- Abstract
Background: Major depressive disorder (MDD) is prevalent and disabling among older adults. Standing on its tolerability profile, vortioxetine might be a promising alternative to selective serotonin reuptake inhibitors (SSRIs) in such a vulnerable population. Methods: We conducted a randomised, assessor- and statistician-blinded, superiority trial including older adults with MDD. The study was conducted between 02/02/2019 and 02/22/2023 in 11 Italian Psychiatric Services. Participants were randomised to vortioxetine or one of the SSRIs, selected according to common practice. Treatment discontinuation due to adverse events after six months was the primary outcome, for which we aimed to detect a 12% difference in favour of vortioxetine. The study was registered in the online repository clinicaltrials.gov (NCT03779789). Findings: The intention-to-treat population included 179 individuals randomised to vortioxetine and 178 to SSRIs. Mean age was 73.7 years (standard deviation 6.1), and 264 participants (69%) were female. Of those on vortioxetine, 78 (44%) discontinued the treatment due to adverse events at six months, compared to 59 (33%) of those on SSRIs (odds ratio 1.56; 95% confidence interval 1.01–2.39). Adjusted and per-protocol analyses confirmed point estimates in favour of SSRIs, but without a significant difference. With the exception of the unadjusted survival analysis showing SSRIs to outperform vortioxetine, secondary outcomes provided results consistent with a lack of substantial safety and tolerability differences between the two arms. Overall, no significant differences emerged in terms of response rates, depressive symptoms and quality of life, while SSRIs outperformed vortioxetine in terms of cognitive performance. Interpretation: As opposed to what was previously hypothesised, vortioxetine did not show a better tolerability profile compared to SSRIs in older adults with MDD in this study. Item does not contain fulltext, In this paper, a new ecosystem to support older adults and their caregivers in an indoor living environment is proposed. The GUARDIAN ecosystem is designed to address the different needs of end users in different countries. The ecosystem consists of a socially assistive robot (SAR), including sensors, and two mobile applications (one for caregivers and one for the older user). Starting from a user-centred and value-sensitive co-design approach, three key user requirements have been identified: monitoring, reminding, and social companionship. User requirements were translated into technical requirements to develop a flexible system architecture. Usability tests were performed to verify the satisfaction level about the GUARDIAN concept. User tests were carried out in three different countries involving 13 older adults, 14 informal carers and 15 formal carers showing that the proposed ecosystem satisfies the user requirements. No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from "TBI Section" of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. The listed recommendations have been sent to a panel of experts consisting of the 107 members of the "TBI Section" of the SINch and the 111 members of the Neuroanesthesia and Neurocritical Care Study Group of the SIAARTI. The aim of the survey was to test a preliminary evaluation of the grade of predictable future adherence of the recommendations following this intersociety proposal. The following recommendations are suggested as representing best clinical practice, nevertheless, adoption of local multidisciplinary protocols regarding thresholds of ICP values, drug therapies, hemostasis management and perioperative care of decompressed patients is strongly recommended to improve treatment efficiency, to increase the quality of data collection and to provide more powerful evidence with future studies. Thus, for this future perspective a rapid overview of the role of the multimodal neuromonitoring in the optimal severe TBI management is also provided in this document. It is reasonable to assume that the recommendations reported in this paper will in future be updated by new observations arising from future trials. They are not binding, and this document should be offered as a guidance for clinical practice through an intersociety agreement, taking in consideration the low level of evidence.

A literatura sobre impulsividade de compra tem negligenciado que a impulsividade pode fazer parte de processos criativos e lúdicos de descoberta. Para preencher essa lacuna, este estudo investiga as atitudes de consumidores que vivenciam impulsividade durante a prática de bricolagem em mercados de segunda mão. Por meio de um levantamento on-line e modelagem por equações estruturais, foram analisados dados de 775 consumidores, descobrindo que a atitude de caça ao tesouro dispara a impulsividade nesses mercados e a consciência de serendipidade, ambas sendo antecedentes de processos criativos na prática da bricolagem. Este estudo contribui para a literatura ao oferecer um modelo explicativo parcimonioso sobre como a impulsividade, a caça ao tesouro, a serendipidade e a prática da bricolagem podem fazer parte de processos criativos e lúdicos de consumo, promovendo uma visão positiva da impulsividade, em contraste com a visão tradicional majoritariamente negativa.

