Background The immunologic features of children with coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are not clearly delineated. This study was conducted to evaluate SARS-CoV-2-specific antibody responses in children with COVID-19. Background Nasopharyngeal (NP) colonization with Streptococcus pneumoniae and non-typeable Haemophilus influenzae (NTHi) is common in children, and may evolve as the source of invasive infections. In Korea, the pneumococcal conjugate vaccines (PCVs) were introduced >10 years ago, enabling the authors to study the effect of the vaccine in preventing carriage. Methods NP swabs were taken and a household survey was conducted at daycare centres located in different regions of Korea in 2014 and 2019. Pneumococcal serotypes were identified using the Quellung method and sequencing. NTHi were identified based on pilA and bexA genes. Results In total, 1460 NP swabs were obtained with pneumococcal carriage rates of 36.4–42.1% and NTHi carriage rates of 36.5–26.7%. Among children carrying pneumococci, a significant increase was seen in serotype 23A between 2014 and 2019 (from 12.6% to 22.0%; P=0.005). Children who had received PCV were at lower risk of vaccine-type carriage (2.9% vs 0.8%; P=0.005). Conclusions Between 2014 and 2019, the proportion of children carrying serotype 23A increased significantly, while the carriage rate of NTHi decreased. Continuous surveillance is needed to assess the long-term effects of the PCVs on carriage dynamics of pneumococcus and NTHi.

As another wave of COVID-19 outbreak has approached in July 2020, a larger scale COVID-19 pediatric Asian cohort summarizing the clinical observations is warranted. Children confirmed with COVID-19 infection from the Republic of Korea, the Hong Kong Special Administrative Region (HKSAR) and Wuhan, China, during their first waves of local outbreaks were included. This study investigated the roles of β-lactamase and penicillin-binding protein 3 (PBP3) alterations in the development of recent antimicrobial resistance in nontypeable Haemophilus influenzae (NTHi) isolated from Korean children. Nasopharyngeal NTHi isolates from children at a tertiary children's hospital were tested for antimicrobial susceptibility using E-test. β-lactamase production was screened by the paper disc test, and polymerase chain reaction amplification of blaTEM and blaROB-1 was performed. The ftsI gene was amplified to identify PBP3 alteration. Of the 53 NTHi isolates, 69.8% were ampicillin nonsusceptible. The nonsusceptibility rates for cefaclor were 81.1%, cefpodoxime 69.8%, and amoxicillin/clavulanate 32.1%. About 60.3% and 32.1% of the isolates were genetically β-lactamase-nonproducing ampicillin-resistant (gBLNAR) and genetically β-lactamase-producing amoxicillin/clavulanate-resistant (gBLPACR) strains, respectively. Group III amino acid substitutions comprised 65.6% of the gBLNAR strains and 70.6% of the gBLPACR strains. MIC50 for amoxicillin/clavulanate, cefaclor, cefuroxime, cefpodoxime, and cefixime were more than 2-80 times higher in the gBLNAR and gBLPACR strains compared with gBLPAR strains. Group III gBLNAR strains had significantly higher ampicillin, amoxicillin/clavulanate, cefpodoxime, and cefixime minimum inhibitory concentrations than group II strains. Group III gBLNAR and gBLPACR NTHi strains are highly prevalent in Korea, raising the alarm about increasing β-lactam resistance in NTHi.

In areas with high prevalence of macrolide-resistant Mycoplasma pneumoniae (MRMP) pneumonia, treatment in children has become challenging. This study aimed to analyze the efficacy of macrolides and doxycycline with regard to the presence of macrolide resistance. We analyzed children with MP pneumonia during the two recent epidemics of 2014–2015 and 2019–2020 from four hospitals in Korea. Nasopharyngeal samples were obtained from children with pneumonia for MP cultures and polymerase chain reaction (PCR). Macrolide resistance was determined by the analysis of 23S rRNA gene transition. Time to defervescence and to chest X-ray improvement were analyzed. Of 145 cases, the median age was 5.0 years and MRMP accounted for 59 (40.7%). Among macrolide-susceptible MP (MSMP), 78 (90.7%) were treated with macrolides and 21 (35.6%) in the MRMP group with doxycycline. In MRMP pneumonia, shorter days to defervescence (2 vs. 5 days, p &lt, 0.001) and to chest X-ray improvement (3 vs. 6 days, p &lt, 0.001) in the doxycycline group than in the macrolide group was observed, whereas no differences were observed among children with MSMP pneumonia. Compared to macrolides, treatment with doxycycline resulted in better outcomes with a shorter time to defervescence and to chest X-ray improvement among children with MRMP pneumonia.

