研究目的は、Ｃ型慢性肝炎患者におけるインターフェロン（IFN）療法の中断に関連する要因を明らかにし、治療の継続支援を実施するための基礎資料とすることである。研究対象の選定方法は、外来にて治療中又は経過観察中の患者から治療中断者23人（男性8人、女性15人）をケース（中断群）とし、5歳年齢階級で性・年齢をマッチングさせた治療終了者69人（男性24人、女性45人）をコントロール（終了群）として、後ろ向きケース・コントロール研究（case-control study)を行った。中断群と終了群との比較において、有意な差がみられた要因について多重ロジスティック回帰分析を行い、オッズ比を算出した。IFN療法の治療中断に独立して寄与することが明らかとなった要因（オッズ比）は「うつ傾向あり（6.0）」、「副作用の程度がひどい（5.1）」のみで、他の副作用の症状には統計学上有意な差が認められなかった。また、治療中の患者への身体的・精神的サポートの有無も治療中断と有意な関連は認められなかったが、終了群において治療を中断しようと思ったことのある14人（20.6％）のうち、4人（28.6％）が医療関係者が支えてくれたから継続できたと回答していた。本研究の結果から、専門職が治療開始前にうつ傾向の有無を確認したり、治療中・治療後の患者の心理的な変化を注意深く観察していく必要性が再確認された。また、副作用に関しては、患者や家族に対する事前の情報提供や発現時期に応じた症状軽減に対する迅速な対応が、IFN療法の継続支援には重要であることが示唆された。

看護師の看護実践力を育成するには、臨地実習での技術教育を効果的に展開することが急務である。本学では、卒業までに到達すべき看護技術教育の学習目標を活用してSBOを構築し、学生が臨地実習でも主体的に看護技術を学習し、自己評価ができる教育プログラムを作成し教授している。本研究では、この教育プログラムに基づいて、臨地実習を行った学生の学習体験の頻度を調査し、知識と技能の差の有意を明らかにすることとした。さらに、各看護技術項目に対して学生がどの程度自信を持ているかを調査し、臨地実習での学習体験の頻度と学生の自信の関連を明らかにすることとした。調査対象は本学3年次生100名とし、調査・研究に同意して調査票に回答した内の有効回答者79名を分析することとした。研究倫理として、この調査は研究者が実習成績に直接関与しないこと、調査結果は統計処理をするために匿名化することで学生個人の成績とは別のものであることを保証した。調査方法は、厚生労働省の看護技術の在り方検討会が提示した「臨地実習において看護学生が行う基本的な看護技術」水準1の50項目について、臨地実習での学習体験の頻度と自信度を測定することとした。臨地実習での学習体験の頻度はSBOに基づいて、体験したものにチェックをする方法とした。自信は、50項目の看護技術について「できる」～「できない」を4段階の尺度として測定した。分析はWilcoxon野符号付順位検定とPearsonの相関関係を用いて分析をした。その結果、臨地実習での看護技術の学習体験の頻度では、知識面が技術面よりも得点が高く、両者には有意な差が認められた。また、失禁時の援助・入浴介助・陰部洗浄などの12項目の看護技術で臨地実習での学習体験の頻度と自信に有意な正の相関が認められた。これらから、臨地実習での技術教育では、学生が知識だけでなく、実施した技能を自己評価できるような指導を強化・工夫する必要が示唆された。, To raise nursing practical ability of nurses, it is a pressing need to develop effectively skill education in clinical and practical training. In the present college, SBO (specific behavioral objectives) has been constructed by utilizing the learning goal of nursing skill education to arrive at until the graduation time, and the educational programs, which students can learn subjectively nursing skills in the clinical and practical training and can value by themselves, has been made and taught. At this research, in accordance with these programs, learning experience of student's who did the clinical and practical training was investigated, and the significance of the difference between knowledge and skills was made clear. And also, to investigate how much students had self-confidence against each nursing skill items was done, and to make it clear relation with students' self-confidence and learning experience in the clinical and practical training was intended. Investigative subjects were 100 third-year-grade students of this college, and 79 students who replied this inquiry with consenting to the investigation and research were analyzed. As a researching ethics, this investigation assured that researcher would not be concerned directly with their practical training record; result was different from student's individual record owing to anonymity by treating as statistical transaction. The method of this investigation was to measure the degree of learning experience in the clinical and practical training and self-confidence, about 50 items of LEVEL 1, “fundamental nursing skills which nursing students do in clinical and practical training” suggested by investigative commission on shape of nursing skills of Ministry of health, Labour and Welfare. Learning experience in the clinical and practical training was, according to SBO, the way to check the experienced