Metabolic syndrome (MS), conventionally defined by the presence of at least three out of five dysmetabolic traits (abdominal obesity, hypertension, low plasma HDL-cholesterol, high plasma glucose and high triglycerides), has been associated with an increased risk of several age-related chronic diseases, including breast cancer (BC). This may have prognostic implications for BC survivors. 2,092 early stage BC survivors aged 35-70, recruited in eleven Italian centres 0-5 years after surgical treatment (1.74 years on average), were followed-up over 2.8 years on average for additional BC-related events, including BC-specific mortality, distant metastasis, local recurrences and contralateral BC. At recruitment, 20 % of the patients had MS. Logistic regression models were carried out to generate OR and 95 % confidence intervals (CI) for new BC events associated with MS, adjusting for baseline pathological prognostic factors. New BC events occurred in 164 patients, including 89 distant metastases. The adjusted ORs for women with MS versus women without any MS traits were 2.17 (CI 1.31-3.60) overall, and 2.45 (CI 1.24-4.82) for distant metastasis. The OR of new BC events for women with only one or two MS traits was 1.40 (CI 0.91-2.16). All MS traits were positively associated with new BC events, and significantly so for low HDL and high triglycerides. MS is an important prognostic factor in BC. As MS is reversible through lifestyle changes, interventions to decrease MS traits in BC patients should be implemented in BC clinics.

The DIet and ANdrogen-5 study (DIANA-5) for the prevention of BC recurrences recruited 2092 early stage BC survivors aged 35-70. At recruitment, all women completed a 24-hour food frequency and physical activity diary on their consumption and activity of the previous day. Using these diaries we created a score of adherence to five relevant WCRF/AICR recommendations. The prevalence ratios (PRs) and 95% confidence intervals (CIs) of MetS associated with the number of recommendations met were estimated using a binomial regression model. The adjusted PRs of MetS decreased with increasing number of recommendations met (p < 0.001). Meeting all the five recommendations versus meeting none or only one was significantly associated with a 57% lower MetS prevalence (95% CI 0.35-0.73). Our results suggest that adherence to WCRF/AICR recommendations is a major determinant of MetS and may have a clinical impact.

Rispetto alla scelta dicotomica fra un pubblico oramai in crisi e un privato sempre più invadente, la sfida accolta dal concetto di comune è quella di porsi come alternativa tanto all'individuale quanto all'universale, valorizzando ciò che si costituisce nell'interazione tra soggetti e superando in tal modo sia le spinte egoistiche dell'individualismo (che fa di ogni persona un individuo isolato dalla collettività) che le tendenze omologanti dell'universalismo. In virtù di questa peculiare posizione rispetto alle logiche comuni, le pratiche di sapere volte alla creazione di uno spazio comune di condivisione e partecipazione acquistano un valore ed un significato politico dirimente.

Diet therapy is important in renal failure to slow down the progression of the nephropathy and to control uremic symptoms. The main barrier is long-term poor compliance because of flavour and appetizing of a low protein meal.We organized an educational cooking event in a kitchen laboratory with a chef.Fifty patients came accompanied by their care-giver. They could listen to the chefs advices, observe the manipulation of food, taste the dishes prepared with low protein products and directly cook with the assistance of the chef. The menu consisted of appetizers, two different kinds of pasta dishes, a vegetable dish, olive bread and a dessert.The patients collaborated to prepare bread and pasta.Patients appreciated the event and participated in an active way. They showed how important is their direct participation in the control of the disease.

Tesis (Psicopedagogo, Licenciado en Educación) El presente seminario tiene por objetivo formular una propuesta Psicopedagógica para el desarrollo de la escritura creativa en alumnos de Primer Año de Enseñanza Media, con el fin de ser aplicado, tanto por psicopedagogos y docentes de la educación. La propuesta psicopedagógica se basa principalmente, en ser un material innovador, el cual a través de la estimulación del pensamiento divergente, el desarrollo de la creatividad y la mediación apropiada, permita el logro de la producción de textos escritos en forma creativa por alumnos de Primer Año de Enseñanza Media. Esta propuesta se sustenta en la construcción de Zonas de Desarrollo Próximo, tomando en cuenta los postulados constructivistas y considerando la mediación como una guía en el proceso de producción de textos. De acuerdo con lo planteado anteriormente, este seminario se concibe como una propuesta metodológica para ser aplicada por cualquier profesional de la educación, que esté familiarizado con el concepto de mediación, puesto en práctica en el proceso de enseñanza, facilitando al alumno las herramientas para construir su propio aprendizaje hasta que pueda llegar a trabajar de forma independiente, desarrollando sus estrategias metacognitivas.