We explored transmission of severe acute respiratory syndrome coronavirus 2 among 12 children and their uninfected guardians in hospital isolation rooms in South Korea. We found that, even with close frequent contact, guardians who used appropriate personal protective equipment were not infected by children with diagnosed coronavirus disease.

Background Despite high coverage (~98%) of universal varicella vaccination (UVV) in the Republic of Korea since 2005, reduction in the incidence rate of varicella is not obvious. The study aimed to evaluate the vaccine effectiveness (VE) of one-dose UVV by timeline and severity of the disease. Methods All children born in Korea in 2011 were included for this retrospective cohort study that analyzed insurance claims data from 2011–2018 and the varicella vaccination records in the immunization registry. Adjusted hazard ratios by Cox proportional hazard models were used to estimate the VE through propensity score matching by the month of birth, sex, healthcare utilization rate, and region. Results Of the total 421,070 newborns in the 2011 birth cohort, 13,360 were matched for age, sex, healthcare utilization rate, and region by the propensity score matching method. A total of 55,940 (13.29%) children were diagnosed with varicella, with the incidence rate 24.2 per 1000 person-year; 13.4% of vaccinated children and 10.4% of unvaccinated children. The VE of one-dose UVV against any varicella was 86.1% (95% confidence interval [CI], 81.4–89.5) during the first year after vaccination and 49.9% (95% CI, 43.3–55.7) during the 6-year follow-up period since vaccination, resulting in a 7.2% annual decrease of VE. The overall VE for severe varicella was 66.3%. The VE of two-dose compared to one-dose was 73.4% (95% CI, 72.2–74.6). Conclusion We found lower long-term VE in one-dose vaccination and waning of effectiveness over time. Longer follow ups of the vaccinated children as well as appropriately designed studies are needed to establish the optimal strategy in preventing varicella in Korea., Graphical Abstract

Considering the mild degree of coronavirus disease 2019 (COVID-19) in children and the enormous stress caused by isolation in unfamiliar places, policies requiring mandatory isolation at medical facilities should be reevaluated especially given the impact of the pandemic on the availability of hospital beds. In this study, we assessed the usefulness of facility isolation and the transmissibility of severe acute respiratory syndrome coronavirus 2 by infected children to uninfected caregivers in isolation units at a hospital and a residential treatment center in Seoul during August-November 2020. Fifty-three children were included and median age was 4 years (range, 0–18). All were mildly ill or asymptomatic and isolated for a median duration of 12 days. Thirty percent stayed home longer than 2 days before entering isolation units from symptom onset. Among 15 uninfected caregivers, none became infected when they used facemasks and practiced hand hygiene. The results suggest children with mild COVID-19 may be cared safely at home by a caregiver in conditions with adherence to the preventive measures of wearing facemasks and practicing hand hygiene., Graphical Abstract

Objectives The increase in the number of patients with coronavirus disease 2019 (COVID-19) has delayed real-time reverse transcription–quantitative polymerase chain reaction (RT-qPCR), requiring proper shipping and storage conditions, especially in hot weather. This study aims to assess how some conditions, such as storage period, temperature, media or buffer, and sample types, affect the results of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RT-qPCR. Methods SARS-CoV-2–positive specimens were collected from Boramae Medical Center for 2 months (from May to June 2020) and stored in different media or buffers at different temperatures. Results As a result of examining confirmed patient samples, RT-qPCR results were not significantly affected by 2°C to 8°C storage until after 7 days. When stored at 20°C to 22°C or above 35°C, the results were affected negatively even after 1 day. Higher storage temperatures resulted in a lower probability of detecting viral nucleic acids because of degradation. Samples stored in pH-controlled media or buffer were more stable than those stored in nonbuffer states. Conclusions These results emphasize the importance of storage temperature and media or buffer and performing RT-qPCR for SARS-CoV-2 nucleic acid detection as soon as possible after sample collection.

