Abstract

Sleep disorders are one of the most common disorders of human well-being. They cause numerous health consequences, such as attention disorders, memory and recall difficulties, and motor disorders. The diagnosis of sleep disorders and the selection of the appropriate form of treatment is essential for helping patients. Current medicine offers many therapeutic options using pharmaco-logical and non-pharmacological methods. During the treatment process, it is advisable to make changes in life hygiene and some-times pharmacological support is necessary. This article reviews the available treatments for sleep disorders and provides a brief description of each. Aspects of sleep hygiene, relaxation training, psychotherapy, biofeedback, non-invasive brain stimulation methods, and the basic groups of drugs used in treatment, e.g. antidepressants, neuroleptics, benzodiazepines, and non-benzodiazepine GABA receptor agonists, are included.

Abstract

Histamine is one of the biogenic amines produced naturally in the human body, but also in foods, especially those rich in protein. Exogenous and endogenous histamine is subject to degradation in vivo, but in the case of sensitive groups, including children, these degradation processes may be less intense, resulting in adverse health effects from histamine excess. The aim of the study was to determine the histamine content in jarred baby foods containing fish, taking into account the selected product characteristics and storage conditions. The study included 140 meals with added fish, intended for infants and young children, from 5 leading manufacturers available in Poland. The infant meals were analyzed on the day of opening, after 24 h and 48 h of storage in the refrigerator and at room temperature. Histamine concentration was determined by ELISA. The THQ was calculated from the EDI values for histamine. Histamine was present in all analyzed baby foods. On the day of opening, the products had a lower content of this monoamine (Me = 2.59 mg/kg), which increased systematically during storage. Samples taken at 2 °C after 48 h showed an average histamine content of 4.4 mg/kg, while products stored at 22 °C at the same time showed a 1.8-fold higher concentration of this monoamine (Me = 7.9 mg/kg). Dishes containing tuna and sea fish had higher histamine levels on average than those containing pollock. The storage conditions of the children’s food had a significant effect on histamine concentration. The level of histamine in baby foods was related to the amount and type of fish in certain products. The results indicate the need for increased awareness of the risks associated with histamine, especially in a group of people with increased sensitivity to this amine, which may include infants and young children.

Introduction and purpose. Nowadays, artificial gel nails, which require Ultraviolet Radiation (UVR) for polymerization, steadily gain popularity. Gel manicure dried with UV lamps has become a regular part of many women's beauty routines. However, its safety remains controversial. The aim of the study was to assess the women’s knowledge of UVR and their attitudes towards UV nail exposure and its impact on human health. Material and methods. The study was carried out from January to March 2020 with the use of PAPI and CAWI method. The study population consisted of 188 women living in the Silesian Voivodeship in Poland. The mean age was 31.7 ±12.4. The majority of women were familiar with UV nail polish drying devices (74%). Most of the respondents were regularly using UV lamps (51.1%). Abstract

BackgroundFood irradiation is one of the methods of food preservation. Unfortunately, despite many opinions from national and international organizations that confirm the safety of the irradiation technique, the irradiated food market is slowly developing, which is particularly noticeable in European countries, including Poland.ObjectivesThe main objective of this study was to determine the attitude of Polish consumers toward irradiated food and to find out whether familiarizing the respondents with educational materials on the irradiation technique would change their attitudes.Material and methodsIn response to the objective of the study, an online survey (with the presentation of educational materials) was conducted with 609 respondents living in the Silesian Voivodeship, Poland. A specially prepared author's questionnaire was used, containing questions relating to: sociodemographic data, food preservation, food irradiation. An integral part of the survey was a multimedia presentation containing information about the food irradiation process.ResultsA low level of knowledge about food irradiation was found-−90.31% (n = 550) of the respondents had never heard of this method of preservation before. The percentage of respondents with a positive attitude toward radiation-preserved products increased significantly after providing informative material, from 6.20 (n = 38) to 67.16% (n = 409). The final attitude of the respondents toward irradiated food varied and depended on age, education and place of residence—positive attitudes toward irradiation predominated among those who were young (100,000 inhabitants. Educational materials also had a significant impact on consumers' interest in purchasing irradiated food—the percentage of people declaring a willingness to purchase this type of product increased from 19.20 (n = 117) to 59.30% (n = 361). Almost 60% of the respondents were willing to purchase irradiated foods. Women, on average, were more likely to be interested in purchasing irradiated food compared to men.ConclusionsThe survey indicates that irradiated food could be commercially introduced in Poland, but on the condition that an effective educational program is planned.

