent (P792; Guerbet, France) at the carotid origin in vivo in ApoE(-/-) and C57BL/6 mice, using the optimized cardiorespiratory gating processing technique. Both strategies provided images with improved spatial resolution, less artifacts, and 100% correct transistor-to-transistor logic (TTL) signals. Image quality allowed vessel-wall enhancement measurement in all the ApoE(-/-) mice, with maximal (32%) enhancement 27 min postinjection. The study demonstrated the efficiency of both cardiorespiratory gating strategies for dynamic contrast-enhanced vessel-wall imaging.

Abstract: International audience; Fifty consecutive patients aged 52+/-12 years suffering from drug refractory atrial fibrillation (AF) underwent baseline and post-ablation MR angiography (MRA) at a mean follow-up of 4+/-3.5 months. Pulmonary vein (PV) disconnection was performed with a maximum energy delivery of 30 W. MRA allowed a two-plane measurement of each PV ostium. After ablation, no significant stenosis was observed, and only 1/194 (0.5%) and 3/194 (2%) PVs had a diameter reduction of 31-40% in the coronal and axial planes, respectively. There was a significant overall post-procedural PV narrowing of 4.9% in the coronal plane and 6.5% in the axial plane (P=ns between both planes). MRA is an efficient technique that can be used in pre- and postoperative evaluation of AF patients. Using a maximal power delivery limited to 30 W, no significant PV stenosis was observed at mid-term follow-up. Late PV anatomical assessment is needed to confirm these results on long-term follow-up.

Abstract: International audience; PURPOSE: To prospectively evaluate accuracy of gadolinium-enhanced three-dimensional (3D) magnetic resonance (MR) angiography with gadodiamide and gadopentetate dimeglumine (0.1 mmol/kg), with intraarterial DSA as reference standard, for imaging abdominal and iliac arterial stenoses. MATERIALS AND METHODS: The study was approved by all institutional review boards; informed consent was obtained from each subject before procedures. Two hundred forty-seven subjects were included; 240 received either contrast agent and were available for safety analysis; 222 were available for accuracy analysis. Enhanced 3D MR angiography and DSA were performed; image data were evaluated in a double-blinded randomized study. Stenoses were classified as not relevant ( or =50%). For detection of main stenosis, accuracy with enhanced 3D MR angiography compared with that with DSA was determined. RESULTS: The difference in accuracy for imaging with gadodiamide and gadopentetate was 3.6%. Noninferiority was inferred because the lower bound of the exact two-sided 95% confidence interval was -10.1 and was above the noninferiority margin (-15%). Accuracy for detection of the main stenosis was low, 56.4% for gadodiamide and 52.8% for gadopentetate group. Subgroup analysis with exclusion of inferior mesenteric artery and internal iliac arteries and the most false-positive stenosis classifications yielded better results: 76.6% and 71.6%, respectively. Sensitivity, specificity, and negative and positive predictive values did not differ substantially between study groups. In the main analysis, values were 44%, 96%, 35%, and 97% for gadodiamide and 44%, 83%, 30%, and 90% for gadopentetate, respectively. In the subgroup analysis, values were 66%, 95%, 61%, and 96% for gadodiamide and 63%, 86%, 58%, and 88% for gadopentetate, respectively. CONCLUSION: Noninferiority of gadodiamide versus gadopentetate was verified based on the primary end point, which was accuracy for detection of the main stenosis with enhanced 3D MR angiography compared with DSA.

Abstract: Purpose To design an ideal first-pass profile for MR angiography (MRA) by optimizing a multiphasic injection protocol based on two experimental animal models. Materials and Methods An equivalent contrast-enhanced (CE) MRA injection protocol was developed with controlled injection modalities (injection rate, volume, and dose) in rabbits and pigs. P792, a blood pool contrast agent, was injected in 17 male New Zealand rabbits and five farm pigs with variable injection schemes (mono- and multiphasic). From the gadolinium (Gd) blood concentration data, a simulation of an MR acquisition was performed to evaluate the impact of such an injection protocol on MR arterial signal and to select the best injection protocol. Results An empirical relationship between the arterial peak concentration and the injection parameters was found in the rabbits and pigs, allowing precise prediction of the first-pass profile. Of the four injection scheme strategies tested (standard bolus and bi-, tri-, and multiphasic injection protocols), the multiphasic “ramp” injection protocol provided the most optimal contrast agent pharmacokinetics with a durable plateau of concentration. Abstract: International audience; A novel mechanism of MRI contrast enhancement, based on the detection by a balanced steady-state free precession (SSFP) sequence of the proton resonance frequency shift induced by bulk magnetic susceptibility (BMS) contrast agents, was investigated. The potential for this contrast mechanism to image blood vessels was explored. The relaxation time and the frequency shift effects of gadolinium- and dysprosium-DOTA on SSFP signal was first simulated and evaluated on a water phantom at 1.5 T. In vitro, a 5-mM concentration in contrast agent induced a 20-Hz frequency shift, leading to a signal increase of 92% for Dy-DOTA, and a 10-Hz frequency shift, leading to a signal increase of 58% for Gd-DOTA at the reference frequency, taking into account the nonlinear SSFP signal response on frequency offset. The concept was then evaluated in vivo on anesthetized rabbits. Low doses of dysprosium-DOTA were injected in their vascular system, and imaging was performed at the level of neck vessels. Following a bolus injection, mean signal changes of 31%, 20% and 14% were observed in the carotid arteries, the vertebral veins and the jugular veins, respectively. The bolus peak times in arteries and veins were consistent with the rabbit vascular circulation. This frequency-shift based contrast mechanism presents interesting potential for contrast-enhanced MR angiography (CE-MRA) compared to usual relaxation-based contrast, but further investigations on reproducibility will be necessary.