Importance There is limited information describing the full spectrum of coronavirus disease 2019 (COVID-19) and the duration of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA detection in children. Objective To analyze the full clinical course and the duration of SARS-CoV-2 RNA detectability in children confirmed with COVID-19 in the Republic of Korea, where rigorous public health interventions have been implemented. Design, Setting, and Participants This case series of children with COVID-19 was conducted in 20 hospitals and 2 nonhospital isolation facilities across the country from February 18, 2020, to March 31, 2020. Children younger than 19 years who had COVID-19 were included. Exposures Confirmed COVID-19, detected via SARS-CoV-2 RNA in a combined nasopharyngeal and oropharyngeal swab or sputum by real-time reverse transcription–polymerase chain reaction. Main Outcomes and Measures Clinical manifestations during the observation period, including the time and duration of symptom occurrence. The duration of SARS-CoV-2 RNA detection was also analyzed. Results A total of 91 children with COVID-19 were included (median [range] age, 11 [0-18] years; 53 boys [58%]). Twenty children (22%) were asymptomatic during the entire observation period. Among 71 symptomatic cases, 47 children (66%) had unrecognized symptoms before diagnosis, 18 (25%) developed symptoms after diagnosis, and only 6 (9%) were diagnosed at the time of symptom onset. Twenty-two children (24%) had lower respiratory tract infections. The mean (SD) duration of the presence of SARS-CoV-2 RNA in upper respiratory samples was 17.6 (6.7) days. Virus RNA was detected for a mean (SD) of 14.1 (7.7) days in asymptomatic individuals. There was no difference in the duration of virus RNA detection between children with upper respiratory tract infections and lower respiratory tract infections (mean [SD], 18.7 [5.8] days vs 19.9 [5.6] days;P = .54). Fourteen children (15%) were treated with lopinavir-ritonavir and/or hydroxychloroquine. All recovered, without any fatal cases. Conclusions and Relevance In this case series study, inapparent infections in children may have been associated with silent COVID-19 transmission in the community. Heightened surveillance using laboratory screening will allow detection in children with unrecognized SARS-CoV-2 infection.

ObjectiveTransmissibility of COVID-19 by children in the household is not clear. Herein, we describe children’s role in household transmission of COVID-19.Design and settingAll paediatric COVID-19 index cases and their household members reported from 20 January to 6 April 2020 in South Korea were reviewed. The secondary attack rate (SAR) from child index case to household secondary case was calculated. Epidemiological and clinical findings of child index case-household secondary case pair was assessed.ResultsA total of 107 paediatric COVID-19 index cases and 248 of their household members were identified. One pair of paediatric index-secondary household case was identified, giving a household SAR of 0.5% (95% CI 0.0% to 2.6%). The index case was self-quarantined at home after international travel, stayed in her room, but shared a meal table with the secondary case.ConclusionThe SAR from children to household members was low in the setting of social distancing, underscoring the importance of rigorous contact tracing and early isolation in limiting transmission within households.