Abstract

Background. There is a raising awareness that heart failure (HF) is a highly heterogeneous, multiorgan syndrome with an increasing global prevalence and still poor prognosis. The comorbidities of HF are one of the key reasons for presence of various phenotypes with different clinical profile and outcome. Heterogeneity of skeletal muscles (SMs) quantity and function may have an impact on patient’s phenotype. Aim. We intended to compare clinical characteristics of phenotypes defined by a combination of various SM mass taken as a fat-free compartment from DEXA scans and different levels of SUCR (Spot Urinary Creatinine). All-cause mortality with mortality predicted by MAGGIC in such phenotypes were compared. Methods. In 720 HF patients with reduced ejection fraction (age: 52.3 ± 10 years, female: 14%, NYHA: 2.7 ± 0.7, LVEF: 24.3 ± 7.3%), admitted to the hospital for heart transplantation candidacy assessment, morning SUCR along with body composition scanning (DEXA) was performed. All study participants were dichotomized twice, first by low or normal appendicular muscle mass index (ASMI) and second by SUCR (Spot Urinary Creatinine) < and ≥of 1.34 g/L. Abstract

Introduction and objective Food irradiation is a preservation technique, the main purpose of which is to extend shelf life of food products by using ionizing radiation. Today, food irradiation technology is used worldwide. The aim of this study is to discuss the process of food irradiation, especially from the aspect of safety of irradiated products and legal regulations applicable to this process in Poland and worldwide Brief description of the state of knowledge The food irradiation process is subject to numerous legal regulations, including the dose levels used, irradiation purposes, or labelling of irradiated products. Currently, more than 50 countries worldwide allow the use of ionizing radiation in relation to food, 7 in the countries of the European Union, including Poland. The safety of irradiated products has been confirmed by key international and national organizations; however, there are also reports concerning their potential hazardous effect on health, e.g. as a result of the occurrence of 2-alkylcyclobutanones or free radicals. Much attention has been paid to the possibility of deterioration of food quality, especially by reducing the amount or loss of some nutrients, or changing the physical properties of the product. Conclusions Irradiation still remains a relatively controversial method of food preservation. It seems that the key to changing this opinion is making the average consumer more aware of the potential role, that food irradiation can play a part in controlling and even eliminating food-borne diseases, or reducing financial losses associated with its spoilage

7. Ambulophobia as a Specific Phobia—Defining the Problem Among Patients of Long-Term Care Facilities in Poland

Abstract

BackgroundAmbulophobia is a type of specific phobia that involves a fear of walking. This phobia mainly affects older people, who prefer not to leave their bed or home to avoid walking on uneven surfaces and reduce the risk of falling. The problem seems to be very important in terms of public health and the organization of long-term care, as Poland has one of the highest rates of elderly population growth.ObjectivesThe aim of the study was to determine the prevalence of ambulophobia among patients of long-term care facilities in Poland and to identify factors increasing the risk of this specific phobia in the study group.Material and MethodsThe study was conducted between January and July 2021. Data collected from 379 patients of 16 long-term care facilities located in Poland were analyzed. The study used the diagnostic criteria for specific phobias according to the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition, and standardized questionnaires such as MMSE, GDS-16, DOS and ADL. The χ2 test was used to test the significance of differences (p = 0.05).ResultsThe prevalence of ambulophobia in the study group was 30.1%. Ambulophobia significantly more often concerned the female sex (37.7%) and people over 70 years of age (42,1%). Factors such as depression, Parkinson's disease, orthostatic hypotonia, a history of falling or being a witness to another person falling, and disability of at least a moderate degree increased the likelihood of ambulophobia.ConclusionsBased on the obtained results, it was found that the protective factors in the development of ambulophobia are male sex, younger age, high independence, fewer drugs used per day and no previous falls or seeing another person fall.

