Type 2 diabetes mellitus (T2DM) represents the main cause of chronic kidney disease (CKD) and end-stage renal disease (ESKD), and diabetic kidney disease (DKD) is a major cause of morbidity and mortality in diabetes. Despite advances in the nephroprotective treatment of T2DM, DKD remains the most common complication, driving the need for renal replacement therapies (RRT) worldwide, and its incidence is increasing. Until recently, prevention of DKD progression was based around strict blood pressure (BP) control, using renin–angiotensin system blockers that simultaneously reduce BP and proteinuria, adequate glycemic control and control of cardiovascular risk factors. Glucagon-like peptide-1 receptor agonists (GLP-1RA) are a new class of anti-hyperglycemic drugs shown to improve cardiovascular and renal events in DKD. In this regard, GLP-1RA o er the potential for adequate glycemic control in multiple stages of DKD without an increased risk of hypoglycemia, preventing the onset of macroalbuminuria and slowing the decline of glomerular filtration rate (GFR) in diabetic patients, also bringing additional benefit in weight reduction, cardiovascular and other kidney outcomes. Sodium-glucose cotransporter 2 inhibitors (SGLT2i) have clearly demonstrated their beneficial effect in diabetic kidney disease (DKD) on top of the standard of care [blood glucose control, renin–angiotensin system blockade, smoking cessation and blood pressure (BP) control], even in patients with overt DKD. However, the indication of this drug class is still blood glucose lowering in type 2 diabetic patients with estimated glomerular filtration rate >45mL/min/1.73m2. Based on the new evidence, several scientific societies have emphasized the preferential prescription of SGLT2i for patients at risk of heart failure or kidney disease, but still within the limits set by health authorities. A rapid positioning of both the European Medicines Agency and the US Food and Drug Administration will allow patients with overt DKD to benefit from SGLT2i. Clinical experience suggests that SGLT2i safety management may in part mirror renin–angiotensin blockade safety management in patients with overt DKD. Albuminuria; Atrasentan; Canagliflozin Albuminuria; Atrasentan; Canagliflozina Albuminúria; Atrasentan; Canagliflozina In April 2019, two major Phase 3 randomized clinical trials were published that assessed primary renal outcomes in diabetic kidney disease (DKD) in type 2 diabetes mellitus (T2DM). The Canagliflozin and Renal Events in Diabetes with Established Nephropathy Clinical Evaluation (CREDENCE) tested an already available antidiabetic drug, canagliflozin, and the Study of Diabetic Nephropathy with Atrasentan (SONAR) tested a novel molecule, the endothelin-1 receptor blocker atrasentan, both on top of renin-angiotensin system blockade. Both trials demonstrated significant nephroprotection in patients with overt DKD (albuminuria >300 mg/g urinary creatinine) for combined primary endpoints of end-stage kidney disease (ESKD), doubling of serum creatinine or death from renal or cardiovascular causes in CREDENCE {hazard ratio [HR] 0.70 [95% confidence interval (CI) 0.59-0.82]} and ESKD and doubling of serum creatinine in SONAR [HR 0.65 (95% CI 0.49-0.88)]. Canagliflozin also decreased the secondary renal endpoint ESKD, doubling of serum creatinine or renal death [HR 0.66 (95% CI 0.53-0.81)], which was similar in nature and impact to the primary endpoint in SONAR. In addition, canagliflozin decreased a secondary endpoint of cardiovascular death or hospitalization for heart failure [HR 0.69 (95% CI 0.57-0.83)], whereas atrasentan had no significant impact on a secondary cardiovascular composite endpoint or on hospital admissions for heart failure and, despite restrictive exclusion criteria, there was a non-significant trend towards more frequent episodes of heart failure. Based on these results, canagliflozin will likely be approved for the indication of treating DKD in T2DM and the estimated glomerular filtration rate threshold for prescribing it will be lifted, whereas the future and place of atrasentan in the treatment of DKD remain unclear. Background and objective: C.E.R.A. (continuous erythropoietin receptor activator, pegilated-rHuEPO β) corrects and maintains stable hemoglobin levels in once-monthly administration in chronic kidney disease (CKD) patients. The aim of this study was to evaluate the management of anemia with C.E.R.A. in CKD patients not on dialysis in the clinical setting. Methods: Two hundred seventy two anemic CKD patients not on dialysis treated with C.E.R.A. were included in this retrospective, observational, multicentric study during 2010. Demographical characteristics, analytical parameters concerning anemia, treatment data and iron status were recorded. Results: C.E.R.A. achieved a good control of anemia in both naïve patients (mean Hemoglobin 11.6g/dL) and patients converted from a previous ESA (mean Hemoglobin 11.7g/dL). Most naïve patients received C.E.R.A. once monthly during the correction phase and required a low monthly dose (median dose 75µg/month). The same median dose was required in patients converted from a previous ESA, and it was lower than recommended in the Summary of Product Characteristics (SPC). Iron status