Objectives In Korea, the National Immunization Program provided trivalent inactivated influenza vaccines (IIV3) to all children aged 6–59 months during the 2017–2018 season. In this study, we aimed to evaluate the vaccine effectiveness (VE) of IIV3 in children during the 2017–2018 season. Methods Children aged 6–59 months who were tested for influenza for their acute respiratory illness in four hospitals during the 2017–2018 influenza season were included. We estimated the VE of IIV3 by test-negative case-control design based on the rapid influenza diagnostic test (RIDT) or reverse transcription polymerase chain reaction (RT-PCR) test results. Results A total of 4738 children were included in this study. The number of laboratory-confirmed influenza cases was 845 (17.8%), and there were 478 cases of influenza A and 362 cases of influenza B. The adjusted VE based on RT-PCR was 53.4% (95% CI, 25.3–70.5) against any influenza, 68.8% (95% CI, 38.7–84.1) against influenza A, and 29.7% (95% CI, −35.1 to 61.8) for influenza B. The adjusted VE based on RIDT was 14.8% (95% CI, −4.4 to 30.0) against any influenza, 24.2% (95% CI, 3.1–40.2) against influenza A, and −5.1% (95% CI, −42.6 to 21.4) against influenza B. Age-specific VE based on RT-PCR against any influenza was 44.1% (95% CI, −0.2 to 67.8) in children aged 6 months to 2 years and 59.3% (95% CI, 8.8–81.9) in children aged 3– Conclusion Our results suggest moderate protection (53.4%) of IIV3 against RT-PCR laboratory-confirmed influenza in children in the 2017–2018 influenza season. However, the RIDT hampered the validity to assess VE during influenza season. Caution is needed when interpreting an RIDT-based test negative design influenza VE study.

Along with positive SARS-CoV-2 RNA in nasopharyngeal swabs, viral RNA was detectable at high concentration for >3 weeks in fecal samples from 12 mildly symptomatic and asymptomatic children with COVID-19 in Seoul, South Korea. Saliva also tested positive during the early phase of infection. If proven infectious, feces and saliva could serve as transmission sources.

We report changes in viral load over time in a 27-day-old neonate with coronavirus disease 2019 who presented with fever, cough, and vomiting. Severe acute respiratory syndrome coronavirus 2 RNA was detected in the nasopharynx, oropharynx, stool, saliva, plasma, and urine. The highest viral RNA copies in nasopharynx decreased over time while viral load in stool remained high.

Abiotrophia defectiva is a rare pathogen of infective endocarditis (IE) but is frequently involved in embolic complication and valvular dysfunction. IE caused by A. defectiva in children is poorly studied. This study reports four cases of A. defectiva IE in children and reviews previously reported five pediatric cases of A. defectiva IE. Most of the patients presented with a subacute course, with prolonged fever or atypical symptoms. Eight patients had embolic complications at presentation. All nine children were treated with combination antimicrobial therapy and six of them received surgical intervention. All patients recovered well without relapse. A. defectiva should be considered in children with infective endocarditis, especially in those with atypical presentations. As complications are frequent and more than half of the patients need surgical treatment, prompt diagnosis along with appropriate treatment is necessary.

The large outbreak of coronavirus disease 2019 (COVID-19) that started in Wuhan, China has now spread to many countries worldwide. Current epidemiologic knowledge suggests that relatively few cases are seen among children, which limits opportunities to address pediatric specific issues on infection control and the children's contribution to viral spread in the community. Here, we report the first pediatric case of COVID-19 in Korea. The 10-year-old girl was a close contact of her uncle and her mother who were confirmed to have COVID-19. In this report, we present mild clinical course of her pneumonia that did not require antiviral treatment and serial viral test results from multiple specimens. Background Mycoplasma pneumoniae is a common cause of respiratory tract infections in children and adults. This study applied high-throughput whole genome sequencing (WGS) technologies to analyze the genomes of 30 M. pneumoniae strains isolated from children with pneumonia in South Korea during the two epidemics from 2010 to 2016 in comparison with a global collection of 48 M. pneumoniae strains which includes seven countries ranging from 1944 to 2017. Results The 30 Korean strains had approximately 40% GC content and ranged from 815,686 to 818,669 base pairs, coding for a total of 809 to 828 genes. Overall, BRIG revealed 99% to > 99% similarity among strains. The genomic similarity dropped to approximately 95% in the P1 type 2 strains when aligned to the reference M129 genome, which corresponded to the region of the p1 gene. MAUVE detected four subtype-specific insertions (three in P1 type 1 and one in P1 type 2), of which were all hypothetical proteins except one tRNA insertion in all P1 type 1 strains. The phylogenetic associations of 30 strains were generally consistent with the multilocus sequence typing results. The phylogenetic tree constructed with 78 genomes including 30 genomes from Korea formed two clusters and further divided into two sub-clusters. eBURST analysis revealed two clonal complexes according to P1 typing results showing higher diversity among P1 type 2 strains. Conclusions The comparative whole genome approach was able to define high genetic identity, unique structural diversity, and phylogenetic associations among the 78 M. pneumoniae strains isolated worldwide.