8. Assessment of the Risk of Depression in Residents Staying at Long-Term Care Institutions in Poland During the COVID-19 Pandemic Depending on the Quality of Cognitive Functioning

Abstract

Background: The development of the COVID-19 pandemic has prompted the implementation of many procedures to safeguard against further increases in illness. Unfortunately, this has drastically reduced residents’ contact with their families, which has increased feelings of loneliness and isolation. This is particularly difficult in long-term care facilities, where the risk of developing depression is higher than in the general population.Objectives: The aim of the study was to assess the risk of depression among the residents of long-term care institutions in Poland during the COVID-19 pandemic and to determine the relationship between the risk of depression and the occurrence of cognitive impairment in the study group.Methods: The study included 273 residents from long-term care institutions in Poland. The risk of depression was determined based on an originally designed questionnaire. The cognitive state of the subjects was assessed using the screening test Mini-Mental State Examination (MMSE). Both the depression risk assessment and the MMSE test were conducted twice: in March and December 2020.Results: In March, severe dementia was present in 28.2% of the residents and normal MMSE scores were observed in 16.1% of the subjects; in December, the prevalence of severe dementia increased to 31.1% and that of normal scores decreased to 10.3%. In March, no participant was found to be at high risk of depression and moderate risk was observed in 14.3% of the subjects; in December, 2.6% of the residents had a high risk score and 45.4% had a moderate risk score. Statistical analysis revealed that higher MMSE scores correspond with a higher risk of depression.Conclusion: A higher risk of depression was observed with the development of the pandemic. Residents with cognitive impairment were characterised by a lower risk of depression compared to individuals with normal MMSE scores. During the study, progression of cognitive impairment was observed in the residents.

9. Feelings Related to the COVID-19 Pandemic Among Patients Treated in the Oncology Clinics (Poland)

Abstract

Background: The number of cancer patients is constantly growing. Both WHO and IARC report that this number may reach up to 24 million new diagnosed cases in the next two decades. The proposed treatment and especially the diagnosis can have a significant impact on an individual's approach to the disease, as well as on the patient's quality of life.Objectives: The study aimed to assess the quality of life, feelings, and fear of cancer-treating oncological patients, before and during the COVID-19 pandemic.Material and Methods: The study used the standardized WHOQOL quality of life questionnaire in a shortened version, the COVID-19 fear scale (FCV-19S), and the AIS disease acceptance scale (in terms of cancer-related sensations). The questionnaire survey was conducted among patients of cancer clinics (Poland). The study was conducted in two stages–before the COVID-19 pandemic (May 2019) and during the COVID-19 pandemic (May 2020). Data from 450 correctly completed questionnaires were analyzed statistically. The obtained data were statistically processed using the Kruskal-Wallis and Mann-Whitney U test (p = 0.05).Results: Among the surveyed patients of the cancer clinic, the quality of life during the COVID-19 pandemic decreased by 2%, compared to the period before the pandemic. The frequency of negative feelings associated with cancer increased during the COVID-19 pandemic–by 11% more men, and 4.4% of women determined the frequency of negative feelings to be 2–3 times a week. The level of fear associated with COVID-19 was moderate (57.1%), with women having a higher level of fear (12.5% higher than men).Conclusion: The development of the epidemic is very important in terms of public health. COVID-19 should be considered as one of the factors that bring about sudden changes in the mental health of the population, which may result from the dynamic development of this disease, dramatic media coverage, and own experiences. It has been shown that the sudden appearance of such a large stressor causes a decrease in patients' quality of life and an increase in negative feelings associated with chronic disease.

Abstract

Psoriasis is a chronic inflammatory skin disease. Immunological, genetic, and environmental factors, including diet, play a part in the pathogenesis of psoriasis. Metabolic syndrome or its components are frequent co-morbidities in persons with psoriasis. A change of eating habits can improve the quality of life of patients by relieving skin lesions and by reducing the risk of other diseases. A low-energy diet is recommended for patients with excess body weight. Persons suffering from psoriasis should limit the intake of saturated fatty acids and replace them with polyunsaturated fatty acids from the omega-3 family, which have an anti-inflammatory effect. In diet therapy for persons with psoriasis, the introduction of antioxidants such as vitamin A, vitamin C, vitamin E, carotenoids, flavonoids, and selenium is extremely important. Vitamin D supplementation is also recommended. Some authors suggest that alternative diets have a positive effect on the course of psoriasis. These diets include: a gluten-free diet, a vegetarian diet, and a Mediterranean diet. Diet therapy for patients with psoriasis should also be tailored to pharmacological treatment. For instance, folic acid supplementation is introduced in persons taking methotrexate. The purpose of this paper is to discuss in detail the nutritional recommendations for persons with psoriasis.