The aim of this paper was to test in teenagers with type 1 diabetes mellitus (T1DM) the Glucobeeb (Gb), a web based tool to support the diabetes care.Gb transfers glucometer's data by phone and Internet to the PC of practitioner in files dedicated to each patient; the response returns to patient as 1-min vocal message. From outpatients paediatric clinic 28 teenagers (mean 14.8 years, range 10-20, male 14) with T1DM on multiple daily injections insulin therapy, with glicated haemoglobin (HbA1c) over 7% and2 years' duration of the disease (9.1 years, range 2-15), were consequently randomized to telecare (glucometer transmission with feedback, group A) or control (standard communication by phone and face-to-face visits, group B). Glycaemia was tested four times per day and data transmitted every 2 weeks; clinician feedback returned within the following week. Two controls were excluded after randomization. Outcomes of 14 patients of A were compared with 12 of B.In intervention group average HbA1c% decreased from baseline at 3 and 6 months in comparison with controls (9.5, 9.0, 9.1, vs 9.1, 9.4, 9.4 respectively). Controls after 6 months were introduced to Gb, and similar trend of HbA1c was observed in the following examinations at 3 and 6 month (9.4, 8.9, 8.7). Then, in both groups HbA1c after 12 months of Gb increased, and after 18 reduced (A: 9.2, and 8.8, B 9.1 and 8.5 respectively). The enhancement of HbA1c from baseline to end was significant (P=0.01).The tool improves metabolic control in teenagers with T1DM.

Tesis (Psicopedagogo, Licenciado en Educación) Hoy en día es necesario para una completa e íntegra educación, tener profesionales integras o competentes que dominen ciertas habilidades que les permitan, a su vez formar personas que puedan desenvolverse en la sociedad de hoy. Para el ingreso a la universidad a la carrera de Psicopedagogía se cree fundamental el dominio y manejo de habilidades y aptitudes que pueden incidir en el futuro quehacer Psicopedagógico, es por esta razón que nuestro propósito es identificar con qué capacidades y aptitudes ingresan las alumnas de primer año. Por lo tanto, es de nuestro interés aplicar la prueba BADyG-S, ya que ésta mide aptitudes y habilidades que deberían ya estar desarrolladas en las alumnas. Con esta aplicación se quiere realizar un análisis cualitativo de los resultados, ya que no es posible hacerlo cuantitativamente, ya que esta prueba no está estandarizada para nuestro país. La prueba BADyG-S, consta de nueve bloques, conformados cada uno por treinta y dos preguntas las que apuntan a distintas habilidades, por lo que a su vez también se analizarán cada una de ellas con el fin de constatar, específicamente, donde se encuentran las debilidades de dichas alumnas.

This paper is based on 3,500 cases of otosclerosis operated on during the past 14 years restoring the continuity of the ossicular chain by means of an artificial stapes. Eighty percent showed practically a closure of the air-bone gap. There was no hearing gain in 17 percent of the patients, many of which showed reversal to preoperative levels. Three percent of the patients suffered sensori-neural hearing loss and 1 percent cophosis. Necrosis of the incus occurred in 0.3 percent of the patients and perilymphatic fistula in 0.2 percent. Only one case of inflammatory granuloma was observed as a complication of stapedectomy.

The Authors studied the response of 49 subjects (23 affected by toxoplasmosis, 6 with systemic lymphopaties, and 20 apparently healthy controls) to the stimulation of lymphocytes both with an aspecific mitogen (PHA) and a specific one (toxoplasma antigen). No difference was found in the response to PHA, but when the specific antigen was used, patients with toxoplasmosis showed a significant increase of the lymphocytes stimulation index when compared to patients with systemic lymphopaties and to controls.