BACKGROUND Cytomegalovirus (CMV) disease is underrecognized in children with retinoblastoma. This study investigated rates of CMV infection and disease in this specific population receiving chemotherapy. METHODS From a cohort of 164 patients with retinoblastoma diagnosed from 2011 to 2018, 107 patients were evaluated for CMV infection determined by antigenemia assay or real-time PCR. Preemptive CMV screening was implemented in 2013. CMV disease was diagnosed by tissue biopsy, culture, or ophthalmic examination. RESULTS Thirty-seven and 70 patients before and after the screening strategy, respectively, were included. Before screening, 10/37 (27%) were diagnosed with CMV infection during chemotherapy. Among them, 5 (50%) developed CMV disease (hepatitis, pneumonia, and retinitis) and one patient died of CMV pneumonia. During screening, 18/70 (26%) were documented with 36 episodes of CMV infection and 9 patients received 25 preemptive antiviral therapies. Background Norovirus outbreaks in health care facilities are difficult to prevent and control. Methods We describe a norovirus outbreak that occurred in a single pediatric ward with multi-bed rooms. Results The outbreak began with 4 clustered cases with acute diarrhea in a pediatric ward between December 22 and 25, 2017. A total of 10 cases were identified during the outbreak, with a cumulative incidence of 8.77%. The median age of the cases was 10 months. Symptoms lasted for a median of 7.5 days, and norovirus shed in stool for a median of 19.5 days. The first 5 cases consecutively developed diarrhea; 4 of them were in the same room. The sixth case, which was linked with the clustered cases, had stayed in the intensive care unit and infected 3 other patients during the stay. After these 4 cases were transferred back to the ward, an additional patient was infected. Strict infection control measures were implemented, and the outbreak was successfully terminated 24 days after the initial case. Conclusions Controlling norovirus outbreak in a pediatric ward with multi-bed rooms is challenging. Early detection of the outbreak and prompt implementation of strict infection control measures are critical.

Background Mycobacterium bovis Bacille Calmette-Guérin (BCG) osteitis, a rare complication of BCG vaccination, has not been well investigated in Korea. This study aimed to evaluate the clinical characteristics of BCG osteitis during the recent 10 years in Korea. Methods Children diagnosed with BCG osteitis at the Seoul National University Children's Hospital from January 2007 to March 2018 were included. M. bovis BCG was confirmed by multiplex polymerase chain reaction (PCR) in the affected bone. BCG immunization status and clinical information were reviewed retrospectively. Results Twenty-one patients were diagnosed with BCG osteitis and their median symptom onset from BCG vaccination was 13.8 months (range, 6.0–32.5). Sixteen children (76.2%) received Tokyo-172 vaccine by percutaneous multiple puncture method, while four (19.0%) and one (4.8%) received intradermal Tokyo-172 and Danish strain, respectively. Common presenting symptoms were swelling (76.2%), limited movement of the affected site (63.2%), and pain (61.9%) while fever was only accompanied in 19.0%. Femur (33.3%) and the tarsal bones (23.8%) were the most frequently involved sites; and demarcated osteolytic lesions (63.1%) and cortical breakages (42.1%) were observed on plain radiographs. Surgical drainage was performed in 90.5%, and 33.3% of them required repeated surgical interventions due to persistent symptoms. Antituberculosis medications were administered for a median duration of 12 months (range, 12–31). Most patients recovered without evident sequelae. Conclusion Highly suspecting BCG osteitis based on clinical manifestations is important for prompt management. A comprehensive national surveillance system is needed to understand the exact incidence of serious adverse reactions following BCG vaccination and establish safe vaccination policy in Korea., Graphical Abstract