12. Type D Personality and Health Behaviors in People Living with Obesity

Background: Considering that health behaviors and personality traits play an important role in the formation of health attitudes, the main objective of this study was to evaluate the relations that occur between type D personality and health behaviors in a group of obese patients. Methods: 443 adult patients with BMI ≥ 30 kg/m2, who had been hospitalized in selected hospital facilities in the Silesian Voivodeship (Poland), participated in the study. Respondents completed three standardized questionnaires—the Multidimensional Health Locus of Control Scale, version A (MHLC-A), the Inventory of Health Behaviors (IZZ), and the Type D Scale (DS-14). Results: Patients with type D personality were characterized by the least effective mental attitudes and preventive behaviors, and differed significantly from the other personality types (intermediate and non-type D). Type D personality increased the risk of initiating improper health behaviors by more than five times. Regarding the sense of health control, patients with type D personality had significantly lower scores for the Internal Dimension subscale (21.3 ± 3.1) and higher for the Powerful Others Dimension subscale (24.0 ± 2.6), compared to patients with intermediate and non-type D personality. Proper health behaviors correlated with an internal sense of health control; the strongest correlation, defined as a medium, was with Preventive Behaviors (R = 0.42; p < 0.0001). Conclusions: Type D personality was associated with poorer attitudes towards health. Among obese respondents with a type D personality, there was a significantly higher prevalence of those who believed that their health status was a consequence of chance events.

13. Penicillin and tetracycline residues in selected fresh and UHT milk with different fat contents

Abstract

The aim of the present work was to determine the residues of penicillin and tetracycline in selected fresh and UHT milk with different fat contents (< 2, 2, and > 2%). A total of 84 different milk samples (36 fresh and 48 UHT) purchased from retail chains in Silesia Region (Poland) were analysed. Penicillin and tetracycline residues were determined by the ELISA immunoenzymatic method. The obtained results were compared with MRL values adopted in Commission Regulation (EU) No. 37/2010. The results were additionally analysed statistically. Penicillin and tetracycline were found to be common in the analysed milk samples. Both antibiotics were present in low concentrations, and not exceeding their MRL. The median of penicillin residues was 0.298 µg/L, and 0.970 µg/L for tetracycline. For some samples, the determined levels of antibiotic residues did not exceed the limit of detection (LOD): three samples for penicillin (1.15%), and 24 samples for tetracycline (28.57%). A weak negative correlation between the level of penicillin and tetracycline residues was found (p = -0.218), where the decrease in penicillin concentration weakly correlated to the increase in tetracycline concentration. The level of residues of both antibiotics was significantly higher in fresh milk than in UHT milk (by 11.75% for penicillin, and 17.73% for tetracycline); for penicillin, Zcorrected = 2.445, and p = 0.0145, and for tetracycline, Zcorrected = 3.914, and p = 0.000091.