- Abstract
No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from “TBI Section” of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. The spread of COVID-19 is severely testing the economic systems of most advanced countries, posing new important challenges for the States involved. These include the decision taken by several European countries to offer support and protection to companies deemed strategic. The late 20th century was characterized by the progressive reduction of the public sphere as a result of increased privatization processes in most European countries, while the 21st century appears to be fueling a re-expansion of public interventions in the economy to support companies hit by the recent economic crises. This reversal of economic policy is also closely related to the political evolution and development of the countries’ social systems, linked to the relevance of particular economic activities for the satisfaction of specific public interests and needs. The provision of special powers for the State is back in vogue nowadays as a tool for defending the national economy and companies of national interest from potential external aggressions, regardless of the State’s share in the equity structure of such companies. However, this issue raises questions of compatibility with the European discipline: what is the rationale for multiple Foreign Direct Investments Screening policies within a European common market ruled by a single competition policy? Is this compliant with the perimeter of legitimacy identified by the European Commission? Is there room for a new European common strategy to guarantee that of Foreign Direct Investment Screening policies will be effectively tailored to the need of protecting safety and essential community interests?

The conceptual design for a proposed blockerless engine thrust reverser system that will supply reverse thrust without a mechanical blocker is analyzed.

Purpose – The purpose of this study is to analyze the effects of the metaphor of verticality on how individuals assess prices, having regulatory focus as a moderator of this relationship. Design/methodology/approach – Two experiments were conducted with a 2 × 2 between-subjects design (metaphor of verticality: physically higher vs physically lower × regulatory focus: promotion vs prevention). The second study performed moderated mediation by incorporating the self-esteem variable. Findings – The results show that the treatment group consisting of prevention-focused individuals who consider themselves physically higher assessed prices according to what was proposed for the study compared to the group consisting of promotion-focused individuals who consider themselves physically lower. Participants in Treatment Group 1 attributed the lowest prices to products, demanded more significant discounts to go to another store searching for a product and considered the prices more unfair. Originality/value – The primary contribution of this study is to reveal that the position of one's body on the vertical axis influences their thoughts and, therefore, their decision-making in the scope of products and services prices. Moreover, regulatory focus can attenuate such effects.

The purpose of these studies was to evaluate chitosan films impregnated with silver sulfadiazine as potential wound dressings, based on their mechanical properties and the controlled-release phenomenon. The mechanical properties of such films were investigated under varying plasticizers (glycerol or sorbitol) concentrations, as well as in the presence of a crosslinking agent (formaldehyde). The drug release was also determined under such varying conditions, as well as using different thicknesses of film and drug concentrations. The results showed that the additives decreased the tensile strength of the chitosan films (except for sorbitol at 20% w/w), while at the same time remarkably enhancing the percentage elongation of the films. This elongation was especially pronounced in the case of glycerol. The type of plasticizer also influenced the release of silver sulfadiazine. Glycerol had a greater effect than sorbitol on the release rate, regardless of the amount used, probably due to leakage of this additive from the film, which leaves pores that enhance the water uptake of the film. As might be expected, increased concentrations of entrapped silver sulfadiazine yielded increasingly higher release rates. This research aims to analyse the influence of optimism on technology readiness and co-creation experiences in the consumption experiences of the millennial generation at Disney's theme parks as well as their relationship to customer delight. Quantitative research was carried out. The sample obtained 530 respondents, and the Structural Equation Modeling for data analysis was used. The results indicate that the consumption experiences of the Millenials are linked to factors not previously evidenced by other generations, such as the co-creation experiences and the optimism for technological readiness, which increase the degree of these experiences and can lead to consumer delight. On a practical level, it is worth highlighting the importance of theme park marketing teams understanding the behaviour of the millennial consumer, given his/her continuous growth in purchasing power compared to other generations.