목적: 중추신경계 감염의 적절한 치료를 위해서 신속한 원인 병원체의 확인이 중요하다. 본 연구는 열이 있는 영아의 뇌척수액 검체에서 원인 병원체 검출을 위한 BioFire® Meningitis/Encephalitis (ME) panel 검사 방법의 진단적 가치를 평가하고자 수행되었다. 방법: 2016년 1월부터 2019년 7월까지 발열을 주소로 서울대학교 어린이병원에 내원한 90일 미만의 영아로부터 채취한 뇌척수액으로 기존검사(세균 배양, Xpert® enterovirus assay, herpes simplex virus-1 and -2 중합효소 연쇄반응 검사)를 시행한 후 -70°C 초저온 냉동고에 보관된 검체를 대상으로 BioFire® ME panel 검사를 시행하였다. 결과: 총 72개 검체(원인 병원체가 검출된 24개와 검출되지 않은 48개)가 포함되었다. BioFire® ME panel 검사 결과, 기존검사로 원인 병원체가 검출되지 않은 48개의 검체 중 41개(85.4%)는 음성이었고 원인 병원체가 검출된 24개 검체 중 22개(91.7%)가 동일한 결과(enterovirus 19개, Streptococcus agalactiae 2개, Streptococcus pneumoniae 1개)를 보여 전체 일치율은 87.5% (63/72)였다. 병원체가 기존검사에서 검출되지 않았으나 BioFire® ME panel에서 검출된 7개의 검체 중 6개에서 human parechovirus (HPeV)가 검출되었다. 결론: 열이 있는 90일 미만 영아에서 BioFire® ME panel 검사법은 원인 병원체가 밝혀진 기존검사 결과와는 비교적 높은 일치도를 보이며, HPeV를 추가적으로 진단할 수 있었다. 향후, 소아청소년 진료 영역에서 BioFire® ME panel 검사법을 적용할 근거를 마련하기 위한 임상적 유용성과 비용 효과에 대한 연구가 필요하다.

Alkaline hydrolysis is one of the most classic fiber finishing methods, however, its potential as tuning surface superhydrophobicity in mass scale has not been studied much. In this research, fine roughness was formed on the polyester fiber surfaces by alkaline hydrolysis at room temperature and fluorinated polymer mixtures were further coated. The developed superhydrophobic fabrics were evaluated in terms of structural changes, mechanical properties, surface hydrophobicity, and permeability for practical applications. As alkaline hydrolysis treatment time increased, surface roughness was increased as a lot of nano-craters were generated with the decrease of fabrics weight and tensile strength as well. As air pockets formed through nano-craters on the fiber surfaces, static contact angle increased, and shedding angle tended to decrease. In this study, the sample treated with alkaline hydrolysis for 20 minutes showed the highest static contact angle of 167.8±1.3° and lowest shedding angle of 4.4±2.3°. Considering tensile strength loss, however, the 15-minute alkaline hydrolyzed fabrics which showed static contact angle of 162.2±2.7° and shedding angle of 8.8±0.2° was selected as the optimal condition for practical application. The newly developed superhydrophobic fabrics were found to have higher water vapor and air permeability than those of untreated samples. At the same time, fluoropolymer coating played a certain role for tensile strength and water vapor permeability demonstrating the importance of understanding and designing proper fluorinated-compound treatment processes.

거대세포바이러스병은 혈액종양 질환을 가진 환자군에서는 주로 조혈모세포이식을 받은 환자에게서 발현하는 것으로 보고되고 있으나, 드물게 조혈모세포이식을 받지 않고 항암 치료 중에 발현하는 경우가 있다. 저자들은 발열과 시력 저하를 주소로 입원한 7세 남자에게서 백혈병 유지 치료 중 발현한 거대세포바이러스 망막염을 진단하여 보고하는 바이다. 초기에 거대세포바이러스 항원혈증검사 수치가 51 positive cells/200,000 leukocytes로 높게 보고되었으나 조혈모세포이식을 받은 병력이 없어 항바이러스 치료를 시행하지 않았다가 3주 후 항원혈증검사 수치가 170 positive cells/200,000 leukocytes로 증가하여 시행한 안과 검진에서 양안의 망막 침범 소견과 과립형 병변이 확인되어 거대세포바이러스 망막염으로 진단되었다. 총 4주간의 정맥 내 ganciclovir 치료와 6회의 유리체강 내 ganciclovir 주입술을 시행한 후 경구 valganciclovir 치료를 1달간 더 시행하였다. 치료 시작 1달 후 거대세포바이러스 항원혈증검사가 음성이 되었고 안저검사에서 호전 소견을 보였다. 본 증례와 같이 거대세포바이러스병에 이환되기 쉬운 고위험군 환자들에 대한 선별 검사와 적절한 치료 방침에 대한 논의가 필요하겠다.