Abstract

Streszczenie Barwniki karmelowe to jedne z najbardziej rozpowszechnionych na świecie dodatków do żywności, które podzielono na cztery klasy (I–IV), oznaczone odpowiednio symbolami E150 a-d. Poszczególne klasy karmeli różnią się właściwościami fizyko-chemicznymi oraz sposobem otrzymywania, co wpływa na powstawanie różnych związków, istotnych dla oceny bezpieczeństwa żywności. Przeprowadzono wiele badań wszystkich klas karmeli, w tym badania toksykokinetyczne, genotoksyczne, rakotwórcze oraz badania toksyczności reprodukcyjnej i rozwojowej, które nie wykazały ich szkodliwości, w dawkach nieprzekraczających ADI. Rośnie jednak liczba doniesień naukowych o możliwym działaniu toksycznym, obecnych w karmelach związków niskocząsteczkowych. Obecnie za ważne z toksykologicznego punktu widzenia oraz ze względu na możliwe stężenie w produkcie końcowym, uznaje się trzy związki: 5-HMF (występujący we wszystkich klasach), 4(5)-MeI (obecny w karmelu klasy III i IV) czy THI (obecny w karmelu klasa III). 4(5)-MeI działa neurotoksycznie, a w 2011 r. został uznany za związek o możliwym działaniu rakotwórczym dla człowieka (klasa 2B, wg IARC). W przypadku THI badania potwierdziły jego aktywność limfopeniczną, prawdopodobnie wtórną do działania immunosupresyjnego. W latach 80. ub. w. JECFA ustalił dopuszczalne stężenia 4(5)-MeI oraz THI w odniesieniu do klas karmeli, w których związki te mogą występować. Toksyczność 5-HMF nie została jednoznacznie potwierdzona, ale przeprowadzone badania wskazują, że związek ten nie jest obojętny dla organizmów żywych. Obecnie większość organizacji międzynarodowych i instytutów naukowych, uznaje te dodatki za bezpieczne dla konsumentów, ale jednocześnie naukowcy podkreślają potrzebę prowadzenia dalszym badań. Celem pracy jest przegląd dostępnego piśmiennictwa i doniesień dotyczących karmeli ze szczególnym uwzględnieniem ich charakterystyki toksykologicznej.

Do oceny stopnia nasilenia zaburzeń depresyjnych zastosowano krótką formę Geriatrycznej Skali Oceny Depresji. Porównano również liczbę interwencji medycznych i psychologicznych w poszczególnych miesiącach izolacji.WynikiW lutym 2020 roku, kiedy nie było żadnej izolacji, 87,9% badanej grupy nie cierpiało na depresję w porównaniu z 72,4% w okresie pełnej izolacji. Po wprowadzeniu metod kontrolowanego kontaktu z najbliższymi, liczba osób bez objawów depresyjnych ponownie wzrosła. W okresie pełnej izolacji wykonano średnio 1,76 interwencji medycznych i 0,23 interwencji psychologicznych na jednego pacjenta.WnioskiLiczba interwencji medycznych i psychologicznych była większa w okresie pełnej izolacji w porównaniu z miesiącami, w których nie wprowadzono ograniczeń związanych z pandemią COVID19. Po wprowadzeniu pełnej izolacji wyniki w GDS były istotnie wyższe, co oznacza, że pacjenci byli w większym stopniu narażeni na zaburzenia depresyjne niż w miesiącach bez izolacji.

Abstract

BackgroundAmbulophobia is a type of specific phobia that involves a fear of walking. This phobia mainly affects older people, who prefer not to leave their bed or home to avoid walking on uneven surfaces and reduce the risk of falling. The problem seems to be very important in terms of public health and the organization of long-term care, as Poland has one of the highest rates of elderly population growth.ObjectivesThe aim of the study was to determine the prevalence of ambulophobia among patients of long-term care facilities in Poland and to identify factors increasing the risk of this specific phobia in the study group.Material and MethodsThe study was conducted between January and July 2021. Data collected from 379 patients of 16 long-term care facilities located in Poland were analyzed. The study used the diagnostic criteria for specific phobias according to the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition, and standardized questionnaires such as MMSE, GDS-16, DOS and ADL. The χ2 test was used to test the significance of differences (p = 0.05).ResultsThe prevalence of ambulophobia in the study group was 30.1%. Ambulophobia significantly more often concerned the female sex (37.7%) and people over 70 years of age (42,1%). Factors such as depression, Parkinson's disease, orthostatic hypotonia, a history of falling or being a witness to another person falling, and disability of at least a moderate degree increased the likelihood of ambulophobia.ConclusionsBased on the obtained results, it was found that the protective factors in the development of ambulophobia are male sex, younger age, high independence, fewer drugs used per day and no previous falls or seeing another person fall.