things. Self-confidence was measured, as standards of 4 steps, by “can” or “cannot” about 50 items nursing skills. Analysis was done by using the Wilcoxon Signed-Ranks Test and Pearson's correlation. In result, on the learning experience in the clinical and practical training, the knowledge aspect was higher points than the skill one, the significant difference was observed on the both aspects. On 12 items of nursing skills; support of urinary incontinence, bath assistance, or pubic washing etc., between learning experience in the clinical and practical training and self-confidence, significant positive correlation was observed. From this result, it was suggested the necessity to intensify and to device the guidance that students can value by themselves not only knowledge but also technical skills, on skilleducation in the clinical and practical training.

人間は性的存在（Sexual being) であり、人間の性はセクシュアリティとして捉えられ、身体的、心理的、社会文化的に規定され、統合される。慢性、急性の病やその治療は人間の性機能、自己概念、生活様式、人間関係に関わり、病者のセクシュアリティは影響を受けるため、セクシュアリティへの看護ケアが必要となってくる。 一方、1860年代F.Nightingaleによって体系化が始まる看護理論は看護の基礎教育においても、実践や研究においても広く活用されており、特に初学者である看護学生に看護の概念や方法論を教授するためには看護理論は重要であり、多くの教育機関において、看護理論が教授されている。しかし看護における理論家の中で性を取り上げているのはD.E Johnson, J. Watoson、Sister C.Royと極めて少ない現状にあり、これらの理論を活用しながら学生が人間を性的存在として捉え、援助するための教育が重要となる。したがって本稿では３人の看護理論家における性のとらえ方やその看護を概観し、学生が臨地実習時にRoyモデルを活用して展開した記録を分析した。その結果、看護学生は看護モデルを活用して患者の性をセクシュアリティと捉え、援助を実施することが可能であることが実証された。さらに(1)学生がRoyモデルを活用し、受け持ち患者のセクシュアリティへの援助を実施する看護過程のそれぞれの段階における指導の必要性、(2)人間の性、病者の性、看護者自身の性に関する教育を看護モデルを活用し、効果的に実施していくために授業－臨地実習を有機的に連動させる必要性が示唆された。, The human being is regarded as sexual being and people's sexuality is an integrated complex of physical psychological, and socio-cultural foctors and influences. Chronic and acute illnesses, and their treatment, affect patient's sexual function, self-concept, lifestyle, and human relations, and therefore their sexuality. As such, nursing care regarding sexuality is inportant. On the other hand, nursing theories, which began to be systematized by Florence Nightingale in the 1860s, are frequently used for basic education in nursing, actual practice, and research. Especially in teaching the concepts and methodologies of nursing to nursing students, who are beginners in nursing, these theories are important, and they are taught at a number of educational institutions. However, only three have addressed the topic of sexuality in their nursing theory and models: D.E. Johnson, J. Watson, and Sister C.Roy. Therefore, necessary is the education encouraging students consider their patients as sexual beings and care them by using the nursing theory and models. This paper examines how to consider sexuality and its related nursing theory and models, analyses the records of a student's nursing process using the Roy model and her report, and provides some suggestions for education. The results prove that the nursing students can consider their patients as sexual beings and care them by using the Roy model. They also indicate the following necessities, (1) giving insturuction in each nursing process to the students providing care regarding sexuality for their patients by using the Roy madel, and (2) linking the class and clinical practicum organically in order to educate nursing students effectively regarding sexuality of human being, patients and nurses themselves, through using these nursing theory and models.