was adequate in 75% of anemic CKD patients, but only 50% of anemic patients with iron deficiency received iron supplementation. Conclusions: C.E.R.A. corrects and maintains stable hemoglobin levels in anemic CKD patients not on dialysis, requiring conversion doses lower than those recommended by the SPC, and achieving target hemoglobin levels with once-monthly dosing frequency both in naïve and converted patients. Introducción y objetivo: C.E.R.A. (activador continuo del receptor de la eritropoyetina) administrado una vez al mes corrige y mantiene estables los niveles de hemoglobina en pacientes con enfermedad renal crónica (ERC). Este estudio fue diseñado para evaluar el manejo de la anemia con C.E.R.A. en pacientes con ERC no en diálisis (ERC-ND) en la práctica clínica. Métodos: Estudio observacional, retrospectivo y multicéntrico, llevado a cabo en pacientes con anemia asociada a ERC-ND en tratamiento con C.E.R.A. durante 2010. Se recogieron datos demográficos, parámetros de laboratorio relacionados con la anemia, datos del tratamiento y estado del hierro. Resultados: El tratamiento con C.E.R.A. logró un buen control de la anemia tanto en los pacientes naïve (hemoglobina media 11,6 g/dl) como en aquellos en conversión desde otros agentes estimulantes de la eritropoyesis (hemoglobina media 11,7 g/dl). La mayoría de los pacientes naïve recibieron C.E.R.A. una vez al mes durante el período de corrección y requirieron una dosis mensual baja (dosis mediana 75 µg/mes). Las dosis de C.E.R.A. utilizadas en pacientes en conversión fueron inferiores a las dosis de conversión recomendadas en la ficha técnica. El estatus del hierro fue adecuado en el 75 % de los pacientes con anemia asociada a ERC-ND, y únicamente el 50 % de aquellos con deficiencia de hierro recibieron suplementos de hierro. Conclusiones: En pacientes con anemia asociada a ERC-ND, naïve o en conversión, la administración mensual de C.E.R.A. corrige y mantiene estables los niveles de hemoglobina, incluso en dosis inferiores a las recomendadas en la ficha técnica.

El déficit de vitamina D se asocia a distintas patologías, siendo especialmente significativa con la morbimortalidad en pacientes con enfermedad renal crónica (ERC). La pérdida progresiva de la función renal conduce a una reducción de calcitriol y alteración de la homeostasis de calcio, fósforo, FGF-23 y PTH, entre otros, los cuales influyen a su vez sobre la activación del receptor de vitamina D (RVD) y el desarrollo de hiperparatiroidismo secundario (HPS). El RVD media las acciones biológicas tanto de la vitamina D como de sus análogos sintéticos, actuando sobre distintos genes; existe una estrecha asociación entre niveles bajos de calcitriol y la prevalencia del HPS. Así, la activación de los RVD y la restricción de fósforo, entre otros, desempeñan un papel importante en el tratamiento de la «alteración óseo-mineral asociada a la ERC». La Sociedad Española de Nefrología, dada la uniforme e importante asociación con mortalidad y niveles altos de fósforo, aconseja su normalización, así como la de los niveles de calcidiol. Igualmente considera que, aparte de la utilización de activadores selectivos/no selectivos de RVD para la prevención y tratamiento del HPS, se podría asegurar la activación de los RVD en pacientes en diálisis, con vitamina D nativa o incluso bajas dosis de paricalcitol, independientemente de la PTH, dado que algunos estudios de cohortes y un metaanálisis reciente han observado una asociación entre el tratamiento con vitamina D activa y la disminución de la mortalidad en pacientes con ERC. En general, se considera que es razonable utilizar toda esta información para individualizar la toma de decisiones. Vitamin D deficiency has been linked to many different pathologies, especially with morbimortality in patients with chronic kidney disease. The progressive loss of renal function leads to calcitriol deficiency and homeostatic changes in calcium, phosphorus, FGF-23 and PTH, among others. All these changes can also influence vitamin D receptor (VDR) activation and the development of secondary hyperparathyroidism (SHPT). The biologic actions of both vitamin D and its synthetic analogues are mediated by binding to the same VDR, acting on different genes. There is a narrow relationship between low levels of calcitriol and SHPT. The combined approach of VDR activation and phosphate restriction, among others, plays an important role in the early treatment of the chronic kidney disease-mineral and bone disorder (CKD-MBD). The Spanish Society of Nephrology, in order to reduce the uniform and significant association with CKD-associated mortality, calcidiol and high phosphate levels suggests normalization of phosphate as well as calcidiol levels in both CKD and dialysis patients. Moreover, it considers that, in addition to selective/non selective activation of VDR for the prevention and treatment of SHPT, VDR could be activated in dialysis patients by native vitamin D or even low paricalcitol doses, independently of PTH levels, as some cohort studies and a recent metaanalysis have found an association between treatment with active vitamin D and decreased mortality in patients with CKD. In general it is considered reasonable to use all this information to individualise decision making.