Purpose: In view of the contradictory results found in the extant literature, this study aims to analyse the overall effect of attitude, perceived behavioural control, and subjective norms on the intention to reduce food waste. Method: A meta-analytic study was carried out. 25 empirical articles were examined, with a total of 67 effect sizes and a cumulative sample of 30,960 consumers. Findings: The key findings reveal a consistent positive relationship between attitude, perceived behavioural control, subjective norms, and the intention to reduce food waste. Moreover, the effect size tends to be larger in the relationship between subjective norms and the intention to reduce food waste in samples composed of non-students. Theoretical contributions: This study reveals the general effect of the attitude, perceived behavioural control, and subjective norms on the intention to reduce food waste. The main contribution of this work was to analyse contradictory results from previous research and reveal generalizable global effects of the antecedents of the intention to reduce food waste. Practical contributions: In short, public and private educational institutions, in partnership with state or federal government agencies, could propose workshops and design online educational booklets to encourage food waste reduction. Moreover, advertisements including precise instructions on reducing waste can make the respective tasks more tangible for consumers, making them easier to perform. Marketing practitioners should also delineate strategies to reduce food waste, such as selling ugly food that is still suitable for human consumption and employing influencer marketing. Such strategies should be segmented according to the consumers' profiles. For instance, those more sensitized to environmental causes (e.g., environmental activists) may be more willing to reduce food waste.

The results of carotid endarterectomy can be improved by reducing the perioperative embolic and hemodynamic risks. Electrophysiologic monitoring, although reliable, cannot provide full information. In this study, we report on the combined use of transcranial Doppler (TCD) and electrophysiologic monitoring in 153 patients undergoing 166 carotid endarterectomy procedures. TCD monitoring confirmed the low incidence of intolerance to cross-clamp (1.8%), showing a good correlation with electrophysiologic monitoring. In addition, it frequently showed embolic signals immediately after clamp release, but never during carotid dissection or in the final operative phases. Furthermore, TCD allowed the detection of hyperperfusional flow patterns in four cases, making immediate and aggressive control of arterial pressure possible.

The increase in water consumption combined to its scarcity has caused reflections about its maintenance for the next generations. In this sense, it pays attention to the need of consuming this resource responsibly. It is known that different factors, such as goals, the willingness to achieve them, and ecological awareness, can act as predictors of this responsible behavior. Thus, the present study sought to analyze the influence of ecological awareness and chronic regulatory focus on conscious water consumption in Agreste of Pernambuco - a region with water availability problems. To do so, a quantitative approach was used, with data obtained through an online survey, resulting in a sample of 179 valid participants. Data processing was done using SPSS and AMOS software. The hypotheses were tested using structural equation modelling. The results show that consumers, from the studied region, have a strong inclination towards conscious water consumption and the positive influence of ecological awareness on this consumption is revealed. In addition, there is a positive relationship between the focus on promotion and the conscious water consumption. However, no relationship was identified between this behavior and the chronic orientation to prevention focus. These findings can be explained by the fact that external aspects, such as the quantity and quality of water, act in the decision process more forcefully. It is concluded that the willingness to conscious behavior is possibly related to obtaining immediate gains. Therefore, it is evident that conservative public policies may not be effective for holistic awareness regarding the use of this resource.

Layer-by-layer (LBL) films consisting of layers of the azo dye Sunset Yellow alternated with chitosan display spontaneous birefringence, which is attributed to the film anisotropy imparted by the LBL method. This is unusual for azobenzene-containing materials as they normally form films with randomly oriented molecules, presenting birefringence only due to photoinduced isomerization cycles. Spontaneous birefringence does not appear in cast films, but occurs for LBL films obtained under various experimental conditions.