Abstract: International audience; OBJECTIVE: Part of the planning and performance of neurosurgery consists of determining target areas, areas to be avoided, landmark areas, and trajectories, all of which are components of the surgical script. Nowadays, neurosurgeons have access to multimodal medical imaging to support the definition of the surgical script. The purpose of this paper is to present a software environment developed by the authors that allows full multimodal and multi-informational planning as well as neuronavigation for epilepsy and tumor surgery. MATERIALS AND METHODS: We have developed a data fusion environment dedicated to neuronavigation around the Surgical Microscope Neuronavigator system (Carl Zeiss, Oberkochen, Germany). This environment includes registration, segmentation, 3D visualization, and interaction-applied tools. It provides the neuronavigation system with the multimodal information involved in the definition of the surgical script: lesional areas, sulci, ventricles segmented from magnetic resonance imaging (MRI), vessels segmented from magnetic resonance angiography (MRA), functional areas from magneto-encephalography (MEG), and functional magnetic resonance imaging (fMRI) for somatosensory, motor, or language activation. These data are considered to be relevant for the performance of the surgical procedure. The definition of each entity results from the same procedure: registration to the anatomical MRI data set (defined as the reference data set), segmentation, fused 3D display, selection of the relevant entities for the surgical step, encoding in 3D surface-based representation, and storage of the 3D surfaces in a file recognized by the neuronavigation software (STP 3.4, Leibinger; Freiburg, Germany). RESULTS: Multimodal neuronavigation is illustrated with two clinical cases for which multimodal information was introduced into the neuronavigation system. Lesional areas were used to define and follow the surgical path, sulci and vessels helped identify the anatomical environment of the surgical field, and, finally, MEG and fMRI functional information helped determine the position of functional high-risk areas. CONCLUSION: In this short evaluation, the ability to access preoperative multi-functional and anatomical data within the neuronavigation system was a valuable support for the surgical procedure.

Abstract: International audience; Recent developments in the use of Magnetic Resonance Imaging (MRI) to measure and image molecular diffusion and blood microcirculation (perfusion) hold significant promise in the noninvasive evaluation of normal brain function and functional disorders. Molecular diffusion is the result of spontaneous random motion that involves all molecules and probes molecular motion at microscopic level. Using diffusion MRI, information on tissue geometry and compartmentation effects can be obtained. Diffusion MRI has been used to map myelin fiber orientation in brain with high accuracy. Diffusion MRI is also the only imaging modality which shows brain ischemia at a very early stage, even before T1w or T2w MR images become abnormal, offering great promises in the management of stroke patients. Also, diffusion imaging may be used to monitor tissue temperature changes noninvasively during hyperthermia or laser surgery. On the other hand, MRI can provide information on tissue perfusion. Several methods have been proposed, some of them including tracers or contrast agents. The most successful approach for brain function studies, however, is based on the sensitivity of MRI to magnetic effects induced by changes in the oxygenation status of hemoglobin (deoxyhemoglobin). These effects have already been used to characterize hematomas. These effects may also be exploited to detect small modulation in red blood cell oxygen content related to local variations in blood flow and oxygen consumption in tissues. In the brain cortex, such variations may be induced by external stimuli or internal cognitive processes. Capillary blood deoxyhemoglobin thus acts as a natural endogeneous contrast agent.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract: BACKGROUND AND PURPOSE: Brain hypervascular diseases are complex and induce hemodynamic disturbances on brain parenchyma, which are difficult to accurately evaluate by using perfusion-weighted (PWI) MR imaging. Our purpose was to test and to assess the best AIF estimation method among 4 patients with brain hypervascular disease and healthy volunteers. METHODS: Thirty-three patients and 10 healthy volunteers underwent brain perfusion studies by using a 1.5T MR imaging scanner with gadolinium-chelate bolus injection. PWI was performed with the indicator dilution method. AIF estimation methods were performed with local, regional, regional scaled, and global estimated arterial input function (AIF), and PWI measurements (cerebral blood volume [CBV] and cerebral blood flow [CBF]) were performed with regions of interest drawn on the thalami and centrum semiovale in all subjects, remote from the brain hypervascular disease nidus. Abnormal PWI results were assessed by using Z Score, and evaluation of the best AIF estimation method was performed by using a no gold standard evaluation method. RESULTS: From 88% to 97% of patients had overall abnormal perfusion areas of hypo- (decreased CBV and CBF) and/or hyperperfusion (increased CBV and CBF) and/or venous congestion (increased CBV, normal or decreased CBF), depending on the AIF estimation method used for PWI computations. No gold standard evaluation of the 4 AIF estimates found the regional and the regional scaled methods to be the most accurate. CONCLUSION: Brain hypervascular disease induces remote brain perfusion abnormalities that can be better detected by using PWI with regional or regional scaled AIF estimation methods.