Abstract

Low spot urinary creatinine concentration (SUCR) is a marker of muscle wasting and clinical outcome. The risk factors for low SUCR in heart failure (HF) remain poorly understood. We explored the risk factors for low SUCR related to poor outcomes. In 721 HF patients (age: 52.3 ± 11 years, female: 14%, NYHA: 2.7 ± 0.7) SUCR and Dexa body composition scans were performed. BMI prior HF-onset, weight loss, and appendicular muscle mass were obtained. Each patient was classified as malnutrition or normal by GLIM criteria and three other biochemical indices (CONUT, PNI, and GRNI). Sarcopenia index (SI) as creatinine to cystatin C ratio was also calculated. Within 1 year, 80 (11.1%) patients died. In ROC curve we identified a SUCR value of 0.628 g/L as optimally discriminating surviving from dead. In low SUCR group more advanced HF, higher weight loss and catabolic components of weight trajectory (CCWT), more frequent under-nutrition by GLIM, and lower SI were observed. In multivariate analysis the independent predictors of low SUCR were SI, CCWT, and GNRI score. In conclusion: the risk of low SUCR was associated with a worse outcome. Low SUCR was associated with greater catabolism and sarcopenia but not with biochemical indices of malnutrition.

WstępNeurotropowy czynnik pochodzenia mózgowego (Brain-Derived Neutortophic Factor – BDNF) jest białkiem należącym do rodziny neurotrofin, które odgrywa kluczową rolę w prawidłowym rozwoju i funkcjonowaniu ośrodkowego układu nerwowego ssaków. Dotychczasowe badania dotyczyły głównie oceny stężenia BDNF w surowicy krwi, jako potencjalnego biomarkera w schorzeniach neurologicznych. Ostatnio ścieżka sygnałowa BDNF uznana została za potencjalne miejsce uchwytu leków przeciwnowotworowych, a jego receptor (TrkB) za onkogen w komórkach raka jelita grubego. Pomimo znaczącej roli w nowotworzeniu, niewiele jest prac dotyczących BDNF jako biomarkera w raku jelita grubego.Materiał i metodyBadaniem objęto grupę 25 osób z potwierdzonym klinicznie i histopatologicznie rakiem jelita grubego, którzy zostali zakwalifikowani do leczenia. Przed pierwszorazowym podaniem chemioterapii od pacjentów pobrano próbki krwi żylnej i dokonano oceny parametrów biochemicznych oznaczanych rutynowo przed leczeniem. Dodatkowo metodą immunoenzymatyczną oznaczono stężenia BDNF w surowicy krwi wszystkich badanych pacjentów.WynikiStężenie BDNF w surowicy pacjentów wyniosło 50,24 ± 23,37 ng/ml i nie różniło się istotnie pomiędzy kobietami i mężczyznami. Stwierdzono ujemną korelację między stężeniem BDNF i CRP oraz BDNF i LDH. Stężenie BDNF było znamiennie wyższe u chorych, którzy przed chemioterapią byli poddani resekcji guza pierwotnego. Nie wykazano zależności pomiędzy stężeniem BDNF a wiekiem, płcią, wskaźnikiem BMI, markerem CEA oraz enzymami wskaźnikowymi wątroby u pacjentów z RJG. Nie zaobserwowano zależności pomiędzy stężeniem BDNF a odpowiedzią kliniczną na zastosowane leczenie.WnioskiBDNF nie może być uznany za czynnik prognostyczny u chorych z rakiem jelita grubego.

Abstract

Background: The number of cancer patients is constantly growing. Both WHO and IARC report that this number may reach up to 24 million new diagnosed cases in the next two decades. The proposed treatment and especially the diagnosis can have a significant impact on an individual's approach to the disease, as well as on the patient's quality of life.Objectives: The study aimed to assess the quality of life, feelings, and fear of cancer-treating oncological patients, before and during the COVID-19 pandemic.Material and Methods: The study used the standardized WHOQOL quality of life questionnaire in a shortened version, the COVID-19 fear scale (FCV-19S), and the AIS disease acceptance scale (in terms of cancer-related sensations). The questionnaire survey was conducted among patients of cancer clinics (Poland). The study was conducted in two stages–before the COVID-19 pandemic (May 2019) and during the COVID-19 pandemic (May 2020). Data from 450 correctly completed questionnaires were analyzed statistically. The obtained data were statistically processed using the Kruskal-Wallis and Mann-Whitney U test (p = 0.05).Results: Among the surveyed patients of the cancer clinic, the quality of life during the COVID-19 pandemic decreased by 2%, compared to the period before the pandemic. The frequency of negative feelings associated with cancer increased during the COVID-19 pandemic–by 11% more men, and 4.4% of women determined the frequency of negative feelings to be 2–3 times a week. The level of fear associated with COVID-19 was moderate (57.1%), with women having a higher level of fear (12.5% higher than men).Conclusion: The development of the epidemic is very important in terms of public health. COVID-19 should be considered as one of the factors that bring about sudden changes in the mental health of the population, which may result from the dynamic development of this disease, dramatic media coverage, and own experiences. It has been shown that the sudden appearance of such a large stressor causes a decrease in patients' quality of life and an increase in negative feelings associated with chronic disease.