The Korean Society of Pediatric Infectious Diseases and the Korea Centers for Disease Control and Prevention issued the guidelines about coronavirus disease 2019 (COVID-19) for children and adolescents. Case definitions and management of COVID-19 in neonates, infants, children and adolescents are presented in this guideline. In addition, guidelines for caregiver management are also provided. In this review, we introduce the contents of the current guidelines for COVID-19 in children and adolescents in Korea.

The clinical spectrum of Mycoplasma pneumoniae pneumonia is widely variable. This study evaluated the clinical manifestations of M. pneumoniae pneumonia in children of different age groups and by the presence of co-detected respiratory virus.This study included children hospitalized with M. pneumoniae pneumonia between 2010 and 2015. At the time of pneumonia diagnosis, a nasopharyngeal aspirate was analyzed for respiratory viruses by reverse transcription-polymerase chain reaction (RT-PCR). The clinical manifestations and laboratory findings were reviewed from medical records.Of the 286 children with M. pneumoniae pneumonia, 84 (29.4%) had a co-detected respiratory virus, with the highest co-detection rate in young children (51.9% of children2 years; P = 0.002). In children2 years, with and without co-detected virus, wheezing occurred in 35.7% and 15.4%, respectively. Among the 202 children without any virus detected, only 6.4% were2 years. These young children showed fewer median days of fever than the children ≥2 years (8 vs. 11 days; P = 0.022). Children ≥2 years tended to have accompanying skin rashes (21.7% vs.7.7%; P = 0.310) and elevated liver enzymes (21.7% vs. 0%; P = 0.075) more frequently than children2 years. Only 53.8% of the patients2 years were treated with macrolide compared with 94.1% of the patients ≥2 years (P0.001).The clinical manifestations of M. pneumoniae pneumonia in young children are milder than those in older children. A high prevalence of co-detected respiratory virus in young children suggests that virus might play a role in making pneumonia clinically apparent in this age group.

BackgroundRadiologic evaluation of children with Mycoplasma pneumoniae is important for diagnosis and management.ObjectiveTo investigate the correlation between chest radiographic findings and the clinical features in children with Mycoplasma pneumoniae pneumonia.Materials and methodsThis study included 393 hospitalized children diagnosed with M. pneumoniae pneumonia between January 2000 and August 2016. Their clinical features and chest radiographs were reviewed. Radiographic findings were categorized and grouped as consolidation group (lobar or segmental consolidation) and non-consolidation group (patchy infiltration, localized reticulonodular infiltration, or parahilar peribronchial infiltration).ResultsLobar or segmental consolidation (37%) was the most common finding, followed by parahilar or peribronchial infiltration (27%), localized reticulonodular infiltration (21%) and patchy infiltration (15%). The consolidation group was more frequently accompanied by pleural effusions (63%), compared to the non-consolidation group (16%). Compared with patients in the non-consolidation group, those in the consolidation group were associated with a significantly higher rate of hypoxia, tachypnea, tachycardia, extrapulmonary manifestations, prolonged fever, and longer periods of anti-mycoplasma therapy and hospitalization. Lobar or segmental consolidation was significantly more frequent in children ≥5 years old (44%) compared with children 2-5 years old (34%) and ConclusionThe chest radiographic findings of children with M. pneumoniae pneumonia correlate well with the clinical features. Consolidative lesions were frequently observed in older children and were associated with more severe clinical features.