El déficit de vitamina D se asocia a distintas patologías, siendo especialmente significativa con la morbimortalidad en pacientes con enfermedad renal crónica (ERC). La pérdida progresiva de la función renal conduce a una reducción de calcitriol y alteración de la homeostasis de calcio, fósforo, FGF-23 y PTH, entre otros, los cuales influyen a su vez sobre la activación del receptor de vitamina D (RVD) y el desarrollo de hiperparatiroidismo secundario (HPS). El objetivo de este documento es actualizar las recomendaciones sobre la evaluación y el manejo de la afectación renal en pacientes con infección por el VIH del año 2010. La función renal debe monitorizarse en todos los pacientes e incluir la medida de la concentración sérica de creatinina, la estimación del filtrado glomerular (ecuación CKD-EPI), la medida del cociente proteína/creatinina en orina y un sedimento urinario. El estudio básico de la función tubular ha de incluir la concentración sérica de fosfato y la tira reactiva de orina (glucosuria). En ausencia de alteraciones, el cribado será anual. En pacientes tratados con tenofovir o con factores de riesgo para el desarrollo de enfermedad renal crónica (ERC), se recomienda una evaluación más frecuente. Se debe evitar el uso de antirretrovirales potencialmente nefrotóxicos en pacientes con ERC o factores de riesgo para evitar su progresión. También se revisan las indicaciones de la biopsia renal, cuándo enviar el paciente al nefrólogo y las indicaciones, evaluación y manejo del paciente en diálisis o del trasplante renal.

Las nuevas guías internacionales del consorcio KDIGO (Kidney Disease: Improving Global Outcomes) sobre la enfermedad renal crónica (ERC) y sobre el manejo de la presión arterial (PA) en pacientes con ERC constituyen la actualización de las correspondientes guías KDOQI (Kidney Disease Outcomes Quality Initiative) de 2002 y 2004. El objetivo de estos documentos es ofrecer una guía actualizada para el diagnóstico, la evaluación, el manejo y el tratamiento del paciente con ERC. La primera guía conserva la definición de ERC de 2002, pero ofrece una clasificación pronóstica mejorada. Además, se revalúan los conceptos sobre el pronóstico de la ERC, y se establecen recomendaciones para el manejo de los pacientes y sobre los criterios de derivación al especialista en nefrología. La segunda guía conserva el objetivo de una PA < 130/80 mmHg para pacientes con ERC que curse con una albuminuria elevada (cociente albúmina/creatinina en muestra aislada de orina entre 30 y 300 mg/g) o proteinuria (cociente albúmina/creatinina en muestra aislada de orina > 300 mg/g), pero recomienda el objetivo menos estricto de PA < 140/90 mmHg para pacientes con albuminuria normal. El desarrollo de las guías siguió un proceso predeterminado de revisión y evaluación de las evidencias disponibles. Las recomendaciones sobre el manejo y el tratamiento están basadas en la revisión sistemática de los estudios relevantes. El sistema GRADE (Grading of Recommendations Assessment, Development and Evaluation) se utilizó para evaluar la calidad de la evidencia y emitir el grado de recomendación. También se discuten las áreas de incertidumbre de los distintos aspectos tratados. La enfermedad renal crónica (ERC) y la diabetes mellitus tipo 2 (DM2) son afecciones crónicas de elevada prevalencia que representan un importante problema de salud pública y requieren un abordaje interdisciplinario. La DM2 es la principal causa de ERC en nuestro medio y también constituye una importante comorbilidad de la nefropatía no diabética. Los pacientes con diabetes e insuficiencia renal son un grupo de especial riesgo, pues presentan una mayor morbimortalidad y un superior riesgo de hipoglucemias que los sujetos diabéticos con función renal normal. El tratamiento de la DM2 en los pacientes con ERC resulta controvertido dada la escasez de evidencias disponibles. El presente documento de consenso pretende facilitar la adecuada elección y dosificación de los fármacos antidiabéticos y el establecimiento de unos objetivos seguros de control glucémico en los pacientes con ERC. Chronic kidney disease (CKD) and type 2 diabetes mellitus (T2DM) are highly prevalent chronic diseases that represent a significant public health problem and require multidisciplinary management. T2DM is the main cause of CKD in our setting and it is also a major comorbidity of non-diabetic nephropathy. Patients with diabetes and renal failure represent a special risk group as they have higher morbidity and mortality and are at a higher risk of hypoglycaemia than diabetic individuals with normal renal function. Treatment of T2DM in patients with CKD is controversial because of the scarcity of evidence available. This consensus document aims to facilitate the appropriate selection and dosage of anti-diabetic drugs as well as establishing glycaemic control safety targets in patients with CKD.