Abstract

Introduction: Ovarian cancer is most frequently detected in the advanced stage. Although its pathogenesis is not fully elucidated, it is assumed that body susceptibility and hormonal disorders are responsible. The role of some cytokines as predictors in the treatment process is still investigated. The aim of the study was to determine the relationship of adiponectin and leptin with the disease severity and response to chemotherapy. Material and methods: Forty-three ovarian cancer patients were treated by systemic treatment. Patients received 5–7 cycles of chemotherapy — paclitaxel/carboplatin with or without bevacizumab. Using standard ELISA kits before and after chemotherapy, adiponectin and leptin concentrations were determined in the blood serum. Results: The average adiponectin concentration before chemotherapy was found to be 8.83 ± 3.19 μg/ml, as compared to 10.37 ± 4.18 μg/ml (increase by 17.44%, p < 0.001) after treatment. Mean pre-treatment leptin concentration was 16.89 ± 15.54 ng/ml, and 21.77 ± 14.69 ng/ml after chemotherapy (increase by 28.89%, p < 0.01). A positive correlation was found between leptin concentration and age and BMI. There was no relationship of the disease severity with the response to treatment and the concentration of the adipokines. The leptin/adiponectin ratio (L/A) before treatment correlated with better response to chemotherapy. Conclusions: Adiponectin and leptin did not correlate with the stage of ovarian cancer and response to chemotherapy. The L/A ratio may be considered a predictor of clinical response to treatment.

Abstract

In chronic heart failure (HF), some parameters of oxidative stress are correlated with disease severity. The aim of this study was to evaluate the importance of oxidative stress biomarkers in prognostic risk stratification (death and combined endpoint: heart transplantation or death). In 774 patients, aged 48-59 years, with chronic HF with reduced ejection fraction (median: 24.0 (20-29)%), parameters such as total antioxidant capacity, total oxidant status, oxidative stress index, and concentration of uric acid (UA), bilirubin, protein sulfhydryl groups (PSH), and malondialdehyde (MDA) were measured. The parameters were assessed as predictive biomarkers of mortality and combined endpoint in a 1-year follow-up. The multivariate Cox regression analysis was adjusted for other important clinical and laboratory prognostic markers. Among all the oxidative stress markers examined in multivariate analysis, only MDA and UA were found to be independent predictors of death and combined endpoint. Higher serum MDA concentration increased the risk of death by 103.0% (HR=2.103; 95% CI (1.330-3.325)) and of combined endpoint occurrence by 100% (HR=2.000; 95% CI (1.366-2.928)) per μmol/L. Baseline levels of MDA in the 4th quartile were associated with an increased risk of death with a relative risk (RR) of 3.64 (95% CI (1.917 to 6.926), p<0.001) and RR of 2.71 (95% CI (1.551 to 4.739), p<0.001) for the occurrence of combined endpoint as compared to levels of MDA in the 1st quartile. Higher serum UA concentration increased the risk of death by 2.1% (HR=1.021; 95% CI (1.005-1.038), p<0.001) and increased combined endpoint occurrence by 1.4% (HR=1.014; 95% CI (1.005-1.028), p<0.001), for every 10 μmol/L. Baseline levels of UA in the 4th quartile were associated with an increased risk for death with a RR of 3.21 (95% CI (1.734 to 5.931)) and RR of 2.73 (95% CI (1.560 to 4.766)) for the occurrence of combined endpoint as compared to the levels of UA in the 1st quartile. In patients with chronic HF, increased MDA and UA concentrations were independently related to poor prognosis in a 1-year follow-up.