Eikenella corrodens는 구강 상재균으로 두경부 감염을 주로 일으키나 면역력이 정상인 소아청소년에게 감염을 일으키는 경우는 흔하지 않다. 저자들은 발열, 두통, 구토를 증상으로 입원한 이전까지 건강하였던 13세 남자에게서 E. corrodens 감염에 의한 중추신경계 경막 외 농양을 진단하여 보고하는 바이다. 초기에 시행한 신체검진에서 경부 강직은 없었으나, 우측 이루가 관찰되었다. 뇌 magnetic resonance imaging (MRI) 영상소견에서 4.5 cm 크기의 경막 외 농양이 우측 측두엽 부근에서 발견되었고, 양측 사골동염 및 접형동염, 우측 유양돌기염과 중이염이 동반되었다. Vancomycin과 cefotaxime 투약 중에도, 임상증상이 호전되지 않고 추적 MRI에서 경막 외 농양의 크기가 증가하고, 우측 접형동에 농양이 형성되어 항균요법과 함께 천두술과 내시경적 접형동 절개술을 시행하였다. 수술 시 경막 외 농양과 접형동에서 악취가 나는 고름이 다량 흡인되었으며 두 부위에서 흡인된 검체 모두에서 E. corrodens가 배양되었다. 수술 이후 3주 동안 cefotaxime 정맥주사로 치료받고 합병증 없이 회복하였다. 결론적으로, E. corrodens는 건강한 소아청소년에서 치료되지 않은 세균성 부비동염에 합병하여 중추신경계 농양의 원인이 될 수 있다.

목적: Mycoplasma pneumoniae 폐렴은 학동기 소아와 청소년의 지역사회 획득 폐렴 중 가장 흔한 원인으로, 조기에 원인 진단이 가능하다면 적절한 항균 요법을 결정하는데 도움이 된다. 본 연구는 하기도 감염 소아의 호흡기 검체에서 M. pneumoniae를 검출하기 위한 신속 항원 검사 방법의 진단적 가치를 평가하고자 하였다. 방법: 2010년 8월부터 2018년 8월까지 하기도 감염으로 서울대학교 어린이병원에서 응급실 또는 입원 치료를 받은 소아로부터 채취한 비인두 흡인물 중 M. pneumoniae 배양 검사를 시행한 후 -70°C 초저온냉동고에 보관되어 있는 검체 215개를 선정하였다. 비인두 흡인물 검체를 실온에서 해동하고 면역크로마토그래피를 이용한 Ribotest Mycoplasma®를 시행한 후 두 명의 검사자가 결과를 판독하였다. 검사를 시행하는 자와 판독하는 자는 배양 검사 결과를 모르는 상태에서 검사를 진행하였다. 결과: 총 215개의 비인두 흡인물 검체 중 M. pneumoniae가 배양 양성인 검체는 119개, 배양 음성인 검체는 96개였다. M. pneumo niae가 배양 양성인 119개 중 74개(62.2%)가 Ribotest Mycoplasma® 검사 결과 양성이었고, 배양 음성인 96개 중 92개(95.8%)가 Ribotest Mycoplasma® 검사 결과 음성이었다. 배양 검사 결과를 기준으로 평가한 Ribotest Mycoplasma®의 민감도는 62.2% (74/119, 95% 신뢰구간, 53.5-70.9%)이었으며, 특이도는 95.8% (92/96, 95% 신뢰구간, 91.8-99.8%)이었다. 또, 양성 예측도는 94.9% (74/78, 95% 신뢰구간, 90.0-99.8%)이었으며, 음성 예측도는 67.2% (92/137, 95% 신뢰구간, 59.3-75.0%), 그리고 일치도 77.21% (166/215, 95% 신뢰구간, 71.6-82.8%)를 보였다. 결론: 본 연구 결과, 신속 항원 검출법인 Ribotest Mycoplasma® 검사결과가 양성인 경우는 M. pneumoniae 배양 양성과의 일치도가 매우 높아서 M. pneumoniae 감염의 진단에 유용하였다. 그러나, Ribotest Mycoplasma® 검사결과가 음성인 경우의 약 1/3은 M. pneumoniae 배양 양성인 검체이었으므로, 음성 검사 결과에 대한 해석은 주의하여야 한다.
